Objective

To analyze the incidence and mortality trends of male breast cancer in Anhui province from 2010 to 2020 based on cancer registry data, with the aim of developing prevention and control strategies for breast cancer in the population.

Methods

Data on male breast cancer cases registered in Anhui province from 2010 through 2020 were collected from the China Cancer Registry Platform or the CANREG 4 system. The age- and area-specific number and rate of incidence and mortality, as well as the age-standardized incidence and mortality rates of male breast cancer, were calculated. The Joinpoint model was used to estimate the annual percentage change (APC) in these indicators to analyze the trends in incidence and mortality of male breast cancer in selected areas with cancer registration in the province during this period.

Results

For the population covered by the cancer registration in the province during the study period, the crude incidence rate (per 100 000) of male breast cancer was 0.58 (0.53 and 0.61 for urban and rural populations, respectively), and the age-standardized incidence rate was 0.44 (0.38 and 0.49 for urban and rural populations, respectively). There was no significant changing trend in the age-standardized incidence rate for the total population (APC = –1.17%, P > 0.05) or for both urban and rural populations (APC = –0.24% and –2.59%, respectively, both P > 0.05). The crude mortality rate of male breast cancer was 0.21 for the total population, with 0.20 and 0.21 for urban and rural populations, respectively. The age-standardized mortality rate was 0.14 for the total population, with 0.13 and 0.16 for urban and rural populations, respectively. There were no significant changing trends in the age-standardized mortality rate for the total population (APC = 3.08%, P > 0.05) or for the rural population (APC = 0.83%, P > 0.05), but there was a significant upward trend in the age-standardized mortality rate for the urban population (APC = 6.29%, P = 0.003).

Conclusions

The registered mortality rate of male breast cancer in the urban population of Anhui province showed an increasing trend from 2010 to 2020, suggesting that more attention should be given to the prevention and control of male breast cancer in urban areas.

目的

分析安徽省肿瘤登记地区2010—2020年男性乳腺癌的发病和死亡趋势，为该地区制定乳腺癌的防控策略提供参考依据。

方法

收集2010年1月1日—2020年12月31日安徽省各肿瘤登记处从中国肿瘤登记平台或CANREG 4系统上报的男性乳腺癌相关数据，按照城乡分层计算男性乳腺癌的发病率、标化发病率、死亡率和标化死亡率，应用Joinpoint模型计算年度变化百分比（APC）分析安徽省肿瘤登记地区2010—2020年男性乳腺癌的发病和死亡趋势。

结果

安徽省肿瘤登记地区2010—2020年男性乳腺癌发病率为0.58/10万（城市为0.53/10万、农村为0.61/10万），标化发病率为0.44/10万（城市为0.38/10万、农村为0.49/10万）； 2010—2020年安徽省肿瘤登记地区男性乳腺癌标化发病率变化趋势无统计学意义（APC= –1.17%， P>0.05），其中城市地区与农村地区男性乳腺癌标化发病率变化趋势均无统计学意义（APC= –0.24%和–2.59%，均P>0.05）。安徽省肿瘤登记地区2010—2020年男性乳腺癌死亡率为0.21/10万（城市为0.20/10万、农村为0.21/10万），标化死亡率为0.14/10万（城市为0.13/10万、农村为0.16/10万）；2010—2020年安徽省肿瘤登记地区男性乳腺癌标化死亡率变化趋势无统计学意义（APC=3.08%，P>0.05），其中城市地区男性乳腺癌标化死亡率呈上升趋势（APC=6.29%，P=0.003），农村地区男性乳腺癌标化死亡率变化趋势无统计学意义（APC=0.83%，P>0.05）。

目的

分析安徽省肿瘤登记地区2010—2020年男性乳腺癌的发病和死亡趋势，为该地区制定乳腺癌的防控策略提供参考依据。

方法

收集2010年1月1日—2020年12月31日安徽省各肿瘤登记处从中国肿瘤登记平台或CANREG 4系统上报的男性乳腺癌相关数据，按照城乡分层计算男性乳腺癌的发病率、标化发病率、死亡率和标化死亡率，应用Joinpoint模型计算年度变化百分比（APC）分析安徽省肿瘤登记地区2010—2020年男性乳腺癌的发病和死亡趋势。

结果

安徽省肿瘤登记地区2010—2020年男性乳腺癌发病率为0.58/10万（城市为0.53/10万、农村为0.61/10万），标化发病率为0.44/10万（城市为0.38/10万、农村为0.49/10万）； 2010—2020年安徽省肿瘤登记地区男性乳腺癌标化发病率变化趋势无统计学意义（APC= –1.17%， P>0.05），其中城市地区与农村地区男性乳腺癌标化发病率变化趋势均无统计学意义（APC= –0.24%和–2.59%，均P>0.05）。安徽省肿瘤登记地区2010—2020年男性乳腺癌死亡率为0.21/10万（城市为0.20/10万、农村为0.21/10万），标化死亡率为0.14/10万（城市为0.13/10万、农村为0.16/10万）；2010—2020年安徽省肿瘤登记地区男性乳腺癌标化死亡率变化趋势无统计学意义（APC=3.08%，P>0.05），其中城市地区男性乳腺癌标化死亡率呈上升趋势（APC=6.29%，P=0.003），农村地区男性乳腺癌标化死亡率变化趋势无统计学意义（APC=0.83%，P>0.05）。

结论

安徽省城市肿瘤登记地区2010—2020年男性乳腺癌死亡率呈上升趋势，应重视城市地区男性乳腺癌的预防和控制工作。

Objective

To analyze the incidence and mortality trends of male breast cancer in Anhui province from 2010 to 2020 based on cancer registry data, with the aim of developing prevention and control strategies for breast cancer in the population.

Methods

Data on male breast cancer cases registered in Anhui province from 2010 through 2020 were collected from the China Cancer Registry Platform or the CANREG 4 system. The age- and area-specific number and rate of incidence and mortality, as well as the age-standardized incidence and mortality rates of male breast cancer, were calculated. The Joinpoint model was used to estimate the annual percentage change (APC) in these indicators to analyze the trends in incidence and mortality of male breast cancer in selected areas with cancer registration in the province during this period.

Results

For the population covered by the cancer registration in the province during the study period, the crude incidence rate (per 100 000) of male breast cancer was 0.58 (0.53 and 0.61 for urban and rural populations, respectively), and the age-standardized incidence rate was 0.44 (0.38 and 0.49 for urban and rural populations, respectively). There was no significant changing trend in the age-standardized incidence rate for the total population (APC = –1.17%, P > 0.05) or for both urban and rural populations (APC = –0.24% and –2.59%, respectively, both P > 0.05). The crude mortality rate of male breast cancer was 0.21 for the total population, with 0.20 and 0.21 for urban and rural populations, respectively. The age-standardized mortality rate was 0.14 for the total population, with 0.13 and 0.16 for urban and rural populations, respectively. There were no significant changing trends in the age-standardized mortality rate for the total population (APC = 3.08%, P > 0.05) or for the rural population (APC = 0.83%, P > 0.05), but there was a significant upward trend in the age-standardized mortality rate for the urban population (APC = 6.29%, P = 0.003).

Conclusions

The registered mortality rate of male breast cancer in the urban population of Anhui province showed an increasing trend from 2010 to 2020, suggesting that more attention should be given to the prevention and control of male breast cancer in urban areas.

据世界卫生组织国际癌症研究署（International Agency for Research on Cancer, IARC）统计，2020年女性乳腺癌已经超过肺癌，成为居于女性发病和死亡第1位的癌症[1]。国家癌症中心数据显示，2018年乳腺癌发病率和死亡率分别为女性发病的第2位和死亡的第5位[2]，且近年女性乳腺癌发病率呈快速上升趋势[3–4]，而这些数据中分析的均为女性乳腺癌的结果，关于男性乳腺癌的研究较少，且大多为病例分析。有研究表明，美国2019年新发男性乳腺癌病例2 670例、死亡约500例[5]，中国2015年新发男性乳腺癌病例3 800例、死亡1 200例[6]。美国国家癌症研究所的肿瘤监测项目（The Surveillance, Epidemiology, and End Results Program, SEER）数据显示男性乳腺癌发病率有明显升高[7]。为分析安徽省肿瘤登记地区2010—2020年男性乳腺癌的发病和死亡趋势，为该地区制定乳腺癌的防控策略提供参考依据，本研究收集2010年1月1日—2020年12月31日安徽省各肿瘤登记处从中国肿瘤登记平台或CANREG 4系统上报的男性乳腺癌相关数据，按照城乡分层计算男性乳腺癌的发病率、标化发病率、死亡率和标化死亡率，应用Joinpoint模型计算年度变化百分比（annual percentage change, APC）分析安徽省肿瘤登记地区2010—2020年男性乳腺癌的发病和死亡趋势。结果报告如下。

1.1　资料来源
资料来源于2010年1月1日—2020年12月31日安徽省各肿瘤登记处从中国肿瘤登记平台或CANREG 4系统上报的男性乳腺癌相关数据，数据采用《国际疾病分类第十版（International Classification of Diseases Version 10，ICD-10）》[8]进行分类和编码，乳腺癌编码为C50。按照中国肿瘤登记工作指导手册（2016）要求[9]，参照相关标准[10–11]对数据完整性和有效性进行审核和评价后，选取符合标准的登记处数据纳入分析。各登记处户籍人口数来源于各登记处统计局或公安局。
1.2　统计分析
按地级以上城市和县（县级市）划分为城市和农村登记处，采用Excel 2013按照城乡计算男性乳腺癌发病率、标化发病率、死亡率、标化死亡率，其中标化发病率和标化死亡率采用2000年全国普查标准人口年龄构成[12]进行计算；应用Joinpoint 4.9.1.0统计软件计算APC值分析安徽省2010—2020年男性乳腺癌标化发病率和标化死亡率的年度变化趋势。检验水准为双侧检验α=0.05。

2.1　安徽省肿瘤登记地区2010—2020年男性乳腺癌发病情况（表1、图1）
2010—2020年安徽省各肿瘤登记处共报告742例男性乳腺癌发病病例，发病率为0.58/10万，标化发病率为0.44/10万。其中，城市地区发病数为286例，发病率为0.53/10万，标化发病率为0.38/10万；农村地区发病数为456例，发病率为0.61/10万，标化发病率为0.49/10万。各年份男性乳腺癌发病数、发病率和标化发病率见表1。由于男性乳腺癌发病率较低，本研究对2010—2020年发病数据合并后分析年龄别发病率情况，结果显示，城市地区和农村地区在<30岁年龄组发病率很低，随后发病率快速上升，均在85岁～年龄组发病率达到最高；其中，城市地区在55岁～年龄组达到前期发病高峰，随后有所下降，在≥75岁年龄组快速上升，在85岁～年龄组发病率达到最高；农村地区在<70岁年龄组逐渐上升，在70岁～年龄组略有下降，之后再次上升，在85岁～年龄组发病率达到最高（见图1）。
2.2　安徽省肿瘤登记地区2010—2020年男性乳腺癌死亡情况（表2、图2）
2010—2020年安徽省各肿瘤登记处共报告267例男性乳腺癌死亡病例，死亡率为0.21/10万，标化死亡率为0.14/10万。其中，城市地区死亡数为108例，死亡率为0.20/10万，标化死亡率为0.13/10万；农村地区死亡数为159例，死亡率为0.21/10万，标化死亡率为0.16/10万。各年份男性乳腺癌死亡数、死亡率和标化死亡率见表2。由于男性乳腺癌死亡率较低，本研究对2010—2020年死亡数据合并后分析年龄别死亡率情况，结果显示，城市地区和农村地区在<40岁年龄组死亡率很低，随后死亡率快速上升，均在85岁～年龄组死亡率达到最高；其中，城市地区死亡率一直上升，在85岁～年龄组死亡率达到最高；农村地区65～69岁年龄组达到前期死亡高峰，随后有所下降，在≥80岁年龄组快速上升，在85岁～年龄组死亡率达到最高（见图2）。
2.3　安徽省肿瘤登记地区2010—2020年男性乳腺癌标化发病率和标化死亡率变化趋势
Joinpoint模型分析结果显示，2010—2020年安徽省肿瘤登记地区男性乳腺癌标化发病率变化趋势无统计学意义（APC= –1.17%，P>0.05），其中城市地区和农村地区男性乳腺癌标化发病率变化趋势均无统计学意义（APC= –0.24%和–2.59%，均P>0.05）；2010—2020年安徽省肿瘤登记地区男性乳腺癌标化死亡率变化趋势无统计学意义（APC=3.08%，P>0.05），其中城市地区男性乳腺癌标化死亡率呈上升趋势（APC=6.29%，P=0.003），农村地区男性乳腺癌标化死亡率变化趋势无统计学意义（APC=0.83%，P>0.05）。

本研究结果显示，安徽省肿瘤登记地区2010—2020年男性乳腺癌发病率为0.58/10万，标化发病率为0.44/10万，与河北省2020年标化发病率的0.43/10万[13]接近，低于湖南省2020年标化发病率的0.56/10万[14]，但高于2018年全国标化发病率的0.38/10万[2]、江苏省2019年标化发病率的0.31/10万[15]和浙江省2020年标化发病率的0.32/10万[16]，远高于福建省2020年标化发病率的0.13/10万[17]和广东省中山市2000—2016年标化发病率的0.13/10万[18]；安徽省肿瘤登记地区2010—2020年男性乳腺癌死亡率为0.21/10万，标化死亡率为0.14/10万，与2018年全国标化死亡率的0.14/10万[2]相同，与河北省2020年标化死亡率的0.15/10万[13]接近，低于湖南省2020年标化死亡率的0.33/10万[14]，但远高于江苏省2019年标化死亡率的0.06/10万[15]、浙江省2020年标化死亡率的0.04/10万[16]和福建省2020年标化死亡率的0.02/10万[17]。提示安徽省肿瘤登记地区2010—2020年男性乳腺癌发病和死亡水平在全国均处于中上等水平。本研究中，安徽省肿瘤登记地区2010—2020年城市和农村地区男性乳腺癌发病率分别为0.53/10万和0.61/10万，标化发病率分别为0.38/10万和0.49/10万，死亡率分别为0.20/10万和0.21/10万，标化死亡率分别为0.13/10万和0.16/10万，农村地区男性乳腺癌无论是发病率还是死亡率均高于城市地区，与女性乳腺癌发病和死亡特点相反[19]，这可能与男性和女性乳腺癌发病危险因素不同有关，这些危险因素（如高蛋白质的饮食可增加女性乳腺癌的风险，但对男性乳腺癌并不显著[20]）在城市地区和农村地区分布不同。
本研究结果显示，安徽省肿瘤登记地区2010—2020年男性乳腺癌年龄别发病率呈随年龄增长而上升趋势，在85岁～年龄组达到最高，这与女性乳腺癌发病率呈“双峰”且在之后年龄别发病率下降的趋势不同[2]；安徽省肿瘤登记地区2010—2020年男性乳腺癌年龄别死亡率亦呈随年龄增长而上升趋势，在85岁～年龄组达到最高，这与女性乳腺癌死亡特点相似[2]。
本研究Joinpoint模型分析结果显示，安徽省肿瘤登记地区2010—2020年除城市地区男性乳腺癌标化死亡率呈上升趋势（APC=6.29%，P=0.003）外，城乡合计、城市地区、农村地区标化发病率及城乡合计、农村地区标化死亡率变化趋势均无统计学意义（均P>0.05），提示应更重点关注城市男性乳腺癌的生存率。
综上所述，安徽省城市肿瘤登记地区2010—2020年男性乳腺癌死亡率呈上升趋势，提示应重视城市地区男性乳腺癌的预防和控制工作。男性乳腺癌发病率虽然相对较低，但由于绝对人口的基数，男性乳腺癌发病数仍然不可忽视。然而目前乳腺癌的筛查、诊疗指南和规范[21–23]等大多针对女性乳腺癌患者，除了《中国男性乳腺癌临床诊治实践指南（2023版）》[24]外，目前我国尚无针对男性乳腺癌人群筛查和精准治疗的规范或指南，因此亟需加强对男性乳腺癌相关筛查指南、临床治疗和规范的研究，使男性乳腺癌筛查、诊治和治疗更加精准化，更具有针对性，从而降低男性乳腺癌的发病率和死亡率。
