新药创制是复杂的智力活动, 涉及科学研究、技术创造、产品开发和医疗效果等多维科技活动。每个药物都有自身的研发轨迹, 而构建化学结构是最重要的环节, 因为它涵盖了药效、药代、安全性和生物药剂学等多维性质。本栏目以药物化学视角, 对有代表性的药物的成功构建, 加以剖析和解读。
本文讨论了继9年前首创的芬戈莫德之后的跟随性药物辛波莫德的研制历程。首创药物的风险在于对靶标可药性自始至终的验证, 风险贯穿于始末, 乃至上市后; 跟随研制药物的难度在于风险置后于批准上市后的命运。这就要求跟随研制应优于或不劣于首创品, 因而在启动项目前须制定研发的目标, 有针对性地克服先行药物之不足。辛波莫德的优化内容是, 设定了避免前药活化过程, 和不干扰S1P3受体的功能, 体现在药代和选择性的优势, 当然在活性强度、物化和安全性方面也要满足成药的属性。本药品结构中同时存在必需的药效团胺基和羧基, 避免了磷酸的活化带来的多余负电荷, 但同时存在两性基团, 对吸收可能有不利影响, 这就要求化合物的高强度活性, 低剂量弥补物化性质的不足。

鞘氨醇-1-磷酸(1, S1P) 是鞘脂类的一种活性成分, 对体内多种生理功能有调节作用, 如淋巴细胞运行、心脏功能、血管发育和炎症等。S1P的这些生理活性是通过与5种S1P受体亚型结合起作用的, S1P1~S1P5都是G蛋白偶联受体。
2010年FDA批准的S1P1受体调节剂芬戈莫德(2, fingolimod) 上市, 治疗复发性多发性硬化病。2通过抑制胸腺和淋巴结释放淋巴细胞, 阻止外周炎性组织的淋巴细胞的再分布。芬戈莫德是个前药, 在体内需经活化, 经鞘氨醇激酶催化生成磷酸酯起作用, 以纳摩尔水平与4种受体结合, 其中主要对S1P1受体呈调节作用, 因而呈现免疫抑制(Brinkmann V, Billich A, Baumruker T, et al. Fingolimod (FTY720): discovery and development of an oral drug to treat multiple sclerosis. Nat Rev Drug Discov, 2010, 9: 883-897)。临床的不良反应是发生剂量依赖性的瞬息心率减慢, 是由于干扰了S1P3受体的缘故。诺华公司研制新的药物的目标, 是对S1P1和S1P5双重激动剂, 而不影响S1P3的功能。另一目标是去除体内活化的前药性质, 将酸性基团置于药物结构之中, 以降低研制过程的复杂性。
评价目标化合物的活性是分别用表达人S1P1和S1P3蛋白的N-段质粒转染CHO细胞, 得到高表达相应的受体蛋白两种细胞。细胞悬浮在冷却的营养液中匀浆化, 离心, 得到2~3 mg·mL-1膜蛋白, 加入全基因扩增SPA磁珠和不同浓度的受试物, 室温定时温孵后, 加入定量的GTPγ35S, 温孵后离心, 用多功能同位素测定仪(TOPCount) 测定与GTPγ35S结合的放射性活性, 分别计算对S1P1和S1P3受体的EC50
为了提高化合物的选择性, 将芬戈莫德结构中正辛基用联苯肟醚的刚性结构替换, 化合物3对S1P1的活性与芬戈莫德相同, 但消除半衰期也很长(t1/2 > 30 h), 有较高的分布容积(Vss = 36.9 L·kg-1) 的缺点, 这是由于活化后的单磷酸酯结构中有剩余的负电荷, 易于同组织中的脂蛋白结合。因而结构变换的策略是在分子中预构一个羧基, 不必到体内经磷酸酯化, 也避免研发过程的复杂性。
图 1是探索的多种胺基羧酸的模块, 通过与芳香醛经还原胺化制备多样性胺基羧酸的末端结构, 在这有利于制备的组装式的合成中, 发现化合物4对S1P1的EC50达到300 nmol·L-1, 联苯片段是满足受体对疏水性结合的要求, 从而以4为先导化合物进行了优化。
首先优化的部位是在联苯上做不同取代, 代表性化合物的结构及其对S1P1受体的激动作用列于表 1。分析构效关系显示: ①苯环A的2位取代有利于提高激动活性, 例如2-F (6) 和2-CF3 (8) 化合物的活性都强于无取代的3, 尤其是2位的三氟甲基取代, 活性强度达到纳摩尔水平。② A环的1位取代不利于提高活性。例如1-F (5) 的活性弱于6一倍, 1-CF3 (7) 的EC50值为10 μmol·L-1, 提示1位不宜连接基团。③在B环作单取代, 无论哪个位置(1′, 2′和3′) 都不利于活性, 如化合物9~14活性都较低。④在A环作2-CF3取代、B环的任何位置引入取代基都使活性显著提高, 例如化合物15~18活性都较高。但多数没有超过化合物8
在后来获得了化合物4与S1P1蛋白的晶体结构后(Hanson MA, Roth CB, Jo E, et al. Crystal structure of a lipid G-protein-coupled receptor. Science, 2012, 335: 851-855), 对上述的构效关系进行了分子模拟验证, 得到了合理的解释。图 2显示化合物4与S1P1蛋白的晶体结构的结合模式, 分子的弯曲形状是在肟醚键处为拐点; 蓝色代表正电荷区域, 红色为负电荷区域, 绿色为疏水性区域。4的结合模式概括如下: ① 4与S1P1的结合部位是封闭而大的疏水腔, 入口处有电荷分布。②羧基负离子与质子化的Lys34和Arg120形成盐键, 还与Tyr29的羟基发生氢键结合。③质子化的胺基与Glu121的侧链羧基形成盐键。这样, 羧基和胺基与受体形成多离子键结合, 将化合物锁定在受体结合域中。④联苯基充满于疏水腔中, 发生疏水-疏水相互作用。⑤分子模拟提示, 化合物6的2-F增加了疏水结合, 使活性提高, 化合物8的2-CF3接近Leu276和Leu272, 发生强力的疏水相互作用, 活性进一步提高。而且2-位的大基团迫使B环呈近于正交取向, 有利于同疏水腔发生范德华引力作用。而化合物57的取代基处在1位, 不能发生这些结合作用。
下一步的优化是深入考察苯环B与疏水腔的结合作用, 为此, 固定A环为2-CF3取代, 合成了表 2的代表性化合物, 评价了目标物对S1P1和S1P3的双重激动活性, 以优化选择性作用。用呋喃或噻吩替换末端的苯基, 化合物1920的活性与8相近, 当用环戊基或环己基替换, 2122对S1P1的激动作用提高了10倍, 而对S1P3的作用弱, 提示饱和脂肪环的变换对活性强度和选择性都是有利的。
下一步是优化苯环C, 为此固定疏水端为3-CF3-4-环己基取代的苯基片段, 变换苯环C环及其连接的取代基, 合成的代表性化合物列于表 3。构效关系简述如下: ①苯环C用吡啶(23, 24) 或噻吩(25) 替换, S1P1的活性略有降低, 但对S1P3的活性显著减弱, 所以提高了选择性。呋喃化合物26的S1P1活性显著降低(20倍)。②苯环C经卤素取代, 27~29对S1P1保持活性, 与无取代的22相近, 对S1P3的活性有所降低。③小烷基取代的30~32对S1P1也有较好的活性, 甲基取代的30对S1P3的活性很弱。④用氮杂环丁羧酸替换上述的β-胺基丙酸, 化合物3334的活性强度和选择性都显著提高(Pan SF, Gray NS, Gao WQ, et al. 化合物34的活性和选择性最强, 进而评价了体内的药代动力学性质, 对大鼠和猴的药代主要参数列于表 4
进而Lewis大鼠灌胃化合物34, 评价体内的药效/药代(PD/PK) 的关联特征, 给药1 mg·kg-1后在不同的时间点测定循环血中的淋巴细胞计数, 图 3是各个时间点34的浓度和淋巴细胞计数的相对值(%)。图中两条曲线显示, 灌胃后血药浓度迅速升高, 同时淋巴计数下降。Tmax = 8 h, 淋巴细胞降低88%, 到48 h, 外周淋巴细胞回复正常, 药物从血液中完全消除, 提示化合物34在体内的持续时间比芬戈莫德短。遂将34确定为候选化合物, 定名为辛波莫德(siponimod), 经临床前和临床研究, 表明辛波莫德可有效地延缓继发性多发性硬化病的残疾进展, 于2019年经FDA批准上市。
