恶性肿瘤目前已成为严重威胁人类身心健康的重大疾病之一。现代临床实践表明，从药用植物和海洋生物等天然产物中提取的活性成分，如萜类、生物碱类、多糖类、挥发油类和多肽类等均能有效抑制肿瘤细胞生长。本文将近几年天然产物抗肿瘤的活性成分进行归纳并阐明其相关的作用机制，以期为抗肿瘤天然药物的进一步开发提供理论依据。

Tumor, especially malignant tumor has become one of the major diseases, a serious threat to the health of people around the world. Modern clinical practice shows that the natural active products extracted from traditional Chinese medicine, marine medicine and other natural drugs, such as terpenes, alkaloids, polysaccharides, volatile oils and peptides, can effectively inhibit the growth of tumor cells. In this paper, the active components of natural antitumor products in recent years were summarized and their related mechanism was elucidated, so as to provide theoretical basis for the further development of natural antitumor drugs.

肿瘤是世界范围内死亡率极高的疾病, 恶性肿瘤(癌症)已成为严重威胁世界各国人民身心健康的重大疾病之一, 且发病例数逐年增加。根据GLOBOCAN2018显示, 死亡病例约956万例, 中国约占30%, 发病率和死亡率均高于全球平均水平^[1]。

天然药物是抗肿瘤药物来源的重要途经之一, 因此天然产物是鉴别和开发新的癌症治疗方案的宝贵资源^{[2, 3]}。我国天然产物资源丰富, 其中具有抗肿瘤活性的天然产物资源更是种类繁多, 我国一大批科研人员对于天然抗瘤药物的开发也在不断的探索中。本文综述了近年来开发最为成功的几大类天然抗肿瘤药物的研究进展。

1 萜类

萜类化合物是指存在于自然界中分子式以异戊二烯为单位倍数的烃类及其含氧衍生物, 经研究发现许多天然萜类化合物具有抗肿瘤活性, 典型代表有柠檬烯、紫苏醇、榄香烯、斑蝥素和紫杉醇等, 详情见表 1。

1.1 单萜类

单萜(monoterpene)是一类分子骨架由2个异戊二烯单位构成, 含10个碳原子的化合物类群, 其含氧衍生物多香气浓郁并具有一定生物活性, 但目前已知的自然界中具有抗癌活性的天然单萜类种类很少。

柠檬烯(limonene)是一种从柑橘类水果中提取的单萜烯, 已被发现在各种癌症中具有化学预防和化疗活性^[4]。对某些化学致癌物如苯并芘等诱发的肿瘤等具有一定的防治作用, 尤其是肿瘤发展的初期^[5], 其作用机制可能是通过抑制异戊烯基转移酶, 从而抑制参与细胞生长相关的小分子G蛋白的异戊二烯化^[6]。斑蝥素的衍生物去甲斑蝥素(norcantharidin, NCTD), 主要对肝癌、食管癌和胃癌等起作用, 其机制主要为通过抑制肿瘤细胞蛋白质的合成和DNA复制, 影响细胞有丝分裂, 促进肿瘤细胞凋亡, 进而提高机体免疫能力^[7]。另有研究发现, NCTD能通过靶向乳腺癌细胞中的miR-873/细胞周期蛋白依赖性激酶(cyclin-dependent kinase 3, cdk3)来调节雌激素受体α (estrogen receptor α, ERα)信号传导, 可为三苯氧胺耐药乳腺癌提供一种替代治疗策略^[8]。芳樟醇(linalool)单萜也能通过诱导细胞周期停滞、线粒体膜电位变化, 抑制磷脂酰肌醇3-激酶/丝氨酸/苏氨酸激酶(PI3K/AKT)信号通路, 对OECM 1人口腔癌细胞进行抑制, 有较好的抗肿瘤作用^[9]。

1.2 倍半萜类

倍半萜(sesquiterpene)是一类分子骨架由3个异戊二烯单位构成, 含15个碳原子的化合物类群, 在自然界中分布较广, 具有抗炎和抗肿瘤等显著生物活性。代表性的化合物种类包含愈创木烷型、桉叶烷型、广藿香烷型、胡萝卜烷型以及Bakkane型, 其作用机制与抑制核因子kappa B (NF-κB)、丝裂原活化蛋白激酶(MAPK)细胞信号和信号转导子与转录激活子(STAT)信号通路等相关因子的活化, 下调肿瘤坏死因子α (TNF-α)、前列腺素E₂ (PGE₂)和白细胞介素等炎性因子的蛋白表达, 减少炎症因子的产生与释放有关^[10]。倍半萜的一个子集具有独特的化学结构, 可以靶向有丝分裂所需的精确蛋白质。由于有丝分裂停止的过程先于细胞死亡的过程, 一些目前被归类为细胞毒性的倍半萜可能也会导致有丝分裂停止。此外, 一些倍半萜在联合使用时能够提高抗癌药物的疗效, 如β-石竹烯可以增加紫杉醇的抗增殖作用及其在几种癌症细胞体系中的细胞内积累^[11]。

榄香烯(elemene)是从姜科植物中提取的一种有效抗癌的倍半萜成分, 其注射液是我国具有自主知识产权的抗肿瘤新药, 对肺癌^[12]、食道癌^[13]、乳腺癌^[14]、胰腺癌^[15]等具有显著疗效, 可通过抑制肿瘤细胞DNA的合成, 诱导肿瘤细胞凋亡发挥作用。

1.3 二萜类

二萜(diterpene)类分子骨架由4个异戊二烯单位构成, 含20个碳原子, 是一类重要的、分布广泛的天然化合物, 生物活性广泛, 如抗肿瘤和抗炎等作用。目前对二萜类化合物的植物化学研究已取得显著成效, 设计并合成了一系列结构修饰衍生物和药物包合物, 是抗肿瘤药物发展的重要来源, 具有良好的治疗效果。

紫杉醇(paclitaxel, PTX)属是新型的植物类抗肿瘤二萜类药物, 属于植物次生代谢产物^[16], 对卵巢癌、乳腺癌和非小细胞肺癌治疗效果较好, 另外可辅助治疗转移性食管癌、肝癌、鼻咽癌和前列腺癌等, 其作用机制与抑制微管蛋白解聚, 促进微管形成, 从而导致肿瘤细胞有丝分裂终止促使细胞凋亡有关^[17]。

1.4 三萜类

三萜(triterpene)类化合物含30个左右碳原子, 基本骨架为多个异戊二烯, 来源广泛, 药用价值高, 在许多植物中含量丰富, 已作为一种重要化学成分运用于药物之中。人参皂苷Rg3、Rh2, 同属于达玛烷型四环三萜类皂苷, 有较强的抗肿瘤活性, 能够从调控细胞周期、影响肿瘤信号传递及主要基因表达、诱导细胞凋亡等方面发挥抗肿瘤作用^[18]。

天然五环三萜(pentacyclic triterpenoids, PTs)具有广泛的生物活性, 抗肿瘤典型的代表包含齐墩果烷型的齐墩果酸、甘草次酸, 乌苏烷型的熊果酸、积雪草酸, 羽扇豆烷型的桦木酸, 木栓烷型的雷公藤红素等, 现代研究发现PTs具有良好的抗肿瘤活性, 且毒副作用较小。在过去的20年里, 报道PTs抗癌作用的出版物数量呈指数级增长, 反映出人们对开发新的三萜类抗肿瘤药物越来越感兴趣。该领域的广泛研究表明, PTs的抗癌作用是由多种机制介导的, 它们参与调节多种癌细胞生存及增殖的分子靶点和信号通路, 最终能够通过诱导细胞凋亡、阻滞细胞周期、抑制细胞增殖等途径发挥抗肿瘤作用^{[19, 20]}。考虑到该类化合物水溶性差、生物利用度低的化学特性及其潜在的抗癌潜力, 目前已合成了一些半合成衍生物, 旨在提高其生物利用度和治疗功效, 为该类化合物的开发提供依据。

2 多糖类

天然多糖(polysaccharide)具有丰富的膳食成分, 在治疗和预防方面具有较好的抗肿瘤作用。与传统的肿瘤治疗方法相比, 天然多糖具有毒性低和调节免疫等特点。近年来的研究表明, 多糖的抗肿瘤作用在一定程度上是由于多糖被消化后, 肠道菌群组成发生了改变。事实上, 在肿瘤发生过程中已经报道了微生物的失调或某些细菌种类的改变, 天然多糖的摄入可以逆转这种情况。多糖具有较强的抗肿瘤活性, 可以保护肠道不渗漏, 平衡机体能量摄入, 调节机体免疫状态, 达到局部和整体抗肿瘤作用^[21]。

多糖属于生物大分子, 来源较广泛。它普遍存在于动植物、微生物和藻类中。从真菌中提取的多糖已被开发用于食用和药用^[22], 如香菇多糖、灵芝多糖、黑木耳多糖、茯苓多糖和黄芪多糖等。多种多糖已被公认为是肿瘤治疗中安全无毒的佐剂^[23], 然而具体作用机制尚不清楚。进一步的研究表明, 天然多糖的糖酵解吸收实际发生在肠道远端微生物群存在的情况下, 摄入多糖可以改变肠道微生物群, 具有潜在的肿瘤预防作用。

香菇多糖(lentinan)是来自于香菇中的一种葡聚糖, 早在上世纪80年代被中国和日本批准为治疗癌症的辅助治疗药物, 整理临床资料显示香菇多糖对提高肿瘤治疗期间患者的生活质量, 提高化疗和放疗的疗效有显著作用^{[24, 25]}。岩藻多糖(fucoidan)是从褐藻中分离得到的硫酸多糖, 具有广泛的生物活性。已有研究表明, 岩藻多糖可诱导HT-29和HCT116人结肠癌细胞凋亡。猪苓多糖(polyporus polysaccharide)对于增强免疫活性、抑制肿瘤生长具有显著的作用, 可应用于原发性肺癌、肝癌、子宫癌和食管癌等恶性肿瘤放疗、化疗的辅助治疗^[26]。

3 生物碱类

生物碱(alkaloid)是自然界植物中所含有的一类含氮的碱性有机化合物, 其抗肿瘤作用机制主要包括对肿瘤细胞的直接杀伤作用、阻滞细胞周期、诱导细胞凋亡、抑制肿瘤血管生成、抑制肿瘤细胞浸润和转移、逆转肿瘤细胞多药耐药性等^[27]。抗肿瘤的代表性化合物包含喜树碱类、长春碱类、苦参碱、秋水仙碱和三尖杉酯碱等。

喜树碱(camptothecin)是上世纪60年代从喜树中分离纯化出的一种对白血病、胃癌和直肠癌具有显著疗效的生物碱, 但上世纪末期, 由于严重的恶心、呕吐、腹泻、脱发毒副作用及水溶性差, 成钠盐后抗肿瘤作用降低等缺陷, 应用受限, 研究停滞不前。故研究者对其结构进行修饰, 旨在增加其溶解性及稳定性, 同时提高抗肿瘤活性, 其作用机制主要是作为拓扑异构酶1抑制剂进行研究^{[28, 29]}。长春新碱(vincristine)作为一种微管抑制剂, 能够附着在微管蛋白二聚体上, 抑制细胞分裂中期的有丝分裂, 从而阻止微管结构的合成^[30]。毛丽梅等^[31]研究苦参碱(matrine)可联合卡帕用于治疗中晚期宫颈癌, 研究所用为苦参碱注射液, 主要成分为氧化苦参碱、苦参碱及少量氧化槐果碱。实验证明, 经联合治疗后, 鳞状细胞癌相关抗原(squmaous cell carcinoma antigen, SCC-A)和恶性肿瘤特异性生长因子(tumor specific growth factor, TSGF)表达均下降, 表明细胞增殖作用减慢, 联合治疗方案可有效杀灭肿瘤细胞, 阻止病情恶化。SCC-A水平的升高与宫颈癌淋巴结转移、浸润深度密切相关^[32]。TSGF是参与多种肿瘤细胞增殖、浸润及转移的相关代谢物的总称, 在肿瘤的发生、发展及预后具有重要意义^[33], 详情见表 2。

4 多酚类

多酚(polyphenol)类是指具有多个酚基的化合物, 代表性的抗肿瘤天然产物有大豆异黄酮、槲皮素、白藜芦醇、姜黄素等。日常生活中常见食品如大豆, 所含大豆异黄酮(soy isoflavone)目前已开发为保健食品使用, 具有抗肿瘤作用, 可能与抑制DNA拓扑异构酶2活性、抗氧化、抑制自由基及新生血管生成有关^[34]。槲皮素(quercetin)在很多蔬菜、水果中都有分布, 对卵巢癌、淋巴癌、乳腺癌、前列腺癌和肺癌等^[35-38]具有抑制作用, 能阻断雄激素诱发的人类前列腺癌, 诱导细胞凋亡, 抑制微血管形成, 抗氧化并有辅助增强其他抗肿瘤药物的作用, 另外槲皮素可通过下调热休克蛋白的表达来逆转肿瘤细胞的耐药性。白藜芦醇(resveratrol)是植物受到细菌或真菌等病原体攻击后合成的多酚类化合物^[39]。有研究表明, 白藜芦醇可通过诱导细胞周期及细胞凋亡, 促进肿瘤血管破坏等途径发挥对肝癌、乳腺癌、胃癌和白血病等肿瘤细胞的抑制作用^[40-44]。姜黄素(curcumin)是姜科植物姜黄根茎的提取物, 美国国立肿瘤研究所将其列为第三代恶性肿瘤预防用物, 能够影响肿瘤细胞周期, 抑制细胞增殖, 促进其自我凋亡, 对肿瘤血管生成也具有一定抑制作用^[45], 详情见表 3。

5 有机酸类

有机酸(organic acid)类是指具有酸性的有机化合物, 一部分有机酸可参与到动植物代谢的生命过程中, 属于代谢的中间产物, 具有显著的生物活性, 能防病、治病。其中抗肿瘤天然产物典型代表有白桦酸、五倍子酸及其衍生物、熊果酸、大黄酸等。

有研究表明, 白桦酸(betulinic acid)可选择性抑制黑色素瘤, 能抑制NF-κB的活性, 从而通过其所介导的吞噬作用发挥抗凋亡作用^[46]。潘洪双等^[47]对熊果酸(ursolic acid)结构进行修饰, 在C-28位连接氨基醇类化合物、成酯等结构改造均能使熊果酸抗肿瘤活性有不同程度的提高。袁向飞等^[48]研究发现中药有效成分大黄酸(rhein)能够通过增强低氧环境中效应T细胞的抗结肠癌活性, 与T细胞过继免疫疗法联合使用治疗结肠癌, 详情见表 4。

6 挥发油类

挥发油(essential oils)又称精油, 具有挥发性并可随水蒸气蒸馏, 大部分具有香气。在菊科、芸香科和伞形科等植物中, 挥发油含量较高, 主要是由萜类和芳香族化合物以及他们的含氧衍生物如醇、醛、酮、酚、醚、内脂等组成, 此外还包括含氮及含硫化合物^[49]。挥发油发挥抗肿瘤的作用主要与抑制增殖和诱导凋亡、增强免疫和调控功能、抗氧化、抑制突变、调节肿瘤细胞耐药性等有关^[50]。挥发油类抗肿瘤天然产物典型代表有白术挥发油、姜黄挥发油、当归挥发油、花椒挥发油等。

白术挥发油(Atractylodes macrocephala volatile oil)主要成分是苍术酮, 对肝癌和胃癌效果显著^[51]。当归挥发油(Angelica volatile oil)对宫颈癌、前列腺癌、乳腺癌等效果较好^[52]。姜黄挥发油(Turmeric volatile oil)能有效抑制人宫颈癌HeLa细胞^[53]、急性单核细胞白血病THP-1细胞^[54]增殖。花椒挥发油(essential oil of zanthoxylum maxim)具有广谱抗肿瘤作用, 能够有效诱导人肺癌肝癌细胞、黑色素瘤细胞A375、人胃癌SGC-7901细胞^[55]凋亡。乳香精油(frankincense oil)可显著降低小鼠(B16-F10)和人类黑色素瘤(FM94)的细胞存活率(P < 0.05), 但对正常人上皮黑色素细胞(HNEM)无影响^[56]。

7 皂苷类

皂苷(saponin)类可以显著抑制肿瘤细胞增殖, 当质量浓度为200 μg·mL^-1时, 对肺癌A549细胞的抑制率可达到47.0%^[57]。经研究, 人参皂苷CK可通过抑制细胞外信号调节激酶(extracellular signal-regulated kinase, ERK)和Akt信号通路来阻断肝癌细胞的迁移和侵袭^[58]。胡双双等^[59]研究人参皂苷Rh2与多柔比星联用能够提高人胃癌SGC7901/ADR细胞的化疗敏感性。人参皂苷Rg3能够抑制胃癌(SNU-216)细胞增殖, 并呈现剂量依赖性, 作用机制可能与抑制肿瘤细胞的黏附、浸润及抗新生血管的生产有关^[60], 详情见表 5。

8 多肽类

多肽(polypeptide)类是α-氨基酸以肽键形式连接在一起形成的化合物, 是蛋白质水解的中间产物, 一般由10~100个氨基酸分子经脱水缩合而成。近年来, 不仅多肽的抗菌作用得到广泛的应用, 其抗肿瘤作用也引起了业界的关注。与传统化疗药物相比, 多肽类药物具有分子量小、对肿瘤敏感、不易耐药、成本低等特点。在治疗结直肠癌、胃癌和胰腺癌方面得到广泛应用^[61]。

多肽类药物的抗肿瘤作用机制主要为调节免疫功能、抑制肿瘤心血管形成、促进细胞凋亡等^[62]。目前多肽主要以3种形式应用在肿瘤治疗中:单独使用多肽; 多肽类疫苗; 制成多肽与纳米载体共轭物。欧红利等^[63]研究表明, 美洲大蠊多肽发挥抗肿瘤活性是通过增强荷瘤小鼠抗肿瘤免疫功能来实现的。Ding等^[64]报道指出蝎子的毒液可直接提取用于治疗肿瘤, 其毒液中存在一种天然抗菌肽, 其可通过抑制肿瘤细胞生长达到抗肿瘤的功效。

由于海洋生态环境的特殊性, 如一定水压、较高盐度、较小温差、有限光照、有限溶解氧等, 形成了独特的海水化学缓冲体系, 加之独特生物合成途径、特殊的酶反应机制, 造就了化学结构新颖、生物活性显著、作用机制独特的海洋天然产物^[65]。基于此特性, 海洋天然产物常用于抗肿瘤、抗病毒、抗炎症、抗细菌等领域, 而海洋抗肿瘤活性物质的研究也一直占据重要地位^[66]。

苔藓抑素(bryostatin1)来源于苔藓虫, 属于大环内酯类化合物, 具有抗卵巢癌和乳腺癌等作用, 可阻断蛋白激酶C合成, 抑制肿瘤增殖, 目前进入Ⅲ期临床研究^[67]。海兔素(dolastatin10)来源于海兔, 属于肽类化合物, 具有抗乳腺癌、肝癌、实体瘤及白血病等作用, 其作用机制主要是通过对抗细胞有丝分裂发挥作用, 目前进入Ⅱ期临床研究^[68]。海鞘素(ecteinascidin743)属于四氢异喹啉类生物碱, 大部分由半合成获得, 可通过影响细胞内微管网组织, 破坏细胞DNA链而对抗细胞有丝分裂来抑制肿瘤细胞增殖, 目前已上市销售^[69]。

化疗是目前治疗癌症的主要手段之一, 但癌症本身易复发, 多以失败告终, 并且化疗产生的一系列毒副作用和并发症也不同程度地影响癌症患者的生活质量。目前使用的大多数抗癌药物, 都能通过激活抗生长信号以防止肿瘤细胞增殖。然而, 高昂的治疗费用和对这些药物产生的耐药性是医生和患者面临的巨大挑战。因此, 寻找更易获得的天然抗肿瘤成分或前体药物具有极大的吸引力, 自然界中的动植物、微生物以及海洋生物都是新药研究发展的重要源泉, 从中寻找天然的抗肿瘤成分是抗癌药物研发的重要手段^[70]。许多天然产物已被证明可以通过控制有丝分裂和细胞周期抑制癌细胞增殖, 因此是化疗药物的重要潜在替代品^[71]。

天然产物是一个丰富的药物开发宝库, 对我国这样的天然产物大国来说, 更好更准确地利用优势资源, 是引领我国新药研发的正确方向。但天然产物往往具有水溶性差、不良反应显著等诸多问题, 因此在天然产物的开发利用中, 对天然产物进行修饰结构、计算机辅助设计、新型药物递送等多种技术的应用有着重要的科学意义和应用价值。天然抗肿瘤产物的药物开发仍然任重而道远, 急需研究者们一起为之努力。
