Objective: To clarify the differences between the “Room temperature” definitions in the European Pharmacopoeia and the Chinese Pharmacopoeia, to emphasize that storage temperature was closely related to the design of long-term stability studies and product labeling, and to investigate the effects of storage temperature variations on the critical quality attributes (CQAs) of terlipressin for injection, so as to raise awareness among generic drug manufacturers regarding this critical issue. Methods: Given the discrepancies in storage temperature recommendations across different manufacturers’ product inserts, the original drug’s long-term stability studies and storage conditions were traced. Validated methods for related substances and polymer content were employed to assess the impact of temperature differences on the product’s CQAs. Results: Samples stored at 30 ℃exhibited a significantly higher increase in related substances compared to those stored at 15 ℃. In one manufacturer’s product, polymer levels exceeded specification limits within just five months of storage at 30 ℃. The divergence among manufacturers stems from some companies misinterpreting the original drug’s labeling by directly translating without considering the differences between Chinese and European Pharmacopoeias. Conclusion: Storage temperature has a significant impact on the levels of related substances and polymer content in terlipressin for injection. To ensure product quality, the storage temperature in the labeling should be restricted to “not exceeding 25 ℃.”

目的:欧洲药典“Room temperature”与《中华人民共和国药典》“室温”概念不同,贮藏温度与长期稳定性试验设计、说明书撰写密切相关,本文探讨贮藏温度差异对注射用特利加压素关键质量属性的影响,以期引起仿制药企业对此问题的关注。方法:基于不同厂家说明书贮藏温度不一致的问题,追溯原研药长期稳定性试验及贮藏温度,采用经过验证的有关物质和聚合物检查方法评价温度差异对产品关键质量属性的影响。结果:放置于30 ℃的样品,有关物质增长幅度显著高于放置于15 ℃的样品。一家企业的样品于30 ℃放置仅5个月,聚合物就已超限。企业分歧源自部分企业忽视中欧药典差异,将原研药说明书“Room temperature”简单直译为“室温”。结论:贮藏温度对注射用特利加压素的有关物质和聚合物影响显著,应将说明书中贮藏温度要求收窄至“不超过25 ℃”。

目的:欧洲药典“Room temperature”与《中华人民共和国药典》“室温”概念不同,贮藏温度与长期稳定性试验设计、说明书撰写密切相关,本文探讨贮藏温度差异对注射用特利加压素关键质量属性的影响,以期引起仿制药企业对此问题的关注。方法:基于不同厂家说明书贮藏温度不一致的问题,追溯原研药长期稳定性试验及贮藏温度,采用经过验证的有关物质和聚合物检查方法评价温度差异对产品关键质量属性的影响。结果:放置于30 ℃的样品,有关物质增长幅度显著高于放置于15 ℃的样品。一家企业的样品于30 ℃放置仅5个月,聚合物就已超限。企业分歧源自部分企业忽视中欧药典差异,将原研药说明书“Room temperature”简单直译为“室温”。结论:贮藏温度对注射用特利加压素的有关物质和聚合物影响显著,应将说明书中贮藏温度要求收窄至“不超过25 ℃”。

Objective: To clarify the differences between the “Room temperature” definitions in the European Pharmacopoeia and the Chinese Pharmacopoeia, to emphasize that storage temperature was closely related to the design of long-term stability studies and product labeling, and to investigate the effects of storage temperature variations on the critical quality attributes (CQAs) of terlipressin for injection, so as to raise awareness among generic drug manufacturers regarding this critical issue. Methods: Given the discrepancies in storage temperature recommendations across different manufacturers’ product inserts, the original drug’s long-term stability studies and storage conditions were traced. Validated methods for related substances and polymer content were employed to assess the impact of temperature differences on the product’s CQAs. Results: Samples stored at 30 ℃exhibited a significantly higher increase in related substances compared to those stored at 15 ℃. In one manufacturer’s product, polymer levels exceeded specification limits within just five months of storage at 30 ℃. The divergence among manufacturers stems from some companies misinterpreting the original drug’s labeling by directly translating without considering the differences between Chinese and European Pharmacopoeias. Conclusion: Storage temperature has a significant impact on the levels of related substances and polymer content in terlipressin for injection. To ensure product quality, the storage temperature in the labeling should be restricted to “not exceeding 25 ℃.”

“Room temperature”与“室温”直译完全对应,但在中欧药典中,二者代表不同概念。《欧洲药典》 (EP)的“Room temperature”为15~25 ℃,《中华人民共和国药典》(简称《中国药典》)“室温”为10~30 ℃。部分仿制药企业对该差异认识不足,简单与原研药长期稳定性试验或说明书中的“Room temperature”保持“一致”,相当于放宽原研贮藏条件,给产品质量带来隐患。后期弥补此类错误代价极大,重新开展长期稳定性试验耗时久、周期长,变更说明书贮藏温度需补充申请重大变更。充分认知各国药典“室温”差异,是保障仿制药稳定性研究、效期、贮藏条件科学合理的重要前提。本文以注射用特利加压素为例,阐述该问题对产品关键质量属性的影响。
注射用特利加压素国外原研品种依照EP“Room temperature”,长期稳定性试验在25 ℃开展,说明书贮藏温度为15~25 ℃。考察5家国内仿制药企业,其中4家将原研“Room temperature”错误理解为“室温”,导致2个重大后果:①说明书贮藏温度标示为“室温”。调研走访多家医院药剂科,均表示贮藏温度要求“不超过25 ℃”应存放于阴凉库(库内温度通常为15 ℃左右),贮藏温度要求为室温则无特殊处理。②贮藏温度上限与稳定性研究温度不一致。各企业稳定性研究均参照原研药企业在25 ℃进行,而贮藏温度上限为30 ℃,长期稳定性试验能否反映实际贮藏条件下的稳定性存疑。
特利加压素(结构式见图1)是由12个氨基酸缩合而成的多肽类药物,N端未进行乙酰化,存在2个伯氨基,碱性较强。7/8/12位存在3个末端酰胺,碱性条件下易水解。特利加压素临床用于治疗食管静脉曲张出血,是首个获得FDA批准用于治疗成人肝肾综合征(HRS)的药物[1]。近年来,特利加压素联合用药治疗脓毒症休克、肝硬化并发食管胃静脉曲张出血、乙型肝炎并发急性肾损伤等的疗效研究多有文献报道[2-9]
高温会引发多肽的降解和杂质的形成[10],包括:①酰胺键水解,生成断裂肽和游离氨基酸。②色氨酸、酪氨酸等氨基酸残基被氧化,改变多肽的生物活性和稳定性。③天冬酰胺、谷氨酰胺发生脱酰胺反应,生成相应酸性基团,影响多肽的溶解性和功能。④部分氨基酸残基发生顺反异构化或肽链的构象变化,影响多肽的生物活性和结构特性。⑤多肽侧链发生脱水反应,导致环状化合物形成,具有不同的生物活性或毒性。⑥多肽链间的交联、聚合反应,生成的聚合物影响药物的溶解性、生物利用度和稳定性。⑦赖氨酸等氨基酸残基与糖类发生美拉德反应,影响多肽的外观和生物活性。
模拟实际储存情况,课题组将样品分别在30、15 ℃放置5个月,参照国家药品监督管理局药品注册标准,对关键质量属性有关物质和聚合物进行测定[11]。结果30 ℃放置仅5个月的样品有关物质明显增加,一家企业的聚合物超出限度要求,30 ℃放置显著影响药物质量。
Waters e2695型高效液相色谱仪(沃特世公司),MS-205十万分之一电子天平(梅特勒-托利多公司),Milli-Q IQ7000型超纯水仪(默克密理博公司),LHH-150GSD稳定性试验箱(上海-恒科学仪器有限公司)。
乙腈、三氟乙酸为色谱纯,其他试剂为分析纯,水为超纯水。注射用特利加压素样品来源于企业A、B、C、D,各1批,规格均为1 mg(相当于0.86 mg特利加压素)。对照品醋酸特利加压素(批号150701-202002,含量86.4%),中国食品药品检定研究院;[Asp8,Gly12-OH]特利加压素(杂质1,批号C-HY017-IML-20171103,含量97.97%)、[Glu7,Gly12-OH]特利加压素(杂质2,批号C-HY017-IMK-20171215,含量97.81%)、[Gly12-OH] 特利加压素(杂质4,批号CHY017-IM-09-20180524,含量96.42%)、[Ac-Gly1]特利加压素(杂质5,批号C-HY017-IMK-20171108,含量97.32%)、[Asp8]特利加压素(杂质6,批号CHY017-IMH-20191010,含量99.38%)、[Glu7]特利加压素(杂质7,批号C-HY017-IMF-20180524,含量98.30%)、add-Gly12 特利加压素(杂质9,批号20191010,含量98.87%)、des-Gly1,Gly2特利加压素(杂质10,批号C-HY017-IMB-20170718,含量97.02%)均来自深圳翰宇药业股份有限公司;[β-Asp8] 特利加压素(杂质3,批号CT-11-01157,含量91.0%)、add-Gly1特利加压素(杂质8,批号CU-02-00315,含量97.5%)均来自中肽生化有限公司;(4,9’),(4’,9)-双二硫键特利加压素二聚体(批号C-HY017-IM-43-20180718,含量98.42%,相对分子质量2 455.1)、(1’,12)-脱氨特利加压素二聚体(批号20191010,含量98.42%,相对分子质量2 438)均来自深圳翰宇药业股份有限公司,杂质结构信息引自各企业注册标准。
精密称取醋酸特利加压素对照品适量,用0.9%氯化钠溶液溶解并稀释制成含特利加压素8.6 μg · mL-1的溶液。
取本品1瓶,加0.9%氯化钠溶液1 mL溶解,制成含特利加压素8.6 mg · mL-1的溶液。
采用Waters Symmetry C18(4.6 mm×250 mm,5 μm)色谱柱,以磷酸二氢钾缓冲液(取磷酸二氢钾6.8 g,庚烷磺酸钠2 g,调节pH至4.1,加水至1 000 mL)-乙腈(90 : 10)为流动相A,磷酸二氢钾缓冲液-乙腈(60 : 40)为流动相B,梯度洗脱(0~60 min,16%B→24%B;60~85 min,24% B→60%B;85~86 min,60% B→16%B;86~95 min,16%B),流速0.7 mL · min-1,检测波长210 nm,进样量20 μL,柱温35 ℃。
准确量取对照品溶液、供试品溶液分别进样,调节流动相比例,使主成分特利加压素保留时间约为65 min,记录色谱图,按主成分对照品外标法计算杂质含量,结果见表1。采用杂质对照品定位,杂质2和3、5和6、8和9、9和特利加压素、特利加压素和10分离度分别为1.0、1.4、1.6、1.3、2.9,见图2
取A~D企业样品,分别在30 ℃(60%RH)、15 ℃(60%RH)放置5个月后进行比较。放置于30 ℃的样品杂质增长幅度显著高于放置于15 ℃的样品,特别是杂质[Gly12-OH]特利加压素和[Glu7]特利加压素,二者均为水解杂质。各企业样品在30 ℃下的总杂质量均高于15 ℃下的,其中企业B差异最大,达到0.57%,但各企业总杂质均未超限度(限度为5.0%)。见图2
精密称取醋酸特利加压素、(4,9’),(4’,9)-双二硫键特利加压素二聚体、(1’,12)-脱氨特利加压素二聚体对照品适量,用水溶解并稀释成含醋酸特利加压素0.86 mg · mL-1、(4,9’),(4’,9)-双二硫键特利加压素二聚体10 μg · mL-1、(1’,12)-脱氨特利加压素二聚体10 μg · mL-1的混合溶液。
取本品1瓶,加水1 mL溶解,摇匀,即得。
采用TSK GEL G2000SWXL(7.8 mm×300 mm,5 μm)色谱柱,以三氟乙酸-乙腈-水(0.5 : 500 : 500)为流动相,流速0.7 mL · min-1,进样体积20 μL,检测波长220 nm,柱温35 ℃。
 准确量取系统适用性溶液、供试品溶液进样,记录色谱图(图34),(4,9’),(4’,9)-双二硫键特利加压素二聚体与(1’,12)-脱氨特利加压素二聚体的分离度为1.5,(1’,12)-脱氨特利加压素二聚体与特利加压素的分离度为3.5。按归一化法计算聚合物含量,结果见表2
样品分别在30 ℃(60%RH)、15 ℃(60%RH)下放置5个月后,进行比较,放置于30 ℃样品聚合物增长幅度显著高于放置于15 ℃样品。特别是(1’,12)-脱氨特利加压素二聚体差异较为明显,其中企业A样品30 ℃下聚合物总量达到0.62%,超出了限度要求(限度为0.5%)。
各企业长期稳定性试验均在25 ℃下展开,而贮藏温度从原研药“Room temperature”直译为“室温”,企业A样品在“室温”上限30 ℃下储存仅5个月,聚合物即超限,而本品有效期为2年,可见长期稳定性试验条件无法体现说明书贮藏条件下的稳定性。因此,应根据长稳试验条件将国内产品贮藏条件设置为“不超过25 ℃保存”。本研究为2023年国家评价性抽检注射用特利加压素品种,抽样、运输、贮藏环节,均规范记录贮藏条件,可以排除其他因素对试验结果的影响。
高温酸性条件下,会促进肽键断裂,通过质子化羰基氧,增强亲和试剂(水分子)对羰基碳的攻击,导致肽键断裂生成游离氨基酸或断裂肽;高温碱性条件下,氢氧根离子攻击肽键的羰基碳原子,形成中间体后断裂生成[12-13]。注射用特利加压素N端未进行乙酰化,存在2个伯氨基,是碱性最强的合成多肽类药物之一。为避免酰胺键碱性水解,各企业在冻干前均使用盐酸调节pH至3.0~4.0,注射用特利加压素酸性较强,在温度较高时,发生酸催化水解反应,是导致有关物质水解杂质[Gly12-OH] 特利加压素和[Glu7]特利加压素含量升高的原因。
分析较高的贮存温度下脱氨二聚体增长的机理[14-15]。特利加压素分子结构中,1位和9位含有伯氨基,而7/8/12位含有酰胺基,伯氨基分子间亲和进攻酰胺,发生酰胺取代反应,生成脱氨二聚体和氨气(图5)。该聚合反应的发生需满足一定条件:一方面,发生亲核进攻需要较强的酸性使酰胺键形成质子化羰基,使碳氧双键T电子云更倾向于氧端,碳原子亲电能力增强,吸引伯氨基孤对电子能力增强,本制剂中的成盐酸根盐酸已经提供了上述酸性条件;另一方面,虽然该反应的最终产物能级与反应底物相似,且产生气体,熵变为正,动力学判断反应可以进行。但是反应过程中需要跨越能量壁垒,生成质子化加和物,此后该反应才能自发发生,且反应需要打破生成氨气的原有氢键。因此该酰胺交换反应由热力学控制,贮藏温度升高促进了该聚合反应的发生。
欧洲药品管理局(EMA)2007年颁布的《对储存条件的声明的基准指南》,要求药品说明书标识的贮藏条件应参考样品稳定性研究结果。稳定性研究的试验温度与说明书贮藏温度上限一致,是保证药品在贮藏期间质量不受环境因素影响的重要因素之一[16-18]。对于贮藏温度的描述依据来源于《中国药典》凡例[19]项下“二十、贮藏项下的规定”的要求。查阅USP[20]、JP[21]及EP[22],贮藏温度设定存在差异(见表3),其中“室温、冷处、阴凉处”是常见的贮藏温度表述方式,各国药典要求的温度上限及范围均不同。其中,《中国药典》“阴凉处温度≤20 ℃”,温度设定宽泛,实际贮藏可放于阴凉库或冰箱冷藏;“室温”等同于常温,存在阴凉库、不特殊控制温度贮藏2种方式。注射用特利加压素的贮藏温度上限与稳定性温度不一致,源于对原研药说明书的英文直译,忽略了不同国家药典温度要求差异造成的。该品种对温度敏感,应严格控制贮藏温度,建议企业更改说明的贮藏温度“室温”为“不超过25 ℃保存”。同样,《中国药典》与其他药典“阴凉处”的温度差异,字面直译同样会导致贮藏温度放宽,无法保障药物贮藏稳定性。综上所述,贮藏温度应尽量避免“室温”“冷处”等表述,由于含义不明确,不同地方含义也不同。使用明确的温度说明储存条件,如“不超过25 ℃”或“2~8 ℃”等,不会产生贮存温度选择歧义,才能保证药品的质量可控。
