新药创制是复杂的智力活动, 涉及科学研究、技术创造、产品开发和医疗效果等多维科技活动。每个药物都有自身的研发轨迹, 而构建化学结构是最重要的环节, 因为它涵盖了药效、药代、安全性和生物药剂学等多维性质。本栏目以药物化学视角, 对有代表性的药物的成功构建, 加以剖析和解读。
阿派西林是诺华公司研制的作用于α亚型磷脂酰肌醇-3激酶(PI3Kα) 的首创性口服药物, 治疗对PI3Kα突变的HR阳性和HER-2阴性的乳腺癌患者。在本品研制的同时不少制药公司也针对该靶标开展了研究, 都在临床研究中因脱靶的不良反应而中止, 反映了在体外到体内、由动物到患者的转化中的不确定性和风险, 以及本品之幸运。研制阿派西林的药物化学特征, 一是利于区分靶标亚型之间的微观结构差异, 分子模拟指导结构设计与优化, 聚焦于特定的PI3Kα激酶选择性和高活性的目标, 二是在保障活性和选择性的前提下, 兼顾化合物的代谢稳定性的修饰和对细胞色素P450的无碍, 实现了活性与成药性的同步优化。(编者按)
磷脂酰肌醇-3-激酶(PI3K) 是类脂质激酶, 催化磷脂酰肌醇的磷酸化, 生成3-磷酸磷脂酰肌醇(PIP3), PIP3作为第二信使参与细胞内信号转导, 在细胞增殖、存活、迁移、炎症和凋亡等过程起重要调节作用。人体细胞中含有编码PI3K-1A的催化亚基, 称作p110α、p110β和p110δ; PI3K-1B只表达酶p110γ, p110γ和p110δ主要存在于白细胞中。PI3K信号通路失控, 使抑癌基因失活, 一些受体酪氨酸激酶发生变异、放大或过表达会发生多种癌症, 例如结肠癌、恶性胶质瘤、胃癌、肝癌和乳腺癌等。
由于p110α发生变异而激活其激酶活性, PI3Kα成为研制抗癌药物的理想靶标, 本品就是以PI3Kα为靶标经体外和体内活性评价研制成功的抗肿瘤药物。
诺华公司研究本项目的起始化合物是经过高通量筛选, 发现化合物1对PI3Kγ呈现活性(Ki = 0.11 μmol·L-1), 随后确定的目标是研制泛PI3K激酶抑制剂, 优化过程中发现将氨磺酰基换成甲磺酰基的化合物2对多种PI3K激酶亚型的活性在0.03~0.08 μmol·L-1水平。继续优化将甲磺酰基变换为1-吡唑化合物3, 泛PI3K激酶亚型的结合作用进一步增强。再将乙酰基用脲基置换, 经过多轮优化得到化合物4, 对PI3Kα激酶具有选择性结合作用(对p110αKi值0.005 μmol·L-1), 比其他亚型的活性高1个数量级, 之所以聚焦于PI3Kα激酶是因为泛PI3K激酶抑制剂常出现不良反应, 而多种癌症的PI3Kα激酶处于高表达和发生突变(Bruce I, Akhlaq M, Bloomfield GC, et al. 诺华公司为了研制PI3Kα激酶选择性抑制剂, 以化合物4为起始物, 设计的化合物5, 是为了同时探索多个位置的构效关系, 即用叔丁基置换吡唑环; 苯环变换为嘧啶环; 噻唑环上的甲基和脯氨酰胺的不变。
将化合物5分子对接到p110α的晶体结构(抽出复合物中的配体p85α) 得到的低能复合物如图 1所示。结合特征如下: ①噻唑环的氮原子和2-NH与Val851形成双齿型氢键(一个给体, 一个接受体); ②嘧啶环结合于由Tyr836、Ile932、Ile848、Ileu800、Asp933、Lys802、Pro778和Met772组成的疏水腔内; ③末端的脯氨酸的S-酰胺的NH2与Ser854的羰基形成氢键, NH2作为氢键给体与Gln859生成两个氢键。Gln859在不同的PI3K亚型是不一样的, p110βδγ亚型在该位置的残基分别是Asp、Asn和Lys, βγ的Asp和Lys显然不能与5形成如同Gln的两个氢键, δ亚型的Asn因长度短于Gln也难以形成氢键, 因而, 化合物5对p110α显示特异性结合作用。测定5的活性表明, 对p110α的IC50为0.007 5 μmol·L-1, 而对其他亚型的抑制作用减弱数十到上百倍(表 1)。
以化合物5为先导化合物变换4个位置的原子或基团, 即叔丁基的变换、嘧啶的变换、噻唑环上单甲基和脯氨酰胺的变换。合成的代表性化合物列于表 1。表中列出了化合物对4种亚型的抑制活性、人肝微粒体的代谢清除率和对CYP3A4的抑制作用, 后两个是评价化合物的体外药代性质。分析构效关系如下: ①分子模拟提示化合物5的嘧啶环N3在ATP结合腔内未显示有电性相互作用, 预示换成碳原子的吡啶环对抑制p110α的活性影响不大。果然, 化合物6对p110α活性与5相近; ②化合物7896的区别分别是N-甲基(仲胺)、无酰胺基团的四氢吡咯, 以及6的对映异构体, 这3个化合物都不能与Gln859发生双齿型氢键结合, 因而活性显著降低; ③ 10是缩环氮杂丁烷化合物, 活性略有下降, 可解释为氮杂丁烷不如四氢吡咯与His855的咪唑环发生的充分范德华作用所致; ④化合物11的活性强于56, 是叔丁基的一个甲基被三氟甲基取代的缘故。分子模拟显示, 容纳叔丁基的疏水腔还有空余的空间, 体积较大的三氟甲基增加了疏水-疏水相互作用。化合物19的活性与6相同, 连接在嘧啶环的甲基环丙基可视作叔丁基的环合物; ⑤其余的化合物活性都有所减弱, 提示那样的变换不利于活性。其中18是二乙胺基取代, 虽然体积比叔丁基大, 但基团的形状与结合腔不匹配, 因而活性也减弱; ⑥研究对人肝微粒体的代谢作用显示56的主要代谢位点是叔丁基的羟基化和噻唑环的4-甲基羟基化。化合物11121617由于三氟甲基或氰基的拉电子效应, 降低了氧化代谢能力, 因而提高了代谢稳定性。17还因为噻唑环有4-氯取代, 进一步提高了稳定性。
用大鼠分别高表达p110α、p110β和p110δ的细胞评价化合物4~19阻断PI3K/Akt信号通路的活性表明, 抑制剂的活性强度和选择性与酶水平测定的结果有相同的趋势, 提示作用机制是相同的。表 2列出了化合物对3种细胞系的抑制活性。数据显示化合物5611对大鼠1-myr-p110α细胞的抑制活性达到纳摩尔水平, 选择性作用很高。
进而选出4个高活性和高选择性的化合物评价对大鼠的药代动力学性质, 结果列于表 3。化合物11的药代性质显著优于其他化合物(Furet P, Guagna V, Fairhurst RA, et al. Discovery of NVP-BYL719 a potent and selective phosphatidylinositol-3 kinase alpha inhibitor selected for clinical evaluation. Bioorg Med Chem Lett, 2013, 23: 3741-3748)。
进一步对化合物11在小鼠、大鼠和犬评价了生物利用度和体内清除率, 并对各种细胞色素P450亚型评价了活性, 证明不是抑制剂和诱导剂。在10 μmol·L-1浓度下对448种激酶没有显示抑制作用, 提示具有选择性抑制PI3Kα激酶的作用, 遂确定为候选化合物, 定名为阿派西林(alpelisib), 经三期临床研究证明对PI3Kα突变的HR-阳性、HER-2阴性的乳腺癌患者可延长无进展生存期, 治疗中患者对阿派西林有更高的耐受性。经美国FDA批准于2019年上市, 成为第一个针对该靶标治疗晚期乳腺癌的口服药物。
