Objective: To establish a dual derivatization method combined with tandem mass spectrometry for the simultaneous determination of 18 different steroid hormones in human serum. Methods: Serum samples were treated with hydroxylamine and 1, 2-dimethylimidazole-5-sulfonyl chloride for derivatization, and the resulting compounds were analyzed by liquid chromatography-tandem mass spectrometry in positive ion selected reaction monitoring (SRM) mode. The Kinetex®C8 column (100 mm×2.1 mm, 2.6 μm) was used for the separation. Mobile phase A (0.1% acetic acid in water) and mobile phase B (0.1% acetic acid in methanol) with gradient elution(0-0.5 min, 35% B; 0.5-5 min, 35% B→100% B; 5.0-5.1 min, 100% B→35% B; 5.1-7 min, 35% B)at the flow rate of 0.4 mL·min-1 were applied. Column temperature was set at 40 ℃. Injection volume was 20 μL and collection time was 6 min. Results: The linearity correlation coefficients for all 18 steroid hormones were greater than 0.99, the recovery rates ranged from 85% to 115%, and the precision RSD was less than 15%. This method was successfully applied to the analysis of serum samples from 156 healthy subjects (75 males and 61 females), and reference intervals were established. Conclusion: This method can be used to simultaneously determine 18 types of steroid hormones, such as pregnenolone, 17-hydroxypregnenolone, progesterone, 17-hydroxyprogesterone, corticosterone, cortisol, 11-deoxycorticosterone, 21-deoxycorticosterone, aldosterone, testosterone, androstanedione, dehydroepiandrosterone, sulfated dehydroepiandrosterone, adrenocorticotropin, 18-hydroxicorticotropin, estrone, estradiol, and estriol using non-solid-phase extraction.

目的:建立一种双衍生法-结合串联质谱技术同时检测人血清中18种类固醇激素的方法。方法:血清样本采用羟胺和1,2-二甲基咪唑-5-磺酰氯对进行衍生处理,采用液相色谱-串联质谱法正离子选择离子监测模式(SRM)下检测,色谱柱为Kinetex®C8(100 mm×2.1 mm,2.6 μm),以0.1%甲酸水溶液为流动相A,0.1%甲酸甲醇溶液为流动相B,梯度洗脱(0~0.5 min,35% B;0.5~5 min,35% B→100% B;5.0~5.1 min,100% B→35% B;5.1~7 min,35% B),柱温40 ℃,流速0.4 mL·min-1,进样量20 μL,采集时间为6 min。结果:18种类固醇激素的线性相关系数均>0.99,回收率均在85%~115%,精密度RSD均<15%。用该方法成功地检测了156份健康人群受试者血清样本(男性75例,女性61例)并制定了参考区间。结论:该方法可用非固相萃取法同时检测孕烯醇酮、17-羟孕烯醇酮、孕酮、17-羟孕酮、皮质酮、皮质醇、11-脱氧皮质醇、21-脱氧皮质醇、醛固酮、睾酮、雄烯二酮、脱氢表雄酮、硫酸脱氢表雄酮、皮质素、18-羟皮质酮、雌酮、雌二醇、雌三醇等18种类固醇激素。

目的:建立一种双衍生法-结合串联质谱技术同时检测人血清中18种类固醇激素的方法。方法:血清样本采用羟胺和1,2-二甲基咪唑-5-磺酰氯对进行衍生处理,采用液相色谱-串联质谱法正离子选择离子监测模式(SRM)下检测,色谱柱为Kinetex®C8(100 mm×2.1 mm,2.6 μm),以0.1%甲酸水溶液为流动相A,0.1%甲酸甲醇溶液为流动相B,梯度洗脱(0~0.5 min,35% B;0.5~5 min,35% B→100% B;5.0~5.1 min,100% B→35% B;5.1~7 min,35% B),柱温40 ℃,流速0.4 mL·min-1,进样量20 μL,采集时间为6 min。结果:18种类固醇激素的线性相关系数均>0.99,回收率均在85%~115%,精密度RSD均<15%。用该方法成功地检测了156份健康人群受试者血清样本(男性75例,女性61例)并制定了参考区间。结论:该方法可用非固相萃取法同时检测孕烯醇酮、17-羟孕烯醇酮、孕酮、17-羟孕酮、皮质酮、皮质醇、11-脱氧皮质醇、21-脱氧皮质醇、醛固酮、睾酮、雄烯二酮、脱氢表雄酮、硫酸脱氢表雄酮、皮质素、18-羟皮质酮、雌酮、雌二醇、雌三醇等18种类固醇激素。

Objective: To establish a dual derivatization method combined with tandem mass spectrometry for the simultaneous determination of 18 different steroid hormones in human serum. Methods: Serum samples were treated with hydroxylamine and 1, 2-dimethylimidazole-5-sulfonyl chloride for derivatization, and the resulting compounds were analyzed by liquid chromatography-tandem mass spectrometry in positive ion selected reaction monitoring (SRM) mode. The Kinetex®C8 column (100 mm×2.1 mm, 2.6 μm) was used for the separation. Mobile phase A (0.1% acetic acid in water) and mobile phase B (0.1% acetic acid in methanol) with gradient elution(0-0.5 min, 35% B; 0.5-5 min, 35% B→100% B; 5.0-5.1 min, 100% B→35% B; 5.1-7 min, 35% B)at the flow rate of 0.4 mL·min-1 were applied. Column temperature was set at 40 ℃. Injection volume was 20 μL and collection time was 6 min. Results: The linearity correlation coefficients for all 18 steroid hormones were greater than 0.99, the recovery rates ranged from 85% to 115%, and the precision RSD was less than 15%. This method was successfully applied to the analysis of serum samples from 156 healthy subjects (75 males and 61 females), and reference intervals were established. Conclusion: This method can be used to simultaneously determine 18 types of steroid hormones, such as pregnenolone, 17-hydroxypregnenolone, progesterone, 17-hydroxyprogesterone, corticosterone, cortisol, 11-deoxycorticosterone, 21-deoxycorticosterone, aldosterone, testosterone, androstanedione, dehydroepiandrosterone, sulfated dehydroepiandrosterone, adrenocorticotropin, 18-hydroxicorticotropin, estrone, estradiol, and estriol using non-solid-phase extraction.

甾体激素又称类固醇激素,主要包括肾上腺皮质激素和性激素,肾上腺皮质激素包括盐皮质激素和糖皮质激素;性激素主要包括雌激素、雄激素、孕激素。它们在人体中能促进个体发育、维持第二性征[1]、调节糖、蛋白质和脂肪的代谢,对人体新陈代谢、体内环境的稳定、器官之间的协调以及生长发育、生殖等起着重要的调节作用[2]。人体内类固醇激素水平的异常与一系列疾病的发生密切相关,包括肾上腺遗传性疾病、肾上腺肿瘤、高血压、多囊卵巢综合征(PCOS)、库欣综合征等[3-6],因此,临床上将类固醇激素水平作为很多疾病的诊断参考指标,人血清中类固醇激素的定量检测已经是医学检验实验室的重要检测项目,是内分泌疾病精准诊疗的重要助力。
类固醇激素在生物体内含量较低、结构相似、种类繁多,常规免疫检测方法受限于抗体的交叉反应很难区分[7]。一般学者认为,液相色谱串联质谱(LC-MS/MS)法具有低检测限、高灵敏度、高特异性的优势,1次进样可同时定量多种类固醇激素,最大程度上规避了传统检测方法的诸多问题与不足,被誉为甾体激素检测的“金标准”[8-9]
尽管液相色谱串联质谱法在类固醇激素检测中有着其他方法无可比拟的优势,但仍面临着不小的挑战。重要的因素为部分类固醇激素不仅在人体内的浓度极低[10],且为甾体结构,离子化效率低,在皮克级别浓度上,灵敏度较差[11]。目前只有高端质谱仪如AB SCIEX 6500、Waters XEVO-TQS等才能实现皮克级别的类固醇激素检测[12]。另一个挑战是,相较于其他的类固醇激素,醛固酮、雌二醇、雌三醇等激素在不仅人体内浓度极低,而且质谱检测时需要负离子扫描模式扫描。因此,醛固酮、雌二醇、雌三醇与其他类固醇同时检测时[13],质谱仪需要较快的正负模式切换速度,否则可能会导致色谱峰宽时无法采集到足够的扫描点,从而影响定量准确性[14],这也对质谱仪的性能提出了较高的要求。目前关于非衍生法检测类固醇激素的报道中,多采用固相萃取法(solid phase extraction,SPE)。固相萃取法需要采用固相萃取柱或固相萃取板等昂贵耗材,且处理方法通量低,这些缺点也进一步限制了类固醇激素检测方法的临床应用[15-16]
应对类固醇激素检测灵敏度过低的另一种解决方案为使用衍生法来提高质谱信号强度。如有文献报道使用羟胺、吉拉德试剂与含羰基的类固醇激素发生衍生反应[17-20],能显著提高质谱中的灵敏度,但这类衍生试剂无法与雌二醇、雌三醇等不含羰基结构的化合物发生衍生反应。也有文献报道可使用丹磺酰氯与雌二醇、雌三醇的酚羟基官能团发生衍生反应,但这类衍生试剂无法与除雌激素外的绝大多数类固醇激素发生衍生反应[21-23]
为此,本研究利用羟胺与1,2-二甲基咪唑-5-磺酰氯与类固醇激素发生双衍生反应,仅需正离子采集模式,即可同时检测孕烯醇酮、17-羟孕烯醇酮、孕酮、17-羟孕酮、皮质酮、皮质醇、11-脱氧皮质醇、21-脱氧皮质醇、醛固酮、睾酮、雄烯二酮、脱氢表雄酮、硫酸脱氢表雄酮、皮质素、18-羟皮质酮、雌酮、雌二醇、雌三醇等18种类固醇激素。该检测方法灵敏度高,重现性好,可作为多种类固醇激素的临床检测方法。

甲醇(LC-MS级)和乙腈(LC-MS级)购于ACS 公司,甲酸(LC-MS级)购于梯希爱(上海)化成工业发展有限公司,盐酸羟胺、碳酸钠、碳酸氢钠、1,2-二甲基咪唑-5-磺酰氯、二丁基羟基甲苯(BHT) 购于阿拉丁公司,超纯水(自制)。
采用Kinetex® C8(100 mm×2.1 mm,2.6 μm)色谱柱,以 0.1%甲酸水溶液为流动相A,0.1%甲酸甲醇溶液为流动B,梯度洗脱(0~0.5 min,35% B;0.5~5 min,35% B→100% B;5.0~5.1 min,100% B→35% B;5.1~7 min,35% B),柱温40 ℃,流速0.4 mL·min-1,进样量20 μL,采集时间6 min。
离子源为电喷雾离子源(H-ESI源),采集方式为正离子下的多反应监测模式(SRM),喷雾电压为4 000 V,鞘气压力为35 Arb,辅助气压力为6 Arb,蒸发温度450 ℃,离子传输管温度350 ℃。
精密称量11-脱氧皮质醇、皮质素、脱氢表雄酮、18-羟皮质酮、硫酸脱氢表雄酮标准品粉末,置于5 mL量瓶中,用含0.1% BHT的甲醇稀释并定容,配成分析物储备液。其中11-脱氧皮质醇储备液、皮质素储备液、脱氢表雄酮储备液和18-羟皮质酮储备液的浓度均为1 mg·mL-1,硫酸脱氢表雄酮储备液质量浓度为5 mg·mL-1。其余分析物对照品均为液体对照品,可直接作为储备液使用,浓度见“1.2.1”项。
用分析天平精密称量11-脱氧皮质醇-d5、皮质素-d8、脱氢表雄酮-d6、18-羟皮质酮-d4和硫酸脱氢表雄酮-d6标准品粉末,置于1 mL量瓶中,用含0.1% BHT的甲醇稀释并定容,配成内标储备液。其中11-脱氧皮质醇-d5、皮质素-d8、脱氢表雄酮-d6、18-羟皮质酮-d4的浓度均为1 mg·mL-1,硫酸脱氢表雄酮-d6的质量浓度为2 mg·mL-1。其余内标对照品均为液体对照品,可直接作为内标储备液使用,浓度见“1.2.2”。
用移液器分别移取18种类固醇激素分析物储备液,置于10 mL量瓶中,加入0.1% BHT的甲醇定容,配制成18种类固醇激素分析物混合工作液。其中,孕烯醇酮储备液、21-脱氧皮质醇储备液、醛固酮储备液的移取体积均为50 μL;孕酮储备液、雌二醇储备液、雌三醇储备液、睾酮储备液的移取体积均为5 μL,17-羟孕烯醇酮储备液和皮质醇储备液的移取体积均为100 μL,17-羟孕酮储备液、11-脱氧皮质醇储备液、皮质素储备液、雄烯二酮储备液、皮质酮储备液、脱氢表雄酮储备液、18-羟皮质酮储备液的移取体积均为10 μL,雌酮储备液的移取体积为2 μL,硫酸脱氢表雄酮储备液的移取体积为1 mL。
在配成的分析物混合工作液中,孕烯醇酮、孕酮、雌二醇、雌三醇、21-脱氧皮质醇、醛固酮、睾酮质量浓度均为 0.5 μg·mL-1,17-羟孕烯醇酮、17-羟孕酮、11-脱氧皮质醇、皮质素、雄烯二酮、皮质酮、脱氢表雄酮的浓度均为1 μg·mL-1,雌酮和18-羟皮质酮质量浓度为0.2 μg·mL-1,硫酸脱氢表雄酮浓度为500 μg·mL-1,皮质醇浓度为10 μg·mL-1
用移液器分别移取18种类固醇激素内标储备液,置于25 mL量瓶中,加入甲醇定容,配成18种类固醇激素内标混合工作液。其中11-脱氧皮质醇-d5储备液、皮质素-d8储备液和18-羟皮质酮-d4储备液的移取体积均为5 μL,硫酸脱氢表雄酮-d6储备液、17-羟孕酮-d8储备液、皮质酮-d8储备液、雄烯二酮-d7储备液、睾酮-d3储备液、雌酮-d4储备液、雌二醇-d5储备液、雌三醇-d3储备液、孕烯醇酮-d4储备液、醛固酮-d7储备液、孕酮-d9储备液、21-脱氧皮质醇-d8储备液、17-羟孕烯醇酮-d3储备液、皮质醇-d4储备液的移取体积均为50 μL。脱氢表雄酮-d6储备液的移取体积均为25 μL。
配成的内标混合工作液中,11-脱氧皮质醇-d5、17-羟孕酮-d8、皮质酮-d8、雄烯二酮-d7、睾酮-d3、皮质素-d8、雌酮-d4、雌二醇-d5、雌三醇-d3、孕烯醇酮-d4、醛固酮-d7、18-羟皮质酮-d4、孕酮-d9、21-脱氧皮质醇-d8、17-羟孕烯醇酮-d3、皮质醇-d4的质量浓度均为200 ng·mL-1,脱氢表雄酮-d6质量浓度为1 μg·mL-1和硫酸脱氢表雄酮-d6质量浓度为4 μg·mL-1
将分析物工作液用1%牛血清白蛋白(1% BSA)稀释成6个浓度的标准曲线和质控样品。其中孕烯醇酮、孕酮、雌二醇、雌三醇、21-脱氧皮质醇、醛固酮、睾酮的标准曲线系列对照品溶液质量浓度均为0.05、 0.125、0.25、1.25、5、10,质控样品质量浓度均为0.2和7.5 ng·mL-1
17-羟孕烯醇酮、17-羟孕酮、11-脱氧皮质醇、皮质素、雄烯二酮、皮质酮、脱氢表雄酮的标准曲线系列溶液质量浓度均为0.1、0.25、0.5、2.5、10、20 ng·mL-1,质控样品质量浓度均为0.4 和15 ng·mL-1
雌酮和18-羟皮质酮的标准曲线系列溶液质量浓度均为0.02、0.05、0.1、0.5、2、4 ng·mL-1,质控样品质量浓度均为0.08 和3 ng·mL-1。硫酸脱氢表雄酮的标准曲线系列溶液浓度为50、125、250、1 250、5 000、10 000 ng·mL-1,质控样品质量浓度为200和7 500 ng·mL-1。皮质醇的标准曲线系列溶液质量浓度为1、2.5、5、25、100、200 ng·mL-1,质控样品质量浓度为4和150 ng·mL-1
称量盐酸羟胺5 g,加入20%甲醇溶液50 mL,制成质量浓度为2 mol·L-1的盐酸羟胺溶液。称量1,2-二甲基咪唑-5-磺酰氯50 mg,加入丙酮50 mL,配成1 mg·mL-1的工作液,备用。称量碳酸氢钠4.2 g,加入超纯水10 mL溶解,配制浓度为50 mmol·L-1碳酸氢钠溶液,并逐滴加入碳酸钠溶液调节pH约为10,备用。
取血清0.3 mL,置于2 mL离心管中,加入乙腈-甲醇(9∶1)和正己烷共1.2 mL,涡旋振荡3 min,12 000 r·min-1离心5 min。取正己烷层置于新的2 mL离心管中,在40 ℃用氮气吹干,向干燥的离心管中加入盐酸羟胺溶液80 μL,在60 ℃振荡孵育15 min。再加入 1,2-二甲基咪唑-5-磺酰氯溶液40 μL和碳酸氢钠溶液30 μL,在60 ℃振荡孵育15 min,取出孵化后的样品,在12 000 r·min-1离心3 min,取上清液100 μL用于LC-MS/MS分析。
本研究采用衍生法对18种类固醇激素进行检测,在建立质谱条件和液相色谱条件前,将分析物进行衍生反应,已得到衍生产物的母离子、碎片离子、碎裂能等信息。对于孕烯醇酮、17-羟孕烯醇酮、孕酮、17-羟孕酮、皮质酮、皮质醇、11-脱氧皮质醇、21-脱氧皮质醇、醛固酮、睾酮、雄烯二酮、脱氢表雄酮、硫酸脱氢表雄酮、皮质素、18-羟皮质酮等,其甾烷结构中至少都有1个羰基官能团,能与盐酸羟胺发生肟化反应(图1)。雌二醇、雌三醇含有酚羟基,可与1,2-二甲基咪唑-5-磺酰氯发生酯化反应(图2),从而提高雌激素的质谱响应。
将18种类固醇激素储备液分别稀释至100 ng·mL-1,按照“2.4”项方法处理后,加入含0.1% BHT的甲醇0.5 mL稀释,注入质谱仪中,优化母离子、碎片离子、传输电压(RF)、碰撞能(CE)等参数,得到响应最强的离子对信息及质谱参数。使用C8色谱柱优化液相色谱条件和洗脱梯度,其保留时间,离子对信息等优化结果见表1
用标准曲线检测定量限浓度处的样本,每个样本重复检测5次,标准曲线的线性范围、斜率、相关系数r、定量限浓度的均值、信噪比(S/N)、RSD和准确度见表2
向人血清中加入18种类固醇激素的储备液,配成高、低2个浓度的精密度验证样品,用标准曲线检测,每个浓度连续测定5次,每天1个分析批次,连续检测3 d。批内精密度和批间精密度统计结果见表3
取“2.3”项制备的18种类固醇激素的混合工作液,用甲醇稀释成高、中、低3个浓度的标准溶液后,加入980 μL人血清中,照“2.4”项方法制备成高、中、低3个浓度的回收率验证供试溶液,用标准曲线检测,每个浓度重复测定5次。结果见表4
采用本研究方法,共检测156份健康人群受试者血清样本,其中75份为男性,81份为女性,年龄为18~70岁,参照WS/T 402-2012 临床实验室检验项目参考区间的制定方法[24],采用非参数统计法处理数据,统计样本的浓度的范围区间见表5
类固醇激素与人类多种内分泌疾病相关,如先天性肾上腺皮质增生症(congenital adrenal hyperplasia,ACH)、多囊卵巢综合征(polycystic ovary syndrome,PCOS)、原发性醛固酮增多症(primary hyperaldosteronism,PA)、库欣综合征(cushing syndrome,CS)等,检测人体代谢通量中的多种类固醇激素可为临床上内分泌疾病的辅助诊断提供更加全面的信息[25]
类固醇检测的难点为其在人体内的水平极低,且大多具有相似的化学结构,导致常规的免疫法灵敏度、重现性差,无法覆盖人体内正常范围,检测过程中干扰物质会产生交叉反应。近年来对类固醇激素检测的相关报道大多集中在LC-MS/MS方法上,但这些方法都无法解决检测通量低,检测成本高的问题。如正压条件下采用96孔型的固相萃取板来处理样本,虽然样本净化程度高,基质效应相对较低,但该方法检测通量较低,且成本极为高昂,无法满足临床上对类固醇激素的常规检测需求。另外由于类固醇在人体内水平极低,且其的化学结构中缺少碱性氮原子等富电基团导致其离子化效率低,在质谱中响应较差[26],如双氢睾酮、醛固酮、孕烯醇酮等,导致类固醇激素检测一般需要灵敏度较高的质谱仪。一些文献报道了用衍生法来提高类固醇激素的质谱响应,但类固醇激素中的雌激素由于缺少羰基导致无法用一种衍生试剂完成雌激素与其他激素的联合检测。
本研究采用盐酸羟胺和1,2-二甲基咪唑-5-磺酰氯,分别与含羰基的类固醇激素、雌激素发生肟化反应和酯化反应,建立了多种类固醇激素同时检测的方法并进行了系统的方法学确证和临床实例应用,该方法检测通量和灵敏度高,重复性好,能快速准确测定18种类固醇激素,为类固醇激素的临床检测提供了高通量的定量分析方法。
