Objective:

To establish an inductively coupled plasma mass spectrometry (ICP-MS) method for the determination of 25 potentially risky elemental impurities in alfentanil hydrochloride injection.

Methods:

The Agilent 7800 ICP-MS inductively coupled plasma mass spectrometer was used, the conventional tuning mode was used, the RF power was 1 550 W, the plasma gas flow rate was 15 L·min-1, the matrix effect was eliminated by external standard, and the sample was diluted for direct injection.

Results:

The method can simultaneously determine the content of 25 kinds of elemental impurities, and its linear relationship was good (r >0.99). The RSD of the repeatability test was ≤10% (n=6), and the recovery rate was 80.0%-120.0% (n=9), all of which met the requirements of methodological validation.

Conclusion:

The content of elemental impurities in alfentanil hydrochloride injection is lower than 30% of the limit specified in ICH·Q3D, which will not bring safety risks to the drug, providing a reference for the quality control and risk assessment of elemental impurities in other similar drugs.

目的:

建立电感耦合等离子体质谱法(ICP-MS),测定盐酸阿芬太尼注射液中25种具有潜在风险的元素杂质的含量。

方法:

采用Agilent 7800 ICP-MS电感耦合等离子体质谱仪,运用常规调谐模式,射频功率1 550 W,等离子气体流量15 L·min-1,通过外标方式消除基质效应,样品稀释直接进样测定。

结果:

该方法能同时测定25种元素杂质含量,其线性关系良好(r>0.99),重复性试验的RSD≤10%(n=6),回收率在80.0%~120.0%(n=9),均满足方法学验证的要求。

结论:

盐酸阿芬太尼注射液元素杂质含量均低于ICH·Q3D规定限度的30%,不会给药品带来安全性风险,本法为其他相似药品元素杂质的质量控制和风险评估提供了参考。

Objective:

To establish an inductively coupled plasma mass spectrometry (ICP-MS) method for the determination of 25 potentially risky elemental impurities in alfentanil hydrochloride injection.

Methods:

The Agilent 7800 ICP-MS inductively coupled plasma mass spectrometer was used, the conventional tuning mode was used, the RF power was 1 550 W, the plasma gas flow rate was 15 L·min-1, the matrix effect was eliminated by external standard, and the sample was diluted for direct injection.

Results:

The method can simultaneously determine the content of 25 kinds of elemental impurities, and its linear relationship was good (r >0.99). The RSD of the repeatability test was ≤10% (n=6), and the recovery rate was 80.0%-120.0% (n=9), all of which met the requirements of methodological validation.

Conclusion:

The content of elemental impurities in alfentanil hydrochloride injection is lower than 30% of the limit specified in ICH·Q3D, which will not bring safety risks to the drug, providing a reference for the quality control and risk assessment of elemental impurities in other similar drugs.

近年来,各国药品监管部门以及药典规范对元素杂质的控制要求都愈加严格,而注射液在生产、流通、储存及使用过程中长时间与药包材接触,可能对包材的内表面发生侵蚀,甚至使其脱落,导致一些有害元素迁移到注射液中[1-2]。为了控制药品质量,防范元素杂质导致的安全性风险,应对药品生产过程中涉及的水、辅料、原料药和所有合成中间体的元素杂质进行控制和监测[3-4]。人用药品注册技术要求国际协调会议(ICH)“Q3D元素杂质指南”[5]基于风险管控的原则,将药品中存在风险的24种元素杂质分为4类(1类、2A类、2B类、3类),并针对口服、注射和吸入制剂等不同剂型规定不同的限值(元素杂质每日最大允许摄入量,PDE)[6]。药物中元素杂质种类较多,元素杂质检测工作艰巨而复杂,电感耦合等离子体原子发射光谱(ICP-AES)法[7-8]和电感耦合等离子体质谱(ICP-MS)法[9-11],因其独特优势而成为USP推荐的元素杂质检测方法,并在药品行业得到广泛的使用。
ICP-MS法作为分析痕量元素的1种方法[12-13],可同时检测多种元素,且线性范围宽,分析能力强,速度快,灵敏度高[14-16]。由于元素杂质不能为患者提供任何治疗效果,为了降低对人体的毒害作用,其含量要控制在可接受的范围内[17-18]。目前,盐酸阿芬太尼注射液中元素杂质的检测尚未见报道,本文根据国家食品药品监督管理局的政策要求,采用ICP-MS法对盐酸阿芬太尼注射液中可能存在的锂(Li)、锑(Sb)、钡(Ba)、铁(Fe)、锌(Zn)、铬(Cr)、铜(Cu)、钒(V)、钴(Co)、镍(Ni)、砷(As)、镉(Cd)、汞(Hg)、铅(Pb)、硼(B)、硅(Si)、铝(Al)、镁(Mg)、钾(K)、钙(Ca)、钛(Ti)、锰(Mn)、锡(Sn)、铈(Ce)、铋(Bi)共25种元素的含量进行测定,为盐酸阿芬太尼注射液元素杂质限量标准的制定提供了依据。
Agilent 7800 ICP-MS电感耦合等离子体质谱仪(安捷伦公司),Mettler XSE205DU十万分之一电子天平(梅特勒托利多公司),硝酸(苏州晶瑞化学股份有限公司),氯化钠(江苏省勤奋药业有限公司),水为超纯水。
铁、锌、硼、硅、铝、钾、钙、铈(1 000 μg·mL-1)单元素标准溶液(批号198009-2、193005-4、196028-2、198004、196047-7、19B030-7、198034-2、197026)、多元素(100 μg·mL-1)标准溶液(含锡、钛、锰、铋、镁各元素均为100 mg·L-1)(批号201002-4)均由国家有色金属及电子材料分析测试中心提供;Elemental Impurities according to ICH Q3D parenteral,Standard 1(含镉元素2 mg·L-1,汞元素3 mg·L-1,铅、钴元素5 mg·L-1,砷元素15 mg·L-1,镍元素20 mg·L-1,钒元素10 mg·L-1)(批号BCBZ3573)、Elemental Impurities according to ICH Q3D parenteral,Standard 3(含锂元素25 mg·L-1,含锑元素9 mg·L-1,钡元素70 mg·L-1,铜元素30 mg·L-1,铬元素110 mg·L-1)(批号BCBZ9593)均由Sigma-Aldrich提供。
样品盐酸阿芬太尼注射液:3批规格为2 mL:1 mg,批号SAF200801、SAF200802、SAF200803;3批规格为10 mL:5 mg,批号SAT200801、SAT200802、SAT200803,均为江苏恩华药业股份有限公司自制。
射频功率:1 550 W;采样深度:8 mm;等离子气体流量:15 L·min-1;蠕动泵转速:0.3 r·min-1;蠕动泵样品引入时间:40 s;调谐模式:常规;重复次数:3;扫描次数:100;积分时间:0.3 s(汞2 s,砷1 s,镉1 s);蠕动泵稳定时间:30 s。
根据《ICH Harmonised Guideline Guideline For Elemental Impurities·Q3D》(简称“ICH·Q3D”)和《化学药品注射剂与药用玻璃包装容器相容性研究技术指导原则(试行)》的建议,对注射液中的一些元素进行风险评估,盐酸阿芬太尼注射液每日最大剂量为307.65 mg,有2 mL:1 mg、10 mL:5 mg 2种规格,根据各元素的PDE,参照ICH·Q3D,按照每日最大剂量为307.65 mg计算盐酸阿芬太尼注射液中各元素的理论计算限度、分析评价阈值(AET)及控制阈值,制定各元素的限度控制指标,其中,钾、钙、镁、钛、铈、铋离子因为毒性较低或在地方法规中的要求不同,其PDE还没有建立,见表1
精密量取盐酸阿芬太尼注射液1.0 mL,置10 mL聚丙烯量瓶中,用2%硝酸溶液(向500 mL聚丙烯量瓶中,加入适量超纯水,再加入硝酸10 mL,用超纯水定容,摇匀,即得)稀释至刻度,摇匀,即得。
取氯化钠约0.45 g,精密称定,置500 mL聚丙烯量瓶中,加入适量超纯水溶解,再加入硝酸10 mL,用超纯水定容,摇匀,即得对照品稀释液(含0.09%氯化钠的2%硝酸溶液)。
分别精密量取铁、锌元素标准溶液各0.13 mL;硼、硅元素标准溶液各0.5 mL;Elemental Impurities according to ICH Q3D parenteral,Standard 1 0.1 mL,置10 mL聚丙烯量瓶中,用对照品稀释液稀释至刻度,摇匀,即得对照品溶液A1;精密量取铝元素标准溶液0.1 mL,置10 mL聚丙烯量瓶中,用对照品稀释液稀释至刻度,摇匀,即得对照品溶液A2;分别精密量取Elemental Impurities according to ICH Q3D parenteral,Standard 3 0.25 mL、对照品溶液A1 2.5 mL、对照品溶液A2 1.25 mL,置25 mL聚丙烯量瓶中,用对照品稀释液至刻度,摇匀,即得对照品溶液A。
精密量取多元素标准溶液1 mL和铈元素标准溶液0.1 mL,置10 mL聚丙烯量瓶中,用对照品稀释液稀释至刻度,摇匀,即得对照品溶液B1;分别精密量取对照品溶液B1 0.4 mL、钙和钾元素标准溶液各0.1 mL,置10 mL聚丙烯量瓶中,用对照品稀释液稀释至刻度,摇匀,即得对照品溶液B。
精密量取对照品溶液A 1 mL,置10 mL聚丙烯量瓶中,加对照品稀释液稀释至刻度,摇匀,即得加标对照品溶液A;精密量取盐酸阿芬太尼注射液1 mL,置10 mL聚丙烯量瓶中,再分别加入对照品溶液A 1 mL,用2%硝酸溶液稀释至刻度,摇匀,即得加标供试品溶液A。同法制备加标对照品溶液B、加标供试品溶液B。
分别精密量取对照品溶液A 0、0.2、0.5、1.0、1.5、2.0 mL,置10 mL量瓶中,用对照品稀释液分别稀释至刻度,摇匀,即得线性测定溶液A;同法制备线性测定溶液B。
取批号为SAF200801的盐酸阿芬太尼注射液作为代表溶液,按照“2.3.1”“2.3.3”项下方法制备供试品溶液、加标对照品溶液A,加标对照品溶液B、加标供试品溶液A、加标供试品溶液B,以对照品稀释液作为空白溶液共6份样品,进样测定响应值(CPS),结果见表2。比值1均<10%,说明空白溶液干扰小;比值2在70%~125%范围内,表明基质无较强干扰,满足专属性要求。
精密量取“2.3.2”项下对照品溶液A 1 mL,置10 mL聚丙烯量瓶中,加对照品稀释液稀释至刻度,摇匀,即得系统适用性溶液A;同法制备系统适用性溶液B。连续进样6次检测,结果见表3。各元素响应值RSD≤10%,说明方法适用性良好。
取“2.3.4”项下线性测定溶液A、B,进样测定响应值,以响应值Y为纵坐标,浓度X为横坐标,进行线性回归,结果见表4。25种元素线性相关系数r均>0.99,说明线性关系良好。
取“2.3.2”项下的空白溶液(即对照品稀释液),连续进样7次测定CPS,计算各元素浓度的标准偏差。按3 SD/r计算得到检测限,10 SD/r计算得到定量限(SD为空白溶液各元素CPS的标准偏差,r为相应元素标准曲线的斜率),结果见表4
取批号为SAF200801的盐酸阿芬太尼注射液6支,混合均匀,分别精密量取1 mL,置于不同10 mL聚丙烯量瓶中,再分别加入“2.3.2”项下对照品溶液A 1 mL,用2%硝酸溶液稀释至刻度,摇匀,即得重复性溶液A;同法制备重复性溶液B。将12份样品依次进样检测,结果见表5;各元素浓度的RSD≤10.0%,说明该方法重复性良好。
精密量取“2.3.2”项下对照品溶液A 1 mL,加对照品稀释液稀释至10 mL,摇匀,即得对照品稳定性溶液A,取批号为SAF200801的盐酸阿芬太尼注射液1支,精密量取1 mL,加入“2.3.2”项下对照品溶液A 1 mL,用2%硝酸溶液稀释至10 mL,摇匀,即得供试品稳定性溶液A;同法制备对照品稳定性溶液B、供试品稳定性溶液B。将4份稳定性溶液室温放置8 h,分别于0、2、4、6、8 h时进样检测,结果见表6。各元素浓度的RSD≤8.0%,说明溶液在8 h内稳定。
取批号为SAF200801的盐酸阿芬太尼注射液9支,混合均匀,分别精密量取1 mL,置于不同10 mL量瓶中,分为3份,分别加入“2.3.2”项下的对照品溶液A 0.5、1.0、1.5 mL,用2%硝酸溶液稀释至刻度,摇匀,即得供试溶液A;同法制备供试溶液B。将18份样品依次进样检测,结果见表7。各元素在各浓度点的回收率在80.0%~120.0%,RSD≤10.0%,说明方法准确度高。
将6批次盐酸阿芬太尼注射液按照“2.3.1”项下方法制备供试品溶液,依次进样检测,结果见表8
保证药品安全、有效、质量可控是药品研发和评价应遵循的基本原则,其中对药品进行质量控制是保证药品安全有效的基础和前提,也是确保药物质量最重要的挑战之一[19]。药品中杂质分析是药品质量控制的重要组成部分,近年来,由于对药物中杂质限度要求的不断提高,许多药典杂质分析方法已经修订或更新。药品中的元素杂质不仅影响药物的稳定性,而且可能存在潜在毒性,进而可能引起健康风险,因此必须严格控制药品中元素杂质的含量[8]
本研究建立了定量测定盐酸阿芬太尼注射液中25种元素杂质含量的ICP-MS法,该方法具有多元素同时分析、各元素线性范围宽、灵敏度和精密度高等优点,为其他相似药品元素杂质的质量控制和风险评估提供了参考。
