OBJECTIVE To investigatte antitumor agent epirubicin hydrochloride loaded in TempSLE in vitro/in vivo antitumor activity and provide evidence in clinical use of transcatheter arterial chemoembolization(TACE). METHODS In this study, the TempSLE was selected as the embolic agent, and epirubicin hydrochloride for injection was used as the chemotherapy drug. Epirubicin hydrochloride TempSLE was prepared and its physical and chemical properties and drug release behavior were investigated. Human liver cancer HepG2 cells were selected. The anti-tumor activity of epirubicin hydrochloride TempSLE was evaluated in vitro HepG2 cells. The anti-tumor efficacy of EPI TempSLE in HepG2 tumor-bearing nude mice was evaluated by singly administered EPI TempSLE via peri-tumoral injection. RESULTS The method for preparation of EPI TempSLE was simple; the added EPI did not affect the temperature-sensitive characteristics of TempSLE. The released EPI from EPI TempSLE was about 80% at 24 h time point, after that, EPI was sustained release for 216h and nearly released 100%; the in vitro antitumor activity of EPI TempSLE in HepG2 cells was similar with that of EPI; the in vivo anti-tumor efficacy of EPI TempSLE was observed in HepG2 tumor-bearing nude mice. CONCLUSION The obtained results would help to support the clinical application of epirubicin hydrochloride TempSLE in TACE.

目的 经动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)是Ⅱb期和Ⅲa期肝癌患者的首选治疗方案。肽普绅(TempSLE)是2020年经国家药品监督管理局批准上市的、我国自主研发的一种新型温度敏感液体栓塞剂,可用于动脉栓塞治疗肝脏肿瘤,本实验针对肽普绅载带抗肿瘤药物盐酸表柔比星及其相关抗肿瘤性能开展研究,为临床新型TACE的使用提供实验依据。方法 本研究选择肽普绅为栓塞剂,注射用盐酸表柔比星为化疗药物,配制盐酸表柔比星肽普绅,对其理化性质及药物释放行为进行考察,选择人源肝癌HepG2细胞为细胞模型,考察盐酸表柔比星肽普绅的抗肿瘤细胞药效,建立荷瘤裸鼠动物模型,瘤旁注射给予盐酸表柔比星肽普绅,考察其体内抗肿瘤药效。结果 盐酸表柔比星肽普绅配制方法简单;盐酸表柔比星不影响肽普绅的温敏性能;释放实验结果表明,24 h时间,药物释放百分率约为80%,随后,药物缓慢释放,216 h时间点,药物释放百分率接近100%;盐酸表柔比星肽普绅与盐酸表柔比星具有相似的抗肿瘤细胞药效;体内抗肿瘤结果表明,瘤旁注射给予盐酸表柔比星肽普绅,可产生明显的体内抗肿瘤药效。结论 本实验结果为盐酸表柔比星肽普绅的TACE临床应用提供有益的数据和支持。

目的 经动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)是Ⅱb期和Ⅲa期肝癌患者的首选治疗方案。肽普绅(TempSLE)是2020年经国家药品监督管理局批准上市的、我国自主研发的一种新型温度敏感液体栓塞剂,可用于动脉栓塞治疗肝脏肿瘤,本实验针对肽普绅载带抗肿瘤药物盐酸表柔比星及其相关抗肿瘤性能开展研究,为临床新型TACE的使用提供实验依据。方法 本研究选择肽普绅为栓塞剂,注射用盐酸表柔比星为化疗药物,配制盐酸表柔比星肽普绅,对其理化性质及药物释放行为进行考察,选择人源肝癌HepG2细胞为细胞模型,考察盐酸表柔比星肽普绅的抗肿瘤细胞药效,建立荷瘤裸鼠动物模型,瘤旁注射给予盐酸表柔比星肽普绅,考察其体内抗肿瘤药效。结果 盐酸表柔比星肽普绅配制方法简单;盐酸表柔比星不影响肽普绅的温敏性能;释放实验结果表明,24 h时间,药物释放百分率约为80%,随后,药物缓慢释放,216 h时间点,药物释放百分率接近100%;盐酸表柔比星肽普绅与盐酸表柔比星具有相似的抗肿瘤细胞药效;体内抗肿瘤结果表明,瘤旁注射给予盐酸表柔比星肽普绅,可产生明显的体内抗肿瘤药效。结论 本实验结果为盐酸表柔比星肽普绅的TACE临床应用提供有益的数据和支持。

OBJECTIVE To investigatte antitumor agent epirubicin hydrochloride loaded in TempSLE in vitro/in vivo antitumor activity and provide evidence in clinical use of transcatheter arterial chemoembolization(TACE). METHODS In this study, the TempSLE was selected as the embolic agent, and epirubicin hydrochloride for injection was used as the chemotherapy drug. Epirubicin hydrochloride TempSLE was prepared and its physical and chemical properties and drug release behavior were investigated. Human liver cancer HepG2 cells were selected. The anti-tumor activity of epirubicin hydrochloride TempSLE was evaluated in vitro HepG2 cells. The anti-tumor efficacy of EPI TempSLE in HepG2 tumor-bearing nude mice was evaluated by singly administered EPI TempSLE via peri-tumoral injection. RESULTS The method for preparation of EPI TempSLE was simple; the added EPI did not affect the temperature-sensitive characteristics of TempSLE. The released EPI from EPI TempSLE was about 80% at 24 h time point, after that, EPI was sustained release for 216h and nearly released 100%; the in vitro antitumor activity of EPI TempSLE in HepG2 cells was similar with that of EPI; the in vivo anti-tumor efficacy of EPI TempSLE was observed in HepG2 tumor-bearing nude mice. CONCLUSION The obtained results would help to support the clinical application of epirubicin hydrochloride TempSLE in TACE.

原发性肝癌 (primary liver cancer)是2020年全球第六大被诊断的癌症及第三大癌症死亡原因[1]。多数地区男性发病率和死亡率均是女性的2~3倍,男性全球癌症发病率中,肝癌排名第五,死亡率排名第二[1]。2020 年我国肝癌新发病率排第五,死亡率排第二[2]。在所有肝癌病例中,有 90%以上是肝细胞癌(hepatocellular carcinoma, HCC)[3]中国肝癌分期方案 (China liver cancer staging,CNLC)将肝癌分为Ⅰa期、Ⅰb期、Ⅱa期、Ⅱb期、Ⅲa期、Ⅲb期和Ⅳ期,不同阶段采用不同的治疗手段[4],其中,对于 CNLC Ⅱb期和Ⅲa期肝癌患者,经动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)是首选治疗方案。TACE是经动脉栓塞术(transcatheter arterial embolization,TAE) 发展起来的一种治疗方法[5-7]。TAE针对治疗不可切除的肝癌,将微导管超选至肿瘤供血动脉,通过微导管注入栓塞剂,阻断肿瘤动脉血供,造成肿瘤细胞缺血性死亡[8-9]。因此,TAE 中仅含有栓塞剂。但是,TACE 中既含有栓塞剂还包含化疗药物,其中,栓塞剂通过栓塞肿瘤供血动脉引起肿瘤细胞缺血性死亡,化疗药物可产生抗肿瘤药效直接杀伤肿瘤细胞。
临床上常见的栓塞剂[10-11]主要包括机械栓塞剂(如金属线圈)、颗粒栓塞剂(如空白微球和载药微球)、液体栓塞剂[如Onyx、PHIL、N-butyl-2-cyanoacrylate(NBCA)],等等。
肽普绅 (TempSLE) 是我国自主研发的一种新型温度敏感液体栓塞剂,可用于动脉栓塞治疗肝脏肿瘤,于2020年经国家药品监督管理局批准上市。肽普绅含有温敏材料 (经N,N-亚甲基双丙烯酰胺交联的N-异丙基丙烯酰胺和N-正丙基丙烯酰胺的共聚物,结构式见图1A)、显影剂(碘海醇)及溶剂(生理盐水)。肽普绅室温条件下流动性好,经微导管给予,推注顺利,可填充任何形状的血管,体温下迅速温敏响应形成半固体凝胶产生动脉栓塞作用,且可术中显影,实时观察栓塞效果。
盐酸表柔比星是临床常用的抗肿瘤药物,结构式见图1B,可用于肝癌 (包括原发性肝细胞癌和转移性癌) 的治疗,也适用于原发性和转移性肝癌的肝动脉插管化疗,是TACE 常用的化疗药物[12-14]
鉴于 TACE 治疗是Ⅱb期和Ⅲa期肝癌患者的首选治疗方案,本研究选择肽普绅为栓塞剂,注射用盐酸表柔比星为化疗药物,配制盐酸表柔比星肽普绅,对其理化性质及药物释放行为进行考察,选择人源肝癌 HepG2细胞为细胞模型,考察盐酸表柔比星肽普绅的体内外抗肿瘤活性,为盐酸表柔比星肽普绅的TACE临床应用提供有益的数据和支持。
注射用盐酸表柔比星[法玛新,批号 EL8525,规格10 mg,辉瑞制药(无锡)有限公司];肽普绅 (TempSLE,批号 20210901,规格 5 mL,北京冠合医疗科技有限公司);生理盐水;其他材料均为商品化试剂或材料。
电热恒温水浴锅 SYG-A1-2(天津市泰斯特仪器有限公司);傅立叶变换红外光谱仪(ThermoFisher公司,美国);ERV085箱式冷冻干燥机(Millrock公司,美国);场发射扫描电镜FE-SEM(日本电子公司,日本);岛津LC-20AT液相色谱仪、荧光分光光度计(RF-5301PC)(SHIMADZU公司,日本);Bio-rad 680型酶标仪(Bio-rad,美国)。
人源肝癌 HepG2 细胞系(中国医学科学院基础医学研究所细胞资源中心),在含体积分数10%FBS的DMEM培养液中培养,其培养条件为37 ℃和体积分数5% CO2
BALB/c nude裸鼠,雌性,4~6周龄,体重16~18 g,购自北京大学医学部实验动物中心。饲养环境:温度20~25 ℃,湿度50%~60%,自然光照明。动物实验均按照北京大学动物实验指导原则进行,实验动物生成许可证号SCXK (京) 2022-0009。
使用2 mL规格注射器,精密吸取1.5 mL生理盐水依次加入3支注射用盐酸表柔比星西林瓶中,使药物完全溶解,之后,使用该2 mL规格注射器,吸取上述溶解的药液,将该注射器与两通的一端相连接。使用规格为10 mL的注射器,全部吸取1支肽普绅,并与两通的另一端相连,推动注射器,将药液与肽普绅混合20 次左右,直至溶液颜色均匀。所配制的盐酸表柔比星肽普绅含有盐酸表柔比星30 mg,药物质量浓度为4.6 mg·mL-1
肽普绅的胶凝温度:取肽普绅,加适量生理盐水进行稀释,使肽普绅的体积占比分别为100%、90%、80%、70%、60%、50%、40%、30%、20%。分别取上述不同体积占比的肽普绅300 μL,加入4.5 mm口径的玻璃管中,于4 ℃放置15 min,使壁上的凝胶完全沉降于玻璃管底部,之后取出,室温放置15 min使其温度恢复室温。开启恒温水浴锅,精密调控温度,初始温度设置为32 ℃(±0.1 ℃)。通过倒置翻转法测定不同体积占比肽普绅的胶凝温度。
盐酸表柔比星肽普绅的胶凝温度:取盐酸表柔比星肽普绅,经适量生理盐水稀释,得肽普绅体积占比分别为 70%、60%、50%、40% 的盐酸表柔比星肽普绅,同上法,测定其胶凝温度。
盐酸表柔比星肽普绅的胶凝时间:分别取肽普绅及盐酸表柔比星肽普绅300 μL,加入4.5 mm口径的玻璃管中,于4 ℃放置15 min,使壁上的凝胶完全沉降于玻璃管底部,之后取出,室温放置15 min使其温度恢复室温。开启恒温水浴锅,精密调控温度,温度设置为37 ℃(±0.1 ℃)。测定肽普绅及盐酸表柔比星肽普绅的胶凝时间。
分别取肽普绅及盐酸表柔比星肽普绅,冷冻干燥 48 h,冻干样品切片,真空条件下喷金,采用场发射扫描电镜FE-SEM (JSM-7900F)在2 kV下观察凝胶的内部形貌。另取肽普绅,同上法操作,测定其内部形貌。
分别取肽普绅及盐酸表柔比星肽普绅冷冻干燥样品,采用溴化钾(KBr)压片法,扫描参数400~4 000 cm-1,测定肽普绅及盐酸表柔比星肽普绅的傅里叶变换红外光谱。
采用溶剂替换法,测定盐酸表柔比星肽普绅中盐酸表柔比星的体外释放,释放介质为生理盐水;释放介质体积为9.0 mL;释放温度设定为37 ℃;通过荧光分光光度计,测定盐酸表柔比星荧光强度,计算盐酸表柔比星的百分累计释放量。具体实验方法如下:精密称取盐酸表柔比星肽普绅,至体积为15 mL的刻度试管,37 ℃ 恒温水浴至完全胶凝,精密加入释放介质9.0 mL,取样时间点分别于 1、2、4、6、8、12、24、48、72、96、120、144、168、192和216 h,在各个取样时间点,将其9.0 mL释放介质完全取出,待测。之后,立即加入 9.0 mL释放介质。
采用荧光分光光度计对药物释放量进行测定,激发波长506 nm、发射波长555 nm,测定其荧光强度值,计算各个取样时间点的释放百分率,根据各个时间的释放百分率计算累计释放百分率,绘制累积释放百分率-时间曲线。
选择人源肝癌细胞株HepG2为细胞模型,采用流式细胞术测定盐酸表柔比星肽普绅的细胞摄取,具体方法如下:取处于对数生长期的 HepG2 细胞,以每孔 2×105个细胞的密度接种于 12 孔细胞培养板中,放置于 37 ℃ 和 5% CO2中孵育 24 h,使细胞贴壁生长。弃去原培养基,分别加入含盐酸表柔比星溶液、盐酸表柔比星肽普绅的培养基(盐酸表柔比星浓度为1 或2 μmol·L-1),在37 ℃下孵育4 h。孵育结束后,弃去含药培养基,用预冷的磷酸盐缓冲液(PBS)(pH=7.4)洗涤各孔3次,以除去未被细胞摄取的游离药物。向各孔中加入100 μL胰蛋白酶液以消化细胞。待细胞开始掉落后,用含血清的培养基终止细胞消化。随后,将细胞转移至1.5 mL离心管(EP)中,1 000 r·min-1离心3 min,用PBS洗涤细胞3次。将细胞重悬于300 μL的PBS中,使用流式细胞仪在激发波长488 nm处检测荧光,测定细胞摄取情况。
选择HepG2为细胞模型,采用磺酰罗丹明B(Sulforhodamine B,SRB)法测定盐酸表柔比星肽普绅的HepG2细胞药效,具体方法如下:将处于对数生长期的HepG2细胞以每孔8×103个细胞的密度接种在96孔板中,37 ℃孵育24 h使细胞贴壁生长。以生理盐水为溶剂,稀释盐酸表柔比星肽普绅,无菌滤膜过滤除菌,完全培养基稀释,配制系列浓度,加入96孔板,二氧化碳培养箱中37 ℃孵育48 h。采用SRB法测定,计算盐酸表柔比星肽普绅的半数致死量(IC50)。
每只雌性裸鼠右前肢腋窝接种体积为100 μL的HepG2细胞悬液(每毫升5×107个细胞),每日观察肿瘤的生长情况。
给药方案一:待肿瘤体积长到100 mm3左右,将荷瘤裸鼠随机分为2组(每组6只),空白对照组:给予生理盐水;给药组1:给予盐酸表柔比星肽普绅(剂量以盐酸表柔比星计为10 mg·kg-1),瘤旁单次注射给予。
给药方案二:待肿瘤体积长到300 mm3左右,将荷瘤裸鼠随机分为2组(每组6只),空白对照组:给予生理盐水;给药组2:给予盐酸表柔比星肽普绅(剂量以盐酸表柔比星计为10 mg·kg-1),瘤旁单次注射给予。
给药方案三:待肿瘤体积长到800 mm3左右,将荷瘤裸鼠随机分为2组(每组3只),空白对照组:给予生理盐水;给药组3:给予盐酸表柔比星肽普绅(剂量以盐酸表柔比星计为20 mg·kg-1),瘤旁单次注射给予。
荷瘤动物给药前及给药后,每两天通过游标卡尺测量肿瘤的长和宽,计算肿瘤体积(公式1)。
肿瘤体积V(mm3)=长(mm)×宽2(mm2)×0.524
绘制肿瘤体积-时间曲线,计算抑瘤率(公式2):
抑瘤率(%)=(V空白-V给药)/(V空白-V0)×100%
实验方案三中,对照组和给药组3荷瘤裸鼠于给药第 8天通过脱颈法处死。取肿瘤组织,称重,加双蒸水,匀浆,加甲醇,涡旋,沉淀蛋白,离心,取上清液,采用HPLC法测定肿瘤组织中的盐酸表柔比星。
HPLC法测定条件:色谱柱:C18柱(4.6 mm×250 mm,5 μm,Phenomenex);检测波长:激发波长505 nm,发射波长555 nm;流速:0.5 mL·min-1;柱温:35 ℃;进样体积:20 μL;流动相:十二烷基硫酸钠溶液(3 mg·mL-1)-乙腈-甲醇(47.2∶47.2∶5.6)。
所有数据用$\stackrel{-}{x}$±s表示。采用单因素方差分析(ANOVA),经过Bonferroni校验,P<0.05认为有显著性差异。
盐酸表柔比星肽普绅的配制方法简单,配制过程主要包括生理盐水溶解盐酸表柔比星以及与肽普绅充分混合。
不同体积占比的肽普绅的胶凝温度见图2。肽普绅体积占比为100%时,即未经生理盐水稀释的肽普绅,其胶凝温度为(34.8±0.5) ℃,随肽普绅体积占比降低,其胶凝温度呈现逐步上升的趋势,当肽普绅体积占比为20%时,其胶凝温度为(38.7±0.6)℃。当肽普绅体积占比为30%~40%时,其胶凝温度为(38.3±0.5) ℃至(37.5±0.7) ℃。当肽普绅体积占比为50%~60%时,其胶凝温度为(36.5±0.7) ℃(图2)。当肽普绅体积占比为70%~90%时,其胶凝温度为(35.5±0.6)℃。上述结果表明,当肽普绅体积占比大于70%时,其胶凝温度低于体温37 ℃。当肽普绅体积占比为50%~60%时,其胶凝温度有可能会高于体温37 ℃。当肽普绅体积占比低于40%时,其胶凝温度高于体温37 ℃。因此,为保持肽普绅固有的温敏凝胶性质,满足温敏栓塞要求,当肽普绅被生理盐水稀释时,肽普绅体积占比应高于70%,每瓶肽普绅(体积为5 mL)可加载药物溶液的最大体积建议不超过2 mL。本研究所配制的盐酸表柔比星肽普绅,药物溶液的体积为1.5 mL,肽普绅体积占比为76.9%。
当盐酸表柔比星肽普绅体积占比分别为70%、60%、50%、40%时,其胶凝温度分别为(35.7±0.6)、(36.7±0.6)、(37±0.0)和(37.3±0.6) ℃,这表明,肽普绅体积占比相同时,盐酸表柔比星肽普绅的胶凝温度与肽普绅的胶凝温度无明显变化,结果说明,盐酸表柔比星的加入不影响肽普绅的胶凝温度,推测加入盐酸表柔比星不影响肽普绅的温敏凝胶性质及温敏栓塞作用。
肽普绅胶凝时间为 (40±3)s,盐酸表柔比星肽普绅的胶凝时间为 (37±6)s,表明,加入盐酸表柔比星不影响肽普绅的胶凝时间,盐酸表柔比星肽普绅仍保持肽普绅的温敏性能。
肽普绅和盐酸表柔比星肽普绅的傅里叶变换红外光谱见图3,对其主要基团进行归属:3 400 cm-1为仲胺基的伸缩振动,2 900 cm-1为甲基、亚甲基的伸缩振动,1 650 cm-1为酰胺的羰基的伸缩振动,1 550 cm-1为酰胺中 N—H 的弯曲振动,此为仲胺的特征吸收峰,1 400 cm-1为C—N的伸缩振动,1 250 cm-1为伯醇O—H的弯曲振动,同时1 100 cm-1和1 000 cm-1为伯醇C—O的伸缩振动。结果表明,两傅里叶变换红外光谱相似,主要基团峰位几乎相同,未见明显的新基团峰出现,这说明,盐酸表柔比星未与肽普绅中的成分发生相互作用。
肽普绅及盐酸表柔比星肽普绅的场发射扫描电镜结果见图4,结果表明,肽普绅内部与盐酸表柔比星肽普绅内部均为多孔的网状结构,二者内部结构相似,说明,盐酸表柔比星的加入并未改变肽普绅内部的微观结构。
盐酸表柔比星肽普绅的药物累计释放百分率-时间曲线见图5。结果表明,1 h时间点,药物释放百分率约 25%,6 h时间点,药物释放百分率约为60%,12 h时间点,药物释放百分率约为73%,24 h时间点,药物释放百分率约为 80%,48 h时间点,药物释放百分率约为89%,96 h时间点,药物释放百分率为97%,随后,药物缓慢释放,216 h时间点,药物释放百分率接近100%。上述结果表明,药物释放具有缓释特征,0~24 h的药物释放速度相对较快,之后,药物释放速度逐渐达到平稳,药物可持续释放达216 h。
HepG2 细胞对盐酸表柔比星和盐酸表柔比星肽普绅的摄取水平见图6。结果表明,当盐酸表柔比星给药浓度为分别为1和2 μmol·L-1时,HepG2 细胞对盐酸表柔比星肽普绅中盐酸表柔比星的摄取与对盐酸表柔比星的摄取水平相当,说明盐酸表柔比星加载到肽普绅后仍保持盐酸表柔比星细胞摄取的性质,肽普绅不影响盐酸表柔比星本身的细胞摄取,为盐酸表柔比星肽普绅发挥抗肿瘤药效提供了依据。
依据盐酸表柔比星及盐酸表柔比星肽普绅的HepG2细胞存活曲线见图7。计算IC50,见表1。结果表明,盐酸表柔比星的IC50为(0.606±0.088) μmol·L-1,即(0.353±0.052)μg·mL-1,说明,盐酸表柔比星对HepG2细胞具有较强的抗肿瘤细胞药效,在μg·mL-1水平即可产生较强的抗 HepG2肝肿瘤细胞药效。
盐酸表柔比星肽普绅中盐酸表柔比星的IC50为 (0.551±0.153) μmol·L-1,即(0.320±0.089)μg·mL-1,说明盐酸表柔比星肽普绅中的盐酸表柔比星的HepG2细胞药效与盐酸表柔比星相似,表明盐酸表柔比星加载到肽普绅仍保持盐酸表柔比星较强的抗肿瘤细胞药效,肽普绅不影响盐酸表柔比星本身的抗肿瘤药效。
给药方案一:荷瘤裸鼠给予盐酸表柔比星肽普绅,其肿瘤体积-时间变化曲线见图8A,抑瘤率结果见表2,结果表明,盐酸表柔比星肽普绅与对照组相比具有明显的抗肿瘤药效,瘤旁单次给药第20天,盐酸表柔比星肽普绅给药组的肿瘤体积为(312±95) mm3,对照组肿瘤体积为 (1 858±402) mm3,抑瘤率达88.9%。
给药方案二:荷瘤裸鼠给予盐酸表柔比星肽普绅,其肿瘤体积-时间变化曲线见图8B,抑瘤率结果见表2,结果表明,盐酸表柔比星肽普绅与对照组相比具有明显的抗肿瘤药效,瘤旁单次给药第14天,盐酸表柔比星肽普绅给药组的肿瘤体积为(513±144) mm3,对照组肿瘤体积为(1 858±402) mm3,抑瘤率达87.3%。
给药方案三:荷瘤裸鼠给予盐酸表柔比星肽普绅,其肿瘤体积-时间变化曲线见图8C,抑瘤率结果见表2,结果表明,盐酸表柔比星肽普绅与对照组相比具有明显的抗肿瘤药效,瘤旁单次给药第8天,盐酸表柔比星肽普绅给药组的肿瘤体积为(969±173) mm3,对照组肿瘤体积为(1 708±94) mm3,抑瘤率达82.6%。
上述结果表明,针对体积为100 mm3及300 mm3左右的肿瘤,盐酸表柔比星肽普绅 (给药剂量为 10 mg·kg-1,单次瘤旁给药) 具有良好的体内抗肿瘤作用。针对体积800 mm3左右的肿瘤,盐酸表柔比星肽普绅 (给药剂量为 20 mg·kg-1,单次瘤旁给药) 同样具有良好的体内抗肿瘤作用,说明盐酸表柔比星肽普绅具有良好的体内抗肿瘤作用。
肿瘤组织内盐酸表柔比星含量测定结果见表3,结果表明,单次瘤旁给药20 mg·kg-1,给药第8天肿瘤组织内盐酸表柔比星平均含量为(37.0±8.30) ng·g-1,说明,盐酸表柔比星肽普绅瘤旁给药后,盐酸表柔比星可从盐酸表柔比星肽普绅中释放出来,被肿瘤细胞摄取,从而发挥抗肿瘤作用,且在单次给药第8天仍可在肿瘤组织内检测到盐酸表柔比星,也表明,盐酸表柔比星肽普绅中盐酸表柔比星的释放具有缓释作用,推测该盐酸表柔比星肽普绅经动脉栓塞化疗可发挥较好的化疗作用。
肽普绅是一种新型的液态温敏栓塞剂,其所含有的温敏材料是经N,N-亚甲基双丙烯酰胺交联的N-异丙基丙烯酰胺和N-正丙基丙烯酰胺的共聚物 (Copolymer of N-isopropylacrylamide and N-propylacrylamide cross linked by N, N-methylene diacrylamide),属于聚 (N-异丙基丙烯酰胺)(poly N-isopropyl acrylamide, pNIPAAm) 类常用的温敏材料[15-20]。温敏凝胶受温度影响发生溶胶-凝胶转变,在最低临界共溶温度 (lower critical solution temperature, LCST)之下呈现溶液状,在高于LCST时呈现凝胶状[21],见图9A。在室温条件下,以生理盐水溶解的注射用盐酸表柔比星,可与同以生理盐水为溶剂的肽普绅均匀混合,呈现溶液状态,而当达到体温(高于LCST)时,则呈现凝胶状态,此时盐酸表柔比星存在于凝胶状肽普绅中,实施了药物的载带,见图9B。与以离子交换为载药机制的栓塞微球不同,栓塞微球表面的离子与药物中离子通过交换结合完成载药,如DC Bead微球表面的磺酸基团结合的钠离子与其药物中带正电的离子进行离子交换,使带正电的药物通过静电作用与磺酸基团结合而载带药物[22-25],此时要求药物上离子的结合能力要强于微球表面上的离子,即药物可离子化且能与微球表面的离子结合,因此在药物选择方面受到了一定的限制。而对于肽普绅载带药物而言,药物溶解于适宜的水性介质(如生理盐水)与肽普绅进行混合,只要不影响肽普绅的温敏性质,如溶胶-凝胶转变,即可完成药物的加载,因此在药物选择方面适用性更广,不受药物需可离子化且能与微球表面离子结合的限制。
载药栓塞微球的药物释放机制与其载药机制一致,即通过离子交换机制置换被载带到栓塞微球上的药物,此时要求释放介质中的离子与栓塞微球的结合要强于药物与栓塞微球的结合。而在带盐酸表柔比星肽普绅中,药物的释放是扩散方式进行的,通过浓度梯度,由高浓度向低浓度扩散,见图9C
相比栓塞微球载药需经过15~60 min孵育,盐酸表柔比星肽普绅的配制时间通常在5 min之内完成,配制过程主要包括生理盐水溶解盐酸表柔比星以及与肽普绅的充分混合。该配制方法简便、快捷、用时短,非常适于临床使用时,根据实际需要,临用临配。鉴于这种简单混合的配制方式,预示肽普绅可与不同的抗肿瘤药物溶液通过物理混合的方式制备成载带药物的肽普绅,显示出肽普绅载带药物的普适性。
稀释肽普绅的生理盐水体积会影响肽普绅的温敏性质,为保持肽普绅的温敏性质,建议稀释肽普绅的生理盐水不要超过2 mL。在此条件下,1 支肽普绅加载注射用盐酸表柔比星的最大量为6支(60 mg,2 mL生理盐水依次溶解盐酸表柔比星),配制方法除生理盐水为2 mL外,其他同“2.1”项下方法。因此,1支肽普绅可加载 1~6支注射用盐酸表柔比星(10~60 mg),为满足临床使用提供了多种选择。
盐酸表柔比星肽普绅中药物释放可维持216 h(9 d),累计释放量接近100%,显示出明显的缓释作用且释药完全,此外,药物在24 h累计释放量约80%,可更好地发挥细胞毒药效,产生良好的抗肿瘤作用。
盐酸表柔比星肽普绅配制方法简便、快捷、用时短,非常适于临床使用时,根据实际需要,临用临配,配制方案灵活,可满足不同药物剂量的使用。推测肽普绅可与不同的抗肿瘤药物溶液通过物理混合的方式制备成载带药物的肽普绅。盐酸表柔比星肽普绅不影响肽普绅本身的温敏性质,药物释放持久完全,具有良好的体内外抗肿瘤作用。上述结果为盐酸表柔比星肽普绅的 TACE 临床应用提供有益的数据支持。
