Oxcarbazepine (OXC) is a common antiepileptic drugs. In this study, one hundred and eighty four epilepsy patients with 196 observations of oxcarbazepine's active metabolite, 10, 11-dihydro-10-monohydroxy carbazepine (MHD) were collected prospectively from routine clinical monitoring. Nonlinear mixed effect modeling was employed to develop a population pharmacokinetic model of oxcarbazepine in Chinese patients with epilepsy to investigate the impact of gender, age, weight, co-medications and genetic polymorphisms of UGT2B7 c.802T > C, ABCC2 c.1249G > A, ABCC 23972C > T on pharmacokinetic characteristics of OXC. The population estimate of apparent clearance (CL/F) and apparent volume of distribution (V/F) was 1.84 L·h−1 and 275 L, respectively. Gender and UGT2B7 c.802T > C affected the clearance rate of MHD significantly. The established model was:CL/F=1.84×0.848UGT2B7×1.17GENDER. Where the genotype of UGT2B7 c.802T > C was CC, UGT2B7=0, otherwise UGT2B7=1. When the patient was male, GENDER=1, otherwise GENDER=0. The final model was evaluated by normalized predictive distribution error (NPDE) and bootstrap method. The model was stable and reliable, which offers a powerful approach for rational use of OXC in epilepsy patients.

奥卡西平(OXC)是常用的新型抗癫痫药物。本研究前瞻性收集了184例癫痫患者的196份常规监测的OXC活性代谢产物10,11-二氢-10-羟基卡马西平(MHD)血药浓度和相关信息。采用非线性混合效应模型法(NONMEM),建立癫痫患者中OXC的群体药动学模型,并考察了性别、年龄、体重、合并用药及与OXC体内代谢相关的基因单核苷酸多态性(UGT2B7 c.802T >CABCC2 c.1249G >AABCC 23972C >T)对MHD药动学的影响。结果显示最终模型的药动学参数的群体典型值为:CL/F=1.84 L·h-1V/F=275 L。性别和UGT2B7 c.802T >C对MHD体内清除率有显著性影响。CL/F的最终模型为:CL/F=1.84×0.848UGT2B7×1.17GENDER。公式中,若患者男性则GENDER=1,若为女性GENDER=0;UGT2B7 c.802T>C若为TT/TC型,则UGT2B7=1,若为CC型,则UGT2B7=0。此外,应用Bootstrap和正态预测分布误差法(NPDE)对建立的最终模型进行验证,结果显示建立的模型稳定、可靠。本研究首次应用NONMEM法考察了代谢酶和转运体的基因多态性对OXC药动学的影响,可为OXC的临床个体化给药提供参考依据。

奥卡西平(OXC)是常用的新型抗癫痫药物。本研究前瞻性收集了184例癫痫患者的196份常规监测的OXC活性代谢产物10,11-二氢-10-羟基卡马西平(MHD)血药浓度和相关信息。采用非线性混合效应模型法(NONMEM),建立癫痫患者中OXC的群体药动学模型,并考察了性别、年龄、体重、合并用药及与OXC体内代谢相关的基因单核苷酸多态性(UGT2B7 c.802T >CABCC2 c.1249G >AABCC 23972C >T)对MHD药动学的影响。结果显示最终模型的药动学参数的群体典型值为:CL/F=1.84 L·h-1V/F=275 L。性别和UGT2B7 c.802T >C对MHD体内清除率有显著性影响。CL/F的最终模型为:CL/F=1.84×0.848UGT2B7×1.17GENDER。公式中,若患者男性则GENDER=1,若为女性GENDER=0;UGT2B7 c.802T>C若为TT/TC型,则UGT2B7=1,若为CC型,则UGT2B7=0。此外,应用Bootstrap和正态预测分布误差法(NPDE)对建立的最终模型进行验证,结果显示建立的模型稳定、可靠。本研究首次应用NONMEM法考察了代谢酶和转运体的基因多态性对OXC药动学的影响,可为OXC的临床个体化给药提供参考依据。

Oxcarbazepine (OXC) is a common antiepileptic drugs. In this study, one hundred and eighty four epilepsy patients with 196 observations of oxcarbazepine's active metabolite, 10, 11-dihydro-10-monohydroxy carbazepine (MHD) were collected prospectively from routine clinical monitoring. Nonlinear mixed effect modeling was employed to develop a population pharmacokinetic model of oxcarbazepine in Chinese patients with epilepsy to investigate the impact of gender, age, weight, co-medications and genetic polymorphisms of UGT2B7 c.802T > C, ABCC2 c.1249G > A, ABCC 23972C > T on pharmacokinetic characteristics of OXC. The population estimate of apparent clearance (CL/F) and apparent volume of distribution (V/F) was 1.84 L·h−1 and 275 L, respectively. Gender and UGT2B7 c.802T > C affected the clearance rate of MHD significantly. The established model was:CL/F=1.84×0.848UGT2B7×1.17GENDER. Where the genotype of UGT2B7 c.802T > C was CC, UGT2B7=0, otherwise UGT2B7=1. When the patient was male, GENDER=1, otherwise GENDER=0. The final model was evaluated by normalized predictive distribution error (NPDE) and bootstrap method. The model was stable and reliable, which offers a powerful approach for rational use of OXC in epilepsy patients.

奥卡西平(oxcarbazepine, OXC)是目前新型抗癫痫药物之一, 为卡马西平10-酮基类衍生物。OXC可阻断电压依赖性钠通道, 还可阻断N型和P型钙通道, 从而抑制突触神经冲动传播, 发挥抗惊厥作用[1]。目前, OXC已成为癫痫全面强直阵挛发作和局灶性发作的一线治疗用药[2]
OXC口服吸收迅速且完全, 在体内经芳香基酮降解酶作用下, 迅速代谢为活性代谢产物10, 11-二氢-10-羟基卡马西平(10, 11-dihydro-10-monohydroxy carbazepine, MHD)。之后, MHD主要经葡萄糖醛酸基转移酶(UDP-glucuronosyltransferase, UGT)代谢, 生成葡糖醛酸化物经肾排出体外[3]。由于OXC不经肝脏CYP450酶代谢, 与其他药物间相互作用较少, 故不良反应发生率较卡马西平降低[4]。OXC的有效治疗浓度为3~35 mg·L-1[5], 需要个体化给药。
有研究表明[6-12], 年龄、体重、性别等因素均可对OXC的体内清除率和表观分布容积产生影响。另有研究结果提示:参与OXC体内代谢的UGT和ATP结合盒转运体C2 (ATP-binding cassette C2, ABCC2)的单核苷酸基因多态性(SNPs)可对OXC维持剂量产生影响[13, 14], 但不同基因多态性对OXC药动学参数的定量分析尚未见报道。为进一步考察OXC在不同患者体内代谢变异的来源, 特别是遗传因素对其药动学的影响, 本研究采用群体药动学(population pharmacokinetics, PPK)方法, 利用临床常规监测数据, 建立OXC群体药动学模型, 获得癫痫患者的群体药动学特征, 为个体化给药提供依据。
患者和样本收集  前瞻性收集了复旦大学附属华山医院神经内科诊治的服用OXC的癫痫患者的资料。入选标准:根据国际癫痫联盟1989年标准诊断为癫痫的患者、男女和年龄不限、接受奥卡西平片口服治疗至少7日以上、肝肾功能正常; 排除标准: ①因不良反应停药患者; ②缺失服药时间、服药剂量、性别、年龄、体重、ABCC2UGT2B7的SNPs或合并用药信息任一项的患者; ③肝肾功能异常的患者; ④依从性不佳的患者。记录患者性别、年龄、体重、合并用药及UGT2B7 (c.802T > C)和ABCC2 (3972C > Tc.1249G > A)的SNPs信息。本研究获得复旦大学附属华山医院伦理委员会的批准, 患者于试验前签署知情同意书。
血药浓度的测定  血药浓度的测定方法采用已验证的高效液相色谱(HPLC)法[15]。色谱条件: Agilent Zorbax RX-C8色谱柱(250 mm × 4.6 mm, 5 µm); 流动相:甲醇-乙腈-0.1%三氟乙酸(200:135:665);流速: 1.5 mL·min-1; 柱温: 40 ℃; 检测波长215 nm。MHD定量的线性范围: 1~50 mg·L-1, 日内和日间的变异均小于 < 15%。
基因分型  留取EDTA抗凝全血1 mL, 采用离心柱法提取外周血基因组DNA。采用Primer Premier (Version 5.0, Premier Corp, Canada)设计引物, ABCC2UGT2B7的3个位点的引物见表 1
采用高分辨率熔解曲线(high resolution melting, HRM)法对ABCC2 c.1249 G > AABCC2 3972C > TUGT2B7 c.802 T > C进行基因型测定。PCR反应体系如下: TaKaRa Premix溶液10 µL, DNA模板1 µL和正反引物各0.5 µL, SYTO-9染料0.6 µL, 加纯水补足至20 µL。根据片段特点调整退火温度, PCR反应条件: 95 ℃预变性10 min, 94 ℃变性20 s, 55~61 ℃退火20 s, 72 ℃延伸20 s, 共40个循环, 72 ℃延伸5 min。熔解曲线温度范围为70~90 ℃。为了确保结果判读的准确性, 每次实验均设突变纯合子对照管、突变杂合子对照管及野生纯合子对照管。通过PCR产物的荧光解链曲线判断基因分型。
数据分析软件  采用非线性混合效应模型软件NONMEM® (Version 7.4, Icon Inc, PA, USA)和gFortran编译器(Version 4.60, http://gcc.gnu.org/fortran/)进行群体药动学分析。采用R软件(Version 3.3.2, http://www.r-project.org)和Xpose (Version 4.5.0, Uppsala University, Sweden, http://xpose.sourceforge.net)进行绘图和可视化评价。建模和参数估算时采用个体间变异与残差变异有交互作用的一级速率条件算法(first order conditional estimation with ƞ-ε interaction, FOCE-I)。
基础模型  根据文献[6-12]报道, 采用一级吸收和消除的一房室模型来描述MHD的体内过程。另外, 由于大部分数据为稳态谷浓度数据, 因此将药动学参数吸收速率常数(Ka)固定为0.598 h-1 [6, 7]
个体间变异模型采用指数模型(式1):

式1中Pi为第i名受试者的PK参数值, TV(P)为该参数的群体典型值, η是呈正态分布、均数为0、方差为ω2的个体间变异。
残差模型分别采用加和型(式2)、比例型(式3)及混合型(式4)对数据进行拟合, 取拟合度最佳者。

式2~4中Y为实测值, F为模型预测值, εn是服从均数为0、方差为σn2正态分布的残差。
协变量筛选  本研究仅采集了谷浓度, 故仅考察了下列因素对CL/F的影响:年龄、性别、体重、UGT2B7 (c.802T > C)和ABCC2 (3972C > Tc.1249G > A)单核苷酸基因多态性以及是否合并使用丙戊酸钠和肝药酶诱导剂(卡马西平、苯妥英钠、苯巴比妥)。NONMEM法计算采用扩展的最小二乘法原理, 用加法、乘法或指数等模型分别引入协变量, 比较协变量纳入前后模型的目标函数值(objective function value, OFV)的差异, 作假设检验和统计学分析。
本研究采用逐步回归法(stepwise regression)筛选协变量, 包括前向纳入(forward inclusion)和逆向剔除(backward elimination)两个过程。前向纳入时, 纳入协变量后的模型与原模型相比, OFV降低大于3.84 (χ2检验, P < 0.05, df = 1)时, 则视该变量为有意义的协变量, 模型中予以保留。逆向剔除中, 减少协变量后的模型与原模型相比, OFV值升高大于6.63时(χ2检验, P < 0.01, df = 1), 协变量在模型中予以保留。协变量筛选时, 还须考虑参数估算值的合理性。
模型评价  首先, 绘制模型拟合优度(goodness- of-fit)的诊断图进行直观评价。拟合优度的诊断图主要包括:群体预测值-实测值(PRED-DV)、个体预测值-实测值(IPRED-DV)、条件权重残差-时间(CWRES-TIME)和条件权重残差-群体预测值(CWRES-PRED)的散点图。其次, 选用Bootstrap法进行模型评价。即从含有n个样本的原数据集中有放回的随机抽取n个样本, 组成一个新的数据集, 然后采用NONMEM求算模型参数值。上述过程重复2 000次后, 计算各参数的中位数、2.5%~97.5%区间, 与原始计算结果值进行统计学比较以评价模型。最后, 采用正态预测分布误差法(normalized predictive distribution error, NPDE)评价模型。其过程如下:基于最终模型和参数估算值, 应用MCS产生2 000套模拟数据集; 用作图和统计学检验考察标准化预测误差的分布。该法评估基于模型的模拟数据集是否可表征真实数据的分布特征。
模型应用  根据最终模型及其参数估算值, 结合MCS法以原数据集为模板, 模拟200次, 绘制不同协变量患者的稳态谷浓度分布的箱型图。
最终纳入癫痫患者184例及其门诊随访血药浓度监测的谷浓度数据196个。人口学和实验室检查资料见表 2ABCC2 3927C > Tc.1249G > AUGT2B7 c.802T > C的基因型测定结果详见表 3, 均符合Hardy- Weinberg平衡(P > 0.05)。
残差模型拟合结果显示, 加和型模型拟合精度优于比例型或混合型模型, 故采用加和型模型。基础模型CL/FV/F的群体典型值分别为1.90 L·h-1和266 L。
前向纳入过程显示, 性别和UGT2B7 c.802T > C基因多态性对奥卡西平的CL/F有显著性影响, (P < 0.05), 逆向剔除过程中上述协变量被保留在最终模型中(P < 0.01)。CL/F的最终模型公式见式5。

式5中, 若患者为男性则GENDER = 1, 若为女性则GENDER = 0; UGT2B7 c.802T > C若为TT/TC型则UGT2B7 = 1, 若为CC型, 则UGT2B7 = 0。最终模型的参数估算结果见表 4
基础模型和最终模型的拟合优度诊断图见图 1。由图可见, 最终模型较基础模型有显著改进。最终模型的PRED和IPRED与观测值数据点在截距为0、斜率为1的直线(即y = x)两侧随机均匀分布; 同时, CWRES对时间和PRED的散点图无明显趋势性分布。
Bootstrap的计算结果参见表 3。Bootstrap计算成功率为93%, bootstrap计算获得的参数和最终模型NONMEM估算值的偏差均 < 10%, 显示模型稳定、参数计算可靠。
NPDE的结果见图 2: QQ图和NPDE分布直方图与标准正态分布接近, NPDE对时间和NPDE对PRED图中, 散点均匀分布在x轴两侧, 无明显偏移。此外, 统计学检验表明NPDE均值为0.075 9, 方差为0.978, t检验、方差检验(Fisher检验)及正态性检验(Shapiro-Wilks检验)的P值均大于0.05, 整体检验P值为0.253, 故可认为NPDE分布与标准正态分布没有显著性差异。最终模型的预测性良好。
根据常用日剂量将患者分为300、600、900、1 200、1 500和1 800 mg共6组, 每组再按照性别和UGT2B7基因型分为4个亚组, 即UGT2B7 TT/TC & 男性组、UGT2B7 TT/TC & 女性组、UGT2B7 CC & 男性组及UGT2B7 CC & 女性组。各组稳态谷浓度箱型图见图 3
目前, 国内外已有OXC的群体药动学研究[6-12], 结果提示性别、年龄、体重、合并用药等因素均可对OXC的药动学参数产生影响。但既往研究均未对遗传因素进行分析。本研究首次通过NONMEM法, 定量考察了UGT2B7 (c.802T > C)和ABCC2 (3972C > Tc.1249 G > A)基因多态性对OXC药动学的影响, 建立了群体药动学模型、估算了群体药动学参数, 可为个体化给药提供依据。
本研究发现了UGT2B7 (c.802 T > C)基因多态性与MHD清除率间存在显著性关系, 携带UGT2B7突变纯合子CC患者的MHD清除率较野生纯合子TT和杂合子TC患者MHD清除率增加17.9%。相对于野生型患者, 携带此等位基因突变的患者对MHD的代谢更快。目前已有关于UGT2B7基因多态性与药物代谢相关的多篇报道[16], 如UGT2B7基因多态性可影响丙戊酸钠[17, 18]、卡马西平[19]、拉莫三嗪[20]等抗癫痫药物的代谢。其原因可能是UGT2B7基因多态性引起的酶亲脂性变化, 从而改变UGT2B7酶的活性, 引起其对底物代谢的改变[21]
本研究结果还提示, 女性患者体内MHD清除率低于男性。这表明OXC在体内的代谢可能存在性别差异。这与Yu等[9]和Peng等[8]的群体药动学研究结果一致。Wang等[22]通过回顾性分析方法考察男女性别间单位剂量血药浓度差异, 发现女性患儿MHD血药浓度显著高于男性患儿。可能的原因: ①与男性相比, 女性脂肪组织含量较多, 肾小球滤过率低, OXC作为脂溶性药物, 清除率可能低于男性; ②动物实验研究表明[23, 24], 受到雄性激素影响, 肝细胞溶质和微粒体内的芳香基酮还原酶活性存在性别差异, 雄性动物显著高于雌性动物。该酶作为人体内将OXC转化为MHD的催化酶, 不同性别的OXC和MHD体内代谢可能存在差异; ③可能存在其他因素引起了男女性别间的体内清除率差异。
此外, 本研究结果表明中国癫痫患者的MHD清除率群体典型值为1.84 L·h-1, 与国内外报道的2.13~2.38 L·h-1[6, 7, 9-10]相近。本研究中MHD表观分布容积群体典型值为275 L, 与文献报道的14.2~177 L[7-10]有一定差异。这可能与不同研究中采用的研究方法、血药浓度数据采集时间以及患者的生理病理差异等因素相关。
本研究为单中心研究, 纳入的样本数有限, 所建模型的外推性有待进一步验证。其次, 本研究仅收集了稳态谷浓度数据且大多数患者仅有一个血药浓度点, 缺少分布相和吸收相的样本, 虽可获得较为可靠的CL/F值, 但对V/F估算效果不佳, 且在建模过程中当考察协变量对V/F影响时, V/F的相对标准误过大, 说明参数估算的可靠性较差。因此, 为获得较为可靠的结果, 本研究中仅考察协变量对CL/F的影响, 下一步研究将在增加样本量的同时, 进一步优化采集方案, 尽可能考察协变量对V/F的影响。
综上所述, 本文首次通过NONMEM法, 考察遗传性因素对OXC代谢产物MHD药动学参数的影响, 结果表明UGT2B7 (c.802 T > C)基因多态性和性别是影响MHD清除率的重要因素, 可为该药的个体化给药方案制定提供依据。
