Idiopathic pulmonary fibrosis (IPF) is usually accompanied with inflammatory response, especially the macrophages. The co-culture model of macrophages and fibroblast, and IPF mice model induced by bleomycin were used here to explore the role of macrophages and interleukin-6 (IL-6) in IPF. All animals welfare and experiments were performed following the regulations of the Animal Ethics Committee of Tianjin University of Traditional Chinese Medicine. The results showed that the content of IL-6 in IPF mice induced by bleomycin was significantly increased, and there was a large amount of inflammatory cell infiltration in the lungs. The results of wound-healing and immunofluorescence showed that alternative activated (M2) macrophages could induce the migration and activation of fibroblasts at 36 h, and the expression of IL-6 was increased in the co-culture system. The results of wound-healing and sirius red assay proved that IL-6 could induce the migration and activation of fibroblast. The results showed that M2 macrophages induced fibroblasts to secrete IL-6, thereby inducing the activation and migration of fibroblast, which affect the development of IPF.

特发性肺纤维化(idiopathic pulmonary fibrosis,IPF)通常伴有炎症反应,其中巨噬细胞与其密切相关,通过巨噬细胞与肺成纤维细胞共培养模型和博来霉素诱导的IPF小鼠模型,本文探究了巨噬细胞和白细胞介素-6(interleukin 6,IL-6)与IPF的联系。动物福利和实验过程均遵循天津中医药大学动物伦理委员会的规定。结果表明,博来霉素(bleomycin,BLM)诱导的IPF小鼠体内IL-6含量明显升高,且肺部有大量的炎性细胞浸润。划痕及免疫荧光实验结果表明,共培养36 h替代活化型(alternatively activated,M2)巨噬细胞可诱导成纤维细胞迁移及活化,且在共培养体系中IL-6的表达随之增多。划痕及天狼星红实验结果证明IL-6可促进成纤维细胞迁移和活化。研究结果证明,M2型巨噬细胞通过促进成纤维细胞分泌IL-6,从而诱导成纤维细胞活化及迁移,影响IPF发展。

特发性肺纤维化(idiopathic pulmonary fibrosis,IPF)通常伴有炎症反应,其中巨噬细胞与其密切相关,通过巨噬细胞与肺成纤维细胞共培养模型和博来霉素诱导的IPF小鼠模型,本文探究了巨噬细胞和白细胞介素-6(interleukin 6,IL-6)与IPF的联系。动物福利和实验过程均遵循天津中医药大学动物伦理委员会的规定。结果表明,博来霉素(bleomycin,BLM)诱导的IPF小鼠体内IL-6含量明显升高,且肺部有大量的炎性细胞浸润。划痕及免疫荧光实验结果表明,共培养36 h替代活化型(alternatively activated,M2)巨噬细胞可诱导成纤维细胞迁移及活化,且在共培养体系中IL-6的表达随之增多。划痕及天狼星红实验结果证明IL-6可促进成纤维细胞迁移和活化。研究结果证明,M2型巨噬细胞通过促进成纤维细胞分泌IL-6,从而诱导成纤维细胞活化及迁移,影响IPF发展。

Idiopathic pulmonary fibrosis (IPF) is usually accompanied with inflammatory response, especially the macrophages. The co-culture model of macrophages and fibroblast, and IPF mice model induced by bleomycin were used here to explore the role of macrophages and interleukin-6 (IL-6) in IPF. All animals welfare and experiments were performed following the regulations of the Animal Ethics Committee of Tianjin University of Traditional Chinese Medicine. The results showed that the content of IL-6 in IPF mice induced by bleomycin was significantly increased, and there was a large amount of inflammatory cell infiltration in the lungs. The results of wound-healing and immunofluorescence showed that alternative activated (M2) macrophages could induce the migration and activation of fibroblasts at 36 h, and the expression of IL-6 was increased in the co-culture system. The results of wound-healing and sirius red assay proved that IL-6 could induce the migration and activation of fibroblast. The results showed that M2 macrophages induced fibroblasts to secrete IL-6, thereby inducing the activation and migration of fibroblast, which affect the development of IPF.

特发性肺纤维化(idiopathic pulmonary fibrosis, IPF)是一种慢性致死性疾病, 是最常见的特发性间质性肺炎, 其发病机制不明, 预后性差, 发病率随年龄增长而明显增加。其病理改变为:肺泡间隔增厚, 成纤维细胞异常活化, 细胞外基质过度沉积, 形成瘢痕, 最终导致气体排出功能受损, 肺功能丧失, 呼吸衰竭死亡[1, 2]。目前, 美国食品药品监督管理局批准用于治疗IPF的药物为吡非尼酮和尼达尼布, 但这两种药物的治疗成本较高, 且无法完全逆转病情[3, 4]
在早期伤口愈合阶段, 经典活化型(classically activated, M1)巨噬细胞等炎性细胞浸润, 并通过分泌肿瘤坏死因子、白细胞介素-6 (interleukin 6, IL-6)等多种因子在伤口愈合中发挥关键作用。在炎症终止的损伤修复阶段, 替代活化型(alternatively activated macrophages, M2)巨噬细胞调节炎症反应, 并分泌转化生长因子-β1 (transforming growth factor beta1, TGF-β1)和血小板衍生生长因子等, 刺激成纤维细胞增殖, 造成胞外基质重塑, 胶原蛋白过度沉积, 进而逐渐发展为不可逆的病理过程[5-7]。M1型巨噬细胞是损伤早期伤口愈合的关键[8], 而增加的M2型巨噬细胞会通过形成纤维微环境导致肺纤维化的发展, 故认为M2型是巨噬细胞的促纤维化表型, 抑制M2型巨噬细胞活化有望成为治疗IPF的理想靶点。
使用单一细胞模型只能检测有限的病理指标, 且其药效与体内结果差异较大, 因此两种或多种细胞类型的细胞共培养系统以能更精确地模拟病灶的复杂性以及多种细胞间的相互作用而被广泛应用。相关IPF的研究发现, 炎症表型巨噬细胞与成纤维细胞构建的共培养模型可评价药物对促炎因子表达的抑制作用[9]。针对M2型巨噬细胞在IPF发展中的作用, 本实验以巨噬细胞表型为切入点, 构建了成纤维细胞和巨噬细胞的共培养模型, 用于抗IPF的药物筛选及机制研究, 为通过调节巨噬细胞极化以改善病情的研究提供了实验依据。
NIH-3T3小鼠胚胎成纤维细胞株及RAW264.7小鼠单核巨噬细胞株(ATCC公司代理商)以含10%胎牛血清的DMEM完全培养基进行细胞培养。
脂多糖(lipopolysaccharide, LPS) (Sigma公司); 白细胞介素4 (interleukin 4, IL-4) (Peprotech公司); TGF-β1 (R & D system公司); DMEM培养基、胎牛血清、1%青/链霉素(Gibco公司); 小鼠IL-6酶联免疫吸附测定法(enzyme linked immunosorbent assay, ELISA)检测试剂盒(上海茁彩生物有限公司); 一氧化氮合成酶检测试剂盒(碧云天公司); CD206抗体和Alexa Fluor 647羊抗兔二抗(Abcam公司); 博来霉素(bleomycin, BLM, 大连美仑生物技术有限公司)。多功能酶标仪(Tecan公司); Operetta高内涵筛选系统及Micro-CT QuantumFX uCT software (PerkinElmer公司); 倒置显微镜(日本Nikon公司)。
将RAW264.7细胞以1×104个/孔接种于96孔板中, 贴壁后加入LPS (100 ng·mL-1), 对照组加入细胞培养基, 作用12 h后按照试剂盒说明书检测iNOS活性。
NIH-3T3细胞用0.25%胰酶消化后, 以细胞数7×104个/孔接种于24孔板中。RAW264.7细胞用细胞刮处理后, 以细胞数5×104个/孔接种于嵌插小室中。待细胞贴壁后, RAW264.7细胞通过LPS (100 ng·mL-1)作用12 h诱导为M1型巨噬细胞, 通过IL-4 (20 ng·mL-1)作用48 h诱导为M2型巨噬细胞, 诱导成功后, 将巨噬细胞与NIH-3T3进行共培养。
在24孔板底画3条直线, 将细胞以适宜细胞数接种于孔板中, 待细胞贴壁后, 用1 mL枪头划一条垂直于3条直线的划痕, 用磷酸缓冲盐溶液(phosphate buffer saline, PBS)清洗3次后, 分别在0、24、48 h通过倒置显微镜进行拍照, 记录细胞迁移情况。
不同表型巨噬细胞与成纤维细胞共培养实验结束后, 弃去上清, 用4%多聚甲醛室温固定15 min, 用0.1% Triton X-100打孔10 min, PBS洗3次后, 用1%牛血清白蛋白室温封闭1 h, 随后加入用封闭液稀释100倍的一抗, 4 ℃孵育过夜。PBS洗3次后, 加入Alexa Fluor 647羊抗兔二抗及Hoechst 33342室温孵育30 min, PBS清洗3次, 每孔留取适量PBS并通过Operetta高内涵筛选系统进行拍照, 通过image J软件对荧光进行量化分析。
将NIH-3T3细胞(每孔5 000个细胞, 100 μL)接种到96孔板中, 24 h后更换为无血清培养基, 再培养24 h。随后于37 ℃, 应用TGF-β1 (5 ng·mL-1)、IL-6 (10 ng·mL-1)和TGF-β1 (5 ng·mL-1)+IL-6 (10 ng·mL-1)分别处理48 h。弃去孔中培养基, 并用PBS (200 μL)洗涤细胞3次, 应用-20 ℃冰甲醇固定过夜后, PBS (200 μL)洗涤3次, 每孔加入天狼星红试剂(100 μL), 室温避光放置4 h, 弃去游离的天狼星红, 每孔用0.01%乙酸(200 μL)洗涤3次后风干, 加入0.1 mol·L-1 NaOH (200 μL), 振荡1 h后应用多功能酶标仪在540 nm处进行吸光度的测定。
SPF级健康雄性C57BL/6小鼠由斯贝福(北京)生物技术有限公司提供, 许可证号SYXK (京) 2019-0030, 体重20~25 g, 饲养于天津国际生物医药联合研究院, 温度19~25 ℃, 相对湿度40%~60%, 每笼5只, 定时给饲料, 自由饮水。适应性饲养1周后, 随机分为正常组(control)和模型组(model) (n = 6)。小鼠按10 g体重0.1 mL的剂量腹腔注射三溴乙醇, 平躺固定于手术架上, 颈部用75%酒精消毒, 沿颈部正中线纵向剪开皮肤约0.8 cm, 用眼科镊子小心分离皮肤及皮下组织, 分离出气管, 用无菌留置针插入气管大约2 cm左右, 缓慢注入BLM (1.5 mg·kg-1) 50 μL后, 退出注射器, 消毒后缝合伤口, 小心将小鼠放于暖床上, 密切观察, 待苏醒后放回笼内。在7~10天后对滴注了BLM的各组小鼠通过微计算机断层扫描(micro computed tomography, micro-CT)采集图像, 像场宽度最大为68 mm, 体素(volume pixel)为35 mm×35 mm×35 mm, 根据肺部阴影面积大小将肺纤维化分为0~5级:肺部正常为0级, 肺部阴影面积占25%为3级, 纤维化评分等级大于3的小鼠为造模成功。动物福利和实验过程均遵循天津中医药大学动物伦理委员会的规定。
将细胞上清及小鼠血液进行3 000 r·min-1, 10 min离心, 留取细胞上清及小鼠血清, 按照ELISA试剂盒说明书检测IL-6的含量。
采用Graphpad Prism 5.0对数据进行统计分析, 数值以 $\bar x \pm s$表示, 两组比较采用t检验, 多组比较采用单因素方差分析(one-way ANOVA)对实验数据进行统计, P < 0.05为差异有统计学意义。
由micro-CT图像可知, BLM诱导的模型组小鼠在造模后第21天肺部有明显阴影, 即出现纤维化(图 1A)。苏木精-伊红(hematoxylin-eosin, H & E)染色同样表明肺部发生实变, 且病变区域有明显的炎性细胞浸润(白色矩形), 如巨噬细胞等; Masson染色可知模型组小鼠肺部有明显的胶原沉积(黑色矩形) (图 1B)。通过ELISA方法检测小鼠血清中IL-6含量可知, 与正常对照组(6.27 ± 0.54) pg·mL-1相比, 模型组小鼠体内IL-6含量显著升高(8.87 ± 0.82) pg·mL-1 (P < 0.001) (图 1C), 因此认为IL-6与肺纤维化的发生密切相关。
通过将100 ng·mL-1 LPS作用于RAW264.7细胞12 h, iNOS含量明显升高(图 2A), RAW264.7细胞转化为M1型巨噬细胞; 将IL-4 (20 ng·mL-1)作用于巨噬细胞48 h后, 通过免疫荧光检测CD206蛋白的表达可知, 诱导后巨噬细胞中CD206表达上调(图 2BC), 表明RAW264.7细胞转化为M2型巨噬细胞。将巨噬细胞与成纤维细胞NIH-3T3进行共培养的共培养模型划痕实验结果表明(图 2D), 与对照组及单独的RAW264.7细胞相比, 巨噬细胞与成纤维细胞共同作用12 h后, M1型和M2型巨噬细胞均可促进NIH-3T3细胞迁移, 迁移率分别为(22.69 ± 1.81)% (P < 0.05)和(40.22 ± 2.14)% (P < 0.001), 与M1型巨噬细胞相比, M2型细胞的促进成纤维细胞迁移作用更为显著(图 2EF)。当细胞共培养36 h后, RAW264.7、M1及M2型巨噬细胞均可促进成纤维细胞迁移, 迁移率明显大于85% (图 2EG)。
α-平滑肌肌动蛋白(alpha-smooth muscle actin, α-SMA)是成纤维细胞活化的标志蛋白, 经免疫荧光染色呈黄色荧光(图 3A)。与正常对照组相比, RAW264.7及M1型巨噬细胞与NIH-3T3细胞相互作用后, α-SMA的表达量上调约6倍, IL-4诱导的M2型巨噬细胞共培养的模型中, α-SMA的表达上调约12倍, 表明M2型巨噬细胞可诱导成纤维细胞活化(图 3AB)。
为检测M2型巨噬细胞诱导肺成纤维细胞活化及迁移的相关机制, 进一步通过ELISA试剂盒检测了共培养体系中分泌型IL-6的表达量。结果表明, RAW264.7及M1型巨噬细胞分别使体系中IL-6含量升高至(1.9 ± 0.08) pg·mL-1 (P < 0.001)和(2.27 ± 0.18) pg·mL-1 (P < 0.001); IL-4诱导的M2型巨噬细胞使共培养体系中IL-6的含量升高3~4倍(3.26 ± 0.71) pg·mL-1 (P < 0.001) (图 3C)。因此推断, M2型的巨噬细胞虽然自身不表达IL-6, 但可明显促进肺成纤维细胞分泌IL-6。
划痕实验结果表明, 作用24 h后, IL-6及TGF-β1无明显促进成纤维细胞迁移的作用, 而TGF-β1+IL-6组成纤维细胞细胞可发生明显的迁移, 其迁移率为(67.59 ± 5.13)% (P < 0.001) (图 4A~C); 作用48 h后, 与空白对照组相比, TGF-β1、IL-6及TGF-β1+IL-6组成纤维细胞细胞均发生不同程度的迁移, 其中TGF-β1+IL-6组细胞迁移率接近100%。当成纤维细胞活化后, 细胞外基质中的胶原含量增多, 本实验通过天狼星红法检测细胞中的胶原含量, 结果表明, 与空白对照组相比, IL-6仅在一定程度上促使成纤维细胞活化, 效果不及TGF-β1 (图 4D), 但两者可协同作用, 大大缩短成纤维化细胞活化所需时间。
IPF是指病因不明的肺纤维化, 也称为特发性间质性肺炎, 该疾病在肺纤维化中所占比例较大。IPF以一定程度的肺部炎症和组织异常修复为主要特征, 炎症和免疫失调被认为是该疾病发生、发展的主要因素。巨噬细胞和成纤维细胞参与了伤口愈合和纤维化的发展过程[10, 11]。巨噬细胞凭借其功能的显著异质性, 以及调节常驻及募集成纤维细胞活化的作用, 在IPF的靶向治疗中具有重要意义, 成为纤维化研究日益关注的焦点[12]。研究表明, M2型巨噬细胞具有促纤维化表型[13, 14], 而IL-6作为M1型巨噬细胞分泌的一种多效性因子, 在BLM诱导的纤维化模型中发挥促炎和促纤维化的作用。已有研究证明, IL-6基因敲除后可明显抑制BLM诱导的小鼠体内促纤维化因子TGF-β1的上调[15, 16]; 且IPF患者体内IL-6增多, 随病情恶化IL-6的含量呈上升趋势[17]。现已广泛证明巨噬细胞与肺成纤维细胞均能分泌IL-6, 成纤维细胞将IL-6作为自分泌生长因子, 可促进自身的增殖; 而IL-6又可通过诱导IL-4受体的表达, 刺激巨噬细胞活化为M2型, 这种调节会促使纤维化进一步恶化[18]
本研究证明, 与正常组小鼠相比, BLM诱导的小鼠体内IL-6含量明显增加, 并且病变区域有大量的炎性细胞浸润, 表明IL-6及炎性细胞与肺纤维化的发生密切相关。共培养模型检测结果证明巨噬细胞具有促成纤维细胞迁移的作用, 其中M2型巨噬细胞可明显促进成纤维细胞的迁移及活化。在M2型巨噬细胞的共培养体系内, IL-6含量远远高于未受刺激的成纤维细胞本身及M1型共培养体系的分泌。由于IL-6主要由M1型巨噬细胞分泌, 因此推测体系中增加的IL-6是M2型巨噬细胞诱导成纤维细胞分泌的, 这与肺纤维化小鼠模型中IL-6上调的结果一致。划痕及天狼星红实验进一步表明IL-6可促进成纤维细胞迁移及胶原表达。综上, 共培养模型及活体实验结果表明, 在IPF发生时, M2型巨噬细胞可促进成纤维细胞分泌IL-6, 而IL-6又能反作用于成纤维细胞, 诱导其活化和迁移, 促进肺纤维化的发生。
综上所述, 本实验涉及的巨噬细胞和成纤维细胞的共培养体系以一种简单、高效的方法探究了多细胞在肺纤维化中复杂的相互作用。证明了M2型巨噬细胞通过促进成纤维细胞分泌IL-6, 并可诱导成纤维细胞活化为肌成纤维细胞, 为巨噬细胞表型调控作为特发性肺纤维化的靶点治疗提供了实验依据。
