本研究建立他克莫司在特发性膜性肾病(IMN)患者中的群体药代动力学(PPK)模型，并定量考察他克莫司药代动力学的影响因素。收集96名IMN患者的610个常规检测的他克莫司谷浓度数据，采用非线性混合效应模型(NONMEM)考察CYP3A5基因型、年龄、性别、体重、肝肾功能、合用药物等对他克莫司药动学参数的影响，并建立他克莫司群体药动学模型。应用拟合优度图(GOT)、自举法(Bootstrap)和预测值校准的直观预测检验(pc-VPC)对构建的模型进行评价。采用一房室模型描述他克莫司体内变化过程，CYP3A5*1/*3型和*1/*1型表观清除率分别是*3/*3型的1.57倍和1.86倍，合用五酯胶囊患者他克莫司清除率是未合用的73.6%，合用金水宝胶囊患者是未合用的1.2倍。模型评价显示构建的模型稳定，结果可靠。本文临床试验经滨州医学院附属医院伦理委员会批准并在滨州医学院附属医院进行。建立的群体药动学模型较好地描述他克莫司在中国IMN患者体内的药动学特征，为他克莫司的个体化治疗提供依据。

The goal of this work was to establish a population pharmacokinetics (PPK) model of tacrolimus in idiopathic membranous nephropathy (IMN) patients and to identify potential covariates that influence pharmacokinetic of tacrolimus. A total of 610 data points on the blood concentration of tacrolimus were collected from 96 IMN patients in routine clinical settings. Nonlinear mixed-effect modeling (NONMEM) was used to investigate the effects of CYP3A5 genotype, age, gender, weight, laboratory tests and co-therapy medications on the pharmacokinetic of tacrolimus. The PPK model was evaluated by the goodness-of-fit (GOT), bootstrap and prediction corrected visual predictive check (pc-VPC). The pharmacokinetic of tacrolimus was described by a one-compartment model. The apparent clearance (CL/F) of CYP3A5*1/*3 and *1/*1 were 1.57 and 1.86 times of that of *3/*3, respectively. The CL/F of tacrolimus was 73.6% in patients undergoing co-therapy with Wuzhi capsules, and 1.2 times than that of the patients undergoing co-therapy with Jinshuibao capsules. The evaluation of the model shows that the model is stable and has satisfactory predictive performance. The clinical trial was approved by the Society of Ethics and conducted in Binzhou Medical University Hospital. The established PPK model can describe the pharmacokinetic characteristics of tacrolimus in Chinese patients with IMN, and can facilitate individualized therapy with tacrolimus.

特发性膜性肾病(idiopathic membranous nephropathy, IMN)是肾小球基底膜增厚的一种疾病, 是成人原发性肾病综合征(nephrotic syndrome, NS)的主要病因之一, 约占我国成人NS的20%~30%^[1]。IMN其病因尚未阐明, 起病隐匿, 预后差异较大, 尽管有自愈倾向, 但国外的数据仍显示约34%~62%的患者可能进展为肾衰竭^[2]。免疫抑制剂的合理使用是延缓IMN患者疾病进展的重要措施。

他克莫司(tacrolimus)为常用钙调磷酸酶抑制剂(calcineurin inhibitors, CNIs), 通过抑制T淋巴细胞的活化增殖而发挥免疫抑制效应, 推荐用于IMN初始治疗或对糖皮质激素联合烷化剂方案抵抗的IMN患者^[3], 其免疫抑制活性是环孢素A (cyclosporin A, CsA)的10~100倍, 治疗效果好, 不良反应低。他克莫司具有口服生物利用率低、血浆蛋白结合率高^[4]、广泛分布于红细胞内、主要经肝脏代谢、药动学个体差异大、治疗窗窄等特点, 需要进行治疗药物监测, 以保证疗效, 减少不良反应的发生^[1]。

群体药动学(population pharmacokinetics, PPK)可以利用患者临床常规监测的数据, 综合考虑患者的生理、病理、遗传、合并用药等因素, 建立群体药动学模型, 为患者临床合理用药提供理论依据。既往他克莫司群体药动学研究主要集中于器官移植领域^[5-10], NS患者^{[11, 12]}、狼疮性肾炎患者^[13]和慢性肾小球肾炎患者^[14]方面也有报道; 但未见针对IMN患者开展的他克莫司群体药动学研究。而IMN患者的血栓和栓塞等并发症的发生率明显高于其他病理类型的NS患者^[1]。因此有必要对IMN患者进行他克莫司的群体药动学研究。

本研究旨在收集IMN患者的他克莫司常规血药浓度监测数据, 建立IMN患者的他克莫司群体药动学模型, 定量考察影响他克莫司的影响因素, 为该类患者他克莫司个体化给药奠定基础。

材料与方法

患者和样本收集 回顾性分析2014年12月至2019年1月滨州医学院附属医院肾内科确诊IMN并口服他克莫司胶囊(杭州中美华东制药)的患者资料。纳入标准: ①病理诊断(光镜+免疫组化)为IMN^[15]; ②血肌酐 < 132 μmol·L^-1; ③诊断明确后采用他克莫司治疗方案, 且治疗≥7天。

受试者如符合以下任一标准, 将从本研究中排除: ①他克莫司应用前1个月内接受过细胞色素P450 3A4/5酶(cytochrome P450 3A4/5酶, CYP3A4/5酶)激动剂或抑制剂(如大环内酯类药物、利福平、地尔硫䓬和抗真菌药物等)、免疫抑制剂或细胞毒类药物治疗; ②合并严重并发症, 如肾衰竭、重症感染及艾滋病病毒、乙型肝炎病毒、丙型肝炎病毒阳性等; ③育龄女性妊娠试验阳性或拒绝避孕; ④对他克莫司过敏; ⑤无症状、非肾病范围的蛋白尿。本研究方案经滨州医学院附属医院伦理委员会批准, 所有患者均告知研究目的。

他克莫司早、晚餐前1 h口服, 间隔12 h, 初始剂量均为每日0.05 mg·kg^-1, 然后依据全血谷浓度的目标浓度5~10 ng·mL^-1, 调整用药剂量。

另收集患者的人口统计学和实验室检查资料, 包括性别(SEX)、年龄(AGE)、体重(WT)、白细胞(WBC)、红细胞(RBC)、血红蛋白(Hb)、血小板(PLT)、天门冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、白蛋白(ALB)、尿素氮(BUN)、血肌酐(SCR)、尿酸(UA)、CYP3A5*3基因多态性和合并用药。

血药浓度测定方法 应用酶联免疫放大法(enzyme multiplied immunoassay technique, EMIT)测定他克莫司的全血谷浓度; 测定仪器为Viva-E自动生化分析仪(Siemens Healthcare Diagnostics Inc., Germany); 测定试剂为EMIT®2000检测试剂(Siemens Healthcare Diagnostics Inc., Germany)。方法的定量范围为2~30 ng·mL^-1, 日间精密度和日内精密度为小于15%。患者在连续服用稳定剂量的他克莫司一周达到稳态后, 于晨起服药前采取静脉全血2 mL进行测定。

基因型检测 采用荧光原位杂交技术测定CYP3A5*3的基因多态性。乙二胺四乙酸(ethylene diamine tetraacetic acid, EDTA)抗凝管采集患者空腹外周静脉血2 mL, 根据TL998A荧光检测仪(西安天隆科技有限公司)及试剂盒(北京华夏时代基因科技发展有限公司)要求, 通过个体化药学服务平台(V1.0.3)进行单核苷酸基因多态性的分型。基因型包括CYP3A5*3/*3、CYP3A5*1/*3 和CYP3A5*1/*1。

群体药动学分析

数据分析软件与算法群体药动学分析采用非线性混合效应模型软件NONMEM^® (Version 7.4.1, Icon Inc, PA, USA)和PsN (Perl-speaks-NONMEM, Version 4.7.0; Uppsala University, Sweden)。绘图和可视化评价采用R软件(Version 3.4.1, https://www.r-project.org)。建模过程中采用个体间变异和残差变异有交互作用的一阶条件估算法(first order conditional estimation with η-ε interaction, FOCE-I)。

基础模型选择一级吸收和消除的一室模型(ADVAN1-TRANS2)作为基础模型, 估算表观清除率(apparent clearance, CL/F)和表观分布容积(apparent volume of distribution, V/F)。

个体间变异模型采用指数模型(式1)。

P_i为第i名受试者的药动学参数, P为该参数的群体典型值, η_i是呈正态分布、均数为0、方差为ω²的个体间变异。

残差模型分别尝试加和型(式2)、比例型(式3)及加和比例的混合模型(式4), 取拟合优度最佳者。

Y为实测值, F为模型预测值, EPS (1)和EPS (2)分别为加和型和比例型的残差变异, 分别服从均数为0、方差分别为σ₁²和σ₂²的正态分布。残差变异指无法解释的变异, 包括取样、分析方法和模型等造成的变异。

协变量筛选筛选的变量包括SEX、AGE、WT、WBC、RBC、Hb、PLT、AST、ALT、ALB、BUN、SCR、UA、CYP3A5*3基因分型, 合并用药率≥10%的药物。由于本研究仅收集了常规监测的全血谷浓度数据, 无法准确估算V/F, 故仅对CL/F的协变量进行考察。

本研究采用了逐步法筛选协变量, 包括前向纳入和逆向剔除两个过程。对可能有意义的连续型协变量分别用线性(式5)、指数(式6)或幂函数(式7)模型逐一加入各因素。分类型协变量采用指数模型(式8, 以性别为例)筛选。

式中P为药动学参数, P_pop为药动学参数的群体典型值, Covariate为拟筛选的协变量, Cov_pop为拟筛选协变量的群体典型值。

NONMEM法计算时采用扩展最小二乘法, 使目标函数值(objective function value, OFV)最小。OFV值近似服从χ²分布, 可通过假设检验确定某一协变量对药动学参数是否存在显著影响。若模型中加入某一协变量后, OFV值下降大于3.84 (χ², df = 1, P < 0.05), 则将该变量纳入模型。逐一考察所有的协变量, 直至无法纳入新的协变量。再用向后剔除法逐一考察各固定效应因素。若OFV上升大于10.83 (χ², df=1, P < 0.001), 则视该因素有显著意义, 模型中予以保留。排除无显著意义的固定效应参数后, 得到最终模型。此外, 参数估算值须符合生理学意义。

模型评价采用拟合优度图(goodness-of-fit, GOF)、自举法(Bootstrap)和预测值校准的直观预测检验(prediction-corrected visual predictive check, pc-VPC)进行模型评价。GOF包括实测值-群体预测值(DV-PRED)、实测值-个体预测值(DV-IPRED)、条件权重残差-群体预测值(CWRES-PRED)和条件权重残差-时间(CWRES-TIME)的散点图。Bootstrap是一种数据重采样技术, 不仅可考察模型的稳定性, 还可估算各参数值2.5%~97.5%的可信区间。本研究采用1 000次Bootstrap进行模型评价。pc-VPC适用于存在剂量调整的预测检验, 观测值落在模拟数据的95%置信区间内, 即表明模型预测性能良好。

模拟根据协变量筛选结果, 设置不同的典型患者, 剂量方案包括0.5、1.0和2.0 mg (q12h)。根据最终模型参数进行蒙特卡洛模拟, 考察不同典型患者在不同剂量方案下的稳态谷浓度水平。模拟次数为1 000次, 目标稳态谷浓度水平为5~10 mg·L^-1。

1 患者纳入及基因型分布

本研究纳入96名患者及其610个他克莫司谷浓度进行群体药动学分析。低于定量下限的观测值有3个, 建模时未予纳入。他克莫司口服日剂量为1.0~4.5 mg, 谷浓度为(7.14 ± 3.38) ng·mL^-1。人口统计学资料及实验室检查资料见表 1。合并用药中大于10%包括维D钙咀嚼片(vitamin D calcium chewable tablets)、五酯胶囊(Wuzhi capsule, Wuzhi)、金水宝胶囊(Jinshuibao capsule, JSB)、骨化三醇胶丸(calcitriol capsule)、黄葵胶囊(ambrette capsule)、百令胶囊(bailing capsule)、醋酸泼尼松(prednisone acetate)、双嘧达莫(dipyridamole capsule)、缬沙坦(valsartan)、阿托伐他丁(atorvastatin)、依那普利(enalapril maleate) 11种。

CYP3A5*3基因分型, 野生基因为A, 突变基因为G。基因型分别为GG、AG、AA, 其中突变型纯合子GG (CYP3A5*3/*3) 46例, 突变型杂合子GA (CYP3A5*1/*3) 40例, 野生型纯合子AA (CYP3A5*1/*1) 10例, 比例分别47.92%、41.67%、10.42%, 其中G、A等位基因频率为68.75%、31.25%。CYP3A5*3基因分型SNP均符合Hargy Weinberg平衡(P > 0.05), 与文献^[11]报道相似。

2 群体药动学模型

2.1 基础模型

采用一级吸收和消除的一房室模型可描述他克莫司在IMN患者中的药动学特征。由于建模数据为谷浓度, 无药物的吸收相信息, 因此本研究采用既往发表中国肾病综合征患者他克莫司群体药动学的文献报道的吸收速率常数(K_a) 4.5 h^-1^{[11, 16, 17]}。另经比较分析K_a的取值对CL/F、V/F等重要参数的估计无显著影响。

在CL/F、V/F都加入个体间变异, 模型估算失败; 只在CL/F加入个体间变异时, 模型收敛成功。因此只对CL/F进行个体间变异的估算。选择残差变异模型时, 混合型模型较其他模型的OFV值有显著下降, 因此残差变异选择混合型模型。基础模型的GOF见图 1。

2.2 协变量筛选和最终模型

将11个合并用药和14个人口学及生化指标作为待考察的协变量, 考察其对他克莫司清除率的影响。协变量筛选前考察了协变量间的相关性, 若相关系数 > 0.5即视为存在相关性, 协变量考察时仅选择其中一个纳入分析。协变量相关性图显示(图 2), ALT与AST、RBC与Hb存在相关性, 建模时ALT、Hb作为协变量进行考察。

向前纳入筛选协变量时, CYP3A5*3基因型、Hb、BUN、SCR、合用金水宝胶囊、五酯胶囊、醋酸泼尼松和双嘧达莫对清除率有显著影响。逆向剔除后, 最终保留的协变量为CYP3A5*3基因型、合用金水宝胶囊、五酯胶囊。

最终模型的CL/F和V/F的群体典型值分别为9.7 L·h^-1和193 L, CL/F的公式(式9)如下:

式中合用金水宝胶囊时, 金水宝胶囊为1, 反之为0;合用五酯胶囊时, 五酯胶囊为1, 反之为0;基因型为CYP3A5 GG时, CYP3A5 GA和CYP3A5 AA为0;基因型为CYP3A5 GA时, CYP3A5 GA为1, CYP3A5 AA为0;基因型为CYP3A5 AA时, CYP3A5 GA为0, CYP3A5 AA为1。最终模型参数的群体典型值及变异估计值见表 2。

2.3 模型评价

基础模型和最终模型的GOF (图 1)。与基础模型相比, 最终模型的GOF有所改善, DV-PRED和DV-IPRED的散点更加均匀的分布在参考线y = x的两侧, 且CWRES随TIME的变化趋势明显降低。

Bootstrap得到的参数值与最终模型参数值偏差均小于7%, 成功率为88.1%, 表明模型稳定可靠。pc-VPC图表明, 除部分观测数据的95百分位数与模拟数据的分布存在差异外, 整体上观测数据与模拟数据具有相似的分布特征, 表明模型能较好地描述原始数据特征(图 3)。

2.4 模拟

最终模型纳入合用金水宝、合用五酯胶囊和CYP3A5基因型共3个协变量, 故设置具有不同协变量、服用不同剂量他克莫司的典型患者共36名。模拟结果如图 4所示。

比较他克莫司谷浓度达标率结果显示:单用他克莫司时, CYP3A5*3/*3基因型患者, 1 mg (q12 h)给药方案谷浓度达标率更高, 但也仅为52.1%;CYP3A5*1/*3和*1/*1基因型患者2 mg (q12 h)给药方案达标率更高(分别为48.4%和41.3%)。

他克莫司合用金水宝胶囊时, CYP3A5*3/*3基因型患者, 两种给药方案谷浓度达标率相近, 分别为43.3%和40.7%; CYP3A5 *1/*3基因型患者2 mg q12h的给药方案达标率为1 mg (q12 h)给药方案的4倍, 为40.4%;而CYP3A5*1/*1基因型的患者两种给药方案谷浓度达标率均未达30%, 这与金水宝加快他克莫司的清除率相符。

他克莫司合用五酯胶囊时, CYP3A5*3/*3和*1/*3基因型患者, 1 mg (q12 h)给药方案谷浓度达标率较高且相近, 约为45%;CYP3A5 *1/*1基因型患者2 mg (q12 h)给药方案达标率为1 mg (q12 h)给药方案的1.4倍, 为47.3%。

他克莫司、五酯胶囊和金水宝胶囊三者合用时, 他克莫司两种给药方案的患者的谷浓度达标率与单用他克莫司时相似。

近几年, 他克莫司广泛用于IMN的治疗, 其疗效和地位逐步得到认可。本研究首次考察了在IMN患者中他克莫司的群体药动学特征, 为该药的个体化治疗提供理论依据。

本研究显示他克莫司在IMN患者中的CYP3A5*1/*1携带者的CL/F典型值为CYP3A5*3/*3携带者的1.86倍, 即18.0 L·h^-1, 显著高于NS患者的13.4 L·h^-1^[11]和成人慢性肾小球肾炎患者的13.8 L·h^-1^[14], 低于国内外成人肾移植患者术后平均CL/F典型值21.7~32.4 L·h^-1^[18-20], 与国内外报道的肝移植患者的范围19.8~20.9 L·h^-1相似^{[5, 6]}。表明不同的疾病状态他克莫司的CL/F典型值不同^[21]。

研究证实CYP3A5的基因多态性能够影响他克莫司的代谢, CYP3A4对他克莫司血药浓度的影响尚无明确结论^{[22, 23]}。IMN患者中的CYP3A5*1/*1和*3/*1清除率为*3/*3的1.86倍和1.57倍, 携带*1患者需要更高的他克莫司口服剂量。既往在肺移植、重症肌无力患者中的研究也表明:携带*1等位基因可使CL/F分别增加30%和85%^{[24, 25]}。本研究结果也显示:单用他克莫司时, 要达到50%的目标谷浓度, CYP3A5 *3/*3患者为1 mg给药方案, 而携带*1等位基因的患者需要更高的剂量, 至少需2 mg。

此外, 他克莫司合并用糖皮质激素、钙通道阻滞剂、抗菌药物、中成药五酯胶囊等对他克莫司药动学的影响已有大量报道^[8-12]。

本研究显示, 合用五酯胶囊的患者的他克莫司清除率是未合用的73.6%, 与Lu等^[11]报道80.7%相近。五酯胶囊是中草药五味子中提取的活性成分混合物, 可以改善肝细胞代谢, 临床主要用于各种类型肝损伤。其升高他克莫司血药浓度可能的机制与抑制体内CYP3A4/5和药物转运体P糖蛋白^[26-28]有关。合用五酯胶囊时需要适当降低他克莫司剂量。

随着IMN发病机制、自然病程和治疗药物研究的深入, 保护肾脏功能作为IMN主要治疗目的之一, 补肺益肾剂百令胶囊和金水宝胶囊在IMN的临床治疗中得到应用^[29-32]。《中国药典》 (2015年版)中百令胶囊和金水宝胶囊“成分”项下分别为“发酵冬虫夏草菌粉”和“发酵虫草菌粉” ^[33]。本研究发现两者对他克莫司清除率影响的不同, 其中百令胶囊未见影响, 与Chen等^[8]报道一致。而合用金水宝胶囊的患者他克莫司清除率提高1.2倍, 由模拟结果可知, CYP3A5*1/*1基因型患者他克莫司2 g (q12 h)合用金水宝胶囊给药方案比单用同剂量他克莫司的谷浓度降低(经计算达标率降低了31%), 其可能的机制有待探讨。

本研究采用的模型评价方法包括pc-VPC, 传统的VPC的诊断意义可能被剂量和有较大影响的协变量干扰, 特别是对于剂量调整后的数据, 而pc-VPC可以将模拟纳入的因变量进行归一化, 消除了自变量的变异性^[34]。本研究中, 大多数患者均有两个剂量水平的观测值, 采用pc-VPC能减少剂量调整对预测性能的干扰。由于本研究为单中心的回顾性研究, 纳入病例数有限, 所建立的模型外推性有待进一步考察。

综上所述, 本研究对中国IMN患者他克莫司群体药动学特征进行了考察, 首次发现了金水宝胶囊对他克莫司药动学参数的影响。此外, 患者CYP3A5*3基因型、合并用五酯胶囊也是影响他克莫司在中国IMN患者中药动学参数的重要因素, 可为该药的个体化给药方案制定提供依据。
