To explore the standardization of the in vitro Nb2-11 cell proliferation bioassay for recombinant human growth hormone (rhGH), we first tested the biological activity of 2 batches of rhGH drug substances, 3 batches of rhGH for injection, and 24 batches of rhGH injections, based on which we proposed the experimental effective criteria. Furthermore, we conducted methodological validation in accordance with General Rule 9401 of the 2020 edition of the Pharmacopoeia of the People's Republic of China. Subsequently, the inter-laboratory consistency, intra-laboratory and inter-laboratory precision of seven labs were studied by determining the in vitro bioactivity of six batches of rhGH products and two batches of rhGH drug substances from four different manufacturers using this method. The consistency of the in vitro and in vivo bioassays was assessed by evaluating 28 batches of rhGH drug substances with both methods. Finally, multiple batches of rhGH samples with different expiration dates were tested to determine the standard limits of the bioassay. The results of this collaborative study indicate that the Nb2-11 cell bioassay exhibits good intra- and inter-laboratory reproducibility, with a 100% pass rate for the experimental effective criteria. The Nb2-11 cell bioassay had high consistency with in vivo animal bioassay. Animal experiments were approved by the Experimental Animal Welfare Ethics Committee of China National Institute of Food and Drug Control (approval number: NIFDC (Fu) No. 2024 (B) 004). This method is applicable to rhGH drug substances and products from different manufacturers. Collectively, the Nb2-11 cell bioassay can be used as a standardized method for determining the biological activity of rhGH products, replacing in vivo animal methods for quality control and release testing.

为探索基于Nb2-11细胞增殖的重组人生长激素(rhGH) 体外生物学活性测定方法的标准化, 本研究首先检测了2批rhGH原液, 3批注射用rhGH和24批rhGH注射液的生物学活性, 初步制订了实验有效标准。进一步按照《中华人民共和国药典》2020年版通则9401进行方法学验证。随后联合7家实验室采用本方法测定来自4家不同生产企业的6批制剂及2批原液的体外生物学活性, 分别对实验室间一致性, 实验室内和实验室间精密度进行了研究。用本方法和体内动物法分别检测28批rhGH原液, 进行体内外结果一致性研究。最后检测多批不同效期rhGH样品, 进行标准限度研究。联合验证结果表明, 本方法具有良好的实验室内及实验室间重现性, 拟订实验有效标准通过率为100%。本方法与体内动物法检测结果具有良好的一致性。动物实验获得中国食品药品检定研究院实验动物福利伦理委员会批准[批准号: 中检动(福) 第2024 (B) 004号]。本方法适用于国内外不同生产厂家的rhGH原液与制剂, 可作为测定rhGH类产品生物学活性的标准化方法替代体内动物法应用于rhGH的质量控制和放行检验。

为探索基于Nb2-11细胞增殖的重组人生长激素(rhGH) 体外生物学活性测定方法的标准化, 本研究首先检测了2批rhGH原液, 3批注射用rhGH和24批rhGH注射液的生物学活性, 初步制订了实验有效标准。进一步按照《中华人民共和国药典》2020年版通则9401进行方法学验证。随后联合7家实验室采用本方法测定来自4家不同生产企业的6批制剂及2批原液的体外生物学活性, 分别对实验室间一致性, 实验室内和实验室间精密度进行了研究。用本方法和体内动物法分别检测28批rhGH原液, 进行体内外结果一致性研究。最后检测多批不同效期rhGH样品, 进行标准限度研究。联合验证结果表明, 本方法具有良好的实验室内及实验室间重现性, 拟订实验有效标准通过率为100%。本方法与体内动物法检测结果具有良好的一致性。动物实验获得中国食品药品检定研究院实验动物福利伦理委员会批准[批准号: 中检动(福) 第2024 (B) 004号]。本方法适用于国内外不同生产厂家的rhGH原液与制剂, 可作为测定rhGH类产品生物学活性的标准化方法替代体内动物法应用于rhGH的质量控制和放行检验。

To explore the standardization of the in vitro Nb2-11 cell proliferation bioassay for recombinant human growth hormone (rhGH), we first tested the biological activity of 2 batches of rhGH drug substances, 3 batches of rhGH for injection, and 24 batches of rhGH injections, based on which we proposed the experimental effective criteria. Furthermore, we conducted methodological validation in accordance with General Rule 9401 of the 2020 edition of the Pharmacopoeia of the People's Republic of China. Subsequently, the inter-laboratory consistency, intra-laboratory and inter-laboratory precision of seven labs were studied by determining the in vitro bioactivity of six batches of rhGH products and two batches of rhGH drug substances from four different manufacturers using this method. The consistency of the in vitro and in vivo bioassays was assessed by evaluating 28 batches of rhGH drug substances with both methods. Finally, multiple batches of rhGH samples with different expiration dates were tested to determine the standard limits of the bioassay. The results of this collaborative study indicate that the Nb2-11 cell bioassay exhibits good intra- and inter-laboratory reproducibility, with a 100% pass rate for the experimental effective criteria. The Nb2-11 cell bioassay had high consistency with in vivo animal bioassay. Animal experiments were approved by the Experimental Animal Welfare Ethics Committee of China National Institute of Food and Drug Control (approval number: NIFDC (Fu) No. 2024 (B) 004). This method is applicable to rhGH drug substances and products from different manufacturers. Collectively, the Nb2-11 cell bioassay can be used as a standardized method for determining the biological activity of rhGH products, replacing in vivo animal methods for quality control and release testing.

重组人生长激素(recombinant human growth hormone, rhGH) 是通过基因重组技术在体外表达的人生长激素蛋白药物。rhGH与人脑垂体产生的生长激素化学结构完全一致, 生理、药理作用也保持一致, 可明显促进矮小儿童的身高增长, 并改善其全身各器官组织的生长发育[1-3]。随着人们对rhGH功能认识的不断提高, rhGH药物的应用也正日益增加[4], 市场规模正在迅速增长, 预计到2030年全球rhGH市场价值将增至92亿美元[5]。我国是目前全球第一大rhGH用药国。截至2024年7月, 国家药品监督管理局已批准5个国内厂家的35个不同规格的rhGH产品注册, 其中4个厂家的30个产品已上市, 分为注射用rhGH和rhGH注射液两种剂型, 批准的适应症已达12个; 另外还批准了2个国外厂家的11个rhGH产品在我国注册。
rhGH生物学活性与其结构功能密切相关, 生物学活性是评价rhGH有效性的关键指标。《中华人民共和国药典》2020版(ChP2020) 收载rhGH生物活性测定方法仅有体内动物法[6], 要求对rhGH原液进行至少每年一次生物学活性测定, 限度要求应不低于2.5 U·mg-1。该方法需要构建去垂体大鼠实验模型, 需要自幼龄(26~28天) 大鼠下颌去除脑垂体, 且不能碰触其他脑组织, 不能有垂体残留, 手术操作繁琐且要求极高, 很多已上市企业都不具备相关检测技术。而且由于术后动物存活率通常不超过50%, 每检测一批rhGH样品的生物学活性都需要消耗上百只大鼠, 不符合“3R”原则要求[7]。术后大鼠还需要恢复2~3周才能用于实验, rhGH的活性测定还需要连续给药6日, 整个实验耗时3~4周, 很难满足常规生物学活性测定的要求。欧洲药典(EP) 从2000年第3版删除体内动物法, 美国药典(USP) 也从2023版删除了体内法, 仅保留体外细胞法, 要求对rhGH原料药或成品进行生物鉴别, 生物学活性应不低于2.0 USP somatropin U·mg-1
为建立高效、简便的rhGH体外活性替代法, 前期研究建立了基于Nb2-11细胞增殖的rhGH体外活性测定法, 该方法的原理是大鼠淋巴瘤细胞Nb2-11表达催乳素受体(prolactin receptor, PRLR), rhGH可以与PRLR结合促进Nb2-11细胞增殖[8]。Nb2-11细胞法不仅可缩短检测时间至2天, 而且检测结果与体内动物法有良好的一致性[9]。为使该方法能更好地应用于rhGH类产品, 基于国家药典委员会《基于Nb2-11细胞增殖的人生长激素生物学活性测定方法研究》课题, 本研究探索了Nb2-11细胞法标准化应用的可行性。首先根据29批rhGH产品的检测结果初步拟定了实验有效标准, 随后征集了国内外已上市8批rhGH产品, 邀请相关产品生产企业、省级药检机构等7家实验室开展实验室间联合验证, 根据7个实验室的测定结果, 对实验室间测定结果的一致性及精密度进行分析, 又用体内动物法和体外细胞法分别检测了28批rhGH原液的生物学活性, 研究了体外细胞法替代体内动物法的可行性。最后征集协作研究单位近、中、远效期共49批rhGH样品的检测结果, 初步拟订了方法的标准限度。本研究为rhGH产品质量控制和放行检验提供了新的标准化体外活性测定方法。
仪器  多功能酶标仪(美国Molecular Devices公司, 内置Softmax Pro 7.1软件); TC-20全自动细胞计数仪(美国Bio-Rad公司); 微孔板振荡器(德国IKA公司); Enspire多功能酶标仪(美国PerkinElmer公司, 内置软件版本4.13.3005.1482)。
供试品与试剂  CellTiter-Glo®发光法细胞活力检测试剂盒(cell Titer-Glo luminescent kit, 批号0000504104)、Fischer's培养基(批号2381043)、胎牛血清(FBS, 批号2236255CP)、马血清(HS, 批号1763585)、β-巯基乙醇(批号2279505)、L-谷氨酰胺(批号2192423)、磷酸盐缓冲液(PBS, 批号2277093), 购于美国Gibco公司。人生长激素标准品(批号140635-202206, 比活性为3 IU·mg-1) 由中国食品药品检定研究院提供。rhGH供试品共35批为本实验室留存, 包括5批rhGH原液(上海联合赛尔2批、长春金赛1批、安徽安科2批), 5批注射用rhGH (上海联合赛尔2批、长春金赛3批) 和25批rhGH注射液(上海联合赛尔4批、长春金赛6批、安徽安科2批、诺和诺德7批, 中山海剂6批)。其中用于联合验证的供试品B1和B2为厂家B的注射用rhGH, C1和C2为厂家C的rhGH注射液, D1和D2为厂家D的rhGH原液, E1和E2为厂家E的rhGH注射液。近、中、远效期的rhGH样品为各协作研究单位留存。牛血清白蛋白冻干粉(北京经科化学试剂公司进口分装); 氯化钠注射液(石家庄四药有限公司, 批号1910113203)。Nb2-11细胞株购自欧洲认证细胞培养物收藏中心(ECACC)。
实验动物  SPF级SD雄性大鼠, 出生日龄26~28 d, 体重60~80 g, 由中国食品药品检定研究院实验动物中心提供[合格证号: SYXK (京) 2022-0014]。
试剂配制  完全培养液: Fischer's培养基+ 10%热灭活FBS + 10%热灭活HS + 碳酸氢钠溶液(终浓度0.075%) + β-巯基乙醇(终浓度0.05 mmol·L-1)。工作培养液: Fischer's培养基+ 1%热灭活HS + 碳酸氢钠溶液(终浓度0.075%) + β-巯基乙醇(终浓度0.05 mmol·L-1)。
细胞生长曲线考察  取冻存的Nb2-11细胞, 置于37 ℃水浴复苏后, 用完全培养液进行培养并扩增。扩增后的细胞用完全培养液调至细胞密度每毫升为1.0×104、2.0×104、5.0×104和1.0×105个, 上述细胞分别接种至2个不同的T25培养瓶中, 每瓶5 mL, 每隔24 h计数。细胞密度为每毫升1.0×105个的细胞悬液由于接种密度高, 可测的对数生长期短, 为此在对数生长期时每隔12 h计数。为提高计数准确率, 每个时间点取2瓶中的细胞计数, 并在计数前将瓶中细胞悬液充分混匀, 如此操作直至第5天结束。以培养时间为横坐标, 活细胞密度为纵坐标, 利用GraphPad Prism绘制细胞生长曲线图, 根据生长曲线图的对数生长期计算细胞倍增时间, 计算公式为Td = Δt×lg2/(lgNt-lgN0), 其中, Td为倍增时间, Δt为计数时间间隔天数, Nt为对数生长期任意一天的观察值, N0为对数生长期初期对应的初始值。
rhGH体外生物学活性检测  将对数生长期的Nb2-11细胞用PBS清洗2次后计数, 加入工作培养液调整细胞密度为每毫升1.0×105个。将细胞悬浮液以每孔50 μL加到96孔不透光细胞培养板中(每孔约5 000个细胞) 备用。将rhGH标准品和rhGH供试品用工作培养液稀释至60 ng·mL-1, 再在96孔细胞培养板中做3倍系列稀释, 共10个稀释度, 每个稀释度3个孔。将稀释好的溶液加入已接种细胞的培养板中, 每孔50 μL, 每个药物浓度标准品和样品交叉上样3个复孔(药物作用起始终浓度为30 ng·mL-1)。将细胞培养板置于37 ℃、5% CO2条件下培养30 ± 2 h。培养结束后, 以每孔100 μL向培养板中加入cell Titer-Glo luminescent发光底物溶液。将细胞培养板置于微孔板振荡器上, 室温避光振荡混匀15 min, 随后静置15 min, 用酶标仪读取各孔luminescence发光值。
实验有效标准的拟订  实验有效标准要求包括系统适用性要求和可靠性测验要求。系统适用性要求评判了该次试验是否有效。USP-NF2024通则 < 126 > 系统适用性要求曲线信噪比应不低于3, 每组数据拟合曲线技术异常值不超过4个, 供试品95%置信区间应为测定值的75%~133%。此外, USP-NF2024通则 < 1034 > 四参数回归计算法中指出完整的四参数是可靠性测验分析和效价计算的前提。ChP2020通则1431四参数回归计算法要求对照品和供试品拟合曲线决定系数R2应≥ 0.98, 每一剂量组反应值的相对标准偏差(relative standard deviation, RSD) 应≤ 10%。基于国内外药典调研, 本研究设置“标准品和供试品的四参数剂量反应曲线应当完整, 上、下渐近线应各至少包含一个浓度点, 线性部分至少包含两个浓度点”的四参数曲线要求, 并对标准品和供试品的剂量反应曲线的上下渐近线比值、拟合度R2、每一剂量组反应值的RSD及供试品95%置信区间作为系统适用性评价指标。可靠性测验指标评价了试验数据拟合后的模型是否可用于效价计算, 样品是否可出具有效报告值。参照ChP2020通则1431四参数回归计算法, 本研究拟对回归项、偏离平行和模型失拟的结果判断显著性水平α的取值进行考察。利用Nb2-11细胞法测定了5个厂家的29批rhGH样品的生物学活性, 包括3批rhGH原液, 3批注射用rhGH和23批rhGH注射液, 并对其中1批注射液在3个不同日期分别测定3次, 共得到31个测定结果。通过对31个结果的分析初步拟定了实验有效标准。
数据结果分析  以标准品或供试品溶液浓度的对数为横坐标, 发光值为纵坐标, 照ChP2020生物检定统计法通则1431中的四参数回归计算法进行实验数据处理, 按下述公式计算供试品相对效价和生物学活性: 供试品相对效价= Er/Es×100%。式中, Er为约束模型中标准品的50%效应浓度, Es为约束模型中供试品的50%效应浓度。供试品生物学活性(IU·mg-1) =供试品相对效价×标准品比活(IU·mg-1)。
方法学验证  按照通则9401采用合并设计, 验证方法的相对准确度、中间精密度、线性和范围。取rhGH标准品, 制备60 ng·mL-1浓度的rhGH标准品溶液作为对照。再将rhGH标准品用工作培养液分别稀释成30、42.6、60、84.6、120 ng·mL-1浓度的待测样品, 5种待测样品对应于60 ng·mL-1浓度的rhGH对照品溶液的相对效价分别为50%、71%、100%、141%和200%。上述待测样品分别在96孔细胞培养板中进行3倍梯度稀释, 共10个稀释度。由4名实验人员在4个不同日期进行相对效价检测。按照拟定的实验有效标准和ChP2020通则9401进行结果分析。
统计学分析  实验数据经GraphPad Prism 8.0软件统计处理, 通过F检验分析回归方程的显著性, P < 0.05认为直线呈显著回归。S曲线所有的点的结果均为各浓度下三复孔平均值±标准差。
精密度联合验证  分发来自3家国内生产企业的2批注射用rhGH、2批rhGH原液、2批rhGH注射液和1家国外生产企业的2批rhGH注射液至6个协作研究实验室。本实验室及6个协作研究实验室均以rhGH标准品为对照品检测各批样品的相对效价, 并在不同日期对其中1批国产注射液(样品C1) 重复检测3次。统计学分析包括: ①利用Shapiro-Wilk检验分析7家实验室检测8批样品结果的正态性, 然后利用单因素方差分析考察7组结果的一致性; ② 7家实验室测定C1的3次结果分别取自然对数后计算几何变异系数GCV%, 分析实验室内精密度; ③分别计算7家实验室测定C1的平均效价并取自然对数后计算几何变异系数GCV%, 计算7家实验室测定8批样品结果的GCV%, 分析实验室间精密度。
rhGH体内生物学活性检测  用0.1%牛血清白蛋白溶液配制高剂量rhGH标准品溶液(0.2 IU·mL-1) 和低剂量rhGH标准品溶液(0.05 IU·mL-1), 按估计效价照标准品溶液的制备配制rhGH原液稀释液作为供试品溶液。取去垂体手术后2~3周、术前体重变化 < 10%的幼龄大鼠, 按体重随机分成4组, 每组至少8只, 每只编号并记录体重。分别自颈部皮下注射标准品溶液或供试品溶液0.5 mL, 每日1次, 连续6天。于最后1次给药后24 h称体重, 处死动物。每只动物给药后体重增加的克数作为反应值, 按照ChP2020反应平行线测定法计算效价及实验误差。本实验获得中国食品药品检定研究院实验动物福利伦理委员会批准[批准号: 中检动(福) 第2024 (B) 004号]。
Nb2-11细胞法的建立基于rhGH与Nb2-11细胞表面的催乳素受体结合后能刺激其增殖。Nb2-11细胞作为关键材料可对实验结果产生重大影响。只有当细胞处于对数生长期, 状态最佳时才可用于活性测定。为获得适宜的Nb2-11细胞日常培养基本条件和参数, 为联合验证参与实验室提供参考, 本研究首先考察了不同细胞接种密度下Nb2-11细胞的倍增时间。如图 1所示, 细胞接种密度影响了Nb2-11细胞进入对数生长期的时间。根据生长曲线图的对数生长期计算细胞倍增时间(表 1), Nb2-11细胞在完全培养液中倍增时间约为12~16 h。根据Nb2-11细胞生长曲线考察结果, 按如下方式进行传代培养: 接种密度约每毫升1×104个, 传代后4天用于实验; 接种密度约每毫升2×104个, 传代后3天用于实验; 接种密度约每毫升1×105个, 传代后2天用于实验。
对本实验室留存的29批样品测定的31次检测结果进行系统适用性设定要求分析(标准品和供试品的四参数剂量反应曲线应当完整, 上、下渐近线应各至少包含一个浓度点, 线性部分至少包含两个浓度点), 四参数曲线要求通过率为100%, 标准品与供试品剂量反应曲线上下渐近线比值在3.77~18.87内, R2值在0.980~0.995内。复孔间RSD ≤ 10%的通过率为41.9%, 复孔间RSD ≤ 20%的通过率为100%。供试品95%置信区间测定值在79.0%~126.6%内。基于上述结果将系统适用性要求拟定为: 标准品和供试品的四参数剂量反应曲线应当完整, 上、下渐近线应各至少包含一个浓度点, 线性部分至少包含两个浓度点。标准品与供试品剂量反应曲线上下渐近线比值应不小于3, R2应不小于0.98, 复孔间RSD应不大于20%。供试品相对效价95%置信区间应为测定值的75%~133%。
该31次检测数据的可靠性测验中, 回归项应非常显著(P < 0.01) 通过率为100%。当显著性水平α取0.05时, 偏离平行项应不显著的通过率为96.8%, 当α取0.01时, 偏离平行项应不显著的通过率为100%。当α取0.05或0.01时, 模型失拟项应不显著的通过率分别为90.3%和83.9%。根据ChP2020通则1431, 个别情况下, 当残差II或误差项的方差非常小时, 偏离平行或模型失拟检验结果可能判为显著。上述验证结果偏离平行和模型失拟应不显著的通过率不能达到100%的原因可能是由于复孔间差异极小导致的误判。因此, 结合本研究31次检测数据及企业历史数据可靠性检验分析结果, 将偏离平行不显著的判定要求设为P ≥ 0.01, 并以R2代替模型失拟进行拟合优度评价。
为确定拟订系统适用性及可靠性检验要求设置的可行性, 由4名人员参照ChP2020四部通则9401对方法的相对准确度、中间精密度、线性和范围进行了验证, 所有试验结果均符合拟订系统适用性及可靠性检验要求。
4名实验人员测定5个效价水平的rhGH标准品的相对效价几何均值分别为: (50.76 ± 3.39)%、(71.94 ± 3.01)%、(98.60 ± 3.50)%、(139.25 ± 3.17)%、(194.99 ± 9.13)%。相对偏倚及其90%置信区间分别为1.06% (-5.65%, 8.25%)、1.15% (-3.04%, 5.52%)、-1.52% (-4.95%, 2.04%)、-1.29% (-3.52%, 0.99%)、-2.70% (-6.94%, 1.74%)。在50%~200%效价内测定的相对效价的相对偏倚在±12%内, 符合通则9401相对偏倚的要求。
根据通则9401, 拟合50%~200%效价理论值自然对数和效价测定值自然对数的直线回归方程, 回归方程为Y = 0.971X-0.007, 相关系数为0.982, 符合通则9401“直线回归方程的斜率应在0.80~1.25, 相关系数应不低于0.98”的要求。
5个效价水平相对效价测定值的几何变异系数(GCV%) 在3.5%~10.8%, 均小于20%, GCV的95%置信上限在6.3%~20.3%, 中间精密度良好。
根据相对准确度、中间精密度和线性的结果, 本方法可检测的效价水平范围为50%~200%。
7个实验室分别对编号为B1~E2的样品进行活性测定, 拟合四参数方程剂量效应曲线如图 2A。7家实验室检测8批rhGH样品结果均符合拟定的实验有效标准要求。如图 2B所示, 除样品C1得到21个测定结果外, 其余7批样品分别得到7个测定结果。7家实验室检测8批样品共得到80个相对效价结果, 在85%~119%内, 利用平均值±3SD计算相对效价范围在74%~127%。8批样品各个实验室检测结果的均值在93%~108%, RSD在4.84%~10.64%内, Shapiro-Wilk检验分析7家实验室测定结果的P值均大于0.05, 结果呈正态分布。单因素方差分析考察7组相关测定结果P值为0.78, 差别不具有统计学意义, 表明7家实验室检测8批样品结果的均值具有一致性。图 2C为7家实验室对C1分别测定3次相对效价的结果, 将上述结果取自然对数, 并计算几何变异系数GCV%, 7家实验室测定结果的几何变异系数GCV%均小于20%, 表明实验室内精密度良好。图 2D为7家实验室对C1分别测定3次相对效价的平均值, 计算几何变异系数GCV%结果为4.56%。7个实验室检测8批样品结果的GCV%为9.22%, 表明实验室间精密度良好。
利用Nb2-11细胞法和体内动物法分别检测28批rhGH原液的生物学活性, 体内法检测比活性结果均值为2.99 IU·mg-1, RSD为9.3%, 体外法检测结果均值为2.95 IU·mg-1, RSD为7.4% (图 3A)。对结果进行双样本t检验, 显著性水平P = 0.581, 表明两种方法检测结果无显著差异。对两种方法测定结果进行Bland-Altman分析(图 3B), 两种方法平均比率为1.016, 几乎所有数据点均在95%一致性区间内(0.82~1.21), 表明两种方法具有较好的一致性。
从4家生产企业征集了各自留样的近、中、远效期共49批rhGH样品的体外活性检测结果, 包括12批注射用rhGH、15批rhGH注射液和22批rhGH原液, 测得相对效价平均值和结果范围分别为100% (89%~109%)、100% (87%~115%)、108% (90%~119%), 比活性平均值和结果范围分别为3.0 IU·mg-1 (2.7~3.2 IU·mg-1)、3.0 IU·mg-1 (2.6~3.5 IU·mg-1)、3.2 IU·mg-1 (2.7~3.5 IU·mg-1)。利用平均值±3SD计算49批rhGH样品相对效价范围在76%~128%, 比活性范围在2.6~3.5 IU·mg-1
自1985年利用大肠杆菌生产的第一个rhGH被FDA批准用于治疗儿童生长激素缺乏症以来, 全世界已有几十款rhGH药物获批上市。尽管rhGH已被公认为是安全有效的药物, 不良反应相对较少, 但rhGH的修饰和变异, 如脱酰胺、氧化和二聚化等仍有可能在发酵和纯化过程中出现。虽然许多精密的理化技术可用于分析rhGH的异质结构, 但仍不能替代生物活性测定。
近年来, 各国监管机构都在推动基于细胞的生物学活性方法应用于生物大分子药物的放行及稳定性分析。由于生长激素体内活性测定法操作难度极高, 需要消耗大量的动物, EP和USP均已删除体内活性测定法。为对rhGH的生物学活性进行有效的质量控制, 在前期研究建立的基于Nb2-11细胞增殖的生物学活性测定方法的基础上, 本研究对国内所有生长激素类药物生产企业进行了技术转移和培训。细胞增殖法对细胞生长状态要求高, 实验条件或操作人员变化均可能影响实验结果。本研究根据Nb2-11细胞生长曲线考察结果, 推测了Nb2-11细胞的倍增时间和传代培养方式, 为该方法的顺利推广奠定了基础。
为建立标准化的测定rhGH活性的平台方法, 本研究首先结合国内外药典规定拟订了实验有效标准, 并通过29批rhGH产品的检测结果初步验证了实验有效标准。随后由4名实验人员分别测定5个效价水平的rhGH标准品的相对效价, 对拟订的实验有效标准、相对准确度、中间精密度、线性和范围进行了研究。结果表明拟订的实验有效标准通过率为100%, 方法在50%~200%的验证范围内准确度高、精密度良好、线性符合ChP2020通则9401要求。进一步, 联合7家实验室, 对8批rhGH样品进行生物学活性测定, 结果表明7家实验室检测结果具有一致性, 实验室间精密度良好, 各实验室内精密度也良好。统计了8批样品共80次相对效价测定结果的范围在85%~119%, 利用平均值±3SD计算相对效价限度范围在74%~127%, 这与49批近、中、远效期rhGH样品测得相对效价平均值±3SD计算限度范围(76%~128%) 基本一致。此外, 7家实验室检测2批rhGH原液的14次测定结果平均值为2.9 IU·mg-1, 22批近、中、远效期rhGH原液测定结果平均值为3.0 IU·mg-1, 表明体外法检测rhGH原液生物学活性均不小于原体内法标准限度规定的2.5 IU·mg-1
为探索Nb2-11细胞法替代体内动物法的可行性, 本研究用两种方法分别检测了28批rhGH原液的生物学活性, 体内法和体外法检测比活性结果均值分别为2.99 IU·mg-1和2.95 IU·mg-1, 双样本t检验表明两种方法检测结果无显著差异, Bland-Altman分析两种方法测定结果的差值95%一致性区间(差值的1.96个标准差范围) 为0.82~1.21, 图 3B可见几乎所有数据点均位于95%一致性区间内, 表明两种方法具有较好的一致性。以上研究为Nb2-11细胞法替代体内动物法提供了可靠数据支持。从作用机制上看, 体内动物法的原理是基于rhGH对去垂体大鼠的增重作用, 而Nb2-11细胞法是基于rhGH与催乳素受体结合后对Nb2-11细胞的增殖促进作用。虽然细胞法实际上反映了rhGH的催乳活性, 且存在专属性差的缺点, 但是相对于体内动物法, 细胞法操作更简便且耐用性高, 且已被广泛应用于rhGH活性研究[10-15], 因此, 本研究联合验证后确立的Nb2-11细胞法可在短期内解决我国rhGH体外活性方法的可及性。
对实验有效标准验证用25批rhGH注射液的测定结果及联合验证用4批rhGH注射液的42次测定结果进行相对效价限度分析, 67个相对效价平均值±3SD计算限度范围为76%~127%, 15批近、中、远效期rhGH注射液相对效价平均值±3SD计算限度范围为82%~133%。实验有效标准验证用3批注射用rhGH及联合验证用14批注射用rhGH相对效价平均值±3SD范围为76%~123%, 14批近、中、远效期注射用rhGH相对效价平均值±3SD计算限度范围为71%~128%。因此, 可以将注射用rhGH和rhGH注射液的质量标准限度定为“应为标准品的70%~140%”。
本联合验证研究表明, Nb2-11细胞增殖法适用于国内外不同生产企业的rhGH原液、注射用rhGH和rhGH注射液产品, 联合验证的方法具有良好的重复性, 拟定的实验有效标准通过率为100%, 本方法可作为体内动物法的替代方法用于rhGH质量控制和放行检验。
