目的

探讨低频脉冲电刺激联合倍他司汀注射液治疗突发性耳聋（Sudden Sensorineural Hearing Loss，SSNHL）对耳蜗功能与血液流变学指标的影响。

方法

研究对象来源于2019年1月—2025年1月上海市普陀区利群医院耳鼻喉科收治的SSNHL患者，共计102例，入选的SSNHL患者根据不同治疗方案分为对照组和试验组各51例。对照组采取倍他司汀注射液治疗，试验组在此基础上联合低频脉冲电刺激治疗。对比两组患者治疗前后耳蜗功能与听阈增益情况以及血流变学相关指标[全血粘度高切变率（High Shear Rate Blood Viscosity，HSR-BV）、全血黏度低切变率（Low Shear Rate Blood Viscosity，LSR-BV）、血浆黏度（Plasma Viscosity，PV）]变化。

结果

治疗后，试验组听力功能缺损分贝数、纯音气导听阈值均低于本组治疗前和同期对照组，差异均有统计学意义（P<0.05）。试验组听阈值增益数值明显高于试验组，差异有统计学意义（P<0.05）。试验组ABR测试结果改善总有效率明显高于对照组，差异有统计学意义（P<0.05）。治疗后，试验组PV、LSR-BV与HSR-BV均低于本组治疗前和同期对照组，差异均有统计学意义（P<0.05）。

结论

低频脉冲电刺激联合倍他司汀注射液治疗SSNHL可发挥显著的协同作用，改善微循环，调节血液流变学，增强神经保护，优化耳蜗功能，提升听力水平。

Objective

To explore the effects of low-frequency pulsed electrical stimulation combined with betahistine injection on cochlear function and hemorheological indicators in patients with sudden sensorineural hearing loss (SSNHL).

Methods

A total of 102 patients with SSNHL admitted to the Department of Otorhinolaryngology of Liqun Hospital, Putuo District, Shanghai from January 2019 to January 2025 were selected and divided into a control group and an experimental group, each consisting of 51 patients, based on different treatment regimens. The control group was treated with betahistine injection, while the experimental group received low-frequency pulsed electrical stimulation in addition to the same treatment. The changes in cochlear function, hearing threshold gain, and hemorheological indicators [high shear rate blood viscosity (HSR-BV), low shear rate blood viscosity (LSR-BV), and plasma viscosity (PV)] before and after treatment were compared between the two groups.

Results

After treatment, the hearing loss decibel number and pure tone air conduction hearing threshold value in the experimental group were lower than those before treatment and those in the control group at the same time, and the differences were statistically significant (P<0.05). The hearing threshold gain value in the experimental group was significantly higher than that in the control group, and the difference was statistically significant (P<0.05). The total effective rate of ABR test results in the experimental group was significantly higher than that in the control group, and the difference was statistically significant (P<0.05). After treatment, the PV, LSR-BV, and HSR-BV in the experimental group were lower than those before treatment and those in the control group at the same time, and the differences were statistically significant (P<0.05).

Conclusion

Low frequency pulse electrical stimulation combined with betahistine injection can play a significant synergistic role in the treatment of SSNHL, improving microcirculation, regulating hemorheology, enhancing neuroprotection, optimizing cochlear function, and improving hearing level.
