2-(4-Methylthiazol-5-yl) ethyl nitrate hydrochloride (W1302) is a nitro containing derivative of clomethiazole, which is a novel neuroprotective agent with both carbon monoxide (NO) donor and weak γ-aminobutyric acid type A (GABAA) receptor allosteric regulatory excitatory effect. The current study used a rat model of transient middle cerebral arteryocclusion (tMCAO) brain injury to evaluate the therapeutic effect of W1302 on ischemic stroke and explore its potential mechanisms of action. This experiment has been reviewed and approved by the Laboratory Animal Management and Use Committee of the Institute of Materia Medica, Chinese Academy of Medical Sciences (ethical review forms No. 620, 632, 5013). The results showed that gavage administration of 1, 3, and 10 mg·kg-1 of W1302 can significantly reduce the volume of cerebral infarction in rats with ischemia for 2 h and reperfusion for 24 h, and the therapeutic effect was better than that of 200 mg·kg-1 of DL-3n-butylphthalide. W1302 significantly increased NO levels in blood and brain tissue. It increased cerebral blood flow and brain adenosine triphosphate (ATP) content after reperfusion, as well as, inhibited the expressions of inflammatory factors tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in brain tissue. The time window of W1302 was between 120-180 min after ischemia. These research results demonstrated that W1302 could increase NO release, dilate blood vessels, and increase cerebral blood flow, improve energy supply to brain tissue and increase ATP levels, and inhibit neuro-inflammation, which playing the protective roles in tMCAO stroke model rats. This provides theoretical support for the clinical application of W1302 in the treatment of ischemic stroke.

硝酸2-(4-甲基噻唑-5-基) 乙酯盐酸盐(W1302) 是含硝基的氯美噻唑衍生物, 它是一种既有一氧化碳(carbon monoxide, NO) 供体又具有弱γ-氨基丁酸A型(γ-aminobutyric acid type A, GABAA) 受体的变构调节激动作用的新型脑保护剂。当前研究采用大脑中动脉缺血再灌注(transient middle cerebral artery occlusion, tMCAO) 脑损伤大鼠模型, 评价W1302对缺血性脑卒中的治疗作用, 并探索其潜在的作用机制。本实验经过中国医学科学院药物研究所实验动物管理和使用委员会的审查批准(伦理审查表号No. 620、632、5013)。研究结果显示: 灌胃给予1、3、10 mg·kg-1剂量的W1302, 可显著减少缺血2 h再灌24 h大鼠脑梗死体积, 且疗效优于200 mg·kg-1丁基苯酞; 显著增加血液和脑组织中NO水平, 提高再灌后脑血流量和脑内三磷酸腺苷(adenosine triphosphate, ATP) 含量; 明显抑制脑组织炎症因子包括肿瘤坏死因子α (tumor necrosis factor-α, TNF-α)、白细胞介素-1β (interleukin-1β, IL-1β) 的表达; 抗脑缺血时间窗可达缺血后120~180 min。这些研究结果证明, W1302可通过增加NO释放, 扩张血管、增加脑血流量, 改善脑组织能量供应, 升高ATP水平, 抑制神经炎症, 发挥其对tMCAO脑卒中模型大鼠的保护作用, 从而为W1302治疗缺血性脑卒中的临床应用提供理论支持。

硝酸2-(4-甲基噻唑-5-基) 乙酯盐酸盐(W1302) 是含硝基的氯美噻唑衍生物, 它是一种既有一氧化碳(carbon monoxide, NO) 供体又具有弱γ-氨基丁酸A型(γ-aminobutyric acid type A, GABAA) 受体的变构调节激动作用的新型脑保护剂。当前研究采用大脑中动脉缺血再灌注(transient middle cerebral artery occlusion, tMCAO) 脑损伤大鼠模型, 评价W1302对缺血性脑卒中的治疗作用, 并探索其潜在的作用机制。本实验经过中国医学科学院药物研究所实验动物管理和使用委员会的审查批准(伦理审查表号No. 620、632、5013)。研究结果显示: 灌胃给予1、3、10 mg·kg-1剂量的W1302, 可显著减少缺血2 h再灌24 h大鼠脑梗死体积, 且疗效优于200 mg·kg-1丁基苯酞; 显著增加血液和脑组织中NO水平, 提高再灌后脑血流量和脑内三磷酸腺苷(adenosine triphosphate, ATP) 含量; 明显抑制脑组织炎症因子包括肿瘤坏死因子α (tumor necrosis factor-α, TNF-α)、白细胞介素-1β (interleukin-1β, IL-1β) 的表达; 抗脑缺血时间窗可达缺血后120~180 min。这些研究结果证明, W1302可通过增加NO释放, 扩张血管、增加脑血流量, 改善脑组织能量供应, 升高ATP水平, 抑制神经炎症, 发挥其对tMCAO脑卒中模型大鼠的保护作用, 从而为W1302治疗缺血性脑卒中的临床应用提供理论支持。

2-(4-Methylthiazol-5-yl) ethyl nitrate hydrochloride (W1302) is a nitro containing derivative of clomethiazole, which is a novel neuroprotective agent with both carbon monoxide (NO) donor and weak γ-aminobutyric acid type A (GABAA) receptor allosteric regulatory excitatory effect. The current study used a rat model of transient middle cerebral arteryocclusion (tMCAO) brain injury to evaluate the therapeutic effect of W1302 on ischemic stroke and explore its potential mechanisms of action. This experiment has been reviewed and approved by the Laboratory Animal Management and Use Committee of the Institute of Materia Medica, Chinese Academy of Medical Sciences (ethical review forms No. 620, 632, 5013). The results showed that gavage administration of 1, 3, and 10 mg·kg-1 of W1302 can significantly reduce the volume of cerebral infarction in rats with ischemia for 2 h and reperfusion for 24 h, and the therapeutic effect was better than that of 200 mg·kg-1 of DL-3n-butylphthalide. W1302 significantly increased NO levels in blood and brain tissue. It increased cerebral blood flow and brain adenosine triphosphate (ATP) content after reperfusion, as well as, inhibited the expressions of inflammatory factors tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in brain tissue. The time window of W1302 was between 120-180 min after ischemia. These research results demonstrated that W1302 could increase NO release, dilate blood vessels, and increase cerebral blood flow, improve energy supply to brain tissue and increase ATP levels, and inhibit neuro-inflammation, which playing the protective roles in tMCAO stroke model rats. This provides theoretical support for the clinical application of W1302 in the treatment of ischemic stroke.

脑血管病是危害人类生命和健康的常见疾病, 具有高致死率和高致残率的特点[1]。在我国, 脑血管病患者死亡率占总死亡率的22.45%, 已跃居死因顺位之首[2]。近年来, 脑血管病死亡率随着我国人口老龄化程度的不断加深, 呈现较快的增长趋势。缺血性脑卒中通常是由大脑中动脉或前动脉阻塞引起神经损伤的严重脑血管疾病[3]。大脑中动脉栓塞致缺血再灌注脑损伤大鼠模型能高度模拟人类脑卒中发病机制, 被广泛用于评价缺血性脑卒中神经保护药物[4, 5]
一氧化碳(carbon monoxide, NO) 供体以及神经炎症抑制剂在治疗脑血管疾病和神经退行性疾病中的作用已被广泛报道。近年来, γ-氨基丁酸A型(γ-aminobutyric acid type A, GABAA) 受体激动剂作为一种新的治疗策略, 在脑保护方面也逐渐被认识[6]。GABAA受体激动剂通过抑制过度的兴奋性氨基酸毒性, 重建神经递质传递系统的平衡发挥脑保护作用[7, 8]。氯美噻唑(clomethiazole, CMZ) 为GABAA受体激动剂, 已在临床中应用, 具有镇静、催眠、抗惊厥作用。此外, CMZ被证明也具有神经保护作用, 发挥抗氧化应激、炎症抑制、改善线粒体功能障碍和抑制兴奋性等作用[6]。前期的研究证实, CMZ在大鼠局灶性脑缺血模型和非人灵长类局灶性脑缺血动物中均表现出明显的神经保护作用[9]。然而, 在Ⅲ期临床研究中, CMZ未表现出脑卒中的明显改善作用而终止试验[10]
硝酸2-(4-甲基噻唑-5-基) 乙酯盐酸盐(W1302) 是CMZ的衍生物, 由本研究团队对CMZ进行结构改造获得的新化合物, 其药理作用在保留CMZ原有药理活性的基础上, 引入NO供体扩张脑血管, 改善缺血部位的脑血流量不足, 并保留了GABAA受体的变构调节作用, 成为具有NO释放、神经炎症抑制、抑制过度兴奋的新型脑保护候选药物。W1302已获得治疗血管性痴呆的临床试验许可, 正在开展Ⅰ期临床试验。
本实验采用大脑中动脉线栓法诱导暂时性局灶性脑缺血再灌注损伤模型(transient middle cerebral artery occlusion, tMCAO), 评价W1302治疗急性脑缺血的作用, 并探讨了W1302对脑卒中保护的作用机制, 为W1302临床试验提供理论支持。
实验动物  本实验所用动物及相关处置符合动物福利的要求, 经过中国医学科学院药物研究所实验动物管理和使用委员会(IACUC) 的审查批准(伦理审查表号620、632、5013)。
W1302灌胃给药的药效学研究使用SD大鼠, 体重260~300 g, SPF级, 由维通利华实验动物中心提供, 动物许可证号: SCXK (京) 2012-0001和SCXK (京) 2016-0011。实验动物饲养于中国医学科学院药物研究所动物屏障环境设施, 室温20~26 ℃, 相对湿度40%~70%。最小换气次数每小时15次, 光照明∶暗=12 h∶12 h。整个饲养过程中保持动物饮食活动自由。手术前后单独饲养, 室温保持23~25 ℃, 自由进食和饮水。
主要药品、试剂  W1302由中国医学科学院药物研究所合成室提供, 纯度大于99%, 批号DP-0561-20160602, 临用前配制。丁基苯酞(DL-NBP, 批号: 11110651, 石药集团), 溶解于植物油, 临用前配制。氯化三苯基四氮唑(2, 3, 5-triphenyltetrazolium chloride, TTC) 购自Sigma公司。异氟烷购自河北一品制药有限公司, 常温保存。
一氧化碳检测试剂盒(NO, 货号: S0024) 和三磷酸腺苷检测试剂盒(adenosine triphosphate, ATP, 货号: S0027) 购于上海碧云天生物科技有限公司。大鼠肿瘤坏死因子α (tumor necrosis factor-α, TNF-α) 酶联免疫试剂盒(G11031062)、大鼠诱导型一氧化氮合成酶(inducible nitric oxide synthase, iNOS) 酶联免疫试剂盒(H160310633)、大鼠白介素1β (interleukin-1β, IL-1β) 酶联免疫试剂盒(G25031064) 购于CUSABIO武汉华美公司。蛋白质定量试剂盒(bicinchoninic acid, BCA法, 货号P1511) 购于北京普利莱基因技术有限公司。
主要仪器  照相机(日本佳能EOS 550D型); 全波长酶标仪(美国µQuant MQX200型); 超声波细胞粉碎机(国产宁波JY92-2D型); 恒温振荡器(国产江苏THZ-C型); 恒温冷冻离心机(美国SIGMA公司3-30K型); 激光多普勒血流仪(瑞士派瑞德公司Laser Doppler Flowmeter, LDF) 系统(包括: PeriFlux 5001 main Unit, PF5010 LDPM Unit检测器, LD Probe 407-1检测探头); 小动物气体麻醉机(美国Matrx VIP3000型); 大鼠脑立体定位仪(国产瑞沃德公司68001型); 牙科手钻(韩国Saeshin公司产#93-15型); 血管阻断用线(北京西浓科技有限公司2636-A1型); 国产恒温加热垫。
tMCAO模型制备及动物分组  按照Longa等[11]方法制备大鼠tMCAO模型。大鼠用气体麻醉机麻醉(1.5%~2.5%异氟烷), 体温维持在(37 ± 0.5) ℃, 仰卧位固定于手术台上。沿颈正中纵向切开皮肤, 仔细分离右侧颈总动脉(common carotid artery, CCA)、颈外动脉(external carotid artery, ECA)、颈内动脉(internal carotid artery, ICA)。将ECA结扎剪断, 拉直与ICA成一直线。在ECA上剪一小口, 将一根长4.0 cm、直径0.26 mm的圆头硅化尼龙线(用0.1%多聚赖氨酸包被) 由此开口插入ICA约1.85~2.00 cm至大鼠大脑前动脉起始处, 阻断大脑中动脉的血流供应, 监测脑血流。缺血2 h后小心抽出尼龙线, 结扎ECA开口并缝合手术切口, 动物放回笼子中再灌24 h。全部过程中室温保持在24~25 ℃。
动物tMCAO术后随机分为5组: 模型对照组(口服蒸馏水+tMCAO)、阳性对照组(口服DL-NBP 200 mg·kg-1+tMCAO)、W1302口服1、3、10 mg·kg-1 (口服相应剂量+tMCAO) 剂量组。动物于tMCAO缺血手术后5 min内口服给予各组受试药物, 给药体积为1 mL·kg-1, 缺血手术后120 min进行再灌注。再灌注24 h后进行行为学评价和TTC染色, 分析统计脑梗死体积及脑组织炎症因子生化测定。
tMCAO神经行为学评分  大鼠缺血再灌注24 h后进行动物的行为学观察。参照Bederson等[12]和Belayev等[13]的方法。采用四级评分法(0~4分): ①行为完全正常为0分; ②提起鼠尾离开地面, 手术对侧前肢内旋、内收者, 为1分; ③大鼠放至地面, 用手挤压两侧检查其抗力, 手术对侧抗力下降者, 为2分; ④大鼠放至地面, 观察其行走, 围绕手术对侧转圈者, 为3分; ⑤损伤极其严重, 已无法自主活动者, 为4分。分数越高, 说明其神经行为损伤越严重。
tMCAO脑梗塞体积的测定  参照Belayev等[13]的方法, 大鼠缺血再灌注24 h后, 即刻断头取脑, 去除嗅束、小脑和低位脑干, 将其冠状切成6片(第1~5片每片厚2 mm, 第6片厚4 mm), 迅速置于5 mL含有4% TTC (1.5 mL) 及1 mol·L-1 K2HPO4 (0.1 mL) 的溶液中染色(37 ℃, 避光) 20~30 min, 其间每隔5 min翻动1次。经TTC染色后, 正常组织染色呈深红色, 梗死组织呈白色。将每组脑片排列整齐, 拍照保存。应用图象分析系统软件(Photoshop7.0) 处理并统计, 求算每片的梗塞面积, 并最终叠加换算成梗塞体积。梗塞体积以所占大脑半球的百分率来表示, 以消除脑水肿的影响。脑梗塞体积比(%) = (手术对侧半球的体积-手术侧半球未梗塞部分的体积)/手术对侧半球的体积×100%。
局部脑血流(rCBF) 的测定  大鼠用异氟烷气体麻醉机(2%~2.5%) 麻醉, 体温维持在37 ± 0.5 ℃。大鼠俯卧位固定于立体定位仪上, 开颅窗并用3%双氧水清理手术视野, 以前囟为坐标原点, 选取前囟后2 mm、右侧3 mm为测定点, 用牙科钻打薄直径约2~3 mm区域(不时以生理盐水冷却), 保持硬脑膜的完整并避开较大的血管, 定位并固定好探头座。
动物仰卧位于手术台上, 记录初始正常稳定期5 min内血流均值为0时血流量。进行tMCAO手术, 记录术后5 min脑血流量为术后血流量, 随后灌胃W1302 10 mg·kg-1 (模型对照组给予等体积的纯水), 缺血手术后120 min再灌注。期间记录缺血后30、60、90、120 min时间点的5 min内稳定血流均值, 及再灌后3、30、60 min时间点的5 min内稳定血流均值。计算并统计缺血血流变化率和再灌血流变化率。各组数据计算均值及标准误, 测算每个时间点缺血百分比。公式如下: 缺血百分比(%) = (缺血时血流量均值-基础血流量均值)/基础血流量均值×100%。
脑缺血损伤后不同时间点灌胃W1302药效时间窗测定  SD大鼠分别于tMCAO缺血后30、60、90、120、180 min灌胃给予W1302 (10 mg·kg-1), 缺血2 h后再灌注。tMCAO术后24 h进行行为学评分, 断头取脑切片TTC染色进行脑梗塞体积评价。
脑缺血损伤后不同时间点血样及脑组织生化测定  血样和脑组织生化含量时间曲线测定采用SD大鼠雌雄各半, 每个时间点n = 12。血浆测定是以未缺血手术动物眼眶静脉取血设为0 min并作为基准值(blank), tMCAO后大鼠灌胃给予10 mg·kg-1 W1302后, 给药后10、30、60、120、240 min和12、26 h眼眶静脉取血, 并以肝素抗凝, 4 ℃离心, 分离血浆, -80 ℃保存, 待测。脑组织取材是以未缺血手术动物脑组织设为0 min并作为基准值(blank), tMCAO后在给药10、30、60、120、240 min和12、26 h, 用生理盐水进行心脏灌流, 冲洗干净脑内残留血液后, 断头取损伤侧半脑, -80 ℃保存, 待测。按照NO试剂盒和ATP试剂盒说明书操作, 进行血浆和脑组织生化指标测定。
脑组织炎症因子等生化组织测定  tMCAO术后24 h, 将大鼠断头取损伤侧半脑, 用滤纸吸干后称重, 按重量体积比1∶9加入4 ℃预冷的生理盐水, 用内切式匀浆机制备10%的组织匀浆, 然后低温低速离心10 min, 取上清。按照TNF-α、IL-β、iNOS试剂盒及蛋白定量试剂盒说明书操作, 进行脑组织测定。
统计学处理  所有数据均用均数(x) ±标准误(SEM) 表示, 采用SPSS 11.5统计软件, 采用单因素方差分析(one-way ANOVA) 进行检验。P < 0.05表示差异具有统计学意义。
在线栓法tMCAO (缺血2 h再灌24 h) 所致大鼠急性脑缺血再灌注损伤模型中(表 1): 模型对照组脑梗塞体积比和行为学评分分别为45.1% ± 4.0%和2.3 ± 0.2, 提示模型动物存在明显的脑缺血损伤。阳性对照药DL-NBP (200 mg·kg-1) 处理后, 能明显减少脑梗塞体积至33.0% ± 3.7% (P < 0.05), 但行为学无明显差异(1.9 ± 0.2, P > 0.05); W1302灌胃给药呈剂量依赖性地减少模型大鼠的脑梗塞体积, 在1、3和10 mg·kg-1剂量下分别为31.3% ± 5.5% (P < 0.05)、27.8% ± 3.5% (P < 0.01) 和25.7% ± 4.7% (P < 0.01), 行为学评分均为1.5~1.8之间, 其中1 mg·kg-1剂量组有显著差异(1.5 ± 0.3, P < 0.05)。
W1302对tMCAO大鼠治疗时间窗研究中, 模型大鼠缺血2 h再灌24 h后脑梗塞体积和行为学评分分别为38.1% ± 2.4%和2.0 ± 0.1。在缺血30、60、90、120 min (再灌) 灌胃给予W1302 (10 mg·kg-1) 能显著减少再灌24 h的脑梗塞体积, 分别为29.9% ± 3.0% (P < 0.05)、31.2% ± 3.5% (P > 0.05)、29.5% ± 3.8% (P < 0.05) 和29.7% ± 2.7% (P < 0.05); 但对行为学评分无显著性差异。此外, 在缺血180 min (再灌后60 min) 灌胃给予W1302, 对脑梗塞体积(39.3% ± 3.1%, P > 0.05) 和神经行为学评分(2.1 ± 0.1, P > 0.05) 均未见明显影响。
结果表明, W1302在缺血后120 min内给药对脑缺血再灌注损伤具有明显的改善作用; 缺血后60 min给药虽然未见统计学差异, 这可能与动物个体差异有关, 但与模型对照组相比较仍然具有较为明显的降低脑梗塞体积的作用。因此, 综合评价W1302对大鼠脑缺血再灌损伤的治疗时间窗为缺血后至120~180 min之间(表 2)。
研究结果显示(图 1), 以缺血术前脑血流量为基准, 模型大鼠tMCAO后脑血流量显著降低, 达60%以上, 再灌后脑血流恢复至术前血流量的-13.9% ± 8.2%~-5.9% ± 6.5%。W1302 (10 mg·kg-1) 在缺血后5 min灌胃给药, 对缺血期间(缺血2 h内) 的脑血流量未见明显的改善作用; 但明显增加再灌后脑血流量, 再灌3、30、和60 min后的数值分别比术前血流量高7.0% ± 6.0% (P < 0.05)、20.9% ± 7.0% (P < 0.05) 和17.5% ± 9.5% (P = 0.050 1)。
tMCAO模型大鼠各缺血时间点血浆或脑组织NO水平未见明显的改变, 在给予W1302后血浆中NO的水平明显升高, 在缺血后2 h达到峰值, 随后NO水平逐渐降低。而在给予W1302后, 脑组织中NO水平也明显升高, 也在给药后大约2 h达到峰值(图 2)。
ATP是体内组织细胞一切生命活动所需能量的直接来源, 其含量反映了组织细胞的活力。tMCAO模型大鼠血浆ATP水平在缺血后明显降低; 而给予W1302后, 血浆ATP水平明显升高且在缺血后0.5 h达到峰值, 持续24 h以上。而模型对照组的脑组织中ATP的水平在缺血后存在瞬时降低。给予W1302后脑组织中ATP水平也明显升高, 持续时间较长。这个结果与脑血流量变化有关, 再灌后明显升高(图 3)。
炎症因子的改变与缺血性脑卒中有着密切的联系。大鼠行大脑中动脉手术后, 各组与模型对照组比较, 阳性药DL-NBP能够显著降低损伤侧脑组织TNF-α (P < 0.01) 和IL-1β (P < 0.05) 水平, 对iNOS的表达也有减少趋势。W1302各剂量组能剂量依赖性降低大鼠TNF-α (P < 0.01) 和IL-1β (P < 0.05) 水平; 对iNOS的表达水平也分别下降了24%、17%和22%。以上结果提示, W1302对大鼠模型缺血再灌注损伤的治疗改善作用可能与其抑制炎症因子的表达和抑制iNOS水平相关(表 3)。
缺血性脑血管疾病是目前严重威胁人类生命的重要疾病之一。脑组织对缺血缺氧极为敏感, 脑部出现缺血缺氧损伤是一个复杂的病理过程[14]
W1302是硝酸酯类化合物, 其硝酸根能释放NO, 可显著扩张血管, 改善脑血流; 此外, NO可激活脑中可溶性鸟苷酸环化酶, 产生cGMP, 激活相关通路, 起到神经保护作用。在当前研究中, W1302在1~10 mg·kg-1剂量时可明显改善tMCAO大鼠术后24 h行为学评分, 减少脑梗死体积, 且随剂量增加呈剂量效应关系; 显著增加血液和脑组织中NO含量, 恢复tMCAO大鼠局部脑血流。这些研究结果说明, W1302可通过释放NO、扩张血管、增加脑血流量、改善脑能量供应(ATP) 发挥脑保护作用。
GABA是哺乳动物中枢神经系统中主要的抑制性神经递质, 在降低神经元兴奋性中起重要作用, 激活GABAA受体, 可产生抑制神经炎症和神经保护作用。W1302是CMZ的衍生物, 本实验室前期的电生理学研究发现W1302具有弱变构激活GABAA受体的作用, 其对GABAA受体的激动作用较CMZ明显降低, 仅为CMZ的1/10 (EC50=996 μmol·L-1), 其结合位点与依托咪酯相同, 与GABAA受体β亚基结合[15]。说明W1302可增强内源性GABA的神经保护作用, 而自身激活GABA受体作用较弱, 减少了此类药物的嗜睡作用。
神经炎症是存在于缺血性脑卒中患者脑内的病理改变之一。在当前研究中, W1302对tMCAO模型大鼠神经炎症具有明显的抑制作用, 能剂量依赖性降低大鼠TNF-α (P < 0.01) 和IL-1β (P < 0.05) 水平; 因而, TNF-α为该药的潜在药效生物标志物。在生理条件下, 体内iNOS水平较低, 小胶质细胞和星形胶质细胞在炎症、创伤和免疫反应后可诱导iNOS表达增加[16]。iNOS基因敲除的AD转基因小鼠或iNOS抑制剂对Aβ诱导的神经毒性具有明显保护作用[17]。脑缺血脑组织中的iNOS受到NO的负反馈调节[18]。本实验显示, W1302给药后脑组织中iNOS含量略有下降, 其原因可能与W1302缓解了脑缺血亚急性期脑组织产生的炎性浸润, 从而减少了损伤后iNOS的激活[19]
综上所述, W1302具有明显的改善缺血性脑卒中的作用, 可通过增加脑血流量、改善脑组织能量供应、激活GABAA受体抑制兴奋性氨基酸损伤、抑制神经炎症等作用发挥脑保护效应。W1302药理作用强, 脑保护作用的有效剂量在大鼠中为1~10 mg·kg-1, 持续作用时间长, 一次口服给药可持续影响NO和ATP水平达24 h以上。W1302于2023年被CFDA批准临床试验许可用于治疗血管性痴呆, 目前处于临床Ⅰ期研究, 本研究结果提示, W1302也有望开发成为新型的治疗缺血性脑卒中药物。
