Objective To analyze the differences in fat deposition in the pancreas, kidneys and vertebral bodies between patients with metabolic syndrome (MetS) and healthy subjects using the mDixon-Quant quantitative technique, and to investigate metabolic parameters among MetS patients with different visceral fat deposition patterns.
Methods This prospective study enrolled 58 MetS patients admitted to the Department of Endocrinology, the Second Affiliated Hospital of Mudanjiang Medical University between June 2023 and September 2024 into the MetS group. Meanwhile, 52 healthy volunteers were recruited as the control group. The fat fraction (FF) and T2* values of the pancreas, renal cortex, renal medulla and vertebral bodies were measured via mDixon-Quant sequence. Clinical data including waist circumference, body mass index (BMI), triglycerides, total cholesterol and high-density lipoprotein cholesterol were collected. The K-means clustering algorithm was applied to stratify MetS patients into subgroups according to fat deposition patterns, and metabolic indicators were compared across subgroups.
Results Waist circumference, BMI, triglyceride levels, as well as FF values of the pancreas, renal cortex and vertebral bodies were significantly higher in the MetS group than in the control group, whereas high-density lipoprotein cholesterol levels and T2* values of the pancreas, renal cortex and renal medulla were markedly lower (all P<0.05). Clustering analysis classified MetS patients into three phenotypes: multi-organ fat deposition, pancreas-skeleton predominant fat deposition, and kidney-predominant fat deposition. Patients with multi-organ fat deposition exhibited significantly higher waist circumference, triglycerides and total cholesterol compared with the other two subgroups (P<0.05).
Conclusion The FF values of the pancreas, kidneys and vertebral bodies are significantly elevated in MetS patients. Among all phenotypes, individuals with multi-organ fat deposition present higher triglyceride and total cholesterol concentrations. The concept of multi-organ synergistic lesions can provide imaging evidence for precise subtyping and risk stratification of metabolic syndrome.

目的 基于mDixon-Quant量化技术分析代谢综合征（metabolic syndrome，MetS）患者与健康人群胰腺、肾及椎体的脂肪沉积差异，探讨MetS患者在不同类型内脏脂肪沉积模式下的代谢参数。
方法 采用前瞻性研究，选取2023年6月至2024年9月牡丹江医科大学附属第二医院内分泌科收治的MetS患者58例纳入MetS组，同期招募健康人群52例纳入对照组。mDixon-Quant检查获取两组研究对象胰腺、肾（皮质和髓质）及椎体的脂肪分数（fat fraction，FF）值和T2*值，收集两组研究对象腰围、身体质量指数（body mass index，BMI）、甘油三酯、总胆固醇及高密度脂蛋白等临床指标。基于K-means聚类算法，根据脂肪沉积模式将MetS组患者分为不同亚组，比较不同亚组各代谢组分的差异。
结果 MetS组患者的腰围、BMI、甘油三酯以及胰腺、肾皮质和椎体的FF值均高于对照组，高密度脂蛋白以及胰腺、肾皮质及肾髓质的T2*值均低于对照组，以上差异均有统计学意义（P<0.05）。根据聚类分析结果，MetS组患者脂肪沉积模式分为多器官脂肪沉积型、胰腺-骨骼脂肪沉积主导型及肾脂肪沉积主导型，不同亚组比较结果显示，多器官脂肪沉积型亚组患者腰围、甘油三酯及总胆固醇水平均高于其他亚组，差异有统计学意义（P<0.05）。
结论 MetS患者胰腺、肾及椎体FF值显著高于健康人群，其中多器官脂肪沉积型MetS患者甘油三酯和总胆固醇水平均明显高于其他脂肪沉积模式，这种“多器官协同病变”的视角可为MetS精准分型和风险分层提供影像学依据。
