Objective

To understand the epidemiological characteristics of foodborne diseases in Liaoning province and provide a theoretical basis for the prevention and control of related diseases.

Methods

Epidemiological analysis was conducted on foodborne disease surveillance data from 2014 to 2023 in Liaoning province. Spatial autocorrelation and spatiotemporal analyses were used to explore the temporal and spatial clustering of cases.

Results

From 2014 to 2023, a total of 208 455 cases of foodborne diseases were reported in Liaoning province, with an incidence rate of 48.94/100 000 population and a hospitalization rate of 9.58%. Foodborne diseases were most prevalent from June to September. Dalian city and Shenyang city reported the highest number of cases, with 39 073 and 33 720 cases, respectively. The male-to-female ratio of reported cases was approximately 1 : 1.05. The 25–44 age group had the highest number of reported cases (65 679 cases, accounting for 31.51%), while the 0-year age group had the highest incidence rate (164.12/100 000 population). Spatial autocorrelation analysis showed that the global Moran′s I index of foodborne diseases in Liaoning province was 0.163 6, indicating significant regional aggregation. Spatiotemporal scanning with a monthly cycle identified four clusters centered in Dandong city, Dalian city, Shenyang city, and Fuxin city, respectively.

Conclusions

Foodborne diseases in Liaoning province are prevalent in summer, and cases are mainly concentrated in coastal areas and areas with high population density, exhibiting significant spatiotemporal clustering. Targeted health education and strengthened supervision are recommended to reduce the number of cases in key areas and during key months.

目的

了解辽宁省食源性疾病流行特征，为相关疾病的预防控制提供理论依据。

方法

对辽宁省2014—2023年食源性疾病监测数据进行流行病学分析，利用空间自相关分析和时空分析探究病例在时间和空间上的聚集情况。

结果

2014—2023年，辽宁省共报告食源性疾病208 455例，发病率为48.94/10万人，住院率为9.58%。食源性疾病高发于每年6—9月，大连市、沈阳市报告病例数多，分别为39 073、33 720例。报告病例男女性别比约为1 : 1.05，25～44岁年龄组报告病例数最多，共65 679例，占31.51%，0～岁年龄组发病率最高，为164.12/10万人。空间自相关分析显示，辽宁省食源性疾病全局Moran′s I指数为0.163 6，区域集聚性明显，以月为周期进行时空扫描，共发现4个聚集区，分别以丹东市、大连市、沈阳市、阜新市为中心。

结论

辽宁省食源性疾病夏季高发，病例主要集中在沿海及人口密度大的地区，存在显著的时空聚集性，建议有针对性地加强宣教，并加大监管力度，以降低重点地区及季节的发病率。

目的

了解辽宁省食源性疾病流行特征，为相关疾病的预防控制提供理论依据。

方法

对辽宁省2014—2023年食源性疾病监测数据进行流行病学分析，利用空间自相关分析和时空分析探究病例在时间和空间上的聚集情况。

结果

2014—2023年，辽宁省共报告食源性疾病208 455例，发病率为48.94/10万人，住院率为9.58%。食源性疾病高发于每年6—9月，大连市、沈阳市报告病例数多，分别为39 073、33 720例。报告病例男女性别比约为1 : 1.05，25～44岁年龄组报告病例数最多，共65 679例，占31.51%，0～岁年龄组发病率最高，为164.12/10万人。空间自相关分析显示，辽宁省食源性疾病全局Moran′s I指数为0.163 6，区域集聚性明显，以月为周期进行时空扫描，共发现4个聚集区，分别以丹东市、大连市、沈阳市、阜新市为中心。

结论

辽宁省食源性疾病夏季高发，病例主要集中在沿海及人口密度大的地区，存在显著的时空聚集性，建议有针对性地加强宣教，并加大监管力度，以降低重点地区及季节的发病率。

Objective

To understand the epidemiological characteristics of foodborne diseases in Liaoning province and provide a theoretical basis for the prevention and control of related diseases.

Methods

Epidemiological analysis was conducted on foodborne disease surveillance data from 2014 to 2023 in Liaoning province. Spatial autocorrelation and spatiotemporal analyses were used to explore the temporal and spatial clustering of cases.

Results

From 2014 to 2023, a total of 208 455 cases of foodborne diseases were reported in Liaoning province, with an incidence rate of 48.94/100 000 population and a hospitalization rate of 9.58%. Foodborne diseases were most prevalent from June to September. Dalian city and Shenyang city reported the highest number of cases, with 39 073 and 33 720 cases, respectively. The male-to-female ratio of reported cases was approximately 1 : 1.05. The 25–44 age group had the highest number of reported cases (65 679 cases, accounting for 31.51%), while the 0-year age group had the highest incidence rate (164.12/100 000 population). Spatial autocorrelation analysis showed that the global Moran′s I index of foodborne diseases in Liaoning province was 0.163 6, indicating significant regional aggregation. Spatiotemporal scanning with a monthly cycle identified four clusters centered in Dandong city, Dalian city, Shenyang city, and Fuxin city, respectively.

Conclusions

Foodborne diseases in Liaoning province are prevalent in summer, and cases are mainly concentrated in coastal areas and areas with high population density, exhibiting significant spatiotemporal clustering. Targeted health education and strengthened supervision are recommended to reduce the number of cases in key areas and during key months.

食源性疾病（foodborne disease，FBD）指通过摄食方式进入人体的各种致病因子如细菌、病毒、微生物、化学物质等，引起的具有感染或中毒性质的一类疾病，全球每年食源性疾病病例数达6亿例，导致42万人死亡[1]。加拿大安大略省食源性疾病每年约177 000例，导致59人死亡[2]。估计我国每年由非伤寒沙门菌及副溶血性弧菌导致的食源性疾病患病人数可达1 400万例[3]。明确食源性疾病的流行特点及负担情况，有助于疾控部门及医疗机构更好地预防控制相关疾病。本研究通过对2014—2023年辽宁省食源性疾病病例监测系统收集到的食源性疾病病例数据进行分析，了解辽宁省食源性疾病流行特征及聚集情况，为食源性疾病防治提供数据依据。
1　资料与方法

1.1　数据来源
数据来源于2014—2023年辽宁省食源性疾病病例监测系统上报的食源性疾病病例，包括各级医疗机构病例监测数据及哨点医院主动监测数据。病例定义为[4]：（1）腹泻：24 h内排便≥3次，且伴有粪便性状异常；（2）发生≥1次呕吐患者。医务人员询问患者基本情况、饮食暴露史等，结合患者样本检测结果、诊断结论等信息共同报告至食源性疾病病例监测系统。医务人员询问患者信息时已征询患者知情同意，监测工作通过上报医院的伦理学审查。人口学数据来源于2020年辽宁省第七次人口普查数据。
1.2　方法
1.2.1　描述性分析
基于食源性疾病病例监测系统数据，描述病例在时间、空间、人群上的分布特点，并分析食源性疾病病例数逐年变化趋势。
1.2.2　时空分析
利用Arcgis10.8.1软件进行空间统计分析，使用莫兰指数（–1<Moran′s I<1）作为指标进对病例数据进行分析，包括全局莫兰指数（Global Moran's I）和局部莫兰指数（Anselin Local Moran's I）两部分。在分析食源性疾病病例在辽宁省全局范围内是否存在空间聚集性关系时，使用反距离法标准化数据，计算全局莫兰指数，当P<0.05时，全局莫兰指数越大，其聚集性越显著。在进行局部相关分析时，使用局部莫兰指数分析不同地区间食源性疾病病例的聚集情况,衡量不同地区病例间的空间相关性。使用SatScan v10.1.2软件对辽宁省食源性疾病病例进行时空扫描，判断病例是否在特定时间或地区出现集群现象，设置最大潜在风险人口为30%，基于离散型泊松分布，分别以月及年为周期对病例数据进行扫描，以对数似然比（log likelihood ratio，LLR）最大且具有统计学意义（P<0.01）的窗口作为Ⅰ类聚集区域，其余窗口作为Ⅱ类聚集区域[5]。
1.3　统计分析
使用Excel 2019软件进行数据整理，计算发病率、住院率等流行病学指标。采用R 4.3.0软件进行χ2检验，比较性别与年龄组间病例数差异，进行Mann-Kendall时间趋势性检验，判断病例是否存在显著的趋势变化，统计分析结果均以P<0.05为差异有统计学意义。

2.1　基本情况（表1、2）
2014年—2023年辽宁省食源性疾病病例监测系统结果显示，共报告食源性疾病208 455例，年均报告20 846例，食源性疾病发病率为48.94/10万人。2017年发病率最高为105.59/10万人，2022年发病率最低为19.80/10万人，2019—2023年报告病例数低于2014—2018年，整体而言，病例数呈下降趋势但差异无统计学意义（Z= –0.894，P=0.371）。2014—2023年共报告食源性疾病死亡病例25例，住院人数共计19 978例，住院率为9.58%（19 978/208 455）。详见表1。男性101 722例，女性106 733例，男女性别比约为1:1.05。≥45岁患者以女性居多，<45岁患者以男性居多，女性患者发病率略高于男性。25～44岁年龄组报告病例数最多，占总病例数的31.51%（65 679/208 455），0～岁年龄组报告病例数最少，仅占1.73%（3 612/208 455）。从发病率来看，0～岁年龄组发病率最高，达164.12/10万人，其次为1～4岁年龄组，10～14岁年龄组发病率最低，为31.14/10万人。不同年龄组与性别间构成比差异均有统计学意义（χ2=19 304.508，P<0.001），见表2。
2.2　报告病例的时间分布
从报告病例的月分布情况来看，辽宁省食源性疾病多发生于夏季，即7—9月，病例数占总病例数的63.73%（132 847/208 455），8月报告病例数最多，为51 014例，占总病例数的24.47%（51 014/208 455），7月死亡人数最多，占总死亡人数的28.00%（7/25）。
2.3　病例地区分布
从病例地区分布情况来看，辽宁省食源性疾病病例以大连市、沈阳市及本溪市最多，病例数分别为39 073、33 720、22 228例，分别占18.74%（39 073/208 455）、16.18%（33 720/208 455）、10.66%（22 228/208 455）；辽阳市病例数最少为6 257例，占3.00%（39 073/208 455）。发病率最高的地区为本溪市167.63/10万人，发病率最低的地区为铁岭市33.35/10万人。
2.3.1　全局空间自相关分析
将2014—2023年辽宁省食源性疾病总病例数据以区县为单位进行全局空间自相关分析，全局莫兰指数为0.163 6，Z值为2.779 7（P<0.01），说明2014—2023年辽宁省食源性疾病病例存在显著的聚集性，且呈正相关趋势。对2014—2023年辽宁省食源性疾病各年度病例数据进行全局空间自相关分析，结果显示2018—2021年食源性疾病病例空间聚集性不显著，其余年份病例均存在空间聚集性，2014、2015和2017年空间聚集性最为明显（P<0.001）。
2.3.2　局部空间自相关分析（表3）
将辽宁省食源性疾病2014—2023年病例数据以区县为单位进行局部空间自相关分析，大连市每年均存在高–高型聚集区，2015年病例数据未识别出低–低型聚集区，可能与该年各地病例数均匀有关。累计病例数局部自相关分析结果显示，大连市瓦房店市、金州区、甘井子区、西岗区、沙河口区、中山区、旅顺口区；沈阳市和平区、沈河区、大东区为高–高聚集区；沈阳市沈北新区、于洪区；本溪市溪湖区、南芬区为低–高聚集区；营口市西市区；锦州市北镇市；铁岭市昌图县、开原市为低–低聚集区，差异均具有统计学意义（P<0.05）。
2.3.3　时空扫描分析（表4～5）
对辽宁省14个市的总病例数以月为周期进行时空扫描，共发现4个聚集区域，包括1个Ⅰ类聚集区域及3个Ⅱ类聚集区域。丹东市、锦州市构成半径为139.80 km的Ⅰ类聚集区域，LLR=17 469.94；大连市、营口市、葫芦岛市构成一个半径为214.51 km的Ⅱ类聚集区域，LLR=17 095.25；沈阳市、抚顺市构成一个半径为41.54 km的Ⅱ类聚集区域，LLR=7 958.90；阜新市、盘锦市构成一个半径为104.88 km的Ⅱ类聚集区域，LLR=6 449.59。对辽宁省14个市的总病例数以年为周期进行时空扫描，共发现3个聚集区域，包括由本溪市为中心的Ⅰ类聚集区域，LLR=17 469.94；锦州市、葫芦岛市、盘锦市、营口市、阜新市构成的半径为108.63 km的Ⅱ类聚集区域，LLR=5 474.59，即由沈阳市、抚顺市构成的半径为41.54 km的Ⅱ类聚集区域，LLR=4 375.33。
3　讨　论

辽宁省2014—2023年共报告食源性疾病208 455例，发病率为48.94/10万人，高于我国食源性疾病发病率（2/10万人）[6]，低于黑龙江急性胃肠炎发病率（354.4/10万人）[7]。本研究结果表明，25～44岁年龄组食源性疾病报告病例数最多，占比31.51%，可能与青年群体人口基数大有关，且青年群体更倾向于在外或外卖就餐，发生食源性相关疾病的风险更高。0～岁年龄组发病率最高，达164.12/10万人，可能与婴幼儿胃肠道系统发育不全，食物耐受性差等原因有关。食源性疾病住院率为9.58%，高于我国急性胃肠炎住院率4.02%[4]。辽宁省食源性疾病病例呈明显的夏季高发特点，与国内外多项研究结果一致[4,8]，此现象与夏季高温，食物保质期短有关，各地区病例报告数量以大连市、沈阳市及本溪市最多，本溪市发病率最高达167.63/10万人，远高于辽宁省平均发病率，可能与本溪市地形多为山地林区，民众喜爱山林野味有关。
通过计算2014—2023年辽宁省食源性疾病病例分布全局莫兰指数，食源性疾病在辽宁省存在显著的聚集性，但2018—2021年病例分布聚集性不显著，可能与新冠疫情下监测工作开展困难，数据质量难以保证有关。对累计病例进行局部自相关分析，大连市瓦房店市、金州区、甘井子区、西岗区、沙河口区、中山区、旅顺口区；沈阳市和平区、沈河区、大东区为高–高聚集区，可以认为，辽宁省食源性疾病集中发病于大连市及沈阳市，可能与上述地区人口基数大，经济水平高，医疗系统完善有关。对病例数进行逐年局部自相关分析，发现大连市每年都存在高–高聚集区，说明大连市是辽宁省食源性疾病发病的重点地区，这可能与大连市为沿海热门旅游城市，人员流动性强且海鲜食用量大导致副溶血性弧菌感染高发有关，此特征与辽宁省副溶血性弧菌感染检出率最高相符合[9]。
对辽宁省食源性疾病病例以月为周期进行时空扫描，发现Ⅰ类聚集区域1个，以丹东市为中心，发现Ⅱ类聚集区域3个，以大连市、沈阳市、阜新市为中心，病例聚集时间主要集中于6—9月份；将病例以年为周期进行时空扫描，发现Ⅰ类聚集区域1个，以本溪市为中心；发现Ⅱ类聚集区域2个，以锦州市、沈阳市为中心，聚集时间为2016—2020年。整体来看，聚集区域集中于人口密度大的城市或沿海地区，聚集时间符合食源性疾病的季节性特征，2020年以后聚集特征不显著可能与民众食品卫生意识提高，及新冠病毒感染疫情背景下，为避免交叉感染居民就医意愿下降，医院报告病例数降低有关。
本研究也存在一定的局限性和不足，如数据来自食源性疾病监测平台，各地监测水平及数据质量可能对研究结果产生影响，且未就医患者不在上报数据之中，故实际食源性疾病患病人数可能被低估，发病率偏低。整体而言，辽宁省食源性疾病存在明显的季节性、地区性及人群特征，病例在整体与局部维度上均存在明显的聚集性，结合时空扫描结果识别出辽宁省食源性疾病高风险地区及高风险时段。因此，建议在夏季加强食源性疾病预防科普宣教工作力度，特别是重点地区和重点人群，使居民全面科学了解食源性疾病，达到有效的疾病预防效果，在人口密度大，食源性疾病高发、空间聚集性强的地区除预防宣教工作外还应加强食品质量监测工作力度。
