Ischemic stroke (IS) remains a leading cause of mortality and disability among adults worldwide, with its core pathological mechanism rooted in ischemic-hypoxic neuronal injury triggered by cerebral blood flow interruption. Current clinical treatments primarily rely on intravenous thrombolysis and endovascular thrombectomy. However, these approaches are constrained by strict therapeutic time windows (typically ≤ 4.5 h), bleeding risks in certain patients, and limited neurorestorative effects following reperfusion. Despite active development of neuroprotective agents and anti-inflammatory drugs in recent years, most have failed in clinical translation due to insufficient efficacy or safety concerns, highlighting the complexity of disease mechanisms and limitations of preclinical evaluation systems. Consequently, developing therapeutics that simultaneously address reperfusion protection and neural repair by targeting the pathological cascade of IS has emerged as a critical direction to overcome current therapeutic bottlenecks. This review synthesizes recent advances in domestic and international IS drug development in recent years, focusing on model establishment, efficacy evaluation metrics, and relevant molecular mechanisms to systematically summarize the preclinical pharmacodynamic evaluation framework for IS therapeutics. The findings provide valuable insights for optimizing preclinical evaluation systems, accelerating drug development, and expanding effective clinical treatment strategies for IS.

Neurological deficitBederson scoreThe Bederson score assesses neurological dysfunction, with higher scores indicating more severe impairment
0: When suspended by the tail, the animal extends both forelimbs naturally, with no flexion or movement asymmetry
1: The forelimb contralateral to the injury shows flexion (e.g., wrist or elbow bending), without additional abnormalities such as circling or reduced resistance to lateral push
2: In addition to forelimb flexion, the animal exhibits significantly reduced resistance to lateral push toward the impaired side
3: The animal displays spontaneous circling behavior toward the injured side, in addition to the above signs
4: The animal shows longitudinal rotation or is unable to move independently
NoAcute phase (0-72 h)
Longa scoreThis scale evaluates motor behavior and neurological deficits, ranging from 0 (no deficit) to 4 (severe deficit or death). It includes assessments of somatosensory function, placement response, grip strength, reflexes, balance, and coordinationNoAcute phase (0-72 h)
mNSS1. Suspended tail test: elevate the tail above the ground and observe any deviation or flexion of the head and limbs
2. Normal gait, inability to walk in a straight line, circling towards the affected side, and leaning towards the affected side are scored 0-3 points respectively
3. Sensory testing: visual testing; tactile testing; proprioceptive testing
4. Balance beam test
5. Reflex testing: auricular reflex; corneal reflex; startle reflex; epileptic seizures, muscle spasms, abnormal muscle tone
mNSS ranges from 0 to 18 points, with higher scores indicating more severe neurological impairment
NoAcute phase (0-72 h), subacute phase (3-14 d), recovery phase (> 14 d)
Garcia neurobehavioral scale1. General activity in the test cage
2. Symmetry of forelimb movement
3. Ability to extend forepaws
4. Climbing ability on a wire
5. Tactile response to body stimulation (cotton swab)
6. Response to vibrissae touch
Each test is scored from 0 to 3, with a maximum total of 18 points. Higher scores indicate milder neurological injury
