Coronary heart disease (CHD) and stroke are the most well-known cardiovascular diseases, which share many common pathological basis. Yindan Xinnaotong soft capsule (YDXNT) is a commonly used Chinese patent medicine in the treatment of stroke and CHD. However, its action of mechanism of co-treatment for stroke and CHD is still unclear. The aim of this study was to explore the common mechanism of YDXNT in co-treatment of CHD and stroke using network pharmacology, experimental verification and molecular docking. An integrated literature mining and databases of IPA, ETCM, HERB, Swiss Target Prediction, OMIM and GeneCards were used to screen and predict active ingredients and potential targets of YDXNT in co-treatment of CHD and stroke. The protein-protein interaction network, GO analysis and pathway analysis were analyzed by IPA software. The effect of YDXNT on core targets was verified by immunofluorescence. UPLC-QTOF/MS and molecular docking were used to screen and predict the main active constituents of YDXNT and their interactions with core targets. A total of 151 potential targets are predicted for YDXNT in co-treatment of CHD and stroke. Hypoxia-inducible factor-1α (HIF1α)-matrix metalloproteinase-9 (MMP9)-mediated HIF1α signaling pathway serves as one of the common mechanisms. YDXNT could reduce the increase of mitochondrial fluorescence intensity and the protein expression of HIF1α and MMP9 in HL-1 and HA induced by oxygen and glucose deprivation/reperfusion (OGD/R) in a dose-dependent manner. Baicalin may be the material basis for treating stroke and CHD with YDXNT. In conclusion, the HIF1α signaling pathway is one of the common key mechanisms of YDXNT in the co-treatment of stroke and CHD. The study provides support and basis for the in-depth scientific connotation of the traditional Chinese medicine theory of "same treatment to different diseases".

冠心病和脑卒中是最常见的心脑血管疾病, 具有诸多共同的病理基础。银丹心脑通软胶囊(YDXNT) 是治疗脑卒中和冠心病的常用药, 但其治疗二者的共同机制尚不十分清楚。运用网络药理、实验验证、分子对接等手段探究YDXNT基于“异病同治”理论治疗脑卒中和冠心病的共同机制。通过文献挖掘并利用IPA、ETCM、HERB、Swiss Target Prediction、OMIM和GeneCards等网络数据库筛选并预测YDXNT治疗冠心病和脑卒中的潜在共同靶点。通过IPA软件进行PPI、GO和KEGG分析, 得到YDXNT治疗脑卒中和冠心病共同的核心靶点、通路和功能。通过免疫荧光验证YDXNT对核心靶点的作用。应用UPLC-QTOF/MS和分子对接筛选和预测YDXNT主要活性成分及其和核心靶点的相互作用。网络分析共得到YDXNT治疗脑卒中和冠心病的151个共同靶点, 缺氧诱导因子1α (hypoxia-inducible factor-1α, HIF1α)-基质金属蛋白酶9 (matrix metalloproteinase-9, MMP9) 介导的HIF1α信号通路为其共同机制之一。YDXNT可呈剂量依赖性地降低氧糖剥夺/复氧诱导的心肌细胞(HL-1) 和星形胶质细胞(HA) 中线粒体质量的增加及HIF1α和MMP9蛋白的表达。野黄芩苷可能为YDXNT治疗缺血性脑卒中和冠心病的主要活性物质。综上, HIF1α-MMP9介导的HIF1α信号通路为YDXNT治疗缺血性心脑疾病的共同机制之一, 为中医药“异病同治”理论科学内涵的深入研究提供支撑和依据。

冠心病和脑卒中是最常见的心脑血管疾病, 具有诸多共同的病理基础。银丹心脑通软胶囊(YDXNT) 是治疗脑卒中和冠心病的常用药, 但其治疗二者的共同机制尚不十分清楚。运用网络药理、实验验证、分子对接等手段探究YDXNT基于“异病同治”理论治疗脑卒中和冠心病的共同机制。通过文献挖掘并利用IPA、ETCM、HERB、Swiss Target Prediction、OMIM和GeneCards等网络数据库筛选并预测YDXNT治疗冠心病和脑卒中的潜在共同靶点。通过IPA软件进行PPI、GO和KEGG分析, 得到YDXNT治疗脑卒中和冠心病共同的核心靶点、通路和功能。通过免疫荧光验证YDXNT对核心靶点的作用。应用UPLC-QTOF/MS和分子对接筛选和预测YDXNT主要活性成分及其和核心靶点的相互作用。网络分析共得到YDXNT治疗脑卒中和冠心病的151个共同靶点, 缺氧诱导因子1α (hypoxia-inducible factor-1α, HIF1α)-基质金属蛋白酶9 (matrix metalloproteinase-9, MMP9) 介导的HIF1α信号通路为其共同机制之一。YDXNT可呈剂量依赖性地降低氧糖剥夺/复氧诱导的心肌细胞(HL-1) 和星形胶质细胞(HA) 中线粒体质量的增加及HIF1α和MMP9蛋白的表达。野黄芩苷可能为YDXNT治疗缺血性脑卒中和冠心病的主要活性物质。综上, HIF1α-MMP9介导的HIF1α信号通路为YDXNT治疗缺血性心脑疾病的共同机制之一, 为中医药“异病同治”理论科学内涵的深入研究提供支撑和依据。

Coronary heart disease (CHD) and stroke are the most well-known cardiovascular diseases, which share many common pathological basis. Yindan Xinnaotong soft capsule (YDXNT) is a commonly used Chinese patent medicine in the treatment of stroke and CHD. However, its action of mechanism of co-treatment for stroke and CHD is still unclear. The aim of this study was to explore the common mechanism of YDXNT in co-treatment of CHD and stroke using network pharmacology, experimental verification and molecular docking. An integrated literature mining and databases of IPA, ETCM, HERB, Swiss Target Prediction, OMIM and GeneCards were used to screen and predict active ingredients and potential targets of YDXNT in co-treatment of CHD and stroke. The protein-protein interaction network, GO analysis and pathway analysis were analyzed by IPA software. The effect of YDXNT on core targets was verified by immunofluorescence. UPLC-QTOF/MS and molecular docking were used to screen and predict the main active constituents of YDXNT and their interactions with core targets. A total of 151 potential targets are predicted for YDXNT in co-treatment of CHD and stroke. Hypoxia-inducible factor-1α (HIF1α)-matrix metalloproteinase-9 (MMP9)-mediated HIF1α signaling pathway serves as one of the common mechanisms. YDXNT could reduce the increase of mitochondrial fluorescence intensity and the protein expression of HIF1α and MMP9 in HL-1 and HA induced by oxygen and glucose deprivation/reperfusion (OGD/R) in a dose-dependent manner. Baicalin may be the material basis for treating stroke and CHD with YDXNT. In conclusion, the HIF1α signaling pathway is one of the common key mechanisms of YDXNT in the co-treatment of stroke and CHD. The study provides support and basis for the in-depth scientific connotation of the traditional Chinese medicine theory of "same treatment to different diseases".

心脑血管疾病占中国城乡居民总死亡原因的首位, 其患病率和死亡率仍稳步上升, 在全球造成巨大的健康和经济负担, 心脑血管系统疾病也与目前全世界范围内流行的新型冠状病毒感染有着高度的联系[1, 2]。冠心病和脑卒中是全世界最常见的两种心脑血管系统疾病, 其共同病理基础为动脉粥样硬化, 且均与炎症、血栓、血脂异常、细胞自噬和凋亡、肠道菌群失调等有关, 而中医理论认为, 二者均由阳气不足、易感外邪、气血亏虚、瘀滞血脉等所导致[3-10]。越来越多的证据显示大脑和心脏关系密切, 神经系统和心血管系统在生理和病理条件下存在错综复杂的相互作用[11]。“异病同治”是在中医辩证思想基础上的延伸法则, 常见于治疗不同疾病的相同或相似的证候或病机, 反映了中医因证而治的特点[12]
银丹心脑通软胶囊(YDXNT) 是我国近几十年治疗心脑血管疾病常用药, 由银杏叶、丹参、灯盏细辛、山楂、绞股蓝、三七、艾片、大蒜8味药物组成, 主治功效为活血化瘀、行气止痛、消食化滞, 可用于治疗胸痹、心绞痛、冠心病、动脉粥样硬化、脑卒中等症[13-15]。网络药理学是一个多学科的研究领域, 是为了对中药方剂的物质基础、生物效应及其作用机制进行综合分析产生的新兴交叉学科[16, 17]。目前, 虽然临床研究显示银丹心脑通对冠心病和脑卒中具有良好的疗效, 但两者相互联系的共同作用机制尚不完全明确。因此, 本文在“异病同治”理论的指导下, 运用网络药理分析及验证、分子对接筛选相结合的方法探究银丹心脑通治疗脑卒中和冠心病的共同机制, 从而进一步揭示中药“异病同治”的作用特点, 为中医药防治心脑血管疾病提供思路和理论基础。
试剂和细胞  YDXNT (批号: 20210914) 由贵州百灵企业集团制药股份有限公司提供; 酒石酸美托洛尔注射液(批号: 2101152231) 购自辰欣药业股份有限公司; 依达拉奉注射液(批号: 220050) 购自国药集团国瑞药业有限公司; 野黄芩苷(DSTDY002601)、大蒜素(DST211022-142)、丹酚酸B (DSTDD000903) 均购自成都乐美天医药科技有限公司; 缺氧诱导因子1α (HIF1α) 抗体(AF1009) 购自Affinity公司; 基质金属蛋白酶9 (MMP9) 抗体(sc-393859) 购自美国Santa Cruz公司; DMEM培养基、青霉素、链霉素购自美国Hyclone公司; 山羊抗小鼠Alexa Fluor647购自美国Abcam公司; 山羊抗兔二抗Alexa Fluor647、Mitotracker Deep Red FM (M22426)和Hoechst 33342(H1399) 购自赛默飞世尔科技有限公司; 4%多聚甲醛购自北京索莱宝公司; CCK-8试剂盒(HY-K0301) 购自美国MedChem Express公司; 无糖DMEM培养基和心肌细胞(HL-1) 购于武汉普诺赛生命科技有限公司(CL-0605); 星形胶质细胞(HA) 细胞由天津中医药大学胡利民老师课题组提供(XY-XB-2238)。
YDXNT主要活性成分及相关靶点的筛选  在目前已发表的YDXNT及其组成中药的化学成分液-质或气-质鉴定的相关文献中筛选其主要活性成分。YDXNT活性成分的靶点通过以下两种方法进行获取: ①查阅国内外相关文献[18-20]; ②利用IPA数据库, 以及Swiss Target Prediction (http://www.swisstargetprediction.ch/)、ETCM (http://nwww.tcmip.cn/ETCM/)、HERB (http://herb.ac.cn/) 等数据库进行筛选补充。
冠心病和脑卒中相关疾病的靶点预测  在已发表的冠心病和脑卒中相关研究的文献基础上, 结合以“coronary heart disease”和“stroke”为关键词在IPA、OMIM (https://www.omim.org/) 和GeneCards (https://www.genecards.org/) 等网络数据库中的检索结果进行整合去重, 得到冠心病和脑卒中的相关靶点[21]。将YDXNT活性成分的靶点与冠心病和脑卒中的疾病靶点取交集, 得到YDXNT治疗冠心病和脑卒中的潜在共同靶点。
网络的构建与分析  将以上重合靶点导入IPA中利用“core analysis”功能进行网络和通路分析。“Build-connection”模块进行蛋白互作分析; “canonical pathway”与常用的KEGG分析类似, 通过靶点之间相互联系分析得到相关通路; “diseases & functions”与常用的GO分析相似, 分析结果为靶标涉及的生物学过程和分子功能。
YDXNT母液的配制  YDXNT的配置如前所述[22]。称取YDXNT内容物160 mg, 加入40 mL DMEM高糖培养液进行溶解, 超声处理2 h, 多次离心取上清, 用0.45 μm微孔滤膜过滤, 最终得浓度为4 mg·mL-1 YDXNT母液, 分装后-20 ℃保存。
CCK-8检测细胞活力  96孔板每孔加入4×103个HL-1细胞或5×103个HA细胞, 培育24 h后对照组的细胞只加基础培养基, 实验组中加入不同浓度的YDXNT (25、50、100、200、400、800、1 600 μg·mL-1), 培养24 h后, 每孔加入10 μL CCK-8试剂, 37 ℃孵育1~4 h, 每30 min检测450 nm波长处的吸光度值。
氧糖剥夺/复氧 (OGD/R) 细胞模型  将HL-1细胞以4×103个/孔密度、HA细胞以5×103个/孔密度, 每孔100 μL的体积接种到96孔培养板中。24 h后更换培养基, 正常组每孔加入无血清的正常DMEM培养基, OGD/R组及给药组加入无血清的无糖DMEM培养基。将含有OGD/R组及给药组的96孔培养板放入缺氧小室中, 关闭出气口, 通入15 min 95% N2和CO2混合气体, 随后将缺氧小室放入37 ℃培养箱中进行氧糖剥夺。4 h后, 吸出培养基, 给予100 μL不同浓度的YDXNT, 培养24 h后, 吸弃上清液, 加入含Hoechst 33342和MitoTracker Deep Red FM的DMEM培养基, 37 ℃下避光孵育30 min, 反复冲洗3次, 最后利用Operetta高内涵细胞成像系统对其进行分析, 检测并计算染料的荧光强度。
免疫荧光  将96孔培养板上的HL-1细胞和HA细胞造模及给药后, 用4%多聚甲醛固定, 5%胎牛血清封闭2 h, 加入HIF1α或MMP9抗体(1∶200) 于4 ℃过夜。次日与相应的荧光二抗和Hoechst 33342在室温下孵育2 h, 反复洗板后用Operetta高内涵细胞成像系统对其进行分析, 检测并计算不同蛋白的荧光强度。
分子对接  通过查阅参考文献[23-25]和《中国药典》综合筛选出YDXNT中的8种质量标志物(Q-marker) 银杏内酯B、野黄芩苷、左旋龙脑、人参皂苷Rb1、三七皂苷R1、枸橼酸、丹酚酸B和大蒜素与筛选出的关键靶点进行分子对接, 进一步研究分子间的相互作用。于RCSB PDB数据库(https://www.rcsb.org/) 下载HIF1α和MMP9蛋白的X射线3D结构, PubChem数据库(https://pubchem.ncbi.nlm.nih.gov/) 下载YDXNT活性成分的化学结构。利用Discovery Studio 2019软件进行分子对接并来展示蛋白质和配体之间的作用关系。“Macromolecules”模块用于蛋白质制备, “small molecules”模块用于配体的制备, 于“receptor-ligand interactions”模块中基于CHARMm的分子动力学方法(CDOCKER) 计算YDXNT与靶标蛋白HIF1α和MMP9之间的相互作用, 并以2D和3D的互作图进行展示。
YDXNT成分鉴定  使用Agilent 6550 UPLC-QTOF/MS液-质联用仪对可用于液-质联用的6种成分银杏内酯B、黄芩苷、人参皂苷Rb1、三七皂苷R1、枸橼酸和丹酚酸B进行化学成分鉴定。
供试品溶液制备  将YDXNT去其外壳, 取内容物共0.597 0 g, 加75%甲醇60.00 mL, 超声震荡提取30 min, 超声完毕后以75%甲醇补重, 离心, 取上清, 上清液稀释4倍后过0.22 μm微孔滤膜测定样品。
对照品溶液制备  称取三七皂苷R1、人参皂苷Rb1、丹酚酸B、银杏内酯B和枸橼酸对照品适量, 加入75%甲醇配置成100.00 μg·mL-1标品母液, 以上标品母液各取100.0 μL混合配制成20 μg·mL-1混标样品, 以相同方法配置成20 μg·mL-1以3%吡啶-75%甲醇溶解的野黄芩苷单标样品, 以及75%甲醇溶液的空白对照样品。
色谱条件  Agilent 6550 UPLC-QTOF/MS液-质联用仪, 色谱柱: Agilent ZORBAX Eclipse Plus C18 column (4.6 mm×150 mm, 1.8 μm), 流速0.3 mL·min-1, 柱温30 ℃, 进样量2 μL, 以乙腈(A) 和0.1%甲酸-水(B) 为流动相, 色谱洗脱程序为0 min: 5% A; 6 min: 12% A; 9 min: 14% A; 27 min: 27% A; 40 min: 55% A; 58 min: 100% A; 66 min: 100% A。
质谱条件  电喷雾离子源(负离子模式); 干燥器温度(gas temp.): 200 ℃; Sheath gas temp.: 325 ℃。干燥气体流量(drying gas): 12 L·min-1; 鞘气压力: 40 psi; Fragmentor: 350 V; 喷嘴电压(nozzle voltage): 2 000 V; 毛细管电压(capillary voltage): 4 000 V; 质谱扫描范围m/z: 50~1 500。MS采集速率分别是3 spectra和333.3 ms/spectrum。
统计学分析  对实验所得数据, 使用GraphPad Prism 7.00软件进行统计分析与作图, 多组间比较使用单因素方差分析法(one-way ANOVA), 当P < 0.05认为两样本差距具有统计学意义。
于已发表文献中筛选得到30种YDXNT的活性成分[24-35], 包括槲皮素、山柰酚、异鼠李素、银杏内酯A、银杏内酯B、银杏内酯C、白果内酯、野黄芩苷、木犀草素、丹参酮ⅡA、丹酚酸A、丹酚酸B、丹参素、隐丹参酮、原儿茶醛、灯盏乙素、灯盏花素、人参皂苷Rb1、β-谷甾醇、枸橼酸、芹菜素、大蒜素、三七皂苷Rb1、左旋龙脑、芦丁、迷迭香酸、咖啡酸、月桂酸、蒜氨酸、商陆素, 8味中药与活性成分的对应关系(图 1)。基于文献挖掘和数据库筛选得到YDXNT相关靶点1 146个, 冠心病相关靶点1 332个, 脑卒中相关靶点874个, 其中重合靶点共151个, YDXNT的活性成分与重合靶点的对应关系(图 2)。
通过IPA软件对151个YDXNT治疗冠心病和脑卒中的关键靶点进行蛋白互作分析, 并按节点数从大到小进行排序。排名较靠前的靶点分别为TNF、IL6、IL1B、IFNG、TGFB1、TLR4、IL10、HIF1A、PTGS2、PPARG等, 具体靶点名称及其对应的节点数见图 3
为深入揭示YDXNT通过“异病同治”体系治疗冠心病和脑卒中的潜在机制, 对YDXNT防治冠心病和脑卒中的关键靶点进行功能及通路分析, 发现其涉及的主要通路为动脉粥样硬化、心肌肥厚、神经炎症、凝血系统、肝X受体/视黄醛受体激活和HIF1α信号通路等, 排名前15名的关键通路, 见图 4A; 涉及的生物功能和疾病包括心血管疾病、神经系统疾病、多器官损伤和异常、骨骼和肌肉疾病、炎症反应和内分泌系统疾病等, 见图 4B。随后对图 3所示的前20个靶点进行进一步的PPI分析, 发现HIF1A排在核心位置, 靶点间相互关系, 见图 4C。且与HIF1A最为相关的HIF1α通路位于YDXNT治疗冠心病和脑卒中的前15条通路中, 这表明HIF1A可能是YDXNT发挥“异病同治”机制的相对关键的靶点。HIF1A及其下游分子MMP9涉及的HIF1α信号通路, 见图 4D
综合以上结果, 本课题组基于“异病同治”理论的网络药理分析预测YDXNT可通过HIF1α-MMP9介导HIF1α信号通路发挥防治冠心病和脑卒中的作用。
为进一步探究YDXNT治疗脑卒中和冠心病的关键机制, 本研究选用HL-1心肌细胞和HA星形胶质细胞进行验证。图 5显示, YDXNT浓度在1 600 μg·mL-1时对HL-1细胞有明显的毒性, 但对HA细胞没有毒性, 在浓度为0~800 μg·mL-1时, 对两种细胞均没有毒性。因此, 本研究在无毒浓度范围内选取了25、100、400 μg·mL-1作为后续验证实验的低、中、高浓度。
通过高内涵多细胞成像系统对HL-1和HA细胞的线粒体功能指标进行了检测, 结果显示, 与正常对照组相比, 两种细胞的模型组MitoTracker荧光强度明显增强, 表明线粒体质量增加, 给药组荧光强度降低(图 6A7A)。综上, 不同浓度的YDXNT (25、100和400 μg·mL-1) 可以减缓线粒体质量, 有效保护OGD/R诱导的HL-1细胞和HA细胞的线粒体功能, 改善心肌细胞及脑细胞损伤。
随后, 本研究采用免疫荧光在HL-1和HA细胞上对YDXNT治疗脑卒中和冠心病共同关键靶点HIF1α和MMP9进行验证。结果显示, 与对照组相比, OGD/R诱导的模型组HIF1α和MMP9的蛋白表达显著升高, 给予不同浓度的YDXNT (25、100和400 μg·mL-1) 和阳性药后能有效抑制HIF1α和MMP9蛋白的表达(图 6BC7BC)。
为确定YDXNT与HIF1α分子相互作用的主要活性成分, 通过分子对接对YDXNT的8个Q-marker成分进行筛选。分子对接结果显示, YDXNT中与HIF1α的亲和力排名靠前的化合物为枸橼酸、野黄芩苷、左旋龙脑和大蒜素, 其亲和力依次为44.65、27.35、21.72和21.54; 与MMP9亲和力排名靠前的化合物为野黄芩苷、银杏内酯B、左旋龙脑等, 对应的亲和力依次为50.63、36.34、28.72。图 8图 9分别展示了HIF1α与枸橼酸和野黄芩苷及MMP9与野黄芩苷和银杏内酯B相互作用的2D、3D图。综合以上结果表明, 野黄芩苷与两个蛋白的结合均排在前两位, 因此它可能是YDXNT中治疗脑卒中和冠心病的重要物质基础。
对YDXNT样品在负离子下的UPLC-QTOF/MS液质数据进行采集, 其总离子流图(TIC图) 结果见图 10, 对标品比对的各成分液质分析结果如表 1。结果表明,YDXNT存在与混合标准品相同的色谱峰, 证明YDXNT中含有银杏内酯B、野黄芩苷、人参皂苷Rb1、三七皂苷R1、枸橼酸和丹酚酸B 6种成分。
YDXNT是根据苗族医学“两病两纲”理论研制的复方制剂, 是目前临床上广泛用于治疗心脑血管疾病的大品种民族药物, 其先后被纳入《中成药临床应用指南·心血管分册》、《冠心病稳定型心绞痛中医诊疗专家共识》、《冠心病稳定型心绞痛中医诊疗指南》的推荐用药[26, 36]。前期研究表明, YDXNT可降低血液高黏滞状态、血浆中同型半胱氨酸和低密度脂蛋白胆固醇水平, 从而降低冠心病患者心绞痛的发作时间和频率[37, 38]; 通过降低全血黏度, 改善血液循环, 加速血栓溶解和改善神经功能缺损等治疗脑卒中及脑卒中后遗症[39-41]; 还可通过调节脂质、保护血管内皮、抗氧化、抗炎等协同缓解动脉粥样硬化[14, 42]; 或通过抑制心肌组织中骨桥蛋白或Bcl-2、Bax的表达来保护急性心肌梗死后的心肌细胞及改善细胞凋亡[43, 44]; 脑卒中和冠心病均为动脉粥样硬化相关的心脑血管疾病, 均以络脉为病变部位, 且有着共同的致病因素、病机演变和病理基础[10, 45]。因此, 本文从异病同治理论进一步探究YDXNT对脑卒中和冠心病的共同治疗机制。本文从细胞层面验证, YDXNT可以保护心肌细胞和脑细胞, 改善缺血性损伤, 从而治疗冠心病及脑卒中, 并且HIF1α和MMP9为关键的作用靶标。
缺血和缺氧均与冠心病和脑卒中的发病机制有着紧密的联系。冠心病是由于冠状动脉粥样硬化斑块引起的冠状动脉狭窄导致灌注减少和心肌缺血和缺氧。脑卒中是一种急性脑血液循环障碍性疾病, 也在很大程度上取决于受影响脑区与缺氧相关的神经元死亡的数量[46]。HIF1α作为缺氧条件下的重要调节因子, 是氧稳态的主要调节剂[47]。据研究报道, HIF1α在缺氧/缺血后迅速表达, 并且可通过血管生成、糖代谢、炎症、氧化应激、细胞凋亡和能量代谢等多途径参与脑卒中的病理过程[48, 49]。HIF1α与冠状动脉斑块的不稳定性紧密相关, 可抑制血小板凝聚、促进致密血管的增生及缺血心肌新生血管的形成, 影响易损斑块的形成和发展[50, 51]。MMP9是细胞外基质降解的生物标志物, 在全身性高血压、心肌梗死和心肌梗死后心力衰竭以及缺血性脑卒中中起重要作用[52]。研究表明, 脑卒中发作时血清MMP9水平可能是缺血性卒中的重要预后因素, 可调节细胞的通透性, 在急性期之后, MMP9可能在脑卒中的恢复中发挥有益作用[53-55]。在冠心病患者中, MMPs可以调节动脉粥样硬化中的各种细胞和信号通路, MMP9水平与冠状动脉粥样硬化斑块坏死核心的大小呈正相关[56]
目前, 越来越多的研究证明大脑与心脏之间有着密切的联系, 然而YDXNT治疗心脑血管的研究多为临床, 并且大多将其治疗脑卒中和冠心病的机制分割开来, 对脑心同治的研究有所缺失。因此, 本文基于“异病同治”理论, 利用网络药理、靶标验证和分子对接筛选等方法综合探讨了YDXNT治疗冠心病和脑卒中的共同机制, 实验结果表明YDXNT可改善损伤的HL-1细胞和HA细胞的线粒体功能, 并且可通过抑制OGD/R诱导的HL-1和HA细胞中HIF1α和MMP9的表达水平, 从而发挥保护作用。这不仅为中医药治疗心脑血管疾病提供依据, 而且为“异病同治”理论的理解和深入起到促进作用。
