Objective:

To explore the biological function methods of mesenchymal stem cells(MSCs) for quality analysis.

Methods:

The surface markers of MSCs were detected by flow cytometry. MSCs osteogenic differentiation was induced by ascorbic acid and β-glycerophosphate sodium, etc., followed by Alizarin Red S staining. MSCs adipogenic differentiation was induced by IBMX, Rosiglitazone, etc., followed by Oil Red O staining. MSCs could differentiate into chondrocytes with treatment of ITS and TGFβ3, etc., followed by Alcian Blue staining. Cell co-culture of THP-1-macrophage with MSCs and ELISA assay were applied to detect the effects of MSCs on macrophage polarization. The expression levels of IL-10 and TNF α in the cell co-culture supernatant were detected by ELISA. To observe the effects of MSCs on lymphocyte proliferation, MSCs cultured with PBMCs, which were labeled with CFSE and activated by CD3/CD28, followed by flow cytometry.

Results:

The expression of MSCs surface markers, CD105, CD73, and CD90, was more than 95% respectively, while the expression of CD45, CD34, CD14, CD19, and HLA-DR expression was less than 2%. MSCs osteogenic differentiation assay showed red calcium nodules. Red lipid vacuoles were observed in MSCs adipogenic induction differentiation. Furthermore, MSCs have the differentiation potential to chondrocyte spheroids, and typical cartilage pits were observed. Coculture of MSCs with THP-1 macrophages, an increase in IL-10 expression and downregulate TNFα secretion were observed. MSCs played inhibitory effects on the proliferation of PBMC activated by CD3/CD28, with an inhibition rate of 76.4%.

Conclusions:

This study established some of biological activity detection methods for MSCs, including MSCs surface markers, differentiation abilities, promotion of macrophage polarization, and inhibitory effects on lymphocyte proliferation. It provides a potential application for MSCs products quality control.

目的:

建立间充质干细胞(MSCs)生物学活性检测方法,应用于MSCs产品的质量控制。

方法:

采用流式细胞术检测MSCs表面标志物;采用抗坏血酸、β-甘油磷酸钠等诱导MSCs成骨分化,诱导后用茜素红S染色,检测成骨分化能力;采用IBMX、罗格列酮等诱导MSCs成脂分化,诱导后用油红O染色,检测成脂分化能力;采用ITS、TGF-β3等诱导MSCs成软骨分化,诱导后用阿利新蓝染色,检测成软骨分化能力;采用细胞共培养和ELISA方法检测MSCs对巨噬细胞极化的作用,将THP-1细胞诱导为巨噬细胞后与MSCs共培养,ELISA方法检测共培养上清中IL-10、TNFα表达水平;采用CFSE标记细胞法、CD3/CD28刺激PBMC活化法和细胞共培养方法,用流式细胞术检测MSCs对淋巴细胞增殖抑制的能力。

结果:

间充质干细胞表面标志物CD105、CD73和CD90表达均≥95%,CD45、CD34、CD14、CD19和HLA-DR表达≤2%;MSCs经成骨诱导分化后显微镜下可观察到红色钙结节;MSCs经成脂诱导分化后显微镜下可见红色的脂质空泡,呈典型的脂质分化;MSCs经软骨诱导分化形成软骨细胞球,显微镜下可见典型的软骨陷窝;MSCs与诱导的THP-1巨噬细胞共培养后,MSCs能刺激IL-10表达增多,并下调TNFα分泌;MSCs对CD3/CD28活化后的PBMC的增殖具有抑制作用,抑制率为76.4%。

结论:

本研究建立了MSCs表面标志物、分化能力、促进巨噬细胞极化和淋巴细胞增殖抑制的检测方法,可用于MSCs产品生物学活性检测。

目的:

建立间充质干细胞(MSCs)生物学活性检测方法,应用于MSCs产品的质量控制。

方法:

采用流式细胞术检测MSCs表面标志物;采用抗坏血酸、β-甘油磷酸钠等诱导MSCs成骨分化,诱导后用茜素红S染色,检测成骨分化能力;采用IBMX、罗格列酮等诱导MSCs成脂分化,诱导后用油红O染色,检测成脂分化能力;采用ITS、TGF-β3等诱导MSCs成软骨分化,诱导后用阿利新蓝染色,检测成软骨分化能力;采用细胞共培养和ELISA方法检测MSCs对巨噬细胞极化的作用,将THP-1细胞诱导为巨噬细胞后与MSCs共培养,ELISA方法检测共培养上清中IL-10、TNFα表达水平;采用CFSE标记细胞法、CD3/CD28刺激PBMC活化法和细胞共培养方法,用流式细胞术检测MSCs对淋巴细胞增殖抑制的能力。

结果:

间充质干细胞表面标志物CD105、CD73和CD90表达均≥95%,CD45、CD34、CD14、CD19和HLA-DR表达≤2%;MSCs经成骨诱导分化后显微镜下可观察到红色钙结节;MSCs经成脂诱导分化后显微镜下可见红色的脂质空泡,呈典型的脂质分化;MSCs经软骨诱导分化形成软骨细胞球,显微镜下可见典型的软骨陷窝;MSCs与诱导的THP-1巨噬细胞共培养后,MSCs能刺激IL-10表达增多,并下调TNFα分泌;MSCs对CD3/CD28活化后的PBMC的增殖具有抑制作用,抑制率为76.4%。

结论:

本研究建立了MSCs表面标志物、分化能力、促进巨噬细胞极化和淋巴细胞增殖抑制的检测方法,可用于MSCs产品生物学活性检测。

Objective:

To explore the biological function methods of mesenchymal stem cells(MSCs) for quality analysis.

Methods:

The surface markers of MSCs were detected by flow cytometry. MSCs osteogenic differentiation was induced by ascorbic acid and β-glycerophosphate sodium, etc., followed by Alizarin Red S staining. MSCs adipogenic differentiation was induced by IBMX, Rosiglitazone, etc., followed by Oil Red O staining. MSCs could differentiate into chondrocytes with treatment of ITS and TGFβ3, etc., followed by Alcian Blue staining. Cell co-culture of THP-1-macrophage with MSCs and ELISA assay were applied to detect the effects of MSCs on macrophage polarization. The expression levels of IL-10 and TNF α in the cell co-culture supernatant were detected by ELISA. To observe the effects of MSCs on lymphocyte proliferation, MSCs cultured with PBMCs, which were labeled with CFSE and activated by CD3/CD28, followed by flow cytometry.

Results:

The expression of MSCs surface markers, CD105, CD73, and CD90, was more than 95% respectively, while the expression of CD45, CD34, CD14, CD19, and HLA-DR expression was less than 2%. MSCs osteogenic differentiation assay showed red calcium nodules. Red lipid vacuoles were observed in MSCs adipogenic induction differentiation. Furthermore, MSCs have the differentiation potential to chondrocyte spheroids, and typical cartilage pits were observed. Coculture of MSCs with THP-1 macrophages, an increase in IL-10 expression and downregulate TNFα secretion were observed. MSCs played inhibitory effects on the proliferation of PBMC activated by CD3/CD28, with an inhibition rate of 76.4%.

Conclusions:

This study established some of biological activity detection methods for MSCs, including MSCs surface markers, differentiation abilities, promotion of macrophage polarization, and inhibitory effects on lymphocyte proliferation. It provides a potential application for MSCs products quality control.

间充质干细胞(mesenchymal stem cells,MSCs)是来源于中胚层的多能干细胞,其生物学有效性可归纳为3大类,即诱导分化潜能、免疫调控功能以及组织再生能力[1]。因其独特的生物学活性,多项临床试验表明MSCs可用于改善多种疾病的症状,如阿尔茨海默病、渐冻症、关节炎、心肌梗死、血液系统疾病等[2-3],针对神经、肝脏、肾脏等多个系统相关的重大疾病均做了有益尝试[4],因此保证干细胞产品安全有效、质量可控对于干细胞产品临床应用尤为重要。
国内外已有多种疾病采用干细胞疗法进行临床研究[5],目前,国际已有19款干细胞产品上市,我国首款干细胞治疗药品-艾米迈托赛注射液(睿铂生)已上市。生物学活性方法建立属于临床前研究阶段,该阶段的方法研究与结果对于预测未来临床阶段的治疗效应提供重要的依据,因此MSCs生物学活性是干细胞产品临床转化的重要质量属性[6]。研究表明MSCs可以定向诱导分化为各种组织细胞,包括心肌细胞、骨细胞、肝细胞、脂肪细胞、软骨细胞、上皮细胞及神经细胞等[7],因MSCs的多向分化能力,在组织再生及修复等方面发挥重要作用,例如骨髓来源的MSCs,具有良好的软骨分化能力,临床上用于治疗膝骨性关节炎等相关疾病[8];脂肪来源的间充质干细胞,利用其良好的内皮分化能力,被用于治疗缺血性疾病等研究[9]。MSCs的免疫调控功能是其治疗各种疾病的重要细胞生物学基础,干细胞的免疫调节作用主要是通过直接接触或旁分泌作用产生多种生长因子、黏附分子、趋化因子等,与T细胞、B细胞、NK细胞相互作用[10]、抑制M1型巨噬细胞的活性并诱导巨噬细胞向M2型极化[11]、并调控Th1/Th17/Treg淋巴细胞亚群的功能等[12-13]。基于上述免疫调节功能,干细胞可用于改善心肌梗死后的心脏纤维化和肝纤维化等[14],对类风湿性关节炎、多发性硬化等多种免疫性疾病也具有疗效[15],并在移植后抗宿主疾病方面发挥作用[16]
目前越来越多企业投入干细胞产品的研发,其生产工艺设计与验证、质量研究和质量控制、稳定性研究等研发过程中应遵循药物研发的一般规律[17]。成骨、成脂和成软骨细胞的三系分化可作为不同组织来源MSCs的质量标准之一[18]。而干细胞的免疫调控功能是治疗各种疾病的细胞生物学基础,其调控水平可以预估临床治疗效果。本研究基于干细胞产品相关指导原则,并结合方法学验证的要素,针对MSCs生物学活性的质量属性进行了相应的方法建立,包括干细胞表面标志物检测,成骨、成脂、成软骨分化能力检测,促巨噬细胞极化和抑制淋巴细胞增殖能力检测,初步建立了MSCs产品质量检测基础技术体系,可用于MSCs产品常规生物学活性检测。
TS2R显微镜(Nikon公司),1574-A-GP生物安全柜(Thermo Fisher公司),M5酶标仪(美谷分子仪器(上海)有限公司),N7-00007流式细胞仪(无锡厦泰生物科技有限公司),Sorvall ST8R离心机(Thermo Fisher公司)。
人脂肪MSCs(供试品留样,或者购自赛百慷(上海)生物技术股份有限公司)、人外周血淋巴细胞(PBMC,妙顺(上海)生物科技有限公司)、人单核细胞白血病细胞(THP-1,协和细胞中心)。
无血清干细胞培养基购自赛百慷,胎牛血清(FBS)购自Gibco公司,Trypsin 0.25% EDTA(0.25%胰酶)购自Gibco公司,表面标志物检测试剂盒hMSC Analysis Kit购自义翘神州公司,成骨诱导分化试剂盒、成软骨诱导分化试剂盒、成脂诱导分化试剂盒均购自大连美仑生物科技有限公司,CFSE购自eBioscience公司,CD3/CD28磁珠购自同立海源生物,ELISA检测试剂盒购自联科生物。
取一瓶正常生长状态下的人脂肪MSCs,用胰酶1 mL进行消化,300 g离心5 min,弃上清后加入细胞培养液2 mL重悬细胞,用台盼蓝染色计数。
取一瓶生长状态良好的THP-1细胞,300 g离心5 min,弃上清后加入细胞培养液2 mL重悬细胞,用台盼蓝染色计数,根据实验调整细胞浓度。
将消化后的MSCs调整细胞浓度为1×107~2×107 cells·mL-1,分别加入相应的阳性抗体CD73、CD90和CD105及阴性抗体混染管(CD45、CD34、CD14、CD19、HLA-DR),4 ℃避光孵育30 min,每管用PBS 1 mL洗涤3次后上机检测,实验分别由同一实验员平行上样6次做重复性验证,2名实验员重复进行检测作为中间精密度,使用2批检测试剂作为耐用性验证。应用GraphPad Prism进行数据分析,实验数据以均数±标准差(X̅ ±S)表示,组间比较用独立样本t检验,以α=0.05为检验标准,P<0.05为差异有统计学意义。
实验结果表明干细胞表面标志物CD73、CD90、CD105阳性率均>95%,阴性标志物CD45、CD34、CD14、CD19、HLA-DR阳性率<2%,重复性(n=6)实验中(实验员A)的RSD均<1%、中间精密度(n=12)实验中(2名实验员比较)的RSD均<1%,耐用性(n=4)实验中(2批试剂盒比较)的RSD均<10%,证明实验具有良好的重复性、中间精密度及耐用性,结果见表12
将0.1%明胶2 mL加入到6孔细胞培养板中,均匀包被培养板底面,放置30 min后吸弃明胶并于无菌条件下室温晾干。将消化后的MSCs按每孔2×105 cells接种于明胶包被的培养板中,培养18~24 h,待细胞融合度达60%~70%后加入成骨诱导完全培养基培养(含基础培养175 mL、血清20 mL、抗坏血酸0.4 mL、β-甘油磷酸钠2 mL、地塞米松20 μL,按试剂盒说明进行配制),每隔2~3 d换液,培养2~4周,结束分化后,吸弃上清,每孔用PBS 2 mL清洗2次,吸弃PBS,加入4%多聚甲醛2 mL固定30 min,用PBS 2 mL清洗细胞培养板2次,每孔加入茜素红1 mL染色5~10 min,吸弃染液,用PBS 2 mL清洗3次,每孔加入PBS 1 mL,显微镜下观察。实验设对照组和诱导分化组,每组双复孔。
MSCs成骨诱导分化结果见图1,成骨诱导分化培养基培养4周后,显微镜下观察,对照组未观察到钙结节,实验组发生干细胞分化,茜素红染色后可见红色钙结节,提示干细胞具有一定的成骨分化的能力。
接种MSCs于6孔细胞培养板,每孔2×105 cells,培养3 d至细胞融合度达100%后再继续培养1~3 d,进行成脂分化诱导:成脂诱导培养基分为A液、B液,分别按试剂盒说明书进行配制,B液含基础培养基175 mL、血清20 mL、重组人胰岛素200 μL,A液在B液基础上额外添加IBMX 200 μL、罗格列酮200 μL、地塞米松200 μL。诱导培养期间A液与B液进行3 d/1 d交替培养,交替3~5次后,用B液维持培养直至镜下观察脂滴变大变圆,结束分化后,吸弃上清液,每孔用PBS 2 mL清洗2遍,吸弃PBS,加入4%多聚甲醛2 mL固定30 min,用PBS 2 mL清洗2遍,每孔加入油红O染色液1 mL,染色30 min,吸弃染液,每孔加入PBS 1 mL,显微镜下观察。实验设对照组和诱导分化组,每组双复孔。
MSCs成脂诱导分化结果见图2,显微镜下观察可见实验组生成圆亮脂滴,油红O染色后呈红色,对照组未见脂滴分化,提示干细胞具有向脂肪细胞分化的能力。
将MSCs按每管4×105 cells接种于15 mL离心管中,250 g离心5 min。吸弃上清用培养基重悬细胞沉淀,150 g离心5 min,重复3次洗涤细胞后,每管加入诱导分化培养基(按试剂盒说明书进行配制,含基础培养基194 mL、地塞米松20 μL、抗坏血酸400 μL、丙酮酸钠200 μL、L-脯氨酸200 μL、ITS添加物2 mL、TGF-β3 200 μL)500 μL,于37 ℃,5% CO2条件下培养,每2~3 d换液1次。诱导21 d后,将软骨球用10%福尔马林固定,经石蜡包埋后切片,脱蜡脱水后用阿利新蓝染色后封片,显微镜下观察染色效果。实验设对照组和诱导分化组,每组双复孔。
干细胞软骨分化结果见图3,经专用培养基诱导28 d后,干细胞在离心管底部可形成软骨球(见图3-A),轻弹离心管,可使软骨球悬浮。将该软骨球用福尔马林固定后进行石蜡包埋,切片后经阿利新蓝染色,显微镜下可观察到形成了类软骨陷窝结构(如图3-C红色箭头所示),标志干细胞向软骨细胞的分化。采用MSCs培养基培养的对照组细胞外观呈球状细胞团,但经上述固定染色操作后可见细胞团呈不规则形,且无明显的软骨陷窝结构(图3-B)。
接种THP-1细胞于12孔板,每孔5×105 cells,加入终浓度100 ng·mL-1的PMA诱导THP-1细胞贴壁,诱导处理3 d。取培养上清作为阴性对照(Blank)。加入LPS/MSCs诱导巨噬细胞极化,实验分为2组,LPS处理组:向诱导分化的THP-1细胞中加入终浓度为100 ng·mL-1的LPS,培养24 h;LPS+MSCs处理组:向已贴壁的THP-1细胞中接种MSCs细胞5×105 cells·mL-1共培养,加入终浓度100 ng·mL-1的LPS,培养24 h。24 h后收集各组实验上清液,ELISA法(双抗夹心法,按试剂盒说明书进行实验)检测TNFα和IL-10的含量。
THP-1经PMA诱导3 d后,由悬浮细胞诱导分化为巨噬细胞(图4)。Blank组IL10的表达水平<3.13 pg·mL-1(Elisa试剂盒标准曲线下限),对照组经LPS诱导IL-10的表达水平为37 pg·mL-1;MSCs共培养组IL-10表达水平为122 pg·mL-1,为对照组的3.2倍;Blank组TNFα的表达水平<15.63 pg·mL-1(Elisa试剂盒标准曲线下限),对照组的TNFα表达水平为4 688 pg·mL-1,MSCs与THP-1共培养组TNFα表达水平为1 805 pg·mL-1,表达降低,约为对照组的39%,结果见图5。结果显示MSCs能明显抑制促炎因子TNFα分泌表达(P<0.05),并上调IL-10的分泌表达(P<0.05)。
96孔板接种MSCs细胞1×105 cells·mL-1,37 ℃,5% CO2条件下培养24 h后加入CFSE染色的PBMC每孔3×105 cells,同时加入CD3/CD28单抗偶联磁珠每孔10 μL,于37 ℃,5% CO2条件培养8 d,收获细胞进行流式检测,实验分组见表3,每组2复孔,同时设置Blank组,分别为正常培养状态下的PBMC及MSCs。按照如下公式计算淋巴细胞增殖抑制率,其中NC为阴性对照(无CD3/CD28刺激)的增殖抑制率,PC为阳性对照(CD3/CD28刺激组)的增殖抑制率,S为实验组(MSCs共培养组)的增殖抑制率,CFSE FITC-H-为增殖细胞百分率。
结果显示CD3/CD28可刺激淋巴细胞增殖,MSCs具有一定的抑制淋巴细胞增殖的能力,抑制率约为76.4%(表4图6)。
目前国内外已出台了一系列针对干细胞产品的质量评价标准及规范,对干细胞产品从研发到生产的质量控制提出了建议。干细胞产品作为一种细胞治疗产品,其生物学属性是决定其疗效的关键,建立评价干细胞产品生物学活性的方法为临床研究提供了有力的依据。干细胞生物学活性检测主要从表面标志物表达、分化能力、免疫调控能力等方面进行研究,并建立质量标准。
本研究采用流式细胞术检测干细胞表面标志物并进行了方法学验证,主要考察了方法的特异性、精密度、重复性和耐用性,证明所建立方法可应用于干细胞产品检测。流式细胞术是检测细胞表面标志蛋白表达的经典方法,操作简便、方法稳定,而且商品化的干细胞表面标志物检测试剂盒已比较成熟,易于推广应用。
干细胞分化采用不同的诱导分化试剂并结合染色法检测干细胞的成骨、成软骨和成脂分化能力,但显微镜下观察结果存在主观因素,且只能判断有无分化能力,尚不能量化,有研究用图片软件对进行染色区域面积进行分析,计算干细胞诱导分化率。在此基础上,还可考虑建立其他方法对干细胞分化能力进行评价,如有研究采用QPCR方法对软骨标志物SOX9等进行mRNA水平的检测[19]
干细胞的免疫调控能力可从多个角度进行评价,如干细胞对免疫调节因子分泌能力、对免疫细胞增殖能力及对巨噬细胞极化的作用等。本研究中建立了细胞共培养的方法联合流式细胞术和ELISA技术检测MSCs对总淋巴细胞增殖的作用以及对巨噬细胞极化的影响,可根据临床应用评价MSCs对Th1/Th17细胞亚群的抑制作用及促Treg细胞亚群增殖的能力。此外,有研究采用QPCR法检测MSCs对淋巴细胞中IL-6、CD206等相关因子mRNA表达水平的影响[20],可考虑进一步进行方法学验证评价其是否可作为干细胞质量标准之一。由于干细胞复杂的生物学特性,国际细胞治疗协会(International Society of Cell Therapy)指出对MSCs的免疫功能检测方法的建立应标准化定量化[21],因此建立可用于MSCs生物学活性评价的标准细胞株对干细胞有效性评价具有重要意义[22],可根据干细胞产品特点建立相应的标准细胞株,用于干细胞产品的有效性评价。
本研究中MSCs生物学活性方法均基于相关法规规范,能满足干细胞检定的基本要求。随着干细胞产品新功能新适应症的不断研究与发展,还需不断研发探索新方法新技术在干细胞产品生物学活性检测领域的应用,为干细胞产品的质量控制提供有力技术支撑。
