Polysaccharide of Balanophora involucrata Hook. f. (BPS), the major component of Balanophora involucrata Hook. f., was confirmed the protective effect on liver injury in our previous study. This research aimed to investigate the protective mechanism of BPS on experimental liver injury by attenuating cell ferroptosis through modulating solute carrier family 7 member 11/glutathione peroxidase 4 (SLC7A11/GPX4) pathway. The animal experiment was approved by the Experimental Animal Ethical Committee of Hubei Minzu University and all rats had received human care in compliance with the institutional animal care guidelines. Rats were given intraperitoneal injection of (D-galactosamine, D-GalN) solution (800 mg·kg-1) one time to establish the acute liver injury model. The results showed aspartate amino transferase (AST), alanine aminotransferase (ALT) and 4-hydroxynonenal (4-HNE) levels in serum were decreased, and the contents of reactive oxygen species (ROS), Fe2+, malondialdehyde (MDA) and lipid peroxide (LPO) in liver tissues also decreased and glutathione (GSH) level increased after BPS administration with 200 mg·kg-1. Besides, BPS reduced iron deposition and increased the expression of SLC7A11 and GPX4 proteins in liver tissue. In conclusion, BPS ameliorated experimental liver injury by alleviating cell ferroptosis through SLC7A11/GPX4 pathway. The present study pointed to the possibility of utilizing BPS for protection against liver injury in clinic.

蛇菰多糖(polysaccharide of Balanophora involucrata Hook. f., BPS) 是土家民族药筒鞘蛇菰的主要活性成分之一, 前期已证实具有保肝护肝作用。本研究旨在深入探讨BPS对实验性肝损伤的保护作用机制, 可能与影响溶质载体家族7成员11/谷胱甘肽过氧化物酶4 (solute carrier family 7 member 11/glutathione peroxidase 4, SLC7A11/GPX4) 通路减轻肝细胞铁死亡有关。本实验方案经湖北民族大学实验动物伦理委员会审查, 符合实验动物福利与伦理原则。实验采用一次性腹腔注射D-氨基半乳糖溶液(D-galactosamine, D-GalN, 800 mg·kg-1) 建立急性肝损伤模型, 根据前期研究给予200 mg·kg-1 BPS溶液灌胃干预, 以铁死亡抑制剂去铁胺(desferrioxamine, DFO) 为对照。结果表明, BPS干预可明显降低大鼠血清谷草转移酶(aspartate amino transferase, AST)和谷丙转移酶(alanine aminotransferase, ALT) 水平, 降低肝组织内活性氧(reactive oxygen species, ROS) 水平和Fe2+含量, 降低脂质过氧化产物丙二醛(malondialdehyde, MDA)、4-羟基壬烯醛(4-hydroxynonenal, 4-HNE) 和脂质过氧化物(lipid peroxide, LPO) 水平, 提高还原型谷胱甘肽(glutathione, GSH) 水平, 同时减少肝内铁超载, 伴肝组织内SLC7A11和GPX4蛋白表达增加, 表明BPS可能通过影响SLC7A11/GPX4通路减轻肝细胞铁死亡, 进而改善肝损伤。本研究为民族药筒鞘蛇菰用于辅助治疗肝损伤提供实验依据。

蛇菰多糖(polysaccharide of Balanophora involucrata Hook. f., BPS) 是土家民族药筒鞘蛇菰的主要活性成分之一, 前期已证实具有保肝护肝作用。本研究旨在深入探讨BPS对实验性肝损伤的保护作用机制, 可能与影响溶质载体家族7成员11/谷胱甘肽过氧化物酶4 (solute carrier family 7 member 11/glutathione peroxidase 4, SLC7A11/GPX4) 通路减轻肝细胞铁死亡有关。本实验方案经湖北民族大学实验动物伦理委员会审查, 符合实验动物福利与伦理原则。实验采用一次性腹腔注射D-氨基半乳糖溶液(D-galactosamine, D-GalN, 800 mg·kg-1) 建立急性肝损伤模型, 根据前期研究给予200 mg·kg-1 BPS溶液灌胃干预, 以铁死亡抑制剂去铁胺(desferrioxamine, DFO) 为对照。结果表明, BPS干预可明显降低大鼠血清谷草转移酶(aspartate amino transferase, AST)和谷丙转移酶(alanine aminotransferase, ALT) 水平, 降低肝组织内活性氧(reactive oxygen species, ROS) 水平和Fe2+含量, 降低脂质过氧化产物丙二醛(malondialdehyde, MDA)、4-羟基壬烯醛(4-hydroxynonenal, 4-HNE) 和脂质过氧化物(lipid peroxide, LPO) 水平, 提高还原型谷胱甘肽(glutathione, GSH) 水平, 同时减少肝内铁超载, 伴肝组织内SLC7A11和GPX4蛋白表达增加, 表明BPS可能通过影响SLC7A11/GPX4通路减轻肝细胞铁死亡, 进而改善肝损伤。本研究为民族药筒鞘蛇菰用于辅助治疗肝损伤提供实验依据。

Polysaccharide of Balanophora involucrata Hook. f. (BPS), the major component of Balanophora involucrata Hook. f., was confirmed the protective effect on liver injury in our previous study. This research aimed to investigate the protective mechanism of BPS on experimental liver injury by attenuating cell ferroptosis through modulating solute carrier family 7 member 11/glutathione peroxidase 4 (SLC7A11/GPX4) pathway. The animal experiment was approved by the Experimental Animal Ethical Committee of Hubei Minzu University and all rats had received human care in compliance with the institutional animal care guidelines. Rats were given intraperitoneal injection of (D-galactosamine, D-GalN) solution (800 mg·kg-1) one time to establish the acute liver injury model. The results showed aspartate amino transferase (AST), alanine aminotransferase (ALT) and 4-hydroxynonenal (4-HNE) levels in serum were decreased, and the contents of reactive oxygen species (ROS), Fe2+, malondialdehyde (MDA) and lipid peroxide (LPO) in liver tissues also decreased and glutathione (GSH) level increased after BPS administration with 200 mg·kg-1. Besides, BPS reduced iron deposition and increased the expression of SLC7A11 and GPX4 proteins in liver tissue. In conclusion, BPS ameliorated experimental liver injury by alleviating cell ferroptosis through SLC7A11/GPX4 pathway. The present study pointed to the possibility of utilizing BPS for protection against liver injury in clinic.

急慢性肝损伤指各种刺激因素导致肝脏出现病理状态, 归属于中医学“胁痛”“黄疸”“积聚”等范畴, 表现为机体内活性氧(reactive oxygen species, ROS) 生成增多, 进而出现脂质过氧化性损伤, 乃至细胞死亡。研究发现, 许多中药有效成分有保肝护肝作用, 如泽泻含三萜及多糖等活性成分, 其水醇提取物均可明显改善实验性肝损伤[1], 且中医药在防治肝损伤方面有独特优势及明确疗效[2]。筒鞘蛇菰, 土家族四大名贵药材之一, 蛇菰科蛇菰属草本植物, 前期研究发现其提取物-蛇菰多糖(BPS) 有抗肝损伤作用[3, 4], 深入探讨其作用机制有重要意义。
铁死亡(ferroptosis), 一种新型程序性细胞死亡方式, 主要病理特征为Fe2+催化的芬顿化学反应促进脂质过氧化, 引起细胞氧化损伤、破坏细胞膜致细胞死亡[5], 已证实铁死亡与临床常见肝损伤性疾病发生密切相关。众多研究表明, 具有护肝作用的天然药物除了参与调控细胞凋亡、坏死及自噬等方式外, 还可调控细胞铁死亡, 抑制铁死亡可减轻肝损伤。然而, 筒鞘蛇菰能否通过抑制铁死亡发挥作用尚不清楚。本研究深入探讨其提取物BPS能否调控SLC7A11/GPX4通路减轻细胞铁死亡发挥作用。
动物模型  雄性SPF级SD大鼠40只, 体质量(200 ± 20) g, 购于湖北省实验动物研究中心[许可证号SCXK (鄂) 2018-0018]。本实验由湖北民族大学实验动物伦理委员会审查通过。随机分为正常对照组(Con)、肝损伤模型组(D-GalN)、蛇菰多糖干预组(BPS)、去铁胺组(DFO), 每组10只。参考前期研究[3, 4]给予BPS组大鼠灌胃BPS溶液(200 mg·kg-1), DFO组腹腔注射DFO溶液(50 mg·kg-1), D-GalN组及Con组灌胃等量生理盐水, 每天1次, 连续2周。第15天时, D-GalN组及BPS组采用一次性腹腔注射D-GalN溶液(800 mg·kg-1) 建立急性肝损伤模型, 24 h后处死大鼠, 取血及肝组织进行检测分析。
药物与试剂  参照文献[6]及前期研究[4, 7]进行提取纯化BPS用于本研究。谷草转移酶(aspartate amino transferase, AST) 测试盒(微板法)、谷丙转移酶(alanine aminotransferase, ALT) 测试盒(赖氏法)、丙二醛(malondialdehyde, MDA) 试剂盒(TBA法)、还原型谷胱甘肽(glutathione, GSH) 试剂盒(分光光度法) 和ROS检测试剂盒(化学荧光法) 均购自南京建成生物工程研究所。Fe2+试剂盒(比色法)、4-羟基壬烯醛(4-hydroxynonenal, 4-HNE) ELISA试剂盒和脂质过氧化物(lipid peroxide, LPO) 测试盒购自武汉Elabscience公司。BCA蛋白定量试剂盒(批号: P0010)、D-GalN (批号: ST1213) 和β-actin抗体(批号: AF5001) 购自上海碧云天公司。普鲁士蓝染色试剂盒(Perls stain, 核固红法) 购自北京索莱宝科技有限公司。谷胱甘肽过氧化物酶4 (glutathione peroxidase 4, GPX4, 批号: ab252833) 和溶质载体家族7成员11 (solute carrier family 7 member 11, SLC7A11, 批号: ab175186) 抗体购自美国Abcam公司。DFO购自美国Sigma公司。
血清生化指标检测  实验结束进行麻醉处死大鼠, 腹主动脉采血, 置于含有4%柠檬酸钠抗凝管中静置30 min后, 1 000 ×g离心25 min, 留血清分析。取肝组织, 一部分置于10%多聚甲醛中固定, 另一部分放-80度冰箱冻存备用。按照试剂盒说明书测定血清AST、ALT及4-HNE水平。
肝组织Fe2+含量、ROS、MDA、LPO及GSH水平检测  取冻存肝组织, 称重后研磨制备10%组织匀浆, 离心后取上清液, 比色法测上清液中Fe2+含量; 采用DCFH-DA荧光探针, 在荧光酶标仪中检测593 nm处各孔吸光度值, 对照标准曲线计算ROS水平; TBA法检测上清液中脂质过氧化产物MDA含量; 微板法检测GSH水平; ELISA法检测LPO水平。均按照各试剂盒说明书操作。
苏木素伊红(HE) 染色观察大鼠肝脏形态  肝组织固定、包埋、制备石蜡切片, 行常规HE染色后置显微镜下观察; 每张切片随机选取5个视野、采用非酒精性脂肪性肝病活动性评分(NAFLD activity score, NAS) 系统[8]进行统计学分析, 来评估肝损伤情况。
Perls铁染色检测肝内铁沉积情况  取石蜡切片, 常规脱蜡后, 加Perls染液(试剂A1与A2等量混合, 现配现用) 染色20 min, 水洗, 核固红试剂B染核10 min, 水洗、脱水透明、封片, 光镜观察肝细胞内蓝色颗粒。每张切片随机选取5个视野, 用Image J软件分析普鲁士蓝染色阳性面积占整个面积的百分比。
蛋白免疫印迹(Western blot) 检测肝组织中GPX4和SLC7A11蛋白表达  取冻存肝组织, 冰上裂解, 离心取上清, BCA法测定蛋白浓度, 制备SDS-PAGE凝胶, 加入蛋白样品电泳分离后转至PVDF膜, 5%脱脂牛奶封闭、相应一抗(1∶1 000) 4 ℃孵育过夜, TBST清洗, 二抗(1∶4 000) 室温孵育2 h后加入显影液, ECL发光和Tanon 5200凝胶成像系统进行显影, 采用Image J软件统计分析目标条带相对灰度值。
统计学分析  本研究的数据整理与统计分析用GraphPad Prism 8.0软件, 数据用均值±标准差(x ± s) 表示, 组间比较用单因素方差分析。P < 0.05具有统计学意义。
血清ALT和AST水平是肝功能常见指标。脂质过氧化产物4-HNE可作为治疗肝损伤的有效靶点, 减少细胞内4-HNE含量可有效减轻肝细胞损伤[9]。本研究结果(图 1) 显示, BPS干预可明显降低大鼠血清ALT和AST水平(P < 0.01), 同时降低了血清4-HNE含量, 表明BPS可减轻D-GalN所致肝损伤、改善肝功能。
细胞铁死亡的实质是依赖铁离子的ROS和脂质过氧化物积累导致的线粒体形态改变和细胞膜磷脂发生过氧化性损伤[10]。GSH是机体抗氧化酶GPX4重要的辅助因子, GSH缺失会导致GPX4失活, 降低清除脂质过氧化物的能力, 而引起铁死亡[11]。肝内GSH水平降低亦可能是引起铁死亡的关键因素[12]。本研究结果显示, BPS干预可降低肝损伤组织内产生的过量ROS和Fe2+含量、增加肝内GSH水平(图 2A~C)。
LPO是一种自由基或活性氧作用于不饱和脂肪酸链后产生的含有过氧基的脂质, 已被证实可以准确反映机体的脂质过氧化程度, 包括MDA和4-HNE等。MDA含量变化代表着脂质过氧化作用强弱, 间接反映机体细胞受自由基攻击的严重程度。结果显示, BPS降低肝组织内脂质过氧化物MDA和LPO的含量(图 2DE)。
以上结果表明, BPS减轻Fe2+依赖性ROS产生及脂质过氧化性损伤, 可能是通过抑制肝细胞铁死亡来发挥保护作用。
HE染色结果(图 3A) 显示正常大鼠肝小叶结构完整, 肝细胞排列规则、细胞核圆居中, 无明显细胞水肿及坏死, 汇管区无炎细胞浸润。D-GalN诱导后可见肝细胞体积增大、弥漫性水肿, 部分细胞呈气球样变性, 另见肝内点灶状细胞坏死及炎细胞浸润, 及轻度肝细胞脂肪变性, 表明肝组织出现损伤。BPS干预大鼠肝细胞水肿及坏死均减轻, 炎细胞数量减少。NAS评分结果(图 3B) 显示BPS和DFO处理均可明显减轻肝内炎症评分和肝细胞水肿评分(P < 0.05, P < 0.01), 亦可降低肝细胞脂肪变性评分(P < 0.05)。这些结果表明, BPS可明显改善D-GalN诱导的肝组织损伤。
Perls染色可直接显示肝细胞内铁沉积情况, 本研究结果如图 4所示, 正常大鼠肝内无明显蓝色颗粒, 表明肝组织内无铁沉积。D-GalN损伤后大鼠肝内可见较多深蓝色颗粒, 且与Con组相比, 蓝染阳性面积的百分比明显增加(P < 0.01), 表明肝损伤后细胞内出现铁超载。与D-GalN组相比, BPS和DFO组肝细胞内蓝染颗粒百分比均明显降低(P < 0.01), 表明BPS和DFO干预均可明显降低肝细胞内铁超载, 进而减轻肝细胞铁死亡。
前期研究发现BPS干预大鼠实验性肝损伤后, 明显增加肝组织内转录因子核因子红系2相关因子2 (nuclear factor erythroid 2-related factor 2, Nrf2) 表达[7], 细胞抗氧化和铁代谢均可能由Nrf2靶基因介导。而且Nrf2可调节SLC7A11和GPX4两个铁死亡的关键靶点, 是铁死亡的负性调节因子[13]。因此本研究采用Western blot检测铁死亡相关蛋白的表达, 结果显示BPS和DFO干预后, 大鼠肝脏组织中GPX4和SLC7A11蛋白表达水平均升高(P < 0.05, P < 0.01), 表明BPS可能通过Nrf2调控SLC7A11/GPX4通路而减轻肝细胞铁死亡(图 5)。
我国药用植物及天然产物防治肝病已有悠久历史, 前期通过网络药理学及动物体内实验分析筒鞘蛇菰治疗肝损伤的分子机制, 发现其具有提高抗氧化酶活性作用, 且可通过调控Nrf2通路减轻氧化应激损伤[7]。已报道多种中药成分可通过靶向Nrf2通路调控氧化应激反应发挥护肝作用, 如人参皂苷Rg1、丹酚酸B等[14, 15]。Nrf2-氧化应激通路为调控细胞铁死亡经典信号通路, 然而, 筒鞘蛇菰抗肝损伤作用是否与调控肝细胞铁死亡有关, 尚不清楚。基于前期基础, 本研究选用有效剂量为200 mg·kg-1 BPS溶液进行灌胃实验, 深入探讨其作用机制有重要意义。
铁是机体必需微量元素, 对维持机体健康至关重要, 细胞内铁离子过量会致ROS生成与清除紊乱, GPX4抗氧化清除能力减弱, 进而引起细胞死亡, 这种死亡方式被称为铁死亡[10]。Nrf2作为氧化应激经典通路, 有报道Nrf2激动剂萝卜硫素通过调控SLC7A11和GPX4抑制肝组织铁死亡而减轻小鼠脓毒症肝损伤[16]。Nrf2激活后与其抑制蛋白Keap1解离活化, 进入细胞核, 启动其下游靶基因如SOD、GPX4、SLC7A11和HO-1等表达[17, 18], 进而通过活化细胞膜上SLC7A11而促进胱氨酸转运, 增强GSH的合成和恢复GPX4的活性; 生理条件下, SLC7A11可将胱氨酸转运至细胞内参与清除过多的ROS, 并促进GSH合成, 降低脂质过氧化产物聚集, 从而阻止铁死亡[10]。GPX4是一种GSH依赖性酶, 是铁死亡氨基酸代谢中的重要调控靶点, 选择性催化脂质过氧化物, 抑制或GPX4缺失直接导致脂质过氧化物积累而激活铁死亡。GSH是GPX4发挥作用的必要因子, GSH合成直接影响GPX4的活性; 而且GSH合成又受SLC7A11的调控。因此, 铁稳态失衡、脂质过氧化及SLC7A11/GSH/GPX4轴失衡是铁死亡发生的重要标志[19]
脂质过氧化反应是细胞膜上多不饱和脂肪酸与羟基自由基或脂氧合酶反应生成MDA和4-HNE等脂质过氧化物的过程。本研究结果发现, 损伤大鼠肝内ROS和Fe2+水平升高, Fe2+可通过芬顿反应催化产生过量ROS, 进而促进脂质发生过氧化反应, 表现为4-HNE、MDA和LPO水平增加, 表明D-GalN引起大鼠肝脏出现抗氧化能力下降及脂质过氧化性损伤, 而BPS可明显降低肝内ROS和Fe2+水平, 进而减轻肝损伤, 表明BPS可减轻铁离子依赖性的脂质过氧化产物的积累, 发挥抑制铁死亡作用。总之, 本研究发现BPS可提高实验性肝损伤大鼠肝组织中SLC7A11和GPX4蛋白表达, 增加GSH水平, 减少脂质过氧化物的生成及肝内铁沉积, 改善肝功能, 表明BPS抗肝损伤作用与提高抗氧化能力及减轻肝细胞铁死亡有关, 且可通过影响SLC7A11/GPX4通路来发挥作用, 然而其抑制铁死亡的分子机制有待探讨。
综上所述, 在中医药防治肝病研究方面, 深入研究药物调控不同细胞死亡方式之间的关系对联合用药及更好发挥药效有所帮助。且与经典铁死亡抑制剂相比, 中药活性成分参与调节铁死亡, 具有价廉、来源广及疗效可靠等优势, 进一步发掘调控铁死亡的天然药物具有重要价值。本研究可为临床应用民族药筒鞘蛇菰辅助治疗肝损伤提供实验依据。
