OBJECTIVE To analyze the characteristics of methylphenidate (MPH) related treatment emergent adverse events (TEAE) in children, so as to provide recommendations for prevention of MPH safety events. METHODS Case reports of MPH-induced TEAE in children were retrieved from CNKI, VIP, Wan Fang, PubMed, Web of Science, EMbase and other databases, and the gender, age, dosage, drug combination, time of adverse reactions and clinical manifestations of the patients were statistically analyzed. RESULTS A total of 39 case reports were collected, involving 50 cases, including 47 cases in the treatment group and 3 cases in the non-treatment group. In the treatment group, the ratio of male to female was 3.7∶1, with 40 cases (80%) in patients aged 6-15 years. Most TEAEs occurred within 1-30 d after drug administration (32 cases, 64%), and the TEAEs mainly involved mental system (12 cases, 21.43%) and nervous system (11 cases, 19.64%). In the non-treatment group, 2 patients were overdosed, and adverse reactions occurred within 1 h, mainly involving mental system and nervous system. Among the 50 cases, 49 cases were improved after drug withdrawal and/or symptomatic treatment, and 1 case had unknown prognosis. CONCLUSION It is necessary to prevent the possible safety problems in the use of MPH, so as to improve the level of rational drug use.

目的 分析盐酸哌甲酯(methylphenidate,MPH)导致的儿童治疗相关不良事件(treatment emergent adverse events,TEAE)的特点,提出防范MPH安全事件的建议。方法 检索中国知网、维普、万方、PubMed、Web of Science、EMbase等数据库中MPH致儿童TEAE的个案报告,对患者的性别、年龄、用药剂量、合并用药情况、TEAE出现时间及临床表现等进行统计分析。结果 收集个案报告39篇,涉及患者50例,其中治疗组47例,非治疗组3例。治疗组中男女比为3.7∶1,6~15岁患者比例最高,共40例(80%),MPH致儿童TEAE多发生在给药后1~30 d内(32例,64%),主要累及精神系统(12例,21.43%)和神经系统(11例,19.64%)。非治疗组中2例患者为超剂量使用,TEAE均发生在1 h内,主要累及神经系统。50例患者中49例好转,1例不详。结论 在使用MPH时,应对其可能发生的安全问题加以防范,从而提高合理用药水平。

目的 分析盐酸哌甲酯(methylphenidate,MPH)导致的儿童治疗相关不良事件(treatment emergent adverse events,TEAE)的特点,提出防范MPH安全事件的建议。方法 检索中国知网、维普、万方、PubMed、Web of Science、EMbase等数据库中MPH致儿童TEAE的个案报告,对患者的性别、年龄、用药剂量、合并用药情况、TEAE出现时间及临床表现等进行统计分析。结果 收集个案报告39篇,涉及患者50例,其中治疗组47例,非治疗组3例。治疗组中男女比为3.7∶1,6~15岁患者比例最高,共40例(80%),MPH致儿童TEAE多发生在给药后1~30 d内(32例,64%),主要累及精神系统(12例,21.43%)和神经系统(11例,19.64%)。非治疗组中2例患者为超剂量使用,TEAE均发生在1 h内,主要累及神经系统。50例患者中49例好转,1例不详。结论 在使用MPH时,应对其可能发生的安全问题加以防范,从而提高合理用药水平。

OBJECTIVE To analyze the characteristics of methylphenidate (MPH) related treatment emergent adverse events (TEAE) in children, so as to provide recommendations for prevention of MPH safety events. METHODS Case reports of MPH-induced TEAE in children were retrieved from CNKI, VIP, Wan Fang, PubMed, Web of Science, EMbase and other databases, and the gender, age, dosage, drug combination, time of adverse reactions and clinical manifestations of the patients were statistically analyzed. RESULTS A total of 39 case reports were collected, involving 50 cases, including 47 cases in the treatment group and 3 cases in the non-treatment group. In the treatment group, the ratio of male to female was 3.7∶1, with 40 cases (80%) in patients aged 6-15 years. Most TEAEs occurred within 1-30 d after drug administration (32 cases, 64%), and the TEAEs mainly involved mental system (12 cases, 21.43%) and nervous system (11 cases, 19.64%). In the non-treatment group, 2 patients were overdosed, and adverse reactions occurred within 1 h, mainly involving mental system and nervous system. Among the 50 cases, 49 cases were improved after drug withdrawal and/or symptomatic treatment, and 1 case had unknown prognosis. CONCLUSION It is necessary to prevent the possible safety problems in the use of MPH, so as to improve the level of rational drug use.

注意缺陷多动症(attention deficit hyperactivity disorder,ADHD)是一种常见的慢性神经发育障碍,起病于童年期,影响可延续至成年,其主要特征是与发育水平不相称的注意缺陷和(或)多动冲动。全球儿童发病率约为7.2%[1]。据统计,我国儿童ADHD患病率为6.26%[2],且其患病率在逐年升高。作为一种常见的儿童精神障碍疾病,严重影响患儿的学习、家庭和社会生活,并带来沉重的负担。盐酸哌甲酯(methylphenidate,MPH)是一种源自哌啶的中枢神经系统兴奋药,其结构类似于苯丙胺,作为去甲肾上腺素-多巴胺再摄取抑制剂,通过增加这些单胺类物质释放至神经元外间隙,对ADHD发挥治疗作用。说明书记载该药用于儿童和青少年治疗相关不良事件(treatment emergent adverse events, TEAE)最常报告为失眠、头晕、口咽疼痛、胃肠系统疾病及发热等。MPH作为国内外儿童及青少年ADHD的一线治疗药物,广泛应用于临床,在国内其属于第一类精神药品,也被称为“聪明药”,因可增加机体的兴奋性,提高孩子的注意力,改善脑部功能,导致该药滥用,但是长期使用易造成患者依赖性,因此MPH用于儿童的TEAE是越来越令人关注的问题。本研究通过查阅国内外个案报道,分析MPH所致儿童TEAE的特点,为临床安全用药提供参考。
通过检索中国知网、维普、万方、PubMed、Web of Science、EMbase等数据库,中文检索关键词为:哌甲酯、不良反应、不良事件、毒性反应。英文检索关键词为:Methylphenidate、case、adverse、side effect、induced。
纳入标准:国内外公开发表的MPH所致0~18岁儿童TEAE的临床个案报告,内容包含患者基本信息、用药信息、临床表现及治疗转归等信息。
排除标准:①基本信息不完整,描述不详细导致无法分析;②MPH致儿童TEAE的文献综述;③动物实验基础研究。由2位研究者按照文献纳入标准和排除标准独立进行文献检索,初步筛选后,最终由小组讨论文献是否纳入。
采用回顾性研究的方法,对纳入的文献提取病例相关信息,包括患者性别、年龄、临床诊断、给药剂量、给药后出现TEAE的时间、TEAE临床表现、联合用药情况及治疗转归。根据患者用药目的分为治疗组和非治疗组。根据国际通用的Naranjo's ADR评估表进行TEAE的关联性评价,总分≥9分,表明该药物与TEAE的因果关系为肯定的;总分为5~8分,为很可能有关;总分为1~4分,为有可能有关。总分≤0分,为可疑。
共检索到MPH致儿童TEAE的临床个案报告39篇,其中国内2篇,国外37篇,纳入病例数为50例。
治疗组中男性37例(78.72%),女性10例(21.27%),男女之比为3.7∶1,年龄2~18岁,平均年龄(9.77±3.50);6~15岁患者比例最高,共40例(80%),见表1
MPH的说明书中指出儿童使用剂量范围一般不超过36 mg·d-1,治疗组儿童使用剂量范围为5~36 mg·d-1,非治疗组三例患者的剂量及年龄分别为486 mg(18岁)、540 mg(12岁),10 mg(2岁),具体用药情况及合并用药情况见表2
50例MPH致儿童TEAE中,治疗组用药原因为2种,分别为ADHD和发作性睡病。非治疗组用药原因为2种,分别为自杀和误服。50例患者均为口服给药。
治疗组患者发生TEAE主要累及精神系统,共计12例(21.43%),其次各类神经系统,共计11例(19.64%),皮肤及附件系统,共计10例(17.86%),生殖系统及乳腺疾病,共计8例(14.29%),血液及淋巴系统疾病,共计3例(5.36%),呼吸系统、胸疾病,共计3例(5.36%),眼器官疾病,共计3例(5.36%),心血管疾病,共计1例(1.79%)。非治疗组患者发生TEAE主要累及精神系统,共计2例(3.57%),各类神经系统,共计2例(3.57%),心血管疾病,共计1例(1.79%)。其中,有6名患者同时出现了多个器官系统的TEAE主要临床表现,见表3
MPH致儿童TEAE多发生在给药后1~7 d内(17例,占比34%),其次是给药后7~30 d内(15例,占比30%),其中非治疗组中2例患者超剂量使用均在1 h内出现TEAE,见表4
根据国际通用的Naranjo's TEAE评估量表,对50例MPH致儿童TEAE进行了关联性评价,其中总分5~8分的为44例(88%),表示为很可能有关。1~4分的为6例(12%),表示为有可能有关。
使用MPH发生TEAE的儿童中,49例患儿最终TEAE消失或缓解,其中43例(86%)患儿经停药及/或对症治疗后症状消失或缓解,症状好转的时间不等,最短的3~4 h症状消失。3例患儿在出现TEAE后未停止给药,也没有降低给药剂量,随着给药时间的延长,TEAE逐渐减轻或消失。1例患儿在出现TEAE后给予对症治疗的药物,并继续给予MPH治疗,TEAE逐渐消失。1例患儿在出现TEAE后,逐渐降低MPH的治疗剂量,TEAE逐渐消失。1例患儿合并用MPH和吲哚美辛出现精神病症状,停用吲哚美辛后,TEAE症状消失,前期单独用吲哚美辛和MPH均未出现任何TEAE。1例患者转归不详。
根据TEAE的关联性评价结果,有6例患者Naranjo's TEAE评估量表得分为1~4分,表示TEAE与MPH之间的关系是有可能有关。其中1例患者在使用MPH的同时也在使用利培酮,出现TEAE后同时停止了两种药物的摄入,TEAE症状得到缓解;1例14岁女孩在使用MPH 3个月内没有出现TEAE,近期由于经痛吃了消炎痛出现精神病症状TEAE,以往单独使用消炎痛未出现过此TEAE,停止使用消炎痛后,TEAE症状消失,且未再复发。1例患者被诊断为癫痫一直在使用丙戊酸钠治疗,后来被诊断为ADHD后开始使用MPH后,5 h出现急性口面部和四肢运动障碍TEAE,停止使用MPH后TEAE得到缓解。1例患者在使用MPH第10天后出现眉毛脱落,鼻出血TEAE,第14天停止该药物,TEAE得到了缓解,Naranjo's TEAE评估量表得到为4分。1例患者在使用MPH时剂量由5 mg·d-1增加到10 mg·d-1后出现急性心肌梗死,停药后给予对症治疗,TEAE得到改善,Naranjo's TEAE评估量表得到为3分。1例患者使用MPH 3年后多次出现自发性气胸,停止药物后症状得到缓解,Naranjo's TEAE评估量表得到为3分。
本研究结果显示,治疗组中MPH所致的TEAE发生率男女比例为3.7∶1,性别差异显著。最新的一项研究报道了法国2010—2019年儿童和青少年 ADHD 诊断和MPH的使用情况,结果显示ADHD患者中,男性约占80%,女性约占20%[3]。且英国的一项研究显示,小于16岁儿童ADHD患者中,男性比例为84%[4]。美国的一项研究显示,儿童和青少年男性和女性的ADHD患病率分别为11.5%和5.16%[5]。复旦大学附属儿科医院2006—2010年ADHD就诊人次数进行统计分析后的结果也符合这一特性[6]。因此认为,儿童ADHD病例中男性患病率高于女性,且MPH所致的儿童TEAE男性高于女性,提示家长需要更加关注男孩出现ADHD症状。治疗组中47例患者中MPH所致TEAE中6~15岁患儿人数最多,为40例(80%),表明学龄期儿童更容易发生ADHD。
本研究结果显示治疗组中MPH致儿童TEAE出现的时间主要发生在用药后1~30 d内(64%),其中7~30 d出现TEAE的比例为30%,这提示在药品TEAE监测工作中,对该药要进行长期监测,可能与其是缓释制剂有关联。其中16例(32%)是发生在剂量增加期间,这提示在增加MPH剂量期间,要密切关注TEAE的发生,加强药学监护。非治疗组中2例超剂量患者出现TEAE均在用药后1 h内,与其药动学有关,MPH口服吸收迅速,1~2 h内达到初始最大值。
MPH致儿童TEAE主要累及精神系统,其次为各类神经系统。MPH致精神症状的TEAE表现主要为精神病症状、梦游综合征、躁狂症状、自杀意念、抑郁症、幻视和磨牙,与以往的研究结果基本一致。在MPH治疗期间,抑郁情绪或精神症状可能是会出现的TEAE。Park-Wyllie等[7]通过TEAE数据库分析了MPH致儿童TEAE的个案报告,其中精神症状的TEAE主要表现为自杀意念、抑郁症、幻视等。唇舌咬伤是说明书未记载的新的TEAE。1例患者被确诊为ADHD,在使用MPH治疗的时候,增加剂量至36 mg·d-1,2周后出现唇舌咬伤TEAE[8],这是首次报道MPH关于唇舌咬伤的TEAE。在过去的一项针对大鼠和兔子的动物研究中,随着MPH剂量的增加,大鼠和兔子自我唇舌咬伤的比例增加[9]。这提示兴奋剂药物可能会导致咬伤嘴唇和舌头的行为,这可能会显著影响药物治疗依从性。此外,1例患者在使用MPH治疗ADHD期间,出现狂躁症,该患者被诊断为痤疮和ADHD,同时在使用异维甲酸治疗,在治疗过程中单用MPH和异维甲酸均未出现TEAE,联合用药时出现狂躁症[10]。Bremner等[11]讨论了抑郁症、自杀等精神病症状与异维甲酸的关系,异维甲酸在中枢神经系统中发挥着多种功能,可对神经系统产生多种影响,认为异维甲酸的使用与抑郁和自杀存在着一定的关联。这提示对于接受异维甲酸和MPH联合治疗的患者,应谨慎注意躁狂症TEAE。
MPH累及各类神经系统的TEAE表现主要为运动障碍,恶心、晕厥,打嗝。7例患儿在使用MPH后出现运动障碍,其中1例转归不详;3例停药后及/或对症治疗症状消失;3例继续给药或继续给药同时给予对症治疗药物后症状消失。抗精神病药物由于其多巴胺能系统阻断作用,被报道可引起运动障碍TEAE[12]。本研究检索到1例患儿在使用MPH后出现了口面部和肢体运动障碍,该患儿被诊断患有癫痫,同时在服用丙戊酸钠治疗,停用MPH后15 h症状缓解[13]。有研究报道抗癫痫治疗可增加MPH引起TEAE的敏感性[14]。此外,1例患者使用MPH后出现口面部和肢体运动障碍,该患儿被诊断为轻度智力迟钝和ADHD[15]。Balazs等[16]和Senecky等[17]报道了智商较低或智力功能边缘的患者在MPH治疗后立即出现口面部和/或肢体运动障碍。因此,在抗ADHD治疗中,智商偏低的儿童和癫痫患者更易发生运动障碍TEAE,且会持续的时间更长,需要密切监测TEAE的发生。
MPH累及皮肤及其附件的TEAE表现主要为脱发、剥脱性黄斑丘疹、瘙痒性皮肤反应、红斑和丘疹及眉毛缺失,均在说明书中有记载。本研究检索到1例MPH致白癜风的个案报道[18],这也是说明书未记载的皮肤相关TEAE。目前导致白癜风皮损黑素细胞丢失的分子机制仍不清楚,但已经提出了多种相关假说,其中包括自身免疫病学假说和神经学假说,根据神经学假说,一些神经递质和儿茶酚胺降解酶水平的升高会损伤黑素细胞[19]。Cucchi等[20]研究发现,在非节段性白癜风活动中,儿茶酚胺水平升高,白癜风黑色素细胞减少,该研究结果支持了神经假说。MPH可增加多巴胺和去甲肾上腺素的水平,从而导致白癜风的发生。这也是首次报道口服MPH导致白癜风的发生,但因果关系不能用单一案例来证明,因此,MPH与白癜风之间的关系还需要进一步的研究来证明,但该病例可为临床医生提供关于口服MPH可能导致白癜风的参考。
MPH累及生殖系统及乳腺疾病的TEAE主要为青春期前儿童异常勃起、男性乳房发育症和遗尿,共8例,出现TEAE后停药及/或给予对症治疗后症状均缓解。MPH的主要作用机制是抑制纹状体区域多巴胺的再摄取,有证据表明多巴胺激动剂可导致阴茎勃起,可能对治疗勃起功能障碍有效[21]。因此MPH诱导的阴茎勃起可能是通过过度的多巴胺活性介导的。目前报道的MPH诱导的青春期前儿童异常勃起病例中,多数患者在停药后症状消失。在MPH治疗中,需要密切注意患儿泌尿生殖系统出现TEAE,出现异常阴茎勃起需进行泌尿系统评估,以减少长期并发症的风险。
MPH累及血液及淋巴系统疾病、呼吸系统、心胸疾病、五官疾病及心血管疾病的TEAE主要表现为血小板减少症、白细胞减少、高热、声音嘶哑、复发性气胸、视物障碍、鼻出血及高血压,均在说明书中有记载。1例患儿服用MPH后出现急性心肌梗死,MPH致心肌梗死被认为是一种非常罕见的TEAE(小于万分之一),且青少年和成人相对更易发生[22]。大部分流行病学研究显示MPH用于治疗ADHD患者可增加心律失常的总体风险,不会增加心肌梗死的风险,但在使用MPH治疗的早期阶段,发生心血管事件风险显著性增加[23-24]。虽然心肌梗死在儿童中很罕见,但在MPH治疗的早期,一旦出现疑似急性冠状动脉综合征,也必须立即进行冠状动脉造影。
本研究收录的个案报道中使用治疗剂量MPH发生TEAE的98%患者后期TEAE症状消失或缓解。MPH过量的患者,1例患者服用540 mg MPH,出现急性拟交感神经毒性,表现为运动痉挛、全身刺疼感、心动加速、幻视和高血压,1 h内给予活性炭和生理盐水,1 d后出院[25]。另1例患者服用486 g MPH,出现心动过速、高血压和烦躁不安,给予活性炭和美托洛尔治疗,第6天患者体内MPH浓度达到正常值[26]。高剂量MPH相关病例报告显示,药物血浆水平与毒性症状有关系[27],对于MPH血浆水平升高可给予活性炭治疗,且早期推荐使用β-受体阻断剂、α-受体阻断剂和钙通道阻断剂来控制血压和心率异常等症状[28]。高剂量MPH中毒主要引起中枢神经系统的副作用,表现为躁动、心动过速和高血压等,一般是非致命的,且不会导致成瘾,需要监测心血管功能。
综上所述,本研究收录的TEAE涉及多个系统-器官,其中唇舌咬伤、白癜风、打嗝、男性乳房发育症、遗尿、急性心肌梗死为说明书尚未记载的TEAE,这些TEAE在临床相对少见,因此临床对使用MPH患者,不仅要询问常见TEAE,还要考虑少见的TEAE。随着全球ADHD药物使用的增加,应仔细权衡MPH在儿童和青少年中的潜在风险。
