This study aimed to investigate the role and mechanism of sappanone A (SA) in regulating renal ischemia-reperfusion injury (IRI) in rats. The animal experiment has been approved by the Ethics Committee of Suzhou Wujiang District Children's Hospital (approval number: 2022010). First, hematoxylin-eosin (H&E) staining was used to evaluate the effects of SA on IRI, and renal damage was scored. Serum creatinine (SCr), blood urea nitrogen (BUN) and cystatin C (Cystatin C) were analyzed. The effect of sappanone A on the apoptosis of renal tubular epithelial cells induced by IRI was analyzed by TUNEL staining. Protein expression levels of p-JNK/JNK, p-ERK/ERK, Bcl2, Bax and cleaved-caspase 3 in renal tissues were detected by Western blot. Finally, H&E staining, serological analysis, TUNEL staining and Western blot were used to determine whether JNK activator anisomycin could reverse the effect of SA on IRI in rats. The results showed SA significantly reduced the renal tubule injury caused by ischemia-reperfusion, and decreased the level of SCr, BUN and Cys C in serum. TUNEL staining showed that SA significantly reduced the apoptosis of renal tubular epithelial cells induced by IRI. Western blot analysis of kidney tissue showed that SA significantly promoted the expression of apoptosis inhibiting protein Bcl2 and inhibited the expression of apoptosis-promoting proteins Bax and cleaved-caspase 3. Further analysis elucidated that SA did not affect the phosphorylation of ERK but decreased the phosphorylation of JNK. Finally, H&E staining, serological analysis, TUNEL staining and Western blot confirmed that JNK activator anisomycin could reverse the alleviating effect of SA on IRI in rats. The above findings suggest that SA could alleviate IRI in rats by inhibiting JNK phosphorylation.

本研究旨在探讨苏木酮A (sappanone A, SA) 调控大鼠肾脏缺血再灌注损伤(ischemia-reperfusion injury, IRI) 的作用和机制。动物实验已获得苏州市吴江区儿童医院伦理委员会批准(批准号: 2022010)。首先利用苏木精-伊红染色法(hematoxylin-eosin staining, H&E) 观察大鼠肾组织形态学变化并进行肾损伤评分; 提取血清检测肌酐(serum creatinine, SCr)、尿素氮(blood urea nitrogen, BUN) 和胱抑素C (cystatin C, Cys C) 的含量; 通过TUNEL染色进一步分析苏木酮A对IRI引起的肾小管上皮细胞凋亡情况的影响; 免疫印迹法(Western blot) 检测肾组织中p-JNK/JNK、p-ERK/ERK、Bcl2、Bax和cleaved-caspase 3的蛋白表达水平。最后, 通过以上研究方法明确JNK激活剂茴香霉素(anisomycin, Ani) 是否可以逆转苏木酮A对大鼠IRI的保护作用。结果显示, 苏木酮A明显减轻IRI引起的肾小管损伤, 减少血清中SCr、BUN和Cys C的含量。TUNEL染色显示, 苏木酮A明显减少IRI引起的肾小管上皮细胞凋亡。Western blot检测肾组织表明, 苏木酮A明显促进凋亡抑制蛋白Bcl2的表达, 抑制凋亡促进蛋白Bax和cleaved-caspase 3的表达, 进一步分析显示苏木酮A不影响ERK的磷酸化, 而抑制JNK的磷酸化。最后, 通过H&E染色、血清学检测、TUNEL染色和免疫印迹法证实JNK激活剂茴香霉素可以逆转苏木酮A对大鼠IRI的保护作用。上述研究结果表明, 苏木酮A通过抑制JNK磷酸化减轻大鼠肾脏缺血再灌注损伤。

This study aimed to investigate the role and mechanism of sappanone A (SA) in regulating renal ischemia-reperfusion injury (IRI) in rats. The animal experiment has been approved by the Ethics Committee of Suzhou Wujiang District Children's Hospital (approval number: 2022010). First, hematoxylin-eosin (H&E) staining was used to evaluate the effects of SA on IRI, and renal damage was scored. Serum creatinine (SCr), blood urea nitrogen (BUN) and cystatin C (Cystatin C) were analyzed. The effect of sappanone A on the apoptosis of renal tubular epithelial cells induced by IRI was analyzed by TUNEL staining. Protein expression levels of p-JNK/JNK, p-ERK/ERK, Bcl2, Bax and cleaved-caspase 3 in renal tissues were detected by Western blot. Finally, H&E staining, serological analysis, TUNEL staining and Western blot were used to determine whether JNK activator anisomycin could reverse the effect of SA on IRI in rats. The results showed SA significantly reduced the renal tubule injury caused by ischemia-reperfusion, and decreased the level of SCr, BUN and Cys C in serum. TUNEL staining showed that SA significantly reduced the apoptosis of renal tubular epithelial cells induced by IRI. Western blot analysis of kidney tissue showed that SA significantly promoted the expression of apoptosis inhibiting protein Bcl2 and inhibited the expression of apoptosis-promoting proteins Bax and cleaved-caspase 3. Further analysis elucidated that SA did not affect the phosphorylation of ERK but decreased the phosphorylation of JNK. Finally, H&E staining, serological analysis, TUNEL staining and Western blot confirmed that JNK activator anisomycin could reverse the alleviating effect of SA on IRI in rats. The above findings suggest that SA could alleviate IRI in rats by inhibiting JNK phosphorylation.

缺血再灌注损伤是指心脏、肺脏、肝脏和肾脏等脏器在缺血一段时间后恢复血流灌注, 供血区域组织损伤加重或产生新损伤的现象。肾脏缺血再灌注损伤是临床上常见的病理生理现象, 其在肾脏移植、肾外伤、失血性休克、心血管疾病手术和弥散性血管内凝血等情况下均可发生[1]。肾缺血再灌注损伤的发病机制目前尚未完全阐明, 大量研究表明肾小管上皮细胞凋亡、ATP的减少、线粒体功能障碍、细胞内钙超载、大量氧自由基和炎性细胞因子的产生及中性粒细胞聚集等均参与调控肾缺血再灌注损伤的发生和发展[2-4]。目前, 临床上主要通过低温减少组织氧耗、再灌注前给予钙阻断剂和清除氧自由基的药物等措施来减轻肾脏缺血再灌注损伤, 但其引发的急性肾损伤的发病率和死亡率仍居高不下[5], 部分患者甚至发展为慢性肾脏疾病或终末期肾病。因此, 深入研究肾缺血再灌注损伤发生发展的机制, 发掘潜在的有效保护肾缺血再灌注损伤的措施具有十分重要的意义。
肾小管上皮细胞凋亡是目前研究最广泛的参与调控肾缺血再灌注损伤的关键机制之一。ERK和JNK作为MAPK通路的主要效应分子, 是调控细胞凋亡的重要因素[6]。目前, ERK磷酸化和激活主要被认为发挥抑制细胞凋亡的作用, 磷酸化的ERK可以通过调控下游转录因子(如Creb和Stat3等) 的活性, 下调促凋亡基因(如Bad) 或上调抑凋亡基因(如Bcl2和Bcl-xl等) mRNA水平的表达, 从而抑制细胞的凋亡[7, 8]。同时, 磷酸化的ERK作为激酶也可以通过磷酸化修饰并调控凋亡相关蛋白(如Bad、Bim、Mcl-1和caspase-9等), 从而促进细胞存活。相反, JNK磷酸化和激活主要被认为发挥促进细胞凋亡的作用, 其可以通过激活下游转录因子(如c-Jun/AP1和p53等), 上调促凋亡基因(如Bad和Bim等) mRNA水平的表达, 从而促进细胞凋亡。同时, 磷酸化的JNK也可以通过磷酸化修饰并调控凋亡相关蛋白(如Bad和Bim和Bcl2等), 促进细胞凋亡[9]。目前, 也有部分研究发现ERK的促凋亡和JNK的抑凋亡作用可能与不同的刺激条件和组织类型有关[6]
苏木酮A (sappanone A, SA) 是一种从植物苏木中提取的高异黄烷酮, 具有抗氧化和抗炎作用。有研究表明, 缺血前苏木酮A预处理可以通过PKC或PI3K/Akt通路激活Nrf2, 从而在体外保护心肌缺血再灌注损伤[10]。同时, 苏木酮A处理后可以通过激活AMPK来调节线粒体质量, 从而改善心肌缺血再灌注损伤和线粒体功能障碍[11]。另有体外研究利用大鼠肾上腺嗜铬细胞瘤细胞(PC12) 等神经细胞建立缺糖缺氧再灌注模型(OGD/R) 模拟脑缺血再灌注损伤环境, 用苏木酮A处理细胞, 结果发现苏木酮A可以通过抑制线粒体介导的caspase凋亡信号通路保护神经细胞免受脑缺血再灌注损伤[12]。然而, 目前对于苏木酮A在肾脏缺血再灌注损伤中是否具有保护作用以及其具体机制尚不清楚。
本研究旨在通过构建大鼠肾缺血再灌注模型, 从组织学、血清学和分子学多角度探究苏木酮A对肾缺血再灌注损伤的作用和具体调控机制, 为肾缺血再灌注损伤的治疗提供新的思路。

大鼠肾脏缺血再灌注损伤模型的构建    Sham组(n = 5): 取雄性SD大鼠, 术前消毒灭菌所有手术器械和手术台。大鼠称重后利用异氟烷吸入麻醉。将大鼠仰卧位固定于手术台架上, 剪毛备皮, 碘伏消毒皮肤, 沿腹中线行纵向切口, 逐层切开皮肤及皮下组织, 充分暴露肾脏, 仅分离肾蒂但不夹闭, 45 min后关腹缝合。IRI组(n = 5): 手术方法基本同Sham组, 切除右肾分离左肾肾蒂后, 夹闭左侧肾蒂45 min, 然后松开动脉夹制备肾脏缺血再灌注损伤模型。IRI+DMSO组(n = 5): 手术方法同IRI组, 于造模前2 h经腹腔注射对照溶剂DMSO。IRI+SA组(n = 5): 手术方法同IRI组, 于造模前2 h经腹腔注射苏木酮A (10、20或30 mg·kg-1)。IRI+MK组(n = 5): 手术方法同IRI组, 于造模前2 h经腹腔注射孟鲁司特(10 mg·kg-1)。IRI+SA+Ani组(n = 5): 手术方法同IRI组, 苏木酮A (30 mg·kg-1) 与茴香霉素(20 mg·kg-1) 于造模前2 h经腹腔注射。苏木酮A、茴香霉素及孟鲁司特均以DMSO溶解。恢复再灌注24 h后留取血清及肾组织标本, 进行相关指标检测。
血清提取和检测    成功构建模型后, 经腹主动脉采血3 mL, 上下轻轻颠倒混匀3~5次, 3 000 r·min-1离心15 min, 吸取上清液并分装, -80 ℃保存。SCr、BUN和Cys C含量根据厂家的说明书, 利用试剂盒进行检测。
H&E染色   60 ℃烘箱中放置30 min, 二甲苯中脱蜡5 min, 换用新鲜的二甲苯, 再脱蜡5 min, 无水乙醇5 min, 90%乙醇5 min, 70%乙醇5 min, 蒸馏水5 min, 自来水冲洗1 min后苏木素染色5 min, 自来水冲洗1 min, 根据苏木素染色深浅进行分化液分化, 自来水冲洗1 min, 进入返蓝液处理5 min, 自来水冲洗1 min后伊红染色2 min, 自来水冲洗1 min, 梯度脱水透明(70%乙醇5 min, 90%乙醇5 min, 无水乙醇5 min, 二甲苯5 min), 最后中性树脂封片, 显微镜下观察。
肾小管损伤评分    每张H&E染色切片拍取5张200倍镜视野的图片用于评估肾小管损伤。评分主要根据肾小管细胞坏死、刷状缘丧失、管型形成和小管扩张的百分比来进行, 0分: 无; 1分: < 10%; 2分: 11%~25%; 3分: 26%~45%; 4分: 46%~75%; 5分: > 76%。
TUNEL染色   大鼠肾脏切片的TUNEL染色利用碧云天TUNEL细胞凋亡检测试剂盒完成, 取不同处理大鼠石蜡切片, 二甲苯中脱蜡5~10 min, 换用新鲜的二甲苯, 再脱蜡5~10 min。无水乙醇5 min, 90%乙醇5 min, 70%乙醇5 min, 蒸馏水5 min, 用油性笔圈定组织范围, 滴加20 μg·mL-1不含DNase的蛋白酶K, 室温作用30 min后PBS洗涤3次。按照TdT酶∶荧光标记液∶TUNEL检测液以1∶9∶10的比例配置TUNEL工作液, 在样品上加50 μL TUNEL工作液, 37 ℃避光孵育60 min, PBS洗涤3次, 用抗荧光淬灭封片液封片后荧光显微镜下观察, 亮绿色荧光胞核即为阳性细胞。采用Image J软件进行细胞计数, 以TUNEL阳性细胞数与总细胞数之比计算肾小管上皮细胞凋亡比例。
组织蛋白提取    称取200 mg组织, 放入2 mL离心管中用消毒过的外科剪刀剪碎。按照RIPA∶蛋白酶抑制剂∶磷酸酶抑制剂以100∶1∶1的比例配制裂解液, 加入1 mL蛋白裂解液, 用组织研磨仪以60 Hz研磨60 s至组织完全裂解, 整个过程在4 ℃进行。裂解后12 000 r·min-1、4 ℃离心20 min, 收集上清转移至新的1.5 mL离心管中, 进行BCA蛋白浓度定量检测, 测定蛋白浓度后, 按4∶1的比例加入5×上样缓冲液, 煮沸变性, 保存于-20 ℃。
蛋白质免疫印迹实验    将准备好的蛋白样品用10%的SDS⁃PAGE电泳分离后用湿转法转至PVDF膜。5%的牛血清白蛋白(BSA) 常温封闭2 h, 根据不同抗体类型配置抗体稀释液, 与条带4 ℃孵育过夜。0.2% TBST缓冲液洗涤条带3次, 每次10 min, 按照不同抗体种属配置含辣根过氧化物酶标记的二抗, 与条带常温孵育1 h, 0.2% TBST缓冲液洗涤条带3次, 每次10 min, 洗涤后用ECL发光液与过氧化物溶液1∶1配置的ECL工作液显影。将所得条带图像用Image J软件进行灰度分析并统计。
统计学分析    所有动物实验的结果以均数±标准差表示, 所得数据采用GraphPad Prism 7进行统计和作图。单因素方差分析和最小显著性差异法用于检验多组数据的差异, P < 0.05为差异有统计学意义。
为了探索苏木酮A在肾缺血再灌注损伤中的作用, 本研究首先利用SD大鼠通过夹闭肾蒂45 min, 再灌注24 h构建肾缺血再灌注模型。研究发现, 与Sham组正常的肾小管结构相比, IRI组及IRI+DMSO组肾小管上皮细胞出现水肿、空泡变性、脱落和坏死等, 部分肾小管正常结构消失, 肾小管损伤评分也明显高于Sham组(图 1AB), 而IRI组及IRI+DMSO组间无统计学差异, 表明单纯DMSO处理并不影响缺血再灌注引起的肾小管损伤, 这与Kar等[13]研究者的报道一致。进一步通过腹腔注射不同浓度的苏木酮A做预处理, 与IRI+DMSO组相比, 苏木酮A处理组肾小管损伤明显减轻, 肾小管损伤评分明显降低(图 1B)。进一步提取大鼠的血清检测肾功能相关指标发现, IRI组及IRI+DMSO组SCr、BUN和Cys C的含量较Sham组增高, 而苏木酮A预处理明显减轻了肾脏的组织学损伤, 血清中SCr、BUN和Cys C的含量较IRI+DMSO组降低, 且与苏木酮A的剂量呈正相关(图 1C~E)。孟鲁司特作为本实验的阳性对照, 也表现出对肾缺血再灌注损伤的保护作用,这与既往研究的结论一致(图 1A~E)[14, 15]
肾小管细胞凋亡是肾脏缺血再灌注损伤的重要特征[16], 本课题组进一步研究了苏木酮A对肾小管细胞凋亡的影响。通过收集Sham组、IRI组、IRI+DMSO组和苏木酮A处理组的肾组织切片进行TUNEL染色发现, 与Sham组正常的肾小管结构相比, IRI组及IRI+DMSO组肾小管上皮细胞凋亡比例明显增多, 而苏木酮A预处理后肾小管上皮细胞凋亡明显减轻(图 2AB), 这与组织学和血清学的结果一致。孟鲁司特作为本实验的阳性对照, 也明显减轻了肾缺血再灌注损伤导致的肾小管上皮细胞凋亡程度, 这与既往研究的结论一致[14, 15]
细胞凋亡是由多种凋亡促进蛋白(Bax、Bak、Bok、Bcl-xS、Bid、Bad和Egl-1等) 和凋亡抑制蛋白(Bcl2、Bcl-xL、Bcl-w、Mcl-1、A1、Boo和Ced-9等) 共同参与调控, 引起凋亡小体的形成、半胱天冬蛋白酶(caspase) 激活级联放大反应导致的一种细胞程序性死亡, 细胞凋亡可以受到PI3K、ERK和JNK等多条信号通路的调控[17]。为了进一步探究苏木酮A调控肾小管细胞凋亡的机制, 本研究利用蛋白质免疫印迹实验分析后发现, 与Sham组相比, IRI+DMSO组中凋亡促进蛋白Bax、凋亡抑制蛋白Bcl2和凋亡效应酶c-caspase 3的表达均反应性上调, 苏木酮A预处理则明显抑制了Bax和c-caspase 3的表达, 而促进了凋亡抑制蛋白Bcl2的表达(图 3AB)。进一步分析发现, 缺血再灌注损伤并不影响JNK和ERK的总量, 但反应性上调了JNK和ERK的磷酸化, 苏木酮A预处理并不影响ERK的磷酸化, 但明显抑制了JNK的磷酸化(图 3AB), 表明苏木酮A可能是通过抑制JNK通路的激活从而调控肾小管细胞凋亡。孟鲁司特作为本实验的阳性对照, 其预处理则明显抑制了Bax和c-caspase 3的表达, 而促进了凋亡抑制蛋白Bcl2的表达, 对JNK和ERK的磷酸化均有抑制作用, 这与既往研究的结论一致[14, 15]
为了进一步验证苏木酮A是否通过抑制JNK通路的激活调控肾小管细胞凋亡以及减轻肾缺血再灌注损伤, 本研究利用JNK特异性激活剂茴香霉素进行回复实验。H&E染色证实, 茴香霉素在组织学上明显逆转了苏木酮A对肾脏缺血再灌注损伤的保护作用(图 4AB)。TUNEL染色表明, 苏木酮A可以明显减少肾脏缺血再灌注损伤导致的肾小管细胞凋亡, 这一现象也同样可以被茴香霉素逆转(图 4CD)。同时, 血清学也得到了同样的结论(图 4E)。通过蛋白质免疫印迹实验进一步分析, 验证了苏木酮A预处理对p-JNK、Bax和c-caspase 3表达的抑制作用, 以及对Bcl2表达的促进作用均可以被茴香霉素所逆转(图 4FG), 以上结果都表明茴香霉素可以逆转苏木酮A对大鼠肾脏缺血再灌注损伤的保护作用, 苏木酮A通过抑制JNK通路的激活调控肾小管细胞凋亡以及肾缺血再灌注损伤。
天然产物已被广泛认为是治疗难治性肾脏疾病的重要方案[18-20]。本研究发现苏木酮A对肾缺血再灌注诱导的大鼠急性肾损伤具有保护作用, 确定了苏木酮A是一种有前途的治疗肾缺血再灌注损伤的药物。
在肾缺血再灌注诱导的急性肾损伤中, 近端小管上皮细胞极易受到损伤, 细胞凋亡在这一过程中起着关键作用。线粒体是促凋亡蛋白和抗凋亡蛋白整合的关键位点。近端小管上皮细胞受到损伤应激时, Bax、Bad等促凋亡蛋白与线粒体膜结合, 导致细胞膜通透性增加, 进而导致细胞色素c氧化酶释放到细胞质中, 激活caspase家族, 促进细胞凋亡的发生。因此, 开发具有抑制线粒体凋亡途径、减轻肾缺血再灌注导致的急性肾损伤的药物至关重要。先前的研究已证明苏木酮A可减轻心肌和脑缺血再灌注损伤[10-12], 同时, 也有研究表明苏木酮A对糖尿病肾病和顺铂诱导的肾损伤均有保护作用[21, 22]。本研究发现苏木酮A可以减轻缺血再灌注导致的肾脏损伤, 并发现其在线粒体途径上对肾缺血再灌注导致的细胞凋亡具有抑制作用。
JNK和ERK是调控肾缺血再灌注相关细胞凋亡最重要的两条通路。目前, 对于ERK通路调控肾缺血再灌注损伤及细胞凋亡的作用仍存在争议, 有研究表明促红细胞生成素和NADPH氧化酶抑制剂DPI对肾缺血再灌注损伤及细胞凋亡的保护作用与激活肾组织ERK信号有关[23, 24]。然而, Alderliesten等[25]研究报道腹腔给予ERK抑制剂U0126后, 缺血再灌注损伤大鼠肾组织的ERK磷酸化水平降低, 再灌注后损伤得到改善, 提示肾缺血再灌注时ERK通路激活介导肾缺血再灌注损伤。类似报道如田蓟苷、绞股蓝皂甙和三七等对肾缺血再灌注损伤保护作用也与抑制肾组织ERK信号通路有关[26-28]。由此可见, ERK信号通路在肾缺血再灌注损伤中的作用仍存在争议。但是, JNK通路对肾缺血再灌注相关细胞凋亡的调控作用目前较为明确, JNK通路激活导致下游c-Jun等转录因子的活化, 调控凋亡相关基因的表达, 介导细胞死亡的发生。JNK抑制剂SP600125可以通过抑制肾小管上皮细胞凋亡从而明显减轻肾缺血再灌注损伤[29]。类似报道如吴茱萸次碱和姜黄素等对肾缺血再灌注损伤的保护作用也与抑制JNK通路有关[30, 31]
本研究表明苏木酮A可以选择性地抑制JNK通路的激活从而减轻肾脏缺血再灌注损伤, 对ERK通路则无明显影响。但是其抑制JNK通路激活的具体机制尚不明确, 仍待进一步研究。
综上, 苏木酮A通过抑制JNK通路的激活调控肾小管细胞凋亡以及减轻肾缺血再灌注损伤, 是一种有治疗肾缺血再灌注损伤前景的天然药物。
