新药创制是复杂的智力活动, 涉及科学研究、技术创造、产品开发和医疗效果等多维科技活动。每个药物都有自身的研发轨迹, 而构建化学结构是最重要的环节, 因为它涵盖了药效、药代、安全性和生物药剂学等多重性质。本栏目以药物化学视角, 对有代表性的药物的成功构建, 加以剖析和解读。
辉瑞公司研制的格拉德吉, 是以干扰刺猬信号通路为靶标治疗急性髓性白血病的首创性药物, 在多个单位致力于研发Smo受体抑制剂中, 成为全球第一个上市的药物。当今处于临床研究状态的Smo抑制剂结构类型归属于两类: 天然活性物质环巴胺的类似物和苯并咪唑联苯胺类的合成化合物。本品是以苯并咪唑联苯化合物为先导物, 通过改变苯环为哌啶连接基, 用构效分析和试错反馈调整分子结构和构象, 优化活性-药代-物化性质而成功的。(编者按)
刺猬信号通路(hedgehog signaling pathway, Hh) 是哺乳动物调控蛋白引发的信号传导, 该蛋白在突变的果蝇胚胎中呈多毛团状, 酷似受惊刺猬而得名。人体存在3种Hh同源基因: sonic hedgehog (SHH)、Indian hedgehog (IHH) 和desert hedgehog (DHH), 分别编码Shh、Ihh和Dhh蛋白。
Hh信号传递受控于两种膜受体: patched (Ptc) 和smoothened (Smo) 受体。Ptc由肿瘤抑制基因patched编码, 能与Hh直接结合, 对信号起负调控作用; Smo由原癌基因smothened编码, 同源于G蛋白偶联受体, 跨膜区氨基酸序列高度保守。C端位于细胞内, 末端的丝氨酸与苏氨酸残基为磷酸化部位, 蛋白激酶催化磷酸基的结合。Smo是Hh信号传递所必需的受体。底物或激动剂激活Smo受体引起Gli1、Gli2和Gli3等3种转录因子的激活, 最终导致细胞增殖; Smo基因发生突变时, 可出现与Hh基因突变相同的表征。
在正常情况下, Ptc抑制跨膜的Smo蛋白活性, 从而抑制下游通路和靶基因的转录。当Ptc发生突变或缺失时, 或是Smo突变导致对Ptc的抑制作用不敏感而致使基因活化, 则Hh信号通路失控。Hh信号通路失调可引起多种癌症发生, 因此, Smo受体拮抗剂成为治疗癌症的潜在靶标。
就刺猬信号通路的作用本质而言, 是刺猬蛋白与受体之间的蛋白-蛋白相互作用, 本项目研发的小分子抑制剂是对Smo受体的活性抑制, 用产生Gli蛋白量表征对Smo的抑制强度。
最早研究的Smo受体抑制剂是从加州藜芦(Veratrum californicum) 分离的生物碱环巴胺(1, cyclopamine), 1作为化学生物学的工具药证明是Smo受体的拮抗剂并阐明了在Hh信号通路中的作用, 还作为临床前药物研究模型的对照药(Taipale J, Chen JK, Cooper MK, et al. Effects of oncogenic mutations in smoothened and patched can be reversed by cyclopamine. Nature, 2000, 406: 1005-1009)。
Infinity公司基于1为先导物做结构优化得到萨利德吉(2, saridegib), 是D环扩为七元环的类似物, 已进入临床研究(Peukert S, Miller-Moslin K. Small-molecule inhibitors of the hedgehog signaling pathway as cancer therapeutics. ChemMedChem, 2010, 5: 500-512)。
最早发现人工合成的Smo抑制剂是2-苯基咪唑化合物3, 一些公司以这个联芳基分子为先导物, 进行了广泛的研究, 同处于临床阶段的有基因泰克公司的维莫德吉(4, vismodegib) (Dlugosz A, Agrawal S, Kirkpatrick P. First-in-class inhibitor of the hedgehog signalling pathway approved in the United States for the treatment of basal cell carcinoma. Nat Rev Drug Discov, 2012, 11: 437-438) 和诺华公司的索尼德吉(5, sonidegib) (Pan W, Wu X, Jiang J, et al. Discovery of NVP-LDE225, a potent and selective smoothened antagonist. ACS Med Chem Lett, 2010, 1: 130-134)。45都保留了联芳基酰胺的骨架结构。
体外评价化合物活性是将载有荧光素酶的Gli蛋白的基因转染到小鼠C3H10T1/2细胞, 细胞培养基中加入一定量的Smo受体激动剂, 再与不同浓度的受试物温孵, 测定由于抑制Smo受体而改变的荧光强度, 作为表征Gli蛋白的生成量, 确定受试物的50%抑制Smo活性的摩尔浓度(IC50)。
评价化合物的代谢稳定性是与大鼠和人肝微粒体温孵, 计算受试物的大鼠或人肝清除率(CL, μL·min-1·kg-1)。
辉瑞公司启动本项目的先导物也是化合物3, 在结构变换中, 将3分为3个片段: 左侧苯并咪唑、中间苯胺和右侧取代的苯甲酰片段。
通过分析进入临床研究的候选药物例如45的结构, 发现都保持了联芳基的结构, 舍弃了苯并咪唑, 而且中间苯环的2位(邻位) 都存在取代基, 意味着两个芳环不成共面性, 因为邻位取代使两个芳环维持一定夹角。辉瑞公司没有遵循联芳基的惯例, 而是另辟蹊径, 保持苯并咪唑环, 改中间的苯环为脂肪环, 并且以1, 3-cis连接苯并咪唑和右侧片段, 以此模拟化合物3~5的间位连接, 合成的代表性分子首先是环己基(6) 化合物, 后优化成哌啶基(7) 化合物。
表 1中列出了代表性分子的结构与活性。化合物6经1R, 3S-环己基替换1, 3连接的苯基, 活性降低了18倍, 然而亲脂性降低了1.6个对数单位, 提高了代谢稳定性, 从而将3的清除率CL = 314 μL·min-1·kg-1降低到33 μL·min-1·kg-1。权衡中间苯环变换为环己基药代和药效的得失, 环己基具有优化的特征和前景。不过, 6的溶解性虽稍高于3, 但仍属于微溶性(37 μg·mL-1), 口服生物利用度(F) 只有4%。
6作为新的起始物, 固定环己基, 变换苯并咪唑和苯并二噁烷片段, 合成了如图 1所示的300多个化合物, 却都没有显示出活性的明显改善(Munchhof MJ, et al. Benzimidazole derivatives. WO 2008, 075096 A1)。
为了提高分子的溶解性, 用四氢吡咯或哌啶环替换环己基, 不仅提高了分子极性, 还可成盐增加水溶性, 在合成的目标物中化合物7为2R, 4R异构体, 活性强于6一倍, 制备并测定了其他差向异构物的活性, 均低于6大约4~50倍。体外实验化合物7的代谢稳定性, 对大鼠和人肝微粒体是稳定的, 对犬的体内实验CL = 18 μL·min-1·kg-1, 口服生物利用度(F) 为33%, 提示口服剂量的50%可被吸收, 显著强于化合物6
哌啶作为中间环优于环己基, 故以7为新的先导物从3个方面做结构优化: ①变换N1取代基; ②苯并咪唑环上取代基的变换; ③氨酰基连接的片段变换。通过广泛的构效关系分析, 合成了2R, 4R-构型70余个化合物如图 2所示的通式。化合物的活性和物化性质优于环己基系列(Munchhof MJ, et al. Benzimidazole derivatives. WO 2009, 0005416 A1)。
在变换连接于氨基哌啶的胺酰片段中, 发现连接基为脲基化合物的活性显著提高, 遂合成了以脲基连接的烷基、环烷基、芳基化合物等80余个, 通式结构列于图 3 (Jones CS, et al. Benzimidazole derivatives. WO 2009, 0004427 A2)。代表性的化合物列于表 2, 构效关系简述如下: ①苯环上邻位取代基不利于活性; ②间位和对位取代基有利于活性, 尤其是对位优于间位取代。例如化合物1314的活性强14倍; ③对位取代的拉电子基团有利于活性, 例如化合物13 (p-CN, σp = 0.66) 和化合物11 (p-CF3, σp = 0.54) 的IC50都是5 nmol·L-1; 化合物12 (p-Cl, σp = 0.23) 和10 (p-OCH3, σp = -0.27) 的活性明显减弱; ④吡啶环化合物(15) 由于替换了苯环活性减弱很多。化合物13的clog D = 2.06, 脂溶性适度, 而且代谢稳定性也较好CL < 12.8 μL·min-1·kg-1, 因而确定了13为候选化合物作进一步研发。
化合物13的相对分子质量374, log D实验测定值2.49, 极性表面积97 Å2, 在高浓度下对多种激酶未呈现抑制作用, 提示对Smo受体有较高选择性抑制作用。对于与药物代谢关系密切的多种细胞色素P450未呈现抑制作用, IC50 > 30 μmol·L-1表 3列出对大鼠和犬的药代动力学数据, 提示有适宜的口服生物利用度、半衰期和血浆蛋白结合率(Munchhof MJ, Li QF, Shavnya A, et al. Discovery of PF-04449913, a potent and orally bioavailable inhibitor of smoothened. ACS Med Chem Lett, 2012, 3: 106-111)。
基于临床前研究结果, 确定13为候选化合物, 定名格拉德吉(glasdegib)。经Ⅲ期临床研究, 表明格拉德吉是口服治疗急性髓性白血病的有效药物, 经美国FDA批准于2018年上市。
