This study aimed to investigate the mechanism of Jiu Wei Bu Xue Oral Liquid on insomnia rats combining the methods of network pharmacology, molecular docking and experimental verification. UPLC-Q-TOF-MS/MS method and TCMIP, TCMSP databases were used to collect the ingredients and targets of Jiu Wei Bu Xue Oral Liquid. Protein-protein interactions and network analysis were performed to screen the key network targets and putative active ingredients of Jiu Wei Bu Xue Oral Liquid in treatment of insomnia, and then following by biological function and KEGG pathway analysis. Then binding ability for key network targets and putative active ingredients were predicted with molecular docking. The prediction targets were validated in para-chlorophenylalanine (PCPA) induced insomnia rats with administration of Jiu Wei Bu Xue Oral Liquid (2, 4, 8 mL·kg-1) for 7 days. Pentobarbital sodium induced sleeping test were performed to evaluate the synergistic sleep-aiding effect of Jiu Wei Bu Xue Oral Liquid. Then glutamic acid (Glu), γ-aminobutyrate (GABA) content and glutamate decarboxylase 1 (GAD67) activity in hypothalamus or hippocampus were evaluated, and the expressions of GAD67, γ-aminobutyric acid receptor subunit α1 (GABRA1) and γ-aminobutyric acid receptor subunit β2 (GABRB2) in hippocampus were detected by qRT-PCR and Western blot methods. Animal experiments were approved by the Institutional Committee on Animal Care of Guangxi Institute of Chinese Medicine & Pharmaceutical Science (the number of permission: 2022060802). Results showed that 16 key network targets and 16 putative active ingredients were obtained by analyzing the herbs-ingredients-targets network of Jiu Wei Bu Xue Oral Liquid in treatment of insomnia. Network pharmacology and molecular docking all indicated these active ingredients, for example atractylenolide Ⅲ, showed better binding ability with GABRA1 and GABRB2. Animal study indicated that, compared to PCPA-induced insomnia model, Jiu Wei Bu Xue Oral Liquid remarkably shortened the sleeping latency and increased the sleeping duration, increased GAD67 activity and the production of GABA in hippocampus of insomnia rats, as well as the expressions of GAD67, GABRA1 and GABRB2, while decreased Glu content in hypothalamus, leading to decreasing of Glu/GABA ratio and recovery of Glu-GABA balance. These results indicated that Jiu Wei Bu Xue Oral Liquid improved insomnia symptoms and helped maintain the Glu-GABA balance within hypothalamus and hippocampus, and reduced the excitatory neurotoxicity within brain. The mechanism may due to the elevation of GAD67 expression and enzyme activity, and the enhancement of type-A GABA receptor (GABAAR)-mediated neurons inhibition.

本研究拟通过网络药理学分析、分子对接结合体内实验验证, 探究九味补血口服液对失眠大鼠的睡眠改善机制。利用UPLC-Q-TOF-MS/MS分析结合数据库获取九味补血口服液的化学成分及其候选靶标, 并通过蛋白质互作和网络分析, 筛选九味补血口服液治疗失眠的关键网络靶标和潜在活性成分, 进行生物功能富集及通路分析。再采用分子对接技术对上述关键靶标和活性成分进行结合能力预测; 通过对氯苯丙氨酸(PCPA) 腹腔注射复制大鼠失眠模型, 给予九味补血口服液(2、4、8 mL·kg-1) 干预7天, 开展戊巴比妥钠阈上剂量诱导睡眠实验, 考察九味补血口服液协同助眠作用。检测海马和下丘脑组织中谷氨酸(Glu)、γ-氨基丁酸(GABA) 含量, 比色法检测海马组织中谷氨酸脱羧酶1 (GAD67) 酶活力, qRT-PCR和免疫印迹法检测海马组织中GAD67、γ-氨基丁酸A受体α1 (GABRA1)、γ-氨基丁酸A受体β2 (GABRB2) mRNA和蛋白质表达水平。动物实验经广西壮族自治区中医药研究院实验动物伦理委员会批准(编号2022060802)。结果表明, 通过构建九味补血口服液中药-成分-失眠疾病靶点网络, 获得该品种治疗失眠的16个关键网络靶标和16个潜在活性成分。网络药理学和分子对接分析显示, 白术内酯Ⅲ等16个潜在活性成分与GABRA1、GABRB2蛋白质具有潜在的结合能力。在动物实验验证中, 与失眠大鼠模型组相比, 九味补血口服液显著缩短失眠大鼠睡眠潜伏期, 延长睡眠时长, 并明显提高其海马组织中GAD67酶活力及GABA生成量, 以及GAD67、GABAA受体亚基GABRA1、GABRB2的mRNA和蛋白质表达水平, 而降低下丘脑组织中Glu含量, 最终降低下丘脑和海马中Glu/GABA比值, 保持脑内Glu、GABA动态平衡。综上表明, 九味补血口服液可以改善失眠大鼠症状, 促进海马、下丘脑内Glu和GABA平衡, 减轻失眠后脑内兴奋性神经毒性, 其机制可能与增强GAD67表达及酶活力, 促进海马GABAA受体亚基GABRA1、GABRB2介导的抑制性神经活动有关。

本研究拟通过网络药理学分析、分子对接结合体内实验验证, 探究九味补血口服液对失眠大鼠的睡眠改善机制。利用UPLC-Q-TOF-MS/MS分析结合数据库获取九味补血口服液的化学成分及其候选靶标, 并通过蛋白质互作和网络分析, 筛选九味补血口服液治疗失眠的关键网络靶标和潜在活性成分, 进行生物功能富集及通路分析。再采用分子对接技术对上述关键靶标和活性成分进行结合能力预测; 通过对氯苯丙氨酸(PCPA) 腹腔注射复制大鼠失眠模型, 给予九味补血口服液(2、4、8 mL·kg-1) 干预7天, 开展戊巴比妥钠阈上剂量诱导睡眠实验, 考察九味补血口服液协同助眠作用。检测海马和下丘脑组织中谷氨酸(Glu)、γ-氨基丁酸(GABA) 含量, 比色法检测海马组织中谷氨酸脱羧酶1 (GAD67) 酶活力, qRT-PCR和免疫印迹法检测海马组织中GAD67、γ-氨基丁酸A受体α1 (GABRA1)、γ-氨基丁酸A受体β2 (GABRB2) mRNA和蛋白质表达水平。动物实验经广西壮族自治区中医药研究院实验动物伦理委员会批准(编号2022060802)。结果表明, 通过构建九味补血口服液中药-成分-失眠疾病靶点网络, 获得该品种治疗失眠的16个关键网络靶标和16个潜在活性成分。网络药理学和分子对接分析显示, 白术内酯Ⅲ等16个潜在活性成分与GABRA1、GABRB2蛋白质具有潜在的结合能力。在动物实验验证中, 与失眠大鼠模型组相比, 九味补血口服液显著缩短失眠大鼠睡眠潜伏期, 延长睡眠时长, 并明显提高其海马组织中GAD67酶活力及GABA生成量, 以及GAD67、GABAA受体亚基GABRA1、GABRB2的mRNA和蛋白质表达水平, 而降低下丘脑组织中Glu含量, 最终降低下丘脑和海马中Glu/GABA比值, 保持脑内Glu、GABA动态平衡。综上表明, 九味补血口服液可以改善失眠大鼠症状, 促进海马、下丘脑内Glu和GABA平衡, 减轻失眠后脑内兴奋性神经毒性, 其机制可能与增强GAD67表达及酶活力, 促进海马GABAA受体亚基GABRA1、GABRB2介导的抑制性神经活动有关。

This study aimed to investigate the mechanism of Jiu Wei Bu Xue Oral Liquid on insomnia rats combining the methods of network pharmacology, molecular docking and experimental verification. UPLC-Q-TOF-MS/MS method and TCMIP, TCMSP databases were used to collect the ingredients and targets of Jiu Wei Bu Xue Oral Liquid. Protein-protein interactions and network analysis were performed to screen the key network targets and putative active ingredients of Jiu Wei Bu Xue Oral Liquid in treatment of insomnia, and then following by biological function and KEGG pathway analysis. Then binding ability for key network targets and putative active ingredients were predicted with molecular docking. The prediction targets were validated in para-chlorophenylalanine (PCPA) induced insomnia rats with administration of Jiu Wei Bu Xue Oral Liquid (2, 4, 8 mL·kg-1) for 7 days. Pentobarbital sodium induced sleeping test were performed to evaluate the synergistic sleep-aiding effect of Jiu Wei Bu Xue Oral Liquid. Then glutamic acid (Glu), γ-aminobutyrate (GABA) content and glutamate decarboxylase 1 (GAD67) activity in hypothalamus or hippocampus were evaluated, and the expressions of GAD67, γ-aminobutyric acid receptor subunit α1 (GABRA1) and γ-aminobutyric acid receptor subunit β2 (GABRB2) in hippocampus were detected by qRT-PCR and Western blot methods. Animal experiments were approved by the Institutional Committee on Animal Care of Guangxi Institute of Chinese Medicine & Pharmaceutical Science (the number of permission: 2022060802). Results showed that 16 key network targets and 16 putative active ingredients were obtained by analyzing the herbs-ingredients-targets network of Jiu Wei Bu Xue Oral Liquid in treatment of insomnia. Network pharmacology and molecular docking all indicated these active ingredients, for example atractylenolide Ⅲ, showed better binding ability with GABRA1 and GABRB2. Animal study indicated that, compared to PCPA-induced insomnia model, Jiu Wei Bu Xue Oral Liquid remarkably shortened the sleeping latency and increased the sleeping duration, increased GAD67 activity and the production of GABA in hippocampus of insomnia rats, as well as the expressions of GAD67, GABRA1 and GABRB2, while decreased Glu content in hypothalamus, leading to decreasing of Glu/GABA ratio and recovery of Glu-GABA balance. These results indicated that Jiu Wei Bu Xue Oral Liquid improved insomnia symptoms and helped maintain the Glu-GABA balance within hypothalamus and hippocampus, and reduced the excitatory neurotoxicity within brain. The mechanism may due to the elevation of GAD67 expression and enzyme activity, and the enhancement of type-A GABA receptor (GABAAR)-mediated neurons inhibition.

失眠属中医“不寐”的范畴, 主要表现为长时间、高频次入睡困难或无法持久维持睡眠等特征。长期失眠可诱发一系列代谢综合征、抑郁、心血管疾病, 严重影响患者正常生活[1, 2]。现代医学研究显示, 失眠疾病机制主要涉及大脑皮层、海马、下丘脑等脑内结构, 与脑内γ-氨基丁酸/谷氨酸(GABA/Glu) 神经递质水平失衡密切相关[3]
九味补血口服液是根据传统中医药学理论和广西壮医药专家多年临床用药经验, 以广西特有草药绞股蓝配以人参、黄芪等9味中药组方, 具有益气补血、健脾益胃功效, 尤其是对气血两虚所致的失眠健忘、心悸怔忡诸证具有良好治疗效果[4]。目前临床上疗效较好, 但由于中药复方成分的复杂性, 其作用机制往往不够明确。网络药理学是一种基于药物成分及作用靶标, 与疾病的相关靶蛋白相互作用网络的分析方法, 能从系统生物学角度出发, 从多组分、多靶点、多途径的整体观揭示中药复杂体系治疗疾病的相关机制, 近年来成为中药研究领域比较常用的研究技术[5]。本研究拟通过网络药理学分析、分子对接结合动物整体实验验证的手段, 深入挖掘九味补血口服液治疗失眠的物质基础及具体作用机制, 为九味补血口服液的临床精准和合理应用提供理论依据。

九味补血口服液治疗失眠的网络药理学分析
九味补血口服液潜在活性成分筛选  采用UPLC-Q-TOF-MS/MS技术对九味补血口服液进行化学成分鉴定。色谱条件: 采用Waters ACQUITY UPLC HSS T3 (2.1 mm × 100 mm, 1.8 μm) 为色谱柱; 正离子模式流动相为水(A1)-乙腈(B), 负离子模式流动相为0.1%甲酸水溶液(A2)-乙腈(B), 梯度洗脱(0~5 min, 2% B; 5~30 min, 2%~80% B; 30~35 min, 80%~95% B); 柱温: 35 ℃; 流速: 0.3 mL·min-1; 进样量: 1 μL; TUV检测波长: 203、254 nm。质谱条件: 离子源为电喷雾电离源(ESI), 正、负离子两种模式分别测定。各参数分别为: 毛细管电压+2.5 kV (ESI+), -2.0 kV (ESI-); 锥孔电压+40 V (ESI+), -40 V (ESI-); 离子源温度140 ℃; 脱溶剂气温度500 ℃; 锥孔气流量50 L·h-1; 脱溶剂气(N2) 流速800 L·h-1; 氩气(Ar) 作为碰撞气体; MSE扫描模式检测, 低碰撞能量通道6 eV, 高碰撞能量通道10~40 eV能量梯度, 质量数检测范围50~1 250 Da。采用Waters Masslynx软件采集数据, 采用Waters UNIFI软件将质谱数据与所建数据库进行自动匹配, 并通过一级和二级质谱信息分析, 结合相关文献及对应标准品, 对九味补血口服液样品图谱进行鉴定和表征。
为了充分挖掘和构建九味补血口服液的成分库, 除上述鉴定所得成分外, 另采用中医药整合药理学研究平台(TCMIP, http://www.tcmip.cn/TCMIP/index.php/Home/Login/login.html)、中药系统药理学平台(TCMSP, https://old.tcmsp-e.com/tcmsp.php) 在线数据库检索获得九味补血口服液(简称: JWBXOL) 组方药材人参、黄芪、绞股蓝、五味子、当归、白术、陈皮、甘草、山楂的成分信息, 利用PubChem数据库(https://pubchem.ncbi.nlm.nih.gov/) 核对成分结构及化合物名称。综合2种方法共同构建九味补血口服液的潜在有效化学成分库, 用于下一步网络药理学分析[6]
药物预测靶点筛选  通过TCMIP数据库预测上述筛选出的化学成分的作用靶标, 筛选相似性阈值大于0.8的靶标作为成分的预测靶标, 构建九味补血口服液所含化合物-作用靶标数据库。
失眠疾病靶标的收集  检索GeneCards (http://www.genecards.org/)、DrugBank (https://go.drugbank.com/)、OMIM (https://www.omim.org/)、DisGeNET (https://www.disgenet.org/) 和TCMIP数据库, 以“insomnia”为筛选关键词, 获取失眠相关的疾病基因信息。去除重复基因后, 得到失眠疾病靶标数据库。
九味补血口服液“中药-化合物-靶标”网络构建与分析  将九味补血口服液成分预测靶点与失眠疾病靶点进行交集, 取交集部分靶点作为药物改善失眠的预测靶标。将上述交集靶标导入STRING平台构建其蛋白互作(PPI) 网络, 并将PPI网络数据导入Cytoscape 3.7.1软件进行网络特征值分析, 以网络节点中的拓扑结构特征值度(degree) 值为重要参考, 选取网络中degree值大于中位数1.5倍的网络节点作为九味补血口服液治疗失眠的关键网络靶标(Hub节点), 用于后续GO分析和KEGG分析。将9种中药及其含有的化合物, 以及Hub节点进一步构建“中药-成分-Hub靶标网络”, 筛选九味补血口服液干预失眠的潜在活性成分及其相应关键网络靶点。
基因本体(GO) 分析和KEGG通路富集分析  利用DAVID数据库(https://david.ncifcrf.gov/) 工具对上述关键网络靶点进行GO分析和KEGG通路富集分析, 获得九味补血口服液治疗失眠的关键靶点蛋白在基因功能和信号通路中的作用。
分子对接   从PubChem数据库中获取潜在活性成分的SDF格式文件, 利用OpenBabel软件转化为PDB格式文件。筛选KEGG分析所获取的关键网络核心靶点, 从AlphaFold数据库(AlphaFold, https://alphafold.ebi.ac.uk//) 中检索其相应的3D结构PDB格式文件。采用AutodockFG进行核心成分与核心靶标间的分子对接验证。利用结合能(affinity) 评估药效分子与靶点的结合能力, 将结合能≤ -7 kcal·mol-1的结合形式纳入选择。
九味补血口服液对失眠大鼠睡眠的改善作用
动物分组、造模及给药  适应性饲养1周后, 大鼠60只, 随机分为6组, 分别为对照组、模型组、阳性药地西泮3 mg·kg-1组及九味补血口服液2、4、8 mL·kg-1剂量组(分别为临床日剂量的4、8、16倍)。除对照组外, 其余5组大鼠在给药前腹腔注射PCPA 450 mg·kg-1, 连续3天, 以复制失眠大鼠模型[7]。造模后, 对所有大鼠腹腔注射35 mg·kg-1戊巴比妥钠溶液, 观察到模型组大鼠的睡眠维持时间较对照组明显缩短且入睡潜伏期延长明显, 显示造模成功。造模成功后, 地西泮组和九味补血口服液组大鼠灌胃给予相应药物, 连续7天。对照组和模型组大鼠给予等量生理盐水。
戊巴比妥钠诱导大鼠睡眠实验  末次给药2 h后, 每组大鼠腹腔注射给予阈上剂量35 mg·kg-1戊巴比妥钠, 观察各组大鼠翻正反射消失和恢复情况, 记录以下数据: ①注射戊巴比妥钠时间; ②入睡时间: 以1 min内翻正反射消失为标准; ③觉醒时间: 以30 s内翻正反射恢复为标准。计算入睡潜伏期和睡眠时长, 入睡潜伏期=入睡时间- 注射戊巴比妥钠时间; 睡眠时长=觉醒时间- 入睡时间[8]
ELISA法检测海马及下丘脑中Glu、GABA含量水平  末次给药24 h后, 以戊巴比妥钠麻醉大鼠, 取大鼠海马及下丘脑组织, 取一定重量组织按1∶9 (w/v) 加入生理盐水, 进行冰浴匀浆, 3 000 r·min-1、4 ℃离心10 min, 收集上清, 按照ELISA试剂盒说明, 检测Glu、GABA含量水平。
比色法检测GAD67酶活力  取一定量海马组织, 按1∶9 (w/v) 加入pH 4.8的磷酸缓冲液, 研磨仪里研磨30 s, 而后10 000 r·min-1、4 ℃离心15 min, 上清液即为粗酶液, BCA法检测其中的蛋白浓度。另取5 mL的EP管中加入0.15 mL粗酶液+ 0.15 mL底物溶液(含30 mmol·L-1 L-Glu和0.2 mmol·L-1磷酸吡哆醛), 混匀, 放入40 ℃烘箱内孵育20 h, 冰浴终止反应。随后在冰浴中依次加入0.2 mol·L-1硼酸盐缓冲液(pH 10) 0.5 mL、6%苯酚1 mL, 混匀, 继续加入次氯酸钠溶液(活性氯为5.2%) 1 mL, 混匀后室温放置5 min, 然后沸水浴10 min, 立即冰浴20 min, 待溶液出现蓝绿色后, 酶标仪测640 nm吸光度。另外, 配制含GABA、L-Glu的混合标准溶液(GABA浓度分别为10、8、4、2、1、0 mmol·L-1), 按上述与硼酸、苯酚、次氯酸钠反应体系方法进行反应, 读取640 nm吸光度, 建立GABA浓度与吸光度的标准曲线。利用标准曲线换算粗酶液中的GABA含量。最后以粗酶液中每mg蛋白通过与底物作用后产生的GABA含量作为GAD67酶活力的表征[9]
qRT-PCR法检测海马中GAD67、GABRA1、GABRB2 mRNA水平  根据网络药理学分析和分子对接所预测的关键网络靶点进行基因水平验证。取大鼠海马组织30 mg置于匀浆管中, 加入Trizol 1 mL充分研磨, 氯仿萃取, 12 000 r·min-1、4 ℃离心10 min。吸取上清液, 加入等量异丙醇, 摇匀后移入-20 ℃冰箱保存30 min。12 000 r·min-1、4 ℃离心10 min, 弃去液体, 加入75%酒精洗涤沉淀, 4 ℃离心10 min, 弃去管中液体, 倒置晾干, 加入无酶水, 测RNA浓度。根据逆转录试剂盒说明书将RNA逆转录为cDNA, 再根据扩增试剂盒说明书进行扩增。GABRA1上游引物: AGTGCCAGAAAT TCCCTCCCAAAG, 下游引物: CAATCAGAGCCGAG AACACGAAGG; GABRB2上游引物: TTCCCTTAAT AATCGCCGCTGTCTG, 下游引物: CAGTCTGTCCA CCGTCTCTTTAACC; GAD67上游引物: CATCATGG CGGCTCGGTACAAG, 下游引物: GTGACTGTGTTC TGAGGTGAAGAGG; β-actin上游引物: TGTCACCA ACTGGGACGATA, 下游引物: GGGGTGTTGAAGGT CTCAAA, 均由生工生物工程(上海) 股份有限公司设计合成。使用2-ΔΔCt法计算基因mRNA相对水平。
免疫印迹法检测海马中GAD67、GABRA1、GABRB2蛋白表达   取上述海马组织, 加入组织裂解液, 提取总蛋白。BCA法测定蛋白浓度, 加入上样缓冲液, 混匀, 95 ℃加热15 min。配胶, 待胶凝固后加入40 μg待测蛋白样本, 电泳, 转膜, 封闭1 h, TBST清洗, 加入GAD67、GABRA1、GABRB2、β-actin等一抗, 4 ℃孵育过夜, TBST清洗, 加入二抗(1∶5 000), 孵育1 h, TBST清洗, 显影, 拍照。Image J获取各条带灰度值, 计算目的蛋白相对于内参β-actin的相对表达量。
统计学分析   实验数据用(x ± s) 表示, 多组样本均数的两两比较采用单因素方差分析, 方差齐性采用LSD检验, 方差不齐采用非参数检验, P < 0.05为差异有统计学意义。
本研究对九味补血口服液进行了UPLC-Q-TOF-MS/MS分析, 从中鉴定了16个药材指标性成分, 如表 1图 1所示。
由于鉴定的成分数量不足以展开充分的网络药理学分析及筛选, 故本研究拟从TCMIP、TCMSP数据库中进一步检索和补充九味补血口服液9味中药的化学成分, 共计373个, 主要为黄酮、皂苷、木酯素、苯酞、有机酸等类型的化合物, 其中人参102个, 甘草103个, 当归49个, 五味子43个, 黄芪39个, 绞股蓝22个, 山楂28个, 陈皮19个, 白术11个。经TCMIP数据库(相似性大于0.8) 预测成分候选靶标, 获得各组方药材预测靶标共计923个, 分别为: 人参783个, 黄芪332个, 绞股蓝156个, 当归265个, 白术91个, 五味子92个, 陈皮82个, 甘草164个, 山楂419个。通过构建中药-成分网络(图 2) 并分析发现, degree值排前5位的成分包括β-谷甾醇(β-sitosterol)、山柰酚(kaempferol)、槲皮素(quercetin)、棕榈酸(palmitic acid)、琥珀酸(succinic acid), 说明这些成分可能是九味补血口服液9味中药协同发挥药效的潜在物质基础。上述成分与中药的对应信息表、中药-成分网络分别如表 2图 2所示。
进一步从DisGenet、OMIM、DrugBank、GeneCards等数据库获取失眠疾病靶点273个, 获取九味补血口服液靶标与失眠疾病靶标的交集, 共计56个。运用STRING数据库构建上述交集靶点PPI网络, 通过Cytoscape 3.7.1软件进行可视化并计算, 显示所有节点的degree值中位数为8.5, 选取degree值> 12.7的靶标节点作为九味补血口服液治疗失眠的关键网络靶标(Hub节点), 共计16个。进一步构建中药-成分-失眠靶点网络和计算, 将中药、关键成分与Hub节点的相互关系进行可视化, 并筛选该网络中degree值> 8 (中位数1.5倍) 的节点, 作为九味补血口服液发挥治疗失眠的关键网络靶标和潜在活性成分。筛选后的网络由9味中药、16个关键网络靶标及16种潜在活性成分组成, 详见图 3。Degree值排名前6的靶点包括: GABRB2、肿瘤坏死因子(TNF)、GABRA1、白介素6 (IL6)、γ氨基丁酸A型受体α2亚基(GABRA2)、γ氨基丁酸A型受体α6亚基(GABRA6)。上述16种活性成分包括: β-谷甾醇、白术内脂Ⅲ (atractylenolide Ⅲ)、菜油甾醇(campesterol)、豆甾醇(stigmasterol)、山柰酚、槲皮素、麦角甾-4, 6, 8, 22-四烯-3-酮(ergosta-4, 6, 8, 22-tetraen-3-one)、齿孔醛(eburical)、齿孔醇(eburicol)、羊毛甾醇(lanosterol)、齿孔酸(eburicoic acid)、脱氢齿孔酸(dehydroeburicoic acid)、熊果酸(ursolic acid)、麦角固醇(ergosterol)、(24S)-乙基胆甾-5, 22, 25-反-3β-醇[(24S)-ethylcholesta-5, 22, 25-trans-3β-ol]、邻甲苯酚(o-cresol)。上述潜在活性成分与中药的对应关系如表 3所示。综上表明, 九味补血口服液的9种中药可能通过16个潜在活性成分与16个关键网络靶标发生相互作用, 充分体现了九味补血口服液是通过多成分、多靶点的方式来干预失眠的。
选取上述筛选出的16个关键网络靶标, 利用DAVID数据库进行生物功能富集及通路分析。选取通路富集分析排名前18的通路(P < 0.01)。结果显示, 九味补血口服液治疗失眠的关键靶标主要富集于尼古丁成瘾、GABA能突触、神经活性配体-受体相互作用、内源性大麻素逆向信号、多巴胺能突触等信号通路。
生物功能富集结果显示, 九味补血口服液治疗失眠的关键网络靶标主要参与的生物学过程包括氯离子跨膜转运、γ-氨基丁酸信号、化学突触传递、神经系统过程、抑制性突触的组装、GABA能突触传递等过程; 在分子功能上, 九味补血口服液主要与GABA门控氯离子通道活性、GABAA受体活性、氯离子通道活性、胞外门控离子通道活性、抑制性胞外媒体门控离子通道活性等相关; 在细胞组分方面, 其主要作用于突触后膜、氯离子通道复合体、GABAA受体复合体、胞浆膜组成、神经投射、胞浆囊泡膜等(图 4)。
为了综合考虑九味补血口服液所有组方中药抗失眠协同作用, 本研究将五味子药材中与GABAergic synapse信号通路关键靶点具有潜在作用的成分五味子丙素(schisandrin C) 也纳入九味补血口服液抗失眠的潜在活性成分, 共计17个。进一步将上述17个潜在活性成分与GABAergic synapse信号通路关键靶点GABRA1、GABRB2靶点进行结合能力预测。一般认为结合能小于-4.25 kcal·mol-1提示配体与受体有一定的结合能力, 小于-5.0 kcal·mol-1有较好的结合能力, 小于-7.0 kcal·mol-1有强烈的结合能力[10]。结果发现, 白术内脂Ⅲ、菜油甾醇、豆甾醇、山柰酚、槲皮素、五味子丙素、麦角固醇、β-谷甾醇等15种成分与GABRA1有强烈的结合能力, 山柰酚、槲皮素、熊果酸、麦角固醇等6种成分与GABRB2有强烈结合能力(表 4), 用PyMOL软件对有强烈结合能力(结合能≤ -7 kcal·mol-1) 的部分成分与靶点对接结果进行可视化(图 5)。
造模给药期结束后, 与对照组相比, PCPA可诱导模型组大鼠睡眠潜伏期明显增长(P < 0.01), 睡眠时长缩短(P < 0.05); 给药7天后, 与模型组比较, 地西泮组和九味补血口服液(2、4、8 mL·kg-1) 干预组大鼠睡眠潜伏期显著缩短, 睡眠时长明显增加(P < 0.05或P < 0.01)。表明九味补血口服液对戊巴比妥钠具有协同对抗失眠作用(表 5)。
根据网络药理学分析和分子对接的预测结果, 九味补血口服液抗失眠作用可能与GABA能神经突触相关, 结合失眠疾病机制的相关研究, 提示九味补血口服液有调控GABA、Glu神经递质稳态的潜在可能。检测结果显示, 与对照组比较, PCPA诱导失眠大鼠海马组织中GABA含量显著降低(P < 0.01), Glu/GABA比值显著升高(P < 0.01), Glu含量有所升高, 但未达到显著性意义(P > 0.05); 给药7天后, 与模型组相比, 九味补血口服液(2、8 mL·kg-1) 可明显逆转失眠大鼠海马中GABA含量及Glu/GABA比值变化(P < 0.05) (表 6)。
结果还显示, PCPA诱导失眠大鼠下丘脑组织中Glu含量及Glu/GABA明显升高(P < 0.01), GABA含量有所波动, 但未达到显著性意义(P > 0. 05); 给药7天后, 与模型组相比, 地西泮、九味补血口服液(2、4、8 mL·kg-1) 可明显下调失眠大鼠下丘脑组织中Glu含量和Glu/GABA比值变化(表 7)。
GAD67蛋白是Glu转化为GABA的关键酶, 检测GAD67酶活力可以有效评估九味补血口服液对GABA合成的影响。比色法检测结果显示, PCPA可显著减少一定重量海马组织经GAD67催化生成GABA的含量(P < 0.01)。与模型组相比, 九味补血口服液(2、8 mL·kg-1) 可明显提高失眠大鼠海马组织中经GAD67催化生成GABA含量(P < 0.05), 表明九味补血口服液(2、8 mL·kg-1) 可明显提高海马组织中GAD67酶活力, 促进GABA的合成(表 8)。
GABA通过与其相应受体结合, 如GABAA受体中的αβ亚基(由GABRA1、GABRB2等基因编码), 可介导对神经兴奋的抑制。PCR结果显示, PCPA可诱导模型组大鼠海马组织中GAD67、GABRB2、GABRA1 mRNA水平降低, 提示PCPA可能通过抑制GABA合成及其受体介导的神经抑制作用, 进而诱导动物神经兴奋, 产生失眠样行为。给药7天后(图 6), 与模型组相比, 九味补血口服液(8 mL·kg-1) 和地西泮可显著性升高海马中GAD67、GABRB2、GABRA1 mRNA相对水平(P < 0.05或P < 0.01)。结合分子对接的结合力分析, 提示九味补血口服液可能正调控失眠大鼠海马区GABA合成及其神经抑制信号的转导。
结果显示(图 7), 与对照组相比, 模型组大鼠海马中GAD67、GABRB2、GABRA1蛋白表达均出现了明显下调(P < 0.01)。与模型组相比, 九味补血口服液(2、4、8 mL·kg-1) 可不同程度升高海马中GAD67、GABRB2、GABRA1蛋白表达(P < 0.05或P < 0.01)。表明九味补血口服液可增强GABA神经递质的合成, 促进GABA与受体结合所介导的兴奋抑制作用。该结果与海马GABA含量水平及mRNA水平结果基本吻合。
九味补血口服液主要由人参、黄芪、绞股蓝、五味子、当归、白术、陈皮、山楂、甘草这9味中药组方而成, 目前临床上主要用于气血两虚所致的失眠健忘、心悸怔忡诸证。本研究网络药理学研究初步揭示, 九味补血口液中可能通过β-谷甾醇、白术内脂Ⅲ、菜油甾醇、豆甾醇、山柰酚、槲皮素等16个化合物发挥抗失眠作用, 其主要作用靶标包括GABRB2、TNF-α、GABRA1、IL6、GABRA2、GABRA6等16个。GO分析结果显示, 九味补血口服液药效成分作用靶点主要涉及GABA能突触传递、GABA门控氯离子通道活性等生物功能, 这些与临床上失眠症的治疗思路相契合, 如苯二氮卓类药物。网络药理学分析和分子对接结果显示, GABRA1和GABRB2可能是九味补血口服液发挥抗失眠作用的关键靶点, 由于GABRA1和GABRB2基因分别编码GABAA受体的αβ亚基, 表明九味补血口服液可能通过调控GABAA受体所介导的神经抑制作用, 进而发挥抗失眠作用。
研究认为, 睡眠-觉醒机制的稳态受GABA和Glu水平双重调节, Glu-GABA动态失衡可能进一步诱发失眠[11]。兴奋性神经递质Glu通过与其相应受体结合发挥神经兴奋作用[12]。GABA神经元是脑内重要的抑制性神经元, 主要分布在大脑皮层、海马、下丘脑等部位。Glu通过谷氨酸脱羧酶(GAD65和GAD67) 合成GABA, 并通过囊泡释放, 与突触后膜受体结合而发挥神经抑制作用[13]。脑内参与睡眠节律调控的GABA受体主要有γ-氨基丁酸A型受体(gamma-aminobutyricacid type A receptor, GABAAR), 这是一类具有5次跨膜结构的配体门控离子通道蛋白, 由αβγ等8种亚基组成, 受GABRA1、GABRA2、GABRB2、GABRB3等19种基因编码[14, 15]。有研究指出, 失眠大鼠海马和下丘脑中均出现GABAAR水平下调的情况, 通过上调GABAAR水平, 可加强大脑抑制性神经元活动和减弱兴奋性神经元的活动, 有利于缓解失眠症状[16, 17]
本研究通过戊巴比妥钠阈上剂量刺激诱导睡眠实验发现, 九味补血口服液(2、4、8 mL·kg-1) 能够缩短模型大鼠的睡眠潜伏期, 延长大鼠的睡眠时间, 提示九味补血口服液(2、4、8 mL·kg-1) 能缓解大鼠的失眠症状。进一步对失眠大鼠海马和下丘脑组织进行检测, 显示PCPA可下调海马组织中GAD67酶活力并减少GABA合成, 升高下丘脑中Glu含量, 进而提高海马和下丘脑内Glu/GABA比值, 表明PCPA诱导了脑内神经递质水平失衡。九味补血口服液通过增强海马组织GAD67酶活力并升高海马中GABA含量, 下调下丘脑Glu含量, 降低Glu/GABA比值, 进而逆转海马和下丘脑中GABA和Glu动态紊乱, 维持兴奋性和抑制性神经递质的平衡。由于网络药理学分析显示九味补血口服液在GABA能信号通路上具有显著富集, 结合神经递质GABA含量在海马和下丘脑组织中含量变化, 本研究进一步选择在海马组织中对GABA能信号通路相关靶点表达水平进行验证。结果显示, 九味补血口服液可以有效上调GABA合成酶—谷氨酸脱羧酶GAD67蛋白、GABAAR受体中α亚基(GABRA1)、β亚基(GABRB2) 的蛋白和mRNA表达水平, 促进抑制性神经活动, 进而缓解失眠症状。该结果也较好地验证了网络药理学和分子对接的预测结果。
本研究还发现了九味补血口服液在对戊巴比妥钠诱导的睡眠时长、海马区GABA含量、GAD67酶活力等干预作用表现出剂量非依赖性, 其原因可能与中药复方基于多成分、多作用靶点产生的药理多效性有关。复方中可能含有相互协同或者相互拮抗的多种成分, 当剂量叠加时, 这些成分所发挥的药效可能增加、维持或者抵消, 故而呈现出较为复杂的剂量与效应关系。
综上所述, 本研究通过网络药理学分析、分子对接结合体内研究的手段, 初步阐明了九味补血口服液通过增强GAD67活力及其介导的GABA的合成, 维持脑内GABA和Glu的动态平衡, 并通过上调GABAAR表达, 促进抑制性神经活动, 缓解大鼠失眠症状。该研究将有助于充实九味补血口服液治疗失眠的相关研究, 为九味补血口服液的临床应用提供更多实验依据。
