The interaction between Mycobacterium tuberculosis and host, as well as the regulation of some signaling pathways in the host, were involved in pathogen latency in macrophages. microRNAs (miRNAs) regulate the gene expression and biological functions, indicating that miRNAs played a regulatory role in bacterial infections. However, whether the host's miRNAs were also involved in the process of Mycobacterium tuberculosis infection had not been thoroughly studied. This study infected macrophages with pathogenic Mycobacterium tuberculosis strain H37Rv and low virulence strain H37Ra to explore the functional miRNAs. By identifying the expression profile of miRNAs in host cells after infection, the expression of 17 miRNAs significantly changed (P<0.05) in the human macrophage THP-1 infected with highly pathogenic H37Rv strain (Rv), H37Rv inactivated strain (Rv-), and non-pathogenic H37Ra (Ra) strains respectively, indicating that host miRNAs may be involved in the interaction of Mycobacterium tuberculosis and host. Meanwhile, 10 types of miRNAs showed significant differences in cells infected with pathogenic and non-pathogenic Mycobacterium tuberculosis, suggesting that host miRNAs may play an important role in the pathogenicity and intracellular survival of Mycobacterium tuberculosis. Further study had found that miR-449a, miR-502-5p, and miR-708 were downregulated in cells infected with Mycobacterium marinum. Overexpression of these three miRNAs displayed the significant inhibitory effect on the growth of Mycobacterium marinum, indicating that miRNAs played a pivotal role in the interaction between the host and Mycobacterium marinum. This study provided the new insights into the pathogenesis of Mycobacterium tuberculosis and the treatment of tuberculosis.

结核分枝杆菌与宿主细胞的相互作用及对宿主的某些信号通路调节与其在巨噬细胞内保持潜伏有关。microRNAs (miRNAs) 可以调控基因表达, 调节机体的许多生物功能, 在细菌感染中发挥调节作用。但是, 宿主的miRNA是否也参与结核分枝杆菌感染尚未深入研究。本研究将致病性结核分枝杆菌H37Rv和低毒株H37Ra感染巨噬细胞, 通过鉴定感染后宿主细胞中miRNA的表达谱, 发现在强致病性H37Rv株(Rv)、H37Rv灭活株(Rv-) 和低致病H37Ra (Ra) 株分别感染人巨噬细胞THP-1中有17种miRNA的表达水平发生显著性变化(P<0.05), 表明宿主miRNA可能参与结核分枝杆菌-宿主的相互作用。同时, 发现了10种miRNA在致病性和非致病性结核分枝杆菌感染的细胞中存在显著差异, 提示宿主miRNA可能在结核分枝杆菌的致病性和细胞内生存中发挥重要作用。进一步研究发现, miR-449a、miR-502-5p和miR-708在海分枝杆菌感染的细胞中下调, 过表达这3种miRNA后的细胞对海分枝杆菌的生长具有明显抑制作用, 表明miRNA在宿主与分枝杆菌的相互作用中发挥重要作用。本研究为结核分枝杆菌致病性机制及治疗结核病方法提供新思路。

结核分枝杆菌与宿主细胞的相互作用及对宿主的某些信号通路调节与其在巨噬细胞内保持潜伏有关。microRNAs (miRNAs) 可以调控基因表达, 调节机体的许多生物功能, 在细菌感染中发挥调节作用。但是, 宿主的miRNA是否也参与结核分枝杆菌感染尚未深入研究。本研究将致病性结核分枝杆菌H37Rv和低毒株H37Ra感染巨噬细胞, 通过鉴定感染后宿主细胞中miRNA的表达谱, 发现在强致病性H37Rv株(Rv)、H37Rv灭活株(Rv-) 和低致病H37Ra (Ra) 株分别感染人巨噬细胞THP-1中有17种miRNA的表达水平发生显著性变化(P<0.05), 表明宿主miRNA可能参与结核分枝杆菌-宿主的相互作用。同时, 发现了10种miRNA在致病性和非致病性结核分枝杆菌感染的细胞中存在显著差异, 提示宿主miRNA可能在结核分枝杆菌的致病性和细胞内生存中发挥重要作用。进一步研究发现, miR-449a、miR-502-5p和miR-708在海分枝杆菌感染的细胞中下调, 过表达这3种miRNA后的细胞对海分枝杆菌的生长具有明显抑制作用, 表明miRNA在宿主与分枝杆菌的相互作用中发挥重要作用。本研究为结核分枝杆菌致病性机制及治疗结核病方法提供新思路。

The interaction between Mycobacterium tuberculosis and host, as well as the regulation of some signaling pathways in the host, were involved in pathogen latency in macrophages. microRNAs (miRNAs) regulate the gene expression and biological functions, indicating that miRNAs played a regulatory role in bacterial infections. However, whether the host's miRNAs were also involved in the process of Mycobacterium tuberculosis infection had not been thoroughly studied. This study infected macrophages with pathogenic Mycobacterium tuberculosis strain H37Rv and low virulence strain H37Ra to explore the functional miRNAs. By identifying the expression profile of miRNAs in host cells after infection, the expression of 17 miRNAs significantly changed (P<0.05) in the human macrophage THP-1 infected with highly pathogenic H37Rv strain (Rv), H37Rv inactivated strain (Rv-), and non-pathogenic H37Ra (Ra) strains respectively, indicating that host miRNAs may be involved in the interaction of Mycobacterium tuberculosis and host. Meanwhile, 10 types of miRNAs showed significant differences in cells infected with pathogenic and non-pathogenic Mycobacterium tuberculosis, suggesting that host miRNAs may play an important role in the pathogenicity and intracellular survival of Mycobacterium tuberculosis. Further study had found that miR-449a, miR-502-5p, and miR-708 were downregulated in cells infected with Mycobacterium marinum. Overexpression of these three miRNAs displayed the significant inhibitory effect on the growth of Mycobacterium marinum, indicating that miRNAs played a pivotal role in the interaction between the host and Mycobacterium marinum. This study provided the new insights into the pathogenesis of Mycobacterium tuberculosis and the treatment of tuberculosis.

由结核分枝杆菌感染引起的结核病是传播较广、死亡率较高的传染病之一。人体感染结核菌后, 巨噬细胞作为免疫清除的主要效应细胞, 会通过一些机制来杀灭或抑制结核分枝杆菌, 参与机体的免疫保护。如结核分枝杆菌通过胞吞作用进入巨噬细胞内, 形成吞噬体-溶酶体复合物; 通过自身凋亡杀死巨噬细胞胞内的结核分枝杆菌; 产生多种细胞因子如白介素-1、白介素-6、白介素-10、肿瘤坏死因子-α、干扰素-γ等参与并调节免疫过程, 发挥其抗结核分枝杆菌作用[1]。尽管大多数结核分枝杆菌病原体能够被巨噬细胞通过以上机制有效清除, 但仍有部分结核菌能够在胞内长期存活, 处于一种潜伏感染(latent tuberculosis infection) 状态, 即宿主感染结核菌后尚未发病的一种特殊状态[2], 任何造成机体免疫力下降的因素都可以使潜伏期结核菌恢复生长繁殖活力, 导致活动性结核的发生。
结核菌之所以能够在巨噬细胞内保持这种潜伏状态, 与其和宿主细胞的相互作用, 对宿主某些信号通路的调节密不可分。它通过抑制吞噬体与溶酶体融合, 分泌宿主样信号分子干扰宿主信号转导, 利用宿主细胞中某些分子帮助自身躲避巨噬细胞的免疫清除[1]等多种方式来逃避巨噬细胞的杀灭。结核分枝杆菌感染宿主细胞后, 会引起巨噬细胞内环境的改变。将强致病性结核分枝杆菌和弱毒株分别感染巨噬细胞, 强毒株所感染的巨噬细胞凋亡率明显低于弱毒株[3, 4]。这一方面是由于强致病性结核分枝杆菌感染巨噬细胞后, 通过改变细胞内环境, 抑制促凋亡物质释放, 使细胞凋亡受到抑制; 另一方面, 它可以抑制一些蛋白的磷酸化来下调caspases 3活性, 从而抑制细胞凋亡[5, 6]。此外, 结核分枝杆菌感染巨噬细胞后, 还可以通过改变细胞内一些代谢通路来创造适合其生存的环境, 抑制细胞凋亡。
随着研究的深入, RNA干扰技术被逐步应用到结核分枝杆菌致病性的研究中。如通过对宿主基因组siRNA筛选的方法, 鉴定对结核分枝杆菌感染有调节作用的宿主因子, 推测结核杆菌主要通过对宿主因子的调控防止宿主细胞自噬[7]; 利用针对鼠巨噬细胞所有已知激酶和磷酸酶的siRNA, 筛选出多种与结核分枝杆菌生存密切相关的宿主因子[8]。尽管大量的研究表明结核分枝杆菌在宿主细胞内的生存并产生致病性需要调节宿主蛋白的表达, 但其具体机制并不清楚。
microRNAs (miRNAs) 是一种内源性大小约为21~23个碱基的单链非编码小分子RNA, 由具有发夹结构约70~90个碱基大小的单链RNA前体(pri-miRNA) 组成, 在核内由核糖核酸酶Drosha加工处理成为前体miRNA (precursor miRNA, pre-miRNA) 后, 被核输出蛋白exportin 5转运入胞质, 经过Dicer酶加工后成为miRNA, 与RNA诱导沉默复合体(RNA-induced silencing complex, RISC) 结合, 并与靶标mRNA的3′非编码区互补结合, 降解mRNA, 或者通过改变靶mRNA上核糖密度或特异性降解新合成的多肽来抑制mRNA翻译[9]
研究表明, miRNAs可以参与基因表达调控, 进而调节机体许多生物功能。由于miRNA序列、结构、丰度和表达方式的多样性, 其可能作为蛋白质编码mRNA的强有力调节因子, 进而影响细胞内有生物学功能的蛋白质表达, 从而实现对细胞生长、增殖、分化和凋亡等生命过程调控[10], 因此对miRNAs功能研究一直是热点问题。
研究发现, 宿主miRNA也参与到病原体感染过程, 如与丙型肝炎病毒相关的miR122参与肝脂代谢, 并发挥调控功能[11]; miR-200b和miR-429调控Epstein-Barr virus的潜伏及溶解复制[12]; 受HIV病毒Vpr调节的miR-34a/b[13], 抑制HIV病毒Nef蛋白表达和长末端重复转录, 从而抑制Nef功能的miR-N367[14]; 鼠伤寒沙门氏菌感染后, miR-21、miR-146和miR-155对宿主巨噬细胞内环境的调节表明, 在细菌感染过程中, LPS和NF-κB的作用与miRNA调节是密不可分的[15]
上述研究表明, miRNA无论是在病毒还是在细菌感染中均发挥调节作用, 但是, 宿主miRNA是否也参与结核分枝杆菌感染过程尚未深入研究。因此, 本研究将致病性结核分枝杆菌H37Rv和低毒株H37Ra感染巨噬细胞, 通过鉴定感染后宿主细胞的miRNA表达谱, 找出在结核分枝杆菌的潜伏感染和持留过程中发挥重要调节作用的miRNA, 并对其进行功能验证, 为结核分枝杆菌致病性机制及结核病治疗提供新思路。
菌株及试剂    结核分枝杆菌H37Rv及结核分枝杆菌H37Ra由北京市结核病胸部肿瘤研究所惠赠; 7H9液体培养基(用于分枝杆菌的培养) 购自BD公司; mirVanaTM miRNA isolation kit、nuclease-free water购自Ambion公司; miScript reverse transcription kit、miScript SYBR green PCR kit购自QIAGEN公司; 佛波酯(phorbol 12-myristate 13-acetate, PMA) 购自Sigma-Adrich公司, 配成l mg·mL-1贮存液, -20 ℃避光保存。
THP-1细胞培养及结核分枝杆菌感染    将2×105个THP-1细胞接种于6孔板, 加入含10% FBS的RPMI 1640培养基, 37 ℃、5% CO2条件下培养, 给予40 ng·mL-1 PMA刺激24 h, 促进悬浮THP-1的成熟分化和贴壁。弃培养基, 用PBS洗涤3次, 以去除未贴壁细胞。将结核分枝杆菌H37Rv、H37Ra株及H37Rv灭活株超声分散后, 使用含0.05%Tween-80的灭菌PBS洗涤2次, 离心弃上清, 用无抗生素的PMI1640重悬, 将菌浓度调至107 CFU·mL-1。按细菌与细胞个数(MOI) 比约10∶l, 分别将上述3种菌株加入到THP-1细胞中, 37 ℃孵育3 h, 弃去培养基, PBS洗涤3次, 加入含200 µg·mL-1庆大霉素培养基作用2 h, 以除去未感染的菌, 弃上清, PBS洗涤3次, 此时作为感染的初始时间。感染24 h后, 收集6孔板中细胞。
THP-1细胞中总RNA提取    将上述收集到的THP-1细胞, 使用mirVanaTM miRNA isolation kit进行总RNA提取, 具体提取步骤见试剂盒说明书。将提取到的RNA进行浓度测定后, -80 ℃保存。基于real-time PCR原理, 在384孔板上对Sanger数据库中人的所有miRNA进行精确定量。实验流程: 组织或细胞样品的RNA提取→RNA荧光标记→芯片杂交/洗涤→图像采集→荧光定量聚合酶链反应(qRT-PCR) 法→结果验证。
qRT-PCR对microRNA在宿主细胞表达验证    用qRT-PCR检测感染分枝杆菌的THP-1细胞中miR-449a、miR-502-5p和miR-708相对表达水平。miRNA与mRNA不同, 在它的末端不包含polyA。因此, 在反转录进行过程中, 首先要将其末端加上polyA, 然后使用oligo-dT引物和随机引物进行逆转录。随后以通用引物和特异引物分别为上下游引物进行qRT-PCR扩增。采用2-ΔΔCt法对目的基因相对表达水平进行计算。
过表达miR-449a、miR-502-5p和miR-708细胞系构建    设计带有EcoRI和BamHI酶切位点的引物扩增miR-449a、miR-502-5p和miR-708的片段, 切胶回收后连入经双酶切(EcoRI和BamHI) 的pCDH-CMV-MCS-EF1-CopRFP载体。转化大肠杆菌, 挑取阳性克隆并进行测序。测序正确代表pCDH-CMV-MCS-EF1-CopRFP-miR-449a、pCDH-CMV-MCS-EF1-CopRFP-miR-502-5p、pCDH-CMV-MCS-EF1-CopRFP-miR-708的质粒构建成功。以上含有正确质粒的大肠杆菌经扩大培养后用质粒抽提试剂盒抽提质粒, 用于细胞转染。提取的质粒用核酸测定仪测定浓度, 并进行琼脂糖凝胶电泳, 鉴定质粒大小、完整性和有无RNA, 质粒-20 ℃保存备用。将构建成功的质粒使用LipofectamineTM 2000转染试剂(10 nmol·L-1) 递送至THP-1细胞, 转染后48 h, 准备细胞用于分析。
海分枝杆菌感染巨噬细胞    按感染复数为2的比例, 使海分枝杆菌感染THP-1-miR-449a、THP-1-miR-502-5p和THP-1-miR-708细胞, 37 ℃饱和湿度、5% CO2条件下感染3 h, 将24孔板中的一孔细胞用胰酶消化后, 取细胞悬液制成细胞涂片, 用预冷的甲醇-醋酸(3∶l) 于4 ℃固定30 min, 做抗酸染色。显微镜下观察到细胞内有抗酸染色阳性的海分枝杆菌后, 吸出原有细胞培养液, 用无菌PBS洗涤细胞3次, 更换含有200 μg·mL-1庆大霉素的新鲜培养液, 以除去未被吞噬的海分枝杆菌, 无菌PBS洗涤细胞3次, 加入1 mL含不同药物浓度的DMEM完全培养基, 置37 ℃、5% CO2培养箱中培养。
统计学处理    采用SPSS26.0软件分析数据。符合正态分布的计量资料以x ± s表示, 多组间比较采用单因素方差分析; 对于独立样本的均值, 两组间比较采用student's t test检验, P<0.05表示在统计学上具有意义。
为了研究宿主miRNAs在结核分枝杆菌与宿主巨噬细胞相互作用中是否发挥作用, 以及结核杆菌致病性、细胞内持留、免疫逃避过程是否与miRNAs调节作用有关, 本研究将强致病H37Rv株(Rv)、H37Rv灭活株(Rv-) 和低致病H37Ra (Ra) 株分别感染人巨噬细胞THP-1, 然后利用miRNA芯片技术, 研究被感染巨噬细胞中754条人源的microRNAs的表达谱。通过将整个miRNAs表达谱以THP-1为对照进行统计学分析, 选取变化趋势差异2倍以上(P<0.05) 的microRNA分子作为候选分子, 找到强毒株H37Rv株(Rv)、H37Rv灭活株(Rv-) 和弱毒株37Ra (Ra) 感染THP-1后, Ra变化的miRNA 5条(hsa-miR-143、hsa-miR-224、hsa-miR580、hsa-miR590-3p、hsa-miR-99b), Rv-表达变化的4条(hsa-miR-519b-3p、hsa-miR-576-3p、hsa-miR-582-5p、hsa-let-7e#), Rv变化的3条(hsa-miR-519-3p、hsa-miR-576-3p、hsa-miR-582-5p)。在这3株中都同时发生变化的miRNA只有1条(hsa-miR-146), Rv与Ra共同变化的3条(hsa-miR-151-5p、hsa-miR-196-1*、hsa-miR-776), Rv与Rv-共同变化的1条(hsa-miR-213), 未找到Rv-与Ra均变化的miRNA (表 1)。以THP-1为对照, miRNA的表达差异提示宿主miRNA参与了病原(结核分枝杆菌) 与宿主的相互作用。
既然miRNAs可能会参与结核分枝杆菌与宿主的相互作用, 本研究又进一步对miRNAs在致病性和非致病性结核分枝杆菌感染的细胞中表达情况进行分析, 结果发现, 致病性和非致病性结核分枝杆菌感染细胞中miRNA表达存在显著差异, 提示宿主miRNA可能在结核分枝杆菌的致病性和细胞内生存中发挥重要作用。为了对芯片结果的可靠性进行验证, 本研究选取相对非致病性的结核分枝杆菌Ra而言, 致病性结核分枝杆菌Rv表达差异大于2倍的miRNAs, 进行qRT-PCR (图 1), 以排除芯片分析的假阳性结果。结果显示, 无论是用miRNAs芯片筛选还是qRT-PCR的方法验证, has-miR-449a、has-miR-502-5p和has-miR-708表达量都一致下调。因此, 本研究选取has-miR-449a、has-miR-502-5p和has-miR-708进行后续研究。
为了探究miR-449a、miR-502-5p和miR-708的功能, 将miR-449a、miR-502-5p和miR-708转染THP-1细胞以达到过表达目的。结果显示, 转染miR-449a、miR-502-5p和miR-708后, 3种miRNA在细胞中显著上调, 其中miR-449a的表达水平最高, miR-502-5p表达水平最低(图 2)。最终得到miR-449a、miR-502-5p和miR-708过表达THP-1细胞系。
结核病产生耐药或者难以治疗的原因是因为结核菌在细胞内长期存活, 在机体免疫力下降的情况下, 潜伏感染菌转化成活性致病菌。本研究在稳定表达miR-449a、miR-502-5p和miR-708的THP-1细胞中感染海分枝杆菌927, 对海分枝杆菌927在细胞中存活情况分析发现, 稳定表达miR-449a、miR-502-5p和miR-708的THP-1中海分枝杆菌927存活显著被抑制(P<0.01, P<0.001), 其中, 稳定表达miR-708的THP-1中海分枝杆菌927存活被抑制最明显(图 3A)。为进一步验证上述结果的可靠性, 本研究用海分枝杆菌M株同样感染稳定过表达miR-449a、miR-502-5p和miR-708的细胞, 与海分枝杆菌927感染细胞的结果相同, 稳定表达miR-449a、miR-502-5p和miR-708的THP-1中海分枝杆菌M株存活也明显减少(P<0.001, 图 3B), 这些结果表明, miR-449a、miR-502-5p和miR-708在海分枝杆菌927和M株感染过程中影响分枝杆菌在巨噬细胞内的存活, miRNAs参与了宿主与分枝杆菌的相互作用过程。
miRNA参与基因表达调控, 进而调节机体许多生物学功能。在结核分枝杆菌感染的过程中, 宿主miRNA是否参与病原感染与机体防御这一病原宿主相互作用, 目前尚不清楚。本研究利用miRNA芯片技术, 发现在强致病性H37Rv株(Rv)、H37Rv灭活株(Rv-) 和低致病H37Ra (Ra) 株分别感染人巨噬细胞THP-1后, 17种miRNA的表达水平发生显著性变化(P<0.05), 这一结果表明宿主miRNA参与了病原(结核分枝杆菌) 宿主的相互作用, 即病原的感染机制和机体的保护机制。同时, 本研究还发现10种miRNA在致病性和非致病性结核分枝杆菌感染细胞中存在显著差异, 提示宿主miRNA还可能在结核分枝杆菌致病性和细胞内生存中发挥重要作用, 病原可能通过调节宿主miRNA, 实现对细胞生长、增殖、分化和凋亡等生命过程调控, 使结核分枝杆菌在宿主细胞中逃避巨噬细胞清除、潜伏、持留, 发挥致病性。
初步分析上述结果发现, miR-146a在3种病原菌感染的情况下都明显下调, 表明miR-146a的改变与结核分枝杆菌的生长无关, 而与结核杆菌菌体成分相关。值得关注的是, 通过对结核病患者和健康人群的研究中发现, miR-146a前体的多态性与结核病的易感相关, 这进一步提示miR-146a在结核感染过程中可能发挥重要功能[16]
已有研究表明, miR-146a在适应性免疫和天然免疫中都起着重要作用。通过对T细胞中miRNAs表达研究分析发现, 在鼠Th1和Th2细胞中只有少数miRNA表达不同, 而miR-146a在这两种细胞中表达存在着显著差异[17]。进一步对人T细胞研究发现, miR-146a在天然T淋巴细胞中表达量很少, 但是在记忆性T淋巴细胞中却丰富表达, 并且它的上调依赖于NF-κB诱导和T细胞抗原受体(TCR) 刺激[18, 19]。巨噬细胞对于感染产生炎性反应与一些miRNA上调有关, 如miR-155、miR-146、miR-147、miR-21和miR-9[20-23]。在NF-κB途径中, LPS刺激会引起miR-146a表达, 以及靶向作用于TNF受体相关因子6 (TRAF6) 和IL-1受体相关激酶-1 (IRAK1), 它们通过负性调节机制在细胞信号转导中发挥着重要作用[18]。此外, miR-146a不但与单核细胞内毒素耐受有关, 并且参与TNF-α产生调节和功能发挥[24]。miR-146a除了与适应性免疫和天然免疫有关, 还与病毒感染和肿瘤发生时在巨噬细胞中调节作用有关。研究表明, 疱疹性病毒(VSV) 感染, 会上调miR-146a表达, 通过下调TRAF6、IRAK1和IRAK2抑制Ⅰ型干扰素产生, 从而刺激VSV在巨噬细胞中复制[25]。当在乳腺癌细胞中过表达miR-146a后, 会引起IRAK1和TRAF6表达减少, 进而使NF-κB活性降低。另外, 在宫颈癌细胞中也发现miR-146a表达增加[26]。尽管, miR-146a与细胞增殖相关具体机制还不是很清楚, 但是其表达变化与癌症发生是密切相关的。
已有研究表明, miR-449a低表达水平与晚期肿瘤分期、转移相关[27]。乳腺癌中miR-708在淋巴结转移和远端转移表达减少, 表明miR-708与肿瘤转移有关[28]。本研究发现, miR-449a、miR-502-5p和miR-708在海分枝杆菌感染细胞中下调, 过表达这3种miRNA后, 细胞对海分枝杆菌生长具有明显抑制作用, 表明miR-449a、miR-502-5p和miR-708在宿主与分枝杆菌相互作用中发挥重要作用。本研究通过对结核分枝杆菌致病性相关的宿主内miRNA分子的分析, 研究宿主与病原相互作用, 为研究结核分枝杆菌致病性机制并为基因治疗结核病及抗结核药物研发提供新思路。
