Objective:

To develop a method for the determination of meropenem and vaborbactam for injection by HPLC.

Methods:

The chromatography was performed on Shimadzu InertSustain C18 (150 mm×4.6 mm,5 μm) column with acetonitrile-0.02 mol·L-1 sodium dihydrogen phosphate solution (adjust pH to 2.8 with phosphoric acid)(10:90) as a mobile phase,the flow rate was 1.0 mL·min-1,the detection wavelength was 210 nm,and the sample volume was 10 μL. A method for the determination of meropenem and vaborbactam for injection was established under the chromatographic conditions.

Results:

In this method,the linearities of meropenem and vaborbactam were good,and the linearity ranges were 21.40-214.0 μg·mL-1 and 19.83-198.3 μg·mL-1. Meropenem and vaborbactam had good precision with RSD of 0.31% and 0.16%,respectively. Within 24 h,meropenem and vaborbactam had good stabilities at 4 ℃,RSDs were 0.31% and 0.16%,respectively. The average recoveries of meropenem and vaborbactam were 101.0% (n=9) and 98.4% (n=9) (RSD<2.0%). The contents of meropenem and vaborbactam were 405.8 mg·g-1 and 399.1 mg·g-1,respectively.

Conclusion:

The method is accurate,simple and rapid,and can be used for the determination of meropenem and vaborbactam for injection.

目的:

方法:

目的:

应用HPLC建立注射用美罗培南韦博巴坦的含量测定方法。

方法:

采用Shimadzu InertSustain C18(150 mm×4.6 mm,5 μm)色谱柱,以乙腈-0.02 mol·L-1磷酸二氢钠溶液(用磷酸调pH至2.8)(10:90)为流动相,流速1.0 mL·min-1,检测波长210 nm,进样体积10 μL。

结果:

在本方法中,美罗培南和韦博巴坦的线性良好,线性范围分别为21.40~214.0 μg·mL-1和19.83~198.3 μg·mL-1;美罗培南和韦博巴坦精密度良好,RSD分别为0.17%和0.23%。24 h内,美罗培南和韦博巴坦在4 ℃条件下稳定性良好,RSD分别为0.31%和0.16%。美罗培南和韦博巴坦的平均加样回收率良好,分别为101.0% (n=9)和98.4% (n=9)(RSD均<2.0%)。运用该方法测得该药品中美罗培南及韦博巴坦的含量分别为405.8、399.1 mg·g-1

结论:

该测定方法准确、简单、快速,可用于测定注射用美罗培南韦博巴坦的含量。

Objective:

To develop a method for the determination of meropenem and vaborbactam for injection by HPLC.

Methods:

The chromatography was performed on Shimadzu InertSustain C18 (150 mm×4.6 mm,5 μm) column with acetonitrile-0.02 mol·L-1 sodium dihydrogen phosphate solution (adjust pH to 2.8 with phosphoric acid)(10:90) as a mobile phase,the flow rate was 1.0 mL·min-1,the detection wavelength was 210 nm,and the sample volume was 10 μL. A method for the determination of meropenem and vaborbactam for injection was established under the chromatographic conditions.

Results:

In this method,the linearities of meropenem and vaborbactam were good,and the linearity ranges were 21.40-214.0 μg·mL-1 and 19.83-198.3 μg·mL-1. Meropenem and vaborbactam had good precision with RSD of 0.31% and 0.16%,respectively. Within 24 h,meropenem and vaborbactam had good stabilities at 4 ℃,RSDs were 0.31% and 0.16%,respectively. The average recoveries of meropenem and vaborbactam were 101.0% (n=9) and 98.4% (n=9) (RSD<2.0%). The contents of meropenem and vaborbactam were 405.8 mg·g-1 and 399.1 mg·g-1,respectively.

Conclusion:

The method is accurate,simple and rapid,and can be used for the determination of meropenem and vaborbactam for injection.

随着碳青霉烯类抗生素的广泛使用,耐碳青霉烯类肠杆菌(carbapenem-resistant Enterobacteriaceae,CRE)越来越常见,严重威胁公众健康,为社会经济带来极大负担[1-2]。美国疾病预防控制中心定义体外药敏试验对任何一种碳青霉烯类(亚胺培南、美罗培南、厄他培南或多利培南)耐药的肠杆菌目细菌为CRE[3-4]。CRE会导致显著的发病率和死亡率[5],治疗过程中花费多,治疗方案有限。临床上急需开发具有抗某些碳青霉烯类耐药病原体活性的新药物。由β-内酰胺类抗生素和具有抗碳青霉烯酶活性的强效β-内酰胺酶抑制剂组成的新治疗组合已经上市。
注射用美罗培南韦博巴坦(商品名Vaborem®)是一种新型抗生素复方制剂[6],由碳青霉烯类抗菌药物美罗培南和新型环硼酸β-内酰胺酶抑制剂韦博巴坦(vaborbactam)组成。韦博巴坦能够抑制多种A类以及C类β-内酰胺酶,保护美罗培南免受丝氨酸碳青霉烯酶降解,恢复其对碳青霉烯耐药菌株的活性[7]。Vaborem®被专门用于抑制对碳青霉烯类药物耐药的肠杆菌科细菌,包括常见的产肺炎克雷伯菌碳青霉烯酶(KPC)细菌。有研究者在美国共收集了1 697株耐多药肠杆菌(所有CRE菌株),这些菌株对美罗培南韦博巴坦的敏感率为99.1%[8-9]。临床试验(NCT01897779、NCT02168946、NCT02020434)结果显示美罗培南韦博巴坦单药治疗CRE感染可提高临床治愈率、降低死亡率并降低肾毒性[10-12]。目前,Vaborem®已在美国、欧盟及其他国家获批用于治疗成人复杂尿路感染(cUTI),包括肾盂肾炎。在部分地区,其亦获批用于治疗复杂性腹腔感染(cIAI),以及医院获得性细菌性肺炎(HABP)与呼吸机相关细菌性肺炎(VABP)[13]
注射用美罗培南韦博巴坦作为一种新药,目前未在中国境内上市,未收载于《中华人民共和国药典》及国外药典,尚无相关质量标准。经文献调研,仅有2篇[14-15]相关的血药浓度测定报道,文献采用的方法对于注射用美罗培南韦博巴坦的含量测定研究参考意义极为有限。本研究建立了注射用美罗培南韦博巴坦新的检测方法,所建分析方法简便、准确,重复性好,可为该药品的质量评价方法提供参考。
Agilent 1260型高效液相色谱仪,配有G1311C四元梯度泵,G1329B自动进样器,G1316A柱温箱,G4212B二极管阵列检测器(DAD)(Agilent公司),Mettler XP-205十万分之一电子天平(梅特勒公司),KQ-800KDE超声仪(昆山市超声仪器有限公司);Milli-Q超纯水处理系统(Millipore公司)。
美罗培南对照品(含量86.8%,批号130506-202004,中国食品药品检定研究院),韦博巴坦对照品(含量98%,批号D211S235667,上海源叶生物科技有限公司),注射用美罗培南韦博巴坦(规格:每瓶含1 g美罗培南、1 g韦博巴坦,批号0001E2,A. Menarini GmbH)。磷酸二氢钠(批号20221220,国药集团化学试剂有限公司),乙腈(批号F23N2D202,Fisher公司),实验用水为超纯水。
采用Shimadzu InertSustain C18(150 mm×4.6 mm,5 μm)色谱柱,以乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调节pH至2.8)(10:90)为流动相,流速1.0 mL·min-1,检测波长210 nm,进样体积10 μL,样品盘控温4 ℃,柱温25 ℃。进样前,溶液均用0.45 μm微孔滤膜过滤。
依据注射用美罗培南韦博巴坦药品说明书提供的配方制得不含药品,只含辅料(磷酸二氢钠)的粉末作为阴性样品。称取该粉末2.5 mg,精密称定,置100 mL量瓶中,加水40 mL,超声(频率40 kHz,功率800 W)10 min,用水稀释至刻度,摇匀,即得阴性样品溶液。
精密称定美罗培南、韦博巴坦的对照品各约80 mg,分别置100 mL量瓶中,加水40 mL,超声(频率40 kHz,功率800 W)10 min使溶解,用水稀释至刻度,摇匀,制成美罗培南对照品储备液(约为800 μg·mL-1)和韦博巴坦对照品储备液(约为800 μg·mL-1)。精密量取上述2种对照品储备液各5 mL,置同一50 mL量瓶中,加水稀释至刻度,摇匀,即得。
称取本品0.020 g,精密称定,置100 mL量瓶中,加水40 mL,超声(频率40 kHz,功率800 W)10 min使溶解,用水稀释至刻度,摇匀,即得。
分别取“2.2”项下阴性样品溶液、混合对照品溶液和供试品溶液,按“2.1”项下色谱条件进样分析。结果,混合对照品溶液中美罗培南峰和韦博巴坦峰的理论塔板数分别为6 097和7 492,两峰分离度为16.5。且阴性无干扰,方法专属性良好,见图1
分别精密量取“2.2.2”项下美罗培南对照品储备液0.5、1、2、2.5、4、5 mL,分别置于20 mL量瓶中,再取相同体积的韦博巴坦对照储备液一一对应置于同一20 mL量瓶中,用水稀释至刻度,摇匀,得到美罗培南质量浓度分别为21.40、42.80、85.60、107.0、171.2、214.0 μg·mL-1,韦博巴坦质量浓度分别为19.83、39.65、79.30、99.13、158.6、198.3 μg·mL-1的混合对照品溶液,按“2.1”项下方法进样测定,记录峰面积。以峰面积(Y)为纵坐标,质量浓度(X,μg·mL-1)为横坐标作标准曲线。得到美罗培南、韦博巴坦的线性方程分别为
结果表明,美罗培南、韦博巴坦质量浓度在21.40~214.0、19.83~198.3 μg·mL-1的范围内,线性相关性良好。
将混合对照品溶液用水逐级稀释后,以信噪比(S/N)约为3.0时的浓度为检测限(LOD),S/N约为10.0时的浓度为定量限(LOQ),测得美罗培南的LOD和LOQ分别为0.064 μg·mL-1和0.22 μg·mL-1;韦博巴坦的LOD和LOQ分别为0.10 μg·mL-1和0.30 μg·mL-1
将“2.2.2”项下混合对照品溶液(各约80 μg·mL-1)连续进样6次,结果美罗培南和韦博巴坦峰面积的RSD(n=6)分别为0.17%和0.23%,说明仪器精密度良好。
精密称取本品0.020 g,按“2.2.3”项下方法制备供试品溶液6份,按“2.1”项下条件进样测定,结果美罗培南和韦博巴坦的平均含量(n=6)分别为405.8、399.1 mg·g-1,RSD分别为1.2%、1.4%,表明该方法重复性良好。
将供试品溶液分别在25 ℃及4 ℃条件下于0、1、2、3、4、5、6、7、8、24 h进样分析,25 ℃、24 h内美罗培南和韦博巴坦色谱峰面积的RSD(n=10)分别为2.2%和0.17%,4 ℃、24 h内美罗培南和韦博巴坦色谱峰面积RSD(n=10)分别为0.31%和0.16%。表明供试品溶液在4 ℃、24 h条件下稳定性良好,美罗培南在25 ℃条件下峰面积变化较大。
精密称取注射用美罗培南韦博巴坦0.010 g,置100 mL量瓶中,分别精密加入美罗培南对照品溶液(质量浓度约为1 mg·mL-1)和韦博巴坦对照品溶液(质量浓度约为1 mg·mL-1)各3.2、4、4.8 mL,即相当于样品中原含量的80%、100%、120%水平,超声(频率40 kHz,功率800 W)10 min使溶解,用水稀释至刻度,摇匀,即得低、中、高浓度的供试溶液。每个浓度制备3份供试溶液,共9份。进样测定,计算加样回收率和RSD,结果见表1。美罗培南和韦博巴坦的平均加样回收率分(n=9)别为101.0%、98.4%,RSD分别为0.36%、1.0%,表明方法的加样回收率良好。
改变色谱条件中的柱温(23、27 ℃),结果显示美罗培南及韦博巴坦保留时间基本不变,RSD分别为0.6%、1.3%,不同柱温下两峰分离度分别为16.1、16.7。改变色谱条件中的流速(0.8、1.2 mL·min-1),柱温在25 ℃条件下,美罗培南的保留时间分别为4.46、6.81 min,韦博巴坦的保留时间分别为10.27、15.57 min,不同流速下两峰分离度分别为15.6、17.5。结果表明本方法的HPLC条件耐用性良好,色谱参数的微调不会影响检测结果。
应用本研究建立的方法对注射用美罗培南韦博巴坦进行含量测定。取注射用美罗培南韦博巴坦,按“2.2.3”项下方法制备供试品溶液6份,按“2.1”项下色谱条件进样测定,采用外标法按照“2.4”项下线性回归方程进行计算,结果测得样品中美罗培南含量分别为409.4、394.9、407.1、406.5、408.3、408.8 mg·g-1,含量平均值为405.8 mg·g-1,RSD为1.2%;韦博巴坦含量分别为391.1、407.0、402.1、399.0、395.6、399.8 mg·g-1,含量平均值为399.1 mg·g-1,RSD为1.4%。
采用液相二极管阵列检测器(DAD)在波长190~400 nm对“2.2.2”项下美罗培南对照品储备液、韦博巴坦对照品储备液进行光谱扫描,美罗培南在304 nm处有最大吸收峰,韦博巴坦在234 nm处有最大吸收峰,2个成分在210 nm处也有较强吸收,且分离度良好,无其他干扰,因此选择210 nm作为检测波长同时测定2个成分含量。
分别选择了Agilent Poroshell C18 (150 mm×4.6 mm,2.7 μm)色谱柱、4根不同批号的Alltima C18 (150 mm×4.6 mm,5 μm)色谱柱进行试验,结果显示以上品牌色谱柱对美罗培南的保留时间、峰形均无影响,但韦博巴坦不出峰。分别选用WatersXbrige C18 (150 mm×4.6 mm,5 μm)色谱柱及Shimadzu InertSustain C18 (150 mm×4.6 mm,5 μm)色谱柱进行试验,这2种品牌的色谱柱对美罗培南及韦博巴坦的检测均无影响,但Waters Xbrige C18柱对韦博巴坦峰形的影响较大,其对称因子不及Shimadzu InertSustain C18色谱柱,因此本研究采用Shimadzu InertSustain C18色谱柱进行检测。
注射用美罗培南韦博巴坦作为一种新药,未收载于《中华人民共和国药典》及国外药典,通过查询文献[14]得知,以甲醇-0.025 mol·L-1磷酸盐缓冲液(pH 6.5)(13:87)作为流动相,所用磷酸盐缓冲液(pH 6.5)为磷酸二氢钠与磷酸氢二钠的混合溶液(体积比为685:315),配制过程复杂,美罗培南及韦博巴坦出峰时间过久且韦博巴坦拖尾严重,对称因子低。本研究通过改变流动相的条件,用磷酸二氢钠溶液(磷酸调节不同pH)代替磷酸盐缓冲液(pH 6.5),简化了配制过程的步骤。通过考察流动相中甲醇或乙腈与不同pH的磷酸二氢钠溶液的配伍,找到最优比例。考察了甲醇-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至6.5)(20:80)、乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至6.5)(20:80)、乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至6.5)(8:92)、乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至6.5)(10:90)、乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至4.6)(10:90)、乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至3.2)(10:90)、乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至2.8)(10:90)等诸多流动相系统,最终确定乙腈-0.02 mol·L-1磷酸二氢钠溶液(磷酸调pH至2.8)(10:90)作为流动相,出峰时间及韦博巴坦的对称因子均为最优,分离效果最佳。
与注射用美罗培南韦博巴坦相关的检测方法仅有2篇文献[14-15],此文献均为考察生物样本中的药物浓度,关于样品称样量及溶剂的选择对于本研究无参考意义。本研究首先尝试了精密称定样品约0.010、0.020、0.050、0.10 g,加水40 mL,溶于100 mL量瓶中(即4种不同浓度样品),与“2.4”项下不同浓度混合对照品溶液峰面积的响应值进行比较,结果确定精密称定约0.020 g样品,加水40 mL,溶于100 mL量瓶时,美罗培南与韦博巴坦的响应值与质量浓度约为80 μg·mL-1的混合对照品溶液基本一致。此外,在查询韦博巴坦对照品时,建议在韦博巴坦溶解性不好的情况下加入适量的DMSO进行助溶,因此在试验过程中,在100 mL量瓶中加水40 mL,观察到韦博巴坦粉末有少量不溶,超声10 min后完全溶解,整个研究未加入DMSO,简化了操作流程。不同的超声提取时间(10、20、30 min)对于样品的峰面积及峰形并无影响,因此选择了超声10 min进行后续试验。说明书标注每瓶药品(1 g美罗培南、1 g韦博巴坦)中含有辅料250 mg磷酸二氢钠,因此做阴性样品需称量2.5 mg磷酸二氢钠进行试验。美罗培南在室温下不如在4 ℃条件下稳定,因此上述试验均采用样品盘控温4 ℃下进样,每次试验前供试品溶液均现配现用。
应用本研究所建方法对注射用美罗培南韦博巴坦进行含量测定,药品说明书未注明标示量,该药在《中华人民共和国药典》及国外药典中均未收载,未查询到相应的质量标准,所测含量结果只能以mg·g-1表示。该药品目前未在中国境内上市,获取来源困难,只获取1批样品进行测定,未进行大规模测定。
本研究建立了测定注射用美罗培南韦博巴坦含量的方法,与已有文献相比,简化了流动相配制步骤,改善了美罗培南与韦博巴坦出峰时间以及韦博巴坦的峰形。经方法学考察,专属性、线性关系、精密度、重复性、回收率及稳定性均良好,所建方法准确、简便,可为注射用美罗培南韦博巴坦的质量评价方法提供参考。
