A simple and sensitive method was developed for quantitation of obeticholic acid in rat plasma with liquid chromatography-tandem mass spectrometry (LC-MS/MS). After liquid-liquid extraction by methyl tert-butyl ether, the chromatographic separation was carried out on an ACE Excel 2 Super C18 column (50 mm×2.1 mm ID, 1.7 μm) with a gradient mobile phase consisting of acetonitrile and 2 mmol·L-1 ammonium formate at a flow rate of 0.2 mL·min-1. The quantitation analysis was performed using multiple reaction monitoring (MRM) at the specific ion transitions of m/z418.9[M-H]-→401.2 for obeticholic acid and m/z469.0[M-H]-→ 425.2 for glycyrrhetinic acid (internal standard) in the negative ion mode with electrospray ionization (ESI) source. This validated LC-MS/MS method yielded a good linearity over the range of 5 -5 000 ng·mL-1 with the lower limit of quantitation (LLOQ) of 5 ng·mL-1. The intra and inter-assay precisions (RSD) were all less than 9.82% and the accuracy (RE) was within ±6.90%. The extraction recovery of obeticholic acid was from 85.4% to 88.5%, and the matrix effect of obeticholic acid ranged from 78.9% to 82.5%. Stability test suggest that obeticholic acid in rat plasma was stable for 24 h on workbench, up to 1 month at -70℃, and after three cycles of freeze-thaw. Extracted samples were stable for more than 24 h in an auto-sampler at 6℃. The precision was less than 7.25%, and the accuracy was within ±11.2%, after being diluted 10 times by blank rat plasma. The method has been successfully applied to a pharmacokinetic study of obeticholic acid in rats following oral administration at the dose of 2.5 mg·kg-1.

本文建立了一种快速、灵敏、准确、可靠的LC-MS/MS分析方法测定大鼠血浆中奥贝胆酸浓度,并应用于奥贝胆酸大鼠灌胃给药后的药动学研究。血浆样品以甲基叔丁基醚提取后,经ACE Excel 2 Super C18色谱柱(50 mm×2.1 mm ID,1.7 μm)分离,以乙腈-2 mmol·L-1甲酸铵水溶液(含10%乙腈)作为流动相,梯度洗脱,流速0.2 mL·min-1。电喷雾离子化源(ESI)负离子条件下,采用多反应监测模式(MRM)进行定量分析,监测离子对分别为奥贝胆酸m/z418.9[M-H]-→401.2,内标甘草次酸m/z 469.0[M-H]-→ 425.2。大鼠血浆中奥贝胆酸在5~5 000 ng·mL-1浓度内线性关系良好(r2 > 0.99),定量下限为5 ng·mL-1;日内、日间精密度(RSD)小于9.82%,准确度(RE)在±6.90%以内;血浆中无内源性物质干扰,内标归一化后的基质效应为78.9%~82.5%,提取回收率为85.4%~88.5%;奥贝胆酸的血浆样品经室温放置24 h、-70℃ 3次冻融循环、-70℃冻存1个月,处理后的样品在进样器内放置24 h均稳定;药物浓度超出曲线范围的样品经空白血浆稀释10倍后,其准确度在±11.2%以内,精密度为7.25%。本方法经验证后,成功应用于大鼠灌胃给予奥贝胆酸后的药动学研究。

本文建立了一种快速、灵敏、准确、可靠的LC-MS/MS分析方法测定大鼠血浆中奥贝胆酸浓度,并应用于奥贝胆酸大鼠灌胃给药后的药动学研究。血浆样品以甲基叔丁基醚提取后,经ACE Excel 2 Super C18色谱柱(50 mm×2.1 mm ID,1.7 μm)分离,以乙腈-2 mmol·L-1甲酸铵水溶液(含10%乙腈)作为流动相,梯度洗脱,流速0.2 mL·min-1。电喷雾离子化源(ESI)负离子条件下,采用多反应监测模式(MRM)进行定量分析,监测离子对分别为奥贝胆酸m/z418.9[M-H]-→401.2,内标甘草次酸m/z 469.0[M-H]-→ 425.2。大鼠血浆中奥贝胆酸在5~5 000 ng·mL-1浓度内线性关系良好(r2 > 0.99),定量下限为5 ng·mL-1;日内、日间精密度(RSD)小于9.82%,准确度(RE)在±6.90%以内;血浆中无内源性物质干扰,内标归一化后的基质效应为78.9%~82.5%,提取回收率为85.4%~88.5%;奥贝胆酸的血浆样品经室温放置24 h、-70℃ 3次冻融循环、-70℃冻存1个月,处理后的样品在进样器内放置24 h均稳定;药物浓度超出曲线范围的样品经空白血浆稀释10倍后,其准确度在±11.2%以内,精密度为7.25%。本方法经验证后,成功应用于大鼠灌胃给予奥贝胆酸后的药动学研究。

A simple and sensitive method was developed for quantitation of obeticholic acid in rat plasma with liquid chromatography-tandem mass spectrometry (LC-MS/MS). After liquid-liquid extraction by methyl tert-butyl ether, the chromatographic separation was carried out on an ACE Excel 2 Super C18 column (50 mm×2.1 mm ID, 1.7 μm) with a gradient mobile phase consisting of acetonitrile and 2 mmol·L-1 ammonium formate at a flow rate of 0.2 mL·min-1. The quantitation analysis was performed using multiple reaction monitoring (MRM) at the specific ion transitions of m/z418.9[M-H]-→401.2 for obeticholic acid and m/z469.0[M-H]-→ 425.2 for glycyrrhetinic acid (internal standard) in the negative ion mode with electrospray ionization (ESI) source. This validated LC-MS/MS method yielded a good linearity over the range of 5 -5 000 ng·mL-1 with the lower limit of quantitation (LLOQ) of 5 ng·mL-1. The intra and inter-assay precisions (RSD) were all less than 9.82% and the accuracy (RE) was within ±6.90%. The extraction recovery of obeticholic acid was from 85.4% to 88.5%, and the matrix effect of obeticholic acid ranged from 78.9% to 82.5%. Stability test suggest that obeticholic acid in rat plasma was stable for 24 h on workbench, up to 1 month at -70℃, and after three cycles of freeze-thaw. Extracted samples were stable for more than 24 h in an auto-sampler at 6℃. The precision was less than 7.25%, and the accuracy was within ±11.2%, after being diluted 10 times by blank rat plasma. The method has been successfully applied to a pharmacokinetic study of obeticholic acid in rats following oral administration at the dose of 2.5 mg·kg-1.

原发性胆汁性胆管炎(primary biliary cholangitis, PBC)又称原发性胆汁性肝硬化, 是一种慢性、进行性自身免疫性胆汁淤积性肝病[1, 2], 其发病机制尚不完全清楚, 大部分学者认为遗传和环境因素相互作用导致免疫耐受丧失[3-6], 从而启动了针对胆管上皮细胞的免疫反应[7]。PBC表现为肝脏小胆管缓慢进行性破坏, 引起胆汁和其他毒素在肝脏中郁积, 导致肝组织进一步损伤及瘢痕形成, 最终导致肝硬化、纤维变性以及肝坏死[3, 8]。PBC在40岁以上的中年女性发病率较高, 患者往往伴有瘙痒、疲劳等临床症状[9, 10]
FDA在1997年批准的熊去氧胆酸(ursodeoxycholic acid, UDCA)是PBC的主要治疗药物。UDCA主要通过上调胆汁酸转运体的表达和抗凋亡的作用, 保护肝细胞和胆管细胞的功能, 从而有效改善肝脏异常生化指标的水平、降低肝纤维化和肝硬化的发病率[11, 12]。PBC患者服用UDCA可以明显提高生存率, 但有高达40%的患者对UDCA治疗反应不佳[13], 与UDCA应答良好的患者相比生存率明显下降[14]
FDA于2016年5月27日批准Intercept公司开发的Ocaliva (obeticholic acid, 奥贝胆酸)上市, 成为时隔近20年后首个批准的PBC治疗药物。奥贝胆酸(obeticholic acid, OCA, 也称6-ECDCA或INT-747)是人体初级胆汁酸—鹅去氧胆酸(chenodeoxycholic acid, CDCA)的半合成衍生物。它是一种具有选择性的法尼酯衍生物X受体(FXR)激动剂, 其活性要比CDCA高出100倍, 具有抗胆汁淤积性和保护肝脏的特性[15, 16]。FXR是一种参与胆汁酸稳态的核受体, 在胆汁酸和胆固醇代谢中发挥重要作用[17]。奥贝胆酸通过活化FXR, 间接抑制胆汁酸生物合成的限速酶CYP7A1的基因表达; 同时胆汁淤积也可激活FXR通过调节小异源二聚体(small heterodimer partner, SHP)的活性, 抑制胆汁酸的摄取[18-22]。因此, FXR激动剂奥贝胆酸可以调节胆汁酸的合成、分泌、转运等, 缓解PBC的症状[23]。临床研究结果表明, PBC患者单独服用奥贝胆酸或是UDCA与奥贝胆酸合用, 都会使胆汁淤积、炎症情况得到改善[2, 24]。因此, 奥贝胆酸可与UDCA联合应用治疗UDCA反应不足的PBC患者, 也可单独应用治疗UDCA不能耐受的成年PBC患者[9, 11]
随着奥贝胆酸在美国的成功上市, 对奥贝胆酸的me-too药、仿制药的研发已经成为国内外许多研究机构的热点, 有关奥贝胆酸的药效学、毒理学、药代动力学的数据也逐渐披露或可以从FDA的官网查到, 但是有关生物样品中奥贝胆酸含量的分析方法研究尚未见报道。Lu等[25]采用HPLC-UV法测定奥贝胆酸片含量, 定量下限为0.2 mg·mL-1, 该方法对奥贝胆酸的检测灵敏度太低, 不能满足其体内药动学、毒代动力学分析的需要。本文的目的是研究基于LC-MS/MS技术的准确、灵敏的定量分析方法, 用于大鼠血浆中奥贝胆酸浓度的测定和药代动力学、毒代动力学研究。
药品与试剂  奥贝胆酸对照品(纯度: 99.1%; LOT: 140103), 由丽珠集团丽珠医药研究所提供。甘草次酸(内标, 纯度: 99.3%; LOT: 110723-201514), 由北京科诺德医药科技有限公司提供。甲醇、乙腈(HPLC级), Thermo Fisher公司。甲酸铵(分析纯), 天津市光复精细化工研究所。分析纯甲基叔丁基醚(分析纯), 美国, Tedia公司。去离子水由实验室自制。
实验动物  健康Sprague Dawley (SD)大鼠6只, 体重180~220 g, 雌雄各半, 由北京维通利华实验技术有限公司提供。实验动物由天津药物研究院新药评价有限公司动物室饲养, 饲养室的温度保持在(22 ± 1) ℃, 湿度保持在(60 ± 5) %, 自由摄食饮水。本研究的动物使用方案已获得天津药物研究院新药评价有限公司IACUC批准。
仪器  LCMS-8060超快速三重四极杆液质联用仪, 配有LC-30AD二元输送泵, SIL-30AC自动进样器, DGU-20A5R真空脱气机和CTO-20A柱温箱, 日本Shimadzu公司。Sorvall Legend Micro 17R台式高速冷冻离心机, Thermo Fisher Scientific公司; TARGINTM VX-Ⅱ型涡旋混合器, 北京踏锦科技有限公司; Turbo Vap LV型样品浓缩仪, 美国Caliper公司; BM-40型纯水制备系统, 北京中盛茂源科技发展有限公司。
色谱条件  色谱柱为ACE Excel 2 Super C18(50 mm × 2.1 mm ID, 1.7 μm) (厂家), 柱温50 ℃。流动相为乙腈(A) -2 mmol·L-1甲酸铵水溶液(含10%乙腈) (B)。梯度洗脱, 0~0.5 min, 55% B; 0.5~4.0 min, 55%~10% B; 4.0~5.5 min, 10% B; 5.5~5.6 min, 10%~55% B; 5.6~7.5 min, 55% B, 流速为0.2 mL·min-1
质谱条件  电喷雾离子化源(ESI)负离子模式扫描; 离子源电压: 10 kV; 雾化气流: 3 L·min-1; 加热气流: 10 L·min-1; 接口温度:300 ℃; 干燥气流: 10 L·min-1; 加热块温度: 300 ℃; 脱溶剂管温度: 250 ℃; 碰撞气: 270 kPa。采用多反应监测模式(MRM)进行定量分析(驻留时间200 ms), 奥贝胆酸: m/z 418.9 [M-H]-→401.2, 碰撞能: 35 V; 内标甘草次酸: m/z469.0 [M-H]-→425.2, 碰撞能: 39 V。
储备液、工作液及校正标样的制备  将精确称量的奥贝胆酸及甘草次酸(内标)对照品溶解在适量甲醇中制备储备液, 分别获得质量浓度为1 mg·mL-1奥贝胆酸、内标储备液。用甲醇梯度稀释奥贝胆酸储备液, 得0.1、0.2、1、4、20、80和100 μg·mL-1系列标准工作液, 用同样的方式稀释奥贝胆酸储备液得质量浓度为0.25、5和75 μg·mL-1的质控工作液。取奥贝胆酸系列标液或质控工作液25 μL, 加入大鼠空白血浆475 μL, 涡旋混匀, 配成质量浓度为5、10、50、200、1 000、4 000和5 000 ng·mL-1校正标样及5 (LLQC)、12.5 (LQC)、250 (MQC)、3 750 (HQC) ng·mL-1 QC样品。稀释内标储备液至50 ng·mL-1作为内标工作液。所有储备液和工作液4 ℃储存备用。
血浆样品处理  取大鼠含药血浆50 μL, 加入内标(甘草次酸, 50 ng·mL-1)甲醇工作液50 μL、0.1 mol·L-1盐酸50 μL、甲基叔丁基醚1 mL, 涡旋2 min, 离心5 min, 分取有机层800 μL经氮气吹干。加入复溶液(50%甲醇水) 200 μL, 进样5 μL进行LC-MS/ MS定量分析。
选择性/特异性  选取6个不同来源的大鼠空白血浆, 除不加内标溶液(改加同体积甲醇溶液)外, 其余按“血浆样品处理”项下操作, 进行LC-MS/MS分析。
线性范围  配制并处理校正标样进行LC-MS/ MS分析, 记录奥贝胆酸及内标甘草次酸MRM检测的色谱图峰面积, 用加权最小二乘法(权重为1/x2)进行回归运算, 得到大鼠血浆校正曲线, 考察3个分析批, 每批测定双样本。
准确度和精密度  配制并处理定量下限、低、中、高浓度水平的QC样品, 按“血浆样品处理”项下操作, 进行LC-MS/MS分析。考察3个分析批, 每批、每个浓度6样本。应用当日校正曲线计算各QC样品浓度, 考察分析方法的准确度和精密度。
基质效应  样品A:取奥贝胆酸工作液25 μL, 加入甲醇475 μL, 涡旋混匀, 配成质量浓度为12.5、3 750 ng·mL-1 LQC、HQC样品。取上述QC样品80 µL加入内标(甘草次酸, 50 ng·mL-1)甲醇工作液80 μL、甲醇40 μL、超纯水200 μL, 涡旋混匀, 进样5 µL进行LC-MS/MS定量分析, 记录待测物和内标的色谱峰面积A。样品B:选取6个不同来源的大鼠空白血浆, 除不加内标溶液(改加同体积甲醇溶液)外, 其余按“血浆样品处理”项下操作(200 μL复溶液由样品A代替), 进行LC-MS/MS定量分析, 记录待测物和内标的色谱峰面积B。样品C:按“血浆样品处理”项下操处理样品LQC、HQC各6样本, 进样5 µL进行LC-MS/MS定量分析, 记录待测物和内标的色谱峰面积C。峰面积B与相应浓度的峰面积A的比值, 为待测物和内标的基质因子, 待测物的基质因子与内标的基质因子之比, 为内标归一化的基质效应因子; 峰面积比C/B×100%即为奥贝胆酸和内标的提取回收率。
稳定性  配制12.5、3 750 ng·mL-1 LQC、HQC稳定性样品, 按“血浆样品处理”项下处理, 进行室温放置、-70 ℃冻存1个月、-70 ℃室温3次冻融循环稳定性考察, 各浓度水平均为三样本。低、高两个浓度水平处理后的大鼠血浆样品, 考察进样器中放置24 h稳定性。应用新鲜制备处理的随行校正曲线, 计算稳定性样品的实测浓度。
稀释效应  制备奥贝胆酸质量浓度为50 000 ng·mL-1的生物样品, 用大鼠空白血浆稀释10倍后, 按“血浆样品处理”项下处理并分析, 考察稀释效应。测定结果与稀释因子相乘后的实际浓度和理论浓度进行对比。
药动学研究  6只健康颈静脉插管SD大鼠, 禁食12 h以上, 灌胃给药奥贝胆酸(2.5 mg·kg-1) 0.5% CMC-Na混悬液。清醒状态下于给药前和给药后0.167、0.5、1、2、3、4、6、9、12、24、30、48和72 h从颈静脉采血0.25 mL左右(各时间点采血后补充相应体积的生理盐水, 确保大鼠具有正常的生理功能), 肝素抗凝, 离心分离血浆, -70 ℃冰箱冻存至LC-MS/MS定量分析。
数据处理  使用LabSolutions软件进行数据采集和处理, 求得血浆校正曲线并且计算每只动物的奥贝胆酸浓度数据; 血浆药物浓度数据应用WinNonlin6.3药代软件拟合, 选择非房室模型统计矩法计算药代参数tmaxt1/2Cmax、AUC0-t、AUC0-∞、MRT0-t、MRT0-∞Vd和CL。
待测物奥贝胆酸及内标甘草次酸在ESI源负离子条件下信号强度优于正离子条件。一级全扫描模式下, 待测物奥贝胆酸及内标甘草次酸分别主要生成准分子离子[M-H]-m/z 418.9、469.0。对相应的准分子离子进行二级质谱全扫描分析(图 1), 待测物奥贝胆酸及内标甘草次酸生成的主要产物离子分别为m/z 401.2和m/z 355.2、425.2, 进一步优化后分别选取m/z 401.2、m/z 425.2作为定量分析时检测的产物离子。
空白血浆样品的色谱图见图 2A; LLOQ样品分析的色谱图见图 2B; 大鼠灌胃给药后血浆样品的色谱图见图 2C。待测物奥贝胆酸及内标甘草次酸色谱保留时间分别为3.79和4.21 min。干扰组分的响应低于当批次定量下限响应的20%及内标响应的5%, 表明大鼠血浆中的内源性物质不干扰分析物和内标的定量分析, 方法具有良好的专属性。
线性范围为5~5 000 ng·mL-1。典型回归方程为: y = 0.001 12 x+ 0.000 168, r2 = 0.998。结果表明, 奥贝胆酸在5~5 000 ng·mL-1内线性良好。
结果见表 1。大鼠血浆中奥贝胆酸在定量下限的批内、批间精密度(RSD)分别为6.63%、18.1%, 准确度(RE)为0.896%;低、中、高浓度水平QC样品的准确度(RE)在-6.90%~5.54%之间, 批内、批间精密度(RSD)分别在3.32%~6.58%、6.54%~9.82%之间, 符合有关生物样品分析方法验证的要求[26, 27]
结果表明, 待测物奥贝胆酸在低、高浓度水平上的内标归一化的基质效应为78.9%和82.5%, 基质效应的RSD在2.59%~8.00%之间, 基质效应不会对样品的分析产生明显影响。奥贝胆酸的回收率为88.5%和85.4%;内标的回收率为84.3%。
结果见表 2, 表明奥贝胆酸的含药血浆在以上贮存条件下及处理后自动进样器放置24 h, 稳定性良好。
结果显示, 准确度在88.8%~108%之间, 精密度为7.25%。表明对大鼠血浆样品进行稀释后分析, 经校正计算后的结果能够准确反映超出曲线范围的药物浓度。
以2.5 mg·kg-1剂量给SD大鼠灌胃给药后, 6只大鼠的平均药时曲线图见图 3, 相应的药动学参数见表 3。结果表明, 奥贝胆酸灌胃给药后吸收迅速, tmax为0.167 h左右, Cmax为2180 ± 904 ng·mL-1。而后血药浓度迅速下降, 至药后6 h出现第二个吸收峰, 血药浓度约为40 ng·mL-1左右, 随后缓慢消除, t1/2为17.2 ± 7.03 h。
奥贝胆酸属于胆酸类化合物, 分析检测时容易受内源性胆酸类化合物的干扰, 因此, 在建立LC-MS/MS定量分析方法时, 对色谱、质谱及样品处理方法进行了优化。
考察了Symmetry C8、Diamonsil C18、Agilent C8和ACE Excel 2 Super C18等色谱柱, 显示1.7 μm小粒径的ACE Excel 2 Super C18色谱柱可以提供更快的分离效率、对称的色谱峰形以及更高的灵敏度。选择流动相中有机相时, 与乙腈相比, 采用甲醇洗脱, 响应提高2倍左右, 但基线提高约4~5倍, 信噪比较低; 尝试乙腈与甲醇按不同比例混合(1:2、1:3、2:3)作为有机相, 信噪比无明显改善, 故最后选用乙腈作为有机相。对于流动相中缓冲盐以及酸碱的加入也进行了优化, 通常在负离子检测模式下, 流动相中加入氨水会提高待测物的检测灵敏度, 但本研究中加入0.5%氨水时, 导致奥贝胆酸成盐极性变大, 色谱保留较弱, 难以与内源性干扰实现良好分离; 采用纯水不加缓冲或酸碱体系时, 色谱峰较宽; 综合考虑色谱峰形、保留时间以及响应等方面, 经考察加入2 mmol·L-1甲酸铵较为合适。并对流速(0.1、0.2 mL·min-1)及洗脱方式(等度、梯度)进行了优化, 显示0.2 mL·min-1的流速下进行梯度洗脱, 可减少内源性物质的共流出。
奥贝胆酸属于胆酸类甾体化合物, 负离子模式下信号强度优于正离子模式, 生成准分子离子[M-H]- m/z 418.9, 且该类化合物结构中的“环戊烷并多氢菲”母核较稳定, 很难经碰撞诱导裂解(CID)产生碎片, 只有C17侧链中的羰基位点经CID裂解产生碎片m/z 401.2。选择m/z 418.9 [M-H]-→401.2作为MRM定量分析离子对, 并优化质谱源及碰撞相关参数, 提高质谱检测灵敏度。
对比了血浆蛋白沉淀及液-液萃取这两种样品处理方法的基质效应程度, 证明后者残留的内源性干扰更少。并对提取溶剂(甲基叔丁基醚、乙酸乙酯、正己烷等)、pH条件(不加酸、1%磷酸、10%磷酸、0.1 mol·L-1盐酸)进行了考察, 最终采用甲基叔丁基醚作为提取溶剂, 加入50 μL 0.1 mol·L-1盐酸, 实现较高的提取回收率及较小的基质效应影响。
在生物样品分析过程中, 内标的选择对于保证测定结果的准确性尤其重要。甘草次酸的结构、理化性质与奥贝胆酸接近, 具有相似的色谱、质谱行为以及相近的提取回收率, 故选为本方法的内标。
许多研究显示[28-30], 胆汁酸类药物口服给药后吸收迅速, 较短时间达峰, 随后血药浓度在很长一段时间内维持在较低水平下波动, 出现高低起伏的现象。这可能与胆汁酸类药物进入小肠后在回肠末端重吸收, 经门静脉返回肝脏再利用, 形成肠肝循环有关。奥贝胆酸为鹅去氧胆酸(CDCA)的半合成衍生物, 其血药浓度-时间曲线也呈现相似的趋势。另外, 胆汁酸药物由于肠肝循环的存在, 药物在血浆中持续时间延长, 因此在给药方案设计时应充分考虑是否可能产生蓄积中毒。
本文建立了一种快速、灵敏、准确的LC-MS/MS方法, 经过全面的分析方法验证, 符合生物样品分析方法指导原则的要求[26, 27], 可以准确测定原形药物奥贝胆酸在大鼠体内的血药浓度, 成功应用于奥贝胆酸灌胃给药后在大鼠体内的药动学研究, 揭示奥贝胆酸在大鼠体内的暴露水平、吸收及清除特点, 对后期毒代动力学研究及药物评价提供参考依据。
