The present quality control methods of traditional Chinese medicine (TCM), such as morphological identification, microscopic identification, physicochemical analysis, and biological activity testing, cannot fully meet the requirements of TCM quality control. Based on the characteristics of digital PCR technology, a new molecular quantitative analysis technology named Digital Dancao molecular detection technology was established, which used the ratio of specific sequence copy number to internal reference sequence copy number (R value) as the quality control indicator. This technology can quantitatively analyze the adulteration of medicinal herbs, monitor the feeding amount and production process of preparation. It conforms to the clinical characteristics of TCM which using herbal dosage, and can be used for quantitative testing of TCM products from herbs to preparation. Digital Dancao molecular detection technology provides a new solution for the problem of quality monitoring of TCM.

目前,中药的质量控制方法如性状鉴别、显微鉴别、理化分析、生物活性检测等,并不能完全满足中药质量控制的要求。基于数字PCR技术的特点,本文建立以特异序列拷贝数和内参序列拷贝数比值(R值)为质控指标的分子定量检测技术,命名为数字丹草分子检测技术。该技术可对药材掺杂进行定量分析,并可对复方制剂的投料量和生产工艺进行定量监测,符合中药以药味剂量用药的临床特点,能用于从药材到制剂中药全产业链产品的定量检测,为中药质量监控难题提供全新的解决方案。

目前,中药的质量控制方法如性状鉴别、显微鉴别、理化分析、生物活性检测等,并不能完全满足中药质量控制的要求。基于数字PCR技术的特点,本文建立以特异序列拷贝数和内参序列拷贝数比值(R值)为质控指标的分子定量检测技术,命名为数字丹草分子检测技术。该技术可对药材掺杂进行定量分析,并可对复方制剂的投料量和生产工艺进行定量监测,符合中药以药味剂量用药的临床特点,能用于从药材到制剂中药全产业链产品的定量检测,为中药质量监控难题提供全新的解决方案。

The present quality control methods of traditional Chinese medicine (TCM), such as morphological identification, microscopic identification, physicochemical analysis, and biological activity testing, cannot fully meet the requirements of TCM quality control. Based on the characteristics of digital PCR technology, a new molecular quantitative analysis technology named Digital Dancao molecular detection technology was established, which used the ratio of specific sequence copy number to internal reference sequence copy number (R value) as the quality control indicator. This technology can quantitatively analyze the adulteration of medicinal herbs, monitor the feeding amount and production process of preparation. It conforms to the clinical characteristics of TCM which using herbal dosage, and can be used for quantitative testing of TCM products from herbs to preparation. Digital Dancao molecular detection technology provides a new solution for the problem of quality monitoring of TCM.

中药质量是中医药产业高质量健康发展的重要保证,中药的质量控制方法有性状鉴别、显微鉴别、理化检测、分子鉴定和生物活性检测等[1-2],但主要以理化分析为主,如薄层鉴别、含量测定、指纹图谱等。由于不同产地批次药材成分及含量差异较大,通常只是对其中一种或几种化学成分进行检测。中药化学成分复杂,大量的现代研究表明,中药的治疗作用是由多成分、多靶点、多通路共同实现的。现有的理化分析方法虽在一定程度上能反映药物的化学成分情况,但并不能完全满足中药质量控制的要求。中药在临床使用时以药味的剂量为基础,中药制剂的生产也按照各药味的处方量比例,以化学成分为指标的理化质量控制方法,难以对药味剂量进行监控,因此,急需建立一套能符合中药临床用药特点的质量控制方法。
在对药材基原进行准确鉴定的基础上,数字PCR技术可对药材的掺杂量进行定量分析,同时也可对制剂生产的药材投料量和生产工艺进行规范和控制,为中药质量的监控难题提供了新的思路和方法。本文建立适用于药材、饮片、提取物、配方颗粒、中药制剂等质量控制的数字丹草分子检测技术,草为本草,泛指药材,丹为膏丹丸散,泛指中药制剂,数字丹草即采用数字PCR技术对从药材到制剂的中药全产业链产品进行定量的检测。
分子检测技术可对遗传物质DNA进行分析,从而准确鉴定药材的基原,在中药质量控制中得到了广泛应用。如:DNA条形码技术已收入2020年版《中华人民共和国药典》(简称《中国药典》),为中药材的法定鉴定方法[3];川贝母、乌梢蛇、石斛(霍山石斛)、金钱白花蛇、蕲蛇等药材的聚合酶链式反应-限制性内切酶长度多态性(PCR-RFLP)鉴定方法也已被2020年版《中国药典》所收载[4];此外,多重连接探针扩增结合熔解曲线法(MLPA-HRM)[5-6]、随机扩增多态性DNA标记(RAPD)[7]、简单序列重复区间的DNA序列(ISSR)[8]、高分辨率熔解曲线(HRM)[9]、环介导等温扩增(LAMP)[10-11]等方法均应用于药材的鉴定研究中。除药材外,鳖甲[12]、牛膝[13]、金银花[14]等多种中药配方颗粒采用位点特异性PCR技术进行了分子鉴定研究。
聚合酶链式反应(PCR)是上述分子检测技术的核心步骤之一,可对样品中的微量核酸进行大量的扩增,甚至少到1个核酸分子,也可通过扩增得到足够量来满足检测的需要。在普通PCR的基础上,发展了可进行定量分析的第2代荧光定量PCR(qPCR)技术和第3代数字PCR(dPCR)技术。
dPCR技术的原理是将含核酸的PCR反应体系进行极限稀释,通过反应室或微滴生成器分散成数万个微小的独立反应单元,使每个反应单元尽可能仅含有1个或不含靶标分子,通过PCR扩增后,采集每个反应单元的荧光信号,对有无靶标分子进行判断,最终根据泊松分布的原理计算待检测靶标分子的浓度或拷贝数[15-18]
泊松分布是一种关于离散概率分布的统计学分析方法,通常情况下,dPCR的反应单元中可能含有2个或更多的靶标分子,可通过泊松统计方法对数据进行校准。假设dPCR中的反应单元含有起始DNA拷贝数为x,λ为反应单元所含有的平均分子拷贝数。x=kk=0,1,2,3…)的概率分布函数P符合泊松概率模型,数学表达式如下[15]:
基于泊松统计分析,每1 μL样品中DNA拷贝数T的计算公式如下[16]
其中,P为阳性反应单元数目,N为总反应单元数目,Vp为单个反应单元体积,D为模板稀释因子。在统计学中,一个概率样本的置信区间是对这个样本的某个总体参数的区间估计。基于泊松分布的统计分析,通常选择95%的置信区间。
dPCR与传统的PCR技术相比,具有多种优点,如,无须校准物和标准曲线即可实现对靶标基因的绝对定量,可准确定量复杂背景下低微浓度的靶基因;不受PCR扩增效率的影响,PCR抑制剂对结果的影响很小,这些都是第二代荧光定量PCR所难以达到的[19-20]。dPCR近年来在疾病诊断、食品监管、环境等领域得到了许多应用,如转基因成分、突变体、病原菌、癌症的检测等[21-24]。由于dPCR具有高特异性、高灵敏度、高精确度、强耐受性的特点,特别适用于成分复杂、核酸含量低微的中药制剂样品的检测,目前已应用于阿胶、人参、木通等药材的分子检测中[25-27]
筛选能鉴定药材基原的特异DNA序列,用其拷贝数代表样品中所含这味药材DNA的量,假设拷贝数为A;同样筛选所有药材均具有的DNA保守序列(即内参序列),用其拷贝数代表制剂中所有药材DNA的总量,假设拷贝数为BAB的比值RR=A/B)即代表某味药材DNA占总DNA的比例,理论上R值基本固定。数字丹草分子检测技术就是采用dPCR方法检测代表某种药材的特异序列拷贝数A和代表药材总量的内参序列拷贝数B,通过二者比值R,用于中药的质量控制。
当代表药材的特异序列和内参序列是同一个基因的不同部位时,由于都在同一条DNA分子上时,样品中特异序列和内参序列的数量相等,R值理论上应为1:1,即100%;当样品中掺杂有其他基原的混伪药材时,其R值将会降低,从而可有效监控药材的纯度,即掺杂率。对于单一生物基原的产品,如药材、饮片、药材粉末、配方颗粒、提取物、单味制剂等,理论上R值均应为100%,均可采用R值进行质量监测。
采用此技术对人参药材进行检测,结果人参特异序列拷贝数为225 297.6,内参序列拷贝数为201 939.9,R=111.57%;对人参配方颗粒进行检测,结果人参特异序列拷贝数为1 926.6,内参序列拷贝数为1 887.4,R=102.08%,均符合1 : 1的理论值。
药材品种和药用部位等不同,不同药材中DNA的含量并不相同,通过内参序列计算得到的R值,虽不能完全等同于药材的实际掺杂率,但有着明显的关联性,所以R值可有效控制药材的纯度即掺杂率,对药材质量进行监测。
中药复方制剂的处方组成固定,各味药的处方量固定,生产工艺固定,因此,每味药材的DNA占所有药材总DNA的比例也基本固定。虽然提取加工过程中DNA会降解和损失,但制剂生产的提取加工条件是固定的。假设各药材每次生产时DNA的降解率和损耗率基本固定,则制剂中每味药材DNA占总DNA的比例还是基本固定的。所以,同样可以采用dPCR方法检测代表某种药材的特异序列拷贝数A和代表药材总量的内参序列拷贝数B,通过其比值R对中药的质量进行控制。影响R值的因素较多,涉及各味药材中DNA的含量、药材投料量、生产提取时DNA的溶出率、降解率、损耗率、辅料量的加入量、检测方法的准确性等。
药材的质量受多种因素影响,包括其产地、采收季节、种植方法和生长环境,这些因素会导致药材中代谢产物的积累量存在差异。此外,药材的含水量也会影响其DNA含量,进而导致不同批次药材中DNA含量的不一致性。这种不一致性会使得药材在制剂中所占的DNA总量比例出现波动,而这种波动的具体程度需要通过实验数据的积累来确定。它是影响不同批次制剂R值结果变化的主要因素之一,为了降低这种影响,可以采取固定药材的产地、种植方法和产地初加工条件等措施。
为确保不同批次制剂间的一致性,必须精确控制投料量。若药材投料量不准确,或药材中混入了其他非目标基原的伪品,都可能导致R值出现差异。通过确保投料药材的基原纯正和投料量的精确,可以有效地控制这些因素对R值的影响,这也是数字丹草分子检测技术在中药质量监控中的核心原理。
在制剂生产过程中,投料饮片的尺寸、厚度、加水量、提取时间、浓缩和干燥工艺参数,以及所使用的生产设备等,都可能对药材的DNA溶出、降解和损耗产生影响。因此,为了减少不同批次间R值的差异,生产工艺参数和设备应当保持一致,这是制剂生产的基本要求。
在制剂生产过程中,不同批次间辅料的用量存在差异。当使用含有DNA的辅料,如淀粉、蜂蜜等时,它们会影响内参序列的拷贝数。可以通过检测代表辅料的特异序列拷贝数C,并从总的内参序列拷贝数中扣除,再计算比值R,即R=A/(B-C),以排除辅料对结果的影响;对于不含DNA的辅料,其加入量不会影响R值,可以通过单独检测辅料中内参序列的拷贝数来判断其是否对结果产生影响。
本方法建立的前提之一是假设DNA降解时断裂的位置是随机的,从而使得代表药味的DNA片段与内参序列的片段长度的分布一致。然而,实际上DNA序列中GC分布并不均匀,不同位置断裂难易程度也有差异,因此,在内参序列的选择上应尽量与代表药味特异DNA序列的GC含量一致,并保持DNA检测模板的长度一致,以减少差异。同时,尽量缩短检测时所需DNA模板的长度,也能减少误差,这给实验设计提出了更高的要求。
由于检测对象是样品中的DNA,不同的DNA提取方法对不同长度DNA片段的提取效果存在差异,应考察提取方法,尽可能将样品中DNA提取完全,以减少差异。
dPCR检测时的测定方式、操作人员、仪器性能等因素都可能带来测定结果的误差。如,采用染料法时,代表药味的特异序列拷贝数和内参序列拷贝数是分别测定的,其加样量带来的误差较大。而探针法可采用双重PCR方式,排除加样量的差异。dPCR检测方法建立后,可进行方法学验证,对检测方法的准确性、可靠性等项目进行考察,以探讨检测方法对结果准确性的影响。
总体来说,当检测方法和条件优选确定后,影响R值的将主要是药材和制剂的生产过程。通过固定药材基原和产地,规范化种植和加工,标准化的制剂生产过程来减少对R值的影响,这也是中药标准化生产的目标,可采用数字丹草分子检测技术进行监测,以确保中药质量的稳定性。
数字丹草分子检测技术通过内参序列检测计算得到的比值,虽然不代表药材的实际投料量和真实含量,但作为一个质量控制指标,其数值相对固定,与药材投料量和生产工艺紧密相关,适用于中药制剂等加工产品的质量控制。对于单一药味产品,如药材、饮片、配方颗粒等,R值可以代表产品的纯度,有效监控药材掺杂、掺假,具有很高的实用性。对于中药复方制剂,R值同样可以作为质量控制的检测指标,确保制剂药材投料量的准确性和生产工艺的规范性。
对于每一种药材,数字丹草分子检测技术测定的是代表其基原的特异DNA序列,从而能够对制剂中的投料药材进行精确的定性鉴定。
dPCR检测方法的灵敏度高,仪器最低检测限一般为每孔3个拷贝数。推荐的检测序列采用核糖体RNA基因,该基因在基因组中的拷贝数为500~40 000个,适合检测中药制剂这类含有微量DNA的样品。
qPCR的检测模板需包含正向引物、反向引物和探针序列,长度需在60 bp以上,但通过特定设计,可以不需要反向引物,并将检测模板缩短至35 bp。
dPCR在数万个反应单元中独立进行扩增,每个反应单元通过阈值判断有无,排除扩增效率不同带来的差异,减少了背景和抑制剂对PCR反应的干扰,有效排除药物成分对检测的影响,适用于成分复杂的中药样品检测。
目前,dPCR仪器基本上是全自动控制,易于操作,DNA的提取也可以通过仪器自动完成。数字丹草分子检测技术虽在方法开发上存在一定难度,但在日常检测分析中,熟悉PCR操作的普通技术人员都能够胜任。
数字丹草分子检测技术不仅适用于中药制剂的质量控制,还适用于药材、饮片、配方颗粒、提取物等,覆盖了除矿物药外的中药全产业链产品的质量控制,同样,该技术也可用于食品、保健食品、饮料等的质量监控。
数字丹草分子检测技术主要通过R值结果进行质量控制。但当某味药所占比例过低,或者辅料种类多或量大等多种原因导致R值检测结果误差太大时,也可不采用内参,而是通过药味之间的R值比来进行质量控制[即RX/RYX(药味拷贝数)/Y(药味拷贝数)],从而扩展质量控制指标。
数字丹草分子检测技术建立在对药材DNA序列拷贝数准确测定的基础上,首先要获得能代表药材基原的特异序列,作者虽已获得60种左右常见中药的特异序列,并申报了专利,但中药材品种众多,还有大量的药用植物基因序列未进行研究。
dPCR检测仪器经过10多年的发展,已经相对成熟。以北京新羿生物科技有限公司等国产品牌为例,其仪器的技术指标、性能与进口设备基本持平,但价格依然较高,分析试剂成本也相对较高。因此,需要进一步降低分析检测成本,以便将该技术作为一种通用方法推广应用。
对于复方制剂的质量控制,由于R值受多种因素影响,所以需要系统性地考察各种影响因素,了解其影响程度,同时,中药剂型众多,生产工艺路线也各不相同,这些也需要逐一研究,以明确数字丹草分子检测技术的适用范围。
数字丹草分子检测技术也存在一些局限性,例如,无法检测不含核酸的矿物药,也无法区分来自同一植物不同药用部位的药材。尽管数字丹草分子检测技术符合中药以药味剂量使用的临床特点,但它并不能取代现有的理化分析方法。作为一种全新的中药质量控制方法,还有许多需要完善和改进的地方。将数字丹草分子检测技术与现有的理化分析方法相结合,在很大程度上可以有效解决中药质量监控的难题,促进中医药事业的健康发展。
