Objective:

To develop the national standard materials of progesterone in frozen human serum and to evaluate the accuracy and standardization of serum progesterone detection kits.

Methods:

Low and high concentrations of serum were from normal people and pregnant women individuals respectively,with a clear appearance,no obvious hemolysis,jaundice and lipid blood. After all tests for four infectious diseases were negative,thrombin,anhydrous calcium chloride and anhydrous sodium carbonate were added according to the proportion of 1 kU·L-1,2.219 6 g·L-1 and 2.65 g·L-1 respectively,and centrifuged (4 000 r·min-1,4 ℃,25 min) after fully mixing. After twice of multiple filtration and sterilization,serum pools were packed in ampoules to prepare candidates Ⅰ and Ⅱand stored at -70 ℃. Single factor variance method and linear regression method were used to evaluate the homogeneity and stability of the candidate. Five labs working together used reference method (isotope dilution liquid chromatography tandem mass spectrometry) to assign value for candidate. The uncertainty was calculated. At the same time,the commutability was evaluated.

Results:

Through statistical analysis, the F values of homogeneity for candidate Ⅰ and Ⅱ were 0.570 9 and 1.200 9, respectively, which were all less than F0.05. Candidate Ⅰ and Ⅱ could be stable at least 8 h at 20-25 ℃. At 2-8 ℃, candidate Ⅰ and Ⅱ could be stable at least 33 d and 7 d respectively. Candidate Ⅰ and Ⅱ cound be stable at least 33 d at -20 ℃. Candidate Ⅰ and Ⅱ remained stable after repeated freeze-thaw cycles for 6 times. The assigned values were:candidate Ⅰ:(4.44±0.24) ng·mL-1 (k=2), Ⅱ:(16.79±0.82) ng·mL-1 (k=2). The determination results of candidate Ⅰ and Ⅱ were all within the 95% confidence interval of the fitting regression line established by the determination values of fresh serum samples.

Conclusion:

Progesterone candidate had good homogeneity,stability,interoperability. It had accurate and reliable determination value,which could be used as national standard. The establishment of this national standard was of great significance for promoting the consistency and standardization of progesterone testing results.

目的:

研制孕酮冰冻人血清国家标准品,用于血清孕酮检测试剂盒的正确度评价及标准化研究。

方法:

分别收集来自正常人及孕妇的血清,且外观澄明、无明显溶血、黄疸和脂血,传染病四项检测均为阴性后,按比例分别添加凝血酶1 kU·L-1,无水氯化钙2.219 6 g·L-1和无水碳酸钠2.65 g·L-1,充分混匀后,离心(4 000 r·min-1,4 ℃,25 min);再经2次多重过滤除菌后分装,制备成候选品Ⅰ和Ⅱ,-70 ℃条件下保存。分别采用单因素方差法和线性回归法评估候选品的均匀性和稳定性。联合5家实验室采用参考方法(同位素稀释液相色谱串联质谱法)定值,并计算不确定度。同时对候选品互通性进行评估。

结果:

经统计分析,候选品Ⅰ、Ⅱ的均匀性检验F值分别为0.570 9和1.200 9,均<F0.05;候选品Ⅰ、Ⅱ在(20~25)℃至少可稳定8 h;在(2~8)℃至少分别可稳定33 d和7 d;在-20 ℃至少可稳定33 d。候选品Ⅰ、Ⅱ反复冻融6次后稳定。候选品Ⅰ、Ⅱ定值结果为:Ⅰ:(4.44±0.24)ng·mL-1k=2);Ⅱ:(16.79±0.82)ng·mL-1k=2)。候选品Ⅰ、Ⅱ测定结果均在新鲜血清样本测定值所建立的拟合回归直线95%置信区间范围内。

结论:

孕酮候选品均匀性、稳定性、互通性良好,定值准确可靠,可作为国家标准品应用。该国家标准品的建立,对促进孕酮检测结果的一致化和标准化具有重要意义。

目的:

研制孕酮冰冻人血清国家标准品,用于血清孕酮检测试剂盒的正确度评价及标准化研究。

方法:

分别收集来自正常人及孕妇的血清,且外观澄明、无明显溶血、黄疸和脂血,传染病四项检测均为阴性后,按比例分别添加凝血酶1 kU·L-1,无水氯化钙2.219 6 g·L-1和无水碳酸钠2.65 g·L-1,充分混匀后,离心(4 000 r·min-1,4 ℃,25 min);再经2次多重过滤除菌后分装,制备成候选品Ⅰ和Ⅱ,-70 ℃条件下保存。分别采用单因素方差法和线性回归法评估候选品的均匀性和稳定性。联合5家实验室采用参考方法(同位素稀释液相色谱串联质谱法)定值,并计算不确定度。同时对候选品互通性进行评估。

结果:

经统计分析,候选品Ⅰ、Ⅱ的均匀性检验F值分别为0.570 9和1.200 9,均<F0.05;候选品Ⅰ、Ⅱ在(20~25)℃至少可稳定8 h;在(2~8)℃至少分别可稳定33 d和7 d;在-20 ℃至少可稳定33 d。候选品Ⅰ、Ⅱ反复冻融6次后稳定。候选品Ⅰ、Ⅱ定值结果为:Ⅰ:(4.44±0.24)ng·mL-1k=2);Ⅱ:(16.79±0.82)ng·mL-1k=2)。候选品Ⅰ、Ⅱ测定结果均在新鲜血清样本测定值所建立的拟合回归直线95%置信区间范围内。

结论:

孕酮候选品均匀性、稳定性、互通性良好,定值准确可靠,可作为国家标准品应用。该国家标准品的建立,对促进孕酮检测结果的一致化和标准化具有重要意义。

Objective:

To develop the national standard materials of progesterone in frozen human serum and to evaluate the accuracy and standardization of serum progesterone detection kits.

Methods:

Low and high concentrations of serum were from normal people and pregnant women individuals respectively,with a clear appearance,no obvious hemolysis,jaundice and lipid blood. After all tests for four infectious diseases were negative,thrombin,anhydrous calcium chloride and anhydrous sodium carbonate were added according to the proportion of 1 kU·L-1,2.219 6 g·L-1 and 2.65 g·L-1 respectively,and centrifuged (4 000 r·min-1,4 ℃,25 min) after fully mixing. After twice of multiple filtration and sterilization,serum pools were packed in ampoules to prepare candidates Ⅰ and Ⅱand stored at -70 ℃. Single factor variance method and linear regression method were used to evaluate the homogeneity and stability of the candidate. Five labs working together used reference method (isotope dilution liquid chromatography tandem mass spectrometry) to assign value for candidate. The uncertainty was calculated. At the same time,the commutability was evaluated.

Results:

Through statistical analysis, the F values of homogeneity for candidate Ⅰ and Ⅱ were 0.570 9 and 1.200 9, respectively, which were all less than F0.05. Candidate Ⅰ and Ⅱ could be stable at least 8 h at 20-25 ℃. At 2-8 ℃, candidate Ⅰ and Ⅱ could be stable at least 33 d and 7 d respectively. Candidate Ⅰ and Ⅱ cound be stable at least 33 d at -20 ℃. Candidate Ⅰ and Ⅱ remained stable after repeated freeze-thaw cycles for 6 times. The assigned values were:candidate Ⅰ:(4.44±0.24) ng·mL-1 (k=2), Ⅱ:(16.79±0.82) ng·mL-1 (k=2). The determination results of candidate Ⅰ and Ⅱ were all within the 95% confidence interval of the fitting regression line established by the determination values of fresh serum samples.

Conclusion:

Progesterone candidate had good homogeneity,stability,interoperability. It had accurate and reliable determination value,which could be used as national standard. The establishment of this national standard was of great significance for promoting the consistency and standardization of progesterone testing results.

孕酮(progesterone)又称黄体酮、黄体激素,是一种类固醇激素,主要由卵巢和妊娠时期的胎盘分泌,在肾上腺也有部分合成[1],与雌激素一起调节与月经周期相关附属器官的功能,尤为重要的是为胚细胞的植床准备子宫内膜以及维持妊娠。测定血清孕酮的主要目的是监测卵巢排卵和黄体功能[2],妊娠期检测血清孕酮对了解妊娠状态及对早期先兆流产的治疗方法选择有重要意义[3-5]。对于未孕女性,孕酮也常与其他项目一起用于诊断不规则子宫出血[6-7]
目前虽已有血清基质的孕酮国际标准品(如ERM-DA347、BCR-348R等),但采购周期较长,可及性较差,国内虽有市场监管总局发布的血清型国家标准品,但不能充分满足临床应用需求。为缓解人血清基质的孕酮标准品匮乏现状,促进孕酮量值传递稳定可靠,本实验室开展了孕酮冰冻人血清国家标准品的研制。
定性滤纸、0.45 μm和0.22 μm微孔过滤膜;ML625-DLL分液装置(汉密尔顿公司);HSC-19T磁力搅拌器(Joanlab公司);2034C-02真空抽滤装置(津腾公司);LC-30AD超高效液相色谱(Shimadzu公司);QTRAP 5500质谱仪(AB SCIEX公司);Maccura i3000全自动化学发光免疫分析仪(迈克生物公司)。
均匀性、稳定性和互通性评价用试剂盒:孕酮测定试剂盒(直接化学发光法)(批号0221011,迈克生物股份有限公司)及化学发光免疫复合质控品(批号 0421021,迈克生物股份有限公司);互通性评价用试剂盒:孕酮测定试剂盒(化学发光法)(批号922986,贝克曼库尔特商贸(中国)有限公司)。
分别收集来自孕妇及正常人的高、低浓度血清,要求外观澄明、无明显溶血、黄疸和脂血,传染病四项(HIV、HBsAg、HCV和梅毒)检测均为阴性。筛选出符合要求的临床血清样品,置-70 ℃保存备用。按照比例分别添加1 kU·L-1凝血酶,2.219 6 g·L-1无水氯化钙和2.65 g·L-1无水碳酸钠[7],充分混匀后,离心(4 000 r·min-1,4 ℃,25 min),再取上清按“定性滤纸(初滤)→0.45 μm微孔滤膜(二次过滤)→0.22 μm微孔滤膜(除菌过滤)”的顺序过滤,静置2~8 ℃过夜,再次按以上顺序过滤后进行分装,制备成候选品Ⅰ和Ⅱ(每支0.5 mL),置-70 ℃保存。
按照JJF 1343-2022《标准物质的定值及均匀性、稳定性评估》进行[8],采用单因素方差分析法评价。根据文件要求,当总体单元数多于1 000个包装单元时,抽取样品数不少于30个。孕酮候选品分装2 000套,因此候选品Ⅰ、Ⅱ各随机抽取30支样品,采用Maccura i3000全自动化学发光免疫分析仪对抽样进行测定,每支样品均检测3次,取平均值。
参考文献同“1.3.2”,采用Excel 2019进行线性拟合回归,对候选品特性量值开展趋势性分析。分别在0、1、2、3、7、10、14、22、26、33 d 10个时间点上,候选品Ⅰ、Ⅱ各随机抽取6支,分别置于(2~8) ℃、-20 ℃条件下各3支。要求取样时间保持一致。采用Maccura i3000全自动化学发光免疫分析仪对抽样进行测定,每支样本均检测3次,取平均值,中间穿插测定质控品。同时,在(20-25) ℃条件下,开展0、2、4、6、8 h稳定性研究,候选品Ⅰ、Ⅱ在各时间点各随机取出3支。以时间X为横坐标,特性值Y为纵坐标作图,得到斜率k和截距b。根据公式(1)计算斜率的标准差sk。在自由度dfn-2,95%的显著水平条件下,得到对应的t(0.95,n-2)值。比较|k|与t(0.95,n-2)·sk,如果|k|<t(0.95,n-2)·sk,则斜率不显著,表明候选品稳定,反之不稳定。可减少时间点再分析,最终得到在不同条件下的稳定时间。对于候选品的反复冻融6次的稳定性的评估如下:取候选品Ⅰ、Ⅱ各6支,每3支合并后,300 μL·支-1分装,Ⅰ、Ⅱ各分装10支;分装后立即存于-70 ℃,此为冻融1次;Ⅰ、Ⅱ冻好之后再取出相应数量,充分解冻后放回-70 ℃,如此反复进行5次,全部样本取样结束后统一检测,每个样品重复测量3次。同样采用线性拟合回归,开展趋势性分析。
要求采用检验医学溯源性联合委员会(JCTLM)推荐的孕酮参考测量程序(同位素稀释液相色谱串联质谱法)[9]。参加实验室不少于3家,采用孕酮参考方法参加过近2年的国际比对或国内参考实验室室间比对,结果均为满意。分3次进行,每次Ⅰ、Ⅱ各取3支平行处理,每支检测3次,候选品Ⅰ、Ⅱ共收集27个有效数据。计算平均值、标准偏差(SD)及相对标准偏差(RSD),数据有效性标准有:1)样本测定结果应在(平均值±2SD)范围内,且室内RSD不大于3.0%;2)质控物质测定结果应在(靶值±3.0%)范围内,且室内RSD不高于3.0%。其中1)中的均值和SD分别指全部实验室所有结果的均值和SD。不满足要求的数据判定为无效数据,直接剔除,取各实验室有效数据的均值,计算总均值,作为Ⅰ、Ⅱ的最终定值结果。
参考文献同“1.3.2”,不确定度来源包括定值、均匀性和稳定性,计算候选品的合成标准不确定度uCRM见公式(2)。
uCRM:合成标准不确定度;uchar:定值引起的标准不确定度;ubb:均匀性引起的标准不确定度;us:稳定性引起的标准不确定度。
按照《WS/T 356-2011基质效应与互通性评估指南》[10]进行。用参考方法和常规方法(迈克试剂盒与迈克全自动化学发光仪、贝克曼试剂盒与贝克曼全自动化学发光仪组成2个常规系统),同时对选定的一系列具有代表性的40份新鲜临床样品和候选品进行分析,利用2种方法(参考方法和上述常规方法)测定临床样品的结果建立数学回归关系。以参考方法测定值为X轴,以常规方法测定值为Y轴,用Origin 8.0软件计算95%置信区间。如果候选品在常规方法上的测定均值落在该区间95%置信区间内,说明该候选品在该常规方法上无基质效应,表明其在参考方法和常规方法之间具有互通性。
通过单因素方差分析,对候选品数据进行评价,F值分别为0.570 9和1.200 9(结果见表1),均<F0.05,表明候选品Ⅰ和Ⅱ均匀性良好。
表2结果可知,候选品Ⅰ和Ⅱ在(20~25)℃ 8 h以及-20 ℃放置33 d稳定,候选品Ⅰ在(2~8)℃ 33 d稳定,但是候选品Ⅱ在(2~8)℃ 33 d不稳定。因此,减少考察时间后再进行计算,直至稳定为止。结果显示:候选品Ⅱ在(2~8)℃可稳定7 d,且候选品Ⅰ和Ⅱ在反复冻融之后6次稳定。具体数据见表2
5家实验室参加本次研究。孕酮候选品Ⅰ、Ⅱ定值结果的室内RSD均不超过2.0%,表明各实验室建立的参考方法重复性好,数据可靠。候选品Ⅰ和Ⅱ在室间的RSD分别为1.6%和2.3%,联合定值结果详见表3

基于表1的方差结果,由于候选品Ⅰ组间MS(0.048 0)小于组内MS(0.084 2),采用公式(4)计算得到候选品Ⅰ的均匀性引入不确定度为0.071 6 ng·mL-1

对于候选品Ⅱ,组间MS(1.197 6)大于组内MS(0.997 3),采用公式(5)计算得到候选品Ⅱ的均匀性引入不确定度为0.258 4 ng·mL-1

公式为:us=sk·X,其中sk为稳定性数据,通过线性拟合得到的方程斜率k的标准差,X为候选品的有效期。候选品Ⅰ、Ⅱ稳定性引入的不确定度结果分别为0.089 0、0.266 0 ng·mL-1
根据定值、均匀性和稳定性所引入的不确定度,得到候选品Ⅰ、Ⅱ的合成标准不确定度为:
经联合定值及不确定度评估,孕酮候选品Ⅰ:(4.44±0.24)ng·mL-1 (k=2);Ⅱ:(16.79±0.82)ng·mL-1 (k=2)。
采用参考方法和2种不同常规方法(国产及进口检测试剂盒)对40份新鲜血清样本同时测定,评估候选品的互通性。结果见表4
目前临床常用的血清孕酮检测,大部分为采用基于抗原抗体反应原理的商品化试剂盒,如化学发光法[11-12]、时间分辨免疫荧光法[13]、放射免疫法[14]、酶联免疫法等,这些方法检测速度快,且多为全自动化操作,临床接受度很高。在我国具有医疗器械注册证的孕酮检测试剂盒有约150余个,然而各家试剂盒因溯源途径、抗原抗体来源、试剂组分等不尽相同,测值之间必然存在一定差异[15],从而对疾病的诊断、治疗和用药监测产生一定影响。因此,孕酮检测结果的标准化迫在眉睫。标准物质是实现检测结果准确、具有可比性的重要工具,也是保证量值传递的实物标准,因此本实验室开展了孕酮国家标准品的研制。
该批孕酮国家标准品以正常人及孕妇血清为基质制备。由于孕妇处于女性特殊生理时期,血清中的纤维蛋白原含量显著高于正常人,因此若仅采用常规血清处理方式(多重过滤除菌),不能完全去除样本中的纤维蛋白析出物。查相关文献[7],进行了氯化钙、凝血酶等的添加,促进纤维蛋白原的充分析出。经多次验证,去除沉淀的效果理想,且对于孕酮测值结果影响较小。
均匀性、稳定性和定值准确是标准物质的三大特征。经均匀性评估,显示孕酮标准品均匀性良好。在稳定性方面,室温放置一段时间后,发现样本浑浊,可能长菌所致,表明其不适宜长时间室温稳定性评估,因此模拟通常实验室检测条件,进行了室温8 h稳定性评估,未见趋势性变化;候选品Ⅰ、Ⅱ在-20 ℃条件下,至少可稳定33 d,在(2~8) ℃条件下,至少分别可稳定33 d和7 d。通常孕酮标准品均储存在-70 ℃下,日常运输为干冰运输。因此上述稳定性的结果表明标准品满足使用要求。本研究中,全部实验室均采用检验医学溯源联合委员会推荐的参考方法:同位素稀释液相色谱串联质谱法(ID/LC/MS/MS)。各参加实验室均已建立并稳定运行该方法较长时间,保证了定值结果的准确性和可靠性。最终的结果也验证了这一点,候选品Ⅰ、Ⅱ定值结果的实验室间RSD均不高于2.3%。且各实验室内部的RSD在1.2%~2.8%,表明定值结果重复性较好,数据可靠。通过与2种主流孕酮检测试剂的互通性评价,亦表明标准品的互通性良好。
综上所述,本次研制的孕酮标准品,均匀性、互通性良好,定值结果准确,稳定性满足相关要求,可以作为国家标准品应用于孕酮常规方法的校准和准确度评价,以推动临床孕酮检测结果的一致化和互认,为孕酮体外诊断试剂的监管提供有力的保障。
