Pharmaceutical cocrystals is an advanced technology to improve the physicochemical and biological properties of drugs. However, there are few studies on the in vivo metabolism of pharmaceutical cocrystals. In this study, the pharmacokinetics of wogonin cocrystal in normal rats was further studied on the basis of the previous preparation of wogonin-aloperine cocrystal. Firstly, an ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method was established for simultaneous determination of wogonin and its metabolite wogonoside in rat plasma, and to investigate the methodology. The method was applied to the pharmacokinetic study of wogonin-aloperine cocrystal (Wog-Alop) in rats. The results showed that wogonin and its metabolite wogonoside had a good linear relationship in the range of 1-800 ng·mL-1, and the precision, accuracy, matrix effect and stability in this range met the requirements of biological analysis. Compared with direct administration of wogonin, the Cmax of wogonin and its metabolite in rats increased to 7.44-fold and 9.15-fold, AUC0-t increased to 1.67-fold and 3.72-fold, and oral bioavailability of wogonin increased to 187.66% after cocrystal administration. Wog-Alop cocrystal can significantly improve the Cmax, AUC, and oral bioavailability of wogonin and its metabolite, which provides a new perspective for the clinical application of wogonin. 药物共晶是改善药物理化性质和生物学特性的前沿技术, 但目前关于药物共晶体内代谢的研究较少。本文在前期制备汉黄芩素-苦豆碱共晶的基础上, 进一步研究汉黄芩素药物共晶在正常大鼠体内的药代动力学。首先, 建立超高效液相色谱-串联质谱方法(ultra-performance liquid chromatography-tandem mass spectrometry, UPLC-MS/MS) 同时测定大鼠血浆中汉黄芩素及其代谢产物汉黄芩苷含量, 并进行方法学考察; 将该方法应用于汉黄芩素-苦豆碱共晶在大鼠体内的药代动力学研究。结果显示, 汉黄芩素及其代谢产物汉黄芩苷在1~800 ng·mL-1内线性关系良好, 在此范围内的精密度、准确度、基质效应及稳定性考察均符合生物分析要求; 相较于汉黄芩素直接给药, 以共晶形式给药后, 大鼠体内汉黄芩素及其代谢产物汉黄芩苷的Cmax分别提高至7.44倍和9.15倍, AUC0-t提高至1.67倍和3.72倍, 汉黄芩素口服生物利用度提高为187.66%。汉黄芩素-苦豆碱共晶可显著提高汉黄芩素及其代谢产物汉黄芩苷的Cmax、AUC及口服生物利用度, 为汉黄芩素的临床应用提供了新思路。本研究经北京中医药大学实验动物伦理审查委员会批准(批准编号为: BUCM-2023032307-1148)。

药物共晶是改善药物理化性质和生物学特性的前沿技术, 但目前关于药物共晶体内代谢的研究较少。本文在前期制备汉黄芩素-苦豆碱共晶的基础上, 进一步研究汉黄芩素药物共晶在正常大鼠体内的药代动力学。首先, 建立超高效液相色谱-串联质谱方法(ultra-performance liquid chromatography-tandem mass spectrometry, UPLC-MS/MS) 同时测定大鼠血浆中汉黄芩素及其代谢产物汉黄芩苷含量, 并进行方法学考察; 将该方法应用于汉黄芩素-苦豆碱共晶在大鼠体内的药代动力学研究。结果显示, 汉黄芩素及其代谢产物汉黄芩苷在1~800 ng·mL-1内线性关系良好, 在此范围内的精密度、准确度、基质效应及稳定性考察均符合生物分析要求; 相较于汉黄芩素直接给药, 以共晶形式给药后, 大鼠体内汉黄芩素及其代谢产物汉黄芩苷的Cmax分别提高至7.44倍和9.15倍, AUC0-t提高至1.67倍和3.72倍, 汉黄芩素口服生物利用度提高为187.66%。汉黄芩素-苦豆碱共晶可显著提高汉黄芩素及其代谢产物汉黄芩苷的Cmax、AUC及口服生物利用度, 为汉黄芩素的临床应用提供了新思路。本研究经北京中医药大学实验动物伦理审查委员会批准(批准编号为: BUCM-2023032307-1148)。

Pharmaceutical cocrystals is an advanced technology to improve the physicochemical and biological properties of drugs. However, there are few studies on the in vivo metabolism of pharmaceutical cocrystals. In this study, the pharmacokinetics of wogonin cocrystal in normal rats was further studied on the basis of the previous preparation of wogonin-aloperine cocrystal. Firstly, an ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method was established for simultaneous determination of wogonin and its metabolite wogonoside in rat plasma, and to investigate the methodology. The method was applied to the pharmacokinetic study of wogonin-aloperine cocrystal (Wog-Alop) in rats. The results showed that wogonin and its metabolite wogonoside had a good linear relationship in the range of 1-800 ng·mL-1, and the precision, accuracy, matrix effect and stability in this range met the requirements of biological analysis. Compared with direct administration of wogonin, the Cmax of wogonin and its metabolite in rats increased to 7.44-fold and 9.15-fold, AUC0-t increased to 1.67-fold and 3.72-fold, and oral bioavailability of wogonin increased to 187.66% after cocrystal administration. Wog-Alop cocrystal can significantly improve the Cmax, AUC, and oral bioavailability of wogonin and its metabolite, which provides a new perspective for the clinical application of wogonin. This study was approved by the Experimental Animal Ethics Review Committee of Beijing University of Chinese Medicine (approval number: BUCM-2023032307-1148).

汉黄芩素(wogonin, Wog) 是中药黄芩中分离得到的一种黄酮类化合物, 具有抗癌[1, 2]、抗炎[3-5]、抗氧化[6]、抗病毒[7]、抗惊厥[8]、抗焦虑和神经保护等[9, 10]多种药理活性。然而, 作为一种BCS Ⅱ类化合物, Wog溶解性差, 口服生物利用度低[11], 在临床应用中受到一定的限制。因此, 改善生物利用度对Wog的临床应用具有重要价值。
药物共晶作为一种有潜力的新型药物开发技术, 在改善难溶性药物的溶解度、稳定性和生物利用度方面受到了极大的关注[12-16]。药物共晶技术[17]可在不改变药物分子化学结构的基础上, 将药物活性成分(active pharmaceutical ingredient, API) 与共晶形成物(cocrystal former, CCF) 以一定化学计量比通过氢键等非共价键的作用结合在同一晶体中, 从而形成更具药用价值的共晶体。
目前, 药物共晶的研究大多聚焦于制备、表征以及体外溶出的改善, 而关于体内代谢与生物利用度的研究较少[18-21]。课题组前期以Wog为API, 苦豆碱(aloperine, Alop) 为CCF (图 1), 化学计量比为1∶1制备了汉黄芩素-苦豆碱共晶(Wog-Alop)。本文在前期制备并表征Wog-Alop的基础上, 建立了UPLC-MS/MS法, 将其应用于单次给药后, Wog及其代谢产物在正常大鼠体内的药代动力学研究, 并进一步考察药物共晶技术对Wog口服生物利用度的影响。为Wog后续的成药性研究提供数据支持, 同时也为药物共晶技术在改善难溶性天然产物口服生物利用度方面的应用奠定基础。
仪器  Waters UPLC-Xevo TQ-S micro三重四极液质联用仪(美国Waters公司, 包括自动进样器、二元梯度泵、柱温箱、真空脱气机、Masslynx 4.1质谱工作站); 超高速低温冷冻离心机(上海安亭科学仪器厂); ME155DU万分之一电子天平(上海梅特勒-托利多仪器有限公司); 动物体重秤(上海佑科仪器仪表有限公司); KQ-100DB型数控超声波清洗器(昆山市超声仪器有限公司); 灌胃针(北京医疗器械厂); N-EVAP型氮吹仪(上海书俊仪器设备有限公司); M375-XH-T型自动漩涡振荡器(北京东方化玻科技有限公司); 移液器(德国Eppendorf公司)。
药品与试剂  生理氯化钠溶液(LOT: L223051001) 购自四川科伦药业股份有限公司; 乌来糖(LOT: C15887472)、Wog (LOT: J15GB154492) 及汉黄芩苷(LOT: A09GB144759) 购自上海源叶生物科技有限公司; 甘草素(LOT: A26IB224335) 购自成都瑞芬思生物科技有限公司; 实验用甲醇(LOT: 222634)、乙腈(LOT: 222372) 及甲酸(LOT: 223682) 均为质谱级, 购自赛默飞世尔科技有限公司; 蒸馏水(LOT: G3 02839) 购自广州屈臣氏食品饮料有限公司。
动物  SPF级SD大鼠, 雄性, 4周龄, 体重220 ± 10 g, 购买于斯贝福(北京) 生物技术有限公司, 实验动物许可证编号: SCXK (京) 2019-0010; 实验单位使用许可证编号: SYXK (京) 2020-0033。16只大鼠随机分笼后饲养于北京中医药大学良乡校区动物房, 每笼4只; 饲养环境保持温度: 22~25 ℃, 湿度: 40%~70%, 自然照明, 期间大鼠自由进食与饮水。本研究在北京中医药大学实验动物伦理审查委员会的批准下进行(批准编号为: BUCM-2023032307-1148), 并严格遵守相关准则。
溶液的配制  分别取Wog、汉黄芩苷及内标(internal standard, IS) 甘草素对照品适量, 精密称定, 加甲醇制成每1 mL约含1 mg的储备液, 4 ℃保存备用。
样品处理  大鼠血浆样品于4 ℃冰箱中解冻, 精密吸取100 μL (或10 μL), 加入内标溶液10 μL、乙腈290 μL (或380 μL), 涡旋1 min, 12 000 r·min-1、4 ℃离心15 min, 取上清液氮气吹干, 向残渣加入100 μL 0.1%甲酸乙腈-0.1%甲酸水(40∶60) 溶液复溶, 涡旋1 min, 12 000 r·min-1、4 ℃离心15 min, 吸取上清液进行UPLC-MS/MS分析。
UPLC-MS/MS方法建立
色谱条件  Acquity UPLC BEH C18色谱柱(2.1 mm × 100 mm, 1.7 μm); 流动相: 0.1%甲酸水(A)-0.1%甲酸乙腈(B), 梯度洗脱(0~0.5 min, 40%~60% B; 0.5~1 min, 60% B; 1~1.5 min, 60%~90% B; 1.5~2 min, 90%~40% B; 2~2.5 min, 40% B), 流速为0.3 mL·min-1; 柱温: 40 ℃; 样品室温度: 10 ℃; 进样量: 5 μL。
质谱条件  采用电喷雾离子源(ESI), 正离子模式扫描, 碰撞气为氩气(Ar), 吹扫气为氮气(N2), 毛细管电压3.5 kV, 锥孔电压15 V, 离子源温度150 ℃, 脱溶剂气温度350 ℃, 脱溶剂气流速650 L·h-1。采用多反应监测模式进行测定, Wog、汉黄芩苷及IS的m/z分别为285.04→269.99、461.20→285.08及256.91→136.93, 锥孔电压15 V, 碰撞电压20 eV。利用Masslynx 4.1质谱工作站进行数据采集及处理。
专属性  分别取6只不同来源的大鼠空白血浆、添加混合对照品溶液的大鼠空白血浆及Wog-Alop给药组48 h时的大鼠血浆, 按照“样品处理”项下方法制备得到空白血浆样品、加样空白血浆样品及给药血浆样品。将上述样品进行UPLC-MS/MS测定, 分析血浆中内源性物质是否对目标分析物Wog、汉黄芩苷以及IS产生干扰。
残留考察  通过在连续测定3次高浓度样品后, 检测空白血浆样品来评价方法残留。残留应不超过定量下限的20%, 且不超过内标的5%。
标准曲线及线性范围  精密量取Wog与汉黄芩苷的对照品储备液适量, 逐级稀释, 配制成一系列浓度梯度(10、20、40、80、400、800、4 000、6 000和8 000 ng·mL-1) 的混合对照品溶液。按照“样品处理”项下方法制备一系列浓度为1、2、4、8、40、80、400、600、800 ng·mL-1的标准血浆样品溶液, 每个浓度平行3份。进行UPLC-MS/MS测定, 记录待测化合物峰面积与内标化合物的峰面积之比。以峰面积比Y为纵坐标, 对照品溶液浓度X为横坐标, 用加权(1/X) 最小二乘法进行线性回归, 得到待测化合物的标准曲线。
准确度与精密度  按照“样品处理”项下方法制备Wog和汉黄芩苷浓度分别为1、2、40、600 ng·mL-1 (即定量下限及低、中、高浓度) 的质控样本, 每个浓度平行6份进样测定。根据随行标准曲线分析质控样品, 以测量值与标示值之比评价准确度; 测量值的相对标准差(relative standard deviation, RSD) 评价精密度。通过分析同一分析批和连续3天分析3个不同分析批的质控样品分别获得批内准确度/精密度和批间准确度/精密度。
基质效应  按照“样品处理”项方法提取空白血浆后加入待测物的混合对照品溶液制备低、中、高浓度的标准血浆样品溶液, 每个浓度平行6份进样测定。根据标准血浆样品中待测物与内标的峰面积比值与相应浓度混合对照品溶液中待测物与内标的峰面积比值计算内标归一化的基质因子, 并根据其变异系数评价基质效应。6批基质的内标归一化基质因子变异系数应不大于15%。
稳定性考察  按照“样品处理”项方法制备Wog及汉黄芩苷的低、中、高浓度质控样本, 每个浓度平行6份。将质控样本于4 ℃放置24 h测定短期稳定性; 于-80 ℃保存30 d测定长期稳定性。每个浓度的样本进行6次分析, RSD值应≤ 15%。
药代动力学研究
动物分组及给药  16只SD大鼠随机分为Wog组和Wog-Alop组, 每组8只。适应性喂养3 d, 禁食(自由饮水) 12 h, 第四天上午灌胃给药, 给药剂量以Wog计均为100 mg·kg-1[11, 22], 分别于给药前和给药后5、10、15、30、45、60 min及2、4、6、8、12、24、32、48 h眼眶取血, 置于肝素化离心管中, 离心分离血浆, -80 ℃保存待测。
数据处理及药代动力学研究  根据QqQ Quantitative Analysis软件计算各采集时间点血药浓度。以取血时间为横坐标, 血浆中样品的浓度为纵坐标, 绘制大鼠给药Wog和Wog-Alop后体内Wog及其代谢产物汉黄芩苷的血药浓度-时间曲线。将血药浓度-时间数据输入DAS 2.1软件计算待测化合物的各项药代动力学参数, 包括药时曲线下面积(AUC)、平均驻留时间(MRT)、消除半衰期(t1/2)、药物达峰时间(tmax)、达峰浓度(Cmax) 及相对口服生物利用度(relative oral bioavailability, RB) 等。
大鼠的空白血浆样品、加样空白血浆及给药血浆样品中待测物和内标的检测结果如图 2所示。待测物和内标在保留时间内没有干扰, 且基质中的内源性组分不干扰待测物与内标的测定, 表明该方法具有良好的专属性。
重复进样3次定量上限样品后, 对空白血浆进行分析, 未检测到待测物的色谱峰, 表明该方法不存在待测物残留。
对大鼠血浆中待测物与内标物的峰面积比值和相应浓度的标准曲线进行考察, Wog与汉黄芩苷在1~800 ng·mL-1内具有良好的线性(r > 0.99), 线性方程分别为Y = 5.97X + 1.17与Y = 1.47X + 0.92。
Wog及其代谢物在定量下限及低、中、高浓度下的准确度在94.2%~99.2%之间, 批内精密度与批间精密度的RSD在3.0%~13.1%之间, 基质因子的批间变异系数在3.0%~12.6%之间。表明该方法准确度与精密度良好, 内源性物质对被测成分干扰较小, 样品前处理方法可行, 符合测定的要求。
样品于4 ℃放置24 h及-80 ℃放置30 d后, Wog及其代谢物均无明显的降解, 低、中、高浓度质控样品的准确度在91.7%~104.3%之间, 说明该方法的短期/长期稳定性良好。
由于口服后Wog经葡萄糖醛酸化代谢转化为汉黄芩苷, 大鼠分别灌胃给药Wog与Wog-Alop, 同时检测Wog与汉黄芩苷的血药浓度, 以评估二者的药代动力学。药时曲线结果如图 3所示, 结合药代动力学参数(表 1) 可以看出, Wog灌胃给药后血药浓度较低, 入血吸收较差, Wog-Alop给药后Wog在体内快速释放, 药代动力学参数Cmax、AUC0-t以及RB均有明显提高。
Wog药理活性广泛, 但由于其溶解性差, 导致口服生物利用度低, 成药性差, 难以应用于临床。近年来, 随着共晶技术的发展, 其在改善成药性和新药研发等领域的应用得到了广泛关注, 但目前有关药物共晶的体内研究仍较少。本文建立了UPLC-MS/MS法测定大鼠血浆中Wog及其代谢产物汉黄芩苷浓度, 对正常SD大鼠灌胃给药Wog及Wog-Alop后的药代动力学进行了评价, 并考察Wog-Alop的相对口服生物利用度。结果显示, 制备共晶后, Wog及其代谢产物汉黄芩苷在正常大鼠体内的Cmax提高为7.44倍和9.15倍; AUC0-48 h提高为1.67倍和3.72倍; Wog的相对口服生物利用度为原药的187.66%。说明药物共晶技术可明显改善Wog的体内药代动力学过程, 进一步提高口服生物利用度, 并初步改善其成药性。
溶解性差与体内广泛的Ⅱ相代谢是Wog口服生物利用度差的主要原因[23]。与Wog相比, Wog-Alop的口服生物利用度得到明显改善, 推测这可能是由于: ①共晶的形成破坏了Wog分子间的紧密结构, 使得溶剂分子能够穿透其中, 从而提高了Wog溶解性; ② Wog以共晶形式进入体内可阻碍其在肠道的葡萄糖醛酸化代谢, 使Wog以未代谢的原形状态存在; 因此, Wog-Alop可能是通过提高Wog溶解度并抑制Ⅱ相代谢来改善其在体内的药代动力学及口服生物利用度。虽然Wog-Alop在100 mg·kg-1的剂量下可明显改善Wog口服生物利用度, 但本研究尚未对不同的给药剂量进行考察, 因此, Wog-Alop的药代动力学是否呈线性尚不明确。后续将对不同给药剂量下Wog-Alop的药代动力学进行考察, 并进一步研究其对Wog体内生物活性的影响, 以评估药物共晶技术对Wog成药性的改善, 为难溶性药物的新药开发及临床应用提供研究基础与新的思路。
