AIM

To provide reference for selection and clinical rational use of third-generation anti-seizure medications in medical institutions through a comprehensive evaluation of perampanel and lacosamide.

METHODS

A percentage based evaluation system was developed based on the Quick Guide for Drug Evaluation and Selection in Chinese Medical Institutions (the Second Edition). Referring to drug instructions, clinical treatment guidelines, and relevant literature, perampanel and lacosamide were quantitatively scored from five dimensions of pharmaceutical characteristics,effectiveness, safety, economy, and other attributes.

RESULTS

The final evaluation score was 77.41 for perampanel and 69.06 for lacosamide. The score of perampanel was higher than 70, so the recommendation for entering the drug use catalog of medical institutions was “strong recommendation”. There was a significant difference in effectiveness and economy between the two drugs. Perampanel had more evidence-based proofs and indications for the treatment of drug-resistant epilepsy and lower average daily treatment cost, so the effectiveness and economy of perampanel was slightly better than that of lacosamide. However, in terms of safety, perampanel had a Boxed Warning from the U. S. Food and Drug Administration about severe psychiatric and behavioral reactions, and it was not as recommended as lacosamide in patients with pregnancy or liver dysfunction.

CONCLUSION

Perampanel and lacosamide each have advantages in different attributes, and medical institutions can decide whether to introduce them based on the characteristics of patient population and the availability of alternative drugs.

目的

通过对吡仑帕奈、拉考沙胺的综合评价,为医疗机构遴选和临床合理使用第三代抗癫痫发作药提供参考。

方法

以《中国医疗机构药品评价与遴选快速指南(第二版)》为依据制定百分制评估体系,参考药品说明书、临床指南及相关文献,从药学特性、有效性、安全性、经济性和其他属性五个维度对吡仑帕奈和拉考沙胺进行量化评分。

结果

最终评估总分吡仑帕奈77.41分,拉考沙胺69.06分。吡仑帕奈的得分大于70分,在医疗机构药品使用目录品种引进时的建议为“强推荐”。两药在有效性和经济性方面得分差异较大。吡仑帕奈治疗药物难治性癫痫的循证证据和适应证较多,日均治疗费用较低,有效性和经济性略优于拉考沙胺。但在安全性方面,吡仑帕奈存在美国食品和药物管理局关于严重精神和行为反应的黑框警告,且在妊娠及肝功能不全患者中的使用推荐不如拉考沙胺。

结论

吡仑帕奈和拉考沙胺在不同属性方面各有优势,医疗机构可根据患者人群特点及是否有可替代药品决定是否引进。

目的

通过对吡仑帕奈、拉考沙胺的综合评价,为医疗机构遴选和临床合理使用第三代抗癫痫发作药提供参考。

方法

以《中国医疗机构药品评价与遴选快速指南(第二版)》为依据制定百分制评估体系,参考药品说明书、临床指南及相关文献,从药学特性、有效性、安全性、经济性和其他属性五个维度对吡仑帕奈和拉考沙胺进行量化评分。

结果

最终评估总分吡仑帕奈77.41分,拉考沙胺69.06分。吡仑帕奈的得分大于70分,在医疗机构药品使用目录品种引进时的建议为“强推荐”。两药在有效性和经济性方面得分差异较大。吡仑帕奈治疗药物难治性癫痫的循证证据和适应证较多,日均治疗费用较低,有效性和经济性略优于拉考沙胺。但在安全性方面,吡仑帕奈存在美国食品和药物管理局关于严重精神和行为反应的黑框警告,且在妊娠及肝功能不全患者中的使用推荐不如拉考沙胺。

结论

吡仑帕奈和拉考沙胺在不同属性方面各有优势,医疗机构可根据患者人群特点及是否有可替代药品决定是否引进。

AIM

To provide reference for selection and clinical rational use of third-generation anti-seizure medications in medical institutions through a comprehensive evaluation of perampanel and lacosamide.

METHODS

A percentage based evaluation system was developed based on the Quick Guide for Drug Evaluation and Selection in Chinese Medical Institutions (the Second Edition). Referring to drug instructions, clinical treatment guidelines, and relevant literature, perampanel and lacosamide were quantitatively scored from five dimensions of pharmaceutical characteristics,effectiveness, safety, economy, and other attributes.

RESULTS

The final evaluation score was 77.41 for perampanel and 69.06 for lacosamide. The score of perampanel was higher than 70, so the recommendation for entering the drug use catalog of medical institutions was “strong recommendation”. There was a significant difference in effectiveness and economy between the two drugs. Perampanel had more evidence-based proofs and indications for the treatment of drug-resistant epilepsy and lower average daily treatment cost, so the effectiveness and economy of perampanel was slightly better than that of lacosamide. However, in terms of safety, perampanel had a Boxed Warning from the U. S. Food and Drug Administration about severe psychiatric and behavioral reactions, and it was not as recommended as lacosamide in patients with pregnancy or liver dysfunction.

CONCLUSION

Perampanel and lacosamide each have advantages in different attributes, and medical institutions can decide whether to introduce them based on the characteristics of patient population and the availability of alternative drugs.

癫痫是常见的慢性神经系统疾病之一,全世界有超过7 000万人患有癫痫,其发病率呈双峰分布,婴儿和老年人风险最高1。新诊断的癫痫患者,若接受规范、合理的抗癫痫发作药(anti-seizure medications,ASMs)治疗,70%~80%患者的发作是可控的2。20世纪80年代以后陆续上市了多种新型ASMs,第三代ASMs包含拉考沙胺、吡仑帕奈、布瓦西坦、艾司利卡西平等2。目前在中国上市的第三代ASMs只有吡仑帕奈和拉考沙胺。《中国医疗机构药品评价与遴选快速指南》(以下简称遴选指南)由健康中国研究中心药品与健康产品专家委员会等多个协会联合制定,以卫生技术评估为基础,提供了一种符合我国国情的快速、科学、可量化的评估手段,协助医疗机构遴选优质药品3。遴选指南第二版4在第一版的基础上,结合已有的实际应用经验,调整了不同评价指标的权重,使药品评价重点更侧重于药品本身,同时细化了评分细则使评分标准更加明确。本研究根据遴选指南对吡仑帕奈和拉考沙胺进行卫生技术评估,以期为医疗机构开展药品遴选提供实践经验。
依据遴选指南,采用百分制评分体系对吡仑帕奈和拉考沙胺进行量化评分,评价内容包含药学特性(28分)、有效性(27分)、安全性(25分)、经济性(10分)和其他属性(10分)五个维度4
评价项目由两位药师独立完成,如遇分歧则通过讨论及咨询第三位药师解决。通过检索中国知网、万方及PubMed、Embase等数据库,查阅医脉通、药智数据、UpToDate、Lexicomp等,获得吡仑帕奈和拉考沙胺的药学特性、有效性、安全性等相关资料,检索时限从建库至2023年3月16日。药品价格通过查询河南省医疗机构药品交易采购平台获得。国家医保和基本药物情况通过检索2022年版《国家基本医疗保险、工伤保险和生育保险药品目录》及2018版《国家基本药物目录》获得。药品的市场信息及生产企业的相关信息通过查阅欧、美、日药品监督管理部门网站发布的药品信息及美国《制药经理人》公布的2022年全球制药企业50强目录5获得。
依据遴选指南,用于新品种引进时,评分70分以上建议为强推荐;60~70分,根据临床是否有替代治疗药物,建议为弱推荐或不推荐;60分以下建议为不推荐。
吡仑帕奈是突触后神经元离子型α-氨基-3-羟基-5-甲基-4-异噁唑丙酸(AMPA)谷氨酸受体的非竞争性拮抗剂1。AMPA为抗癫痫药物作用的新靶点 6。拉考沙胺是一种新型N-甲基-D-天冬氨酸(NMDA)受体甘氨酸位点拮抗剂,可选择性增强电压门控钠通道的慢失活,且不影响钠通道快失活,从而稳定过度兴奋的神经元细胞膜并抑制神经元反复放电7。相关研究表明两药临床疗效确切1, 6, 7,药理作用方面,两药均得5分。
吡仑帕奈目前只有片剂一种剂型。拉考沙胺有片剂、糖浆及静脉溶液制剂7。本次主要评价吡仑帕奈及原研拉考沙胺片剂。参考药品说明书8,9及相关文献10,两药体内过程均明确,药动学参数完整,具体见表1,均得5分。
在主要成分与辅料方面,两药主要成分结构明确,拉考沙胺说明书未列出辅料成分,该项吡仑帕奈2分,拉考沙胺1分。在规格与包装方面,吡仑帕奈每片2 mg、4 mg,复合铝/聚氯乙烯PVC硬片泡罩包装;拉考沙胺每片50 mg、100 mg、150 mg、200 mg,聚氯乙烯/聚偏二氯乙烯复合铝塑泡罩包装。两药均适宜临床应用和剂量调整,均得2分。在剂型方面,两药均为口服剂型,均得2分。在给药剂量方面,吡仑帕奈每次2~12 mg,拉考沙胺每次50~200 mg,治疗过程中均需要根据病情调整,均得1.5分。在给药频次方面,吡仑帕奈为每日1次,得2分;拉考沙胺需每日2次,得1.5分。在使用便利方面,两药均为片剂,均无需医务人员或他人辅助,可自行服药,均得 2分。
两药贮藏条件均为不超过30 ℃密闭保存,均得4分。
吡仑帕奈有效期36个月,得1.5分。拉考沙胺有效期60个月,得2分。
吡仑帕奈于2012年经美国食品和药物管理局(FDA)批准上市6,于2019年在我国上市,目前获批适应证为4岁及以上癫痫部分性发作(伴有或不伴有继发全面性发作)患者的治疗。欧洲药品管理局(EMA)还批准其用于7岁及以上患者原发性全面性强直阵挛(PGTC)发作的辅助治疗,FDA也批准用于12岁及以上患者PGTC的辅助治疗1。拉考沙胺2008年由FDA和EMA批准上市,于2018 年在我国上市,目前获批适应证为4岁及以上患者癫痫部分性发作的单药及联合治疗。2021年FDA 调整为1月龄以上患者使用11。综上所述,两药疗效确切,为临床需要品种,适应证评分均为3分。
吡仑帕奈、拉考沙胺用于局灶性癫痫的治疗均获得较多指南推荐,其中吡仑帕奈获推荐适应证较多,且其在美国神经病学学会(AAN)、美国癫痫学会(AES)难治性癫痫指南12及中国成人局灶性癫痫规范化诊治指南 13中推荐等级A级,得12分,拉考沙胺推荐等级B级,得8分,见表2
有限的数据表明,在药物难治性癫痫方面,吡仑帕奈与拉考沙胺相比,50%反应率(48.6% vs. 28.6%)和无癫痫发作率(14.3% vs. 4.3%)在数据上更优14。但间接分析表明,吡仑帕奈、拉考沙胺与其他三代ASMs的50%反应率和无癫痫发作率无显著差异15, 16,但与左乙拉西坦相比,吡仑帕奈在最高有效推荐剂量下显示出较低的50%反应率17。因此,以主要疗效终点50%反应率指标评分,吡仑帕奈得5.5分,拉考沙胺得5分;以次要疗效终点无癫痫发作率指标评分,两药均得4分。
根据不良事件通用术语评价标准(CTCAE 5.0)判断药品不良反应严重程度。
中度不良反应:吡仑帕奈说明书记载中十分常见(发生率≥ 10%)头晕、嗜睡,常见攻击、愤怒、易激、食欲下降/增加、体重增加、疲乏、背痛、构音障碍、共济失调、复视、鼻咽炎等。一项吡仑帕奈Ⅲ期非盲延期阶段(OLEx)研究(307号研究) 18评估了长期使用吡仑帕奈(≤4年)的安全性和耐受性,发现OLEx期间十分常见(发生率>10%)的治疗紧急不良事件为眩晕、嗜睡、头痛、疲劳、体重增加、易激和鼻咽炎等;长期使用未导致新的药物安全性及耐受性问题。拉考沙胺说明书记载中十分常见(发生率≥10%)头晕、头痛、恶心和复视,常见抑郁、平衡障碍、认知障碍、嗜睡、震颤、感觉异常、视物模糊、耳鸣、挫伤等。一项长达8年的开放标签扩展试验(SP615试验)19评价拉考沙胺在局灶性癫痫添加治疗中的安全性和有效性,提示其长期安全性与之前的双盲试验相似,并且可以保持疗效。十分常见(发生率>10%)的治疗紧急不良事件为头晕、头痛、恶心、复视、疲劳、上呼吸道感染、鼻咽炎和挫伤等。因此,在中度不良反应方面,两药均得1分。
重度不良反应:吡仑帕奈说明书记载有自杀观念、自杀未遂,发生率为偶见(0.1%~1%);药物反应伴嗜酸性粒细胞增多和系统性症状(DRESS)、Stevens-Johnson综合征,发生率不详。307 号研究18提示在4年吡仑帕奈暴露期较常见(>1%)的严重不良事件为惊厥、癫痫、头部受伤等。拉考沙胺说明书记载有自杀企图、自杀意念,发生率为偶见(0.1%~1%);粒细胞缺乏症、DRESS、惊厥、Stevens-Johnson综合征、中毒性表皮坏死松解症,发生率未知。SP615试验19提示,在长达8年的研究中,与拉考沙胺治疗相关的严重不良事件中,常见惊厥(1.6%)和胸痛、头晕、呼吸困难、ECG延长的QRS综合征、头痛、窦性心动过缓、癫痫持续状态和呕吐(均为0.5%)。因此,在严重不反应方面,两药均得2分。
吡仑帕奈在我国获批可以用于4岁以上儿童,也有文献报道了2岁以上儿童的有效性和安全性数据20, 21。我国儿童癫痫患者中吡仑帕奈使用的专家建议22 指出,对4岁以下或体重20 kg以下的患儿,可考虑以0.5 mg·d-1作为起始剂量。老年人使用无需调整剂量。妊娠期不建议使用。小样本报道提示23,使用吡仑帕奈的96次妊娠中,有28次未足月,其中包含流产、早产、死产;5个足月新生儿报告了不良事件,如低Apgar评分、致命性新生儿误吸、囊性纤维化、先天性耳聋、吮吸反射不良和浅呼吸。用药期间应停止哺乳,基于个案报道,吡仑帕奈在乳汁和血清中的浓度比为0.1324 。基于Lexicomp数据库及药品说明书,轻度和中度肝功能损害(Child-Pugh A和B)患者的吡仑帕奈起始剂量为2 mg qd,缓慢滴定。重度肝功能损害患者不建议使用。肾功能损害者,肌酐清除率(CrCl)50 mL·min-1以上无需调整剂量;CrCl 30~49 mL·min-1无需调整剂量,需严密监测及缓慢滴定;因无研究资料,CrCl<30 mL·min-1及血液透析患者不建议使用。吡仑帕奈在特殊人群方面得6.3分,具体明细见表3
拉考沙胺在我国批准4岁以上儿童可以使用,FDA批准1月龄以上患者可用。老年人使用无需调整剂量。妊娠期获益大于风险时才可使用。个案报道整个妊娠期和母乳喂养期间持续使用拉考沙胺,在后代中未观察到严重或轻微的先天性畸形,所有新生儿均达到正常发育标准25。用药期间应停止哺乳,基于个案报道,拉考沙胺在乳汁和血清中的浓度比为0.8324。个案报道部分服用拉考沙胺者母乳喂养的婴儿,出现喂养不佳和嗜睡26。基于Lexicomp数据库及药品说明书,轻-中度肝功能损害患者使用拉考沙胺最大推荐剂量的75%,重度肝功能损害患者不建议使用(获益大于风险时可用)。肾功能损害者,CrCl>30 mL·min-1无需调整剂量;CrCl<30 mL·min-1起始量不用调整,维持量为最大推荐剂量的75%。接受血液透析患者,起始量无需调整,透析后补充50%剂量;维持量为最大推荐剂量的75%。腹膜透析者,起始量无需调整,维持量为75%最大推荐量。拉考沙胺被连续性肾脏替代治疗显著清除,建议此类患者进行治疗药物浓度监测。拉考沙胺在特殊人群方面得8.3分,具体明细见表3
酶诱导型ASMs卡马西平、奥卡西平、苯妥英钠和托吡酯使吡仑帕奈的药-时曲线下面积(AUC)分别降低 64%、48%、43%和19%8。酶抑制剂酮康唑使吡仑帕奈的AUC增加20%27。吡仑帕奈对CYP2C8、CYP3A4、UGT1A9和UGT2B7有弱抑制作用,对CYP2B6、CYP3A4、CYP3A5、UGT1A1和UGT1A4有弱诱导作用,能使拉莫三嗪、氯巴占、丙戊酸、卡马西平、咪达唑仑的AUC分别降低9.3%、7.7%、5%、4.3%、13%,左炔诺孕酮的AUC和峰浓度(cmax)降低40%28,奥卡西平的AUC增加35%27-29。吡仑帕奈与以上药物合用时可能需要进行剂量调整。儿童癫痫患者中吡仑帕奈使用的专家建议指出,在联用卡马西平、奥卡西平等酶诱导剂时,无需改变起始剂量,可以考虑加快加量速度22。相互作用方面,吡仑帕奈得2分。
拉考沙胺慎用于同时服用影响心脏传导药物(钠通道阻滞剂、β-受体阻滞剂、钙通道阻滞剂、钾通道阻滞剂)的患者,包括延长PR 间期的药物(包括钠通道阻滞剂ASMs)。正在服用CYP3A4 (如伊曲康唑、酮康唑、利托那韦、克拉霉素等)和CYP2C9(如氟康唑等) 强抑制剂的肝肾功能不全患者可能会显著增加对拉考沙胺的暴露9,这些患者可能需要减少剂量。与酶诱导型ASMs卡马西平、苯妥英钠、苯巴比妥合用时,拉考沙胺血药浓度降低30,合用时需要调整剂量。相互作用方面,拉考沙胺得2分。
吡仑帕奈存在惊厥、严重的不良心理后果18,拉考沙胺存在脑出血、心肌病等19,均为不可逆的不良反应,此项两者均不得分。两者均无致畸致癌报道,但吡仑帕奈存在FDA关于严重精神和行为反应的黑框警告,因此在其他安全性方面,吡仑帕奈得1分,拉考沙胺得2分。
经济性评价通过比较同通用名药品及主要适应证可替代药品的日均治疗费用获得。日均治疗费用=每片药品的价格×每日治疗所需药品的平均片数。
同通用名药品经济性比较:吡仑帕奈的生产厂家是日本卫材药业,为进口原研品种,截止评价时我国无其他厂家生产该药,日均治疗费用为7.68元,日均治疗费用比较视为最低,得3分。拉考沙胺的生产厂家是比利时优时比制药,为进口原研品种,日均治疗费用23.95元;河南省公共资源交易中心药品采购平台挂网销售的还有江西青峰药业有限公司4种包装规格和山东朗诺制药有限公司产品,日均治疗费用分别为4.61元、4.12元、4.02元、3.92元、6.81元,其中3.92元为最低日均治疗费用;同通用名药品经济性比较评分=最低日均治疗费用×3/评价药品日均治疗费用,拉考沙胺此项得0.49分。
主要适应证可替代药品经济性比较:第二代ASMs中日均治疗费用最低的是拉莫三嗪(美国葛兰素史克)5.72元,主要适应证可替代药品经济性比较评分=最低日均治疗费用×7/评价药品日均治疗费用,吡仑帕奈得5.21分,拉考沙胺得1.67分,具体见表4
吡仑帕奈为国家谈判药品,按医保乙类支付,无支付条件限制。拉考沙胺为医保乙类药品,无支付条件限制。两药均为原研药品,非国家基本药物,非集采药品。吡仑帕奈生产企业日本卫材药业和拉考沙胺生产企业比利时优时比制药,分别在世界销量前50 制药企业中排名第 31、29 名。因此其他属性方面,吡仑帕奈得分5.4,拉考沙胺得分5.6,具体见表5
经量化评分,吡仑帕奈、拉考沙胺的最终得分分别为77.41 分、69.06 分,两药得分差异较大的项目为有效性和经济性,详见表6。吡仑帕奈的得分大于70分,在医疗机构药品使用目录新品种引进时的建议为“强推荐”。
本研究依据遴选指南对吡仑帕奈和拉考沙胺从各项属性进行了快速遴选评价,单项得分可直观地反映出药品在相关属性的优劣势,而总分可反映出药品的整体优势。本研究中,吡仑帕奈评估总分为77.41分,拉考沙胺为69.06分,吡仑帕奈的得分大于70分,在医疗机构药品使用目录新品种引进时的建议为“强推荐”。
吡仑帕奈和拉考沙胺在不同属性方面各有优势。两药在有效性及经济性方面得分差异较大。与拉考沙胺相比,吡仑帕奈治疗药物难治性癫痫的循证证据相对较多,获得指南推荐的适应证也相对较多,因此在有效性方面得分相对较高。有文献采用成本效益分析法 31表明,与拉考沙胺相比,吡仑帕奈(8或4 mg·d-1)作为局灶性癫痫的附加治疗方案在中国具有相对较高的价值(质量调整生命年提高 0.054或0.01)。原研拉考沙胺在局灶性癫痫治疗药物中日均治疗费用最高,经济性得分较低。目前国产拉考沙胺已经进入第七批国家集采目录,为局灶性癫痫的治疗提供了更多选择。在安全性方面,吡仑帕奈存在FDA关于严重精神和行为反应的黑框警告,且在妊娠及肝功能不全患者中的使用推荐不如拉考沙胺,因此安全性得分相对较低。使用吡仑帕奈时,应密切监测患者精神科不良事件的发生,特别是易怒和攻击行为。此外,吡仑帕奈由于可能被滥用,美国缉毒局在2014年将其列为Ⅲ类特殊管制药物32,我国在2023年7月1日将其列为第二类精神药品管理33。有文献报道吡仑帕奈不良事件与剂量有关,在滴定和维持阶段均有发生18,为减少吡仑帕奈不良事件建议给药从低剂量开始并缓慢滴定34 ,在使用过程中应注意用药剂量和处方疗程。本研究开展之前,两药均非本院基本用药供应目录内品种,临床亦尚未向药学部提交国产集采拉考沙胺的用药采购申请。药师将评估结果提交本院医院药事管理与药物治疗学委员会作为药品遴选参考,经专家讨论及投票,最终将吡仑帕奈纳入医院基本用药供应目录。
本研究的评估方法可快速、科学、客观、全面地汇总药品信息,但也存在一定的局限性:(1)评分结果具有一定的时效性。评分受到国家医保及基本药物目录的调整、权威证据及疾病诊疗指南的更新、药品价格及生产企业排名信息的变化等影响,药品评分应随上述因素的变化而动态调整,以协助医疗机构决策者做出更客观的判断。(2)个别评分细则有待优化。吡仑帕奈存在黑框警告,以及被纳入第二类精神药品管理,评分细则中未给予扣分。这些都需要在以后的实践中进一步优化。
综上,吡仑帕奈和拉考沙胺在不同属性方面各有优势,医疗机构可根据本机构患者人群特点及是否有可替代药品决定是否引进。通过本次对吡仑帕奈、拉考沙胺的遴选实践,为医疗机构使用遴选指南开展药品遴选提供了实践经验,同时为临床合理使用两药提供了循证依据。
