A bioassay method for inhibiting platelet aggregation in vitro was established to quantify the pharmacological effects of Compound Danshen Tablets and support its quality control. The inhibition of platelet aggregation in rabbit plasma in vitro by Compound Danshen Tablets was used as the experimental system. The titer was calculated by using the method of dose-response parallel lines. As a result, a bioassay for the inhibition of platelet aggregation in vitro by Compound Danshen Tablets was established. Linearity was good in the concentration range of 0.128 g·mL-1 to 0.205 g·mL-1, and the titer of standard Compound Danshen tablets was 7 659 U·g-1 according to the titer definition. This in vitro assay was simple, reliable, reproducible and convenient. The activity of Compound Danshen Tablets in inhibiting platelet aggregation was quantified by potency assay and the quality of different batches was evaluated. The method can be applied for the quality control of Compound Danshen Tablets.

本文建立复方丹参片体外抑制血小板聚集的生物活性测定法，量化复方丹参片的药理作用，补充其质量控制方法。以复方丹参片体外抑制家兔血浆血小板聚集作用为实验体系，通过量效关系考察和效价定义，利用量反应平行线法计算效价，建立复方丹参片体外抑制血小板聚集的生物活性测定方法，并进行方法学验证，考察方法的可行性。通过量效关系考察，发现在浓度0.128~0.205 g·mL-1范围内线性良好，并根据效价定义得出复方丹参片标准品效价为7 659 U·g-1，方法学考察表明方法可靠，重现性和适用性良好。复方丹参片抑制血小板聚集的生物活性测定法为体外方法，操作简单、方便，测定结果可靠，并能通过效价测定量化了复方丹参片抑制血小板聚集的活性，评价不同批次质量，可以尝试将该方法推广到其质量控制中，为中药质量控制开辟新的途径。

本文建立复方丹参片体外抑制血小板聚集的生物活性测定法，量化复方丹参片的药理作用，补充其质量控制方法。以复方丹参片体外抑制家兔血浆血小板聚集作用为实验体系，通过量效关系考察和效价定义，利用量反应平行线法计算效价，建立复方丹参片体外抑制血小板聚集的生物活性测定方法，并进行方法学验证，考察方法的可行性。通过量效关系考察，发现在浓度0.128~0.205 g·mL-1范围内线性良好，并根据效价定义得出复方丹参片标准品效价为7 659 U·g-1，方法学考察表明方法可靠，重现性和适用性良好。复方丹参片抑制血小板聚集的生物活性测定法为体外方法，操作简单、方便，测定结果可靠，并能通过效价测定量化了复方丹参片抑制血小板聚集的活性，评价不同批次质量，可以尝试将该方法推广到其质量控制中，为中药质量控制开辟新的途径。

A bioassay method for inhibiting platelet aggregation in vitro was established to quantify the pharmacological effects of Compound Danshen Tablets and support its quality control. The inhibition of platelet aggregation in rabbit plasma in vitro by Compound Danshen Tablets was used as the experimental system. The titer was calculated by using the method of dose-response parallel lines. As a result, a bioassay for the inhibition of platelet aggregation in vitro by Compound Danshen Tablets was established. Linearity was good in the concentration range of 0.128 g·mL-1 to 0.205 g·mL-1, and the titer of standard Compound Danshen tablets was 7 659 U·g-1 according to the titer definition. This in vitro assay was simple, reliable, reproducible and convenient. The activity of Compound Danshen Tablets in inhibiting platelet aggregation was quantified by potency assay and the quality of different batches was evaluated. The method can be applied for the quality control of Compound Danshen Tablets.

复方丹参片是含丹参的一种制剂, 主要含有丹参、三七和冰片, 具有活血化瘀, 理气止痛的功效, 用于治疗气滞血瘀所致的胸闷(症见胸闷, 心前区刺痛)、冠心病和心绞痛。目前复方丹参片临床使用广泛, 前期研究发现不同生产企业, 复方丹参片质量参差不齐[1], 不同厂家或同一厂家不同批号的丹参片中丹参素、人参皂苷Rg1、三七皂苷R1、丹参酮ⅡA等成分的含量差异较大, 产品质量不稳定。近年来, 复方丹参片作为全国统一抽验品种, 部分单位也因抽验不合格被药品监督管理部门撤销批准文号, 并且有些企业为了谋取更多利润, 存在掺假现象, 这表明现行的质量控制方法存在不足。
中药成分复杂, 而单一的控制少数指标性成分难以控制药物的有效性和安全性, 急需对现有的中药质量控制方法进行改进[2]。生物检定法最近几年日趋成熟, 并且在某些西药质量控制中生物检定法已经占据了主导地位, 《中国药典》2010年版一部中, 增加了中药的生物活性测定研究指导原则[3], 说明在中药质量标准中增订生物活性测定项, 已经是一种方向[4]。如果能够率先在质量标准中制定生物活性测定项, 必将大大推动中药的发展。因此生物活性测定法用于中药质量控制已是一种必然的趋势。
前期药效学研究发现复方丹参片在体内外对血小板聚集均具有较好的抑制作用, 并且具有明确的剂量依赖关系, 因此本文尝试建立复方丹参片体外抑制血小板聚集的生物活性测定法, 量化复方丹参片的活血化瘀作用, 对其质量控制方法进行补充, 更好的指导复方丹参片的临床使用。
材料与方法

实验动物  普通级日本大耳白家兔, 许可证号SCXK (京) 2006-0009, 由北京维通利华实验动物技术有限公司提供。
实验试剂  5 μmol·L-1二磷酸腺苷ADP (北京世帝科学仪器公司); 0.9%氯化钠注射液(中国大冢制药有限公司), 批号0B89H4;枸橼酸纳(北京化学试剂公司), 批号20140804;灭菌注射用水(大同市惠达药业有限责任公司), 批号20140312。复方丹参片, 批号: 12120393。
实验仪器  LG-PABER-Ⅰ型血小板聚集凝血因子分析仪(北京世帝科学仪器公司), DT5-1离心机(北京时代北利离心机有限公司), MEK-6318血液分析仪(北京健长医疗器械有限公司)。
血浆制备  家兔采取颈总动脉放血75 mL, 加3.8%枸橼酸纳(1:9)抗凝, 1 000 r·min-1离心10 min制备富血小板血浆(PRP), 剩余血样继续3 500 r·min-1离心10 min制备贫血小板血浆(PPP)。用血液分析仪测定PPP和PRP中的血小板数, PPP中血小板数一般在5×109个·L-1以下, 利用PPP调节PRP中的血小板数在4.0×1012~4.5×1012个·L-1之间[5], 为保证测量的准确性, 血小板聚集的测定应在4 h内完成, 且PRP不可冷冻保存和过度振荡[6]。
测试样品配制  取复方丹参片10片, 研磨后, 加0.9%生理盐水至10 mL, 超声(功率300 W, 50 kHz) 20 min, 然后用离心机以3 000 r·min-1离心10 min, 取上清液, 用0.22 μm针头过滤器滤过, 得复方丹参片提取原液。配制成剂间比为0.8, 用生理盐水进行稀释, 质量浓度为0.32、0.256、0.205、0.16、0.128、0.102、0.082、0.066、0.052、0.042和0.032 g·mL-1溶液, 备用。
血小板聚集测定  取PPP 210 μL加入测试杯, 进行仪器调零。再取PRP 180 μL[7], 分别加入不同浓度的样品液20 μL, 37 ℃孵育180 s[8], 分别加入聚集诱导剂10 μL, 测定5 min内的最大聚集率[9]。
量效关系考察  取配置好的复方丹参溶液, 依“血小板聚集测定”项下方法, 分别测定含药兔血浆在5 min内的血小板最大聚集率, 多次测定考察量效关系。
标准品确立  由于复方丹参片还没有相关的标准品, 为了采用生物活性测定法进行样品的效价测定, 必须建立一批标准品作为标尺来衡量其他样品的效价。因此需要建立复方丹参片的标准品, 考虑到中药成分的复杂性, 很难找到合适的西药作为标准品, 在2010年中国药典中的中药生物活性测定方法的指导原则中指出, 最好选择与供试品同质的标准品, 即主要成分相似, 因此作者选择一批复方丹参片作为标准品。该批样品的原料为道地药材, 且生产工艺稳定, 理化检验合格, 临床疗效明确。结合临床调研和前期药效学实验, 确立本次实验以批号为12120393的复方丹参片作为标准品。
效价定义  本文规定浓度为0.205 g·mL-1的复方丹参片溶液使空白(加0.9%生理盐水)血浆血小板最大聚集率降低1个百分点定义为1个效价单位(U)。连续测定4次, 取平均值作为该批样品的效价。1 g复方丹参片的效价(U·g-1) = x×[1/(0.205×0.02 mL)], 其中, x为复方丹参片溶液使空白家兔血浆血小板最大聚集率降低值(%), 根据复方丹参片效价的定义, 测定复方丹参片标准品的效价。
效价测定  取复方丹参片标准品, 按照“测试样品配制”操作, 用0.9%氯化钠注射液将样品配制为0.205、0.164和0.128 g·mL-1的溶液, 剂间比0.8, 作为标准品组(S); 取复方丹参片待测样品, 按照2.2中样品处理办法, 用0.9%氯化钠注射液将样品配制为0.205、0.164和0.128 g·mL-1的溶液, 剂间比0.8, 作为试品组(T)。依“血小板聚集测定”项下方法, 分别测定标准品组和供试品组含药血浆的血小板最大聚集率。按照S1、T1、T2、S2、S3、T3的顺序(排除因S组和T组测定的先后次序引起的误差), 平行测定3次。按《中国药典》生物检定统计法中量反应平行线(3, 3)法进行统计学处理, 考察结果的可靠性, 计算T组的效价(PT)和可信限率(FL)。
方法学考察  对建立的方法进行方法学考察, 主要考察重复性、中间精密度及加样回收率, 同时测定其他批次的复方丹参片样品, 进行方法的适用性考察。
重复性考察  用效价测定方法, 在同样的条件下, 对待测的1批复方丹参片样品连续进行生物活性效价测定, 重复测定6次, 并进行可靠性检验, 计算结果的平均值和相对标准偏差(RSD%)。
中间精密度考察  用效价测定方法, 在同样的条件下, 对待测的1批复方丹参片样品进行不同人员生物活性效价测定, 每个人员重复测定3次, 进行可靠性检验, 并计算结果的平均值和相对标准偏差(RSD%)。
回收率考察  取复方丹参片标准品, 精密配制成相当于约1倍已知效价的样品浓度的标准品, 然后将上述标准品溶液与已知效价浓度的样品按1:1体积混合, 得到加标样品溶液, 其效价应为已知效价样品的100%。用效价测定方法, 对上述浓度的加标样品进行6次效价测定。记录每份加标样品中含有已知样品的效价浓度(A)、加入标准品的效价浓度(B)、加标样品的实测效价浓度(C)。按照以下公式计算加标回收率(%), 并计算回收率的相对标准偏差(RSD%)。回收率(%) = (C-A)/B×100%
适用性考察  按拟建立的体外血小板聚集的生物活性测定方法对其他2批次的复方丹参片进行效价测定, 以累积数据, 考察质量标准中该测定方法的适用性。

1 量效关系考察
研究发现, 复方丹参片浓度较低时, 体外对血小板聚集作用不明显, 不具有剂量依赖性, 浓度在0.102~0.205 g·mL-1内剂量依赖性较为明显, 在该浓度范围内随着复方丹参片浓度的增加, 体外对血小板聚集的抑制作用更强, 血小板聚集率降低值越大, 为确定上述结论进行进一步的量效关系考察。
为了进一步确立浓度在0.102~0.205 g·mL-1内量效关系, 将复方丹参片提取溶液配制成0.205、0.164、0.128和0.102 g·mL-1等4个浓度, 剂间比为0.8, 进行6次的重复测定, 结果见表 1。
由表 1可见, 复方丹参片浓度在0.102~0.205 g·mL-1, 体外对血小板聚集抑制作用基本成线性, 6次测定结果的重复性较好, 但浓度在0.102 g·mL-1时有拐点影响整体的线性, 因此在线性考察的过程中不用该浓度。
量效关系考察发现, 复方丹参片在浓度0.128~0.205 g·mL-1线性良好, 可以利用生物活性测定法中的量反应平行线(3, 3)法进行下一步实验, 使用量反应平行线法要求量效曲线必须成线性, 因此需要进行一步线性考察, 以复方丹参片的浓度为X轴, 以体外血小板最大聚集率降低值为Y轴作图, 标示线性方程和相关系数, 结果见表 2。
经计算3次测定结果的回归方程分别为Y1=70.76x+15.54、Y2=106.3x+9.145、Y3=105.9x+6743, 相关系数分别为0.972 6、0.959 2和0.981 3。可以看出, 复方丹参片的浓度在0.128~0.205 g·mL-1内, 血小板最大聚集率降低值与复方丹参片的浓度线性较好, 相关系数均在0.9以上, 可以考虑用量反应平行线法测定效价。
2 标准品效价及统计方法的选择
根据效价的定义, 4次测定加样浓度为0.205 g·mL-1的血浆血小板最大聚集率降低值分别为30.6、31.6、28.9和34.4, 降低值的平均值为31.4, 则1 g复方丹参片的效价为31.4×(1/0.205×0.02) = 7 659 U·g-1。
结果发现, 复方丹参片在一定的浓度范围内, 血小板最大聚集率降低值和复方丹参片浸提液的浓度呈现较好的线性关系(相关系数r > 0.9), 因此本文选用了量反应平行线(3, 3)法作为生物统计的方法。
3 效价测定与可靠性考察
标准品与供试品按剂间比按1:0.8各配置高、中、低3个浓度, 每一浓度测定3~5个值, 用量反应平行线(3, 3)法进行统计, 并进行可靠性检验, 结果如下: sm = 0.013 378, 测得效价PT = 8 121.5 U·g-1, 测得效价的可信限率为FL = 6.488%, 测得效价的可信限范围为7 626.3~8 680.1 U·g-1。
可靠性检验的结果分析, 药品间差异无显著性意义, 表示供试品的标示量基本正确。本实验结果P > 0.05, 说明供试品的标示量准确。偏离平行、二次曲线、反向二次曲线三者均无显著性意义, 表示两条直线基本平行并符合直线关系。回归间及组间差异有显著性意义, 表示实验剂量安排合理。以上结果表明测定批次的复方丹参片的效价准确, 可信限率 < 20%, 说明方法可靠。
由以上结果可以看出, 复方丹参片标准品和测定样品虽然同质, 理化检验结果基本一致, 但测得效价却不相同, 说明不同批次的复方丹参片样品抑制血小板聚集的活性存在差异, 进一步说明该方法能够衡量不同批次样品间抑制血小板聚集的活性。
4 方法学考察
4.1 重复性考察
由表 5可见, 6次测定结果为7 790.6 ± 857.7 U·g-1, 测得结果均通过可靠性检验, FL平均值12.08%, 小于20%, RSD%为7.2%, 小于10%, 说明该方法测定的效价重复性良好, 方法的重复性较好。
4.2 中间精密度考察
表 6可见, 不同实验人员测得结果均通过可靠性检验, FL平均值13.94%, 小于20%, 相对标准偏差4.4%, 小于10%, 说明不同实验人员对测定结果的影响不大, 中间精密度良好。
4.3 回收率考察
按照复方丹参片效价测定的反应体系, 用平行线3*3进行统计, 用回收率公式1进行计算, 结果见表 7。由表 7可以看出, 6次效价浓度测定的结果均通过可靠性检验, FL小于20%, 回收率均值108.2%, 接近100%, 回收率的相对标准偏差小于15%, 说明测定方法的准确度高。
4.4 适用性考察
适用性考察表明2次分别测定结果的可靠性检验均合格, 回归显著(P < 0.01);偏离平行、二次曲线、反向二次曲线均不显著(P > 0.05);可信限率均小于10%, 测得效价平均值的RSD均小于5%, 因此说明用血小板聚集的方法测定出复方丹参片的生物效价方法可靠, 适用性较好, 具有一定的可行性。

由上述结果可以看出, 复方丹参片体外对血小板聚集有较好的抑制作用, 并且剂量依赖性明显。线性关系考察发现不同浓度的复方丹参片提取溶液对抑制家兔血浆血小板聚集具有良好的线性关系, 可靠性检验均合格表明该方法得到的测得效价可靠。方法学考察表明, 建立的复方丹参片体外血小板聚集的生物检定方法学, 重复性、中间精密度以及加样回收率较好, 适用性良好, 测定出的复方丹参片生物效价可靠, 能够很好地衡量不同批次复方丹参片抑制血小板聚集的活性, 方法具有一定的可行性, 值得进行深入的研究。
目前复方丹参片生产厂家较多, 不同厂家生产的产品质量层次不齐, 现有质量控制方法以理化法为主, 不能衡量不同厂家及批次产品间药效的差异, 这就进一步影响复方丹参片临床使用的疗效, 因此通过对复方丹参片生物活性测定方法的研究, 将大大提高复方丹参片质量评价标准, 确保其临床使用的准确性。
目前中药生物活性测定法还处在起步阶段, 该方法的关键问题是标准品确立和实验体系选择[10], 目前中药还没有标准品建立参考原则, 因此考虑到中药的复杂性与生物制品类似, 因此本文按照生物制品标准品建立思路[11], 尝试建立了复方丹参片抑制血小板聚集活性的标准品, 并进行了效价定义, 标定其效价[12], 测定其他批次样品的效价。通过本次研究, 为中药标准品的建立及生物测定方法的研究开创了新的思路, 如果能将复方丹参片标准品进行协作标定, 确定其稳定性, 必将极大的推进复方丹参片生物活性测定法进步。
测定体外血小板聚集实验时, 血小板数量是影响实验结果的主要因素, 因此每次实验可通过贫血小板血浆调节, 以保证血小板数在合适范围内。制备好的血浆最好在4 h内完成测定, 时间过长血浆的稳定性会下降。操作过程中避免过度震荡, 过度震荡也会使血浆的稳定性下降, 同时避免冷藏, 温度过低会加速血小板聚集。以上操作会影响实验结果, 操作过程中应当注意。
由于复方丹参片体外抗血小板聚集的实验操作简单、快速、准确、费用低, 便于建立的生物活性测定方法[13], 利于在药检部门推广。该方法是对复方丹参片的质量控制方法进行了很好的补充, 完善其质量控制方法, 保证其临床使用的安全有效。由于其他类中药及制剂也具有抑制血小板聚集的作用[14, 15], 可以尝试着将这种方法推广应用在其质量控制中, 为中药质量控制方法提供新的思路。
