Gouty arthritis is an inflammatory joint disease triggered by urate crystal deposition, with nucleotide binding oligomerization domain (NOD)-like receptor protein 3 (NLRP3) inflammasome overactivation serving as the core driver of its acute inflammatory response. Although NLRP3-targeting drugs have advanced to clinical trials, their clinical application remains limited by adverse effects and uncertain long-term risks. In this context, traditional Chinese medicine (TCM), with its multi-component, multi-target, and synergistic regulatory properties, offers novel strategies for the prevention and treatment of gout. Studies have shown that TCM exerts multidimensional therapeutic effects, including alleviation of acute joint inflammation, reduction of serum uric acid levels, and prevention of disease recurrence, underscoring its potential for systemic intervention. This review systematically summarizes the mechanisms by which active components and formulations of TCM modulate key aspects of NLRP3 inflammasome activation pathways. These include inhibition of upstream initiation signals, blockade of activation signal transduction, direct interference with inflammasome assembly, and enhancement of autophagic clearance, collectively leading to suppressed release of key inflammatory mediators such as IL-1β. This review aims to provide theoretical foundations and directional insights for further research and clinical application of TCM regulating NLRP3 in gouty arthritis treatment.
