Objective

To explore the connotations of community self-organization efficiency from the perspective of public health crisis governance, to provide theoretical support and practical guidance for improving the efficiency of community self-organization in responding to public health emergencies.

Methods

Based on the public health resilience cycle analysis framework, this study used a combined method of literature review and qualitative interviews. From July to September 2023, a total of 22 interview transcripts were collected and qualitatively analyzed using NVivo 12 software.

Results

A total of 194 valid initial nodes were sorted and 96 conceptual labels, 24 initial categories, 10 major categories, and 4 core categories were identified. The core categories included efficiency of community organization in resource acquisition, mobilization, efficiency of social services, and restoration of order in response to public health emergencies.

Conclusions

To ensure that community self-organization can effectively respond to public health emergencies, it is necessary to strengthen and enhance the positive aspects of "community self-organization efficiency", improve the overall risk resistance and efficiency of the community, and thus build a resilient public health system.

目的

从公共卫生危机治理的视角出发，对社区自组织效率的内涵进行深入探讨，以期为提升社区自组织在应对突发公共卫生事件方面的效率提供理论支撑和实践指导。

方法

基于公共卫生韧性周期分析框架，采用文献梳理与质性访谈相结合的方法，于2023年7—9月共收集22份访谈资料，并通过运用NVivo 12软件对访谈资料进行质性分析。

结果

整理194个有效初始节点，识别出96个概念化标签、24个初始范畴、10个主范畴、4个核心范畴。其中，核心范畴包括资源获取效率、组织动员效率、社会服务效率和秩序恢复效率。

结论

为了确保社区自组织能够高效应对突发公共卫生事件，必须加强培养并提升其所蕴含的“社区自组织效率”的积极方面，提高社区的整体抗风险能力和效率，才能更好地高效应对突发公共卫生事件，进而构建具有韧性的公共卫生体系。

目的

从公共卫生危机治理的视角出发，对社区自组织效率的内涵进行深入探讨，以期为提升社区自组织在应对突发公共卫生事件方面的效率提供理论支撑和实践指导。

方法

基于公共卫生韧性周期分析框架，采用文献梳理与质性访谈相结合的方法，于2023年7—9月共收集22份访谈资料，并通过运用NVivo 12软件对访谈资料进行质性分析。

结果

整理194个有效初始节点，识别出96个概念化标签、24个初始范畴、10个主范畴、4个核心范畴。其中，核心范畴包括资源获取效率、组织动员效率、社会服务效率和秩序恢复效率。

结论

为了确保社区自组织能够高效应对突发公共卫生事件，必须加强培养并提升其所蕴含的“社区自组织效率”的积极方面，提高社区的整体抗风险能力和效率，才能更好地高效应对突发公共卫生事件，进而构建具有韧性的公共卫生体系。

Objective

To explore the connotations of community self-organization efficiency from the perspective of public health crisis governance, to provide theoretical support and practical guidance for improving the efficiency of community self-organization in responding to public health emergencies.

Methods

Based on the public health resilience cycle analysis framework, this study used a combined method of literature review and qualitative interviews. From July to September 2023, a total of 22 interview transcripts were collected and qualitatively analyzed using NVivo 12 software.

Results

A total of 194 valid initial nodes were sorted and 96 conceptual labels, 24 initial categories, 10 major categories, and 4 core categories were identified. The core categories included efficiency of community organization in resource acquisition, mobilization, efficiency of social services, and restoration of order in response to public health emergencies.

Conclusions

To ensure that community self-organization can effectively respond to public health emergencies, it is necessary to strengthen and enhance the positive aspects of "community self-organization efficiency", improve the overall risk resistance and efficiency of the community, and thus build a resilient public health system.

突发公共卫生事件日益频发的现实背景要求构建能够快速应对、有效恢复的具有“韧性”的公共卫生防护网络[1]。公共卫生韧性是在韧性治理基础之上提出的概念[2]，主要指在突发公共卫生事件的演变及其应急管理过程中预见、准备和适应变化条件的能力以及承受、应对和迅速从断裂中恢复的能力[3–4]。高度的自组织性是社区能拥有这种能力的关键所在。社区自组织是指社区志愿服务群体在没有物质激励和机构监督管理的情况下以抵御、恢复、适应突发公共卫生事件为目的，以参与治理为活动内容，基于自愿原则自发形成的组织[5]，有效弥补政府在社会公共服务提供领域的不足。在2020年之后的后疫情时代，由于公共卫生风险突发性、紧迫性和不确定性的特点[6]，快速恢复是突发公共卫生事件治理关键[7]，即从公共卫生韧性视角来看，其本质在于提升危机治理效率[8]。作为突发公共卫生事件治理的助推器，社区自组织的治理效率将影响社区应急体系的完整性和公共卫生韧性的强弱[9]。因此，社区自组织如何高效运行并快速缓解、控制和消除其不利影响是社区公共卫生危机治理场域中的重要议题[10]。现有的研究大多聚焦于社区自组织培育以及治理突发公共卫生事件的效能[11–14]，鲜有研究关注社区自组织的效率问题，“社区自组织效率”的本土内涵仍缺乏一个清晰、系统的理论框架。本文通过对2023年6—9月在湖北省、海南省收集的关键知情人士访谈资料运用扎根理论进行编码分析，旨在初步探索公共卫生韧性视角下社区自组织效率的内在含义，以期为未来的相关研究提供理论支撑和测量依据。
1　资料与方法

1.1　资料来源
为了保证样本和调研内容多样性，本研究以受2019年末新冠病毒感染疫情（简称“新冠疫情”）冲击较为严重的地区和2022年8月集中暴发新冠疫情的地区为对象，选择湖北荆州市、潜江市共4个镇以及海南省海口市、东方市共2个镇为样本地区进行实地调研。为更全面地获取与研究主题相关的资料，通过目的抽样和滚雪球抽样，选取与研究主题相关的22名访谈对象，包括：（1）卫生行政管理部门人员（卫生健康委、疾控局领导）4人；（2）乡镇卫生院领导相关负责人及职能科室负责人（公卫科）6人；（3）参与过应对突发公共卫生事件或参与社区组织培育的村委会成员7人；（4）村民5人。本研究已获得调研对象的知情同意。
1.2　框架构建
19世纪中叶提出的韧性理论能够帮助精准把握系统危机治理在动态变化过程中所展现出的阶段性特征和目标[15–16]。美国国家科学院2012年提出规划（plan）、吸收（absorb）、恢复（recover）和适应（adapt）四个韧性阶段[17]。而在公共卫生危机治理不同阶段中社区自组织的角色扮演过程是动态化且富有弹性的，扮演着“规划”阶段的危机信息的搜集者、“吸收”阶段的危机防范的动员者、“恢复”阶段的危机治理的协同者、“适应”阶段的危机修复的协助者[18]。这些角色定位反映了政府及社会对社区自组织在危机管理中的期望，是社区自组织功能发挥的基本前提与行动逻辑。因此，本研究高度耦合了突发公共卫生事件传播特点和韧性目标[19]，搭建“规划–吸收–恢复–适应”公共卫生韧性分析框架（图1），并运用至分析编码中，确保所构建的“社区自组织效率”内涵更加科学。
1.3　分析方法
本文采用Nvivo12软件对访谈资料进行质性分析。根据Strauss和Corbin提出“程序化扎根理论研究”，本研究将编码分为开放性编码（open coding）、主轴性编码（axial coding）与选择性编码（selective coding）[20]。开放性编码过程涉及在仔细审查原始资料的基础上，提炼出与研究主题相契合的信息，并对其进行概念化和范畴化处理；在此基础上，主轴性编码进一步挖掘范畴间的潜在关联，概括出更高层次的主范畴；选择性编码则通过“故事线”方式，在多个主范畴中确定一个核心范畴并围绕其组织理论。本研究通过半结构化焦点访谈的方法进行信息收集，并对收集到的原始访谈资料运用程序化扎根理论进行三级编码，并以社区自组织角色定位为出发点，以危机阶段管理为主线，最后确定“社区自组织效率”的内涵。
1.4　质量控制
在研究设计阶段，邀请从事公共卫生应急管理、基层卫生政策与管理、健康服务整合等研究领域的2名教授、3名副教授进行专家讨论制定预访谈提纲，并后续针对访谈提纲在湖北省潜江市某村随机抽选4位村民进行一轮预访谈。在访谈阶段，借助描述2019年末新冠疫情背景，引导访谈对象回顾当时社会情境，并适时追问以获得完整清晰答复。在编码前，随机抽样6份访谈资料用以检验理论饱和度。若检验结果显示未能发掘新的概念、范畴及其相互关系，即意味着理论饱和度的达成，从而可停止进一步的样本抽取[21–22]。在编码分析阶段，采纳三角互证法作为效度检验策略：录音访谈并澄清歧义，深入描述各类别和范畴，采用“本土概念”作为初级范畴，通过文献调研和小组讨论丰富验证编码范畴。
2　结　果

2.1　研究对象
本研究访谈对象共22人，年龄为31～59岁，平均年龄为46岁。其中，来自村委会的村委会主任7人，占31.82%，行政管理人员4人，占18.18%，临床医师6人，占27.27%，村民5人，占22.73%。学历硕士及以上6人，占27.27%，大学本科1人，占4.55%，高中、中专或职高5人，占22.73%，大专5人，占22.73%，初中4人，占18.18%，小学及以下1人，占4.55%。工作年限超过10年的有20人，占90.91%。
2.2　开放性编码结果
该阶段识别标签和现象摘要，最终共贴出了236个标签，形成了原始资料编码的一级节点，涵盖了194个有效初始节点。在这些节点中，“满足群众诉求”“党员参与度高”“微信沟通”“志愿者参与度”等议题被提及的频率尤为突出。随后，进一步对标签进行了概念化总结与对比，并通过归纳得出了相应的范畴，共计96个概念化标签，24个初始范畴。为了准确展现编码过程，特选取部分访谈原始语句，并展示其初始范畴开放式编码的结果，具体参见表1。
2.3　主轴编码结果
该阶段充分利用了实际数据中的证据、事故、事件等进行详尽的解释和阐述[22]。经过反复推导、归纳整理，成功提炼出10个相对独立且高度凝练的主范畴。具体的主轴编码情况如表2所示，涵盖了信息资源传递效率、物资资源调配效率、人力资源调配效率、协作配合效率、服务辐射范围、行动执行效率以及服务对象需求满足程度等关键要素。
2.4　选择性编码结果
经过初步的三级编码分析，本研究进一步抽象出4个主要类属，即资源获取效率、组织动员效率、社会服务效率以及秩序恢复效率。这四个类属准确反映了社区自组织在应对突发公共卫生事件、提升公共卫生韧性时所需承担的职能和角色定位。详细的编码结果见表3。
2.5　理论饱和度
检验在编码初始随机抽选6份访谈资料用作理论饱和度检验，编码步骤和方法与前文一致。在对6份访谈材料进行编码过程中，并未发现任何新的范畴及其联结关系。据此，通过扎根理论所构建的范畴体系已全面，且已达到了理论饱和的状态。

本研究通过对22位关键知情人士访谈，从提升公共卫生韧性的视角出发，基于公共卫生韧性周期分析框架，运用扎根理论分析发现，“社区自组织效率”是由“资源获取效率”“组织动员效率”“社会服务效率”以及“秩序恢复效率”等核心内涵构成。
首先，“资源获取效率”是“社区自组织效率”的基础，概念意义包括“信息资源传递效率”和“物资资源调配效率”。高效的应急资源获取和调配是实现社区自组织效率的前提[23]。在突发公共卫生事件发生时，社区自组织需要迅速获取所需的资源，如医疗设备、药品、消毒用品等，以保障公共卫生安全。资源获取效率的高低直接影响到社区自组织应对突发公共卫生事件的反应速度和处理能力。其中，“信息资源传递效率”是关键[24]，准确、及时的信息可以帮助社区更好地了解疫情动态、防护知识和应对措施，从而提高自救互救的能力。信息资源传递效率高意味着社区可以更快速地做出反应，提高防控效果。“物资资源调配效率”是保障[24]。高效的物资资源调配意味着社区自组织可以及时弥补公共危机治理中应急资源的短缺，提高对突发公共卫生事件的应对能力，减轻危机带来的影响。
其次，“组织动员效率”是“社区自组织效率”的核心，由“人力资源调配效率”和“协作配合效率”概念意义构成。高效的组织动员是实现社区自组织效率的关键[25]。组织动员效率高意味着社区能够迅速、有效地调动人力、物资和信息等资源，有助于建立一个协作、快速响应和有准备性的公共卫生体系，提升社会对抗突发公共卫生事件的能力和韧性。这种效率体现在人力资源的合理调配和协作配合的紧密程度两个方面。其一，人力资源是社区自组织应对突发公共卫生事件的重要支撑，高效的人力资源调配意味着可以迅速形成应对力量，加强监测和预警工作。其二，信息共享是协作配合的基础，它能够帮助社区自组织了解社会危机的实时情况，以便更好地进行应对。资源互助是协作配合的关键，它能够帮助社区自组织充分利用现有资源，提高应对社会危机的能力。任务分工则是协作配合的保障，它能够确保每个社区自组织都能够明确自己的职责，以便更好地完成应对社会危机的任务。这种高效的协作配合有助于提高应对社会危机的效率，形成整体应对力量，避免资源浪费和信息孤岛。
再次，“社会服务效率”是“社区自组织效率”的重要方面，概念意义包括“服务辐射效率”“行动执行效率”和“服务对象需求满足程度”。高效的社会服务直接影响到社区服务的质量和效果，通过提高社会服务效率，可以更好地满足社会需求，确保平时治理有效、战时战之能胜[26]。“服务辐射效率”和“行动执行效率”是“社会服务效率”的基础，能够提高服务效率和质量。而“服务对象需求满足程度”则是“社会服务效率”的目标，能够提高服务的社会效益。“服务辐射效率”是衡量社区自组织在危机救援与处置阶段覆盖面和服务深度的关键指标，其包括服务辐射范围、服务覆盖面和服务辐射深度等。高效的服务辐射意味着社区自组织可以最大程度地覆盖受灾群体，提供帮助和支持，最大限度减少灾害造成的损失，加强社区的抗灾能力。高效的行动执行意味着社区自组织可以在危机中迅速组织起来，有效地展开救援行动，提高救援效率，减少灾害带来的伤亡和损失。高满足程度意味着社区自组织可以更好地了解和满足受灾群体的需求，提高灾后重建效率，增强社区的凝聚力和复原能力。
最后，“秩序恢复效率”是“社区自组织效率”的保障。概念意义包括“卫生防疫体系健全效率”“经济重建效率”和“社会维稳效率”。秩序恢复效率的高低直接影响到社区自组织效率的发挥和持续性。高效的秩序恢复是提升社区公共卫生突发公共卫生事件治理韧性的关键特征，不仅标志着本次突发公共卫生事件应对的圆满结束，更是对下次可能发生的突发公共卫生事件的准备。在恢复与重建时期，高效地重建完善卫生防疫体系是至关重要的。可以确保疫情得到控制并防止二次暴发，有效保障人们的健康和生命安全。高效的经济重建意味着社区自组织可以通过恢复疫情引发的经济停滞或衰退，影响社会的稳定和人民的生活水平。高效的社会稳定恢复意味着社区自组织发挥自身自发性、专业性和灵活性等特点，针对疫情后村容整合整治、生活失序等问题，及时采取有效措施，避免大规模社会动荡和混乱的发生。提升社会维稳效率，不仅有助于增强社会的凝聚力和稳定性，更为社会秩序的恢复和社会的发展奠定了坚实基础。
后疫情时代，提升公共卫生韧性迫在眉睫。在中国社区公共卫生危机治理场域中，社区自组织效率是构建公共卫生韧性的重要内容，也是推进公共卫生体系韧性建设的重要抓手。通过上述分析，本研究认为社区自组织只有具备“社区自组织效率”核心内涵的积极方面，提高社区的整体抗风险能力和效率，才能更好地高效应对突发公共卫生事件，进而构建具有韧性的公共卫生体系。
