Through literature analysis, the advantages of oral pharmaceutical vehicles in solving the temporary dispensing problems for special populations are summarized. Detailed introduction is made on the research, development, production, use, and regulatory experience of oral pharmaceutical vehicles in the United States, as well as the current research progress, achievements, and challenges faced by domestic oral pharmaceutical vehicles. The United States has formed a mature system in the field of oral pharmaceutical vehicles, and China can learn from the experience of the United States to improve relevant laws and policies, promote the establishment of technical guidance principles and technical standards for the use of oral pharmaceutical vehicles in temporary dispensing, and accelerate its clinical use.

Through literature analysis, the advantages of oral pharmaceutical vehicles in solving the temporary dispensing problems for special populations are summarized. Detailed introduction is made on the research, development, production, use, and regulatory experience of oral pharmaceutical vehicles in the United States, as well as the current research progress, achievements, and challenges faced by domestic oral pharmaceutical vehicles. The United States has formed a mature system in the field of oral pharmaceutical vehicles, and China can learn from the experience of the United States to improve relevant laws and policies, promote the establishment of technical guidance principles and technical standards for the use of oral pharmaceutical vehicles in temporary dispensing, and accelerate its clinical use.

根据2020年的统计数据显示, 美国儿童和老年人占总人口的比例高达35%[1]。这些特殊用药人群面临着独特的临床需求, 例如, 无法正常吞服药片或胶囊、可能需要更精准的剂量调整等。其中, 不同年龄段儿童对不同剂型的接受能力存在差异, 对药物的吸收、分布也存在显著差异, 基于公斤体重的简单剂量计算常常会导致用量不足或过量, 从而影响药效和安全性[2]。此外, 部分患者可能对某些药物中的一些成分产生如过敏等不良反应。市场上的药品规格不足, 无法满足儿童需要精准分剂量的需求, 也是一个亟待解决的问题。
面对这些特殊用药需求, 美国采取临时调配药物(compounded medication) 的方法来解决, 根据患者的年龄段、生理特征和疾病情况来确定剂型、剂量和服用方式[3], 以满足个性化的用药需求。我国目前主要采取药物混合、研磨分包、药物切分等方式来适应患者的特殊需求。
美国食品药品监督管理局(Food and Drug Administration, FDA) 将调配药物定义为由执业药师、执业医师或外包机构将药物的成分进行混合、更改以满足单个患者需求的制剂[4]。为了提供最稳定的调配药物, 药师在确认上市药品的可用性后, 可以采用将该药品进行拆分剂量的方式来调配药物。拆分剂量是指通过物理切割、研磨或称重等方式, 将药品的最小规格剂量单位进一步拆分成更小的剂量, 以满足患者个性化用药需求, 提高用药的安全性和有效性。这些已批准药品的有效性和稳定性已经被FDA审核通过, 因此, 认为使用商品药进行拆分剂量是一种可取的做法[5]。口服溶剂介质是药物制剂中用于实现拆分剂量的重要载体之一。
口服溶剂介质(oral pharmaceutical vehicle) 是一种用于口服药物制剂的液体基质, 旨在溶解、稀释或悬浮药物成分, 同时改善药物的适口性、稳定性和患者依从性[6]。口服溶剂介质主要成分包括纯化水和甘油等溶剂, 用于提供液体基质; 蔗糖、山梨醇或糖精钠等甜味剂, 以增强适口性; 微晶纤维素-羧甲基纤维素钠、黄原胶等增稠剂和悬浮剂, 确保药物均匀分布并防止沉淀; 柠檬酸、樱桃香精等矫味剂, 用于改善口感; 以及苯甲酸钠、对羟基苯甲酸酯类(甲酯、丙酯) 等防腐剂, 以防止微生物污染。例如, Padagis公司的Ora-Sweet主要由纯化水、蔗糖、甘油、柠檬酸、苯甲酸钠和樱桃香精组成[7], 而Ora-Sweet SF采用糖精钠代替蔗糖以提供无糖配方[8], Ora-Plus则额外含有悬浮稳定剂以确保难溶性药物的均匀分布[9]。这些成分的合理配比使口服溶剂介质在药物配制中具有良好的稳定性和依从性。口服溶剂介质的药物体系分散均一性越好, 用药剂量越精准; 对于吞咽困难或对某些药物成分存在特殊限制的患者, 口服溶剂介质在一定程度上也可以提供便利。具体而言, 这种调配方式尤其适用于儿童患者, 用于解决儿童用药中精准性和顺应性的两大难点问题。
最早的口服溶剂介质于1996年由Perrigo公司研制, 在美国首次上市。经过近30年的发展, 美国已有多家医药公司上市了口服溶剂介质。
口服溶剂介质被归类为“Ⅳ”类辅料[10](表 1), 按照美国药物主控档案(Drug Master Files, DMF) 制度进行登记备案。DMF制度是药物主控档案管理系统, 用于监管药品的研发、生产和流通等环节。在中国, 类似的管理制度是国家药品监督管理局药品审评中心(Center for Drug Evaluation, CDE) 辅料备案系统。
口服溶剂介质在生产、上市和使用调配的整个过程中都受到FDA的严格监管, 它所涉及到的监管法规包括DMF、CGMP (Current Good Manufacture Practices, 动态药品生产管理规范)、外包机构信息(Outsourcing Facility Information)、联邦法规(Code of Federal Regulations)、FDA 503A、FDA 503B。如产品出现违反法规的情况, FDA将采取相应的处罚措施, 包括无题信函、监管会议、警告信、警告停业函和禁制令等[11]
DMF“Ⅳ”类辅料的注册审批流程主要包括以下几个步骤(图 1): ①资料准备与备案: 将产品相关信息资料按照DMF“Ⅳ”类辅料的要求进行登记备案, 其涉及完整的技术、生产和质量控制数据的提交; ②资料审查: FDA收到申报资料后, 审查人员对资料的完整性进行审查, 此时状态为“Ⅰ”状态(inactivate, 未激活状态); ③行政信息公布与状态激活: FDA在官网公布DMF序列号及持有人等相关行政信息, DMF行政备案成功, 激活为行政“A”状态(activate, 激活状态), 这意味着制药公司在进行联合申报的时候, 可以引用该公司的申报资料; ④资料更新: FDA要求备案人按照DMF“Ⅳ”类要求定期主动更新资料, 如未及时更新, 状态将会被退回“Ⅰ”状态; ⑤制剂企业关联申报: 当制剂企业在FDA官网上搜寻到该辅料信息时, 考虑自身药物品种的情况, 并联合口服溶剂介质进行前期的药学研究。制剂企业在研究中证明口服溶剂介质和该品种的安全性后, 将自身药物品种的制剂资料关联口服溶剂介质的技术资料一起递交给FDA。如果制剂企业的品种通过了FDA的审查, 那么将允许口服溶剂介质进行应用; 如若技术信息不完整, 审查不通过, 制剂企业则需要根据FDA的意见进行相关修订和信息补充。
在药物调配过程中, 为确保操作的规范性和安全性, 美国卫生系统药师协会特别制定了《溶媒临配指南书》 (Extemporaneous Formulations)[12]及《美国卫生系统药师协会——临配制剂技术标准》 (American Society of Hospital Pharmacists: Technical Assistance Bulletin on Compounding Nonsterile Products in Pharmacies) 等参考资料[13]。《溶媒临配指南书》包含了129种药物与溶媒的配方信息, 内容全面, 涵盖了临配方法、储存条件及有效期等关键内容。《临配制剂技术标准》则更深入地探讨了药物调配的各个方面, 从基本的调配设施设备, 到成分的选择、培训、文件和记录的保存方式, 再到稳定性、使用期限、包装、标签、有限批次配制都提出了明确的要求。
美国Perrigo公司Ora系列口服溶剂介质已有108种药物制剂配置记录和稳定性研究数据[14]。这些配制方法在《溶媒临配指南书》、USP/NF (美国药典/国家处方集) 等中都有所体现。
图 2所示, FDA对药物是否可进行调配做了非常详细的规定[15]。首先, 需要评估市场上是否有等效的上市药品。如市场上没有等效的上市药品, 如患者所需的药物已停止上市, 或被列入FDA药品短缺列表中, 则该药物可以根据FDA 503A法规进行调配; 当存在等效的上市药品时, 则需要进一步考察所需调配的药物与市售药的主要成分、规格或给药途径是否相同, 如存在不同, 则该药物可以根据FDA 503A法规进行调配。目前国内儿童用药临配制剂所面临的大部分是这种情况, 如螺内酯, 针对不同患儿的需求, 它可能被拆分成1/2、1/4、1/8等剂量。虽然螺内酯有市售药物, 也不在药品短缺列表中, 但儿童患者需要的剂量规格与市售成人药不同, 在此种情况下, 螺内酯就可以进行调配。此外, 如果市售药品仍在销售, 也不在FDA药品短缺列表中, 而且主成分、规格及给药途径都能满足患者的需求, 则需要考虑市售药物中含有的辅料是否会使个体患者产生不良反应。如果产生不良反应, 则根据FDA 503A法规, 调配机构可以为这部分特定患者进行个性化调配。例如, 已批准了的儿童专用退烧药布洛芬混悬液中所含的色素或香精可能会在极个别儿童患者中引发过敏反应, 这种情况下, FDA允许调配机构为这类儿童患者进行个性化调配。最后, 如果所需进行调配的药物有等效上市药品在售, 其也不在FDA药品短缺列表中, 且主成分、规格及给药途径都能满足患者的需求, 并且患者对市售药物中含有的辅料也不会产生不良反应, 那么就不能进行调配。因为已有市售药物就能满足患者的要求, 这种情况下患者可以直接购买市售药物来接受治疗。
图 3所示[16], 医师根据患者情况开具处方, 临床药师对该处方进行审核, 以判断该药物是否需要进行调配。如判断为无需调配, 则患者可凭处方在药房或者药店直接购买市售药物。若需进行调配, 则根据处方开具情况分为处方药或非处方药, 对于处方药, 根据其是否属于无菌制剂, 调配过程也有所不同。如处方药属于无菌制剂, 则只能在FDA注册的外包机构进行调配, 这类外包机构受到FDA 503B法规的监管。药物调配完成后, 需将调配完成的药物运送至医疗机构及医疗从业人员手中, 后续由医护人员按要求执行药事操作。如处方药属于非无菌制剂, 则可在FDA注册的外包机构和未在FDA注册的非外包机构进行调配。对于非处方药, 其调配可以在非外包机构中进行。非外包机构受到FDA 503A法规的监管, 并由美国药房理事会(National Association of Boards of Pharmacy, NABP) 进行管理。完成调配后, 药物可被运送至医疗机构或医疗从业人员手中, 或由药师直接发放给患者。
FDA设立了卓越配置中心(Compounding Quality Center of Excellence)[17], 以支持外包机构和利益相关者为有需求的患者提供高质量的药物。该中心主要通过以下3种途径提升调配药物的质量: ①市场调研: 包括进行数据分析及和利益相关者进行对话, 深入了解调配药物的安全性、有效性和市场需求; ②培训: 采用自主在线微课程及演示文稿等多种形式进行培训, 包括CGMP培训、洁净室操作培训、无菌处理等, 确保从业人员具备专业知识和技能; ③年度会议: 定期举办主题讨论会议, 邀请行业专家和利益相关者共同参与, 交流经验, 探讨热点问题, 进一步推动调配药物质量管理的提升。
患者有多种途径可以获取口服溶剂介质。混药前的口服溶剂介质可直接在线下医院药房或药店购买, 这些场所通常有专业的医师或药师提供咨询和指导。随着电子商务的兴起, 也可以在亚马逊、eBay等线上平台购买。除此之外, 患者也可以将医师或药师开具的处方提交至调配服务机构。这些机构需要根据处方上的要求进行药物的调配, 确保药物剂量、用法和安全性等遵循医嘱。
FDA对于混药前后的口服溶剂介质都建立了相应的监管报告制度。医疗保健专业人士, 如医生、药师、护士等, 以及消费者和外包机构, 均有责任和义务提交不良反应报告。不良事件包括产品质量问题(如污染、颗粒物或pH问题)、调配机构的卫生条件、配制者对调配药物做出虚假或误导性声明、使用不合格原料药进行调配等[18], 每半年提交一次报告[19]。除此之外, 外包机构还需根据《联邦法规》的规定, 向药物警戒办公室提交不良事件报告[20], 以确保调配药物的安全性和有效性得到及时监控和处理。
2022年4月, Fagron公司收到3起关于SyrSpend SF Cherry口服溶剂介质存在不良气味的投诉, 因此对两批SyrSpend SF Cherry产品进行召回[21]
根据FDA发布的调配药物不良反应的统计数据显示[18], 2017年共调查了59家外包机构, 这些机构在半年内生产了1 200多万例的调配药物, 2017年全年不良事件或投诉报告数据为197例, 这意味着调配药物的不良事件报告率不超过十万分之二(0.001 45%)。横向对比往年数据, 2019年的不良事件数量不超过300例[22], 这表明在FDA的监管和临床配制指南的指导下, 调配药物的不良事件的发生率较低。
我国老年人口规模总体呈上升趋势, 60岁及以上老年人口达2.64亿, 占总人口比例18.70%, 与此同时, 儿童人口规模达2.98亿, 占比21.1%[23, 24]。这两大特殊用药群体在药物治疗领域面临共性挑战, 即传统剂型的剂量适配难题。
老年患者受吞咽功能及代谢能力下降的影响, 通常需要对片剂进行拆分。而儿童用药领域存在更为严峻的药品短缺问题, 国家药品监督管理局2022年数据显示, 儿童专用化学药品制剂占比不足2%, 临床基本药物中儿童专用剂型比例低于10%[25]。临床实践中普遍采用的磨粉分包、药片切割等传统方法存在显著缺陷: 低剂量药物拆分误差较大, 药物晶型破坏导致生物利用度下降, 操作过程辅料暴露于空气中引发的适口性降低等问题严重影响用药安全性与依从性。
近年来, 国家出台了多项鼓励儿童用药申报及优先审评审批的政策, 以满足儿科临床用药需求。在一系列利好政策的支持下, 口服溶剂介质的临床应用优势逐步受到关注, 首都医科大学附属北京儿童医院药学团队与浙江贝灵生物医药有限公司合作开展口服溶剂介质开发和临床应用探索工作, 已成功仿制Padagis厂家的Ora-sweet SF (果味口服溶剂介质) 和Ora-plus (混悬口服溶剂介质), 并在CDE完成备案登记(状态为“Ⅰ”)。
本课题组通过系统研究建立了口服溶剂介质的质量控制体系, 首先进行空白口服溶剂介质的质量属性研究, 对其物理化学性质进行检验(pH值、黏度、密度等), 确保符合GMP规范, 通过加速和长期稳定性试验验证其在不同储存条件下的质量稳定性。研究数据表明, 自主研发的口服溶剂介质质量稳定可靠。随后进行了一系列针对调配后药物质量属性的研究, 课题组评估了药物在介质中的稳定性、含量均匀度和适口性。结果显示, 调配后药物在有效期内保持稳定, 药物液体的上中下层分布均匀性良好[26], 通过电子舌技术优化口感后儿童接受度显著提升。
在临床转化方面, 课题组构建了涵盖45种常见药物的临时调配清单, 并且参照ASHP (American Society of Hospital Pharmacists) 技术标准制定了《药物与口服溶媒配置操作规程》, 为其临床应用提供了理论参考。
口服溶剂介质分剂量技术在提高儿童用药精准性、依从性、安全性等方面具有显著优势, 尽管当前面临诸多挑战, 但随着国家政策的推动和技术的发展, 未来这一领域仍然展现出较大的发展空间。
口服溶剂介质在国内的临床应用需要进一步探讨和研究, 未来研发企业可与相关监管机构、专业协会和行业组织密切协作, 加强研究、交流和合作, 推动临时调配技术指导原则及标准的建立与完善, 并制定适合我国国情的法规和研发路径, 从而为提升儿童用药的安全性和有效性提供有力保障。
