Objective To explore the effects of repetitive transcranial magnetic stimulation (rTMS) combined with mindfulness training on sleep quality, clinical symptoms and suicide risk during depressive episodes, as well as gender differences, and observe the safety of the treatment.
Methods A total of 200 patients with depressive episodes admitted to Huizhou Second People's Hospital from July 2024 to April 2025 were included for analysis. They were divided into control group, a mindfulness group, an rTMS group, and a combined group by random number table method, with 50 cases in each group, and then subgroups were further divided according to gender. The control group received conventional Western medicine treatment, the mindfulness group received mindfulness training based on the control group, the rTMS group received rTMS based on the control group, and the combined group received rTMS combined with mindfulness training based on the control group. The sleep monitoring indicators [total sleep time (TST), sleep latency (SL), wake time (WASO), sleep efficiency (SE), proportion of non-rapid eye movement sleep stage 1 (N1), stage 2 (N2), stage 3 (N3), proportion of rapid eye movement (REM)], clinical symptoms [Hamilton depression/anxiety scale (HAMD/HAMA)], nurses' global assessment of suicide risk (NGASR), and the occurrence of adverse reactions were compared between the four groups, as well as the gender differences of the above indicators between the four groups.
Results Compared with before treatment, the sleep monitoring indicators of four groups improved after treatment, and the improvement of men and women in the combined group was respectively better than that of the control group of the same sex (P<0.05). The HAMD and HAMA scores of four groups decreased after treatment, and the scores of men and women in the combined group were respectively lower than those of the same-sex group in the control group (P<0.05). The NGASR scores of four groups after treatment were lower than those before treatment, and the scores of men and women in the combined group were respectively lower than those of the same-sex group in the control group (P<0.05). Comparison of the overall incidence of adverse reactions between the four treatment groups showed no statistically significant differences (P>0.05). There was no statistically significant difference in each index among patients of different genders after treatment in the same group (P>0.05).
Conclusion rTMS combined with mindfulness training can effectively improve the sleep quality and clinical symptoms of patients with depressive episodes, reduce the risk of suicide, and does not increase the risk of adverse reactions. However, no significant gender differences were found in terms of its effectiveness and safety.

目的 探究重复经颅磁刺激（repetitive transcranial magnetic stimulation，rTMS）治疗联合正念训练对抑郁发作患者睡眠质量、临床症状、自杀风险的影响及性别差异，并观察治疗安全性。
方法 选取2024年7月至2025年4月惠州市第二人民医院收治的200例抑郁发作患者进行分析，以随机数字表法分为对照组、正念组、rTMS组和联合组，各50例，再根据性别分别分亚组。对照组接受常规西药治疗，正念组基于对照组予以正念训练，rTMS组基于对照组予以rTMS治疗，联合组基于对照组予以rTMS治疗联合正念训练。比较4组睡眠监测指标[睡眠总时间（total sleep time，TST）、入睡潜伏期（sleep latency，SL）、觉醒时间（wake after sleep onset，WASO）、睡眠效率（sleep efficiency，SE），非快速眼动睡眠1期（N1）、2期（N2）、3期（N3）占比，快速眼动睡眠（rapid eye movement，REM）占比]、临床症状[汉密尔顿抑郁量表（Hamilton depression scale，HAMD）与焦虑量表（Hamilton anxiety scale，HAMA）评分]、自杀风险评估量表（nurses' global assessment of suicide risk，NGASR）评分、不良反应发生情况，以及上述各指标在4组间的性别差异。
结果 4组治疗后的睡眠监测指标较治疗前均有改善，且联合组男性、女性改善程度均优于对照组同性群体（P<0.05）；4组治疗后的HAMD、HAMA评分较治疗前均降低，且联合组男性、女性评分均低于对照组同性群体（P<0.05）；4组治疗后的NGASR评分较治疗前均降低，且联合组男性、女性评分均低于对照组同性群体（P<0.05）；4组治疗期间不良反应总发生率比较，差异均无统计学意义（P>0.05）；各指标在同组治疗后不同性别患者间的比较均无统计学差异（P>0.05）。
结论 rTMS治疗联合正念训练可有效改善抑郁发作患者睡眠质量及临床症状，降低自杀风险，且不增加不良反应发生风险，同时其改善效果与安全性，并未发现存在明显性别差异性。
