Objective: To characterize the primary structure of the active national standard candidate of infliximab to provide a technical basis for this monoclonal antibody. Methods: The complete relative molecular mass and the relative molecular masses of the light and heavy chain subunits of the active national standard candidate of infliximab were detected based on ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS). The relative molecular masses of the light and heavy chain subunits of the active national standard candidate of infliximab were analyzed by UPLC-MS/MS. UPLC-MS/MS was used to analyze the sequence coverage,glycosylation sites and disulfide bond localization of infliximab activity candidate national standard products. Results: The complete relative molecular mass of infliximab was 148 515. The relative masses of its light and heavy chain subunits were 23 435 and 50 827 respectively. The sequence coverage of infliximab was 100% for light chain and 100% for heavy chain. The glycosylation sites of infliximab were analyzed by UPLC-MS/MS; and the disulfide bonds were localized. The glycosylation site of the monoclonal antibody candidate was located at N300 of the heavy chain; there were 16 pairs of disulfide bonds, 4 pairs within each of the two heavy chains, 2 pairs between chains,2 pairs within each of the 2 light chains, and 1 pair between each of the two groups of light and heavy chains. Conclusion: The primary structural confirmation of the national standard candidate of infliximab monoclonal antibody activity provides a technical guarantee for the establishment of the monoclonal antibody standard substance, the standardization of analytical methods and the control of product quality.

目的:对英夫利西单抗活性候选国家标准品进行一级结构表征,为该单抗的后续研究提供技术依据。方法:基于超高效液相色谱-串联质谱(ultra-performance liquid chromatography-tandem mass spectrometry,UPLC-MS)对英夫利西单抗活性候选国家标准品的完整相对分子质量、轻重链亚基相对分子质量进行检测;使用UPLC-MS/MS对英夫利西单抗活性候选国家标准品的序列覆盖率、糖基化位点及二硫键进行分析。结果:英夫利西单抗活性候选国家标准品的完整相对分子质量为148 515;轻链相对分子质量23 435、重链相对分子质量50 827;英夫利西单抗活性候选国家标准品轻链序列覆盖率100%、重链序列覆盖率100%;该单抗活性候选标准品糖基化位点位于重链N300;共有16对二硫键,2条重链链内各4对,链间2对,2条轻链链内各2对,2组轻重链间各1对。结论:对英夫利西单抗活性候选国家标准品进行一级结构确证,为后续该单抗标准物质的建立和分析方法的标准化及产品质量控制提供技术保障。

目的:对英夫利西单抗活性候选国家标准品进行一级结构表征,为该单抗的后续研究提供技术依据。方法:基于超高效液相色谱-串联质谱(ultra-performance liquid chromatography-tandem mass spectrometry,UPLC-MS)对英夫利西单抗活性候选国家标准品的完整相对分子质量、轻重链亚基相对分子质量进行检测;使用UPLC-MS/MS对英夫利西单抗活性候选国家标准品的序列覆盖率、糖基化位点及二硫键进行分析。结果:英夫利西单抗活性候选国家标准品的完整相对分子质量为148 515;轻链相对分子质量23 435、重链相对分子质量50 827;英夫利西单抗活性候选国家标准品轻链序列覆盖率100%、重链序列覆盖率100%;该单抗活性候选标准品糖基化位点位于重链N300;共有16对二硫键,2条重链链内各4对,链间2对,2条轻链链内各2对,2组轻重链间各1对。结论:对英夫利西单抗活性候选国家标准品进行一级结构确证,为后续该单抗标准物质的建立和分析方法的标准化及产品质量控制提供技术保障。

Objective: To characterize the primary structure of the active national standard candidate of infliximab to provide a technical basis for this monoclonal antibody. Methods: The complete relative molecular mass and the relative molecular masses of the light and heavy chain subunits of the active national standard candidate of infliximab were detected based on ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS). The relative molecular masses of the light and heavy chain subunits of the active national standard candidate of infliximab were analyzed by UPLC-MS/MS. UPLC-MS/MS was used to analyze the sequence coverage,glycosylation sites and disulfide bond localization of infliximab activity candidate national standard products. Results: The complete relative molecular mass of infliximab was 148 515. The relative masses of its light and heavy chain subunits were 23 435 and 50 827 respectively. The sequence coverage of infliximab was 100% for light chain and 100% for heavy chain. The glycosylation sites of infliximab were analyzed by UPLC-MS/MS; and the disulfide bonds were localized. The glycosylation site of the monoclonal antibody candidate was located at N300 of the heavy chain; there were 16 pairs of disulfide bonds, 4 pairs within each of the two heavy chains, 2 pairs between chains,2 pairs within each of the 2 light chains, and 1 pair between each of the two groups of light and heavy chains. Conclusion: The primary structural confirmation of the national standard candidate of infliximab monoclonal antibody activity provides a technical guarantee for the establishment of the monoclonal antibody standard substance, the standardization of analytical methods and the control of product quality.

自1986年首个鼠源性单克隆抗体问世以来,单克隆抗体药物已经发展到了第4代[1]。由于其对特定靶点的高度选择性和亲和力,能够与特定的分子或细胞结合,阻断疾病发展的关键信号通路,因此在肿瘤、感染病、自身免疫性疾病、器官性移植等多个领域发挥有效治疗作用[2]。截至2024年5月,全球约有140种单克隆抗体药物获美国食品药品监督管理局(Food and Drug Administration,FDA)批准,并且在2023年全球药品销售TOP 20产品中抗体类药物有8种,且销售额占据全球医药市场的约50%;与此同时,2023年中国药品销售TOP 25榜上,抗体类药物有9种,抗体占全国销售额40%[3],因此治疗性单克隆抗体药物是国内外研发的热点之一。
英夫利西单抗是一种人鼠嵌合型免疫球蛋白G(IgG)类单克隆抗体,具有高亲和力和特异性结合肿瘤坏死因子-α(TNF-α)的能力[4],通过与TNF-α分子结合,英夫利西单抗能够阻断TNF-α的活性,并且能够有效中和其对目标细胞的作用[5-6]。TNF-α是由免疫系统所产生的一类炎症性的细胞调节因子,通过与其TNF受体(TNFR)结合,可以促进炎症细胞间的活化和迁移,增加炎症介质的释放[7]。实验显示,当机体长期暴露于高水平的TNF-α或者TNF-α在体内过度表达会导致多种炎症性疾病的发生,因此TNF-α被认为是参与自身免疫性疾病的主要炎症细胞因子以及重要药物靶点[6-7]。目前已广泛用于治疗类风湿性关节炎(rheumatoid arthritis,RA)、强直性脊柱炎(ankylosing spondylitis,AS)、银屑病性关节炎(psoriaticarthritis,PsA)、溃疡性结肠炎(ulcerative colitis,UC)、成人和儿童克罗恩病(Crohn’s disease,CD)等疾病[8]
欧洲药品管理局(EMA)和国际协调会议(ICH)Q6B以及《中华人民共和国药典》中有关生物制品的指导原则指出了生物制品制备过程中对其质量特性分析的重要性,包括组分测定、物理性质和一级结构等测定[9]。国家标准品的建立为药企、监督机构提供标准,而一级结构的表征可以使国家标准品获得更全面、更准确的信息[10]。因此本研究目的对英夫利西单抗活性候选国家标准品进行结构确证,为该国家标准品的研制提供技术依据。
英夫利西单抗候选国家标准品(批号330008)为中国食品药品检定研究院(中检院)自制;碳酸氢铵(NH4HCO3,纯度≤100%,LC-MS级,Merck公司)、乙腈(ACN,纯度≥99.9%,LC-MS级,Fisher Scientific公司)、氨水(纯度≥25%,LC-MS级,Fluka公司)、超纯水(18.2 MΩ×cm,25 ℃,Millipore公司)、二硫苏糖醇(DTT,纯度≥99.0%,上海博光生物科技有限公司)、碘乙酸钠(MIA,纯度≥98%,Sigma公司)、乙二胺四乙酸(纯度≥99.5%,AR,国药集团化学试剂有限公司)、甲酸铵(NH4FA,纯度≥99.0%,LC-MS级,Merck公司)、甲酸(FA,纯度≥99.0%,LC-MS级,Thermo公司)、胰蛋白酶(Trypsin酶,16.5 mg · mL-1,上海迈泰君奥生物技术有限公司)、组氨酸(上海协和氨基酸有限公司)、盐酸胍(AR级,纯度≥99.0%,国药集团化学试剂有限公司)。
ACQUITY BioAccord LC-MS超高效液相色谱仪(配备电喷雾电离源,ESI源),液相色谱分析色谱柱为ACQUITY UPLC Protein BEH C4(50 mm×2.1 mm,1.7 μm),XEVO G2-S Q-TOF高分辨质谱仪(配备ESI源),质量肽图分析色谱柱为ACQUITY UPLC Peptide BEH C18(100 mm×2.1 mm,1.7 μm),均购自Waters公司。AKTA avant蛋白纯化仪(avant 150,GE公司)、超滤离心管(10 KD/30 KD/50 KD,Millipore公司)、G25脱盐柱(5 mL,GE Healthcare公司)。
使用移液器将待处理样品转移至10 KD超滤离心管中,并使用50 mmol · L-1 NH4HCO3溶液(pH 8.0)置换3~6次,保证置换后样品浓度在5 mg · mL-1以上。将置换后的样品用50 mmol · L-1 NH4HCO3(pH 8.0)溶液稀释至1 μg · μL-1左右;室温下样品经过17 000 g,离心10 min,小心吸取上清至清洁的进样小瓶中等待上机检测。色谱柱:ACQUITY UPLC Protein BEH C4(50 mm×2.1 mm,1.7 μm);流动相A1:含0.1%FA的水溶液;流动相A2:超纯水;流动相B1:含0.1%FA的 ACN溶液;流动相B2:ACN;流速:0.4 mL · min-1;主洗脱梯度为0~12 min,10% B1→95% B1;紫外检测波长:280 nm;柱温:80 ℃;进样体积:10 μL。质谱参数:正离子全扫描Full scan;扫描范围:m/z 400~7 000。使用UNIFI软件(1.9版本,Waters公司)对采集的数据进行解卷积处理,添加N端焦谷氨酸环化、N-糖基化、末端赖氨酸截除等翻译后修饰后使用理论分子量和实测分子量进行匹配,分子量允许误差范围为理论分子量±50。

取检测样品(浓度>5 mg · mL-1)500 μg,加入8 mol · L-1盐酸胍375 μL以及1 mol · L-1 DTT,使DTT终浓度为15 mmol · L-1,补充pH 6.0的甲酸铵(NH4FA)缓冲液至样品总体积为500 μL,37 ℃孵育1 h;加入碘乙酸钠,始终浓度为50 mmol · L-1,混匀,室温避光孵育45 min;加入1 mol · L-1 DTT,使终浓度为35 mmol · L-1以终止烷基化,混匀后进行脱盐;将AKTA avant蛋白纯化仪上设置Alam system puressure High和Alam pre column pressure High为0.5 MPa,Delta C为0.3 MPa,并使用超纯水75 mL冲洗系统,在选中柱位上安装5 mL G25预装脱盐柱,5 mL · min-1流速超纯水冲洗色谱柱5~10 min,将管路放入50 mmol · L-1 NH4HCO3溶液(pH 8.0)中,待电导缓慢升起走平后接500 μL上样环,用1 mL注射器吸取缓冲液连续冲洗3次,然后用注射器吸取样品并打入上样环,Inject模块由injection valve改为inject,同时流速降为2 mL · min-1并且紫外归零,收集280 nm紫外吸收峰,收峰阈值设定为100 mAU,样品最终脱盐至pH 8.0的50 mmol · L-1 NH4HCO3缓冲液中,检测脱盐后的样品浓度,应为0.2~0.4 mg · mL-1(如低于此区间浓度进行浓缩处理);取脱盐后样品200 μL,按样品-酶(10 ∶ 1,w/w)加入Trypsin酶。37 ℃水浴2~4 h;向酶切后的样品中加入甲酸,使终浓度0.5%~1.0%,终止酶切反应。室温下样品经过17 000 g,离心10 min,吸取上清。色谱柱:ACQUITY UPLC Peptide BEH C18(100 mm×2.1 mm,1.7 μm);流动相A:含0.1%FA水溶液;流动相B:含0.1%FA 的ACN溶液;流速:0.2 mL· min-1;主洗脱梯度为2~82 min,1% B→37% B。紫外检测波长:214 nm;柱温45 ℃;进样体积10 μL。质谱参数:采集模式:ESI+、Resolution、MSe;扫描范围:m/z 50~1 990;毛细管电压:3.00 kV;锥孔电压:40 V;偏置电压:80 V;源温度:120 ℃;脱溶剂温度:450 ℃;锥孔气流速:100 L · h-1;脱溶剂气流速:800 L· h-1。使用UNIFI软件(1.9版本)对采集的数据进行解卷积处理,添加烷基化、N端焦谷氨酸环化、N-糖基化、末端赖氨酸截除、氧化等翻译后修饰后对其相对分子质量进行匹配,一级质谱分子量允差范围为±30,二级碎片允差范围为±10,b/y离子碎片数量≥5。
取待检测样品(浓度大于5 mg · mL-1,若小于则进行浓缩)500 μg,依次加入8 mol · L-1盐酸胍375 μL,补充甲酸铵缓冲液(pH 6.0)至总体积为491.3 μL,37 ℃孵育1 h;加入2.9 mol · L-1的碘乙酸钠溶液8.70 μL,避光孵育45 min。将AKTA avant蛋白纯化仪上设置Alam system puressure High和Alam pre column pressure High为0.5 MPa,Delta C为0.3 MPa,并使用75 mL超纯水冲洗系统,在选中柱位上安装5 mL G25预装脱盐柱,5 mL · min-1流速超纯水冲洗色谱柱5~10 min,将管路放入50 mmol · L-1 NH4HCO3溶液(pH 8.0)中,待电导缓慢升起走平后接500 μL上样环,用1 mL注射器吸取缓冲液连续冲洗3次,然后用注射器吸取样品并打入上样环,Inject模块由injection valve改为inject,同时流速降为2 mL · min-1,紫外归零,收集280 nm紫外吸收峰,收峰阈值设定为100 mAU,样品最终脱盐至pH 8.0的50 mmol · L-1 NH4HCO3缓冲液中,检测脱盐后的样品浓度,应为0.2~0.4 mg · mL-1(如低于此区间浓度进行浓缩处理);取200 μL脱盐后样品,按样品-酶(10 ∶ 1 w/w),加入Trypsin酶。37 ℃水浴孵育2~4 h,向酶切后的样品中加入甲酸,使终浓度0.5%~1.0%,终止酶切反应;室温下样品经过17 000 g,10 min离心后,吸取上清。色谱柱:Acquity BEH C18(1.7 μm,2.1×100 mm);流动相A:含0.1%FA的水溶液;流动相B:含0.1%FA的ACN溶液;流速:0.2 mL · min-1;洗脱梯度为2~82 min,1%B1→37%B1。检测波长:214 nm;柱温:45 ℃;进样体积:10 μL。质谱参数:采集模式:ESI+、Resolution、MSe;扫描范围:m/z 50~1 990;毛细管电压:3.00 kV;锥孔电压:40 V;偏置电压:80 V;源温度:120 ℃;脱溶剂温度:450 ℃;锥孔气流速:100 L · h-1;脱溶剂气流速:800 L · h-1。使用UNIFI软件(1.9版本)对采集的数据进行解卷积处理,分析模式调整为二硫键匹配,一级质谱分子量允差范围为±30,二级碎片允差范围为±10,b/y离子碎片数量≥5。
取未脱糖的标准品采用LC-MS进行完整蛋白分子质量检测,检测数据通过使用Waters公司的UNIFI软件进行分析,并且根据去卷积图谱进行分子量确认。结果显示(表1图1),英夫利西单抗候选标准品的实测分子质量为148 515.16,理论值为148 511.79,相对误差22.7,在仪器误差允许范围内,实测分子量与理论分子量相符合基本一致。此外,在完整蛋白水平上检测出其主要修饰组分为:C末端赖氨酸截除(-K)及G0F、G1F、G2F等糖基化修饰为主。
亚基水平分析结果显示(表2图2),轻、重链主峰分子量均与理论分子量相符(重链修饰以C端-K,G0F糖型为主)。对不同修饰质谱峰强度进行归一化计算,结果显示重链翻译后修饰(表3)的C端赖氨酸切除占比89.32%,此外还检测到少量非糖基化重链(NGHC)的存在。
肽图分析在单克隆抗体检验中作为关键质量属性分析方法之一,能在一次分析中同时获得一级序列覆盖率、二硫键位置、翻译后修饰等信息,形成特征性指纹图谱,在结构确证与特征性鉴别中发挥重要作用。
通过变性、还原、烷基化、酶解等反应,按以上“1.2.3”项方法进行检测,得到轻、重链一级结构覆盖率。分析结果如图3所示,候选标准品的轻、重链序列覆盖率均为100%;该结果显示英夫利西单抗活性候选国家标准品氨基酸序列与理论氨基酸序列一致。
互补决定区(complementarity-determining region,CDR)是抗体分子中可变区(V区)的一部分,作为抗原结合的核心功能域,主要负责与抗原的结合。这些区域的氨基酸序列高度变异,使得每种抗体能够识别不同的抗原,同时也决定了抗体的特异性。每个抗体通常有3对H(重链)和L(轻链)互补决定区,分别称为CDR1、CDR2和CDR3。
本研究在质量肽图谱水平对该候选物的CDR区进行了分析(表4图4表5图5),结果显示英夫利西单抗候选国家标准品CDR区序列与理论序列相一致,该结果也为后续功能验证提供了可靠的分子基础。
翻译后修饰(PTMs)会对抗体药物的结构与功能产生重要影响,原始质量肽图数据通过UNIFI软件分析、使用峰强度归一化进行计算,结果显示部分肽段产生氧化和脱酰胺翻译后修饰,其种类和相对含量结果汇总如表67,甲硫氨酸(M)氧化修饰分别在轻/重链中均能检测到;脱酰胺反应发生在重链天冬酰胺和谷氨酰胺残基上。此外在重链N端检测到0.8%的焦谷氨酸环化修饰(表8)。
通过对比PNGase F 酶切与非酶切肽图结果显示,英夫利西单抗活性候选标准品的N-糖基化发生在肽段NXS/T(X为非脯氨酸的其他任意氨基酸)保守位点,糖基化修饰肽段为EEQYNSTYR(图6),糖基化发生在该肽段的天冬酰胺上。对该肽段使用糖苷酶F进行酶切处理,并对比酶切前后分子量之间的差异,结果显示,该肽段酶切前后具有1 444的分子量差异,与抗体中含量最高的糖型G0F糖型的分子量相匹配,再次证明了该肽段中具有糖基化修饰。
氨基酸序列是蛋白质的一级结构的基础,《中华人民共和国药典》的质量控制中也指出单克隆抗体N末端序列验证的重要性。在经酶解之后,按上述“1.2.3”项方法进行还原,样品经过LC-MS/MS检测后,结果显示(图7),轻、重链N端及C端均与理论序列相一致。得到验证。
取英夫利西单抗国家标准品经Trypsin酶酶解后,进行非还原肽图检测,结果显示共有16对二硫键(表9),2条重链链内各4对,重链链间2对,2条轻链链内各2对,2组轻重链间各1对,分析得到的标准品二硫键符合IgG1抗体理论二硫键。
注射用英夫利西单抗上市多年以来,在治疗自身免疫性疾病领域发挥了重要作用[11-12]。WHO在《生物参考标准品的制备,表征和建立》 (技术报告932号)技术报告中提到,开发和建立国际标准品在确保全球使用生物医药产品的质量和剂量一致性方面发挥了重要作用[13-14]。目前我国积极开展英夫利西单抗活性国家标准品的研制及表征工作。
在英夫利西单抗活性候选标准品表征中,一级结构的分析能确保序列准确性。本研究采用高分辨率LC-MS对其完整相对分子质量、轻重链检测,得到理论分子量与实际分子量相符合,未检测到氨基酸变异位点,证实此候选标准品正确表达,为后续结构与功能研究奠定基础[15-16]。肽图分析得到序列覆盖率、二硫键位置、糖基化位点等结构信息,其中二硫键位点鉴定中,是采用Trypsin酶对候选标准品酶切,通过质量肽图谱来确证二硫键的位置,对应位点的肽段能被检测到。二硫键的确证有效保证抗体分子的正确折叠以及正确三级结构的形成。
单抗药物作为1种生物大分子糖蛋白,本身具有复杂的糖链结构[17-19],糖基化修饰分析结果显示,主要糖型与已知候选标准品特征糖型相符,包括特定的岩藻糖基化、半乳糖基化和唾液酸化等。糖基化的差异能导致药物在体内的药效发生改变,例如不同的岩藻糖基化水平与抗体依赖的细胞介导的细胞毒(ADCC)活性密切相关[20]。糖基化的有效控制是确保英夫利西单抗Fc效应功能的重要手段。
英国国家生物制品检定所(NIBSC)于2019年研制了首批国际英夫利西标准品[21],欧洲药品质量管理局(EDQM)也研制了相应单抗的理化标准品和活性标准品[22]。在此基础上,我国也急需研制相应的英夫利西单抗活性标准品为药企、研发、监管等机构提供质量控制支撑[23]。本研究通过对英夫利西单抗活性候选国家标准品的完整相对分子质量、轻重链亚基相对分子质量、序列覆盖率、糖基化位点等进行分析来表征一级结构,为英夫利西单抗活性标准品的建立提供技术依据,为后续其他单抗品种的国家标准品研制提供可借鉴的思路。
