Diabetic kidney disease is a significant microvascular complication of diabetes that can progress to end-stage renal disease, severely threatening patients' quality of life and longevity. Its pathogenesis is complex, involving multiple pathways including inflammatory responses, oxidative stress, metabolic disorders, and disrupted intercellular signaling networks in the kidney, all significantly influenced by individual variability. Current clinical treatment options remain limited, with a lack of targeted effective drugs posing substantial challenges for prevention and management. This systematic review examines the pathogenesis and existing treatment strategies for diabetic kidney disease. By integrating recent advances, it explores emerging therapeutic approaches and potential breakthrough directions, aiming to provide a theoretical and practical foundation for innovative interventions and clinical translation.

ACEI: Ramipril, benazepril
ARB: Valsartan, losartan
Blocking the renin-angiotensin system reduces glomerular pressure, thereby decreasing proteinuria and fibrosisUACR reduced by 30%-40%, with annual eGFR decline slowed to -4.4 mL·min-1; cardiovascular event risk reduced by 27%, heart failure risk by 85%, and ESKD risk by 30%Decreased renal function leads to reduced potassium excretion (hyperkalemia); RAAS blockade inhibits aldosterone secretion[55]
SGLT2 inhibitorDapagliflozin, empagliflozin, canagliflozinInhibit glucose reabsorption in the proximal renal tubules, improve tubular hyperfiltration, and alleviate inflammation and oxidative stressA 30% reduction in the risk of the primary composite endpoint (end-stage kidney disease, renal or cardiovascular death); a 39% reduction in the risk of hospitalisation for heart failureMonitor renal function changes during the initial phase of treatment
Increased risk of genitourinary infections

RAAS
inhibitor
