Objective:

To develop a rapid, specific and sensitive UPLC-MS/MS method for the determination of vonoprazan in human plasma and its application in a bioequivalence study of two types of tablets.

Methods:

A single dose, two-cycle, two products, and self-cross controlled trial design on bioequivalence was used. Plasma samples were collected from healthy human volunteers at different time points after oral administration with the test or reference product of 20 mg fumarate vonoprazan tablets under both fasting and fed conditions, respectively. The plasma samples were treated by acetonitrile protein precipitation and then analyzed by UPLC-MS/MS. Chromatographic separation of vonoprazan was achieved using a Waters ACQUITY UPLC® BEH C18(50 mm × 2.1 mm, 1.7 μm)column at 40 ℃. The mobile phase consisted of water (containing 0.1% formic acid) for eluent A and acetonitrile (containing 0.1% formic acid) for eluent B under a gradient elution. An electrospray ionization (ESI) with multiple reaction monitoring (MRM) mode was used to monitor the precursor-product ion transitions of m/z 346.1→315.4 for vonoprazan and m/z 350.1→316.2 for vonoprazan-d4.

Results:

The rang of linearity was 0.30-50.00 ng·mL-1r>0.998 9), and the LLOQ was 0.30 ng·mL-1. Intra- and inter-day precision values were within 5.7%, and intra- and inter-day accuracy values were ranged from -2.15% to 0.82%. Recovery, specificity, matrix effect and stability met the guiding principles. This method has been successfully applied to study the bioequivalence of vonoprazan fumarate tablets. The Cmax of the test product in postprandial and fasting tests were (29.08±11.59) ng·mL-1 and (26.87±8.14) ng·mL-1, respectively, and the AUC0-t was (258.90±87.71) h·ng·mL-1 and (223.08±43.27) h·ng·mL-1, respectively. The Cmax of the reference product in postprandial and fasting tests were (28.73±10.25) ng·mL-1 and (26.93±8.09) ng·mL-1, respectively, and the AUC0-t was (250.33±73.13) h·ng·mL-1 and (227.56±46.26) h·ng·mL-1, respectively. In the postprandial trial, the 90% CIs for the geometric mean ratios of Cmax, AUC0-t and AUC0-∞ of the test and reference products were 88.64%-112.28%, 96.1%-108.2% and 96.6%-108.7%, respectively. And in the fasting trail, the 90% CIs for the geometric mean ratios of Cmax, AUC0-t and AUC0-∞ of the test and reference products were 94.01%-106.23%, 94.71%-102.03% and 95.18%-102.47%, respectively.

Conclusion:

This validated method has the advantages of simplicity, rapid, and high sensitivity. Test vonoprazan fumarate tablets are bioequivalent to the reference product.

目的:

建立并验证一种简便、快速、灵敏的UPLC-MS/MS法,测定人体血浆中伏诺拉生浓度并用于其2种片剂生物等效性研究。

方法:

采用单次给药、两周期、两制剂、双交叉对照试验设计进行生物等效性研究,健康人体受试者分别在空腹和餐后条件下单剂量口服20 mg富马酸伏诺拉生片受试制剂或参比制剂后在不同时间点采集血浆样品。乙腈蛋白沉淀处理血浆样品后用UPLC-MS/MS法分析。采用Waters ACQUITY UPLC® BEH C18(50 mm × 2.1 mm,1.7 μm)色谱柱,柱温40 ℃,以0.1%甲酸水-0.1%甲酸乙腈为流动相梯度洗脱。ESI离子源,正离子模式,多反应离子监测(MRM),用于定量分析的离子对为m/z 346.1→315.4,内标化合物伏诺拉生-d4m/z 350.1→316.2。

结果:

目的:

建立并验证一种简便、快速、灵敏的UPLC-MS/MS法,测定人体血浆中伏诺拉生浓度并用于其2种片剂生物等效性研究。

方法:

采用单次给药、两周期、两制剂、双交叉对照试验设计进行生物等效性研究,健康人体受试者分别在空腹和餐后条件下单剂量口服20 mg富马酸伏诺拉生片受试制剂或参比制剂后在不同时间点采集血浆样品。乙腈蛋白沉淀处理血浆样品后用UPLC-MS/MS法分析。采用Waters ACQUITY UPLC® BEH C18(50 mm × 2.1 mm,1.7 μm)色谱柱,柱温40 ℃,以0.1%甲酸水-0.1%甲酸乙腈为流动相梯度洗脱。ESI离子源,正离子模式,多反应离子监测(MRM),用于定量分析的离子对为m/z 346.1→315.4,内标化合物伏诺拉生-d4m/z 350.1→316.2。

结果:

线性范围为0.30~50.00 ng·mL-1r>0.998 9),定量下限为0.30 ng·mL-1;批内和批间精密度RSD均<5.7%,准确度在-2.15%~0.82%范围内;提取回收率、选择性、基质效应、稳定性等均符合接受标准。该方法被成功应用于富马酸伏诺拉生片的生物等效性研究。受试制剂在餐后和空腹试验中Cmax分别为(29.08±11.59)ng·mL-1和(26.87±8.14)ng·mL-1,AUC0-t分别为(258.90±87.71)h·ng·mL-1和(223.08±43.27)h·ng·mL-1。参比制剂在餐后和空腹试验中Cmax分别为(28.73±10.25)ng·mL-1和(26.93±8.09)ng·mL-1,AUC0-t分别为(250.33±73.13)h·ng·mL-1和(227.56±46.26)h·ng·mL-1。餐后试验中,受试制剂和参比制剂的Cmax、AUC0-t、和AUC0-∞几何均值比的90%置信区间分别为88.64%~112.28%、96.1%~108.2%、96.6%~108.7%;空腹试验中,受试制剂和参比制剂的的Cmax、AUC0-t和AUC0-∞几何均值比的90%置信区间分别为94.0%~106.2%、94.7%~102.0%和95.2%~102.5%。

结论:

该方法具有简单、快速,灵敏度高等优点。富马酸伏诺拉生片受试制剂和参比制剂具有生物等效性。

Objective:

To develop a rapid, specific and sensitive UPLC-MS/MS method for the determination of vonoprazan in human plasma and its application in a bioequivalence study of two types of tablets.

Methods:

A single dose, two-cycle, two products, and self-cross controlled trial design on bioequivalence was used. Plasma samples were collected from healthy human volunteers at different time points after oral administration with the test or reference product of 20 mg fumarate vonoprazan tablets under both fasting and fed conditions, respectively. The plasma samples were treated by acetonitrile protein precipitation and then analyzed by UPLC-MS/MS. Chromatographic separation of vonoprazan was achieved using a Waters ACQUITY UPLC® BEH C18(50 mm × 2.1 mm, 1.7 μm)column at 40 ℃. The mobile phase consisted of water (containing 0.1% formic acid) for eluent A and acetonitrile (containing 0.1% formic acid) for eluent B under a gradient elution. An electrospray ionization (ESI) with multiple reaction monitoring (MRM) mode was used to monitor the precursor-product ion transitions of m/z 346.1→315.4 for vonoprazan and m/z 350.1→316.2 for vonoprazan-d4.

Results:

The rang of linearity was 0.30-50.00 ng·mL-1r>0.998 9), and the LLOQ was 0.30 ng·mL-1. Intra- and inter-day precision values were within 5.7%, and intra- and inter-day accuracy values were ranged from -2.15% to 0.82%. Recovery, specificity, matrix effect and stability met the guiding principles. This method has been successfully applied to study the bioequivalence of vonoprazan fumarate tablets. The Cmax of the test product in postprandial and fasting tests were (29.08±11.59) ng·mL-1 and (26.87±8.14) ng·mL-1, respectively, and the AUC0-t was (258.90±87.71) h·ng·mL-1 and (223.08±43.27) h·ng·mL-1, respectively. The Cmax of the reference product in postprandial and fasting tests were (28.73±10.25) ng·mL-1 and (26.93±8.09) ng·mL-1, respectively, and the AUC0-t was (250.33±73.13) h·ng·mL-1 and (227.56±46.26) h·ng·mL-1, respectively. In the postprandial trial, the 90% CIs for the geometric mean ratios of Cmax, AUC0-t and AUC0-∞ of the test and reference products were 88.64%-112.28%, 96.1%-108.2% and 96.6%-108.7%, respectively. And in the fasting trail, the 90% CIs for the geometric mean ratios of Cmax, AUC0-t and AUC0-∞ of the test and reference products were 94.01%-106.23%, 94.71%-102.03% and 95.18%-102.47%, respectively.

Conclusion:

This validated method has the advantages of simplicity, rapid, and high sensitivity. Test vonoprazan fumarate tablets are bioequivalent to the reference product.

酸相关性疾病(acid-related disorders,ARDs)是消化系统疾病中常见的一类疾病,一般是由胃酸分泌过多或对胃酸特别敏感所致。质子泵抑制剂(proton pump inhibitors,PPIs)可有效地抑制胃酸分泌,是治疗胃酸相关疾病的一线药物[1]。然而,PPIs疗效和使用存在局限性,如半衰期短,夜间抑酸效果欠佳,起效缓慢以及由于CYP2C19代谢导致患者疗效差异大等[2-4]。长期服用PPIs会产生骨折风险增加、感染(肺炎和艰难梭菌感染)、营养不良(维生素B12、镁和铁吸收受损)、贫血等不良反应[5-7]
富马酸伏诺拉生(vonoprazan fumarate,FNL)是一种新型的钾竞争性酸阻滞剂(potassium-competitive acid blockers,P-CAB),其竞争性阻断胃H+/K+-ATP酶的钾结合位点[8]。与PPIs相比,伏诺拉生在药代动力学和药效学方面更有优势,比如:起效快,抑酸作用更强和持久[9-13],耐受性更好[14]。临床上用于治疗侵蚀性食道炎、胃溃疡、十二指肠溃疡、消化性溃疡、胃食管反流、反流性食道炎和根除幽门螺杆菌[15-17]。陈博伦等[18]采用LC-MS/MS法测定比格犬血浆中伏诺拉生的浓度,定量下限为5 ng·mL-1,难以达到本试验要求目标定量下限。Tomoki等[19]采用蛋白沉淀后上清液氮气吹干复溶的方式进行样品前处理,过程不够简单,不适用于大批量样品的处理。本文旨在建立并验证一个简便、快速、灵敏的UPLC-MS/MS法测定人口服FNL片后血浆中伏诺拉生浓度并用于生物等效性研究。
LC-30AD超高效液相色谱系统(岛津公司),TQ 6500+三重四极杆串联质谱仪(AB SCIEX公司),Analyst 1.6.3(AB SCIEX公司)软件、Watson LIMS 7.5 SP1(Thermo Fisher公司)软件、Phoenix WinNonlin软件(V8.0,Pharsight公司)。
FNL对照品(含量99.1%,批号2140-010A1)和FNL-d4对照品(含量98.1%,批号3486-034A1)均购于TLC公司。受试制剂为FNL片(扬子江药业集团四川海蓉药业有限公司,批号20019002,含量100.4%,规格20 mg·片-1),参比制剂为Takecab®(武田药品工业株式会社公司,批号HH1214,含量102.3%,规格20 mg·片-1)。实验用甲醇、乙腈、异丙醇均为HPLC级,甲酸及乙酸铵为LC/MS级,纯水为Milli-Q型超纯水仪制备的超纯水。
采用Waters ACQUITY UPLC® BEH C18(50 mm×2.1 mm,1.7 μm)色谱柱,预柱为Waters ACQUITY UPLC® BEH VanGuardTM Pre-Column(5 mm×2.1 mm,1.7 μm)色谱柱,柱温40 ℃,以0.1%甲酸水(A)-0.1%甲酸乙腈(B)为流动相,梯度洗脱(洗脱程序见表1),流速0.5 mL·min-1,进样体积2 μL。
采用电喷雾电离(ESI)正离子化,喷雾电压为5 500 V,GS1和GS2气压为310.3 kPa,气帘气气压为241.3 kPa,喷雾温度为450 ℃。FNL和FNL-d4用于定量分析的离子对为m/z 346.1→315.2和m/z 350.2→316.3,FNL和FNL-d4的碰撞电压(CE)分别为14、15 V,入口电压(EP)分别为12、8 V,去簇电压(DP)、碰撞室出口电压(CXP)均为20、10 V。FNL和FNL-d4化学结构及MRM模式下离子碎片见图1
精密称取适量FNL和FNL-d4对照品,根据校正因子折算其质量后,加入适量甲醇各配制成0.1 mg·mL-1的储备液,于-20 ℃条件下保存。
工作液由50%甲醇溶液序列稀释储备液得到。标准曲线工作液浓度为6、12、36、100、240、500、800、1 000 ng·mL-1;质控工作液浓度为6、18、80、400、750 ng·mL-1;内标工作液浓度为70 ng·mL-1
本试验通过南京中医药大学附属医院(江苏省中医院)伦理委员会批准(登记号为CTR20201301;备案号为202000167-01),于江苏省中医院Ⅰ期临床试验研究室进行,采用单次给药、两周期、两制剂、双交叉对照试验设计进行生物等效性研究。招募18岁及以上男性和女性健康受试者共48名(餐后试验和空腹试验各24名)。受试者在空腹或高脂餐后状态下口服受试制剂(T,20 mg)或参比制剂(R,20 mg)1片,7 d清洗期后进行第二周期交叉服药。空腹采血时间点为给药前1 h内(0 h)和服药后0.25、0.5、0.75、1、1.33、1.67、2、2.33、2.67、3、3.5、4、6、8、12、24、36和48 h;餐后采血时间点为给药前1 h内(0 h)和服药后0.33、0.67、1、1.33、1.67、2、2.33、2.67、3、3.33、3.67、4、4.5、5、6、8、12、24、36和48 h。室温白光条件下采集静脉血于含有肝素钠抗凝剂的采血管中,在1 h内于2 000 g(离心预设温度4 ℃)离心10 min后将血浆取出置于冻存管中,储存于-70 ℃冰箱中待分析。
取血浆样品100 μL,加入内标工作液(70 ng·mL-1)20 μL,涡旋混匀后,加入乙腈380 μL,1 300 r·min-1混匀提取5 min,再4 ℃,2 120 g离心10 min,取上清液100 μL,加入10%乙腈(含0.1%甲酸)溶液100 μL,涡旋混匀后,取2 μL进样分析。
使用WinNonlin软件(ver 8.2)采用非房室模型对临床样品中伏诺拉生的血药浓度数据进行药代动力学参数计算,经对数转换后,采用多因素方差分析(ANOVA)、双单侧t检验和90%置信区间进行统计分析,评价和判断2种制剂的生物等效性。
本文中方法学验证按照2020年版《中华人民共和国药典》四部附录9012——生物样品定量分析方法验证指导原则进行[20]
取6个批次不添加待测物和内标的空白血浆、6个批次空白血浆制备的定量下限血浆样品,按照“2.4”项下方法进行样品预处理后按“2.1”项条件进行分析,考察空白基质中内源性化合物对待测物和内标的影响。如图2所示,FNL和FNL-d4的保留时间分别约为1.044和1.041 min,结果表明空白基质中的内源性化合物对待测物和内标均无干扰。
此外,当FNL(1 000 ng·mL-1)和FNL-d4(70 ng·mL-1)单独添加至空白基质中时,待测物通道最大峰面积和内标通道最大峰面积小于该批次有效标准曲线定量下限样品中待测物平均峰面积的5.46%和0.04%,均符合规定标准,待测物和同位素内标互不干扰。
以19∶1的体积比向空白血浆中加入标准曲线工作液得到血浆样品(质量浓度为0.30、0.60、1.80、5.00、12.00、25.00、40.00、50.00 ng·mL-1)。按照“2.4”项下方法进行样品预处理后按“2.1”项下条件进行分析,以待测物与内标峰面积比值Y为纵坐标,待测物浓度X为横坐标,权重为1/X2,进行线性回归。回归方程:
所有分析批标准曲线均符合规定标准。表明FNL在0.30~50.00 ng·mL-1线性关系良好,定量下限为0.30 ng·mL-1
以19∶1的体积比向空白血浆中加入质控工作液得到血浆样品,质控血浆样品浓度为0.30(定量下限质控样品)、0.90(低浓度质控样品)、4.00(几何中浓度质控样品)、20.00(中浓度质控样品)、37.50 ng·mL-1(高浓度质控样品)。3 d配制的不同批次的上述各浓度质控血浆样品溶液平行制备6份预处理后进样分析,考察批内和批间精密度和准确度。精密度RSD≤15.0%(对于定量下限质控样品,RSD≤20.0%);准确度的相对偏差(Bias,%)在±15.0%之间(对于定量下限质控样品,Bias在±20.0%之间)。结果符合规定标准,见表2
低(0.90 ng·mL-1)、中(20.00 ng·mL-1)、高(37.50 ng·mL-1)3个浓度的质控血浆样品,按照“2.4”项条件下进行样品预处理后,得到提取回收率供试溶液;空白血浆提取后的上清液中加入FNL和FNL-d4制备得到低、中、高3个浓度的质控样品,得到提取回收率参比样品。比较各浓度提取回收率供试溶液和提取回收率参比样品中FNL和FNL-d4峰面积考察提取回收率。
表3所示,低、中、高3个浓度下质控浓度水平的平均提取回收率均>50.0%且RSD<15%,符合标准规定。
6个批次的空白血浆提取后的上清液中加入FNL和FNL-d4制备得到低(0.90 ng·mL-1)、中(20.00 ng·mL-1)、高(37.50 ng·mL-1)3个浓度的质控样品作为基质样品;另以不含基质成分的纯溶液制备与上述具有FNL和FNL-d4相同浓度的溶液作为参比溶液,以各浓度的质控样品与参比溶液的比值计算基质效应。如表3所示,对于不同来源的6个批次血浆质控样品,内标归一化基质效应因子的RSD<1.8%,符合标准规定。此外,2%溶血血浆和2%高脂血浆基质制备低、中、高3个浓度的测定浓度与理论浓度的偏差在±15.0%之间(RSD<15.0%),满足指导原则要求。
按照实际样品在测定过程中所涉及的稳定性要求进行考察。低(0.90 ng·mL-1)、高(37.50 ng·mL-1)浓度质控血浆样品,放置于考察条件一段时间后,用新鲜制备标准曲线样品测定浓度与理论浓度的偏差应在±15.0%以内。如表4所示,FNL血浆样品在室温白光条件下放置48 h、在-20 ℃和-70 ℃条件下放置35 d、反复冻融6次均稳定;经前处理后的血浆样品在自动进样器内(8 ℃)放置51 h稳定。此外,FNL全血样品在室温白光条件下放置2 h稳定、精密度和准确度样品进样完成后再在自动进样器内(8 ℃)放置18 h稳定。FNL和FNL-d4储备液在室温白光条件下放置48 h稳定,在-20 ℃条件下放置22 d稳定;工作液在室温白光条件下放置45 h稳定,在4 ℃条件下放置21 d稳定。
用空白血浆以1∶2体积比稀释稀释质控(90.00 ng·mL-1)血浆样品,测定浓度与理论浓度偏差范围在±15.0%之间(RSD<15.0%),表明血浆样品稀释3倍结果可靠。定量上限样品进样后的空白样品中,待测物和内标残留分别小于在定量下限样品中FNL峰面积平均值的20.0%和FNL-d4峰面积平均值的5.0%,残留不会影响样品定量。分析批最大连续进样230针样品,质控样品精密度和准确度均符合标准。
在1 920个有效样品中选择在Cmax附近和消除相样品的198个未知样品进行试验样品再分析(ISR),同一受试者的ISR在其2个周期样品检测完成后开展。以重分析浓度与首次测定浓度的差值除以二者均值计算偏差,要求至少2/3的ISR样品的偏差在±20.0%以内。结果显示,192个样品的偏差范围为-5.5%~5.9%,通过率为100%。
健康人体受试者空腹和餐后服用受试制剂和参比制剂后的平均血药浓度-时间曲线见图3,主要药代动力学参数见表5
餐后试验和空腹试验各24例受试者纳入生物等效性数据集(BES),按照“2.5”项下进行生物等效性分析。如表6所示,餐后试验中和空腹试验中,受试制剂和参比制剂的Cmax、AUC0-t、AUC0-∞几何均值比90%置信区间分别为:88.64%~112.28%和94.01%~106.23%、96.06%~108.20%和94.71%~102.03%、96.60%~108.65%和95.18%~102.47%,均在80.00%~125.00%[21],受试制剂和参比制剂生物等效。
本试验建立了简便、快速、灵敏的UPLC-MS/MS法测定人血浆中伏诺拉生的浓度。本试验中,比较0.1%甲酸水-0.1%甲酸乙腈和0.1%甲酸水-0.1%甲酸甲醇作为流动相进行梯度洗脱,由于乙腈比甲醇的洗脱能力更强,使用0.1%甲酸乙腈作为流动相B可使待测物的色谱峰峰形更好。另外,当流动相中含有低浓度乙酸铵时可使化合物响应增高。本试验采用甲醇-异丙醇-水-甲酸-10 mol·L-1乙酸铵(40∶40∶20∶0.5∶0.05,v/v)作为洗针液,洗脱程序设置抽吸前后洗针可以有效地降低样品针残留。采用同位素化合物作为内标,减少样品前处理及分析过程中带来的误差,结果更加准确可靠。采用乙腈蛋白沉淀方法进行预处理,线性范围为0.30~50.00 ng·mL-1,具有灵敏度高,基质效应小,选择性好,检测效率高等优点,适合高通量检测人血浆中伏诺拉生的浓度,为富马酸伏诺拉生片的生物等效性研究提供了一种可靠的分析方法。
文献报道,日本健康男性空腹口服给药伏诺拉生20 mg时,Cmax为(25.0± 5.6)ng·mL-1tmax>2 h,t1/2在5.1~8.7 h,第一天和第七天重复给药无显著差别[21-22]。在本试验中,餐后试验和空腹试验tmax在1.51~2.49 h,餐后达峰时间比空腹晚,因此,食物可延缓伏诺拉生的吸收。餐后给药受试制剂和参比制剂的Cmax分别为29.08和28.73 ng·mL-1Cmax在受试者之间的体内RSD为24.2%。空腹给药受试制剂和参比制剂的Cmax分别为26.87和26.93 ng·mL-1Cmax在受试者之间的体内RSD为12.4%。因此,该药物制剂非高变异。在餐后和空腹状态下的Cmax、AUC0-t、AUC0-∞经对数转换后几何均值比的90%置信区间均符合规定标准,即在吸收速度、程度上均无差异。因此,认为伏诺拉生受试制剂和参比制剂在空腹和餐后状态下生物等效。
