Orodispersible films (oral dispersible films), a novel form of oral solid dosage forms, are widely used for patients with dysphagia and those with uncontrollable autonomic behavior. In this study, suvorexant orodispersible film was prepared by hot melt extrusion technology, and the disintegration time, mechanical properties, in vitro dissolution and pharmacokinetics were evaluated, compared with other orodispersible films and commercially available tablets Belsomra. All experiments were approved by the Committee on the Management and Use of Laboratory Animals of Academy of Military Medical Sciences (IACUC-DWZX-2024-504). The results showed that the dissolution rate of suvorexant orodispersible film was faster and the mechanical strength was better than that of other commercially available orodispersible film, which could meet the needs of storage and transportation. Differential scanning calorimetry and X-ray diffraction results showed that suvorexant was dispersed within the orodispersible film in an amorphous state. The in vitro dissolution of the film was observed to be four times faster than that of the commercial tablets, achieving complete dissolution within five minutes. Pharmacokinetic evaluations in Beagle dogs revealed that the self-formulated orodispersible film exhibited no significant differences in the area under the blood concentration-time curve when compared with Belsomra. However, the film showed a faster onset of action, with a peak time that was twice as rapid, and a maximum blood concentration that was twice as high as that of Belsomra. Leveraging hot melt extrusion technology, the suvorexant orodispersible film offers a straightforward, continuous production process with consistent quality. It serves as an excellent platform for the development of solvent-free film preparations tailored for patients with special needs.

口溶膜是一种新型的口服固体制剂, 其在吞咽困难、自主行为不受控制等患者群体中广泛应用。本研究采用热熔挤出技术制备了苏沃雷生口溶膜, 并通过崩解时限、机械性能、体外溶出及体内药代动力学等参数进行体内外评价, 对比了已上市其他品种口溶膜及市售片剂Belsomra。所有动物实验经军事医学研究院实验动物管理与使用委员会批准(批准号: IACUC-DWZX-2024-504)。结果显示, 苏沃雷生口溶膜崩解速度快, 机械强度较市售其他品种口溶膜更优, 可满足储存与运输需求。差示扫描量热法与X射线衍射法的结果表明, 苏沃雷生以无定形状态分散在口溶膜中。其体外溶出较市售片剂快4倍, 5 min即完全溶出; 比格犬体内药代动力学研究表明, 自制口溶膜与Belsomra相比血药浓度时间曲线下面积无统计学差异, 但起效更快, 其达峰时间较Belsomra快1倍, 最大血药浓度是Belsomra的2倍。基于热熔挤出技术制备的苏沃雷生口溶膜工艺简单、可连续化生产、质量稳定, 可作为开发针对特殊患者的无溶剂薄膜制剂的平台。

口溶膜是一种新型的口服固体制剂, 其在吞咽困难、自主行为不受控制等患者群体中广泛应用。本研究采用热熔挤出技术制备了苏沃雷生口溶膜, 并通过崩解时限、机械性能、体外溶出及体内药代动力学等参数进行体内外评价, 对比了已上市其他品种口溶膜及市售片剂Belsomra。所有动物实验经军事医学研究院实验动物管理与使用委员会批准(批准号: IACUC-DWZX-2024-504)。结果显示, 苏沃雷生口溶膜崩解速度快, 机械强度较市售其他品种口溶膜更优, 可满足储存与运输需求。差示扫描量热法与X射线衍射法的结果表明, 苏沃雷生以无定形状态分散在口溶膜中。其体外溶出较市售片剂快4倍, 5 min即完全溶出; 比格犬体内药代动力学研究表明, 自制口溶膜与Belsomra相比血药浓度时间曲线下面积无统计学差异, 但起效更快, 其达峰时间较Belsomra快1倍, 最大血药浓度是Belsomra的2倍。基于热熔挤出技术制备的苏沃雷生口溶膜工艺简单、可连续化生产、质量稳定, 可作为开发针对特殊患者的无溶剂薄膜制剂的平台。

Orodispersible films (oral dispersible films), a novel form of oral solid dosage forms, are widely used for patients with dysphagia and those with uncontrollable autonomic behavior. In this study, suvorexant orodispersible film was prepared by hot melt extrusion technology, and the disintegration time, mechanical properties, in vitro dissolution and pharmacokinetics were evaluated, compared with other orodispersible films and commercially available tablets Belsomra. All experiments were approved by the Committee on the Management and Use of Laboratory Animals of Academy of Military Medical Sciences (IACUC-DWZX-2024-504). The results showed that the dissolution rate of suvorexant orodispersible film was faster and the mechanical strength was better than that of other commercially available orodispersible film, which could meet the needs of storage and transportation. Differential scanning calorimetry and X-ray diffraction results showed that suvorexant was dispersed within the orodispersible film in an amorphous state. The in vitro dissolution of the film was observed to be four times faster than that of the commercial tablets, achieving complete dissolution within five minutes. Pharmacokinetic evaluations in Beagle dogs revealed that the self-formulated orodispersible film exhibited no significant differences in the area under the blood concentration-time curve when compared with Belsomra. However, the film showed a faster onset of action, with a peak time that was twice as rapid, and a maximum blood concentration that was twice as high as that of Belsomra. Leveraging hot melt extrusion technology, the suvorexant orodispersible film offers a straightforward, continuous production process with consistent quality. It serves as an excellent platform for the development of solvent-free film preparations tailored for patients with special needs.

口服给药途径简单便捷, 患者依从性好[1], 但是对于老年人和自主行为不受控制的精神疾病患者等吞咽困难的人和儿童来说, 服用片剂与胶囊等固体口服制剂非常困难并极具挑战[2, 3], 而液体口服制剂使用前可能需要摇匀, 用量杯量取, 并且面临物理、化学、微生物稳定性等问题[4]。为了解决上述传统口服制剂面临的挑战与问题, 药剂学家开发了口溶膜。口溶膜一般由主药成分、成膜材料、崩解剂和矫味剂等组成, 其具有一定的机械强度, 易于储存与运输, 同时给药方便, 不需要用水吞服且在口腔内黏附后不易被吐出。这些特性使得口溶膜具有良好的患者依从性[5]。目前, 全球市场上已有多款口溶膜产品成功上市[6]
目前, 口溶膜的制备方法主要有溶剂浇铸法、热熔挤出法、静电纺丝法、静电喷雾沉积技术、3D打印技术等方法[7]。口溶膜制备应用最广泛的方法为溶剂浇铸法, 其工艺简单但是步骤较多, 同时其大量使用溶剂, 需要较长的时间干燥去除溶剂, 若使用有机溶剂还涉及到溶剂残留、环保与安全问题[8]。热熔挤出技术制备口溶膜仅需将物料混匀加入挤出机内, 物料经挤出机加热熔融混合, 通过模具直接一步成膜[9]。其具有多种优势, 制造过程耗时短、可连续化生产, 无需使用水或有机溶剂, 熔融状态下药物和辅料分散均匀, 薄膜含量均一度高, 生产过程中的参数控制更为方便, 减小原辅料的浪费, 也可以达到提升溶解度作用。
苏沃雷生是全球首个以食欲素受体为作用位点的失眠治疗药物[10], 其相对苯二氮䓬类药物不良反应小, 滥用潜力低, 停药后无戒断症状[11-13]。目前苏沃雷生仅有片剂Belsomra上市, 这对于老年人、精神疾病患者和吞咽困难的人来说服用不便。苏沃雷生制备成口溶膜后, 给药优势明显: ①无需用水送服, 使用方便; ②在服用后可快速溶解, 释药迅速, 可快速起效; ③体积小, 质量轻, 便于携带。本研究采用热熔挤出技术制备苏沃雷生口溶膜, 评价其外观、机械性能、溶出曲线等, 同时与市售其他品种口溶膜比较机械性能, 与市售片剂比较溶出曲线, 最后进行药代动力学研究并与市售片剂进行比较, 评价自制口溶膜的制剂特性。
实验动物    比格犬购于北京玛斯生物技术有限公司, 实验动物合格证号: SCXK (京) 2016-0001。动物实验经军事医学研究院动物伦理与使用委员会批准, 且实验均按照相关指导原则和规定进行, 伦理审批编号为IACUC-DWZX-2024-504。
试剂    苏沃雷生(批号: SV20090001, Hetero Drugs Limited); 聚氧乙烯(polyethylene oxide, PEO, 批号: INR537885, 美国Dupont公司); 共聚维酮(KollidonVA64, VA64, 批号: 19969836WO)、泊洛沙姆188 (poloxamer 188, P188, 批号: GND30021WT) (德国BASF公司); 纽甜(批号: HT22032413, 武汉华甜生物科技有限公司); 苏沃雷生片(10 mg, 批号: WO39327, 美国MSD公司); 奥氮平口溶膜(5 mg, 批号: 1C0011Q06, 齐鲁制药有限公司); 其余试剂均为分析纯, 实验用水为纯化水。
仪器    热熔挤出机(型号: Process 11)、高效液相色谱仪(型号: Vanquish) (美国Thermo Scientific公司); 电子天平(型号: BS323S)、电子天平(型号: BT25S) (德国Sartorius公司); 螺旋测微仪(型号: 211-101, 东莞市三量精密量仪有限公司); 质构仪(型号: TA-Touch, 上海保圣有限公司); 崩解仪(型号: ZB-ID, 天津鑫洲科技有限公司); 溶出仪(型号: RC806D, 天津天大天发科技有限公司); 热重分析仪(型号: TGA55, 美国TA公司); 差示扫描量热仪(型号: HS-DSC-101, 上海和晟仪器科技有限公司); X射线衍射仪(型号: D/MAX-2600X, 日本Rigaku公司); 液相色谱-质谱联用仪(型号: 6460, 美国Agilent公司)。
热重分析    使用热重分析仪对苏沃雷生原料药与各辅料的热稳定性进行考察。称取适量样品于铝制样品盘中, 氮气流速60 mL·min-1, 升温速率10 ℃·min-1, 升温范围30~250 ℃。
热熔挤出制备口溶膜    按照表 1中处方(F1~F6) 进行称量与混合, 加入喂料器中, 设定热熔挤出机加工温度160 ℃, 螺杆转速150 r·min-1, 设定喂料器喂料速度20%, 收集薄膜, 储存在聚酯/铝/聚乙烯复合袋中, 待测。
苏沃雷生口溶膜表征
厚度   取口溶膜在6个不同位置测量其厚度, 其平均值即为口溶膜的厚度。
机械性能   将获得的口溶膜裁剪为2×5 cm2大小, 夹在质构仪上下两个夹具中, 设定测试速度为1 mm·s-1, 进行测定, 记录应力-应变曲线, 每个样品重复3次, 根据公式(1) 计算抗张强度(TS), 公式(2) 计算断裂伸长率(E)。

耐折次数   取裁剪好的口溶膜一端固定在桌面上, 用手反复折叠直至口溶膜断裂, 记录断裂时的折叠次数, 每个样品重复3次[14]
崩解时限    由于目前尚无检测口溶膜崩解时限的标准方法, 采用体内外相关性较好的药典崩解仪法进行测试[15]。使用符合中国药典2020版四部通则0921 (崩解时限检查法) 标准的崩解仪, 将口溶膜裁剪成1×1 cm2大小, 用回形针夹住, 将其投入崩解篮中, 记录口溶膜崩散的时间, 每个样品重复测量3次。
含量    采用高效液相色谱法按下列色谱条件测定苏沃雷生固体分散体的含量。色谱条件: 流动相为乙腈∶0.1 mol·L-1高氯酸钠溶液(48∶52), 流速1.0 mL·min-1, 检测波长254 nm, 进样量10 μL, 柱温40 ℃。
体外溶出实验    参照《中国药典》2020年版四部通则0931溶出度与释放度测定法第二法(桨法) 测定6个处方制备的口溶膜、苏沃雷生原料药与F2物理混合物(F2 physical mixture, PM) 的溶出, 水浴温度37 ± 0.1 ℃, 转速75 r·min-1, 以纯化水(非漏槽条件) 和0.4%十二烷基硫酸钠(SDS) (漏槽条件) 溶液为溶出介质, 分别于5、10、15、20、30、45、60、90 min取液5 mL并补充同体积和温度的溶出介质, 取出液体经0.22 μm滤膜过滤, 取续滤液, 作为供试品溶液。另精密称取苏沃雷生原料药适量, 用甲醇溶解并用流动相稀释定容为20 μg·mL-1的溶液作为对照品溶液。取两种溶液按照含量项下色谱条件测定, 按外标法以峰面积计算含量。
X射线衍射法(X-ray diffraction, XRD)    使用X射线衍射仪, 以Cu-Ka作为辐射源, 在5~90°的2θ范围内, 扫描速度2°·min-1检测苏沃雷生原料药、F2未载药物理混合物(F2 blank physical mixture, BPM)、未载药F2 (F2 blank film, BF2)、PM与各处方苏沃雷生口溶膜的晶型状态。
傅里叶变换红外光谱法(Fourier transform infrared spectroscopy, FTIR)    使用红外光谱仪, 采用KBr压片法, 扫描范围400~4 000 cm-1, 分辨率4 cm-1, 对苏沃雷生原料药、PEO、VA64、P188、PM、BF2、F2进行考察。
药代动力学
色谱条件   色谱柱Kinetex C18柱(50 mm × 3 mm, 2.6 μm); 流动相A (乙腈含0.1%甲酸) 和B (水含0.1%甲酸) 梯度洗脱: 40% A (0 min), 40%~80% A (0~3 min), 80% A (3~4 min); 流速: 0.4 mL·min-1; 柱温: 35 ℃; 进样量: 5 μL。
质谱条件   离子源: 电喷雾离子化源, 正离子方式检测; 扫描方式: 多反应离子监测(MRM); 雾化器温度300 ℃; 雾化气流速10 L·min-1; 苏沃雷生与内标咪达唑仑的监测离子对分别为m/z 451.2→186.0和m/z 326.0→291.0。
采用双周期双交叉试验, 清洗期为5天。6只比格犬随机分为2个实验组, 每组3只, 分别给予自制口溶膜和市售片剂, 给药剂量为10 mg。口溶膜服药方法: 将口溶膜置于比格犬舌上, 用少量生理盐水润湿, 握住狗嘴约5 min, 待其全部溶解咽下即可。给药后0.25、0.5、0.75、1、1.5、2、2.5、3、4、6、8、10、12、24 h于前肢静脉取血0.5 mL于肝素锂抗凝管中。采集的血样立即4 000 r·min-1离心10 min, 分离血浆, -80 ℃保存。使用LC-MS/MS测定自制口溶膜与市售片剂两组的血药浓度, 绘制药时曲线, 使用Phoenix软件处理数据得到药代动力学参数, 并使用SPSS软件对两组数据进行Mann-Whitney U检验。
热熔挤出制备口溶膜时, 原辅料暴露在高温下有可能会发生降解, 在进行制备口溶膜之前, 有必要对原辅料的热稳定性进行判断。TGA结果如图 1所示, VA64和纽甜在100 ℃以下时表现出重量损失但 < 5%, 这是由于物料中的水分挥发造成的。纽甜在约150 ℃时开始大量的重量损失, 这期间主要为纽甜脱脂化降解产生脱脂纽甜, 其仅会导致甜度降低, 对于安全性无影响。其他原辅料在本实验加工温度下均是热稳定的, 适合进行热熔挤出工艺操作。
本研究采用带有两段混合区的螺杆组成, 以保证足够的机械能引入, 使物料充分混匀。同时, 为了将加工过程中产生的水汽排出, 在挤出机盖板最后一个区域设置了排气孔, 以确保制备的口溶膜不会产生气泡与孔洞。热熔挤出制备口溶膜过程中的熔体压力与扭矩如表 2所示, 各处方口溶膜加工过程中扭矩在20%~27%之间, 在合适的加工区间范围内。同时, 随着处方中药物与增塑剂P188的加入, 扭矩会下降, 加入量越多, 扭矩下降得越多, 使得更易加工。这是因为药物与P188插入到成膜聚合物的链中, 增加了成膜聚合物的自由体积, 降低了熔体黏度。最终各处方口溶膜均成功制备, 其外观光滑平整, 呈淡黄色半透明状。
对所有自制口溶膜与已上市的口溶膜进行了厚度、崩解时限与机械强度的测定, 结果见表 3。制备的口溶膜厚度均在100 μm左右, 其崩解时间短, 均在60 s以内, 与已上市奥氮平口溶膜崩解时限相当。但由于苏沃雷生为难溶性药物, 随着苏沃雷生添加增多, F1、F2、F3的崩解时间逐渐增加。同时, 随着P188的加入, F4、F5、F2、F6的崩解时间逐渐缩短, 这可能是因为P188作为表面活性剂的加入使口溶膜整体亲水性增强, 更易润湿。各处方苏沃雷生口溶膜的耐折次数均较已上市奥氮平口溶膜多, 这表明其柔韧性良好。自制口溶膜F1、F2、F4、F5的抗拉强度均大于已上市奥氮平口溶膜, 但是F3与F6中因为有更多的苏沃雷生与P188这种相对小分子的物质加入口溶膜中, 插入成膜高分子聚合物链间, 导致了更低的抗拉强度。这说明口溶膜中小分子物质不能加入过多, 否则会导致机械强度的下降。自制口溶膜的断裂伸长率均小于已上市奥氮平口溶膜, 这说明自制口溶膜纵向延展性小。但是自制口溶膜的柔韧性与抗拉强度均较强, 可以满足储存与运输的需求。
溶出结果如图 2所示, F1、F2、F6在水中表现为相似的溶出曲线; 而F3前期溶出较慢, 这可能是由于F3载药量较大, 导致口溶膜润湿铺展效果较差, 药物溶出较慢; 同时F4、F5溶出也较慢, 且F4较F5溶出更慢, 这说明P188作为表面活性剂的加入增加了口溶膜的亲水性, 起到加快溶出的效果。同时, 6个处方的口溶膜相对苏沃雷生原料药与口溶膜物理混合物溶出速率与溶出度均有所提高, 这说明经热熔挤出加工后口溶膜中苏沃雷生可能转变为无定形状态增加了苏沃雷生的溶解度, 并且PEO、P188等亲水性辅料的加入可能也提高了其溶出速率与溶出度。
在漏槽条件介质0.4% SDS溶液中, F2相比于市售片剂溶出更快, 5 min即可完全溶出, 而市售片剂20 min才可完全溶出(图 3)。两种制剂溶出速率的差别可能与其崩解相关, F2由于其亲水性辅料与表面活性剂P188的加入约10 s即可完全崩解, 而市售片剂Belsomra由于是固体分散体片剂依赖溶蚀型崩解, 崩解较为缓慢约12 min才可完全崩解。F2更快的崩解与溶出时间可能意味着苏沃雷生可以被更快的吸收, 进而更快的起效。
XRD结果显示, 苏沃雷生原料药在17.5°、20.9°等2θ角有明显的吸收峰, 且各辅料吸收峰对其无影响。各口溶膜XRD曲线均未出现以上苏沃雷生原料药吸收峰, 表明苏沃雷生以无定形形式分散在口溶膜中(图 4)。苏沃雷生晶型转化为无定形状态的原因是热熔挤出制备口溶膜过程中输入了大量的机械能与热能。
采用傅里叶变换红外光谱探究药物与其他辅料的相互作用(图 5)。在苏沃雷生的红外光谱图中, 1 640 cm-1对应其羰基的伸缩振动峰, PEO谱图中包含氢键供体羟基峰3 421 cm-1。而在形成口溶膜后, PEO的酚羟基峰消失了, 苏沃雷生中羰基峰从1 640 cm-1蓝移至1 635 cm-1。说明口溶膜中苏沃雷生的羰基作为氢键受体, PEO中羟基作为氢键供体, 它们之间形成了氢键。苏沃雷生与PEO的氢键作用可能对于口溶膜中苏沃雷生的无定形状态起稳定作用。
自制口溶膜F2与市售片剂的药代动力学曲线如图 6所示, 选择处方F2与市售片剂作为对照进行对比研究, 确定被试处方口溶膜的药代动力学特征如表 4所示。结果表明, 自制口溶膜F2和市售片剂Belsomra的达到峰值血药浓度的时间(time to peak plasma concentration, Tmax) 分别为0.54和1.04 h。F2和市售片剂的峰值血药浓度(peak plasma concentration, Cmax) 分别为1 313.02 ± 353.36和779.62 ± 179.80 ng·mL-1, F2组Cmax显著高于市售片剂组, Tmax显著小于市售片剂组(P < 0.05), F2较高的Cmax和较低Tmax表明F2和市售片剂的吸收速率存在差异, 结合F2和市售片剂的溶出结果, 可能是由于F2迅速崩解溶出, 导致其更快到达更高的血药浓度峰值。自制口溶膜F2和市售片剂的药时曲线下面积(area under the curve, AUC0→t) 分别为3 957.78 ± 1 410.17和3 588.52 ± 2 162.04 ng·mL-1·h, 从平均值上看F2组AUC0→t略高于Belsomra, 但统计学差异不显著。同时, 苏沃雷生口溶膜对于安全性及给药剂量的评估需结合后续研究进行确定。并且F2的药动学参数仅在比格犬体内进行了初步研究。需要对其他动物进行进一步深入和广泛的研究, 以阐明可能与临床环境相关的关键药代动力学参数。药代动力学结果表明, 口溶膜可以达到和片剂一样的AUC, 但是由于药物吸收部位与口溶膜本身快速崩解与溶出特性的影响, 口溶膜起效时间可能区别于片剂。
目前应用最广泛的口溶膜制备方法为溶液浇铸法, 但是其涉及较长时间的溶剂干燥去除, 同时若使用有机溶剂还涉及安全与环保问题。不同于溶液浇铸法, 本研究使用热熔挤出连续制造技术, 直接一步成膜, 制备的口溶膜厚度薄、无气泡、均匀且美观。由于处方中亲水性成膜材料的存在和厚度薄, 薄膜表现出迅速的崩解和溶出。其中部分处方的力学特性较已上市奥氮平口溶膜更强, 可满足储存与运输需求。自制口溶膜与市售片剂相比, 体外溶出显著加快, 在比格犬体内有着更快的血药达峰时间与相同的AUC0→t
综上所述, 热熔挤出技术是一种应用前景良好的口溶膜制备技术, 制备的苏沃雷生口溶膜崩解快、机械强度高、生物利用度高, 该技术与制剂可作为无溶剂口溶膜制备平台。
