Objective:

Based on a sequential analysis method of “physicochemical properties-main component content-fingerprint similarity-anaphylactoid reaction”,the stability and safety of Danhong injection with commonly used clinical solvents 0.9% sodium chloride and 5% glucose injections were evaluated.

Methods:

According to the clinical application,Danhong injection was mixed with 0.9% sodium chloride injection or 5% glucose injection in the ratios of 20 mL:100 mL,30 mL:100 mL,40 mL:100 mL and 30 mL:250 mL,40 mL:250 mL,respectively. Then,they were placed at room temperature within 10 h. The properties,osmotic pressure,insoluble particles and pH value in the solutions were observed or measured at different time points. The contents of the main components at different time points were determined. The HPLC and 1H NMR fingerprints at different time points were analyzed by similarity evaluation. The in vitro anaphylactoid reaction assay of RBL-2H3 cell degranulation was used to evaluate the safety of the mixture solutions.

Results:

The properties,osmotic pressure,insoluble particles and pH value of solutions were stable within 10 h after mixing,and there was no significant difference in the contents of the main components. The HPLC and 1H NMR fingerprints showed good similarity,and the mixture solutions didn’t induce obvious degranulation in RBL-2H3 cells.

Conclusion:

In this study,the comprehensive evaluation from the physical,chemical and biological perspectives showed that in the concentration range of 30 mL:250-40 mL:100 mL,Danhong injection combined with 0.9% sodium chloride injection or 5% glucose injection had good stability and safety within 10 h,which provides evidences for the clinical practice.

目的:

基于“理化性质-主要成分含量-指纹图谱相似度-类过敏反应”的序贯分析策略,评价丹红注射液与临床常用输液溶媒0.9%氯化钠注射液和5%葡萄糖注射液在不同配伍条件下溶液的稳定性及安全性。

方法:

根据临床使用情况,将丹红注射液与0.9%氯化钠注射液或5%葡萄糖注射液分别按20 mL:100 mL、30 mL:100 mL、40 mL:100 mL和30 mL:250 mL、40 mL:250 mL的比例进行配伍,在室温条件下10 h内,分别观察和检测各溶液在配伍后不同时间点的性状、渗透压、不溶性微粒和pH;测定溶液在配伍后不同时间点的各主要成分含量;计算配伍后不同时间点各溶液HPLC指纹图谱和1H NMR指纹图谱的相似度;采用RBL-2H3细胞脱颗粒体外类过敏反应检测体系,评价各配伍溶液的安全性。

结果:

配伍10 h内溶液性状、渗透压、不溶性微粒含量、pH无明显变化,主要成分含量无显著差异,HPLC和1H NMR指纹图谱均有良好的相似度,且配伍溶液无明显致RBL-2H3细胞脱颗粒作用。

结论:

本研究从物理、化学、生物角度综合评价,表明在30 mL:250 mL~40 mL:100 mL浓度范围内,丹红注射液与0.9%氯化钠注射液或5%葡萄糖注射液在配伍10 h内溶液均有良好的稳定性和安全性,可为临床使用提供参考。

目的:

基于“理化性质-主要成分含量-指纹图谱相似度-类过敏反应”的序贯分析策略,评价丹红注射液与临床常用输液溶媒0.9%氯化钠注射液和5%葡萄糖注射液在不同配伍条件下溶液的稳定性及安全性。

方法:

根据临床使用情况,将丹红注射液与0.9%氯化钠注射液或5%葡萄糖注射液分别按20 mL:100 mL、30 mL:100 mL、40 mL:100 mL和30 mL:250 mL、40 mL:250 mL的比例进行配伍,在室温条件下10 h内,分别观察和检测各溶液在配伍后不同时间点的性状、渗透压、不溶性微粒和pH;测定溶液在配伍后不同时间点的各主要成分含量;计算配伍后不同时间点各溶液HPLC指纹图谱和1H NMR指纹图谱的相似度;采用RBL-2H3细胞脱颗粒体外类过敏反应检测体系,评价各配伍溶液的安全性。

结果:

配伍10 h内溶液性状、渗透压、不溶性微粒含量、pH无明显变化,主要成分含量无显著差异,HPLC和1H NMR指纹图谱均有良好的相似度,且配伍溶液无明显致RBL-2H3细胞脱颗粒作用。

结论:

本研究从物理、化学、生物角度综合评价,表明在30 mL:250 mL~40 mL:100 mL浓度范围内,丹红注射液与0.9%氯化钠注射液或5%葡萄糖注射液在配伍10 h内溶液均有良好的稳定性和安全性,可为临床使用提供参考。

Objective:

Based on a sequential analysis method of “physicochemical properties-main component content-fingerprint similarity-anaphylactoid reaction”,the stability and safety of Danhong injection with commonly used clinical solvents 0.9% sodium chloride and 5% glucose injections were evaluated.

Methods:

According to the clinical application,Danhong injection was mixed with 0.9% sodium chloride injection or 5% glucose injection in the ratios of 20 mL:100 mL,30 mL:100 mL,40 mL:100 mL and 30 mL:250 mL,40 mL:250 mL,respectively. Then,they were placed at room temperature within 10 h. The properties,osmotic pressure,insoluble particles and pH value in the solutions were observed or measured at different time points. The contents of the main components at different time points were determined. The HPLC and 1H NMR fingerprints at different time points were analyzed by similarity evaluation. The in vitro anaphylactoid reaction assay of RBL-2H3 cell degranulation was used to evaluate the safety of the mixture solutions.

Results:

The properties,osmotic pressure,insoluble particles and pH value of solutions were stable within 10 h after mixing,and there was no significant difference in the contents of the main components. The HPLC and 1H NMR fingerprints showed good similarity,and the mixture solutions didn’t induce obvious degranulation in RBL-2H3 cells.

Conclusion:

In this study,the comprehensive evaluation from the physical,chemical and biological perspectives showed that in the concentration range of 30 mL:250-40 mL:100 mL,Danhong injection combined with 0.9% sodium chloride injection or 5% glucose injection had good stability and safety within 10 h,which provides evidences for the clinical practice.

中药注射液结合中医药理论与现代生产工艺,突破传统给药方式,具有剂量准确、起效迅速、生物利用度高等优势,目前被广泛应用于临床[1]。然而,随着中药注射液使用的增加,其临床不良反应亦时有报道。在众多引起不良反应的原因中,溶媒种类、药物浓度、药物配制后的存放时间是普遍存在的影响因素[2]。已有研究报道中药注射液与溶媒配伍使用可能导致不溶性微粒增多、所含成分改变等从而影响药液的质量,质量改变的药液进入人体可能会对机体造成一定风险[3]。因此,中药注射液与输液溶媒配伍稳定性、安全性的研究将有助于其临床正确合理的使用,从而更有效地发挥中药注射液独特的疗效。
丹红注射液是由中药丹参、红花经现代工艺制备得到的红棕色澄明液体,是临床上常用的中药注射液,其主要组成及效用成分为酚酸类、黄酮类等[4-5]。依据丹红注射液说明书,静脉滴注1次20~40 mL,溶媒首选5%葡萄糖注射液100~500 mL;若伴有糖尿病等特殊情况时,改用0.9%氯化钠注射液。目前,关于丹红注射液配伍溶液的研究仅考察了pH、不溶性微粒和主要成分含量,难以全面地反映配伍后溶液的稳定性和安全性。因此,构建符合临床需求、更为全面的丹红注射液与溶媒配伍合理性评价方法具有重要的意义。
本研究基于“理化性质-主要成分含量-指纹图谱相似度-类过敏反应”的序贯分析策略,评价丹红注射液与0.9%氯化钠注射液和5%葡萄糖注射液在不同配伍条件下10 h内溶液的稳定性和安全性。首先,对配伍溶液的性状、渗透压、不溶性微粒、pH进行检测,从常见理化指标角度分析配伍稳定性;其次,考察了配伍溶液中总酚和5种主要成分含量的稳定性;通过综合运用HPLC指纹图谱和1H NMR指纹图谱,分析配伍溶液中的各类成分含量及比例的稳定性。由于中药注射液成分复杂,与溶媒配伍后引起临床类过敏反应的因素较多,本研究采用RBL-2H3细胞脱颗粒体外类过敏检测体系评价丹红注射液与溶媒配伍后的安全性。基于上述序贯分析,旨在从物理、化学、生物角度综合评价丹红注射液与临床常用溶媒配伍后溶液的质量,为其临床安全用药、合理配伍提供依据,为规范其静脉用药调配提供参考。
STY-1渗透压测定仪和GWJ-16微粒检测仪(天大天发科技有限公司),S40K型酸度计(Mettler公司),SC-4000A型澄明度检测仪(上海黄海药检仪器有限公司),Cary 60 UV-Vis分光光度计和Agilent 1100型高效液相色谱仪(Agilent公司),JEOL 500M超导核磁共振波谱仪(日本电子株式会社),Milli-Q超纯水制备系统(Millipore公司),Tecan Infinite M1000型多功能酶标仪(Tecan公司)。
丹红注射液(批号22071041;10 mL·支-1)由山东丹红制药有限公司提供,5%葡萄糖注射液(批号W22021005、N22032406;规格:100 mL、250 mL)、0.9%氯化钠注射液(批号G222070802、N22050103;规格:100 mL、250 mL)购自四川科伦药业有限公司。
对照品丹参素钠(批号110855-201915)、原儿茶醛(批号110810-201909)、4-香豆酸(批号112037-202102)、迷迭香酸(批号111871-202007)、丹酚酸B(批号111562-201917)均购自中国食品药品检定研究院;色谱纯三氟乙酸购自北京百灵威科技有限公司;色谱纯冰醋酸购自国药集团化学试剂有限公司,色谱纯甲醇和乙腈购自Merck公司,氘代水(氘代度99.9%)购自青岛腾龙微波科技有限公司,DMEM基础培养基(货号319-005-CL)、DMEM无酚红培养基(货号319-051-CL)购自MUTILCELL公司;TritonTM X-100(货号T8787)、Compound 48/80(货号C2313)、4-nitrophenyl N-acetyl-β-D-glucosaminide(货号N9376)购自Sigma公司;其他试剂均为分析纯。
RBL-2H3细胞:为大鼠嗜碱性白血病细胞,由杭州美森细胞生物科技有限公司提供。
根据丹红注射液说明书,该品每次可用20~40 mL,用0.9%氯化钠注射液或5%葡萄糖注射液100~500 mL稀释后静脉滴注。根据临床常用剂量,本实验考察2种溶媒各5个配伍(20、30、40 mL丹红注射液与0.9%氯化钠注射液100 mL或5%葡萄糖注射液配伍,丹红注射液30、40 mL与0.9%氯化钠注射液250 mL或5%葡萄糖注射液配伍)的稳定性和安全性。配制好的成品溶液置于室温条件下,考察不同时间点性状、渗透压、不溶性微粒和pH等理化性质,测定主要成分含量,分析指纹图谱相似度并检测其类过敏反应。
实验在室温自然光下进行,分别于配伍后0、4、10 h取样,测定溶液的性状、渗透压、不溶性微粒和pH。其中渗透压、不溶性微粒和pH的测定分别参照渗透压摩尔浓度测定法(2020年版《中华人民共和国药典》(简称《中国药典》)四部0632[6]85)、不溶性微粒检查法(2020年版《中国药典》四部0903[6]125)和pH测定法(2020年版《中国药典》通则0631[6]84)进行检测。观察发现配伍溶液均为黄色澄明液体,10 h内始终保持澄清透明,无浑浊和可见异物的产生。以丹红注射液与0.9%氯化钠注射液或5%葡萄糖注射液按照低浓度(30 mL:250 mL)、中浓度(20 mL:100 mL)、高浓度(40 mL:100 mL)进行配伍后所得溶液为例,其渗透压、pH和不溶性微粒的测定结果见表12。配伍10 h内渗透压摩尔浓度和pH均无明显变化,不溶性微粒个数虽有变化,但溶液每1 mL含10 μm及其以上的不溶性微粒未超过25粒,含25 μm及其以上的不溶性微粒未超过3粒。以上结果表明丹红注射液与5%葡萄糖注射液和0.9%氯化钠注射液在配伍10 h内有较稳定的渗透压、pH,且不溶性微粒含量、性状均无显著变化。
精密称定丹参素钠对照品,加适量超纯水制成1.000 mg·mL-1的储备液。分别取配好的储备液适量,加超纯水制成20.00、30.00、40.00、50.00、60.00、70.00 μg·mL-1的系列丹参素钠对照品溶液。在25 ℃时,按照紫外-可见分光光度法(2020年版《中国药典》四部0401[6]39),在280 nm的波长处测定吸光度,绘制标准曲线。依据系列丹参素钠对照品溶液测定结果绘制得到的标准曲线方程计算各样本中总酚含量,每1 mL含总酚以丹参素钠(C9H9O5Na)计,具体结果见表3。各配伍溶液不同测试时间点的总酚含量的RSD<2.4%,表明各配伍溶液在室温下放置10 h总酚含量稳定。
色谱条件:采用Kromasil 100-5-C18色谱柱(250 mm×4.6 mm,5 μm),流动相为0.05%三氟乙酸(A)-乙腈(B),梯度洗脱(0~65 min,98%A→70%A;65~75 min,70%A;75~76 min,70%A→98%A),流速为0.8 mL·min-1,柱温为40℃,检测波长为288 nm,进样量为5 μL。
对照品溶液制备:分别取对照品丹参素钠、原儿茶醛、4-香豆酸、迷迭香酸和丹酚酸B适量,置棕色量瓶中,精密称定,加入含0.2%醋酸的10%甲醇水溶液配制成备用的混合对照品溶液(丹参素钠、原儿茶醛、4-香豆酸、迷迭香酸、丹酚酸B质量浓度分别为250.0、30.00、10.00、30.00、40.00 μg·mL-1)。
在上述色谱条件下,分别记录不同稀释梯度的混合对照品溶液和室温条件下0、4、10 h时经处理后的各配伍溶液的色谱图,其HPLC谱图如图1所示,经与对照品比对,确定图中标注为1~5的5个特征峰分别为丹参素钠、原儿茶醛、4-香豆酸、迷迭香酸和丹酚酸B。依据各系列对照品溶液得到相应标准曲线方程,分别计算得到丹参素、原儿茶醛、4-香豆酸、迷迭香酸和丹酚酸B含量,具体结果见表3。各配伍溶液不同测试时间点上述主要成分含量的RSD小于4.6%,表明配伍溶液在室温下放置10 h丹参素、原儿茶醛、4-香豆酸、迷迭香酸、丹酚酸B含量较稳定。
色谱条件同“2.3.2”项,各配伍溶液均在0、4、10 h取样进行HPLC指纹图谱检测,采用“中药色谱指纹图谱相似度评价系统”软件(国家药典委员会)对各色谱数据进行分析处理,以各配伍0 h谱图为参照,通过Mark峰匹配计算色谱指纹图谱相似度。结果显示指纹图谱相似度均为1.000,说明在10 h内各配伍溶液主要化学成分总体稳定。以丹红注射液最高配伍浓度为例(40 mL:100 mL),其与0.9%氯化钠注射液和5%葡萄糖注射液配伍后不同时间点HPLC指纹图谱分别见图2图3
核磁分析样品的制备:取室温条件下0、4、10 h时的各配伍溶液样本450 μL,加氘代水50 μL后混匀,转移至核磁管中,加封,分别采集1H NMR数据。
核磁共振波谱条件:以90% H2O+10% D2O锁场,用预饱和脉冲序列压制水峰;主要参数:谱宽10 kHz;扫描次数8次;延迟时间1 s;增益46。采用MetresNova软件手动进行傅里叶变换、相位与基线校正、定标。
将所得核磁共振指纹图谱数据Normalization处理,运用夹角余弦法计算各溶液配伍后4、10 h时所采集谱图与0 h采集谱图的相似度,结果显示各谱图相似度均大于0.97,表明不同配伍时间点溶液在化学成分含量和比例上无明显差异。以丹红注射液最高配伍浓度为例(40 mL:100 mL),其与0.9%氯化钠注射液和5%葡萄糖注射液配伍后不同时间点1H NMR指纹图谱分别见图4图5
取生长状况良好的RBL-2H3细胞,均匀接种于96孔板,接种密度为2×104个·孔-1,于37 ℃、5% CO2培养箱中培养24 h。实验设置对照组(5%葡萄糖注射液或0.9%氯化钠注射液)、阳性药组(C48/80,90 μg·mL-1)和不同配伍溶液组,其中各配伍10 h后溶液均用无酚红无血清DMEM培养基(避免血清和酚红对β-氨基己糖苷酶测定的影响)稀释至5%,同时以1% Triton X-100裂解细胞设为总酶孔,于37℃、5% CO2培养箱中孵育1 h。
取细胞上清和裂解液,在96孔板中进行实验,设置实验组和空白组。实验组每孔均加入待测上清或裂解液50 μL,空白组每孔单独加入对应50 μL空白对照溶液、阳性药、配伍10 h后试样本底,立即加入β-氨基己糖苷酶底物(用pH 4.5的柠檬酸盐缓冲液配成1 mmol·L-1溶液)50 μL,37 ℃孵育1 h。然后每孔加入碳酸钠/碳酸氢钠的缓冲液(pH 10)100 μL终止反应,酶标仪405 nm处测定吸光度,按下式计算β-氨基己糖苷酶释放率。
β-氨基己糖苷酶释放率%=(A上清-A本底)/(A裂解-A空白)×100%。
前期实验结果表明,RBL-2H3细胞在5%丹红注射液作用下细胞存活率超过75%,故选取该浓度进行实验。如图6图7所示,与对照组比较,阳性药C48/80作用后可致RBL-2H3细胞β-氨基己糖苷酶释放率显著升高,而丹红注射液配伍溶液各给药组与对照组相比未引起β-氨基己糖苷酶释放率显著增加,表明在该条件下,丹红注射液与溶媒配伍10 h后的溶液无明显致类过敏作用。
丹红注射液是临床治疗心脑血管疾病的常用药物之一,现代研究表明,丹红注射液有活血化瘀、通脉舒络的作用[7],在冠心病、心绞痛、心肌梗塞、瘀血型肺心病、缺血性脑病和脑血栓等的治疗过程中均有良好的临床效果[8-10]。在静脉滴注过程中,合理选择配伍溶媒是十分重要的环节,目前临床常用溶媒主要有5%葡萄糖注射液、0.9%氯化钠注射液等。本文基于“理化性质-主要成分含量-指纹图谱相似度-类过敏反应”的序贯分析策略,对丹红注射液与临床常用输液溶媒0.9%氯化钠注射液和5%葡萄糖注射液配伍后进行多指标分析,综合评价丹红注射液配伍溶液的稳定性和安全性。
医师在临床用药时,常常只关注药品的功能主治,根据临床使用习惯选择溶媒,忽视溶媒配伍后的理化性质,使得不良反应发生的概率大大增加。目前注射液配伍溶液常见的稳定性检测指标有性状、pH、渗透压、不溶性微粒等。本研究将丹红注射液与临床常用输液溶媒0.9%氯化钠注射液和5%葡萄糖注射液按照一定比例配伍,观察其在配伍后10 h内理化性质的稳定性。结果表明,丹红注射液各配伍溶液在室温下放置10 h均有良好的性状、渗透压稳定性。静脉输液时,pH一般控制在4~9范围内,若超出此范围,易诱发血栓性静脉炎和血小板聚集[11]。丹红注射液与溶媒配伍10 h内pH较稳定。不溶性微粒是引起热原反应、炎症反应、肉芽肿及血管栓塞甚至癌症的主要原因,本文测量结果发现,每1 mL含10 μm及以上的不溶性微粒未超过25粒,每1 mL含25 μm及以上的不溶性微粒未超过3粒[12]
中药注射液配伍后主要成分含量的变化直接影响注射剂的质量和其临床有效性。在丹红注射液与临床常用输液溶媒0.9%氯化钠注射液和5%葡萄糖注射液配伍10 h内,分析主要成分含量变化,发现各配伍溶液中总酚、丹参素、原儿茶醛、4-香豆酸、迷迭香酸和丹酚酸B的含量均无明显变化,说明10 h上述各配伍溶液内主要成分含量有良好的稳定性。
运用指纹图谱对丹红注射液各配伍溶液不同观测时间点的化学成分进行整体性评价,揭示各化学成分的相对关系。HPLC指纹图谱可反映丹红注射液主要药效物质在各配伍溶液中含量和比例的变化情况。相比HPLC指纹图谱,1H NMR指纹图谱具有样品预处理简单、无损、分析速度快、对检测成分无偏向性的优势,可反映糖类、氨基酸类等成分的含量变化[13]。研究发现,配伍后溶液的HPLC和1H NMR指纹图谱均有较高的相似度,表明配伍10 h内溶液化学成分含量和比例未发生明显变化,有良好的化学稳定性。
丹红注射液不良反应累及系统、器官的前3位分别是皮肤及其附件、神经系统和循环系统,主要不良反应为潮红、皮疹、头痛、头晕、胸闷、心悸、恶心、呼吸困难、发热等[14]。鉴于中药注射剂引起的变态反应以类过敏反应为主,本研究采用RBL-2H3细胞脱颗粒模型,以β-氨基己糖苷酶释放率作为丹红注射液配伍溶液在室温放置10 h后的类过敏反应检测指标,以C48/80作为阳性药刺激细胞过敏性介质及细胞因子释放剂,模拟体内、外细胞脱颗粒过程及临床反应[15]。在测试条件下,各配伍溶液作用组中β-氨基己糖苷酶释放率与对照组相比无明显增加,故本研究未发现丹红注射液与0.9%氯化钠注射液和5%葡萄糖注射液配伍10 h后溶液会引起类过敏反应。
综合而言,本研究对丹红注射液与常用溶媒配伍后0、4、10 h 3个测试时间点溶液的理化性质、主要成分含量、指纹图谱相似度、类过敏反应进行序贯分析,从物理、化学、生物角度综合评价,发现配伍后10 h丹红注射液与临床常用输液溶媒0.9%氯化钠注射液和5%葡萄糖注射液在30 mL:250 mL~40 mL:100 mL浓度范围内均有较好的配伍稳定性和安全性,为丹红注射液的临床配伍应用提供了参考。
