Oral iron is a commonly used preparation for the treatment of iron deficiency and iron-deficiency anemia in children, but its undesirable taste has become a major factor affecting clinical adherence to the medication. No studies have been conducted to evaluate the palatability of oral iron supplements. Thirteen representative oral iron supplements were selected to evaluate the palatability of oral iron supplements from different perspectives of in vivo and ex vivo by combining the electronic tongue (e-tongue) test, FaceReader facial expression analysis technology and taste interviews. E-tongue test results showed that iron dextran granlues were closest to odorless potassium chloride solution, basically free of bitterness and astringency, which was superior to the other products. FaceReader facial expression results showed that 58.7% of the subjects valued iron dextran granlues for their emotional efficacy higher than that of iron proteinsuccinylate oral solution. Among 43 participants aged ≥ 5 years, the taste preference for iron dextran granlues was 83.7%, and 83.7% of participants chose to take iron dextran granlues again. This study found that iron dextran granlues have a better taste and their flavor is more popular among children, which provides a reference for pediatric clinical use of oral iron supplementation preparations and a new idea for the evaluation of palatability of medications for children. This in vivo palatability evaluation is an investigational clinical trial that was approved by the Institutional Review Board of the Beijing Children's Hospital (No. 2021-149-Y).

口服铁剂是治疗儿童铁缺乏和缺铁性贫血的常用制剂, 但其不良口感成为影响临床服药依从性的主要因素。目前尚未有针对口服铁剂的适口性评价研究。选取13款具有代表性的口服铁剂产品, 结合电子舌、FaceReader面部表情分析技术及口感访谈, 从体内外不同角度对口服铁剂进行适口性评价。电子舌结果表明, 右旋糖酐铁颗粒最接近无味的氯化钾溶液, 基本没有苦味和涩味, 优于其他产品。FaceReader面部表情结果表明, 58.7%的受试者对右旋糖酐铁颗粒的情绪效价值高于蛋白琥珀酸铁口服溶液。在43名年龄≥ 5岁的参与者中, 右旋糖酐铁颗粒的口感偏好率为83.7%, 83.7%的参与者选择再次服用右旋糖酐铁颗粒。本研究发现右旋糖酐铁颗粒具有较好的口感, 其味道更受儿童的欢迎, 为口服铁剂的儿科临床用药提供了参考依据, 也为儿童用药的适口性评价提供了新思路。本文的体内适口性评价实验属于研究性质的临床试验, 已取得北京儿童医院机构审查委员会批准(编号: 2021-149-Y)。

Palatability evaluation of oral iron for children based on in vivo and in vitro evaluation techniques

口服铁剂是治疗儿童铁缺乏和缺铁性贫血的常用制剂, 但其不良口感成为影响临床服药依从性的主要因素。目前尚未有针对口服铁剂的适口性评价研究。选取13款具有代表性的口服铁剂产品, 结合电子舌、FaceReader面部表情分析技术及口感访谈, 从体内外不同角度对口服铁剂进行适口性评价。电子舌结果表明, 右旋糖酐铁颗粒最接近无味的氯化钾溶液, 基本没有苦味和涩味, 优于其他产品。FaceReader面部表情结果表明, 58.7%的受试者对右旋糖酐铁颗粒的情绪效价值高于蛋白琥珀酸铁口服溶液。在43名年龄≥ 5岁的参与者中, 右旋糖酐铁颗粒的口感偏好率为83.7%, 83.7%的参与者选择再次服用右旋糖酐铁颗粒。本研究发现右旋糖酐铁颗粒具有较好的口感, 其味道更受儿童的欢迎, 为口服铁剂的儿科临床用药提供了参考依据, 也为儿童用药的适口性评价提供了新思路。本文的体内适口性评价实验属于研究性质的临床试验, 已取得北京儿童医院机构审查委员会批准(编号: 2021-149-Y)。

Oral iron is a commonly used preparation for the treatment of iron deficiency and iron-deficiency anemia in children, but its undesirable taste has become a major factor affecting clinical adherence to the medication. No studies have been conducted to evaluate the palatability of oral iron supplements. Thirteen representative oral iron supplements were selected to evaluate the palatability of oral iron supplements from different perspectives of in vivo and ex vivo by combining the electronic tongue (e-tongue) test, FaceReader facial expression analysis technology and taste interviews. E-tongue test results showed that iron dextran granlues were closest to odorless potassium chloride solution, basically free of bitterness and astringency, which was superior to the other products. FaceReader facial expression results showed that 58.7% of the subjects valued iron dextran granlues for their emotional efficacy higher than that of iron proteinsuccinylate oral solution. Among 43 participants aged ≥ 5 years, the taste preference for iron dextran granlues was 83.7%, and 83.7% of participants chose to take iron dextran granlues again. This study found that iron dextran granlues have a better taste and their flavor is more popular among children, which provides a reference for pediatric clinical use of oral iron supplementation preparations and a new idea for the evaluation of palatability of medications for children. This in vivo palatability evaluation is an investigational clinical trial that was approved by the Institutional Review Board of the Beijing Children's Hospital (No. 2021-149-Y).

铁元素是人体必需的微量营养元素之一, 主要存在于铁蛋白、肌红蛋白、血红蛋白和各种酶中, 在许多新陈代谢过程中发挥着重要作用, 如氧运输、电子运输和脱氧核糖核酸合成等[1]。儿童铁缺乏症从多个层面会对儿童健康造成危害, 包括影响生长发育、智力和运动能力降低、免疫功能下降、影响其他营养元素代谢等[2]。口服铁剂是应对儿童铁缺乏症的主要预防和治疗手段, 它的发展经历了不同的研发和应用阶段, 分别是无机盐铁(第一代, 硫酸亚铁、焦磷酸铁等)、小分子有机盐铁(第二代, 乳酸亚铁、富马酸亚铁等)和有机铁络合剂(第三代, 右旋糖酐铁、蛋白琥珀酸铁、多糖铁复合物等)[3]。目前铁剂的主要上市剂型包括片剂、胶囊、糖浆剂、口服溶液剂和颗粒剂等, 其中口服溶液剂和颗粒剂具有服用方便、利于吞咽等优点, 在儿科临床应用中是比较理想的给药剂型[4]。但是, 由于各种不同的铁剂存在不同程度的铁腥味重、矫味掩味过度、刺激性气味等口感问题, 导致患儿用药依从性差, 影响了临床治疗效果。因此, 产品“适口性”是口服铁剂临床应用和药物研发的重要因素。
目前的适口性评价方法主要包括体外溶出法、电子舌法、动物味觉喜好实验、感官评价法(又称人群尝味法)等[5]。电子舌能够将检测到的味觉信号转换为味觉测定值, 具有结果客观性强、检测速度快、重现性好等优势, 适合评价处方筛选阶段大样本量的样品, 避免了人群尝味法对受试者的潜在安全风险[6]。临床常用的感官评价法包括尝味小组、视觉模拟量表、面部表情量表、排序或偏好倾向、多因素调查评价法等, 但在儿童群体应用时存在一定局限性。尤其在临床评价过程中, 开发药物的目标治疗人群是低龄儿童时, 其理解能力和主观表达能力相对较差, 难以通过主观评价或综合打分等方法进行口感评价, 导致评价结果存在偏差。然而, 成人与儿童之间、不同年龄儿童之间的味觉感知、敏感性和耐受性存在差异, 以成人尝味法评价上市的药品用于儿童人群时, 仍然面临儿童用药依从性不佳的问题[7]。目前, 仍然缺乏适宜儿童用药适口性评价的标准规范、客观真实、接受度高的口感评价方法。近年来, 本团队尝试利用面部表情分析系统及深度神经网络算法, 开展羧甲司坦口服溶液在儿童人群中的适口性评价, 体内外评价结果一致并相互验证, 为基于人工智能的面部表情分析技术用于适口性评价提供了参考依据[8]
本研究选取了13款具有代表性的口服铁剂产品, 结合电子舌、FaceReader面部表情分析技术及口感访谈, 从体内外不同角度对口服铁剂进行适口性评价, 建立儿童用药适宜的体内外适口性评价方法, 也为口服铁剂的儿科临床用药选择提供参考依据。
仪器  味觉分析系统-电子舌SA-402B型(日本Insent公司); 面部表情分析系统FaceReaderTM (荷兰Noldus信息技术有限公司); 电子天平ME403/E型(瑞士梅特勒-托利多仪器有限公司); 磁力搅拌器98-2型(上海司乐仪器有限公司)。
试药与试剂  口服铁剂样品分别来自13家生产厂家(表 1); L (+)-酒石酸(批号: 20140901)、氯化钾(批号: 20161130)、氢氧化钾(批号: 20201110)均购自国药集团化学试剂有限公司; 盐酸(批号: 200308010A)、无水乙醇(批号: 191011228H)均购自南京化学试剂股份有限公司。
电子舌待测样品制备方法
A、B、E号  取药品1袋, 加50 mL热开水溶解, 离心后取上清液, 置100 mL量瓶中, 加20 mmol·L-1氯化钾溶液50 mL, 用水稀释定容至100 mL。
C号  取药品1片, 加50 mL温开水溶解, 离心后取上清液, 置100 mL量瓶中, 加20 mmol·L-1氯化钾溶液50 mL, 用水稀释定容至100 mL。
D号  取颗粒矫味剂1袋, 加50 mL热开水溶解, 然后加入1粒胶囊内容物溶解, 离心后取上清液, 置100 mL量瓶中, 加20 mmol·L-1氯化钾溶液50 mL, 用水稀释定容至100 mL。
F~M号  量取药液5 mL置100 mL量瓶中, 加20 mmol·L-1氯化钾溶液50 mL, 用水稀释定容至100 mL。
体外适口性评价方法  采用SA-402B型电子舌, 以电子舌配套的苦味传感器: 盐酸盐类苦味(hydrochloride bitternes, BT0); 基本苦味(basic bitterness, AN0); 酸性苦味(acidic bitterness, C00); 涩味传感器(astringency, AE1), 分别对不同供试液进行测定, 同时以10 mmol·L-1氯化钾溶液作为空白溶液。以空白氯化钾溶液为起始点, 依次从1~13号顺序测定, 表 2为一个样品的测定程序, 每个样品重复测量4次, 取末3次平均数据作为测试结果。将空白氯化钾溶液(10 mmol·L-1)的电位值设置为零点, 测定样品的电位值(Vs)与零点电位值(Vr)的差值被认定为味觉先味值(relative value, R), 传感器测定样品后经清洗溶液4、5清洗传感器各3 s后再次测得样品的电位值Vr′Vr的差值被认定为味觉回味值(change of membrane potential caused by adsorption, CPA)。
体内适口性评价方法
研究设计与参与者  本研究基于体外电子舌试验的评价结果, 结合文献调研中胃肠道不良反应较小、临床应用较多[3, 9], 选择两个代表性产品进一步开展体内适口性评价试验。本研究为随机、双盲、交叉、单中心(首都医科大学附属北京儿童医院)的适口性临床试验, 试验方案由多位临床、药学、伦理学和统计学专家指导和批准。该研究已在线注册(中国临床试验注册中心, ChiCTR2100052443), 符合1975年《赫尔辛基宣言》的伦理指导原则, 并经北京儿童医院机构审查委员会批准(编号: 2021-149-Y)。年龄为2~12岁、遵守研究说明的健康儿童志愿者被纳入本研究。纳入标准为: 体温正常、表观健康。排除标准包括: 无法正常进食、吞咽困难、味觉障碍或精神异常的儿童(不能正确表达真实的味觉), 及对右旋糖酐铁颗粒、蛋白琥珀酸铁口服溶液或具有过敏倾向的受试者, 或正在接受与试验药可能存在配伍禁忌的药物治疗的受试者。本研究招募了60名符合研究说明的2~12岁健康儿童志愿者。通过约10 min的短片和研究者的详细指导, 让儿童及其法定代表人(父母或监护人)了解研究过程, 充分了解研究目的、意义和具体过程。临床试验开始前, 获得8岁或8岁以下儿童的法定代表人知情同意。此外, 8岁以上的参与者自行签署知情同意书, 并得到8岁以上儿童的法定代表人的同意。要求儿童在参与研究前保持清醒, 并且所有的程序都在环保的会议室里进行的。试验前1 h内不服用碳酸饮料、含咖啡因的饮料, 及柚子、酸橙、巧克力等有刺激味道的水果与食物。评测期间, 所有儿童均禁止剧烈运动, 并尽力保持情绪稳定。
随机分配和双盲处理  采用双盲交叉设计, 将受试者随机分配品尝两种补铁制剂。使用SAS 9.4对两种制剂的给药顺序进行随机盲法, 并密封在信封中, 以便紧急揭盲。一名独立的临床药师根据盲码将处方留在配药室。采用5 mL注射器分别吸取2 mL的两种补铁制剂, 取下针头, 并根据序列号放在架子上。注射器的外表面用锡纸避光包裹, 除分配铁剂的临床药师外, 研究者、参与者和参与数据采集的其他人员(包括采访者、数据管理人员和统计人员)均不知道给药顺序。
试验过程  确认研究资格后, 进行了口味评估临床试验(试验流程如图 1所示)。参加试验的儿童按照随机给药顺序分别服用了两种2 mL的补铁制剂。记录给药的整个过程, 并且通过放置在参与者前面的两个相机同时为每个儿童收集给药过程期间的面部表情。在给药过程中, 2岁以下儿童由监护人协助。所有参与者都被要求在服用药物时尽可能面对摄像机。
评估工具  采用FaceReader面部表情分析系统、优先再试法和适口性访谈, 对两种补铁制剂的适口性进行综合评价。采集受试儿童服药过程视频后, 导入FaceReader面部表情分析系统进行数据分析。一般来说人的面部表情会代表不同的心情或感受, 比如高兴、悲伤、愤怒、惊讶、害怕、厌恶和中性等, 该软件系统基于深度神经网络算法(图 2), 对上述七种基本面部表情及复杂环境中的情绪价值、觉醒和头部朝向进行分析。本次研究招募的60名健康志愿者在品尝了这两种药物后, 其中有43名志愿者(5~12岁)的儿童被问及两个问题: ①“刚才喝的两个药, 你更喜欢第一个的口味还是第二个?”②“如果再让你喝一次, 选第一个还是第二个?”, 参与者回答说他们更喜欢哪种制剂, 研究人员负责记录受试者的回答。此外, 需观察参与者在服药过程中是否出现呛咳和呕吐。采用访谈的方式, 由一名经验丰富的研究人员对5~12岁儿童进行访谈, 访问儿童对服药用药的气味、口感、口味与余味的描述。
主要指标是通过FaceReader面部表情分析系统评估儿童参与者服用铁剂的平均情绪效价值。情绪效价值代表服药后表情的愉悦程度, 较高的值对应较高的愉悦程度。积极的情绪效价表明快乐, 而消极的反映不愉快的情绪。次要指标是在给药过程中吐出或未摄入两种制剂, 及对5岁及以上儿童通过访谈进行再次选择的口味喜好率、再试偏好率。
数据分析方法  采用Origin 2018软件处理体外电子舌检测的响应值绘制雷达图。体内适口性评价数据以连续变量的平均值和标准偏差(SD)及分类变量的频率(百分比)表示, 所有数据采用SPSS 21.0 (美国IBM公司)软件进行统计分析。对于计量资料组间比较, 符合正态分布的采用配对t检验, 非正态分布的采用秩和检验。
本研究选用日本INSENT电子舌(SA-402B型), 其采用的人工脂膜传感器技术与人舌头味觉细胞工作原理相类似, 能够数字化地评价样品的苦、涩、酸、咸、鲜、甜等基本味觉感官指标。在图 3中, BT0 (盐酸盐类苦味)、AN0 (基本苦味)、C00 (酸性苦味)、AE1 (涩味)维度分别表示各样品在不同传感器上所测得味觉值, CPA (BT0)、CPA (AN0)、CPA (C00)、CPA (AE1)维度分别表示各样品在BT0、AN0、C00、AE1传感器上所测得味觉回味值。例如, AN0维度上的数值表示不同厂家铁剂样品在AN0传感器上所测得原始响应值(基本苦味), 而CPA (AN0)维度上的数值表示样品在C00传感器上所测得原始响应值(基本苦味回味)。从图 3可以看出, 不同厂家的口服铁剂对苦味传感器AN0和C00的响应值较高, 部分产品对苦味传感器BT0有一定的响应。从回味测定结果(CPA值)来看, 部分产品对BT0及AN0传感器有一定的响应, 对其余两种传感器的响应值较小, 说明各种铁剂在传感器人工脂膜上的残留量较少, 经服用后的苦味回味比较低, 其苦味主要是先味所导致。综合分析, 与不同厂家的口服铁剂相比, D、G~M等产品的响应值较大, 表明苦味较大。产品编号A最接近无味, 基本没有苦味(B-bitterness 2和aftertaste-B)和涩味(aftertaste-A), 苦味和涩味优于其他产品, 然后是产品编号C和F。揭盲发现, A产品为第三代铁剂产品右旋糖酐铁颗粒, 口感优于其他产品。为了进一步评价市售儿童铁剂的适口性, 本研究选择文献调研中胃肠道不良反应较小、临床应用较多的第三代铁剂产品蛋白琥珀酸铁口服溶液(K产品), 对A和K两个产品进行对比研究, 评估其体内适口性。
60名儿童被纳入本研究, 其中女孩43.3%, 男孩56.7% (平均年龄: 7.4 ± 2.9岁)。将两种制剂分为A、B两种药物, 设置编码D001~D060, 生成随机数字表对编码进行随机分组, 确定A、B的服药先后顺序。试验揭盲后统计数据, 本研究中30名儿童先品尝右旋糖酐铁颗粒后再品尝蛋白琥珀酸铁口服溶液(按A-B顺序), 另外30名儿童先品尝蛋白琥珀酸铁口服溶液后再品尝右旋糖酐铁颗粒(按B-A顺序)。
本研究共采集到58组有效视频数据用于FaceReader软件处理分析。FaceReader是用于自动分析面部表情的专业软件, 其应用Viola-Jones算法对人脸位置进行识别, 根据面部超过500个关键点的位置及脸部结构, 建立人脸三维模型, 可分析高兴、悲伤、愤怒、惊讶、害怕、厌恶和中性这七种基本情绪, 能够准确、可靠地识别面部表情。FaceReader软件分析面部表情视频后, 可输出情绪效价值代表服药后表情的愉悦程度, 较高的值对应较高的愉悦程度。如图 4结果所示, 34名受试者(58.7%)品尝右旋糖酐铁颗粒的情绪效价值高于蛋白琥珀酸铁口服溶液, 24名受试者(41.3%)对蛋白琥珀酸铁口服溶液的情绪效价较高。通过t检验, A药和B药两组口服铁剂的效价值存在明显差异(P < 0.05), 表明A药(右旋糖酐铁颗粒)的适口性好于B药(蛋白琥珀酸铁口服溶液)。
此外, 本研究对入组的5岁以上的43名志愿者开展主观评价, 即口味喜好和再试偏好, 及安全性评价结果, 即是否发生呛咳或呕吐。从表 3的结果可以看出, 无论是先服用A药(右旋糖酐铁颗粒)还是服用B药(蛋白琥珀酸铁口服溶液), A药的口味偏好率和再试偏好率均高于B药。右旋糖酐铁颗粒的口味偏好率为83.7% (36/43) (口味偏好缺失1个值), 如果必须再次服药, 83.7% (36/43)的受试者选择右旋糖酐铁颗粒。志愿者访谈环节的问题可详细参见“体外适口性评价方法”。然而, B药(蛋白琥珀酸铁口服溶液)的“吐出”发生率高于A药(右旋糖酐铁颗粒), 但差异无统计学意义。综上, 本研究结果发现A药的适口性好于B药, 为儿科口服铁剂的临床应用提供了参考依据。本研究未观察到不良事件。
口服铁剂是儿童铁缺乏和缺铁性贫血的一线治疗方式[3, 9, 10]。但是目前市场上口服补铁药物种类繁多, 且不同口服铁剂的成分、口感、疗效、不良反应等存在较大差异。研究发现, 口味不佳是导致口服铁剂在儿科临床用药依从性差的主要影响因素[3]。口服铁剂的第1代和第2代产品存在较重铁腥味, 通过分子结构的改良和矫掩味技术, 第3代产品的口感有所改善, 但仍影响儿科用药依从性。口感问题已成为口服铁剂儿科临床应用的重要考量因素, 目前尚缺乏针对口服铁剂全面的、客观的适口性评价研究。
本研究首先采用体外电子舌技术对不同厂家的13款代表性口服铁剂进行大规模的体外适口性筛选, 通过盲法评估了不同口服铁剂产品的口感, 从而判断口感最佳的产品。基于电子舌检测结果和文献调研分析, 进一步开展了右旋糖酐铁颗粒与蛋白琥珀酸铁口服溶液在儿童人群的适口性评价研究, 创新采用FaceReader面部表情分析技术分析儿童服药的真实感受, 结合口感访谈和安全性评价结果综合分析适口性最佳的产品。本研究不仅为口服铁剂的儿科临床用药选择提供了参考依据, 也同时促进矫掩味技术的发展和儿童用药适口性评价体系的建立。
目前中国儿童用药适口性评价仍处于起步阶段, 尚有诸多问题亟待解决。对于电子舌技术, 其在早期的适口性评价中适应性较强, 但仍需根据药物的剂型特点、苦味成分组成等选择适宜的电子舌检测设备[11]。此外, 本课题组初步论证了FaceReader面部表情分析技术应用于儿童用药口感评价的可行性, 但仍有其局限性。FaceReader是以成人面部表情为基础数据开发的一款表情识别软件, 目前已应用于心理学研究、人机交互反应、食品偏好研究等非医疗场景[12-14]。本研究在预试验阶段采集了5名不同年龄健康儿童的3~5 min自然面部表情, 并且在品尝药物口感之前根据儿童的中性面部表情进行校准, 以尽量减少数据分析过程中对特定面部特征的偏差。虽然在预试验阶段以5名不同年龄健康儿童的面部表情数据建立了小样本的儿童面部表情参数, 但仍然缺乏儿童面部表情的基本数据和参数。因此, 后续的研究中应通过大样本采集建立中国儿童面部参数和基础数据, 进一步促进面部表情分析技术的细化和准确。总的来说, 本研究为儿童用药的适口性评价提供了新思路。
