Objective:

The early warning of new psychoactive substance iso-propoxate (homologue of etomidate) was carried out through sewage drug monitoring technology, and quantitative research was conducted to provide technical support for management and law enforcement.

Methods:

The abnormal results were obtained by quantitative analysis of multiple reaction monitoring (MRM) mode using online solid phase extraction-ultra high performance liquid chromatography tandem mass spectrometry (SPE-UPLC-MS/MS), and the basic information of abnormal substance was determined by precursor ion scanning mode. The qualitative determination was carried out using parallel reaction monitoring (PRM) mode by ultra high performance liquid chromatography quadrupole-orbitrap high resolution mass spectrometry (UPLC-Q exactive HRMS) after the directional synthesis of the compound. Quantitative study was then conducted by commercial reference products. The correlation analysis was carried out by SPSS 27.

Results:

The abnormal substance was determined to be iso-propoxate. The optimized MRM parameters could be distinguished from its isomers by specific ion abundance ratio. The correlation analysis of quantitative results showed that it was similar to etomidate and ketamine abuse areas.

Conclusion:

In the process of sewage drug monitoring, it is necessary to pay attention to the proportional relationship between prototype and metabolite. It is important to summarize the characteristic fragments and characteristic neutral loss information of specific types of compounds, carry out screening non-targeted detection for key samples in key areas in the monitoring process, and analyze potential substances using high-resolution mass spectrometry and other methods, so as to fully leveraging the early warning role of sewage drug monitoring.

目的:

通过污水毒品监测技术进行新精神活性物质异丙咪酯(依托咪酯同系物)的预警,并开展定量研究,为列管及执法打击工作提供技术支撑。

方法:

通过在线固相萃取-超高效液相色谱-三重四极杆质谱联用(online solid phase extraction-ultra high performance liquid chromatography tandem mass spectrometry,Online-SPE-UPLC-MS/MS)的多反应监测模式(multiple reaction monitoring,MRM)定量分析获知异常结果,前体离子扫描模式定性确定异常物质基本信息,结合化合物定向合成后的超高效液相色谱-四极杆-静电场轨道阱高分辨质谱(ultra high performance liquid chromatography quadrupole-orbitrap high resolution mass spectrometry,UPLC-Q Exactive HRMS)平行反应监测(parallel reaction monitoring,PRM)模式进行定性确定,后购置市售对照品开展定量研究,并将所得结果运用SPSS 27进行相关性分析。

结果:

确定异常物质为异丙咪酯,并通过优化MRM参数可利用特定离子丰度比实现与其同分异构体的区分,定量结果相关性分析结果表明其与依托咪酯和氯胺酮滥用地区趋同。

结论:

在污水毒品监测过程中,需关注原型与代谢物的比例关系,总结归纳特定类型化合物的特征碎片及特征中性丢失信息,针对监测过程中的重点地区重点样本开展筛查性非靶向检测,结合高分辨质谱等手段进行潜在物质的分析,充分发挥污水毒品监测的预警作用。

目的:

通过污水毒品监测技术进行新精神活性物质异丙咪酯(依托咪酯同系物)的预警,并开展定量研究,为列管及执法打击工作提供技术支撑。

方法:

通过在线固相萃取-超高效液相色谱-三重四极杆质谱联用(online solid phase extraction-ultra high performance liquid chromatography tandem mass spectrometry,Online-SPE-UPLC-MS/MS)的多反应监测模式(multiple reaction monitoring,MRM)定量分析获知异常结果,前体离子扫描模式定性确定异常物质基本信息,结合化合物定向合成后的超高效液相色谱-四极杆-静电场轨道阱高分辨质谱(ultra high performance liquid chromatography quadrupole-orbitrap high resolution mass spectrometry,UPLC-Q Exactive HRMS)平行反应监测(parallel reaction monitoring,PRM)模式进行定性确定,后购置市售对照品开展定量研究,并将所得结果运用SPSS 27进行相关性分析。

结果:

确定异常物质为异丙咪酯,并通过优化MRM参数可利用特定离子丰度比实现与其同分异构体的区分,定量结果相关性分析结果表明其与依托咪酯和氯胺酮滥用地区趋同。

结论:

在污水毒品监测过程中,需关注原型与代谢物的比例关系,总结归纳特定类型化合物的特征碎片及特征中性丢失信息,针对监测过程中的重点地区重点样本开展筛查性非靶向检测,结合高分辨质谱等手段进行潜在物质的分析,充分发挥污水毒品监测的预警作用。

Objective:

The early warning of new psychoactive substance iso-propoxate (homologue of etomidate) was carried out through sewage drug monitoring technology, and quantitative research was conducted to provide technical support for management and law enforcement.

Methods:

The abnormal results were obtained by quantitative analysis of multiple reaction monitoring (MRM) mode using online solid phase extraction-ultra high performance liquid chromatography tandem mass spectrometry (SPE-UPLC-MS/MS), and the basic information of abnormal substance was determined by precursor ion scanning mode. The qualitative determination was carried out using parallel reaction monitoring (PRM) mode by ultra high performance liquid chromatography quadrupole-orbitrap high resolution mass spectrometry (UPLC-Q exactive HRMS) after the directional synthesis of the compound. Quantitative study was then conducted by commercial reference products. The correlation analysis was carried out by SPSS 27.

Results:

The abnormal substance was determined to be iso-propoxate. The optimized MRM parameters could be distinguished from its isomers by specific ion abundance ratio. The correlation analysis of quantitative results showed that it was similar to etomidate and ketamine abuse areas.

Conclusion:

In the process of sewage drug monitoring, it is necessary to pay attention to the proportional relationship between prototype and metabolite. It is important to summarize the characteristic fragments and characteristic neutral loss information of specific types of compounds, carry out screening non-targeted detection for key samples in key areas in the monitoring process, and analyze potential substances using high-resolution mass spectrometry and other methods, so as to fully leveraging the early warning role of sewage drug monitoring.

污水中毒品监测起源于污水流行病学,在国内外已经有近20年的研究和发展。该技术原理是人体摄入毒品后,其原体及代谢产物会通过尿液或粪便进入到污水管道系统,通过测定污水处理厂、泵站或人群聚集点、地表水等未经生化处理的污水中毒品原体及相应代谢产物和人口标记物的浓度,结合毒品原体或代谢产物的排泄率,根据污水流量,反推监测区域内的毒品滥用量和滥用规模。欧洲[1-3]、大洋洲[4-5]、北美洲[6-7]、亚洲[8-9]的多个国家和地区已广泛采用此种方法监测甲基苯丙胺、苯丙胺、氯胺酮、可卡因、“摇头丸”和大麻等毒品的滥用。2018年,我国国家毒品实验室首次应用污水毒品监测技术,对国内36个大城市的生活污水进行采样分析,开展毒情监测工作。该技术目前已经成为各级禁毒办常态化指导、评估、推动禁毒工作的主要科技手段之一,在禁毒工作中发挥了重要的“利剑”和“指南针”作用[10]
新精神活性物质,又被称为新型策划毒品、毒品类似物、合法兴奋剂,成为继传统毒品(海洛因、吗啡、可卡因等)、合成毒品(冰毒、摇头丸、K粉等)之后的“第三代毒品”。多变易变的物质结构和物质属性,使得对其发现和监管缺乏及时有效的手段,犯罪分子为了获取非法利益,规避法律的打击,通过对已列管毒品进行结构修饰来精心制作相应衍生物,如氯胺酮的结构类似物氟胺酮[11]、溴胺酮[12]、甲基胺酮[13]N-乙基氟胺酮[14],均为衍生类的新精神活性物质。依托咪酯是一种催眠型静脉麻醉药,自1973年起被逐渐应用于临床的麻醉诱导及短暂手术,其主要代谢产物为依托咪酯酸[15]。由依托咪酯滥用导致的自伤、自杀、寻衅滋事、交通肇事、强奸等恶性案件时有发生,严重危害社会安全稳定[16-18]。故2023年10月1日起已将依托咪酯(在中国境内批准上市的含依托咪酯的药品制剂除外)列入第二类精神药品目录[19]
本实验室在污水毒品监测工作中对美托咪酯(依托咪酯同系物)、依托咪酯、依托咪酯酸(相关结构见图1)进行检测,美托咪酯及依托咪酯在体内的主要代谢物均为依托咪酯酸,通过对原体及其主要代谢物进行浓度分析可用于相应地区倾倒风险及滥用规模的评估。在监测过程中发现某地依托咪酯酸与依托咪酯和美托咪酯的比值大幅升高,故怀疑出现代谢物为依托咪酯酸的依托咪酯替代物,进而针对该地污水样本开展定性研究,确定替代物结构后购买对照品对某省全部样本开展此种物质的定量分析工作,在滥用初期开展滥用规模监测,为禁毒实战提供技术支撑。
污水样品均来自某省开展的污水毒品监测工作所收集的各县区污水处理厂样品,共计135份。
仪器:配备在线固相萃取AOE (Automatic Online Extraction) 模块的超高效液相色谱三重四极杆质谱联用系统由Nexera LC-40液相色谱仪和LCMS-8050三重四极杆质谱仪组成(Shimadzu公司),超高效液相色谱-四极杆-静电场轨道阱高分辨质谱联用系统由Ultimate 3000液相色谱仪和Q Exactive质谱仪组成(ThermoFisher Scientific公司),Fotector PLUS高通量全自动固相萃取仪(睿科集团(厦门)股份有限公司),AutoEVA-60全自动平行浓缩仪(睿科集团(厦门)股份有限公司),XSR105DU/A型分析天平(感量 0.01 mg,Mettler Toledo公司),Research®plus手动移液器(Eppendorf公司),Milli-Q超纯水系统(Millipore公司)。
试剂:甲酸铵(LC-MS级)、乙酸铵(LC-MS级)、甲醇(色谱纯)、乙腈(色谱纯)、异丙醇(色谱纯)均购自Merck公司,甲酸(色谱纯)、正丙醇(分析纯)购自上海阿拉丁生化科技股份有限公司,盐酸(分析纯)、氢氧化钠(分析纯)均购自广东广试试剂科技有限公司,氨水(优级纯)购自天津市科密欧化学试剂有限公司,超纯水由Milli-Q超纯水系统制备。
标准品:依托咪酯甲醇溶液(100 μg·mL-1)、依托咪酯酸甲醇溶液(100 μg·mL-1)、依托咪酯-d5甲醇溶液(100 μg·mL-1)均购自天津阿尔塔科技有限公司。依托咪酯酸-d5溶液经依托咪酯-d5甲醇溶液碱水解制备[20]。异丙咪酯(依托咪酯EP杂质C)购自湖北摩信生物技术有限公司。美托咪酯购自Cerilliant公司。
污水样品加入混合内标溶液使各同位素内标浓度为50 ng·L-1,加入30% 乙酸铵水溶液调节pH至7,混匀后用0.22 μm滤膜过滤后供仪器检测。
在线固相萃取柱:Oasis®HLB Direct Connect Hp(30 mm×2.1mm,20 μm,Waters公司),色谱柱:Shim-pack GIST-HP C18-AQ色谱柱(100 mm×2.1 mm (HSS),1.9 μm),进样量1 mL,在线固相萃取四元泵流动相A为纯水,流动相B为甲醇-乙腈-异丙醇(1∶1∶1),流动相C为0.1%甲酸水-甲醇(为1∶1),在线固相萃取采取双柱模式,于2.8 min进行柱切换,将已加载样品的固相萃取柱切换至液相色谱流路,同时将前序完成样品洗脱的固相萃取柱切换至固相萃取流路进行清洗和平衡,具体参数见表1。液相色谱流动相A为0.1%甲酸水溶液,流动相B为乙腈溶液,流速0.4 mL·min-1,柱温40 ℃,梯度洗脱(见表2)。电喷雾离子源正离子模式(ESI+),毛细管电压0.5 kV,加热模块温度400 ℃,脱溶剂管温度250 ℃,雾化气流速3 L·min-1,加热气流速10 L·min-1,干燥气流速10 L·min-1,碰撞气为氩气,采集模式为多反应监测(MRM)。美托咪酯、依托咪酯、依托咪酯酸、依托咪酯-d5、依托咪酯酸-d5采集参数见表3
污水样品用浓盐酸调节pH小于2,玻璃纤维滤膜过滤后移取滤液50 mL,以4 mL·min-1流速加载至Oasis® PRiME MCX固相萃取柱中,甲醇4 mL淋洗后气推固相萃取柱至干燥,再用含5%氨水甲醇溶液(5∶95)4 mL洗脱,洗脱液使用浓缩仪40 ℃浓缩至干,加入“2.2.2”项下初始比例流动相复溶,0.22 μm滤膜过滤后供仪器检测。
流动相A为0.02%甲酸2 mmol·L-1甲酸铵水溶液,流动相B为乙腈,流速0.3 mL·min-1,色谱柱为Shim-packGIST-HP C18-AQ色谱柱(100 mm×2.1 mm,1.9 μm),进样量10 μL,柱温40 ℃,梯度洗脱(表4)。
测试前使用 Thermo PierceTM LTQ Velos ESI阳离子校正液进行仪器校正,校正有效期维持在5 d内。喷雾电压3 500 V,雾化气30 L·h-1,辅助气10 L·h-1,离子传输温度320 ℃,辅助加热温度320 ℃,分辨率17 500,AGC target2e5,C-trap最大注入时间100 ms,隔离窗口0.4 m/z,碰撞能10/25 eV,目标物精确质量数为259.144 1。
在某省2024年第一季度污水毒品监测样本检验结果中发现部分样本依托咪酯酸与依托咪酯和美托咪酯的比例异常偏高,在同等浓度水平的依托咪酯样本中,依托咪酯酸浓度为其他样本的3倍以上,故怀疑出现代谢物为依托咪酯酸的依托咪酯类似物并有一定滥用范围。
通过对依托咪酯、美托咪酯的质谱特征碎片进行归属,详见图2。确定113、105、95为此类物质的特征碎片,故以此3个特征碎片进行前体离子扫描,发现污水样本在保留时间11 min处均出现响应信号,对其质谱图分析可知其分子量为259,与依托咪酯相差14,应该为其酯键部分多一甲基的同系物,结合美托咪酯、依托咪酯的碎裂规律,155应为其特征碎片,故以155进行前体离子扫描,在11 min处出现明显信号,质谱图显示特征质量数为259,进而再次佐证。依托咪酯的乙基部分多一甲基的结构可能为正丙酯/异丙酯,其化合物结构详见图3
正丙酯和异丙酯在缺少相应信息的情况下很难通过质谱进行确定,故本实验室通过Fischer-Speie酯缩合的手段,利用依托咪酯酸和正丙醇/异丙醇在浓盐酸的催化下加热定向合成正丙酯和异丙酯[21],以利于后续的定性分析。
丙咪酯/异丙咪酯合成:取依托咪酯酸的甲醇溶液对照品适量,用乙腈稀释为10 μg·mL-1的溶液,取上述溶液100 μL,加入浓盐酸200 μL,正丙醇/异丙醇700 μL,60 ℃水浴加热16 h,反应液冷却至室温后,用1 mol·L-1氢氧化钠溶液100 μL调pH 7,加超纯水700 μL,混匀即得(见图4)。
运用“2.2.2”项液相色谱方法将定向合成化合物分离后引入质谱,高分辨质谱采用PRM的方式进行数据采集,即选择特定的前体离子,在指定的碰撞能下进行碎裂,进而采集其所有碎片质谱信息的扫描模式,采集结果见图5
将污水样本按照“2.2.1”项下方法富集浓缩后按“2.2.2”条件进样分析,与“3.4”项下结果进行对比,确定其保留时间及相应质谱碎片与异丙咪酯一致(详见图5),后采购市售对照品进行验证,与上述结果相符。
异丙咪酯,化学名称为(R)-1-(1-苯乙基)-1H-咪唑-5羧酸异丙酯,为英国药典收录的依托咪酯杂质C[22],据文献报道,其对大鼠肾上腺膜特异性结合抑制作用的半抑制浓度(50% inhibitory concentration,IC50,IC50越低,表明药物越强效)为1.42 nmol·L-1,依托咪酯的IC50为1.08 nmol·L-1[23],即理论上异丙咪酯药效略低于依托咪酯,但其同样具有麻醉诱导的作用。
高分辨质谱分析时发现在碰撞能为10 eV的条件下,丙咪酯与异丙咪酯的特征碎片113相对强度有较大差异(图5),故对“3.3”所得合成物分别进行MRM参数的优化,结果表明两化合物最优碰撞条件基本一致,259.1/155.1最优碰撞能为-10 eV,259.1/113.1最优碰撞能为-15 eV,259.1/105.1最优碰撞能为-25 eV,259.1/95.1最优碰撞能为-25eV,2个化合物多通道下的色谱叠加图详见图6,丙咪酯保留时间为11.142 min,异丙咪酯保留时间为10.995 min,污水样本中该通道色谱峰保留时间为10.997 min;以259.1/113.1通道所得峰面积作为基准,丙咪酯中259.1/155.1通道离子丰度比为339%,259.1/105.1通道离子丰度比为139%,259.1/95.1通道离子丰度比为433%;异丙咪酯中259.1/155.1通道离子丰度比为103%,259.1/105.1通道离子丰度比为52%,259.1/95.1通道离子丰度比为161%;污水样本中259.1/155.1通道离子丰度比为120%,259.1/105.1通道离子丰度比为56%,259.1/95.1通道离子丰度比为153%;丙咪酯与异丙咪酯各通道离子丰度比相差极大,故在样本检测过程中可通过添加259.1/113.1这一参比离子对实现丙咪酯与异丙咪酯的区分,本次监测过程中污水样本各通道离子丰度比均与异丙咪酯相符。
对某省共计135份污水样本检测后,运用SPSS 27对异丙咪酯与其他毒品检测数据进行斯皮尔曼相关性分析,结果显示,其与氯胺酮的相关系数为0.405,P<0.001;与依托咪酯的相关系数为0.701,P<0.001;说明异丙咪酯检出地区与依托咪酯和氯胺酮滥用地区趋同,提示该物质滥用程度及范围有进一步扩大的风险。
通过质谱解析、定向合成和对照品确证,本研究最终确定污水样本中引起依托咪酯酸浓度偏高的化合物为异丙咪酯,该物质为依托咪酯的同系物,并可通过特定离子的离子丰度比与其同分异构体丙咪酯进行区分,无需改变现有检测方法,该参数可供开展污水检测的法庭科学实验室参考。
当前污水毒品检测大多采用靶向的MRM分析方法,即根据特定的检测目标物建立相应的分析检测方法,仅能监测已列入当前方法的化合物,对于未列入的新精神活性物质无法监测,整体的监测行为比较滞后,难以在替代物滥用初期发现并提供滥用规模的技术支持,未能充分利用污水样本所代表的信息。在后续的污水毒品监测过程中,相关实验室不仅要注重MRM模式下的定量分析,亦要总结归纳特定类型化合物的特征碎片及特征中性丢失信息,针对监测过程中的重点地区重点样本开展筛查性非靶向检测,结合高分辨质谱等手段进行结构预警,实现滥用初期数据收集,最大限度避免监测行为的滞后性,为新兴物质的列管,立法打击尽早提供滥用程度,滥用规模等方面的数据支撑。
