In this study, we developed a rapid and sensitive ultra high-performance liquid chromatographytandem mass spectrometry (UPLC-MS/MS) method to detect a sulfide bond doxorubicin conjugation prodrug (DOX-S-DOX) in human breast cancer tumor cells (MCF-7). The samples were prepared by acetonitrile precipitation using daunorubicin as internal standard (IS). A reversed phase C18 analytical column (Agilent Eclipse plus C18 RRHD 1.8 μm, 2.1 mm×50 mm) was utilized to separate the samples under gradient elution conditions. Mobile phase was a mixture of 0.1% formic acid in water and methanol at a flow rate of 0.4 mL ·min-1. The analysis was conducted on the mass spectrometer using an electrospray interface (ESI) in the positive ionization model. The calibration range was 20.0-400 ng·mL-1 with the correlation coefficients (r2) ≥ 0.99. The inter-and intra-assay precision (relative standard deviation, RSD%) of quality control samples was within 3.77%-8.35% and relative error (RE%) for accuracy was between -2.04% and 2.62%. Recovery (97.67%-104.2%) and matrix effect (104.8%-113.9%) were consistent, precise, and reproducible at different quality control levels in accordance with FDA guidance. The assay was successfully used in the cellular pharmacokinetics study of DOX-S-DOX, which may provide a clue to explore analytical methods of other prodrug forms of DOX.

本研究采用超高效液相色谱-三重四级杆串联质谱技术,建立在肿瘤细胞中测定单硫键连接的多柔比星前药(DOX-S-DOX)浓度的方法学,并应用于人乳腺癌细胞(MCF-7)的体外药代动力学研究。细胞样品前处理采用乙腈沉淀蛋白的方法。以柔红霉素为内标,采用反相C18色谱柱(Agilent Eclipse plus C18 RRHD 1.8 μm,2.1 mm×50 mm),流动相为甲醇-0.1%甲酸水溶液,以0.4 mL·min-1的流速进行梯度洗脱,每个样品的分析时间为6.0 min,样品经电喷雾离子源正离子化后,在多反应监测模式下测定DOX-S-DOX(m/z 1251.3→459.4)的浓度。线性范围为20.0~400 ng·mL-1r2 ≥ 0.99)。高、中、低三个浓度质控样本的准确度(RE%)在-2.04%~2.62%之间,批内、批间精密度(RSD%)均在3.77%~8.35%以内。基质效应在104.8%~113.9%之间,提取回收率在97.67%~104.2%之间。该分析方法较为简便,其线性范围和灵敏度能够满足DOX-S-DOX(10 μg·mL-1)在MCF-7细胞中的药代动力学研究。

本研究采用超高效液相色谱-三重四级杆串联质谱技术,建立在肿瘤细胞中测定单硫键连接的多柔比星前药(DOX-S-DOX)浓度的方法学,并应用于人乳腺癌细胞(MCF-7)的体外药代动力学研究。细胞样品前处理采用乙腈沉淀蛋白的方法。以柔红霉素为内标,采用反相C18色谱柱(Agilent Eclipse plus C18 RRHD 1.8 μm,2.1 mm×50 mm),流动相为甲醇-0.1%甲酸水溶液,以0.4 mL·min-1的流速进行梯度洗脱,每个样品的分析时间为6.0 min,样品经电喷雾离子源正离子化后,在多反应监测模式下测定DOX-S-DOX(m/z 1251.3→459.4)的浓度。线性范围为20.0~400 ng·mL-1r2 ≥ 0.99)。高、中、低三个浓度质控样本的准确度(RE%)在-2.04%~2.62%之间,批内、批间精密度(RSD%)均在3.77%~8.35%以内。基质效应在104.8%~113.9%之间,提取回收率在97.67%~104.2%之间。该分析方法较为简便,其线性范围和灵敏度能够满足DOX-S-DOX(10 μg·mL-1)在MCF-7细胞中的药代动力学研究。

In this study, we developed a rapid and sensitive ultra high-performance liquid chromatographytandem mass spectrometry (UPLC-MS/MS) method to detect a sulfide bond doxorubicin conjugation prodrug (DOX-S-DOX) in human breast cancer tumor cells (MCF-7). The samples were prepared by acetonitrile precipitation using daunorubicin as internal standard (IS). A reversed phase C18 analytical column (Agilent Eclipse plus C18 RRHD 1.8 μm, 2.1 mm×50 mm) was utilized to separate the samples under gradient elution conditions. Mobile phase was a mixture of 0.1% formic acid in water and methanol at a flow rate of 0.4 mL ·min-1. The analysis was conducted on the mass spectrometer using an electrospray interface (ESI) in the positive ionization model. The calibration range was 20.0-400 ng·mL-1 with the correlation coefficients (r2) ≥ 0.99. The inter-and intra-assay precision (relative standard deviation, RSD%) of quality control samples was within 3.77%-8.35% and relative error (RE%) for accuracy was between -2.04% and 2.62%. Recovery (97.67%-104.2%) and matrix effect (104.8%-113.9%) were consistent, precise, and reproducible at different quality control levels in accordance with FDA guidance. The assay was successfully used in the cellular pharmacokinetics study of DOX-S-DOX, which may provide a clue to explore analytical methods of other prodrug forms of DOX.

多柔比星(doxorubicin, DOX)是从Streptomyces peucerius var. caesius的发酵液中提取的一种糖苷类抗生素, 可抑制RNA和DNA的合成。其抗瘤谱较广, 对各种生长周期的肿瘤细胞均有杀灭作用, 适用于急性白血病(淋巴细胞性和粒细胞性)、恶性淋巴瘤、乳腺癌、支气管肺癌等恶性肿瘤的治疗[1]。然而, 该药具有明显的心脏毒性、剂量依赖性骨髓抑制效应、原发性或继发性耐药等不良反应[2], 限制了该药的临床应用。针对多柔比星存在的不良反应, 作者开发了一系列通过不同连接位点以及连接臂偶联的多柔比星前药纳米粒, 并在前期研究中证明多柔比星的氨基经二硫键共价连接形成酰胺键的多柔比星前药(DOX-S-S-DOX)纳米粒在荷瘤裸鼠体内不但具有明显的增效作用, 而且减轻了多柔比星的不良反应[3], 进一步研究发现多柔比星羟基位点经单硫键连接形成酯键的前药(DOX-S-DOX, 图 1)纳米粒也具有相似作用。但是, 该前药的胞内过程并不明确, 推测其发挥药效是通过溶酶体内的酸性环境分解代谢。为了进一步阐明该类前药胞内的药理学机制和动态规律, 有必要建立一种灵敏的分析方法研究这类前药的细胞药代动力学过程。
近年来, 液相色谱-三重四级杆串联质谱技术(LC-MS/MS)开始应用于定量分析细胞内药物浓度, 其较高的灵敏度, 适宜胞内微量药物的定量研究。根据国内外文献报道, 可以采用固相萃取[4, 5]、液液萃取[6]或沉淀蛋白[7]的方法提取肿瘤细胞内或亚细胞器中的紫杉醇药物, 通过LC-MS/MS技术检测到纳克甚至皮克级别的胞内药物浓度, 从而建立灵敏稳定且特异性强的分析方法, 应用于细胞药代动力学研究。对于多柔比星类药物, 有研究已经涉及到测定细胞或者亚细胞器中多柔比星的浓度[8-10], 但是这些研究并没有对细胞中多柔比星药物浓度分析方法进行完整系统的验证, 方法的特异性和可靠性都需要探讨。作者已建立了测定二硫键连接的多柔比星前药胞内浓度的分析方法[11], 但这种方法对经单硫键共价连接多柔比星前药的普适性还有待验证。
因此, 本研究拟在前期研究方法[11]的基础上, 建立并验证一种灵敏度高, 特异性好的LC-MS/MS方法, 首次测定DOX-S-DOX在细胞内的浓度。同时, 选择一种高效的样本萃取方法, 并将此方法应用于DOX-S-DOX在人乳腺癌细胞(MCF-7)中的药代动力学研究。
仪器  液质联用仪: G6460A三重四极杆质谱仪, Agilent公司; Agilent1290超高压液相色谱仪, Agilent公司; 数据处理系统为Masshunter workstation software, Agilent公司。分析天平: XS 105型, Mettler Toledo公司。
对照品和试剂  多柔比星对照品(北京华奉联博科技有限公司, 含量 > 99.0%); 柔红霉素对照品(Tokyo Chemical Industry, 含量 > 99.0%); DOX-S-DOX (本实验室合成, 含量 > 99.0%); 乙腈和甲醇(HPLC级, Merck KGaA); 甲酸(质谱级, Sigma-Aldrich, 含量 > 98.0%); 水(色谱用超纯水, Milli-Q⁠®纯水仪制备); MCF-7人乳腺癌细胞(北京协和基础医学院); 细胞用1640培养基(北京迈晨生物科技公司); 胎牛血清(Gibco公司)。
色谱条件  色谱柱为Agilent公司Eclipse plus C18反相色谱柱(1.8 μm, 2.1 mm × 50 mm), 柱前保护为Agilent公司Infinity在线过滤器(0.3 μm); 流动相为甲醇(A) -0.1%甲酸水(B)溶液; 流速0.4 mL·min-1; 梯度洗脱程序为: 0~1.5 min, 10%~60% A; 1.5~3.0 min, 60%~80% A; 3.0~4.5 min, 80% A; 4.6~6.0 min, 10% A。分析时间6.0 min, 其中0~1.5 min排废, 1.5~4.5 min进入质谱系统, 4.5~6.0 min排废; 进样量5 μL; 柱温40 ℃; 自动进样室温度4 ℃。洗针液为甲醇-乙腈-异丙醇-水(1:1:1:1), flush port模式洗针15 s。
质谱条件  电喷雾离子源(ESI), 正离子电离模式; 氮气干燥气流速6 L·min-1, 温度250 ℃; 雾化气压力45 psi (1 psi ≈ 6.9 kPa); 鞘气流速10 L·min-1, 温度250 ℃; 毛细管电压3 500 V; 采用多反应监测(MRM)。
空白细胞基质的制备  MCF-7细胞(人乳腺癌细胞)接种在75 cm2的细胞培养瓶中, 在含有5%胎牛血清的1640培养液中连续培养(5% CO2、37 ℃培养箱)至细胞汇合度达到90%。用胰酶消化细胞, PBS洗涤离心, 7.50 mL超纯水重悬细胞(细胞数约为每毫升1×106个), 在液氮与37 ℃水浴条件下反复冻融3个循环, 超声30 min裂解细胞, 得到的空白细胞基质储存在-30 ℃备用。
DOX-S-DOX储备液、标准曲线样本和质控样本的配制  称量DOX-S-DOX 10.0 mg于10 mL量瓶用二甲亚砜定容, 混匀得1.00 mg·mL-1 DOX-S-DOX标准曲线储备液。标准曲线储备液用乙腈-水(1:1)稀释至10.0 μg·mL-1, 随后用细胞基质稀释至浓度为400、320、240、160、120、80.0、40.0和20.0 ng·mL-1标准曲线系列样本。另外称量一份DOX-S-DOX 10.0 mg于10 mL量瓶用二甲亚砜定容, 混匀得1.00 mg·mL-1 DOX-S-DOX质控储备液。质控储备液用乙腈-水(1:1)稀释至10.0 μg·mL-1, 随后用细胞基质稀释至质量浓度为320 ng·mL-1 (HQC)、100 ng·mL-1 (MQC)、40.0 ng·mL-1 (LQC)的质控系列样本(QC)。
内标储备液及工作液  称取柔红霉素对照品10.0 mg于10 mL量瓶用甲醇定容, 混匀得1.00 mg·mL-1内标储备液, 并用乙腈-水(1:1)稀释至200 ng·mL-1内标工作液。
样本提取方法  取样品100 μL置1.5 mL离心管中, 加入内标工作液10 μL (200 ng·mL-1), 混匀后加入乙腈290 μL沉淀蛋白, 涡旋10 min, 于12 000 r·min-1离心10 min, 取上清200 μL, 加入0.3%甲酸水溶液100 μL, 混合均匀, 12 000 r·min-1离心10 min, 取上清100 μL置于进样瓶中, 进样测定DOX-S-DOX浓度。
方法学验证  按照FDA指导原则规定, 对所建立的DOX-S-DOX定量分析方法从特异性、线性范围、定量下限、精密度、准确度、回收率、基质效应和稳定性等方面进行确证[12, 13]
特异性  取6份空白细胞基质, 除不加入内标溶液并加入与内标溶液等体积的乙腈-水(1:1)溶液外, 其余按“样本提取方法”项下操作并进样检测, 记录色谱图; 用空白细胞基质配制DOX-S-DOX浓度为20.0 ng·mL-1的细胞样本, 按“样本提取方法”项下操作并进样检测, 记录色谱图; DOX-S-DOX (10 μg·mL-1)孵育MCF-7细胞0.25 h后获得的细胞样品, 按“样本提取方法”项下操作并进样检测, 记录色谱图。
线性和定量下限  方法确证3天中, 按照“DOX-S-DOX储备液、标准曲线样本和质控样本的配制”项下制备标准曲线样本, 每天制备两条标准曲线, 按“细胞样品提取方法”项进行操作并进样分析。以DOX-S-DOX浓度为横坐标, DOX-S-DOX与内标柔红霉素的峰面积比值为纵坐标, 权重系数为1/x, 采用最小二乘法进行回归运算, 得到的直线回归方程即为标准曲线。定量下限(LLOQ)指同时满足精密度≤20%、准确度在± 20%以内、信噪比≥10的能准确测定的最小浓度。按“DOX-S-DOX储备液、标准曲线样本和质控样本的配制”方法, 配制20.0 ng·mL-1 DOX-S-DOX细胞样品, 平行制备6个样本并测定。根据当日的标准曲线, 计算DOX-S-DOX浓度, 得到定量下限的准确度和精密度。
精密度与准确度  取“DOX-S-DOX储备液、标准曲线样本和质控样本的配制”中制备的高、中、低3个质量浓度(320、100和40.0 ng·mL-1)的质控样品, 每个浓度6个平行样品, 按“样本提取方法”项处理, 连续测定3天, 根据每天的随行标准曲线, 计算样品的实测浓度, 用SPSS软件中单因素方差分析计算DOX-S-DOX准确度(RE%)、批间及批内精密度(RSD%)。
提取回收率  提取回收率通过提取后的QC样品的色谱图峰面积与未经提取加入相应浓度待测物直接进样得到的色谱图峰面积之比进行计算。高、中、低3个浓度的QC样品, 每个浓度6个样本, 经提取后进行分析。同时, 另取空白细胞基质100 μL, 经蛋白沉淀后直接向190 μL上清液中加入DOX-S-DOX标准溶液(133、333和1 067 ng·mL-1)或内标溶液(66.7 ng·mL-1) 10 μL, 涡旋混合, 直接进样分析。通过对两种处理方式得到的相应峰面积的比值, 计算样品的提取回收率。
基质效应  基质效应以含有基质的样品的色谱图峰面积与纯标准溶液获得的色谱图峰面积之比作为指标进行考察。取空白细胞基质100 μL, 按“样本提取方法”项下操作, 然后向提取后的上清液中加入相应浓度的DOX-S-DOX标准溶液和内标溶液, 涡旋混合, 进样分析。同时, 用乙腈-水(1:1)代替细胞基质, 配制相应浓度的DOX-S-DOX和内标混合标准溶液, 不经提取, 直接进样分析。按上述方法配置DOX-S-DOX浓度为320、100和40.0 ng·mL-1的溶液, 每个浓度平行测定6次, 将获得的峰面积取平均值, 计算样品的基质效应。
样品稳定性  细胞样品中DOX-S-DOX的稳定性, 通过配制低、中、高3个浓度的QC样品, 考察细胞样品室温放置6 h、-30 ℃冻存15天、反复冻融(置于-30 ℃冰箱至样品完全冻上, 然后置于室温至完全解冻) 3次稳定性, 细胞样品处理后4 ℃放置24 h (按“细胞样品提取方法”处理后, 在自动进样器4 ℃条件下放置24 h)稳定性。实验测得的浓度与理论浓度比较, 得到稳定性的准确度和精密度。
稀释稳定性  对于超出定量上限的细胞样品, 需要用空白细胞基质稀释到标准曲线的线性范围内, 再进行测定。因此, 需要考察稀释过程是否对细胞样品测定的准确度和精密度产生影响。配制相当于QC样品浓度10倍或20倍(1 000和2 000 ng·mL-1)的DOX-S-DOX细胞样品, 每个浓度6个样本, 用空白细胞裂解液稀释10倍或20倍得到100 ng·mL-1样本, 提取后进样分析, 计算稀释后样品的准确度和精密度。
细胞药代动力学实验  MCF-7细胞以2.5×105个/孔的密度接种于12孔板中, 培养24 h后细胞贴壁生长。吸弃原培养液, 用PBS漂洗后, 每孔加入用无血清培养液稀释的DOX-S-DOX (10 μg·mL-1)溶液1 mL, 置37 ℃孵箱分别孵育0.25、0.5、1、3、6、9和12 h。孵育结束后, 吸弃药液, 用预冷的PBS洗3次, 每孔用300 μL 0.25%胰蛋白酶消化细胞。用1 mL含血清培养液终止消化, 并将细胞吹打成单细胞悬液, 于1 000 r·min-1离心5 min。预冷PBS洗两次后, 用250 μL预冷的超纯水重悬细胞(细胞数约为每毫升1×106个), 置于-30 ℃冰箱待用[4]。测定前, 在液氮与37 ℃水浴条件下反复冻融3个循环, 超声30 min, 充分裂解细胞后, 按照“样本提取方法”制备样品并进样分析。
图 2结果显示, DOX-S-DOX和内标柔红霉素(IS)的保留时间分别为2.92和2.40 min, 出峰位置无明显干扰峰, 说明细胞中的内源性物质不干扰DOX-S-DOX和内标的测定。LLOQ (20.0 ng·mL-1)信噪比大于10, 能够满足测定的需求。
DOX-S-DOX定量分析方法线性范围为20.0~400 ng·mL-1, 标准曲线为y = 0.038 4 x -0.136 9, 回归系数r2值为0.999 2。结果表明, DOX-S-DOX在其线性范围内线性关系良好, 标准曲线上各点准确度及精密度均符合要求(LLOQ≤ 20%, 其余浓度≤15%), 可以满足定量分析的要求, 进行后续细胞药代动力学测定。
高、中、低3个浓度的QC样品, 日内精密度在4.11%~5.45%内, 日间精密度在3.77%~8.35%内, 准确度在-2.04%~2.62%之间, 均在15%以内, 说明该方法在其线性范围内准确、精密。
细胞基质中DOX-S-DOX高、中、低(320、100和40.0 ng·mL-1) 3个浓度的基质效应在104.8%~113.9%之间, 提取回收率在97.67%~104.2%之间; 内标柔红霉素的基质效应和回收率分别为102.8%和99.42%。结果表明, 不同浓度的DOX-S-DOX和内标柔红霉素的提取回收率和基质效应均一致稳定, 重现性好, 说明内源性物质基本不会影响待测物和内标的检测, 并且乙腈沉淀蛋白的方法适宜本方法学分析。
DOX-S-DOX细胞样品室温放置6 h、-30 ℃冻存15天、反复冻融3次(-30 ℃到室温), 细胞样品处理后4 ℃放置24 h测定结果的准确度均在± 15%以内, 说明在这些条件下样本稳定, 满足测定要求。
DOX-S-DOX质量浓度为1 000 ng·mL-1的细胞样品稀释10倍后, 测定结果准确度和精密度分别为6.12%和2.95%; DOX-S-DOX浓度为2 000 ng·mL-1的细胞样品稀释20倍后, 测定结果准确度和精密度分别为0.72%和4.84%。测定结果在线性范围内准确度和精密度都符合要求(±15%以内), 说明细胞样品稀释10倍或20倍再测定, 对测定的结果不会有显著影响。
应用该LC-MS/MS方法进行10 μg·mL-1 DOX-S-DOX孵育MCF-7细胞的细胞药代动力学研究。如图 3所示, 在观测时间内, 细胞中DOX-S-DOX和游离多柔比星(DOX)的浓度随孵育时间延长而增加, 给药0.25 h时DOX-S-DOX质量浓度为21.5 ng·mL-1, DOX质量浓度为12.4 ng·mL-1, 12 h时DOX-S-DOX药物质量浓度达到624.0 ng·mL-1, DOX质量浓度为150.0 ng·mL-1。DOX胞内浓度测定的方法学已在之前研究中进行验证[11]。DOX-S-DOX的胞内浓度均在本研究建立的方法学定量范围之内, 说明该分析方法能够满足细胞药代动力学研究的要求。但是在这个实验中并没有观察到药物浓度下降的现象, 提示该实验的观测时间需要进行合理延长, 以便获得胞内药物外排的信息。
实验中通过单针进样对DOX-S-DOX及内标柔红霉素质谱参数进行了优化。结果发现, DOX-S-DOX在电喷雾正离子模式下易于带电荷, 质子化的[M+Na]+峰为响应最高的母离子峰, MS2检测信号较高, 可以提供较高的灵敏度。DOX-S-DOX监测离子对为m/z 1251.3 [M+Na]+→ 459.4, 碎裂电压为380 V, 碰撞能量为74 eV; 内标柔红霉素监测离子对为m/z 527.9 [M+ H]+→ 321.0, 碎裂电压为165 V, 碰撞能量为27 eV。
待测物的稳定同位素内标是UPLC-MS/MS定量分析方法的理想内标, 但稳定同位素内标难以分离纯化, 价格也较高[14]。本实验选择柔红霉素作为内标[15], 与DOX-S-DOX结构相近。结果显示, 内标和待测物液相色谱出峰时间相近, 提取回收率相似, 离子化效率较为一致, 可以很大程度上消除前处理过程和仪器响应变化产生的影响, 保证了方法的准确性和精密度。
生物样品中含有的内源性物质成分复杂, 经色谱分离后与待测物共流出会对测定方法的重现性和待测物的质谱信号响应产生影响, 即基质效应。基质效应对待测物响应可以产生不同的影响, 分为基质增强和基质抑制。本方法中的DOX-S-DOX高、中、低3个浓度的基质效应均大于100%, 可能是由于细胞基质中未除去的蛋白或磷脂引起的。根据FDA指南, 基质效应并不要求100%, 只需不同浓度待测物的基质效应一致, 稳定并且具有重现性[13]
生物样品中待测物需要经过提取再进入质谱分析。常用的生物样品前处理法有沉淀蛋白法、液-液萃取法和固相萃取法。本研究采用沉淀蛋白法[16]简便、易操作, 虽加入沉淀剂后会稀释样本浓度, 导致待测物信号响应小, 但LLOQ能满足实验需要。
