Drug-resistance is a challenge in the treatment of epilepsy, and traditional Chinese medicine (TCM) prescription may have a potential in therapy of the epilepsy.Here we established a modified 6 Hz corneal kindled mouse model of epilepsy, and verified its drug-resistance to four commonly used western medicines.We evaluated the efficacy of three classical TCM prescriptions in this drug-resistant epilepsy model.The results showed that:① most C57BL/6J mice with stimulation current intensity at 44 mA (8 out of 10) were fully kindled, while none ICR mice with stimulation current intensity at 44 mA or C57BL/6J mice with stimulation current intensity at 24 mA were fully kindled.Fully kindled mice exhibited epileptic electroencephalograms after 44 mA and 6 Hz corneal kindling stimulation and increased activation of astrocytes in the hippocampus (by staining the glial fibrillary acidic protein); ② 50 mg·kg-1 phenytoin sodium, 100 mg·kg-1 valproic acid sodium and 15 mg·kg-1 lamotrigine had no significant effects on the drug-resistant seizures in 6 Hz corneal kindled C57BL/6J mice, while 100 mg·kg-1 levetiracetam significantly reduced the seizure stage (P < 0.05) and incidence of generalized seizure (P < 0.05) in it; ③ TCM prescriptions, Chai-Hu Plus Long-Gu Mu-Li decoction (9.36 or 28.08 g·kg-1) and Tian-Ma Gou-Teng decoction (14.82 or 44.46 g·kg-1), had no significant effects on the drug-resistant seizures in 6 Hz corneal kindled C57BL/6J mice, while TCM prescription Feng-Yin decoction (19.11 g·kg-1) significantly reduced both the seizure stage (P < 0.01) and incidence of generalized seizure (P < 0.05) in it.Thus, the modified 6 Hz corneal kindled C57BL/6J mouse model is an excellent drug-resistant epilepsy model, Feng-Yin decoction have antiepileptic effects on it, suggesting Feng-Yin decoction may be an effective TCM prescription for clinical treatment of drug-resistant epilepsy.

耐药是癫痫治疗领域一大难题, 而中药方剂可能有效治疗耐药癫痫。本研究建立改良的6 Hz角膜点燃耐药癫痫小鼠模型, 验证其对4种抗癫痫药物的耐药情况, 并利用该耐药癫痫模型对3种中药方剂进行药效学研究。研究发现:① 80% C57BL/6J小鼠经过21天电流强度为44 mA的6 Hz角膜点燃刺激后被完全点燃, 而相同刺激条件下的ICR小鼠和电流强度为24 mA的C57BL/6J小鼠均无法被完全点燃; ②苯妥英钠(50 mg·kg-1)、丙戊酸钠(100 mg·kg-1)和拉莫三嗪(15 mg·kg-1)对6 Hz角膜点燃的C57BL/6J小鼠耐药癫痫发作没有明显作用, 而左乙拉西坦(100 mg·kg-1)明显降低其发作等级(P < 0.05)和大发作几率(P < 0.05);③柴胡加龙骨牡蛎汤(9.36或28.08 g·kg-1)和天麻钩藤饮(14.82或44.46 g·kg-1)对6 Hz角膜点燃的C57BL/6J小鼠耐药癫痫发作没有明显作用, 而风引汤(19.11 g·kg-1)显著降低动物发作等级(P < 0.01)和大发作几率(P < 0.05)。本研究结果表明, C57BL/6J小鼠6 Hz角膜点燃模型是潜在耐药癫痫模型, 而风引汤对其有一定治疗作用, 可能是临床治疗耐药癫痫的潜在中药方剂。

耐药是癫痫治疗领域一大难题, 而中药方剂可能有效治疗耐药癫痫。本研究建立改良的6 Hz角膜点燃耐药癫痫小鼠模型, 验证其对4种抗癫痫药物的耐药情况, 并利用该耐药癫痫模型对3种中药方剂进行药效学研究。研究发现:① 80% C57BL/6J小鼠经过21天电流强度为44 mA的6 Hz角膜点燃刺激后被完全点燃, 而相同刺激条件下的ICR小鼠和电流强度为24 mA的C57BL/6J小鼠均无法被完全点燃; ②苯妥英钠(50 mg·kg-1)、丙戊酸钠(100 mg·kg-1)和拉莫三嗪(15 mg·kg-1)对6 Hz角膜点燃的C57BL/6J小鼠耐药癫痫发作没有明显作用, 而左乙拉西坦(100 mg·kg-1)明显降低其发作等级(P < 0.05)和大发作几率(P < 0.05);③柴胡加龙骨牡蛎汤(9.36或28.08 g·kg-1)和天麻钩藤饮(14.82或44.46 g·kg-1)对6 Hz角膜点燃的C57BL/6J小鼠耐药癫痫发作没有明显作用, 而风引汤(19.11 g·kg-1)显著降低动物发作等级(P < 0.01)和大发作几率(P < 0.05)。本研究结果表明, C57BL/6J小鼠6 Hz角膜点燃模型是潜在耐药癫痫模型, 而风引汤对其有一定治疗作用, 可能是临床治疗耐药癫痫的潜在中药方剂。

Drug-resistance is a challenge in the treatment of epilepsy, and traditional Chinese medicine (TCM) prescription may have a potential in therapy of the epilepsy.Here we established a modified 6 Hz corneal kindled mouse model of epilepsy, and verified its drug-resistance to four commonly used western medicines.We evaluated the efficacy of three classical TCM prescriptions in this drug-resistant epilepsy model.The results showed that:① most C57BL/6J mice with stimulation current intensity at 44 mA (8 out of 10) were fully kindled, while none ICR mice with stimulation current intensity at 44 mA or C57BL/6J mice with stimulation current intensity at 24 mA were fully kindled.Fully kindled mice exhibited epileptic electroencephalograms after 44 mA and 6 Hz corneal kindling stimulation and increased activation of astrocytes in the hippocampus (by staining the glial fibrillary acidic protein); ② 50 mg·kg-1 phenytoin sodium, 100 mg·kg-1 valproic acid sodium and 15 mg·kg-1 lamotrigine had no significant effects on the drug-resistant seizures in 6 Hz corneal kindled C57BL/6J mice, while 100 mg·kg-1 levetiracetam significantly reduced the seizure stage (P < 0.05) and incidence of generalized seizure (P < 0.05) in it; ③ TCM prescriptions, Chai-Hu Plus Long-Gu Mu-Li decoction (9.36 or 28.08 g·kg-1) and Tian-Ma Gou-Teng decoction (14.82 or 44.46 g·kg-1), had no significant effects on the drug-resistant seizures in 6 Hz corneal kindled C57BL/6J mice, while TCM prescription Feng-Yin decoction (19.11 g·kg-1) significantly reduced both the seizure stage (P < 0.01) and incidence of generalized seizure (P < 0.05) in it.Thus, the modified 6 Hz corneal kindled C57BL/6J mouse model is an excellent drug-resistant epilepsy model, Feng-Yin decoction have antiepileptic effects on it, suggesting Feng-Yin decoction may be an effective TCM prescription for clinical treatment of drug-resistant epilepsy.

癫痫是神经科最常见疾病之一, 其以脑神经元的异常放电引起的反复性的癫痫发作为特征, 在全世界大概有6 500万患者[1]。虽然大部分患者可以通过服用常规抗癫痫药物控制癫痫发作, 但约40%患者会对多种抗癫痫药物耐药而成为耐药性癫痫患者[2]。耐药性癫痫一般定义为对两种或两种以上的抗癫痫治疗方案失败(不论是单药治疗还是多种药物的组合)的癫痫, 并且每种方案均是患者能够耐受的和根据患者发作情况正确选择的合适治疗方案。耐药性癫痫患者中仅小于1%会接受手术治疗, 绝大部分患者依然期待药物治疗[3]。不受控制的癫痫反复发作不但影响患者日常生活而且还可能导致患者认知功能受损甚至突发性死亡[4, 5]。因此耐药性癫痫依然是神经科重大难题, 有必要进一步寻找新的耐药性癫痫治疗方法。
癫痫属于中医“痫病”范畴, 早在《内经》、《千金要方》等古籍上就有记载, 且有不少方药流传至今或被历代医家改良[6, 7]。不少临床及基础研究也表明中药方剂可能对耐药性癫痫有潜在治疗作用[8]。然而由于中医对癫痫的证候诊断特殊性等原因, 中药方剂对耐药性癫痫的作用及其机制尚不清楚, 有待进一步研究。疾病动物模型是筛选疾病治疗新药的重要方法, 通过建立可靠的耐药性癫痫模型将有助于筛选新的抗癫痫药物及相关机制研究。以往常见的癫痫动物模型包括遗传性癫痫模型、化学模型和杏仁核电点燃模型等, 但是这些模型往往耐药比例较低、过于费时费力或存在药物相互作用干扰等缺点[9-11]。最近, 研究者发现通过6 Hz角膜电刺激可以在NMRI品系和CF1品系小鼠建立角膜电点燃癫痫模型, 该模型不但制作相对简单而且可能对多种抗癫痫药物耐受[12, 13]。然而NMRI品系和CF1品系小鼠国内尚未普及, 若采用国内常用C57BL/6J和ICR小鼠品系进行造模, 其刺激参数及造模后的耐药情况尚不明确。因此, 本研究拟在C57BL/6J和ICR小鼠品系采用不同的刺激参数尝试建立和优化6 Hz角膜点燃模型, 并就4种常用西药耐药情况和3种传统中药方剂的药效学进行研究。
动物  选用8周龄C57BL/6J和ICR雄性小鼠[由浙江中医药大学实验动物中心提供, 实验动物使用许可证号SYXK (浙) 2013-0184]。小鼠的饲养环境相同, 控制环境温度(22 ± 1) ℃, 控制湿度(50 ± 5) %, 自由摄水饮食。所有的动物实验严格遵照浙江中医药大学动物实验中心的动物伦理学条例及国家实验动物饲养和使用指南。
药品与仪器  利多卡因(批号Y05J6C3)、苯妥英钠(批号Y16N6C6003)、丙戊酸钠(批号Y18J7C16394)、拉莫三嗪(批号S29A6B2883)和左乙拉西坦(批号B07N8E46912)均购自上海源叶生物科技有限公司; 柴胡加龙骨牡蛎汤(柴胡12 g, 煅龙骨、黄芩、生姜、桂枝、茯苓各4.5 g, 党参9 g, 姜半夏6 g, 生大黄6 g, 生牡蛎4.5 g, 红枣12 g, 铅丹未用)、天麻钩藤饮(天麻片9 g, 钩藤、川牛膝各12 g, 石决明18 g, 生山栀、生杜仲、黄芩、益母草、桑寄生、夜交藤、茯神各9 g)和风引汤(大黄、干姜、龙骨各12 g, 桂枝9 g, 甘草、牡蛎各6 g, 寒水石、滑石、赤石脂、紫石英、石膏各18 g)由浙江中医药大学门诊部提供; 电惊厥仪(ECT UNIT 57800, 意大利UGO BASLE公司)。
6 Hz角膜点燃刺激  将电惊厥仪刺激参数调整为(44或24 mA, 0.2 ms, 3 s, 6 Hz)。角膜刺激电极先用含0.5%利多卡因的生理盐水浸泡后, 贴近动物眼睛直至完全接触, 解锁电惊厥仪后踩踏触发开关进行角膜刺激, 刺激结束后立即将动物放入观察箱内, 记录动物癫痫行为学发作情况。
癫痫行为学判定  根据Racine等级分类法判定动物癫痫行为等级[14], 1级:面部抽搐; 2级:点头; 3级:单侧前肢阵挛发作; 4级:起身, 双侧前肢阵挛发作; 5级:双侧前肢阵挛发作伴倒地。1~3级为局灶性癫痫发作, 4~5级发作为全身性癫痫大发作。若动物出现连续3次或累计10次4~5级发作则认为造模成功。
6 Hz角膜点燃模型建立及优化  采用8周龄C57BL/6J与ICR小鼠在电惊厥仪刺激参数(44或24 mA, 0.2 ms, 3 s, 6 Hz)的条件下进行角膜点燃, 连续3周, 每天2次, 两次间隔大于4 h, 观察并记录C57BL/6J小鼠与ICR小鼠的癫痫行为。
手术及脑电图记录  小鼠腹腔注射戊巴比妥钠50 mg·kg-1麻醉, 后固定于立体定位仪, 将电极根据小鼠脑图谱(Paxinos and Franklin's the Mouse Brain)坐标将不锈钢电极植入海马(AP: -2.0 mm; L: -1.5 mm; V: -2.0 mm), 小脑上方(AP: -4.5 mm, L: 0 mm)作为参考电极。脑电图记录采用Powerlab系统(澳大利亚AD Instruments公司), 采样率为1 000 Hz, 滤波范围为0~100 Hz。
脑组织切片及免疫组化  小鼠腹腔注射水合氯醛400 mg·kg-1深度麻醉, 用0~4 ℃冰浴生理盐水进行心脏灌流, 待流出液不含血色后, 换4%多聚甲醛溶液灌流进行前固定。之后将小鼠置于冰上, 断头取脑, 随后置入4%多聚甲醛在4 ℃冰箱内固定24 h, 30%蔗糖脱水。完全脱水的脑组织用于冰冻切片(MICROM HM 550, Thermo公司), 制成20 μm切片, 并进行免疫组化染色。免疫组化一抗为anti-GFAP (glial fibrillary acidic protein) antibody (1:400, ab7260), 荧光二抗为anti-rabbit IgG H & L (Alexa Fluor® 488, 1:200, ab150077) (Abcam公司)。最后用含有DAPI的防淬灭封片液封片(上海源叶生物科技有限公司), 并使用荧光显微镜采集图像。
药效学评价  采用8周龄C57BL/6J小鼠, 每天2次角膜点燃刺激(44 mA, 0.2 ms, 3 s, 6 Hz), 两次间隔大于4 h, 直至出现连续3次或累计10次4或5级发作即完全点燃。完全点燃动物进行每天1次角膜刺激, 参数同前, 动物一旦连续3天稳定4或5级发作, 则第4天被用于药效学研究。
抗癫痫药物(西药)均采用腹腔注射30 min后进行药效学测试, 剂量分别为苯妥英钠50 mg·kg-1、丙戊酸钠100 mg·kg-1、拉莫三嗪15 mg·kg-1和左乙拉西坦100 mg·kg-1。本部分实验中, 采用自身对照设计, 动物第1天给予溶剂, 第2天给予药物。测定1种药物后, 休息3天待药物清除后被继续重复使用。在当天给药30 min后立即进行6 Hz角膜点燃测试。
中药方剂均为水煎浓缩后采用灌胃给药, 剂量按生药量计算分别为柴胡加龙骨牡蛎汤9.36 g·kg-1、天麻钩藤饮14.82 g·kg-1和风引汤19.11 g·kg-1, 对照组给予相当体积的水(0.3 mL)。每天1次, 给药3天, 在末次给药30 min后进行6 Hz角膜点燃测试。该部分实验中, 采用组间随机对照设计, 动物模型稳定后随机分入各组立即开始给药, 各组间动物不重复使用。
统计学分析  采用Grapad Prism 5.0软件进行统计分析。所有数据均表示为均值±标准差($\overline{x}±s$)。采用非参数Mann-Whitney检验分析各组间的癫痫发作等级差异, 采用Fisher精确检验统计各组间癫痫大发作几率差异。P < 0.05被认为有显著性差异。
本研究比较国内常用的ICR和C57BL/6J品系小鼠在24或44 mA刺激强度下的造模成功率。实验发现, 在ICR小鼠中, 44 mA的刺激强度无法诱导明显癫痫行为, 经过21天6 Hz角膜点燃刺激后所有动物均无法达到典型的4级或5级癫痫发作, 代表性行为表现如图 1A左所示; 在C57BL/6J小鼠中, 经过21天6 Hz角膜点燃刺激后24或44 mA电流强度均可以诱导动物出现癫痫的4级或5级癫痫发作, 代表性行为表现如图 1A右所示。但在24 mA电流强度下动物无法连续3次稳定诱导4级或5级癫痫发作。如图 1B所示, 第21天C57BL/6J-44 mA组动物发作等级明显高于ICR-44 mA组(P < 0.001)和C57BL/6J-24 mA组(P < 0.01)。各组动物21天后的造模成功率(点燃率)如表 1所示, C57BL/6J-44 mA组造模成功率明显高于ICR-44 mA组(P < 0.01)和C57BL/6J-24 mA组(P < 0.01)。进一步进行脑电图记录发现, C57BL/6J小鼠在44 mA、6 Hz角膜点燃诱导的癫痫发作的过程中, 脑电图出现明显的癫痫样放电(图 2)。利用免疫组化还发现C57BL/6J-44 mA组完全点燃的小鼠海马GFAP标记的胶质细胞激活明显增加(图 3), 其为颞叶癫痫的特征病理标志之一。
为了进一步确认C57BL/6J小鼠角膜点燃癫痫模型对抗癫痫药物的耐药情况, 本研究观察了4种抗癫痫药物(苯妥英钠、丙戊酸钠、拉莫三嗪、左乙拉西坦)对点燃后动物癫痫发作的影响。结果表明, 苯妥英钠、丙戊酸钠、拉莫三嗪对动物发作等级及大发作几率均没有明显影响(P > 0.05), 而左乙拉西坦明显降低动物发作等级(P < 0.05)和大发作几率(P < 0.05)。具体各组发作等级和大发作几率如表 2所示。
在基本明确C57BL/6J小鼠角膜点燃癫痫模型为耐药癫痫模型后, 本研究观察3种中药方剂(柴胡加龙骨牡蛎汤、天麻钩藤饮和风引汤)对点燃动物癫痫发作的作用。结果发现, 连续3天给予柴胡加龙骨牡蛎汤和天麻钩藤饮对动物癫痫发作等级和大发作几率均没有明显作用(P > 0.05), 而风引汤可以显著降低动物癫痫发作等级(P < 0.01)和大发作几率(P < 0.05)。具体各组发作等级和大发作几率如表 3所示。
考虑剂量的影响, 上述实验结束后动物再继续给予3天3倍剂量的柴胡加龙骨牡蛎汤和天麻钩藤饮(将药物浓缩3倍)依然没有明显的抗癫痫作用, 各给药组癫痫发作等级和大发作几率与对照组没有统计学差异(P > 0.05)。具体各组发作等级和大发作几率如表 4所示。天麻钩藤饮组有1例动物给药过程中死亡(非灌胃过程中死亡)。风引汤因含矿物类药物太多, 浓缩后药液过稠而未完成3倍剂量给药实验。
本研究通过比较ICR和C57BL/6J品系小鼠及不同刺激强度(24或44 mA), 建立和优化了6 Hz角膜点燃小鼠模型, 该模型国内尚未见报道。本研究发现该模型对3种抗癫痫药物(苯妥英钠、丙戊酸钠、拉莫三嗪)和2种中药方剂(柴胡加龙骨牡蛎汤、天麻钩藤饮)存在一定耐药性, 确证其是一种潜在耐药癫痫模型。此外, 本研究还首次发现中药经方风引汤可以降低该模型动物发作等级和大发作几率, 提示其可能有助于临床耐药性癫痫治疗。
以往药物研发过程中主要采用最大电惊厥、戊四唑癫痫模型和杏仁核点燃癫痫模型等进行筛选[15]。本研究中所使用的苯妥英钠是最大电休克模型的经典阳性药, 丙戊酸钠是戊四唑癫痫模型的经典阳性药, 左乙拉西坦是杏仁核电点燃癫痫模型的经典阳性药物, 而拉莫三嗪是广谱抗癫痫药, 对部分性和全身性癫痫发作等多种类型均有效。本研究结果表明苯妥英钠、丙戊酸钠和拉莫三嗪对6 Hz角膜点燃模型的发作等级及大发作几率均没有明显作用, 仅左乙拉西坦明显降低动物的发作等级和大发作几率。这些结果与国外其他品系动物相关研究结果[12, 16]及临床耐药癫痫患者类似[17], 提示本研究建立的C57BL/6J小鼠6 Hz角膜点燃模型可能是一个较好的多药耐药的癫痫模型。
进一步与以往文献比较发现: ①文献报道在NMRI小鼠6 Hz角膜点燃模型中丙戊酸钠的半数有效剂量(ED50)为92.4 mg·kg-1[12], 而本研究中100 mg·kg-1丙戊酸钠在C57BL/6J品系小鼠6 Hz角膜点燃模型中大发作几率为80% (可能未达到该模型的半数有效剂量)。②在NMRI小鼠6 Hz角膜点燃模型中左乙拉西坦的ED50约为18.7 mg·kg-1[12], 给予100 mg·kg-1左乙拉西坦大发作几率约为30%[18], 而本研究中给予100 mg·kg-1左乙拉西坦, C57BL/6J品系小鼠6 Hz角膜点燃模型大发作几率为44.4% (比例相对略高)。③苯妥英钠和拉莫三嗪的药效情况仅在CF1小鼠60 Hz角膜点燃模型中有所报道[16], 常规剂量具有一定治疗作用(苯妥英钠ED50约为18.3 mg·kg-1、拉莫三嗪ED50约为9.5 mg·kg-1), 而本研究C57BL/6J品系小鼠6 Hz角膜点燃模型中苯妥英钠50 mg·kg-1和拉莫三嗪15 mg·kg-1均没有明显作用。当然, 由于6 Hz角膜点燃模型耐药性本身可能强于60 Hz角膜点燃模型[12], 因而暂时无法与本文模型进行直接比较。总之, 上述实验及他人的研究结果提示C57BL/6J品系小鼠6 Hz角膜点燃模型相对于NMRI品系和CF1品系角膜点燃模型的耐药性可能略强。
成功建立6 Hz角膜点燃癫痫模型并验证其耐药性后, 本研究基于以往古籍经典和临床相关研究选择了3种抗癫痫经典中药方剂, 分别为柴胡加龙骨牡蛎汤、天麻钩藤饮和风引汤。柴胡加龙骨牡蛎汤首载于张仲景《伤寒论》, 后历代医家逐渐发现该方可治疗癫痫。如徐灵胎《伤寒论类方》中“本方下肝胆之惊痰, 治癫痫必效。”和《类聚方广义》中“癫痫, 居常胸满上逆, 每月二三发者, 常服此方, 则免屡发之患。”特别是Wu等[19]通过对20例难治性癫痫患者研究发现, 柴胡加龙骨牡蛎汤可能降低难治性癫痫发作频率, 提示其可能具有难治性癫痫治疗作用。天麻钩藤饮出自《中医内科杂病证治新义》, 有平肝熄风, 清热活血之功, 常用于肝阳偏亢, 肝风上扰证, 有可能有助于治疗癫痫[20]。风引汤出自《金匮要略·中风篇》“大人风引, 少小惊痫瘛疭, 日数十发, 医所不能治者, 此汤主之。”, 后历代医家用于主治癫痫、风瘫等[21]。现代药理学研究表明上述中药方中钩藤、柴胡、大黄等中药成分可能作用于如丝裂原活化蛋白激酶(mitogen-activated protein kinase, MAPK)等难治性癫痫发病相关的信号通路[22-24]。本研究结果表明柴胡加龙骨牡蛎汤和天麻钩藤饮在临床相当剂量或3倍临床相当剂量下对6 Hz角膜点燃模型均没有明显的抗难治性癫痫作用, 而风引汤在临床相当剂量便能明显降低动物的癫痫发作等级和大发作几率。这些结果表明风引汤是耐药性癫痫治疗的潜在有效中药方, 提示在中药经典方剂中寻找对耐药性癫痫有治疗作用的药物可能可行, 值得后续进一步研究。
当然, 由于本研究存在一些干扰因素, 如: ①所用癫痫模型与特定方剂所针对的中医证候可能不一致; ②柴胡加龙骨牡蛎汤按目前临床惯例弃用了铅丹; ③动物实验与临床患者存在差异等。有关中药方剂对耐药性癫痫的治疗作用仍需要进一步考察, 尤其需要后续大样本、随机对照及双盲的循证医学研究, 以及更深入的作用机制研究。
