Heart failure with preserved ejection fraction (HFpEF) accounts for about half of the number of patients with heart failure. In addition to the typical features of heart failure such as myocardial stiffness and diastolic function impairment, the key characteristic of HFpEF is the normal left ventricular ejection fraction, which increases the difficulty of clinical diagnosis. QiShenYiQi Dripping Pills (QSYQ) is a standardized traditional Chinese medicine approved by the China Food and Drug Administration (CFDA), and many clinical studies have demonstrated the efficacy and safety of QSYQ in the treatment of heart failure with reduced ejection fraction, but the role of QSYQ in HFpEF has not been clarified. In this paper, high fat diet (HFD) and drinking water containing N-nitro-L-arginine methyl ester (L-NAME, 0.5 g·L-1, pH=7.4) were used in C57BL/6N male mice to construct the classical HFpEF model (the experiment was approved by the Animal Ethics Committee of Hefei University of Technology, the approval number is HFUT20220921002), and at the 8th week, the mice were dosed with ① empagliflozin, ② low-dose QSYQ (LQ), ③ high-dose QSYQ (HQ), ④ empagliflozin plus low-dose QSYQ (ELQ) for 4 weeks, the body weight of the mice was recorded during the experiment, echocardiography, blood pressure and glucose tolerance were detected and the exercise capacity of mice was evaluated, and pathological and biochemical experiments were used to detect cardiac fibrosis, liver fibrosis and serum biochemical indexes in mice, RNAseq assay was performed with mice heart tissues. The results showed that QSYQ as well as QSYQ+empagliflozin could significantly reduce the body weight, improve diastolic function and hypertension, improve glucose tolerance and enhance exercise ability of HFpEF mice. At the biochemical and molecular levels, QSYQ and QSYQ+empagliflozin can reduce the cross-sectional area of cardiomyocytes and reduce cardiac collagen contents, thereby alleviating myocardial hypertrophy and fibrotic phenotypes, and improve metabolic disorders. The RNAseq results suggest that the function of QSYQ in improving HFpEF may be related to calsequestrin 1. In conclusion, this study shows that QSYQ and QSYQ+empagliflozin can significantly improve HFpEF-related cardiac dysfunction and metabolic disorders, which provides a theoretical basis for the clinical treatment of HFpEF.

射血分数保留型心力衰竭(heart failure with preserved ejection fraction, HFpEF) 约占心衰患者的一半数量, 其特征除了心衰的典型特征如心肌僵硬、心脏舒张功能损伤等, 最主要的特征是左心室射血正常, 因此也给HFpEF的临床诊断增加了难度。芪参益气滴丸(QiShenYiQi Dripping Pills, QSYQ) 是经中国食品药品监督管理局(CFDA) 批准的标准化中药制剂, 许多基础及临床研究均已证明QSYQ在治疗射血分数降低型心力衰竭中的有效性和安全性, 然而QSYQ在HFpEF中的作用机制尚未明确。本文利用经典造模方法即高脂食物(high fat diet, HFD) 和含N-硝基-L-精氨酸甲酯盐酸盐(L-NAME, 0.5 g·L-1, pH=7.4) 的饮用水喂食C57BL/6N雄性小鼠构建HFpEF模型(实验获得合肥工业大学动物伦理委员会批准, 批准号为HFUT20220921002), 在第8周, 小鼠分别给药: ①恩格列净(empagliflozin)、②低剂量QSYQ (LQ)、③高剂量QSYQ (HQ)、④恩格列净联用低剂量QSYQ (ELQ), 连续给药4周, 实验过程中记录小鼠的体重, 实验结束后对小鼠进行超声心动检测、血压检测、葡萄糖耐量检测及评估小鼠运动情况, 利用病理实验和生化实验检测小鼠心脏纤维化、肝脏纤维化及血清生化指标等, 利用RNAseq测序对小鼠心脏进行RNAseq检测。结果表明, QSYQ及联用恩格列净均能够显著降低HFpEF小鼠体重, 改善小鼠心脏舒张功能障碍和高血压, 改善葡萄糖耐量异常并增强小鼠运动能力。在生化和分子水平上, QSYQ能够降低心肌细胞横截面积和减少心脏胶原蛋白含量进而减轻心肌肥大和纤维化表型, 同时改善HFpEF小鼠机体脂质代谢紊乱。进一步地, 小鼠心脏RNAseq结果表明, QSYQ改善HFpEF的功能可能与调控肌钙集蛋白1 (calsequestrin 1, Casq1) 有关。综上, 本研究表明QSYQ能够显著改善HFpEF相关的心脏功能障碍和代谢紊乱, 为HFpEF的临床治疗提供了一定实验数据支持。

射血分数保留型心力衰竭(heart failure with preserved ejection fraction, HFpEF) 约占心衰患者的一半数量, 其特征除了心衰的典型特征如心肌僵硬、心脏舒张功能损伤等, 最主要的特征是左心室射血正常, 因此也给HFpEF的临床诊断增加了难度。芪参益气滴丸(QiShenYiQi Dripping Pills, QSYQ) 是经中国食品药品监督管理局(CFDA) 批准的标准化中药制剂, 许多基础及临床研究均已证明QSYQ在治疗射血分数降低型心力衰竭中的有效性和安全性, 然而QSYQ在HFpEF中的作用机制尚未明确。本文利用经典造模方法即高脂食物(high fat diet, HFD) 和含N-硝基-L-精氨酸甲酯盐酸盐(L-NAME, 0.5 g·L-1, pH=7.4) 的饮用水喂食C57BL/6N雄性小鼠构建HFpEF模型(实验获得合肥工业大学动物伦理委员会批准, 批准号为HFUT20220921002), 在第8周, 小鼠分别给药: ①恩格列净(empagliflozin)、②低剂量QSYQ (LQ)、③高剂量QSYQ (HQ)、④恩格列净联用低剂量QSYQ (ELQ), 连续给药4周, 实验过程中记录小鼠的体重, 实验结束后对小鼠进行超声心动检测、血压检测、葡萄糖耐量检测及评估小鼠运动情况, 利用病理实验和生化实验检测小鼠心脏纤维化、肝脏纤维化及血清生化指标等, 利用RNAseq测序对小鼠心脏进行RNAseq检测。结果表明, QSYQ及联用恩格列净均能够显著降低HFpEF小鼠体重, 改善小鼠心脏舒张功能障碍和高血压, 改善葡萄糖耐量异常并增强小鼠运动能力。在生化和分子水平上, QSYQ能够降低心肌细胞横截面积和减少心脏胶原蛋白含量进而减轻心肌肥大和纤维化表型, 同时改善HFpEF小鼠机体脂质代谢紊乱。进一步地, 小鼠心脏RNAseq结果表明, QSYQ改善HFpEF的功能可能与调控肌钙集蛋白1 (calsequestrin 1, Casq1) 有关。综上, 本研究表明QSYQ能够显著改善HFpEF相关的心脏功能障碍和代谢紊乱, 为HFpEF的临床治疗提供了一定实验数据支持。

Heart failure with preserved ejection fraction (HFpEF) accounts for about half of the number of patients with heart failure. In addition to the typical features of heart failure such as myocardial stiffness and diastolic function impairment, the key characteristic of HFpEF is the normal left ventricular ejection fraction, which increases the difficulty of clinical diagnosis. QiShenYiQi Dripping Pills (QSYQ) is a standardized traditional Chinese medicine approved by the China Food and Drug Administration (CFDA), and many clinical studies have demonstrated the efficacy and safety of QSYQ in the treatment of heart failure with reduced ejection fraction, but the role of QSYQ in HFpEF has not been clarified. In this paper, high fat diet (HFD) and drinking water containing N-nitro-L-arginine methyl ester (L-NAME, 0.5 g·L-1, pH=7.4) were used in C57BL/6N male mice to construct the classical HFpEF model (the experiment was approved by the Animal Ethics Committee of Hefei University of Technology, the approval number is HFUT20220921002), and at the 8th week, the mice were dosed with ① empagliflozin, ② low-dose QSYQ (LQ), ③ high-dose QSYQ (HQ), ④ empagliflozin plus low-dose QSYQ (ELQ) for 4 weeks, the body weight of the mice was recorded during the experiment, echocardiography, blood pressure and glucose tolerance were detected and the exercise capacity of mice was evaluated, and pathological and biochemical experiments were used to detect cardiac fibrosis, liver fibrosis and serum biochemical indexes in mice, RNAseq assay was performed with mice heart tissues. The results showed that QSYQ as well as QSYQ+empagliflozin could significantly reduce the body weight, improve diastolic function and hypertension, improve glucose tolerance and enhance exercise ability of HFpEF mice. At the biochemical and molecular levels, QSYQ and QSYQ+empagliflozin can reduce the cross-sectional area of cardiomyocytes and reduce cardiac collagen contents, thereby alleviating myocardial hypertrophy and fibrotic phenotypes, and improve metabolic disorders. The RNAseq results suggest that the function of QSYQ in improving HFpEF may be related to calsequestrin 1. In conclusion, this study shows that QSYQ and QSYQ+empagliflozin can significantly improve HFpEF-related cardiac dysfunction and metabolic disorders, which provides a theoretical basis for the clinical treatment of HFpEF.

心力衰竭(heart failure, HF) 是一种多方面的、危及生命的临床综合征, 其特点是死亡率和发病率高、多器官功能受损、患者生活质量差、治疗费用高, 影响着全球6 400多万人[1]。2022年AHA/ACC/HFSA心力衰竭管理指南根据左室射血分数(LVEF) 将HF分为4类, 即射血分数降低的心力衰竭(HFrEF)、射血分数保留型心力衰竭(HFpEF)、射血分数轻度降低的心力衰竭(HFmrEF)、射血分数改善的心力衰竭(HFimpEF)[2]。而HFpEF是一种复杂的心血管疾病, 它已经成为全球心力衰竭发病的主要形式, 随着人口的老龄化以及肥胖、糖尿病和高血压患病率的增加, 其患病率也在不断增长[3]。HFpEF主要表现为运动耐受性降低和组织充血等症状[4]。尽管HFpEF发病的分子机制尚不完全清楚, 但人们普遍认为, 心脏纤维化和肥大是HFpEF的基本病理特征, 并伴随着左心室僵硬增加和心脏舒张功能受损[5, 6]
芪参益气滴丸(QiShenYiQi Dripping Pills, QSYQ) 于2003年被中国食品药品监督管理局(CFDA) 批准用于治疗心血管疾病[7]。它由黄芪、丹参、三七、降香组成, 比例为10∶5∶1∶0.067。研究表明, QSYQ具有多种药理活性, 包括改善能量代谢和心肌重构[7], 抑制心肌细胞凋亡[8], 抑制急性心肌梗死中的炎症反应[9], 调控糖、脂和蛋白质代谢等[10]。此外, 许多临床研究已经证明了QSYQ治疗HFrEF的安全性和有效性[11, 12], 一项荟萃分析证实, QSYQ联合西药治疗可显著改善慢性HF[13]; 此外, 网络药理学分析同样揭示了包括cGMP-PKG信号通路在内的几种途径介导了QSYQ治疗HFpEF的作用[14]。在2型糖尿病肥胖db/db小鼠中, 低强度脉冲超声治疗通过激活eNOS-NO-cGMP-PKG通路和Ca2+调节系统来改善心脏舒张功能障碍[15]。目前的证据表明, QSYQ治疗HFpEF具有巨大潜力。
本研究旨在探究QSYQ在HFpEF中的治疗作用。给予C57BL/6N小鼠HFD喂食以及含有N-硝基-L-精氨酸甲酯盐酸盐(L-NAME) 的饮用水, 构建HFpEF模型, 同时给予QSYQ以及QSYQ联合恩格列净治疗, 并通过一系列实验验证不同剂量QSYQ以及QSYQ联合恩格列净改善HFpEF的药理作用, 为HFpEF的治疗提供实验数据支撑。
药品与试剂  芪参益气滴丸(国药准字Z20030139) 购自天津天士力医药集团股份有限公司; 高脂饲料(货号: MD12033) 购自江苏美迪森生物医药有限公司; L-NAME (货号: IL0320) 和天狼星红染色试剂盒(货号: G1472) 均购自北京索莱宝科技有限公司; 恩格列净(货号: HY-15409) 购自美国MedChemExpress公司。小鼠脑钠肽(brain natriuretic peptide, BNP)、N末端前体脑利钠肽(N-terminal pro-brain natriuretic peptide, NT-proBNP) ELISA检测试剂盒(货号: RX202726M、RXAg20776) 购自泉州市睿信生物科技有限公司。
实验动物  本实验获得合肥工业大学动物伦理委会批准, 动物伦理批准号为HFUT20220921002。健康雄性C57BL/6N野生型(WT) 小鼠(8周龄) 购自江苏集萃药康生物科技股份有限公司[动物生产许可证号: SCXK (苏) 2023-0009], 所有实验均在合肥工业大学食品与生物工程学院SPF级动物实验中心进行, 饲养环境温度控制在(23 ± 1) ℃, 明暗周期为12 h∶12 h, 实验期间小鼠可以自由进食和饮水。
HFpEF动物模型的建立及实验设计  小鼠适应性喂养一周后随机分为正常组(C)、模型组(HFpEF)、恩格列净给药组(HFpEF+E)、低剂量QSYQ组(HFpEF+LQ)、高剂量QSYQ组(HFpEF+HQ) 和低剂量QSYQ与恩格列净联用组(HFpEF+ELQ), 每组8只。C57BL/6N小鼠喂食高脂食物(high fat diet, HFD) 并给予含L-NAME (0.5 g·L-1, pH=7.4) 的饮用水8周, 建立HFpEF模型[16]。正常对照组饲喂对照饲料和普通饮用水。恩格列净临床用量为0.17 mg·kg-1[17], QSYQ高低剂量临床用量为6和3 g·kg-1[18], 根据因数剂量法应用身体表面积指数(0.67) 来换算动物和人类之间的剂量[19], 可以得到小鼠剂量, 分别为1.3 mg·kg-1恩格列净、390 mg·kg-1 QSYQ、780 mg·kg-1 QSYQ。8周后, 小鼠分别给予1.3 mg·kg-1恩格列净、390 mg·kg-1 QSYQ、780 mg·kg-1 QSYQ或390 mg·kg-1 QSYQ联合1.3 mg·kg-1恩格列净灌胃4周。实验流程如图 1所示。
常规超声心动图和多普勒成像  经胸超声心动图使用VINNO 6 LAB系统(飞依诺科技股份有限公司)。用氧和异氟烷(1%~2%) 麻醉小鼠, 左室收缩分数缩短(LVFS)、左室射血分数(LVEF)、舒张早期经二尖瓣血流速度峰值(E)、舒张晚期经二尖瓣血流速度峰值(A)、舒张早期二尖瓣环心肌舒张速度组织多普勒峰值(E′) 通过心室中水平短轴M型扫描获得。使用脉冲波和组织多普勒测量二尖瓣水平的舒张功能。
小鼠血压记录  采用小动物无创血压测量仪(北京众实迪创科技发展有限责任公司, NIBP-600) 评估清醒小鼠血压。将小鼠置于小鼠固定器中, 在稳态条件下进行记录, 直到获得3个稳定的连续测量值。
腹腔葡萄糖耐量试验(intraperitoneal glucose tolerance test, IPGTT)  小鼠于空腹12 h后注射葡萄糖(2 g·kg-1溶于生理盐水) 进行腹腔葡萄糖耐量试验。在给药前(0 min) 和给药后30、60、90和120 min用血糖仪测量尾血糖水平。为了确定葡萄糖耐量, 本研究计算了曲线下面积(area under the curve, AUC)。
运动疲劳测验  本研究使用小动物跑步机(北京众实迪创科技发展有限公司, ZS-PT-Ⅲ) 来测量各组小鼠的运动能力。动物在跑步机上以5 m·min-1的热身速度跑4 min, 然后将速度提高到14 m·min-1跑2 min, 之后每2 min增加2 m·min-1, 直到动物筋疲力尽。疲劳被定义为动物在直接接触电刺激电网后10 s内无法恢复跑步。当小鼠筋疲力尽时, 记录小鼠的跑步时间和距离。所有小鼠都在测试前进行训练。
H&E染色  取小鼠心脏和肝脏组织, 4%多聚甲醛固定24 h, 然后脱水、包埋并制备5 μm石蜡切片。取制备好的石蜡切片进行脱蜡水化, 苏木素30 s、伊红3 min染色, 然后脱水, 并用中性树脂封片。显微镜下观察心脏和肝脏组织病理学变化并拍照记录。
油红O染色  取石蜡包埋的心脏切片, 常规脱蜡, 油红O染色30 min后立即用60%异丙醇溶液脱色3~5 s, 然后用苏木素染细胞核1 min, 完成后用甘油封片, 在显微镜下观察并拍照记录。
WGA染色  小麦胚芽凝集素WGA染色, 取石蜡包埋的心脏切片, 先脱蜡, 用FITC-WGA (1∶100) 在37 ℃避光条件下孵育15 min。用PBS洗涤后封片, 拍照记录。
天狼星红染色  取石蜡包埋的心脏切片, 常规脱蜡, 苏木素染色10 min, 洗3次, 天狼星红染液染30 min, 完成后用中性树脂封片, 拍照记录。
酶联免疫吸附  收集小鼠血清后, 根据相应的酶联免疫吸附试剂盒的操作步骤, 检测小鼠血清中的BNP和NT-proBNP。
血清生化指标分析  对小鼠进行眼球取血并收集, 室温放置2 h后, 全血3 000 r·min-1离心15 min, 上清即为血清, 然后利用生化分析仪(日本东京HITACHI公司, 日立3100) 进行生化检测, 包括乳酸脱氢酶(lactate dehydrogenase, LDH)、甘油三酯(triglyceride, TG)、天冬氨酸转氨酶(aspartate aminotransferase, AST)、谷丙转氨酶(alanine aminotransferase, ALT)、碱性磷酸酶(alkaline phosphatase, ALP)、总胆固醇(total cholesterol, TCHO)、低密度脂蛋白胆固醇(low-density lipoprotein, LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein, HDL-C)、肌酸激酶(creatine kinase, CK)。
转录组学分析  用Trizol试剂(美国Invitrogen公司) 和DNase I (日本Takara公司) 从小鼠心脏组织中提取总RNA。随后, 使用2100 Bioanalyzer (美国Agilent公司) 和NanoDrop 2000 (美国Thermo Fisher Scientific公司) 测定RNA的纯度和浓度。用带有oligo的磁珠与富含poly A的RNA片段进行A-T碱基配对, 从总RNA中分离mRNA进行转录组测序(RNAseq)。富集的mRNA被化学分割成约300 bp长的片段。采用上标双链cDNA合成试剂盒(美国Invitrogen公司) 和随机六聚体引物(美国Illumina公司) 从mRNA合成cDNA。构建的文库使用上海美吉生物医药科技有限公司的Illumina Novaseq 6000 (美国Illumina公司) 进行测序。在RNAseq分析中, 基因表达水平根据定位于基因组区域的干净reads计数来计算。使用RSEM软件定量分析基因和转录物的表达水平。采用DESeq2方法进行差异表达分析。|FC| > 1.2且P-adjust < 0.05为差异表达基因(differential expression gene, DEG)。P < 0.05的KEGG通路被鉴定为DEG显著富集。
统计学分析  实验数据以均值±标准差表示, 采用GraphPad Prism进行绘图和统计分析。采用单因素方差分析one-way ANOVA评价组间差异。P < 0.05被认为具有显著性差异。
肥胖、糖尿病和高血压是HFpEF常见的合并症[20], HFD不仅会导致体重显著增加, 也会导致葡萄糖耐受不良。首先, 与C组相比, HFpEF组小鼠体重显著增加, 葡萄糖耐受出现异常, 而低剂量和高剂量QSYQ以及与恩格列净联用均可显著减少HFpEF小鼠体重增加, 并改善葡萄糖耐受不良(图 2A~C)。
L-NAME是一种一氧化氮合酶抑制剂, 主要通过引起一氧化氮(nitric oxide, NO) 缺乏、炎症、氧化应激和内皮受损而导致心脏和机体损伤, L-NAME对NO产生的慢性抑制会促进持续的外周血管收缩, 并导致血压升高[21, 22]。本研究结果显示, HFpEF小鼠表现出高血压的表型, 包括收缩压(systolic blood pressure, SBP) 和舒张压(diastolic blood pressure, DBP) 增加, 相比之下, 低剂量和高剂量QSYQ以及与恩格列净联用治疗后明显降低HFpEF小鼠SBP和DBP, 且联用表现出最佳效果(图 2DE)。
为了探究QSYQ对HFpEF小鼠运动能力的改善作用, 本研究对不同组小鼠进行了力竭实验。结果显示, 与对照组相比, HFpEF小鼠的跑步距离显著减少, 而E、LQ、HQ和ELQ治疗均可恢复小鼠的运动能力(图 3A)。进一步地, HFpEF小鼠的肺干湿重比、心脏重量与胫骨长度比值以及心脏大小出现显著增加, 并伴有间质和血管周围纤维化增加, 心脏肥大和纤维化表型的出现, 而E、LQ、HQ和ELQ治疗则显著降低了HFpEF模型中的肺干湿重比、心脏肥大和纤维化(图 3B~D)。同样地, WGA染色结果也显示出HFpEF小鼠心肌细胞肥大以及细胞结构不规则, E、LQ、HQ和ELQ处理均显著改善上述差异(图 3E)。此外, H&E染色结果显示HFpEF小鼠心肌细胞细胞核聚集且排列紊乱, 出现炎症细胞浸润(图 3F), 天狼星红染色显示HFpEF小鼠心肌纤维化严重(图 3G), E、LQ、HQ和ELQ处理对于上述表型均有显著改善(图 3FG)。上述结果表明, QSYQ以及与恩格列净联用的治疗显著改善了HFpEF小鼠的肺充血、心肌肥大和心肌纤维化。
本研究还对QSYQ在HFpEF小鼠心脏功能中的作用进行验证。超声心动结果显示, 在实验结束后, 各组间小鼠的LVEF (图 4AB) 和LVFS (图 4C) 均无明显变化, 而HFpEF模型组E/A、E/E′与对照组相比显著升高, E、LQ、HQ和ELQ均可显著改善E/A、E/E′的升高(图 4DE), 其中联用表现出最佳的效果。
BNP和NT-proBNP为反映心脏功能的常用生物学指标[23, 24]。且LDH和AST已被用于作为诊断心肌损伤以及心衰、心肌肥厚等的生物标志物[25]。与正常组相比, HFpEF小鼠血清中BNP、NT-proBNP、LDH和AST水平显著升高, E、LQ、HQ和ELQ治疗则显著逆转HFpEF小鼠中BNP、NT-proBNP、LDH和AST水平的升高(图 5A~D)。CK活性测定也可以用于心肌疾病的诊断, 本研究发现在HFpEF小鼠中CK水平显著上升, 而在QSYQ治疗后则显著下降(图 5E)。
长期高脂饮食可导致脂质紊乱, 同时, 高脂血症也是HFpEF的风险因素之一[26]。结果表明, HFpEF小鼠血清中TG、TCHO、LDL-C水平均明显高于正常喂养小鼠(图 5F~H), 一致的是, HDL-C被认为是一种有益的脂蛋白, HFpEF模型组小鼠血清HDL-C水平明显降低, 给药治疗后则显著恢复(图 5I)。上述结果表明, E、LQ、HQ和ELQ均可有效地调节HFD喂食下血清脂质水平, 改善HFpEF小鼠整体脂质代谢紊乱, 从而在一定程度上有助于心脏功能改善。
本研究进一步对6组小鼠肝脏组织进行油红O、天狼星红和H&E染色, 并检测血清中肝损伤关键指标ALT、AST、ALP水平。结果显示, HFpEF小鼠表现出一定程度的肝损伤, 表现为肝脏脂质增加、纤维化炎症因子浸润增多(图 6A)、血清中ALT、AST和ALP水平同样显著升高(图 6B5D6C), 而E、LQ、HQ和ELQ治疗均显著改善了肝脏脂肪积累和肝脏炎症水平, 同时降低了血清中ALT、AST、ALP水平。因此, QSYQ治疗以及与恩格列净联用同样改善HFpEF模型下的肝损伤表型。
为了进一步挖掘QSYQ改善HFpEF的潜在靶点和分子机制, 首先本研究计算了HFpEF相关疾病基因与恩格列净药物靶点、QSYQ文献靶点之间的网络邻近度。结果显示, HFpEF相关疾病基因与QSYQ文献靶点之间的网络邻近度(z-score < -1.5代表疾病和药物比较邻近, 越负代表疾病与药物在网络上越邻近) 具有更高的意义(图 7A), 更进一步地支持了QSYQ改善HFpEF的潜在可能性。
接下来对6组小鼠心脏组织进行了RNAseq测序并进行差异基因分析, HFpEF组和C组共10个样本中有7个以上的样本表达counts数大于10的59个基因在纳入范围, 并对上述59个基因进行聚类, 本研究发现用药组更接近于正常对照组; 其中QSYQ与恩格列净联用效果更好(图 7B)。从59个DEGs (HFpEF vs C)热图log2FC的结果来看, 59个基因中有11个基因在HFpEF中表达上调, 用药组均可逆转疾病组特异表达基因, HFpEF+ELQ组逆转效果最好(图 7C)。然后对各组差异基因取交集, 总共获得22个差异基因(图 7DE), 发现Casq1包含在其中, Casq1和Casq2是CASQ的两种亚型, Casq1可在小鼠和人心脏的心肌细胞肌浆网中表达, 主要以聚合物的形式存在, 并与心室中的兰尼碱受体2相互作用[27]。此外, 研究表明肌浆网中的Casq1聚集可能是特发性空泡性心肌病的致病条件之一[28]。综上所述, QSYQ是一种有效改善HFpEF的潜在药物, 同时与恩格列净联用效果可优于恩格列净单独使用。此外, Casq1可能是QSYQ改善HFpEF的潜在分子靶点, 后续需要进行深入的机制验证。
HFpEF是一个重大的公共卫生问题, 具有较高的发病率和死亡率。HFpEF约占所有HF病例的一半, 同时由于其多重合并症、缺乏临床前模型以及对HFpEF病理生理过程的认识不足等, 尚无循证治疗方法[29]。本文的实验结果表明, 在HFD和L-NAME诱导的HFpEF小鼠模型中, 小鼠发展为肥胖、糖代谢紊乱、高血压、肺充血、心脏肥厚、运动能力降低和心脏舒张功能不全, 而QSYQ及与恩格列净联用治疗4周后可以显著改善上述表型。
超声心动图被广泛用于评估心脏功能, 包括收缩功能和舒张功能。多普勒图像记录了两个显著峰, E峰(心室舒张) 和A峰(心房收缩)。正常情况下, E峰值大于A, 而在HFpEF中, 由于心脏舒张功能受损, 大量的血液滞留在心房, 增加了心房对心室充盈的贡献, 表现为E峰增加, A峰降低, 因此E/A比值升高反映了舒张功能障碍[30]。经HFD+L-NAME造模12周后, 超声心动图结果显示HFpEF组与正常组相比, E/A、E/E′比值明显升高, 而LVEF和LVFS均无显著差异, 符合HFpEF动物模型构建成功的标准[16]。HFpEF+E、HFpEF+LQ、HFpEF+HQ、HFpEF+ELQ治疗组均有一定程度的改善, 特别是QSYQ高剂量在大部分指标中表现出与恩格列净相同的效果, 并且在一些指标中(SBP、E/A) QSYQ和恩格列净联用表现出更好的效果, 说明QSYQ不仅可以作为改善小鼠心脏舒张功能障碍的潜在药物, 还可以与恩格列净联用, 达到更好的治疗效果。
有研究发现, 心脏纤维化与HFpEF患者心律失常、住院率和死亡率增加正相关[31]。本研究观察到QSYQ通过降低HW/TL比、心肌细胞横截面积和心脏胶原体积对HFpEF小鼠的心脏炎症、纤维化及心脏肥大表型均有显著改善。此外, QSYQ还通过降低血清TCHO、TG、LDL-C和升高HDL-C来调节脂质紊乱。而HFpEF小鼠长期饲喂HFD不仅会导致肥胖, 还会导致肝脏受损, QSYQ治疗后显著改善了肝脏脂肪积累、炎症和纤维化, 同时降低了血清中ALT、AST、ALP水平, 这表明QSYQ不仅可以治疗HFpEF引起的心脏损伤, 还可以治疗肝脏损伤。未来, 本研究可以结合数智中药[32]这一新兴理论, 将QSYQ的理论知识和临床经验与大数据、云计算及人工智能等现代科技相结合, 使QSYQ变为作用机制更加明确的现代创新中药。
肌钙集蛋白(CASQ) 是一种关键的肌浆内网Ca2+处理蛋白, 在心脏和骨骼肌的收缩中起着关键作用[33], 其亚型Casq1可缓冲肌浆网中的Ca2+, 具有低亲和力和高容量Ca2+结合的能力[34, 35]。同样也有研究表明, Casq1被鉴定为心肌恢复过程中Ca2+稳态调控的关键基因[36]。在本研究中, Casq1在HFpEF中上升, 在QSYQ、恩格列净以及联用治疗组均下降, 且RNAseq结果也提示其在QSYQ改善HFpEF中的潜在作用, 因此后续本课题组需要开展Casq1在QSYQ改善HFpEF中的关键作用及深入机制探究。
综上所述, QSYQ及与恩格列净联用可以显著改善HFpEF小鼠高血压、运动能力下降以及心脏肥大、心肌纤维化、心脏舒张功能障碍及相关代谢表型。因此, QSYQ有望为HFpEF患者的临床治疗带来额外选择或协同疗效。
