Objective:

To establish a method for determination genotoxic impurity furfural in furosemide and its tablets. To analyze and evaluate the measurement results to provide a reference for national drug supervision and ensure drug safety.

Methods:

The chromatographic column was filled with octadecylsilane bonded silica gel,and the gradient elution was performed with phosphoric acid solution and acetonitrile as the mobile phase. The detection wavelength was 276 nm. The method was validated and used to determine the furfural content in 6 batches of furosemide and 148 batches of furosemide tablets.

Results:

The detection limit of furfural was 4 ng·mL-1 and the quantitation limit was 12 ng·mL-1. The linearity between 0.012 0-0.602 3 μg·mL-1 (r=1.000) was good. The average recoveries of furosemide and its tablets were 99.4% (RSD=0.99%) and 101.4% (RSD=1.3%). Furfural was detected in all batches of raw materials and tablet samples. The furfural content in the tablet products,which used the raw materials from company I,was significantly higher than that of the products obtained from company H.

Conclusion:

This method has strong specificity,high sensitivity and good accuracy. It can be used for the determination of genotoxic impurity furfural in furosemide and its tablets. By analyzing the measurement results and discussing the sources and control strategies of furfural,quality control of furosemide tablets can be achieved.

目的:

建立测定呋塞米及其片剂中遗传毒性杂质糠醛的方法,通过对测定结果进行分析和评估,为国家药品监管提供参考,保障用药安全。

方法:

采用十八烷基硅烷键合硅胶为填充剂的色谱柱,以磷酸溶液-乙腈为流动相进行梯度洗脱,检测波长为276 nm,对方法进行验证并测定6批呋塞米和148批呋塞米片中的糠醛含量。

结果:

糠醛的检测限为4 ng·mL-1,定量限为12 ng·mL-1;在0.012 0~0.602 3 μg·mL-1范围内线性关系良好(r=1.000);原料药及片剂中糠醛的平均回收率分别为99.4%(RSD=0.99%)和101.4%(RSD=1.3%)。所有批次的原料药及片剂样品中均检出糠醛,使用企业I原料药的片剂中糠醛含量明显高于使用企业H原料药的片剂。

结论:

本方法专属性强,灵敏度高,准确度好,可用于呋塞米及其片剂中杂质糠醛的测定。通过对测定结果进行分析和对糠醛来源及控制策略进行讨论,为实现对呋塞米片的质量控制打下基础。

目的:

建立测定呋塞米及其片剂中遗传毒性杂质糠醛的方法,通过对测定结果进行分析和评估,为国家药品监管提供参考,保障用药安全。

方法:

采用十八烷基硅烷键合硅胶为填充剂的色谱柱,以磷酸溶液-乙腈为流动相进行梯度洗脱,检测波长为276 nm,对方法进行验证并测定6批呋塞米和148批呋塞米片中的糠醛含量。

结果:

糠醛的检测限为4 ng·mL-1,定量限为12 ng·mL-1;在0.012 0~0.602 3 μg·mL-1范围内线性关系良好(r=1.000);原料药及片剂中糠醛的平均回收率分别为99.4%(RSD=0.99%)和101.4%(RSD=1.3%)。所有批次的原料药及片剂样品中均检出糠醛,使用企业I原料药的片剂中糠醛含量明显高于使用企业H原料药的片剂。

结论:

本方法专属性强,灵敏度高,准确度好,可用于呋塞米及其片剂中杂质糠醛的测定。通过对测定结果进行分析和对糠醛来源及控制策略进行讨论,为实现对呋塞米片的质量控制打下基础。

Objective:

To establish a method for determination genotoxic impurity furfural in furosemide and its tablets. To analyze and evaluate the measurement results to provide a reference for national drug supervision and ensure drug safety.

Methods:

The chromatographic column was filled with octadecylsilane bonded silica gel,and the gradient elution was performed with phosphoric acid solution and acetonitrile as the mobile phase. The detection wavelength was 276 nm. The method was validated and used to determine the furfural content in 6 batches of furosemide and 148 batches of furosemide tablets.

Results:

The detection limit of furfural was 4 ng·mL-1 and the quantitation limit was 12 ng·mL-1. The linearity between 0.012 0-0.602 3 μg·mL-1 (r=1.000) was good. The average recoveries of furosemide and its tablets were 99.4% (RSD=0.99%) and 101.4% (RSD=1.3%). Furfural was detected in all batches of raw materials and tablet samples. The furfural content in the tablet products,which used the raw materials from company I,was significantly higher than that of the products obtained from company H.

Conclusion:

This method has strong specificity,high sensitivity and good accuracy. It can be used for the determination of genotoxic impurity furfural in furosemide and its tablets. By analyzing the measurement results and discussing the sources and control strategies of furfural,quality control of furosemide tablets can be achieved.

呋塞米(furosemide)是1种强效利尿药,主要用于水肿性疾病、高血压、高血钾症等疾病的治疗,1969年于我国上市以来,临床应用广泛[1]。呋塞米片作为国家基本药物,是2022年国家药品抽检品种,本次共收集到148批呋塞米片样品,涉及7个生产企业,规格均为20 mg。国家药品抽检是我国规模最大,层级最高,涉及面最广的对上市药品进行监管的重要技术手段,在品种选择、样品量、检验技术及人员等方面具有显著优势,在提升药品质量,促进医药高质量发展中发挥着重要作用[2-3]
呋塞米的合成路线有多种,糠胺是其合成中重要的起始物料[4]。糠胺是糠醛的氮衍生物,糠醛经胺化还原反应生成糠胺[5]。赵亚男等[6]采用高效液相色谱法在呋塞米及其制剂中检测出糠醛。糠醛含有遗传毒性杂质警示结构(醛基),具有遗传毒性风险[7]。相对于普通杂质,遗传毒性杂质的安全阈值较低,在很低的浓度下即可能产生致癌风险,因此对其控制要求更加严格。2014年,人用药品注册技术要求国际协调会议(ICH)在欧洲药品管理局(EMA)和美国食品药品监督管理局(FDA)的基础上,颁布了M7指导原则,最新一版的《M7 (R2):评估和控制药物中DNA反应性(致突变)杂质以限制潜在致癌风险》[8]于2023年正式实施。由于遗传毒性杂质通常以痕量的形式存在于药品中,对分离检测技术的要求较高,故遗传毒性杂质的检测方法也逐渐增多[9-10]
呋塞米质量标准在2020年版《中华人民共和国药典》二部、USP 43、EP 11.0和JP 18中均有收载,但各国标准中均未收载糠醛检查项。本研究建立了HPLC法,对收集到的148批呋塞米片及6批原料药进行糠醛测定,结果所有批次的样品中均检出糠醛,且片剂中的糠醛含量呈现出明显的原料药来源相关性。本文通过对测定结果进行分析和风险评估,提出了关于制定糠醛控制策略的建议,为呋塞米原料药及片剂的生产及质量控制提供参考。
Agilent 1260高效液相色谱仪(紫外检测器,四元泵,自动进样器,Agilent公司);Mettler XS205型百万分之一电子天平(Mettler-Toledo公司);Millipore超纯水机(Millipore公司)。
糠醛对照品(批号I818500,含量97.05%)购买于TRC公司。乙腈为色谱纯,磷酸为分析纯(广州化学试剂厂),水为超纯水。
样品为2022年国家药品抽检中收集到的来自7家企业的148批呋塞米片及调研收集到的2个企业各3批原料药。企业A~G为片剂生产企业,企业H和企业I为原料药生产企业,其中企业A~E的原料药购自企业H,企业F和企业G的原料药购自企业I。
采用Kromasil C18(250 mm×4.6 mm,5 μm)色谱柱,以磷酸溶液(取水1 000 mL,用磷酸调节pH至2.6)(A)-乙腈(B)为流动相,梯度洗脱(见表1),检测波长276 nm,流速1.0 mL·min-1,进样量20 μL,柱温35 ℃。
精密称取糠醛对照品适量,用95%乙腈溶液溶解并定量稀释制成每1 mL中约含3.3 μg的对照品储备液(1)。精密量取对照品储备液(1)适量,用95%乙腈溶液稀释制成0.396 μg·mL-1的对照品储备液(2)。分别精密量取对照品储备液(1)适量,用95%乙腈溶液稀释制成0.33 μg·mL-1的片剂对照品溶液和0.099 μg·mL-1的原料药对照品溶液。
取片剂细粉适量(约相当于呋塞米20 mg),精密称定,置20 mL量瓶中,加95%乙腈溶液10 mL,超声(功率500 W,频率40 kHz)10 min使溶解,放冷,用95%乙腈溶液稀释至刻度,摇匀,滤过,取续滤液作为片剂供试品溶液。取呋塞米原料药约20 mg,精密称定,置20 mL量瓶中,加95%乙腈溶液10 mL,超声(功率500 W,频率40 kHz)10 min使溶解,放冷,用95%乙腈溶液稀释至刻度,摇匀,作为原料药供试品溶液。
按照G企业提供的资料配制空白辅料,取适量按“2.2.2”项配制空白辅料溶液。
取片剂(F企业,批号5210601)细粉适量(约相当于呋塞米50 mg),9份,精密称定,分别置50 mL量瓶中,精密加入对照品储备液(1)4、5、6 mL,各3份,加95%乙腈溶液20 mL,超声(功率500 W,频率40 kHz)10 min使溶解,放冷,用95%乙腈溶液稀释至刻度,摇匀,滤过,即得。
取呋塞米原料药(I企业,批号00821030013)约20 mg,9份,精密称定,分别置20 mL量瓶中,精密加入对照品储备液(2)4、5、6 mL,各3份,加95%乙腈溶液5 mL,超声(功率500 W,频率40 kHz)10 min使溶解,放冷,用95%乙腈溶液稀释至刻度,摇匀,即得。
精密量取“2.2”项下片剂对照品溶液、空白辅料溶液和原料药回收率供试溶液(中浓度)、片剂回收率供试溶液(中浓度)各20 μL,按“2.1”项下色谱条件测定,记录色谱图(图1)。结果表明,在该色谱条件下,空白辅料溶液在与对照品溶液相同保留时间处无干扰,方法专属性良好。
精密称取糠醛对照品适量,用95%乙腈溶液溶解并稀释制成每1 mL中分别约含糠醛0.012 0、0.060 2、0.120 5、0.240 9、0.361 4、0.602 3 μg的对照品溶液,按照“2.1”项下色谱条件进行检测。以峰面积Y为纵坐标,质量浓度X为横坐标,进行线性回归,得线性方程:
糠醛在0.012 0~0.602 3 μg·mL-1范围内,峰面积与质量浓度呈良好线性关系。
取对照品储备液(2)逐级稀释,按“2.1”项下色谱条件进行测定,以信噪比(S/N)为3时的浓度为检测限,信噪比(S/N)为10时的浓度为定量限,结果检测限为4 ng·mL-1,定量限为12 ng·mL-1。按照G企业提供的资料配制空白辅料,取适量置20 mL量瓶中,加入浓度为12.047 ng·mL-1的对照品溶液10 mL,超声10 min,放冷,用对照品溶液稀释至刻度,摇匀,滤过,取续滤液作为定量限供试品溶液,平行制备6份,按“2.1”项下色谱条件进行测定。糠醛的保留时间分别为5.655、5.651、5.653、5.654、5.655及5.653 min,RSD小于0.10%;糠醛含量计算值分别为12.098、12.052、12.155、12.005、12.126、12.089 ng·mL-1,平均值为12.088 ng·mL-1,RSD为0.44%。
取原料药对照品溶液,按“2.1”项下色谱条件,连续进样6次,糠醛峰面积的RSD(n=6)为0.39%,说明仪器精密度良好。
取呋塞米片(G企业,批号为220101)按“2.2.2”项下片剂供试品溶液配制方法,平行配制6份溶液,按“2.1”项下色谱条件进行测定,糠醛含量平均值为0.008 4%,RSD为0.62%。取呋塞米原料药(I企业,批号为00821030013)按“2.2.2”项下原料药供试品溶液配制方法,平行配制6份溶液,按“2.1”项下色谱条件进行测定,糠醛含量平均值为0.009 0%,RSD为1.9%。
取“2.7”项下片剂和原料药供试品溶液,室温放置0、1、4、8、12、16、20、24 h,按“2.1”项下色谱条件进样分析,记录色谱图并计算主峰峰面积的RSD,片剂为0.87%、原料药为0.72%,表明供试品溶液在室温条件下24 h内稳定。
取“2.2.4”项下的片剂回收率供试溶液和“2.2.5”项下的原料药回收率供试溶液,按“2.1”项下色谱条件分析,计算加样回收率,结果见表2。片剂和原料药中糠醛的总平均回收率(n=9)分别为101.4%和99.4%。
以“2.1”项下色谱条件为基础,调整流速(0.9、1.0、1.1 mL·min-1)、柱温(30、35、40 ℃)和流动相pH(2.55、2.60、2.65),采用Agilent、Kromasil、Phenomenex 3种品牌的色谱柱(规格均为250 mm×4.6 mm,5 μm),取“2.2.4”项下片剂回收率供试溶液(中浓度),按“2.1”项下色谱条件进样测定。结果表明,在不同的流速、柱温、流动相pH、色谱柱品牌条件下,糠醛峰均能与相邻色谱峰有效分离且峰形良好,表明方法耐用性良好。
取呋塞米片样品148批和原料药样品6批,分别按“2.2”项下方法制备供试品溶液,按“2.1”项下色谱条件进样分析。结果所有批次的样品中均检出糠醛,结果详见表3,统计图见图2
各国药典的呋塞米及呋塞米片质量标准中均未收载糠醛检查项,而糠醛作为遗传毒性杂质,可能对人体的遗传物质造成损伤。本研究建立了以磷酸溶液-乙腈为流动相系统的HPLC法,测定呋塞米原料药及其片剂中糠醛的含量。方法灵敏度高,准确度高,稳定性好。
根据2020年版《中华人民共和国药典》四部通则9306遗传毒性杂质控制指导原则[11],按导致50%肿瘤发生率的给药剂量(median toxic dose,TD50)线性外推法计算糠醛限值。指导原则中指出杂质限值一般按下式计算:杂质限值=杂质每日可接受摄入量/药物每日最大用量。在TOXNET数据库查询糠醛的TD50值,以大鼠计为197 mg·kg-1·d-1[12]。采用TD50值的1/50000作为摄入量,则可接受摄入量AI=TD50/50 000×50 kg=197 μg·kg-1·d-1。通过查询马丁代尔药典,口服呋塞米制剂的最大特殊日剂量为2 g[13],故供口服用呋塞米原料药中糠醛的限值=197×10-6/2×100=0.009 85%。查询7个企业的药品说明书,呋塞米片的每日最大用量均为600 mg,故呋塞米片中糠醛的限值=197×10-3/600×100=0.033%。
本次探索性研究中,用已验证的方法对收集到的148批呋塞米片和6批原料药进行糠醛测定,结果表明所有批次的样品中均有检出。其中,片剂中的糠醛检出量均低于参考限度0.033%,但企业F和企业G的产品中的糠醛含量明显高于其余五个企业(企业A~E);原料药测定结果中,企业H的糠醛含量较低,企业I的糠醛平均含量为0.010 2%。经调研,企业A~E的呋塞米原料药均购自企业H,企业F和企业G的呋塞米原料药购自企业I,可见片剂中糠醛的含量呈现出较强的原料药来源相关性。
杂质控制策略实际是分析从起始物料开始的杂质的产生、转化和消除,研究生产工艺对杂质的消除能力,从而确定关键工艺参数范围及起始物料、中间体和原料药的质量控制标准[14]。起始物料是生产控制的起点,是生产出高质量药品的首要关键,为了预防缬沙坦毒性杂质类似事件的发生,应该对起始物料的选择予以高度的重视,国家药监局也陆续发布了相关规定[15]。糠胺作为呋塞米合成的起始物料,企业应严格控制糠胺中糠醛的含量并建立充分的杂质控制策略,能够追踪后续工艺步骤中糠醛的转化和消除,优化相关工艺参数及确定原料药的质量控制标准,最终做到对杂质和产品质量的充分控制。
呋塞米在强光照条件下易降解出杂质糠醛,企业应在实际的贮藏和运输过程中关注避光条件[6]。收集到的7个企业的呋塞米片产品中,2个企业(企业A和B)采用白色塑料瓶为内包装、纸盒为外包装,1个企业(企业C)采用棕色塑料瓶包装,4个企业(企业D、E、F和G)采用白色塑料瓶包装。针对呋塞米见光易降解的特点,企业应选择合适的包装材料和包装工艺,如上述3种包装方式中,棕色瓶和纸盒均可起到避光效果,白色塑料瓶则无法避光。
本次国家药品抽检工作中收集到的148批呋塞米片样品,分别来自生产环节、经营环节和使用环节,抽样分布于31个省级行政区(全国覆盖率为91.2%),样品来源广泛,有较强的代表性。采用本研究建立的糠醛测定方法对片剂和原料药样品进行全检,所有样品中均有检出,且企业I部分批次的原料药产品中糠醛含量高于参考限度,购买企业I原料药的片剂企业的产品糠醛含量也较高,但未超过参考限度0.033%。随着对原料药合成过程中遗传毒性杂质引入认知的加深,应加强杂质控制策略的制定,进而优化原料药的合成路线及生产工艺,建立更加合理的质量标准及质量控制体系,从源头降低原料药中遗传毒性杂质的引入,通过合适的清除手段降低杂质含量,并提高包装水平以降低药物降解产生杂质的风险,最终保障用药安全。
