Objectives

To review and analyze the distribution, spatiotemporal trends, and influencing factors of healthy ageing-related themes at the World Health Assembly (WHA) during 2024–2025.

Methods

We collected the agendas, side events, and member state statements from WHA77 and WHA78, and combined them with national socioeconomic development levels, public data, and policy documents. Descriptive analysis and Tobit regression were employed to present the current status and spatiotemporal changes and to examine the influences of the sociodemographic index (SDI), GDP per capita, population aging, and national policies and data capacity.

Results

Neither WHA77 nor WHA78 established an independent agenda item on healthy ageing, while relevant keywords were embedded within broader themes such as universal health coverage. Compared with WHA77, WHA78 demonstrated increased frequency and broader coverage of healthy ageing topics, particularly concentrated in Europe and the Asia-Pacific region. At WHA77, discussions were primarily linked to universal health coverage, whereas at WHA78 they were more frequently associated with chronic diseases and dementia. At WHA78, member states with available national data related to healthy ageing (β = 1.20, 95%CI: 0.21 to 2.18), higher GDP per capita (βQ1 = –3.13, 95%CI: –5.75 to –0.50), and middle SDI status (βHigh SDI = –2.15, 95%CI: –4.27 to –0.04) were more likely to address healthy ageing-related themes.

Conclusions

The frequency of healthy ageing-related themes at the WHA is influenced by factors such as national data capacity and socioeconomic development level. Although the WHA has not yet established an independent healthy ageing agenda item, healthy ageing-related themes are increasingly integrated into both formal agendas and side events, with expanding spatiotemporal coverage.

目的

系统梳理并分析2024—2025年世界卫生大会（WHA）对健康老龄化的主题分布、时空变化及WHO各成员国健康老龄化相关主题频次的影响因素。

方法

收集并整理世界卫生大会第77届和第78届会议（WHA77和WHA78）会议议程、边会议程及成员国发言记录，并结合各国社会经济发展水平、公开数据和政策文件，采用描述性分析与Tobit回归，描述涉及健康老龄化主题的现况及时空变化，并探讨社会人口指数（SDI）、人均GDP、人口老龄化水平以及国家政策与数据建设等因素的影响。

结果

两届WHA均未设立独立健康老龄化议题，但关键词嵌入在全民健康覆盖等议题中。相比WHA77，WHA78健康老龄化涉及频次增加，覆盖范围扩大，尤其集中于欧洲与亚太地区。WHA77多集中在全民健康覆盖议题，而WHA78则更多在慢性病和痴呆症议题。在WHA78中，具备健康老龄化相关国家数据（β=1.20，95%CI= 0.21～2.18）、较高人均GDP（βQ1 = –3.13，95%CI= –5.75～–0.50）以及中SDI（β高SDI= –2.15，95%CI= –4.27～–0.04）的成员国更可能涉及健康老龄化主题。

结论

WHA 中健康老龄化相关主题频次受不同国家数据能力、国家发展水平等因素的影响。尽管目前 WHA 尚未设立独立的健康老龄化议题，但相关主题在大会正式议程及边会议题的跨领域讨论中的涉及频次及其时空分布均呈现出持续上升与扩展的趋势。

目的

系统梳理并分析2024—2025年世界卫生大会（WHA）对健康老龄化的主题分布、时空变化及WHO各成员国健康老龄化相关主题频次的影响因素。

方法

收集并整理世界卫生大会第77届和第78届会议（WHA77和WHA78）会议议程、边会议程及成员国发言记录，并结合各国社会经济发展水平、公开数据和政策文件，采用描述性分析与Tobit回归，描述涉及健康老龄化主题的现况及时空变化，并探讨社会人口指数（SDI）、人均GDP、人口老龄化水平以及国家政策与数据建设等因素的影响。

结果

两届WHA均未设立独立健康老龄化议题，但关键词嵌入在全民健康覆盖等议题中。相比WHA77，WHA78健康老龄化涉及频次增加，覆盖范围扩大，尤其集中于欧洲与亚太地区。WHA77多集中在全民健康覆盖议题，而WHA78则更多在慢性病和痴呆症议题。在WHA78中，具备健康老龄化相关国家数据（β=1.20，95%CI= 0.21～2.18）、较高人均GDP（βQ1 = –3.13，95%CI= –5.75～–0.50）以及中SDI（β高SDI= –2.15，95%CI= –4.27～–0.04）的成员国更可能涉及健康老龄化主题。

结论

WHA 中健康老龄化相关主题频次受不同国家数据能力、国家发展水平等因素的影响。尽管目前 WHA 尚未设立独立的健康老龄化议题，但相关主题在大会正式议程及边会议题的跨领域讨论中的涉及频次及其时空分布均呈现出持续上升与扩展的趋势。

Objectives

To review and analyze the distribution, spatiotemporal trends, and influencing factors of healthy ageing-related themes at the World Health Assembly (WHA) during 2024–2025.

Methods

We collected the agendas, side events, and member state statements from WHA77 and WHA78, and combined them with national socioeconomic development levels, public data, and policy documents. Descriptive analysis and Tobit regression were employed to present the current status and spatiotemporal changes and to examine the influences of the sociodemographic index (SDI), GDP per capita, population aging, and national policies and data capacity.

Results

Neither WHA77 nor WHA78 established an independent agenda item on healthy ageing, while relevant keywords were embedded within broader themes such as universal health coverage. Compared with WHA77, WHA78 demonstrated increased frequency and broader coverage of healthy ageing topics, particularly concentrated in Europe and the Asia-Pacific region. At WHA77, discussions were primarily linked to universal health coverage, whereas at WHA78 they were more frequently associated with chronic diseases and dementia. At WHA78, member states with available national data related to healthy ageing (β = 1.20, 95%CI: 0.21 to 2.18), higher GDP per capita (βQ1 = –3.13, 95%CI: –5.75 to –0.50), and middle SDI status (βHigh SDI = –2.15, 95%CI: –4.27 to –0.04) were more likely to address healthy ageing-related themes.

Conclusions

The frequency of healthy ageing-related themes at the WHA is influenced by factors such as national data capacity and socioeconomic development level. Although the WHA has not yet established an independent healthy ageing agenda item, healthy ageing-related themes are increasingly integrated into both formal agendas and side events, with expanding spatiotemporal coverage.

随着全球人口老龄化进程的加速，健康老龄化问题已成为全球公共卫生领域亟待破解的重大挑战。为应对这一全球性议题，2020 年联合国大会正式通过“联合国健康老龄化十年（2021—2030）”全球行动计划，并明确由世界卫生组织（World Health Organization, WHO）担任核心执行机构[1]，以推动各国健康老龄化目标的实现。与此同时，WHO 为进一步强化健康老龄化工作的系统性推进，在组织架构层面作出关键调整，在全民健康覆盖与健康人口司下专设“人口变动与健康老龄化小组”，以统筹健康老龄化相关工作[2]。世界卫生大会（World Health Assembly ，WHA）作为 WHO 的最高决策机构，不仅承担WHO政策方向确定、审查并批准预算方案等核心职能，更是全球卫生治理领域凝聚共识、协调立场的关键平台。在银发浪潮席卷全球这一背景下，系统剖析 WHA 全会及边会议题设置、讨论焦点及变化趋势，对于精准把握全球卫生治理与国际卫生合作中健康老龄化的前沿动态，以及厘清各成员国在该议题上的立场倾向极为关键，为全球健康老龄化政策的制定重点优化、实施路径调整及国际合作方向提供科学参考依据。对此，本研究以第77、78届WHA可公开获取的发言内容为研究对象，通过梳理两届大会全会、甲、乙委全程发言内容、议程安排及其边会议题，分析健康老龄化相关主题频次、呈现形式和时空分布情况，并探索不同社会经济发展水 平与人口老龄化程度下，各国对健康老龄化相关议题的关注差异及影响因素。
1　资料与方法

1.1　资料来源
本研究资料主要来源于2024年WHA77、2025年WHA78全体会议、甲委员会、乙委员会议程安排[3–4]、边会议程安排[5–6]以及两届大会的全程发言内容健康老龄化相关主题词频次统计；其中，WHA发言内容来源于会议全程发言记录，涵盖总干事、大会主席、成员国、观察国、非国家行为体等发言内容。此外，从联合国《2021—2023年健康老龄化十年规划进展报告》中获取了各国健康老龄化治理规则、治理主体、老年健康相关国家数据和人口老龄化程度数据[7]。从WHO“全球卫生支出数据库（Global Health Expenditure Database）”获取了人均国内生产总值（GDP）数据[8]，从全球疾病负担（GBD）数据库获取了社会人口指数数据[9–10].
1.2　变量及其测量
本研究因变量为WHA中各成员国涉及健康老龄化相关主题词频次，基于上述WHA会议资料提取关键词获得。其中，关键词包含老年相关（aged/older adults/older individuals/ elderly/senior）以及健康老龄化相关（population aging/healthy aging/aging society/aged society; dementia; hearing loss/hearing impairment/deaf; vision loss/vision impairment/blind; disability/disable）。自变量为各成员国是否有健康老龄化治理规则、治理主体、国家数据，国内生产总值（GDP）、社会人口发展指数（sociodemographic index, SDI）和人口老龄化水平[7–11]。其中，治理规则通过该国家是否实施了健康老龄化相关国家计划、政策或战略、全面评估老年人健康和社会护理需求的国家政策进行测量[7]；治理主体通过该国家卫生部门是否设有老龄化与健康问题协调中心、国家层面老龄化和健康问题多利益相关方论坛、委员会进行衡量[7]；国家数据通过该国家是否有包含老年健康状况及需求的全国代表性调查进行衡量[7]；人均GDP通过以2022年美元汇率换算，并按四分位数划分为Q1（最低水平）至Q4（最高水平）[8]；SDI基于生育率、教育水平和人均收入计算，取值范围为0～1，得分越高则综合发展水平越高，依据全球疾病负担分类标准划分为五层级[9–10]；人口老龄化水平通过≥60岁人口占总人口比例测量，基于联合国标准划分老龄化程度[11]。
1.3　统计分析
本研究基于健康老龄化主题关键词的频次，运用描述性方法分析WHA77、78涉及健康老龄化相关主题现况，根据不同SDI和人口老龄化水平，分析各成员国涉及主题平均频次及其时空变化。鉴于健康老龄化相关主题在各国涉及频次分布情况，采用Tobit回归分析主题词频次的相关影响因素，以P<0.05作为差异有统计学意义。所有分析采用Stata17.0软件进行，结果可视化分别运用R语言forest包、ggplot2包绘制。

2.1　WHA关于健康老龄化相关主题词频次分布（图1）
虽然WHA77、78均未设置专门针对健康老龄化的议题，但在全民健康覆盖、传染病大流行、慢性非传染性疾病、痴呆症和巴以冲突等其他议题中均有所涉及。从以上议题涉及健康老龄化相关主题词频次分布看，WHA77主要集中在全民健康覆盖议题下（频次=18次），WHA78则主要集中在痴呆症（频次=25次）和慢性非传染性疾病（频次=36次）议题中。
2.2　不同SDI及人口老龄化水平下WHA中各成员国健康老龄化相关主题词频次分布（图2）
处于不同SDI水平的成员国针对健康老龄化的平均涉及频次呈现出一定差异。其中，低SDI、中低SDI、中等SDI区域健康老龄化平均涉及频次随SDI上升而升高，分别为0.2、0.4和0.6，而中高SDI区域和高SDI区域的涉及频次分别为0.6和0.5。此外，处于非老龄化社会、老龄化社会、老龄社会的成员国针对健康老龄化的平均涉及频次分别为0.3、0.5、0.7，而超老龄社会的国家的涉及频次为0.6。
2.3　WHA中各成员国健康老龄化相关主题词频次的时空分布状况（图3）
如图3所示，2024年WHA77中健康老龄化相关主题词频次的分布呈现低频和稀疏分布的特点，多数国家仅涉及1次，仅少数国家达到2次（如智利、布基纳法索），整体尚未形成广泛稳定的议题聚焦。从空间分布来看，相关主题词主要零散分布于美洲区域（如美国、哥伦比亚等）、非洲西部及东部少数国家（如塞拉利昂、尼日尔等）、东地中海区域（如黎巴嫩、伊朗、巴基斯坦等），以及西太平洋区域（如日本、韩国、澳大利亚、新西兰等），欧洲区域仅出现个别国家参与（如英国、奥地利等）。相比之下，2025年WHA78中健康老龄化相关主题频次和国家覆盖范围均扩大，呈现出由分散走向集中的变化特征。其中，欧洲区域成为最为突出的集聚区域，涵盖英国、法国等十余个国家，其中频次最高的为波兰（5次）；西太平洋区域参与度上升，其中中国有3次；非洲涉及国家的范围从西非扩展到更广区域，以冈比亚（4次）最为突出；美洲区域则在加勒比和中南美形成多点分布情况。总体而言，健康老龄化相关主题词在WHA中的关注由2024年的零散、低频分布，转为2025年的广域覆盖与区域集群化发展，主要集中在欧洲，同时美洲与非洲的参与度提升，频次≥2次的国家数量增加，而东南亚区域的参与度仍相对较低。

如图4所示，是否有健康老龄化相关国家数据、人均GDP以及SDI是2025年WHA78各成员国健康老龄化涉及频次的显著影响因素。其中，在具备健康老龄化相关国家数据的成员国中，其健康老龄化涉及频次平均增加1.20次（β=1.20，95%CI=0.21～2.18）。与人均GDP最高的成员国相比，人均GDP降低会显著减少健康老龄化涉及频次（Q1：β= –3.13，95% CI= –5.75～–0.50），而与中SDI的成员国相比，中高SDI（β= –1.55，95% CI= –3.02～–0.08）和高SDI（β= –2.15，95% CI= –4.27～–0.04）成员国健康老龄化相关主题词频次也显著较低。然而，上述因素对2024年WHA77健康老龄化相关主题词频次均无显著影响。
2.5　WHA边会涉及健康老龄化的议题分布情况（表1）
如表1所示，WHA77会议期间，仅有1个健康老龄化相关主题边会，即挪威卫生和保健服务部主办的“创建对老年人友好的城市和社区”主题边会，共提方的专家代表分别分享了爱尔兰、法国、瑞士、挪威的治理经验。在WHA78会议期间，共有4个涉及健康老龄化的边会主题，分别为：（1）卡塔尔主办的“加速‘2021—2030年联合国健康老龄化行动’的影响力——投资于连续性照护体系和适老化环境建设”，为官方边会；（2）英国国际长寿中心主办的“构建适应未来的健康社会：2025年度健康老龄化与预防指数发布”；（3）国际临终关怀与姑息治疗协会和世界基督教教会联合会共同主办的“全球资金危机下为老年人与残障人士倡导跨宗教安宁疗护服务”；（4）中国复旦大学公共卫生学院主办的“更好的社区健康：老龄化、城市化、信息化时代的社区健康新图景”。

本研究系统分析了WHA77、78议程、边会日程及全会和甲、乙委大会发言内容中健康老龄化相关关键词的涉及情况，并探讨了不同国家特征对其提及频次的影响。研究结果显示，健康老龄化虽未作为独立议题出现，但在多个相关议题中逐渐获得更高关注度。而成员国的人均 GDP 水平、SDI 水平以及是否具备健康老龄化相关国家数据是影响其在2025年WHA78涉及健康老龄化主题频次的重要因素。WHA中各成员国针对健康老龄化涉及频次增加，在空间分布上覆盖范围也扩展至全球多数区域。边会议题的数量和内容也显示出非国家行为体（如学术机构、国际非政府组织等）在推动健康老龄化议题中扮演了重要角色。
尽管WHA77、78均未单独设立健康老龄化相关议题，但其已被广泛嵌入到全民健康覆盖、慢性非传染性疾病、痴呆症以及公共卫生危机等议题之中。尤其在WHA78中，痴呆症与慢性病议题成为健康老龄化相关词汇出现最多的领域。这一方面反映出健康老龄化正在逐步渗透到全球公共卫生治理的主流框架之中，与多项可持续发展目标密切相关，另一方面也提示其尚未获得足够的政治能见度和政策优先性。从时空变化来看，WHA77涉及健康老龄化频次较低且分布零散，至WHA78年不仅频次上升，覆盖范围也扩展至全球多数区域，新兴关注热点逐渐形成于欧洲与亚太，同时非洲和拉美地区的参与度也有所提升。这既反映了全球人口老龄化进程的加速，也提示各地区在应对这一挑战时存在差异化路径和政策重心的动态调整。这种变化可能与“联合国健康老龄化十年（2021—2030）”[1]倡议的持续推进、区域间经验交流的增加以及部分国家国内老龄化政策成熟度的提高有关。部分在2024年有发声的国家未在2025年再次出现，提示健康老龄化的国际关注仍存在波动性，可能受政治议程、国内公共卫生优先事项或国际热点事件（如地区冲突）影响。
本研究发现具备健康老龄化相关国家数据、较高的人均GDP以及处于中等SDI水平的成员国，在WHA中健康老龄化涉及频次的显著更高。这一发现提示，数据监测能力不仅是国家内部健康政策制定的基础，也决定了其能否在国际场合中更有力地提出健康老龄化议题。同时，低GDP国家在大会中涉及频次明显较低，可能反映其在议题竞争中话语权不足，或因资源有限而优先聚焦于传染病防控等更为紧迫的问题。值得注意的是，高SDI国家涉及频次反而低于中SDI国家，可能反映出高SDI国家内部健康老龄化政策体系已较为成熟，其在国际场合更倾向于推动其他更迫切的公共卫生议题；而中SDI国家可能正处于人口老龄化加速与健康体系转型的关键阶段，因此更积极地将健康老龄化纳入国际讨论。此外，健康老龄化涉及的边会数量在2024—2025年显著增加，涵盖成员国、学术机构及社会组织。这一趋势显示，多部门、多利益相关方对健康老龄化问题的关注度提升，尤其集中在长期照护、适老环境建设等领域。这与“联合国健康老龄化十年”的优先行动领域[1]高度契合，并积极响应WHO“人口变动与健康老龄化小组”核心工作方向[2]，也反映出健康老龄化正逐步融入全球卫生对话。
综上所述，WHA 中健康老龄化相关主题频次受到国家数据能力、国家发展水平等因素的影响。尽管目前 WHA 尚未设立独立的健康老龄化议题，但相关主题在大会正式议程及边会议题的跨领域讨论中，其涉及频次及其时空分布均呈现出持续上升与扩展的趋势。未来WHO及成员国应考虑将健康老龄化提升为更加突出的正式议程，同时加强中低收入国家的健康老龄化数据和政策能力建设，鼓励更多中低收入国家参与国际对话，并继续发挥多元主体在边会中的倡议和协调功能，以实现“联合国健康老龄化十年”目标。
