Objective

To sort out the digital health policy texts of China′s central government and its affiliated departments and analyze the characteristics and existing problems of the policies, thus providing suggestions for optimization of digital health policies.

Methods

A total of 37 digital health policies were collected from the websites of PKULaw, National Health Commission, and the State Council of China with the time interval from 2000 to 2024. A two-dimensional framework of policy tools-policy objectives was constructed for the quantitative analysis of the collected policy texts.

Results

In the dimension of policy tools, environment-based tools (67%) were dominant, of which "sound mechanisms" accounted for the highest proportion (40%), while the supply-based (22%) and demand-based (11%) applications were obviously insufficient. Supply-based tools demonstrated high dependence on "scientific and technological support" (84%), with insufficient attention to "talent support" (12%) and "financial support" (4%). In the dimension of policy objectives, the policy distribution was concentrated on "performance guarantee" (32.5%) and "institutional rules" (17.7%), while "data resources" (11.3%) and "platform support" (7.1%) received less attention. Cross-analysis further revealed that there was a structural mismatch between policy tools and policy objectives.

Conclusions

It is recommended that the future efforts should be made to optimize the structure of policy tools and coordinate the combination of tools, balance the distribution of objectives considering the existing policy shortcomings, and focus on the multidimensional synergy of policy tools and policy objectives.

目的

梳理我国中央政府及所辖部门数字健康相关政策文本，分析政策的特征以及存在的问题，为推动数字健康政策的优化提供建议。

方法

通过“北大法宝”、国家卫生健康委员会以及中国政府网等网站，检索时间为2000—2024年，收集37份数字健康相关政策，构建“政策工具—政策目标”二维框架并以此为基础对收集的政策文本进行量化分析。

结果

政策工具维度下以环境型（67%）为主，其中“健全机制”占比最高（40%），而供给型（22%）与需求型（11%）应用明显不足；供给型工具内部高度依赖“科技支持”（84%），“人才支持”（12%）及“财政支持”（4%）存在短板。政策目标维度分析显示，分布集中于“履职保障”（32.5%）与“制度规则”（17.7%），“数据资源”（11.3%）及“平台支撑”（7.1%）关注度较低。交叉分析进一步揭示，政策工具与政策目标间存在结构性错配。

结论

建议优化工具结构，协调工具组合；平衡目标分布，关注政策目标短板；注重政策工具与政策目标的多维协同。

目的

梳理我国中央政府及所辖部门数字健康相关政策文本，分析政策的特征以及存在的问题，为推动数字健康政策的优化提供建议。

方法

通过“北大法宝”、国家卫生健康委员会以及中国政府网等网站，检索时间为2000—2024年，收集37份数字健康相关政策，构建“政策工具—政策目标”二维框架并以此为基础对收集的政策文本进行量化分析。

结果

政策工具维度下以环境型（67%）为主，其中“健全机制”占比最高（40%），而供给型（22%）与需求型（11%）应用明显不足；供给型工具内部高度依赖“科技支持”（84%），“人才支持”（12%）及“财政支持”（4%）存在短板。政策目标维度分析显示，分布集中于“履职保障”（32.5%）与“制度规则”（17.7%），“数据资源”（11.3%）及“平台支撑”（7.1%）关注度较低。交叉分析进一步揭示，政策工具与政策目标间存在结构性错配。

结论

建议优化工具结构，协调工具组合；平衡目标分布，关注政策目标短板；注重政策工具与政策目标的多维协同。

Objective

To sort out the digital health policy texts of China′s central government and its affiliated departments and analyze the characteristics and existing problems of the policies, thus providing suggestions for optimization of digital health policies.

Methods

A total of 37 digital health policies were collected from the websites of PKULaw, National Health Commission, and the State Council of China with the time interval from 2000 to 2024. A two-dimensional framework of policy tools-policy objectives was constructed for the quantitative analysis of the collected policy texts.

Results

In the dimension of policy tools, environment-based tools (67%) were dominant, of which "sound mechanisms" accounted for the highest proportion (40%), while the supply-based (22%) and demand-based (11%) applications were obviously insufficient. Supply-based tools demonstrated high dependence on "scientific and technological support" (84%), with insufficient attention to "talent support" (12%) and "financial support" (4%). In the dimension of policy objectives, the policy distribution was concentrated on "performance guarantee" (32.5%) and "institutional rules" (17.7%), while "data resources" (11.3%) and "platform support" (7.1%) received less attention. Cross-analysis further revealed that there was a structural mismatch between policy tools and policy objectives.

Conclusions

It is recommended that the future efforts should be made to optimize the structure of policy tools and coordinate the combination of tools, balance the distribution of objectives considering the existing policy shortcomings, and focus on the multidimensional synergy of policy tools and policy objectives.

通过“数字+健康”的融合实现“技术+需求”的发展，对构建医疗管理服务新模式，提高服务效率和资源配置水平，推动数据资源要素的流动等具有重要意义[1]。世界卫生组织于2019年在数字健康全球战略中提出：推动数字技术在健康领域的应用，提升医疗服务的可及性和质量[2]。我国当前正处于健康中国和数字中国建设的大背景下，数字健康发展对人民生命健康的维护和高质量数字社会建设起着至关重要的作用。目前关于数字健康的定义尚未有一致定论，欧盟将数字健康定义为运用信息通信技术为医疗主体及其客体提供服务[3]。世界卫生组织认为数字健康是开发利用数字信息技术以改善和服务人们的健康[2]。罗宾逊等学者认为数字健康是“利用数字媒体来改变医疗服务的构想和提供方式”[4]。在此基础上，本研究认为数字健康即利用数字技术对健康方面的管理和服务。近年来，国家相继出台政策措施统筹推进数字健康发展，但目前关于数字健康政策文件的量化分析的研究较少，无法全面了解数字健康政策的现状和进展。因此，本研究收集北大法宝、国家卫生健康委员会以及中国政府网等网站2000—2024年发布的数字健康相关政策文件，采用内容分析法构建“政策工具—政策目标”二维框架对国家层面颁布的数字健康相关政策文件进行了量化分析，旨在分析中国数字健康政策体系现状，为后续我国数字健康政策体系优化提供建议。结果报告如下。
1　资料与方法

1.1　资料来源（表1）
本文收集的政策文本主要来源于北大法宝、国家卫生健康委员会以及中国政府网等网站，以“数字”“健康”“远程医疗”“卫生”“信息化”等作为检索主题词，检索时间为2000—2024年，选取与数字健康相关的政策文件为研究对象，并且根据参考文献中提到的相关政策进行了查漏补缺。
1.2　方法（表2、3，图1）
为保证所纳入政策文本的科学性和有效性，对文本的选取设置3个纳入标准和4个排除标准。3个纳入标准如下：（1）政策文本中包含数字健康或数字健康相关内容，包括“互联网+医疗”、远程医疗、健康服务业、医疗信息化等；（2）考虑到数字健康政策文本的权威性和代表性，纳入政策文本均为中央政府及所辖部门颁布；（3）选择通知、意见、法规等政策文件类型。4个排除标准如下：（1）无数字健康或互联网医疗等相关条款，文本中无实质内容；（2）新闻报道、会议讲话、政策解读等文件；（3）部门间转发、印发等内容重复的政策文本；（4）已失效政策文本。共检索到相关政策文本399份，经过检索筛选，最终选取37份符合条件的政策文本。
为确保编码的客观性和可靠性，本研究采用双人独立编码机制。两人基于统一的编码手册对相关政策条款进行分类，通过Nvivo 12软件交叉验证。初始编码一致性为85%，研究团队针对分歧条款修订编码手册，以明确模糊类别的界定标准。此外，抽取10%的随机样本，邀请第三方进行复核，确认编码结果具有较高可信度。
本研究将政策文本中数字健康相关条款作为基本分析单元，应用Nvivo12将收集的37份数字健康政策按照“政策序号—章节序号—条款序号”的方式，对具体条款进行编码。例如，13–24–1表示《“健康中国2030”规划纲要》的第24章第1节内容“全面建成统一权威、互联互通的人口健康信息平台……”。按照上述方式，共得到480条编码单元。（1）政策工具（X维度）：每一条政策都是目标与工具的有机统一, 工具是通往目标的必经道路[5]。本研究运用Rothwell和Zegveld的理论，将数字健康政策文本中的政策工具划分为供给型、需求型和环境型3大类[6]，供给型政策工具是指政府在资源要素方面的投入，如财政支持、人才支持、科技支持等，表现为对数字健康发展的推动力；需求型政策工具是指政府对数字健康发展进行扶持，降低外部效应，例如设立示范工程、促进数据资源共享、绩效监管与评估等，表现为政府对数字健康发展的拉力；环境性政策工具是指政府利用某种执行手段保证数字健康政策的执行效果，从而为数字健康发展提供良好的社会环境，如法规管制、健全机制、组织保障等，表现为政策对于数字健康发展的指导和保障（见表2）。（2）政策目标（Y维度）：根据相关政策和文献以及数字健康发展的实际意义确立具体政策目标。国务院《关于促进“互联网+医疗健康”发展的意见》对互联网医疗的制度规则确立、数据资源共享、医疗监管体系等方面提出了明确要求[7]。此外，孙茜等[8]在我国医疗治理现代化建设的重要意义中提到数字技术赋能医疗机构和医生，提升医疗服务供给能力，提高医务人员工作效率，同时在数字技术赋能医疗治理的关键问题中提出在数据开放共享中需要明确数据的资产属性、关注数据安全与科技伦理问题。刘佳慧等[9]从患者互联网医疗满意度的角度提出费用合理、流程便捷、完善硬件支持、隐私性强的平台支持不可或缺。综上，将我国数字健康政策目标归纳为安全保障、履职保障、平台支撑、数据资源、医疗监管和制度规则6个方面（见表3）。综合以上两个维度，最终形成我国数字健康政策分析框架图，例如：《医疗卫生机构网络安全管理办法》第二章第13条“上线前应评估新技术的安全风险并进行安全管控”对应“环境型工具—安全保障目标”（见图1）。

2.1　政策工具维度分析（表4）
对纳入的37份文本进行政策工具维度分析表明，环境型政策工具的使用占比最高（67%），其中，健全机制使用最多（40%），而组织保障相对较少（13%）；供给型政策工具使用较少（22%），其中，科技支持（84%）被重点关注，反映了政府在数字健康发展中对信息科技的高度重视，但政府对数字健康发展提供的人才支持（12%）和财政支持（4%）较为欠缺；需求型政策工具使用比例最低（11%），内部各子工具中资源共享（51%）占比最高，表明资源共享是数字化时代重要特征，数字健康发展离不开信息数据资源的共用。
2.2　政策目标维度分析
6个政策目标间分布差异显著：履职保障占比最大，编码156条（32.5%）；其次是制度规则，编码85条（17.71%）；安全保障编码76条（15.83%）；医疗监管75条（15.63%）；数据资源54条（11.25%）；平台支撑使用频率最低，编码34条（7.08%）相对缺乏。
2.3　政策工具与政策目标交叉分析（表5）
综合以上统计结果，可以获得数字健康政策文本“工具—目标”二维分布情况如下（表5）：6个政策目标在3类政策工具中均有体现，但环境型政策工具在各政策目标中应用最多，且主要集中于履职保障方面。总体而言，各类政策目标均高度依赖环境型政策工具，安全保障、履职保障、平台支撑、医疗监管4个政策目标更加偏向于供给型政策工具，数据资源、制度规则2个政策目标则更加偏向需求型政策工具。
3　讨　论

本研究基于“政策工具—政策目标”二维框架，对我国中央政府及所辖部门数字健康政策进行了系统性量化分析。结果表明，我国已初步构建起一个以环境型工具为支撑、以履职保障和制度规则建设为核心目标的数字健康政策体系，这充分体现了中央政府对于数字健康发展的高度重视与战略布局。该体系在顶层设计、规则制定和快速响应突发公共卫生事件等方面展现出显著优势，为数字健康的发展提供了基础性的制度保障和方向指引。然而，本研究也客观揭示了现行政策体系在工具结构、目标分布与二维协同方面存在一定的可优化空间，这既揭示了当前阶段存在的不足，也为未来政策迭代升级提供借鉴。
结果显示，我国中央政府及所辖部门数字健康政策存在政策工具外部结构和内部组合有待进一步平衡，需要优化工具结构，协调工具组合。在数字健康发展中过度倚重环境型工具，易形成强制性命令，不利于地方根据自身实际情况采取针对性措施，从而降低政策文本的可操作性。供给型和需求型工具的缺失容易造成数字健康发展的推动力不足，数字医疗服务体系建设缺乏资金、技术和人才，医疗服务效率和成本难以得到保障。因此，要调整政策工具结构。其一，增加数字医疗领域的资金投入，加强相关业务人员培训，使供给型政策工具更好地发挥对数字健康发展的推动作用。其二，适当增强需求型政策工具的使用力度，增加此类工具的整体占比。另外，需求型工具的内涵也有待完善。例如，海外交流这一政策工具并没有提及，而目前我国数字健康服务还处于拓展国内市场阶段，需求型工具的拉动作用较环境型更为直接，为后续政策制定提供可扩展空间[10]。同时，不同工具之间也需要进一步均衡协调。其中，供给侧工具中更偏向科技支持。需求型政策工具更重视资源共享，推动医疗信息数据开放共享，但示范项目和绩效监管相对不足。环境型工具偏重健全机制和法规管制，数字医疗在对传统医疗服务体系带来了巨大变革的同时,也带来诸多新问题，如：新技术涉及的法律权责、医疗事故责任认定、健康信息隐私保护等，这些新形势和新问题都需要通过健全体制机制和完善法律法规来规范[11] ，然而组织保障和策略性措施的缺乏也会导致数字健康制度规范建设受阻。因此，要关注政策工具之间的协同性。例如，在推动数据资源共享的同时，关注信息的安全保障问题，同时加强制度规则的运用以提供数据使用制度规范，因此需要以相关法律法规政策为支撑，推动医疗数据安全服务体系的建设；在使用示范项目工具的同时，配合供给型政策工具的使用，同时加强绩效监管和评价工具的应用，为项目绩效提供标准和指导；还应尽快完善关于数字医疗服务相关标准、权责分配以及监管体制的运行办法和细则，使数字医疗服务的相关政策有规范性的程序可以参照。此外，部门与部门间的联合互动也有待加强，以提高部门间的协同性。
本研究结果还显示，数字健康政策需要平衡目标分布，关注政策目标短板。我国数字健康政策目标主要集中在履职保障方面。整合医疗资源，提高现有医疗资源的使用效率，促进优质医疗资源深入基层，协同高效的医疗数字化履职能力是数字健康发展的首要目标。促进互联网医疗行业健康有序发展需要建立适应互联网行业特征的医疗制度规则，虽然制度规则目标位于第二位置，但相较于前者所占比例仍然较低。同时，完善医疗服务监管体制是深化医药卫生体制改革的一项重要任务, 然而数字医疗监管体制投入仍有待提高，以进一步完善监管规范，保证线上医疗服务效率和质量。因此，完善医疗体制机制仍然是政府推动数字健康发展的重要手段。此外，坚守医疗质量安全和医疗信息安全的底线离不开数字健康的安全保障建设，安全保障的缺失意味着互联网医疗安全存在隐患，加强数字健康安全建设必不可少。数字健康发展与公共数据资源的高效利用密切相关，但数据资源目标所占比例仅11.25%，政府仍需加强对数据资源价值高效利用的能力。另外，尽管数字健康基础设施（如5G通信技术、AI平台）近年来已成为重点关注领域，但平台支撑目标的比重仅7.08%。这一现象可能与政策文本表述偏好有关：现行政策更强调机制建设和履职保障，而对平台这一具体技术载体的直接支持多隐含于“科技支持”工具中。此外，平台建设涉及高额投资和跨部门协调，政策制定者可能更倾向于通过环境型工具间接推动，从而目标显性化不足。
此外，政策工具和政策目标匹配有待进一步均等化，需要注重政策工具和政策目标的多维协同。数字健康政策出台的目的在于运用多种政策工具实现各类政策目标，工具的选择需要适应所对应的目标，因此政策工具与政策目标间的匹配度尤其重要。根据前文分析数据，在37份数字健康政策文本中仅有不到1/3的政策工具在所有政策目标中都被运用，而其余的政策工具在各类政策目标的运用中都存在空缺，财政支持、人才支持和示范项目等工具与政策目标匹配程度尤其低，由此可见，各类政策目标所运用的政策工具需进一步完善，政府制定政策时应考虑到政策工具与政策目标的匹配度。工具与目标出现结构性失衡，这一现象可归因于政策过程的多主体博弈：一是政府部门间权责分割导致工具选择偏向“环境型”；二是受政策惯性影响，传统医疗治理思维占主导；三是利益相关者参与不足（如医疗机构、科技企业）。因此，要注重政策工具和政策目标的多维协同。一是构建协同治理机制，统筹制定跨领域政策工具组合，推行政策工具兼容性评估，整体提升供给型和需求型工具在各类目标中的使用率，避免工具偏向单一类型。二是数字健康每个阶段都有各自的价值取向，这要求政府推动治理思维与工具创新，在各时期要有针对性地制定符合数字健康阶段性特征的政策，同时也要注意与长期政策相互作用、相互促进[12]。三是提升政策适配性，构建“政-企-医”协同平台，探索建立实质性、无缝隙、可持续的长效合作机制，促进政策与各利益相关者的交互协同[13]，另外可以借鉴欧盟《数字健康行动计划》的公私合作机制。
本研究采用Rothwell和Zegveld的三分法框架，一定程度上可以有效解析政策工具的基本特征，但某些数字健康领域的特殊工具类型未被涵盖，例如国际合作类工具（如欧盟跨境数据协议）[14]以及市场激励工具（如PPP模式）等在现有分类中缺乏体现。后续研究可结合本土实践，拓展政策工具类型，以及通过增设混合型工具增强框架的解释力。另外，本研究运用NVivo 12软件对37份数字健康政策文件进行系统分析，阐明在数字健康发展中只有均衡系统地运用相关政策工具，并协调好各个政策目标，才能最大程度发挥政策合力。但本研究仅从“工具—目标”两个维度入手，缺乏一定全面性，对数字健康动态演进历程、不同地区数字健康发展差异等角度缺乏考虑，未来针对数字健康的政策研究可以从时空维度深化分析。
