OBJECTIVE To prepare and calibrate the first batch of national standard (NS) of pormestriene for the sake of fulfilling the requirement of quality control on it and related drug products in domestic. METHODS The structure of candidate pormestriene was confirmed by MS, NMR, IR and UV. Physical-chemical analysis was carried out. According to different national drug standard methods, the purity of the candidate was determined, the value was assigned by mass balance method, and the uniformity and stability of the candidate were investigated. RESULTS The candidate was homogenous with promestriene, and the content was 99.6% measured by mass balance method.Uniformity inspection met the requirements. The product should be stored at 10-30 ℃, shaded and sealed. CONCLUSION The candidate was confirmed to be promestriene, which can be used as the first national standard, the content is 99.6%, and it can be used as a reference for the identification and content determination of promestriene and related preparations.

目的 为满足国内普罗雌烯及相关制剂质量控制的需要,研制并标定首批普罗雌烯国家标准品。方法 采用高分辨质谱、核磁共振波谱、红外光谱以及紫外光谱对待标品进行定性及结构确认;对待标品进行了理化分析;根据不同国家药品标准方法分别对待标品进行了纯度分析,通过质量平衡法进行赋值,并通过核磁定量进行验证,并对待标品的均匀性和稳定性进行了考察。结果 待标品与普罗雌烯同质,经质量平衡法赋值含量为99.6%;均匀性考察符合要求;本品应在10~30 ℃保存,遮光、密封。结论 本批待标品确认为普罗雌烯,以C22H32O2计,含量为99.6%,可以作为首批普罗雌烯国家标准品,可作为普罗雌烯及相关制剂鉴别和含量测定用对照品。

OBJECTIVE To prepare and calibrate the first batch of national standard (NS) of pormestriene for the sake of fulfilling the requirement of quality control on it and related drug products in domestic. METHODS The structure of candidate pormestriene was confirmed by MS, NMR, IR and UV. Physical-chemical analysis was carried out. According to different national drug standard methods, the purity of the candidate was determined, the value was assigned by mass balance method, and the uniformity and stability of the candidate were investigated. RESULTS The candidate was homogenous with promestriene, and the content was 99.6% measured by mass balance method.Uniformity inspection met the requirements. The product should be stored at 10-30 ℃, shaded and sealed. CONCLUSION The candidate was confirmed to be promestriene, which can be used as the first national standard, the content is 99.6%, and it can be used as a reference for the identification and content determination of promestriene and related preparations.

普罗雌烯(pormestriene)为化学合成的甾体类雌激素,化学名为3-丙氧基-17β-甲氧基雌甾-1,3,5,(10)-三烯,CAS号为39219-28-8,分子式为C22H32O2,相对分子质量为328.50,结构式见图1[1-3]。该品种是一种局部雌激素补充剂,临床用于治疗阴道感染和萎缩性阴道炎,改善绝经期症状等[4-5]。由该原料药生产的制剂主要有普罗雌烯乳膏、普罗雌烯阴道胶丸、普罗雌烯阴道用软胶囊以及氯喹那多-普罗雌烯阴道片。目前《美国药典》《欧洲药典》《英国药典》《日本药典》以及《中国药典》均未收载该品种。国内有两家企业生产该产品原料,有5家企业生产相关制剂。该品种所涉及的标准有国家药品标准YBH32392005、国家药品标准YBH39532005以及普罗雌烯对照品的精制方法及质量标准YBH22062004[6-8]。此次标定对普罗雌烯原料药进行了全面的质量分析,并依据上述标准方法以及《中国药典》2020年版四部通则分别对纯度进行测定,采用质量平衡法[9-11]对普罗雌烯标准物质进行赋值,并通过核磁共振波谱法定量,对标准物质含量进行佐证。

普罗雌烯(北京斯利安药业有限公司,批号:2009002,约250 g)。将原料药以每支100 mg分装于棕色西林瓶中,以丁基胶塞和铝盖密封, 并于常温避光保存,作为第1批普罗雌烯国家标准品待标品。
氘代氯仿(CIL,批号:PR-31256/09169CL2);1,3,5-三甲氧基苯(中国食品药品检定研究院,批号:510137-201601);乙腈(Solvslichro,批号:B2021011000);溴化钾(北京化学试剂公司,批号:011025);二氧六环(批号:20170302)、三氯甲烷(批号:20180514)、无水乙醇(批号:20210127)、硫酸(批号:20190423)、磷酸氢二铵(批号:20200414)、五水合氢氧化四甲基铵(批号:20210719)、N,N-二甲基甲酰胺(批号:20190819)(国药集团化学试剂有限公司);丙酮(北京化工产,批号:20170801);甲醇(优级纯);磷酸(色谱纯)。
称取普罗雌烯待标品适量,加乙腈溶解并稀释制成每1 mL含普罗雌烯约50 μg的溶液。采用正离子模式,离子源温度320 ℃,蒸发温度300 ℃,鞘气流速7.5 L·min-1,MSE扫描模式;扫描范围m/z 50~2 000;解离电压40 V;流动注射速度60 μL·min-1
称取普罗雌烯待标品约10 mg,加200 mg干燥后的溴化钾,置玛瑙研磨中缓慢研磨。按照2020年版《中国药典》四部通则0402测定红外光谱。
称取普罗雌烯待标品约1 mg,加氘代氯仿0.5 mL使溶解,作为待测样品。检测探头温度25 ℃,对待测样品分别进行1H-NMR、13C-NMR、DEPT、1H-1H COSY、HSQC、HMBC、NOESY等核磁共振实验。
称取普罗雌烯待标品适量,加乙腈溶解并稀释制成每1 mL约含普罗雌烯1 mg的溶液。按照2020年版《中国药典》四部通则0401测定。
按2020年版《中国药典》四部凡例项下测定。
按2020年版《中国药典》四部0612测定。
按2020年版《中国药典》四部0621测定。
称取普罗雌烯待标品约2 mg,加硫酸1 mL使溶解,观察溶液颜色,加水1 mL,再次观察溶液颜色。
称取普罗雌烯待标品约100 mg,在25 ℃、湿度从30%到80%分为6个阶梯,每阶梯4 h,共放置24.5 h,累计增重。
称取普罗雌烯待标品约5 mg,精密称定,置铝制坩埚内,压盖,打孔,环境为氩气,温度以-263 ℃·min-1的速率从-70 ℃升至200 ℃,测定。
按照国家药品标准YBH32392005有关物质项下方法进行测定。
供试品溶液:取普罗雌烯待标品5支,分别称取适量,精密称定,加乙腈溶解并稀释制成每1 mL约含普罗雌烯1 mg的溶液。
对照溶液:精密取“2.3.1”项下供试品溶液1 mL,加乙腈定量稀释100倍,作为1%自身对照溶液。
灵敏度溶液:精密量取“2.3.1”项下对照溶液1 mL,加乙腈定量稀释10倍,作为0.1%灵敏度溶液。
分析及计算:采用菲洛门Titank C18色谱柱(4.6 mm×250 mm,5 μm),以乙腈为流动相,流速为1.0 mL·min-1;检测波长为220 nm;柱温35 ℃;等度洗脱。取上述溶液10 μL,注入液相色谱仪,记录色谱图,分别按主成分自身对照法和面积归一化法计算。
照国家药品标准YBH39532005有关物质项下方法进行测定。
供试品溶液:取普罗雌烯待标品3支,分别称取适量,精密称定,加流动相[甲醇-0.01 mol·L-1磷酸氢二铵-0.01 mol·L-1五水合氢氧化四甲基铵(pH 3.0)(98∶2)]溶解并稀释制成每1 mL约含普罗雌烯1 mg的溶液。
对照溶液:精密量取“2.3.2”项下供试品溶液1 mL,加流动相定量稀释100倍,作为1%自身对照溶液。
灵敏度溶液:精密量取“2.3.2”项下对照溶液1 mL,加流动相定量稀释10倍,作为0.1%灵敏度溶液。
分析及计算:采用ACE Excel Super C18色谱柱(4.6 mm×250 mm,5 μm),流速为1.0 mL·min-1;检测波长为220、280 nm;柱温35 ℃;等度洗脱。取上述溶液20 μL,注入液相色谱仪,记录色谱图,分别按主成分自身对照法和面积归一化法计算。
照国家药品标准YBH22062004有关物质项下方法进行测定。
供试品溶液:普罗雌烯待标品3支,分别称取适量,精密称定,加流动相[甲醇-0.15 mol·L-1磷酸(97∶3)]溶解并稀释制成每1 mL约含普罗雌烯0.5 mg的溶液。
对照溶液:精密量取“2.3.3”项下供试品溶液1 mL,加流动相定量稀释100倍,作为1%自身对照溶液。
灵敏度溶液:精密量取“2.3.3”项下对照溶液1 mL,加流动相定量稀释10倍,作为0.1%灵敏度溶液。
分析及计算:采用ACE Excel Super C18色谱柱(4.6 mm×250 mm,5 μm),流速为1.0 mL·min-1;检测波长为220、279 nm;柱温35 ℃;等度洗脱。取上述溶液20 μL,注入液相色谱仪,记录色谱图,分别按主成分自身对照法和面积归一化法计算。
照国家药品标准YBH32392005残留溶剂项下方法进行测定。
样品溶液:称取普罗雌烯待标品约100 mg,精密称定,置顶空瓶中,精密加入N,N-二甲基甲酰胺1 mL使溶解,密封。
标准溶液:分别称取甲醇、乙醇适量,精密称定,加N,N-二甲基甲酰胺稀释制成每1 mL约含甲醇0.3、0.5 μg的混合溶液,精密量取1 mL置顶空瓶中,密封。
分析及计算:采用DB-624毛细管色谱柱(0.53 mm×30 m,3.0 μm);起始温度60 ℃,保持12 min;进样口温度200℃;氮气载气流速5 mL·min-1;FID检测器,检测器温度220 ℃;分流比10∶1;加热箱温度95 ℃,加热时间15 min;定量环温度100 ℃;传输管温度105 ℃;进样时间1 min。取上述溶液注入气相色谱仪,记录色谱图,按外标法计算。
取普罗雌烯待标品,按照2020年版《中国药典》四部通则0831测定。
取普罗雌烯待标品,按照2020年版《中国药典》四部通则0841测定。
取普罗雌烯待标品,以氘代氯仿为溶剂,1,3,5-三甲氧基苯为内标,按照2020年版《中国药典》四部通则0441测定。
取普罗雌烯待标品在40 ℃条件下放置,分别于第0、5、10天取样,按“2.3.1”项下方法测定,按面积归一化法计算。
取普罗雌烯待标品12支,按“2.3.1”项下方法测定,计算其响应因子(峰面积/称样量)的相对标准偏差(RSD),并通过STATA软件进行单因素方差分析。
经高分辨质谱分析见图2,待标品电喷雾电离质谱(ESI-MS)显示[M+H]+m/z为328.240 63,与理论值一致,推测其分子式为C22H32O2
待标品的红外光谱图见图3,其特征吸收峰与文献报道普罗雌烯的特征吸收峰一致[12]。2 956(芳烃${\mathrm{\upsilon }}_{\mathrm{C}\mathrm{H}}$)、2 931(芳环${\mathrm{\upsilon }}_{\mathrm{C}\mathrm{H}}$)、2 866(烷烃${\mathrm{\upsilon }}_{\mathrm{C}\mathrm{H}}$)、1 612(芳环${\mathrm{\upsilon }}_{\mathrm{C}\mathrm{O}}$)、1 573(芳烃骨架${\mathrm{\upsilon }}_{\mathrm{C}\mathrm{C}}$)、1 255(芳香醚${\mathrm{\upsilon }}_{\mathrm{A}\mathrm{r}\mathrm{O}}$)、1 123(脂肪醚${\mathrm{\upsilon }}_{\mathrm{R}\mathrm{O}}$)、1 107(醚${\mathrm{\upsilon }}_{\mathrm{R}\mathrm{O}}$)。
通过NMR进一步确定待标品结构(图4),根据1H-NMR谱、13C-NMR谱数据,以及DEPT、1H-1H COSY、HSQC、HMBC、NOESY等测试数据分析,证明待标品结构与图1结构式一致。
供试品溶液在300~250 nm范围内扫描(图5),结果为分别在280、287 nm的波长处有最大吸收,符合标准规定(应在279、287 nm的波长处有最大吸收)。
待标品为白色结晶性粉末。
经检查,待标品在氯仿中极易溶解,在二氧六环、丙酮中易溶,在无水乙醇和乙腈中略溶,在水中几乎不溶。
经检查,待标品的熔点为66.9 ℃,符合规定(应为64.0~68.0 ℃)。
经检查,待标品的比旋度为+67 °,符合规定(应为+65 °~+75 °)。
称取待标品约2 mg,加硫酸1 mL溶解后,溶液呈淡黄色,加水1 mL,溶液变成橙色并带有荧光,呈正反应。
经检查,待标品在24.5 h内累计增重0.066 5%,无引湿性,以此指导分装。
通过DSC测定(图6),获得待标品的物理特性,如玻璃化转变、熔程、结晶等,进一步确证待标品为普罗雌烯。
经测定(图7),相当于供试品溶液主成分浓度0.1%的量能被定量检出(S/N大于10);分别对5份分装后的待标品进行测定,以峰面积归一化法计算,待标品的纯度平均值为99.804%(n=5,RSD为0.01%);以主成分自身对照法(1%)计算,待标品的纯度平均值为99.811%(n=5,RSD为0.01%)。
经测定(图8),相当于供试品溶液主成分浓度0.1%的量能被定量检出(S/N大于10);分别对3份分装后的待标品进行测定,以峰面积归一化法计算,待标品的纯度平均值为99.819%(n=3,RSD为0.01%)(220 nm);以主成分自身对照法(1%)计算,待标品的纯度平均值为99.811%(n=3,RSD为0.01%)(220 nm);分别对3份分装后的待标品进行测定,以峰面积归一化法计算,待标品的纯度平均值为99.932%(n=3,RSD为0.01%)(280 nm);以主成分自身对照法(1%)计算,待标品的纯度平均值为99.926%(n=3,RSD为0.01%)(280 nm)。
经测定(图9),相当于供试品溶液主成分浓度0.1%的量能被定量检出(S/N大于10);分别对3份分装后的待标品进行测定,以峰面积归一化法计算,待标品的纯度平均值为99.912%(n=3,RSD为0.01%)(220 nm);以主成分自身对照法(1%)计算,待标品的纯度平均值为99.911%(n=3,RSD为0.01%)(220 nm);分别对3份分装后的待标品进行测定,以峰面积归一化法计算,待标品的纯度平均值为99.933%(n=3,RSD为0.01%)(279 nm);以主成分自身对照法(1%)计算,待标品的纯度平均值为99.931%(n=3,RSD为0.01%)(279 nm),数据对比见表1
通过“2.3.1”“2.3.2”“2.3.3”项下色谱条件,图7~9及表1测定结果对比可知:①方法三色谱条件下,在220 nm波长处测定获得的图谱(图9A)基线噪音较大,灵敏度较差;在279 nm波长处测定获得的图谱(图9B)不能对待标品中的有关物质有效的分离,不能真实反映待标品的纯度。②方法二色谱条件下,在220 nm波长处测定获得的图谱(图8A)灵敏度较低,待标品中有关物质响应值较低;在280 nm波长处测定获得的图谱(图8B)中,溶剂峰前后出现较多杂质,且无法有效分离,影响积分条件的设置,使测定结果不准确。③方法一色谱条件下,在220 nm波长处测定获得的图谱(图7)具有较好的灵敏度,且各杂质峰与主成分峰之间能够较好地分离,测定结果更加准确,因此,采用方法一测定的纯度结果进行赋值计算。
经检验(图10),待标品中未检测出甲醇,乙醇含量为0.123 2%,符合规定(甲醇应不得超过0.3%、乙醇应不得超过0.5%)。
待标品在105 ℃下干燥至恒重,减失重量为0.067 2%,符合规定(不得过0.5%)。
经检验,待标品的炽灼残渣量为0.037 3%,符合规定(不得过0.1%)。
根据干燥失重、炽灼残渣、残留溶剂以及纯度的测定结果,以质量平衡法计算(公式1)。
待标品的含量(%)=(100%-干燥失重%-炽灼残渣%-残留溶剂)×HPLC纯度%=(100%-0.067 2%-0.037 3%-0.012 32%)×99.811%=99.58%
取待标品(平行6份)进行测定,含量平均值为99.07%(n=6,RSD为0.13%)。
试验表明,待标品在40 ℃放置10 d,纯度无变化,较稳定。根据稳定性结果建议本品在10~30 ℃保存,遮光、密封(表2)。
取分装后的待标品12支,按国家药品标准YBH32392005含量测定项下方法测定,其响应因子(峰面积/称样量)的RSD为0.39%;通过STATA软件进行单因素方差分析,结果显示12组测定结果P=0.791>0.05,方差无显著齐性,说明本品均匀性良好。
本研究采用高分辨质谱、核磁共振波谱、红外光谱紫外光谱进行解析,待标品可确定为普罗雌烯化学结构。采用质量平衡法对首批普罗雌烯的国家标准物质进行赋值,并采用核磁定量法对赋值结果进行验证,此外,本研究还对普罗雌烯的短期稳定性和均匀性进行了考察,均符合国家标准物质的要求,给出了贮藏指导建议,最终,本批对照品以质量平衡法的测定结果定值,以C22H32O2计,含量为99.6%。可作为普罗雌烯及相关制剂鉴别和含量测定用对照品。
